This thesis covers an attempt to construct a supervised machine learning model, for use in prediction of formation enthalpy values for novel metal hydride compositions. Further work, making use of static density functional theory calculations as well as ab initio and machine learning force field molecular dynamics simulations, to model oxygen transport in a La-Mg co-doped barium titanate system is also reported. Utilising open-source, readily available repositories of previously calculated results, two gradient boosting regression models are developed; separately trained to qualitatively predict formation enthalpy data for metal hydrides, and for intermetallic alloys. Once developed, such predictions are compared to enthalpy values, calculated from first principles, for held-out samples from the original database, and known experimental values for select materials. A process is outlined for generating new ternary hydride compositions, previously unseen to the model, from which a select sample of promising predictions are subjected to crystal structure prediction processes. By introducing structural information, first principles calculations are used to determine formation enthalpies for comparison to predictions. Intentionally trained using descriptors derived solely from chemical composition, without any dependence on crystal structure, the resultant model ultimately struggles to generalise prediction of formation enthalpies across the diverse geometry space of hydride materials. The decades-long quest for reliable crystal structure prediction simply from chemical composition proves to be a challenge for effective model validation by calculation, given the range of hydride classes. Oxygen transport through the prototypical perovskite system of barium titanate is studied to investigate the methodology of characterising oxide ion diffusion through the bulk of such a material. Inspired by unpublished experimental results, this system is then co-doped with lanthanum and magnesium, thus introducing titanium and oxygen vacancies, allowing for investigation of oxygen diffusion by means of dynamic simulation methods. This is performed using the relatively new method of on-the-fly machine learning force field molecular dynamics, an approach to the modelling of dynamical systems which, in theory, drastically reduces the time and computational cost of traditional methods based solely on ab initio molecular dynamics. Approximations of low-energy transition paths for oxygen movement in the local vicinity of point defects suggest energetically favourable diffusion pathways introduced by lanthanum doping. Molecular dynamics simulation methods are used to construct oxygen self-diffusion trajectories, from which diffusion mechanics can be determined. These results suggest higher rates of diffusion events in magnesium-doped barium titanate than when co-doped with magnesium and lanthanum. Additionally, mobility in the co-doped system is shown to be influenced by geometry of lanthanum dopants.

7. New Classes of Moving Anchor Extragradient Algorithms for Saddlepoint Problems

This work introduces a moving anchor acceleration technique to extragradient algorithms for smooth structured minimax problems. The moving anchor is introduced as a generalization of the original algorithmic anchoring framework, i.e. the EAG method introduced in \cite{yoon_ryu}, in hope of further acceleration. We show that the optimal order of convergence in terms of worst-case complexity on the squared gradient, $O(1/k^{2}),$ is achieved by our new method (where $k$ is the number of iterations). We also extend the moving anchor to a more general nonconvex-nonconcave class of saddle point problems using the framework of \cite{lee2021fast}, which slightly generalizes \cite{yoon_ryu}. We obtain similar order-optimal complexity results in this extended case. A preconditioned version of our algorithms is also introduced and analyzed to match optimal theoretical convergence rates. Our final theoretical contribution is the development and analysis of a moving anchor with a stochastic oracle, which matches accelerated convergence rates for convex-concave problems. Underlying these theoretical contributions is a selection of robust new Lyapunov/energy functional techniques that account for the moving anchor structure while maintaining the optimal order of complexity under minimal assumptions.Various numerical experiments demonstrate the efficacy of the moving anchor extragradient algorithms compared to their fixed anchor variants, and in many cases suggest a more optimal constant in the big O notation that may surpass the traditional fixed anchor methods.We conclude by discussing current challenges and future directions this work may lead.

