Many significant psychosocial problems may go undetected and untreated in adolescents. We aim to determine the prevalence of high-risk behaviors among Malaysian adolescents. Retrospec- tive data analysis was performed using data from the National Health and Morbidity Survey (NHMS) 2017 report. The dataset included 27,497 responses from adolescents aged 13 to 17 years old from five established topographic zones: northern peninsular, east coast peninsular, southern peninsular, central, and east Malaysia. The strength of associations between selected high-risk behaviors and different topographical zones were performed using logistic regression analysis. Adolescents from the northern peninsular reported the highest prevalence of suicidal attempt (27.1%) and active drug user (30.1%). The same region reported the highest prevalence of those who ever had sexual intercourse (SI) (27.2%), with 32.5% who had their first SI before 14 years old, while east Malaysia reported the highest prevalence of current smokers (26.1%) and current alcohol consumers (30.6%). Overall poor lifestyle choices were evident in the northern peninsular region. Hence, specific districts breakdown may enable targeted interventional lifestyle strategies for adolescents at risk.

Preclinical data have consistently suggested the pharmaceutical potential of kratom (Mitragyna speciosa), including as a substitute therapy in opioid dependence. In this case report, we present a case of a man with a long history of kratom use to help him cope with his opiate withdrawalswhile abstaining from illicit drug use. This is the first published detailed case report highlightingthe potential use of kratom to aid opioid dependence encountered in the clinical setting. Our articleis also supported by literature review with regards to kratom’s safety profile and pharmacologicalpotential.

Erotomania is a rare subtype of delusional disorder, whereby the affected person believes that another individual, usually someone with higher socio-economic status, is in love with them despite having little or no contact. To the best of our knowledge, this is thefirst published case of same gender erotomania, involving a 28-year-old single lady and a 42-year-old divorcee towards a female psychiatrist. We aimed to share the challenges experienced by the managing psychiatrist as she inopportunely became the theme of her patients’delusion. We also reviewed and discussed recent literature on erotomania to create awareness among psychiatrists towards this rare psychiatric condition.

Introduction : Acute bilateral vision loss in the absence of organic cause may suggest visual conversion disorder. Case report : Herein, the authors present a rare case of dysthymic disorder complicated with visual conversion disorder, presented as painless acute bilateral loss of vision. A series of ophthalmologic diagnostic tests including the anterior and posterior segment ocular examination, RAPD, fogging test, menace reflex, computed tomography and magnetic resonance imaging of the brain, visual evoked potential (VEP), and electroretinography (ERG) were found to be normal, despite patient having no perception to light. Upon referral to a trained psychiatrist, identification of significant psychological stressors was addressed with supportive pharmacotherapy, psychotherapy, psychoeducation and cognitive behavioural therapy, which gradually restored functional visual acuity. Discussion / Conclusion : Therefore, multidisciplinary approach in functional vision loss should be reinstituted early in therapy to avoid delay in management and clinical outcome. Introduction Methadone substitution therapy aimed to improve the health and social performance of opioid-dependents. In view of the local scarcity of data with regards to the program effectiveness, we aimed to remedy this. Methodology This cross-sectional study involved data transcription from clinical records of active opioid dependents undergoing methadone substitution therapy for the year 2021 in Hospital Tuanku Fauziah, Perlis. Data collected include demographic variables, history of illicit drug use, trend in temporal modulation of methadone dosage, and co-use of illicit drugs. Results There were a total of 87 patients, all were male with the mean age of 43.9±8.33 years old. Majority had been diagnosed with Hepatitis C (n=54, 62.1%). Heroin was the most commonly abused substance prior to program enrolment (n=77, 88.5%), followed by kratom (n=45, 51.7%). For the past three years, majority of our patients had continued illicit drug use during the program (n=51, 58.6%) with the urine spot test in 2019 yielded the highest recorded positive tests (n=32, 36.8%). Methamphetamine and amphetamine co-use were the commonest (n=12, 37.5%). Hepatitis C status was neither associated with the current methadone dose, U=539.5, p=0.186, nor the highest dose required, t=-0.291, df=74, p=0.772. We did not identify any predictive factors to successful drug abstinence during methadone substitution program. There was also a poor correlation between methadone dose and retention rates, r=0.22, p=0.042. Discussion / Conclusion Studies looking into potential factors that influence treatment retention and opioid abstinence during methadone substitution program is needed to optimize national cost for healthcare. Background:Ageing population has increased rapidly over the recentdecades, so as the prevalent of mental illness among theelderly population.Objectives:Our study aimed to investigate the preva- lence of mental health problems among the elderly in oursetting, the common diagnoses of elderly (aged 60 years oldand above) psychiatric referrals and their socio-demograph-ic distribution.Method:This is a retrospective cross-sectional study over three-year period (2015-2017) looking at outpatient records of psychiatric clinic in our locality. Individual medi- cal records were retrieved and data pertaining to demog- raphy, cause of referral, diagnosis and follow-up progresswere transcribed.Results:A total of 233 patients were included in the anal-ysis. Common causes of elderly psychiatric referral to our centre include schizophrenia (26.2%), depression (24.9%), dementia (14.2%) and mood disorders (12.0%). Among thecommonest mental health illness in the elderly, schizophre- nia had the youngest age of referral among the elderly at50.7 ± 16.12 years old while dementia had the oldest (72.7 ± 10.56 years old). The nature of the profession requires routine extended working hours, but little is known about the effect of long working hours on selective physiological measures. A cross-sectional feasibility study was conducted from 1 October 2017 until 30 April 2018 on medical doctors in a tertiary health centre in northwest Malaysia.
