The lifetime recurrence of urolithiasis ranges from 10– 75% creating a public health crisis in affected regions. The epidemiology of urolithiasis in most parts of Africa and Asia remains poorly documented as incidence and prevalence rates in these settings are extrapolated from hospital admissions. The surgical management of kidney and ureteral stones is based on the stone location, size, the patient’s preference and the institutional capacity. To date, the available modalities in the management of urolithiasis includes external shock wave lithotripsy (ESWL), percutaneous nephrolithotomy (PCNL), ureterorenoscopy (URS) including flexible and semirigid ureteroscopy. However, regarding the lack of endourological equipment and expertise in most parts of Sub-Saharan Africa (SSA), most urological centers in these regions still consider open surgery for kidney and ureteral stones. This review explores the current trend and surgical management of upper tract urolithiasis in SSA with insight on the available clinical guidelines.Keywords: endourology, kidney stones, ureteral stones, urolithiasis

Ironically, the global testicular cancer mortality rate has shown a reverse trend to its incidence with higher rates in low- and middle-income countries (0.5 per 100 000) than in high-income countries. Data from GLOBOCAN 2008 have shown relatively high mortality rates in sub-Saharan countries like Mali, Ethiopia, Niger and Malawi. The prognosis of testicular tumor is good with remarkable chemosensitivity to cisplatin-based regimen. Early diagnosis, careful staging and a multidisciplinary management approach is crucial to achieve this optimal result. These results are achievable in the sub-Saharan region if the relevant resources are appropriated for cancer care and clinical guidelines are formulated in a regional context.Keywords: germ cell, orchidectomy, testicular cancer

7. Racial variation in prostate cancer upgrading and upstaging among men with low-risk clinical characteristics

Abstract

Background African American (AA) men suffer a higher prostate cancer (PCa) burden than other groups. Objective We aim to determine the impact of race on the risk of upgrading, upstaging, and positive surgical margins (PSM) at radical prostatectomy (RP) among men eligible for active surveillance. Design, setting, and participants We studied men with low-risk PCa treated with RP at two centers. Low clinical risk was defined by National Comprehensive Cancer Network criteria. Outcome variables were upgrading, upstaging, and PSMs at surgery. Associations between race and the outcomes were evaluated with logistic regression adjusted for age, relationship status, diagnostic prostate-specific antigen level, percentage of positive biopsy cores, surgical approach, year of diagnosis, and clinical site. Results and limitations Of 9304 men diagnosed with PCa, 4231 were low risk and underwent RP within 1 yr. Men were categorized as AA (n = 273; 6.5%), Caucasian (n = 3771; 89.1%), or other racial/ethnic group (Other; n = 187; 4.4%). AA men had a significantly younger mean age (58.7 yr; standard deviation: ±7.06), and fewer (85%) were married or had a partner. Upgrading (34%) and upstaging (13%) rates did not significantly differ among the groups. The PSM rate was significantly higher in AA men (31%) than in the Caucasian (21%) and Other (20%) groups (p < 0.01). We found an association between race group and PSM rate (p < 0.03), with higher odds of PSMs in AA men versus Caucasian men (odds ratio [OR]: 1.64; 95% confidence interval [CI], 1.08-2.47). No statistically significant associations between race and rates of upgrading and upstaging were found. This study was limited by the relatively low proportion of AA men in the cohort. Conclusions Among clinically low-risk men who underwent RP, AA men had a higher likelihood of PSMs compared with Caucasian men. We did not find statistically significantly different rates of upgrading and upstaging between the race groups. Patient summary We analyzed two large groups of men with what appeared to be low-risk prostate cancer based on the initial biopsy findings. The likelihood of finding worse disease (higher grade or stage) at the time of surgery was similar across different racial groups.