Abstract Patents and the systematic analysis thereof provide important decision information for a range of stakeholders pursuing diverse goals. However, in particular academia and small‐scale private sector innovators underappreciate the value of patent information for identifying research gaps, ensuring originality of their work and, in turn, maximisation of limited (public) funds. By the same token, pertinent public sector organisations, such as regulators, require overviews of potentially upcoming technologies to adequately adapt regulatory protocols. The latter, in particular if contemporary scientific evidence is not sufficient, can require a substantial amount of time and lead to a delay in the marketing of important innovations. Patent landscape analysis (PLA) is a very useful method to create overviews of technology fields and thereby indicate if, inter alia, a specific line of scientific enquiry and its application have already been pursued or potential regulatory gaps will exist in the near‐ to mid‐term future. The objective of this communication is to increase awareness of the utility of patent information and provide support in retrieval and analysis of pertinent information for those involved in biotech R&D. Based on Espacenet, a patent search engine, we provide basic guidance on search strategy development, piloting and execution, and data preparation and analysis. To highlight the value of PLA, we also summarise selected results of a PLA we performed recently for microbiome targeting interventions, also referred to as live biotherapeutic products.

9. High density heat storage for homes

- Abstract
The United Kingdom has an objective of reducing fossil fuel emissions to 80% lower than 1990s' levels by 2050 mandated by the UK Climate Change Act [1]. With 47% of total final energy demand in the UK as of 2012 attributable to heating demand [2], methods to decarbonise the UK's heating supply are essential to hitting emissions targets. Phase change material (PCM) based heat storage, a subset of energy storage, presents a unique opportunity to temporally displace the consumption of energy from its production, providing a solution to reduce the economic costs of transitioning to a low carbon energy supply. This thesis details experimental and computational fluid dynamic modelling investigations, (with Ansys Fluent), using temperature dependant properties of the melting process of beeswax and paraffin wax in several isothermal configurations intended to emulate underfloor heat storage using phase change. The effect of changing a key parameter of these models, the mushy constant is also investigated and analysis is performed with dimensionless and non-dimensionless variables. Experimentation quantified the melt process of these materials with temperature sensors, thermal and visual camera imagery for validation of the generated models. Moderate agreement is found with the latter paraffin wax model and experimental data however beeswax models were found to be invalid with larger domain models having issues with model convergence and mesh resolution / time step insensitivity. Experimentation had success with visual and thermal imaging despite thermal attenuation by the vessel walls; thermal imaging was found to be unable to visualise small length scale phenomena. The temperature sensor matrix had some success despite over-prediction of melt fraction due to lack of sensors at the vessel edges. In addition, these sensors contributed negatively to isotropy due to heat losses through the wires from the melting material.

- Abstract
The biosphere of planet Earth is a microbial world: a vast reactor of countless microbially driven chemical transformations and energy transfers that push and pull many planetary geochemical processes, including the cycling of the elements of life, mitigate or amplify climate change (e.g., Nature Reviews Microbiology, 2019, 17, 569) and impact the well-being and activities of all organisms, including humans. Microbes are both our ancestors and creators of the planetary chemistry that allowed us to evolve (e.g., Life's engines: How microbes made earth habitable, 2023). To understand how the biosphere functions, how humans can influence its development and live more sustainably with the other organisms sharing it, we need to understand the microbes. In a recent editorial (Environmental Microbiology, 2019, 21, 1513), we advocated for improved microbiology literacy in society. Our concept of microbiology literacy is not based on knowledge of the academic subject of microbiology, with its multitude of component topics, plus the growing number of additional topics from other disciplines that become vitally important elements of current microbiology. Rather it is focused on microbial activities that impact us–individuals/communities/nations/the human world–and the biosphere and that are key to reaching informed decisions on a multitude of issues that regularly confront us, ranging from personal issues to crises of global importance. In other words, it is knowledge and understanding essential for adulthood and the transition to it, knowledge and understanding that must be acquired early in life in school. The 2019 Editorial marked the launch of the International Microbiology Literacy Initiative, the IMiLI.

