2. State-Selective Probing of CO2 Autoionizing Inner Valence Rydberg States with Attosecond Extreme Ultraviolet Four-Wave Mixing Spectroscopy

Abstract

Nonlinear spectroscopies can disentangle spectra that are congested due to inhomogeneous broadening. In conjunction with theoretical calculations, attosecond extreme ultraviolet (XUV) four-wave mixing (FWM) spectroscopy is utilized here to probe the dynamics of autoionizing, inner valence excited Rydberg states of the polyatomic molecule, CO2. This tabletop nonlinear technique employs a short attosecond XUV pulse train and two noncollinear, few-cycle near infrared pulses to generate background-free XUV wave-mixing signals. FWM emission is observed from the n = 5 – 7 states of the Henning sharp ndσ_gRydberg series that converges to the ionic B ̃ (_ ^2)Σ_u^+ state. However, these transient emission signals decay with lifetimes of 33 ± 6 fs, 53 ± 2 fs, and 94 ± 2 fs, respectively, which calculations show are consistent with the lifetimes of the short-lived n = 6 – 8 members of the nsσ_g character Henning diffuse Rydberg series. The oscillator strengths of transitions between states involved in all possible resonant FWM processes are calculated, verifying that the nonlinear spectra are dominated by pathways described by an initial excitation to the diffuse nsσ_g Rydberg series and emission from the sharp ndσ_g Rydberg series. The results substantiate not only that attosecond XUV FWM spectroscopy produces rigorous and meaningful measurements of ultrafast dynamics in polyatomic systems, but also that nonlinear spectroscopic techniques are versatile tools to selectively probe dynamics that are otherwise difficult to access.

Abstract

The endocannabinoid system (ECS) is involved in all aspects of the pathophysiological processes of pain. The nociceptive pathway includes transduction, transmission, modulation and perception, with the ECS playing an essential role in both afferent and efferent pain stimuli. Interest in using cannabinoid products for pain is of continued research and clinical interest. Data in human, lab animals and now companion animals does show efficacy for use especially for chronic pain states. The relative therapeutic index appears favorable and the addition of these compounds at least gives veterinary practitioners another tool to consider for pain management. Careful consideration is important for product selection, not only quality but cannabinoid and terpene profiles that may best fit a patient’s pain state. The clinically relevant science of medical cannabinoids for veterinary patients is in its infancy but is very encouraging. We now have our first well conducted clinical trials with more underway. The future looks promising for medical cannabinoids, specifically CBD, for the treatment of pain conditions in our patients. Given the numerous nuances of products and the ECS dosing for pain, both acute and chronic, dosing should be done in an escalating manner. Reasonable considerations for CBD dominant or isolate products can start as low as 0.5 mg/kg by mouth twice to three times a day and increase significantly until the therapeutic benefit is appreciated.

Abstract

Background Little is known about the incidence of severe critical events in children undergoing general anaesthesia in Europe. We aimed to identify the incidence, nature, and outcome of severe critical events in children undergoing anaesthesia, and the associated potential risk factors. Methods The APRICOT study was a prospective observational multicentre cohort study of children from birth to 15 years of age undergoing elective or urgent anaesthesia for diagnostic or surgical procedures. Children were eligible for inclusion during a 2-week period determined prospectively by each centre. There were 261 participating centres across 33 European countries. The primary endpoint was the occurence of perioperative severe critical events requiring immediate intervention. A severe critical event was defined as the occurrence of respiratory, cardiac, allergic, or neurological complications requiring immediate intervention and that led (or could have led) to major disability or death. This study is registered with ClinicalTrials.gov, number NCT01878760. Findings Between April 1, 2014, and Jan 31, 2015, 31 127 anaesthetic procedures in 30 874 children with a mean age of 6·35 years (SD 4·50) were included. The incidence of perioperative severe critical events was 5·2% (95% CI 5·0–5·5) with an incidence of respiratory critical events of 3·1% (2·9–3·3). Cardiovascular instability occurred in 1·9% (1·7–2·1), with an immediate poor outcome in 5·4% (3·7–7·5) of these cases. The all-cause 30-day in-hospital mortality rate was 10 in 10 000. Abstract