Abstract

The transferability and clinical value of genetic risk scores (GRSs) across populations remain limited due to an imbalance in genetic studies across ancestrally diverse populations. Here we conducted a multi-ancestry genome-wide association study of 156,319 prostate cancer cases and 788,443 controls of European, African, Asian and Hispanic men, reflecting a 57% increase in the number of non-European cases over previous prostate cancer genome-wide association studies. We identified 187 novel risk variants for prostate cancer, increasing the total number of risk variants to 451. An externally replicated multi-ancestry GRS was associated with risk that ranged from 1.8 (per standard deviation) in African ancestry men to 2.2 in European ancestry men. The GRS was associated with a greater risk of aggressive versus non-aggressive disease in men of African ancestry (P = 0.03). Our study presents novel prostate cancer susceptibility loci and a GRS with effective risk stratification across ancestry groups.

Abstract

The 2014-2016 Ebola epidemic disrupted childhood immunization in Sierra Leone, Liberia, and Guinea. After the epidemic, the Government of Sierra Leone prioritized community engagement to increase vaccination confidence and uptake. To support these efforts, we examined potential drivers of vaccination confidence and uptake in Sierra Leone.We conducted a population-based household survey with primary caregivers of children in a birth cohort of 12 to 23 months in four districts with low vaccination coverage in Sierra Leone in 2019. Modified Poisson regression modeling with robust variance estimation was used to examine if perceived community engagement in planning the immunization program in the community was associated with vaccination confidence and having a fully vaccinated child.The sample comprised 621 age-eligible children and their caregivers (91% response rate). Half of the caregivers (52%) reported that it usually takes too long to get to the vaccination site, and 36% perceived that health workers expect money for vaccination services that are supposed to be given at no charge. When mothers were the decision-makers of the children's vaccination, 80% of the children were fully vaccinated versus 69% when fathers were the decision-makers and 56% when other relatives were the decision-makers. Caregivers with high confidence in vaccination were more likely to have fully vaccinated children compared to caregivers with low confidence (78% versus 53%). For example, caregivers who thought vaccines are 'very much' safe were more likely to have fully vaccinated children than those who thought vaccines are 'somewhat' safe (76% versus 48%). Overall, 53% of caregivers perceived high level of community engagement, 41% perceived medium level of engagement, and 6% perceived low level of engagement. Perceiving high community engagement was associated with expressing high vaccination confidence (adjusted prevalence ratio (aPR) = 2.60; 95% confidence interval (CI) = 1.67-4.04) and having a fully vaccinated child (aPR = 1.67; 95% CI = 1.18-2.38).In these four low coverage districts in Sierra Leone, the perceived level of community engagement was strongly associated with vaccination confidence among caregivers and vaccination uptake among children. We have provided exploratory cross-sectional evidence to inform future longitudinal assessments to further investigate the potential causal effect of community engagement on vaccination confidence and uptake.

Abstract

Objective To quantify the potential impact of engaging religious leaders in promoting safe burial practices during the 2014–2016 Ebola virus disease outbreak in Sierra Leone. Methods We analysed population-based household survey data from 3540 respondents collected around the peak of the outbreak in Sierra Leone, December 2014. Respondents were asked if in the past month they had heard an imam or pastor say that people should not touch or wash a dead body. We used multilevel logistic regression modelling to examine if exposure to religious leaders’ messages was associated with protective burial intentions if a family member died at home and other Ebola protective behaviours. Findings Of the respondents, 3148 (89%) had been exposed to faith-based messages from religious leaders on safe Ebola burials and 369 (10%) were unexposed. Exposure to religious leaders’ messages was associated with a nearly twofold increase in the intention to accept safe alternatives to traditional burials and the intention to wait ≥ 2 days for burial teams (adjusted odds ratio, aOR: 1.69; 95% confidence interval, CI: 1.23–2.31 and aOR: 1.84; 95% CI: 1.38–2.44, respectively). Exposure to messages from religious leaders was also associated with avoidance of traditional burials and of contact with suspected Ebola patients (aOR: 1.46; 95% CI: 1.14–1.89 and aOR: 1.65; 95% CI: 1.27–2.13, respectively). Conclusion Public health messages promoted by religious leaders may have influenced safe burial behaviours during the Ebola outbreak in Sierra Leone. Engagement of religious leaders in risk communication should be prioritized during health emergencies in similar settings.