Abstract Background Public health systems internationally are under pressure to meet increasing demand for healthcare in the context of increasing financial resource constraint. There is therefore a need to maximise health outcomes achieved with public healthcare expenditure. This paper aims to establish and synthesize the contemporary evidence base for approaches taken at a system management level to improve efficiency. Methods Rapid Evidence Assessment (REA) methodology was employed. A search strategy was developed and applied (PUBMED, MEDLINE) returning 5,377 unique titles. 172 full-text articles were screened to determine relevance with 82 publications included in the final review. Data regarding country, study design, key findings and approaches to efficiency improvement were extracted and a narrative synthesis produced. Publications covering health systems from developed countries were included. Results Identified study designs included policy reviews, qualitative reviews, mixed methods reviews, systematic reviews, literature reviews, retrospective analyses, scoping reviews, narrative papers, regression analyses and opinion papers. While findings revealed no comprehensive frameworks for system-wide efficiency improvement, a range of specific centrally led improvement approaches were identified. Elements associated with success in current approaches included dedicated central functions to drive system-wide efficiency improvement, managing efficiency in tandem with quality and value, and inclusive stakeholder engagement. Conclusions The requirement for public health systems to improve efficiency is likely to continue to increase. Reactive cost-cutting measures and short-term initiatives aimed only at reducing expenditure are unlikely to deliver sustainable efficiency improvement. By providing dedicated central system-wide efficiency improvement support, public health system management entities can deliver improved financial, health service and stakeholder outcomes.

15. Compositional Signatures of Gold from Different Deposit Types in British Columbia, Canada

A study of both in situ and detrital gold from different deposit types in British Columbia was undertaken to establish deposit-specific compositional characteristics in terms of alloy composition and suites of mineral inclusions. The study is based on 11,840 particles from 160 localities in which nine gold deposit types are represented, although there is a strong bias towards gold of orogenic, low-sulphidation epithermal, and alkalic porphyry origin. In general, Ag values in gold alloys are not a powerful discriminator for deposit type, but minor metals may prove useful where detectable, e.g., Cu in gold from ultramafic associations and Pd and Hg in gold from alkalic porphyry systems. The characterization of inclusion suites is far more illuminating, as they correlate strongly with the mineralogy of auriferous ores from different deposit types. This outcome has confirmed the validity of designing an indicator methodology based on inclusion suites and has permitted the prediction of inclusion suites for gold from other deposit types where data are more scarce. The compositional templates generated in the study were applied to identify the source deposit type(s) of gold from 41 localities (a total of 2916 detrital gold particles) where gold genesis was previously unknown.

16. Efficiency Improvement Strategies for Public Health Systems: Developing and Evaluating a Taxonomy in the Australian Healthcare System

Introduction: As demand for healthcare continues to grow, public health systems are increasingly required to drive efficiency improvement (EI) to address public service funding challenges. Despite this requirement, evidence of EI strategies that have been successful applied at the whole-of-system level is limited. This study reports the development, implementation and evaluation of a novel taxonomy of EI strategies used in public health systems to inform systemwide EI models. Materials and methods: The public health system in New South Wales, Australia, operates a centralised EI model statewide and was the setting for this study. An audit of EI strategies implemented in the NSW Health system between July 2016 and June 2019 was used to identify all available EI strategies within the study timeframe. A content management approach was applied to audit the strategies, with each strategy coded to an EI focus area. Codes were clustered according to similarity, and category names were assigned to each cluster to form a preliminary taxonomy. Each category was defined and examples were provided. The resulting taxonomy was distributed and evaluated by user feedback survey and pre–post study to assess the impact on EI strategy distribution. Results: A total of 1127 EI strategies were identified and coded into 263 unique strategies, which were clustered into nine categories to form the taxonomy of EI strategies. Categories included the following: non-clinical contracts and supplies; avoided and preventable activity; clinical service delivery and patient outcomes; finance and operations; recruitment, vacancies and FTE; staffing models; leave management; staff engagement and development; premium staffing; and clinical contracts and supplies. Evaluative findings revealed a perceived reduction in the duplication of EI work, improved access to EI knowledge and improved engagement with EI processes when using the taxonomy. The taxonomy was also associated with wider use of EI strategies. Conclusions: Whole-of-system EI is an increasing requirement. Using a taxonomy to guide systemwide practice appears to be advanta-geous in reducing duplication and guiding practice, with implications for use in health systems internationally.