Background: A study to assess the efficacy and patient acceptability of Mechanical Endonasal Dacryocystorhinostomy (MENDCR) in patients with acquired nasolacrimal duct obstruction. Methods: A retrospective series of 38 patients undergoing 37 primary and 7 revision MENDCR's between March 2003 and October 2007. Patients included had symptomatic epiphora with anatomical obstruction on syringing orfunctional obstruction on scintillography. Short-term follow up was assessed subjectively and objectively by anatomical patency on nasendoscopy and free flow of fluorescein from eye to nose. Medium term follow up was assessed subjectively by telephone conversation with the patient. The average follow up period was 25.2 months (range 7-50). Results: There were 44 DCR's performed on 38 patients (12 male, 26 female). Average patient age was 67.0 years (range 16.6-97.5). Almost all patients (95%) presented with epiphora, and a further 34% with dacryocystitis and/or mucocele. At short term follow up 40/44 (91 %) were successful objectively. Of two that failed, neither had a patent ostium and one had recurrent mucocele. Both went on to revision surgery, which was successful at later review. A further two had visible ostia but no free flow of fluorescein. At long term follow up 90% were happy with the procedure and would undergo surgery again. All failures occurred by 3-month follow up. Conclusion: The technigue of MENDCR is a reproducible technique with results comparable to the original authors. MENDCR is an acceptable alternative to external DCR. It is well tolerated by patients most of whom were satisfied and would undergo the same procedure again.

Abstract

Autologous adipose-derived mesenchymal stem cell (AD-MSC) therapy involves harvesting fat from the patient, isolating the stem and regenerative cells, and administering the cells back to the patient. Autologous AD-MSC therapy in veterinary regenerative medicine has been commercially available since 2003. Previously reported results from a blinded, controlled trial in dogs with chronic osteoarthritis of the coxofemoral (hip) joint demonstrated efficacy of a single intraarticular injection of autologous AD-MSC therapy. The primary objective of the current study was to evaluate the effectiveness of this therapy in dogs with chronic osteoarthritis of the humeroradial (elbow) joints and to determine the duration of effect. Fourteen dogs were recruited. Veterinarians assessed each dog for lameness, pain on manipulation, range of motion, and functional disability using a numeric rating scale at baseline and specified intervals up to 180 days after treatment. Statistically significant improvement in outcome measures was demonstrated.

Abstract

Autologous stem cell therapy in the field of regenerative veterinary medicine involves harvesting tissue, such as fat, from the patient, isolating the stem and regenerative cells, and administering the cells back to the patient. Autologous adipose-derived stem cell therapy has been commercially available since 2003, and the current study evaluated such therapy in dogs with chronic osteoarthritis of the hip. Dogs treated with adipose-derived stem cell therapy had significantly improved scores for lameness and the compiled scores for lameness, pain, and range of motion compared with control dogs. This is the first randomized, blinded, placebo-controlled clinical trial reporting on the effectiveness of stem cell therapy in dogs.

Abstract

This article describes the development of and psychometric data on a new approach to the measurement of the empathic quality of counselor behavior. The features of this approach are, first, the division of the empathy construct into a set of components tapping different aspects of empathic behavior; second, greater specification of the construct; and third, focus on the empathic qualities of individual counselor responses. An existing measure, the Lister Empathy Scale, provided the basis for the Response Empathy Rating Scale, which consists of eight components: frame, inference, accuracy, here and now, centrality, words, voice, manner, and impact. Good interrater reliability was found for the whole scale, as well as for all but two of the components. Centrality was the "core" component, and factor analysis suggested two underlying factors, Depth Expressiveness and Empathic Exploration. Evidence for validity was found in correlations with client ratings of feeling understood; however, these correlations were strongest when ratings were averaged to form larger units, such as episodes or sessions.