Abstract

Introduction Caring for an Ebola patient is a known risk factor for disease transmission. In Sierra Leone during the outbreak in 2014/2015, isolation of patients in specialised facilities was not always immediately available and caring for a relative at home was sometimes the only alternative. This study sought to assess population-level protective caregiving intentions, to understand how families cared for their sick and to explore perceived barriers and facilitators influencing caregiving behaviours. Methods Data from a nationwide household survey conducted in December 2014 were used to assess intended protective behaviours if caring for a family member with suspected Ebola. Their association with socio-demographic variables, Ebola-specific knowledge and risk perception was analysed using multilevel logistic regression. To put the results into context, semi-structured interviews with caregivers were conducted in Freetown. Results Ebola-specific knowledge was positively associated with the intention to avoid touching a sick person and their bodily fluids (adjusted OR (AOR) 1.29; 95% CI 1.01 to 1.54) and the intention to take multiple protective measures (AOR 1.38; 95% CI 1.16 to 1.63). Compared with residing in the mostly urban Western Area, respondents from the initial epicentre of the outbreak (Eastern Province) had increased odds to avoid touching a sick person or their body fluids (AOR 4.74; 95% CI 2.55 to 8.81) and to take more than one protective measure (AOR 2.94; 95% CI 1.37 to 6.34). However, interviews revealed that caregivers, who were mostly aware of the risk of transmission and general protective measures, felt constrained by different contextual factors. Withholding care was not seen as an option and there was a perceived lack of practical advice. Background: Perceived susceptibility to a disease threat (risk perception) can influence protective behaviour. This study aims to determine how exposure to information sources, knowledge and behaviours potentially influenced risk perceptions during the 2014-2015 Ebola Virus Disease outbreak in Sierra Leone. Methods: The study is based on three cross-sectional, national surveys (August 2014, n = 1413; October 2014, n = 2086; December 2014, n = 3540) that measured Ebola-related knowledge, attitudes, and practices in Sierra Leone. Data were pooled and composite variables were created for knowledge, misconceptions and three Ebola-specific behaviours. Risk perception was measured using a Likert-item and dichotomised into 'no risk perception' and 'some risk perception'. Exposure to five information sources was dichotomised into a binary variable for exposed and unexposed. Multilevel logistic regression models were fitted to examine various associations. Results: Exposure to new media (e.g. internet) and community-level information sources (e.g. religious leaders) were positively associated with expressing risk perception. Ebola-specific knowledge and hand washing were positively associated with expressing risk perception (Adjusted OR [AOR] 1.4, 95% Confidence Interval [CI] 1.2-1.8 and AOR 1.4, 95% CI 1.1-1.7 respectively), whereas misconceptions and avoiding burials were negatively associated with risk perception, (AOR 0.7, 95% CI 0.6-0.8 and AOR 0.8, 95% CI 06-1.0, respectively). Conclusions: Our results illustrate the complexity of how individuals perceived their Ebola acquisition risk based on the way they received information, what they knew about Ebola, and actions they took to protect themselves. Community-level information sources may help to align the public's perceived risk with their actual epidemiological risk. 35. Patterns of Local Failure following Radiation Therapy for Prostate Cancer