Many countries across the globe are facing the challenge of replacing coal and natural gas-derived process heat with low-emission alternatives. In countries such as New Zealand, which have access to renewably generated electricity, industrial heat pumps offer great potential to reduce sitewide industrial carbon emissions. In this paper, a new Pinch-based Total Site Heat Integration (TSHI) method is proposed and used to explore and identify multi-level heat pump integration options at a meat processing site in New Zealand. This novel method improves upon standard methods that are currently used in industry and successfully identifies heat pump opportunities that might otherwise be missed by said standard methods. The results of the novel method application suggest that a Mechanical Vapour Recompression (MVR) system in the Rendering plant and a centralized air-source heat pump around the hot water ring main could reduce site emissions by over 50%. Future research will develop these preliminary results into a dynamic emissions reduction plan for the site, the novel methods for which will be transferrable to similar industrial sites.

BackgroundConventional clinical trials are essential for generating high-quality evidence by measuring the efficacy of interventions in rigorously controlled clinical environments. However, their execution can be expensive and time-consuming. In addition, clinical trials face several logistical challenges regarding the identification, recruitment, and retention of participants; consistent data collection during trials; and adequate patient follow-up. This might lead to inefficient resource utilization. In order to partially address the current problems with conventional clinical trials, there exists the need for innovations. One such innovation is the virtual clinical trial (VCT). VCTs allow for the collection and integration of diverse data from multiple information sources, such as electronic health records, clinical and demographic data, patient-reported outcomes, anthropometric and activity measurements, and data collected by digital biomarkers or (small) samples that participants can collect themselves. Although VCTs have the potential to provide substantial value to clinical research and patients because they can lower clinical trial costs, increase the volume of data collected from patients’ daily environment, and reduce the burden of patient participation, so far VCT adoption is not commonplace. ObjectiveThis paper aims to better understand the barriers and facilitators to VCT adoption by determining the factors that influence individuals’ considerations regarding VCTs from the perspective of various stakeholders. MethodsBased on online semistructured interviews, a qualitative study was conducted with pharmaceutical companies, food and health organizations, and an applied research organization in Europe. Data were thematically analyzed using Rogers’ diffusion of innovation theory. ResultsA total of 16 individuals with interest and experience in VCTs were interviewed, including persons from pharmaceutical companies (n=6), food and health organizations (n=4), and a research organization (n=6). Key barriers included a potentially low degree of acceptance by regulatory authorities, technical issues (standardization, validation, and data storage), compliance and adherence, and lack of knowledge or comprehension regarding the opportunities VCTs have to offer. Involvement of regulators in development processes, stakeholder exposure to the results of pilot studies, and clear and simple instructions and assistance for patients were considered key facilitators. ConclusionsCollaboration among all stakeholders in VCT development is crucial to increase knowledge and awareness. Organizations should invest in accurate data collection technologies, and compliance of patients in VCTs needs to be ensured. Multicriteria decision analysis can help determine if a VCT is a preferred option by stakeholders. The findings of this study can be a good starting point to accelerate the development and widespread implementation of VCTs.

The focus on quantifiable data in sport performance has led to incremental advantages in baseball and has played an important role in the development of new hitting, pitching, fielding, and coaching strategies. Recently, researchers and team representatives have considered the impact of additional factors in baseball, including cognitive functioning. In this study, predictive validity for the Athletic Intelligence Quotient (AIQ) was examined vis-à-vis performance outcomes in professional baseball. Specifically, AIQ scores were obtained from 149 Minor League Baseball (MiLB) players prior to the 2014 baseball season and their subsequent performance was assessed through traditional and newly emphasized baseball statistics. Using hierarchical multiple regression, it was demonstrated that the AIQ predicted statistically significant relationships with hitting and pitching statistics, after controlling for other variables. Given the recent impact of analytics in professional sports, the potential importance of the AIQ in the selection and coaching process was discussed.