Abstract

© 2015 American Urological Association Education and Research, Inc. Purpose: Little is known about patterns of local failure following radiation therapy for prostate cancer. We aimed to characterize post-radiation biopsy findings, including the treatment effect and the zonal distribution of recurrent disease after radiation therapy, in men experiencing biochemical recurrence. Materials and Methods: We identified patients who received post-radiation biopsy in the setting of biochemical recurrence following primary radiation for localized disease. Histological post-radiation biopsy results were categorized by the absence of tumor, demonstration of radiation treatment effect, failure (recurrent cancer) or a combination of treatment effect and failure. We described patterns of histological failure and compared them to the diagnostic biopsy findings. Results: A total of 284 men underwent mapped post-radiation biopsy for biochemical recurrence. Mean age at initial diagnosis was 63 years and median prostate specific antigen was 8.2 ng/ml. Of the men 33%, 32% and 35% were classified at low, intermediate and high risk based on clinical CAPRA (Cancer of the Prostate Risk Assessment) categories. Median time to post-radiation biopsy was 61 months after treatment. Findings were negative in 4% of cases while we noted a treatment effect in 31%, failure in 45% and a combination in 20%. Failure rates were similar across sextants. Of 140 patients with mapped pretreatment and posttreatment biopsies 4% demonstrated cancer in a new location previously identified as negative. Gleason upgrading occurred in 43% of cases with 85% upgraded to 4 + 3 or higher. Conclusions: Men with rising prostate specific antigen after radiotherapy for prostate cancer most often experience recurrence in dominant tumor sites. Whether failure is due to inadequate targeting, dosing or intrinsic radiation resistance remains unknown to our knowledge. Further study is warranted.

Psychosis is a major factor of this catastrophic condition. The major challenge in this condition for the urologist is the preservation of the distal segment and the limited time to reference in order to perform microsurgical repair. 37. Implementation of PSA-based active surveillance in prostate cancer

Abstract

Prostate cancer screening had led to the diagnosis of a large proportion of localized and low-risk disease. Many of these cancer cases are believed to be indolent and would not be clinically perceived in the absence of screening. In addition to that, the wide use of active treatment has exposed these patients to treatment-related quality-of-life impact. In this setting active surveillance as a way of deferring active treatment and reserving such treatment to cases of disease progression only has gained interest. PSA has been widely used to identify patients eligible for active surveillance and also for disease monitoring. The goal of this review was to describe the place of PSA in the monitoring of patients under active surveillance based on the existing studies and to discuss the importance of PSA in light of other existing or emerging tools to monitor prostate cancer in active surveillance. © 2014 Future Medicine Ltd.

39. Résection transurétrale pour hypertrophie bénigne de la prostate au Sénégal

Abstract

Le but de cette étude était d’évaluer la résection transurétrale pour l’hypertrophie bénigne de la prostate au Sénégal. Il s’agissait d’une étude rétrospective descriptive menée entre le 1 janvier 2006 et le 31 décembre 2008. Durant cette période, nous avions enregistré 84 cas de RTUP dont 51 ont fait l’objet de notre étude. Trente trois cas ont été exclus parce que l’anatomo-pathologie révélait un adénocarcinome prostatique. La moyenne d’âge était de 72,49 ans. Le volume prostatique moyenétait de 39,17 ml. La durée moyenne d’intervention était de 55, 6min. En post-opératoire immédiat les complications étaient essentiellement à type d’ hémorragie (5 cas soit 10%). La durée moyenne du cathétérisme opératoire était de 3,1 jours. Les patients avaient séjourné en moyenne 4,45 jours à l’hôpital. A l’ablation de la sonde urinaire, les complicationsétaient à type de RAU (3 cas), des fuites d’urines (2 cas ), d’orchiépididymite (1 cas) et 2 cas de chute d’ escarres . A distance, il y’ avait une dysurie (4 cas), une sténose du méat urétral. La résectiontransurétrale est le traitement de référence de l’hypertrophie bénigne de la prostate actuellement. Cependant en Afrique subsaharienne, l’adenomectomie prostatique prime encore sur la résection.Mots clés :Adénome de la prostate, résection transurétrale, morbidité.

Abstract

Prostatic sarcoma is a rare pathologic feature accounting for 0.1-0.2% of prostatic neoplasms. This condition can occur at different ages and there is no clear consensus regarding the treatment. The authors report a case of a giant prostatic sarcoma in a patient aged 62 years old who complains of voiding symptoms and a pelvic and abdominal mass. The mass did not respond to chemotherapy and radiation therapy was not available. The patient presented therefore a therapeutic problem due to big size of the tumor that does not allow a surgical removal.

42. Primary Varicocele and Fertility: Postoperative Evaluation of Spermiogram and Pregnancy Parameters

Objectif: Il n'existe pas de consensus concernant la capacité de la varicocélectomie à améliorer la fertilité et la fécondité. L'objectif de cette étude rétrospective était d'étudier l'évolution postopératoire de la fertilité masculine (par les paramètres du spermogramme) et celle de la fécondité du couple. Patients et méthodes: Il s'agit d'une étude rétrospective de 50 patients porteurs de varicocèle palpable opérés selon la technique de Palomo au service d'Urologie de l'Hôpital Grand Yoff, Dakar, Sénégal. Les paramètres étudiés sont les paramètres du spermogramme (densité, mobilité totale à la première heure, vitalité) et du spermocytogramme (taux de spermatozoïdes de morphologie normale), une fois avant l'opération et à deux reprises après (entre le 3ème et le 8ème mois et à partir du 9ème mois) et la fécondité postopératoire du couple chez 35 patients. Résultats : L'étude du spermogramme retrouve, pour l'ensemble des patients, une élévation de la valeur moyenne de tous les paramètres, sans normalisation, exception faite de la morphologie (spermocytogramme). L'évolution postopératoire n'était statistiquement significative que pour la densité. La majorité des patients (76% à 92%) présentaient des valeurs du spermogramme préopératoires inférieures à la normale, excepté pour la morphologie (28%) (spermocytogramme). Les paramètres du spermogramme se sont améliorés pour 48 à 64% des patients selon le paramètre étudié. L'amélioration des paramètres est d'autant plus importante que leur valeur préopératoire moyenne était basse. Les facteurs de bon pronostic postopératoires sont représentés par les patients les plus jeunes (moins de 35 ans) et ceux qui présentaient une varicocèle bilatérale, une infertilité de type secondaire et une durée d'infertilité moins longue. Nous n'avons pas étudié la corrélation entre le grade de la varicocèle et la fécondité. Le taux de grossesse postopératoire était de 31,4%. Les sujets féconds étaient plus jeunes, avaient une durée moyenne de l'infertilité moins longue et des paramètres constamment plus élevés par rapport aux patients inféconds. Les améliorations postopératoires de la fertilité et de la fécondité dans cette étude étaient relatives et limitées. Conclusion: Par la fréquence de la varicocèle et ses conséquences sur la fertilité, il s'avère nécessaire aussi de mieux et de plus informer les populations sur cette affection, ses conséquences et les résultats de son traitement sur la fertilité. De plus une collaboration plus étroite entre gynécologues, biologistes de la reproduction, radiologues et urologues-andrologues permettrait une prise en charge intégrée de l'infertilité du couple. Il est également nécessaire, concernant l'infertilité, d'améliorer la prise en charge psychologique du couple, et notamment celle de la conjointe. Objective: There is no general consensus as to the the ability of varicocelectomy to improve fertility. The objective of this retrospective study was to evaluate the post-operative improvement in male fertility (based on sperm parameters) and the pregnancy rate. Patients and Methods: This is a retrospective study of 50 patients with palpable varicocele who were subjected to varicocelectomy by the Palomo technique at the Urological Department of Grand Yoff Hospital, Dakar, Senegal. The examined parameters were the spermiogram characteristics (concentration, total motility after 1 hour, vitality) and the spermiocytogram (percentage of normal morphology). These were tested once before and twice after the operation (between 3 and 9 months postoperatively and then again after 9 months); the postoperative pregnancy rate was evaluated in 35 patients. Results: In all patients the spermiogram revealed an improvement in the average values of all parameters, without actual normalization, except for morphology (spermiocytogram). The post-operative results were statistically significant only for the sperm concentration. The majority of patients (76% to 92%) showed pre-operative spermiogram values below normal, except for morphology (28%) (spermiocytogram). In 48% to 64% of patients the spermiogram characteristics improved, depending on the parameter taken into consideration. Improvement was particularly more evident in those patients who had presented with low average values. Prognostic factors predicting a good postoperative outcome were the age of the patients (less than 35 years), bilateral varicocele, secondary infertility and a shorter duration of infertility. We did not study the possible correlation between the degree of varicocele and pregnancy. The post-operative pregnancy rate was 31.42%. The fertile patients were younger, had a shorter average duration of previous infertility and had parameters which were constantly higher as compared to those patients who remained infertile. The post-operative improvements in fertility and pregnancy were quite limited in this study. Conclusion: Due to the relatively high incidence of varicocele in the male population, it appears necessary to improve awareness on its occurrence, its consequences and the results of its treatment on fertility. Moreover, a stronger cooperation between gynecologists, reproductive biologists, radiologists and uro-andrologists may allow for an integrated study of infertile couples. 45. Profil clinique du cancer de la prostate au Sénégal