Background: Sexually transmitted infections (STIs) and STI treatment remain a challenge in public health. There is little understanding of related factors influencing health seeking behaviour and delay of care among clinic attendees in Jamaica. Aim: To determine socio-demographic profile of clinic attendees with STI and identify factors associated with delay in seeking care for STI-related symptoms. Methods: A cross-sectional study was done. 201 adult patients presenting with STI symptoms from four health centres in Kingston and St Andrew were selected. A 24-item interviewer-assisted questionnaire was used to obtain data on socio-demographic characteristics, patients' symptoms and duration, previous STIs, knowledge of complications and seriousness of STIs, and factors influencing decision to seek medical care. Results: Almost 75% delayed seeking care for STIs. Recurrent STIs was identified in 41% of patients. 'Could not find time' was the most commonly reported reason for delay in seeking care (36%). Females were 3.4 times more likely to delay seeking care for STI symptoms than males (OR 3.42, 95% CI: 1.73–6.73). Those with primary level education and below were 5 times more likely to delay seeking care for STI symptoms than those with at least secondary level education (OR 5.05, 95% CI: 1.09–23.46). Participants viewed staff as confidential (68%) and 65% thought health-care workers spent adequate time during consultations. Conclusion: Lower education level and the female gender are associated with delay in seeking care for STI-related symptoms. Hip arthroplasty is an increasingly prevalent intervention, aimed at reducing pain and restoring function to patients suffering from common musculoskeletal diseases such as arthritis. Metal-on-metal (MoM) hip replacements were intended to be low-wear alternatives to conventional metal-on-polyethylene prostheses. Recent data has shown that revision rates for most MoM prostheses have not been as low as predicted and are not consistent across all models. Many failures result from complications arising from wear debris. Accurately quantifying wear has proved difficult. Using a co-ordinate measuring machine, a method for measuring the wear of ex-vivo MoM hip prostheses was developed and validated as accurate to within 0.5mm3. The method was applied to bearing surfaces and, where available, the internal tapers of femoral heads. Overall, 143 MoM hip explants were measured (95 resurfacings, 48 total hip replacements). Median total wear rates were 4.17mm3/year (mean=11.52, range=0.30-87.28mm3/year), notably higher than most simulator estimates of 1-2mm3/million cycles. Large differences were noted between different models of MoM hip. Time in vivo correlated with wear volume (SRCC=0.387, p<0.001) but not wear rate (SRCC=-0.086, p=0.169) suggesting that the prostheses wore at an unchanging rate through their lifetime. Acetabular cup inclination and anteversion correlated with wear volume (SRCC=0.414 and 0.233) and wear rate (SRCC=0.353 and 0.231, all p<0.001). Wear scars were consistently seen at the rim of the acetabular cup. The distance between these scars and the rim inversely correlated with volume (SRCC=-0.387, p<0.001) and rate (SRCC=-0.357, p<0.001). Patient blood metal ion levels were elevated (median 10.20μg/ml Cr, 9.73μg/ml Co) and correlated with wear volume (p<0.001). Surface roughness measurements were taken on the bearing surfaces and theoretical lubrication regimes (λ-ratio) calculated. There was an inverse correlation between worn λ-ratio and wear volume (p=0.038). Through these findings, recommendations are made for optimising future designs of hip prosthesis to minimise wear.