Abstract

Buts: Décrire les caractéristiques cliniques du cancer de la prostate au Sénégal. Matériels et Méthodes: Nous avons réalisé une étude rétrospective descriptive sur 121 patients suivis pour cancer de la prostate dans deux hôpitaux de Dakar entre 1997 et 2002 et diagnostiqués par l'histologie sur biopsie prostatique ou sur pièce de résection prostatique. Nous avons étudié: l'âge des patients, le taux de PSA, le stade tumoral selon la classification TNM de 1997, la circonstance du diagnostic et les métastases. Résultats: L'âge moyen était de 69 ans (52-88 ans), le taux moyen de PSA de 72,2 ng/ml (6 - 578,9 ng/ml). Les tumeurs localisées, d'une fréquence de 43,8% étaient dominées par les formes T2 (42,1%) avec 41,3% de formes avancées (T3-T4). Les métastases existaient dans 15,7% avec 78% de formes indéterminées. Les circonstances de diagnostic étaient dominées par les symptômes liés à la prostate ou au cancer représentant 49,6%. L'association symptômes et élévation du taux de PSA représentait 16,5%. Conclusion: Cette étude a montré un âge élevé de nos patients corrélé à une élévation du PSA, un stade tumoral avancé, une fréquence des métastases et l'existence de symptômes. D'où l'intérêt du dépistage pour une meilleure prise en charge du cancer de la prostate. Clinical Profile of Prostate Cancer in Senegal Objectives: To describe the clinical features of prostate cancer in Sénégal. Materials and Methods: We underwent a descriptive and retrospective study including 121 patients managed for prostate cancer in two hospitals of Dakar between 1997 and 2002. The histological diagnosis was assessed on prostate biopsy or after transurethral resection of the prostate or prostatectomy. We studied: the age of the patients, the PSA level, the stage of the tumor using the TNM staging of 1997, and the circumstances of diagnosis. Results: The mean age was 69 years (52-88), the mean PSA was 72,2 ng /ml (6-578,9). The localized tumors that represented 43,8% of the cases were mainly T2 (42,1%), while the advanced (T3-T4) tumors represented 41,3% of the cases. Metastases were found in 15% of cases, and in 78% of the cases the metastases could not be assessed. The circumstances of diagnosis were dominated by prostate or cancer-related symptoms which represented 49,6% of the cases. The association between symptoms and high PSA was present in 16,5% of cases. Conclusion: This study shows an elevated age of our patients correlated with a high PSA level, an advanced tumor stage, an important rate of metastases and the presence of symptoms.