Solid-supported membranes present a biomimetic platform that can be adapted for bio-physical, biochemical and electrophysiological studies. In addition to this they offer an environment to host membrane proteins for the purposes of biosensing. This thesis examines the use of such a system and the possibilities it presents for the studies of ion channels and their potential applications in biosensing. Electrochemical impedance spectroscopy (EIS) is a powerful technique in the study of solid-supported membranes giving access to capacitance and resistance data, and as such was employed as the main method of characterisation. Electrodes were designed for this purpose in conjunction with Philips Research, and the suitability of the surface for the formation of insulating tethered bilayer lipid membranes (tBLMs). The development of these electrodes led to the incorporation of a SiO2 insulating layer, however its addition resulted in diculties with the formation of self-assembled monolayers (SAMs). However, further refinement of the manufacturing process should resolve these issues. Despite these diffculties, studies were performed using first generation electrodes (P1). Two ionophores, valinomycin and gramicidin, were employed in the characterisation of the ion transport properties of the tBLM system. These studies yielded important information about the structure of the tBLM system under investigation, as well as the ways ion transport can be presented in EIS. Using the work on ionophores as a foundation, an investigation into the incorporation and characterisation of ion channels in tBLM was conducted. Three channels were studied - a ligand-gated eukaryotic Ca2+-permeant channel (TRPC5), a voltage-gated prokaryotic Na+-permeant channel (NavCbt), and a pH-gated K+-permeant channel (KcsA). The success of these studies varied, but provided strong evidence that ion channel incorporation is possible. Further investigation of channel function in the tBLM is required as measured activity is lower than that suggested by literature.

This thesis explores how digital representations of geography and Geographic Information (GI) may be described, and how these descriptions facilitate the use of the resources they depict. More specifically, it critically examines existing geospatial documentation practices and aims to identify opportunities for refinement therein, whether when used to signpost those data assets documented, for managing and maintaining information assets, or to assist in resource interpretation and discrimination. Documentation of GI can therefore facilitate its utilisation; it can be reasonably expected that by refining documentation practices, GI hold the potential for being better exploited. The underpinning theme connecting the individual papers of the thesis is one of multi-granular documentation. GI may be recorded at varying degrees of granularity, and yet traditional documentation efforts have predominantly focussed on a solitary level (that of the geospatial data layer). Developing documentation practices to account for other granularities permits the description of GI at different levels of detail and can further assist in realising its potential through better discovery, interpretation and use. One of the aims of the current work is to establish the merit of such multi-granular practices. Over the course of four research papers and a short research article, proprietary as well as open source software approaches are accordingly presented and provide proof-of-concept and conceptual solutions that aim to enhance GI utilisation through improved documentation practices. Presented in the context of an existing body of research, the proposed approaches focus on the technological infrastructure supporting data discovery, the automation of documentation processes and the implications of describing geospatial information resources of varying granularity. Each paper successively contributes to the notion that geospatial resources are potentially better exploited when documentation practices account for the multi-granular aspects of GI, and the varying ways in which such documentation may be used. In establishing the merit of multi-granular documentation, it is nevertheless recognised in the current work that instituting a comprehensive documentation strategy at several granularities may be unrealistic for some geospatial applications. Pragmatically, the level of effort required would be excessive, making universal adoption impractical. Considering however the ever-expanding volumes of geospatial data gathered and the demand for ways of managing and maintaining the usefulness of potentially unwieldy repositories, improved documentation practices are required. A system of hierarchical documentation, of self-documenting information, would provide for information discovery and retrieval from such expanding resource pools at multiple granularities, improve the accessibility of GI and ultimately, its utilisation.

Phylogenetic analyses including seven new sequences and published data for I. gordonii and its congeners showed the East African species I. usambarensis is sister to I. gordonii, with I. waileriana sister to that species pair. Nine herbarium samples and 18 live plants of /. usambarensis, I. waileriana, I. gordonii and I. thomassetii were scoored for 21 morphometric characters.

- Abstract
Background The World Health Organization (WHO) states that an expected 200 million women have experienced female genital mutilation (FGM) in Africa. The types of cutting vary depending on the individual’s society or location, with four different types presently being practised. The Horn of Africa comprises Somalia, Djibouti, Kenya, Eritrea and Ethiopia, and WHO has listed the following four countries in order of prevalence of FGM practice, with Somalia at 98%, followed by Ethiopia at 92%, Djibouti at 93% and Eritrea at 89% of the female population. In addition to efforts being made to eradicate FGM by WHO, Non-governmental organisations, governments and women organisations, there is also a demand from those who have already undergone this procedure, whether they are in Africa or elsewhere in the world for help with the biopsychosocial consequences of FGM. Method This paper presents the findings of a mixed-method study on the views of a group of vulnerable migrant women with FGM, as well as healthcare practitioners, who consider that there is an unmet demand for better interculturally sensitive healthcare in their adopted country –Germany. The mixed-method study comprised the participation of 48 female volunteers over 18 years of age, recruited with the help of Somali social workers. Using the Metaplan and a quantitative questionnaire, five workshops were conducted in the German towns of Fulda (N=16), Rathenau (N=9), Kassel (N=13) and Berlin (N=10). analysis Results The investigation in Germany was conducted as part of MyHealth, which was a larger European project aimed at improving the healthcare access of vulnerable migrants and refugees (women and unaccompanied minors) newly arrived in Europe, by developing and implementing models based on information gained from a European multidisciplinary Learning Alliance. The overall data gathered revealed a high demand for information and attention to the issue of FGM by both the participants and healthcare professionals. The interaction during data gathering is also suggested a transfer of knowledge between the researchers and the participants, with the result that participants felt they had been heard and were then able to engage with the German healthcare system. Discussion The study highlights the fact that the provision of healthcare in the host country can be improved by providing healthcare workers with intercultural communication skills, tools and cultural awareness training approaches. Moreover, these types of interventions could be adapted to help other vulnerable groups in different healthcare systems worldwide, particularly women’s issues such as postnatal depression, FGM and menopause.

This study sought to examine the internal consistency reliability and underlying factor structure of the Agency of Health Research on Quality Hospital Survey on Patient Safety Culture (AHRQ HSOPSC) in two large acute care hospitals in a developing country setting (Jamaica). A cross-sectional study was done among 328 doctors and nurses. Reliability (internal consistency) analysis was done for each of the 12 composites and the Cronbach's Alpha coefficients were reported. Principal axis factor analysis (PA) using Varimax rotation was done to identify the underlying factor structure. The Cronbach's Alpha coefficient was ≥ 0.60 in ten composites. 'Overall perception of patient safety' and 'staffing' had Cronbach's Alpha values of 0.585 and 0.553 respectively. A ten-factor solution (34 items) with factor loading of ≥ 0.40 is the best model fit, and composites are now named 'handoff and transitions/information exchange,' 'communication/feedback,' 'frequency of events reported,' 'management support for patient safety,' 'teamwork in unit,' 'non-punitive response to errors,' 'overall perception of patient safety,' 'supervision/management expectations and actions promoting patient safety,' 'organizational learning - continuous improvement' and 'staffing'. The AHRQ HSOPSC is generally reliable in this developing country setting and the ten-factor structure is consistent with suggested modifications for the emergent AHRQ HSOPSC Version 2 tool. Introduction: Healthcare decisions, in particular those affecting entire populations, should be evidence-based and taken by decision-makers sharing broad alignment with affected stakeholders. However, criteria, priorities and procedures for decision-making are sometimes non-transparent, frequently vary considerably across equivalent decision-bodies, do not always consider the broader benefits of new health-measures, and therefore do not necessarily adequately represent the relevant stakeholder-spectrum. Areas covered: To address these issues in the context of the evaluation of new vaccines, we have proposed a first baseline set of core evaluation criteria, primarily selected by members of the vaccine research community, and suggested their implementation in vaccine evaluation procedures. In this communication, we review the consequences and utility of stakeholder-centered core considerations to increase transparency in and accountability of decision-making procedures, in general, and of the benefits gained by their inclusion in Multi-Criteria-Decision-Analysis tools, exemplified by SMART Vaccines, specifically. Expert commentary: To increase effectiveness and comparability of health decision outcomes, decision procedures should be properly standardized across equivalent (national) decision bodies. To this end, including stakeholder-centered criteria in decision procedures would significantly increase their transparency and accountability, support international capacity building to improve health, and reduce societal costs and inequity resulting from suboptimal health decision-making.
