هدفت هذه الدراسة إلى تقييم فعالية برنامج إرشادي باستخدام فنيات العلاج بالقبول والالتزام في تخفيف حدة الشعور بالضغوط النفسية لدى أمهات الأطفال ذوي الإعاقة العقلية، باستخدام التصميم شبه تجريبي مع عينة مكونة من 24 أم من أمهات ذوي الإعاقة العقلية بالملتحق أبنائهن بمراكز الرعاية النهارية، تم توزيعهن عشوائيا إلى مجموعتين تجريبية وضابطة قوام كل منها (12). وتم تطبيق مقياس الضغوط النفسية للأمهات الأطفال المعاقين إعداد/ الشخص والسرطاوي (1998)، ومقياس المستوى الاقتصادي والاجتماعي والثقافي إعداد/ سعفان وخطاب (2016). وبرنامج إرشادي قائم على فنيات العلاج بالقبول والالتزام (إعداد/ الباحثتين). وتوصلت نتائج الدراسة إلى وجود فروق ذات دلالة إحصائية بين متوسطات الرتب لدرجات المجموعتين التجريبية والضابطة على مقياس الضغوط النفسية لأمهات الأطفال ذوي الإعاقة العقلية في القياس البعدي لصالح المجموعة التجريبية، ما يعني انخفاض الضغوط النفسية لدى المجموعة التجريبية. في القياس البعدي ، وعدم وجود فروق ذات دلالة أحصائية في القياس التتبعي مما يدل على فعالية البرنامج الإرشادي القائم على فنيات العلاج بالقبول والالتزام في تخفيف حدة الشعور بالضغوط النفسية واستمرار تأثير البرنامج في خلال فترة المتابعة.

Introduction: Deep venous thrombosis (DVT) is a coagulation disorder that is on the rise due to lifestyle changes. Patients with DVT are at risk for developing a life-threatening complication, pulmonary embolism which urges the importance of its early diagnosis. The Gold standard modality for its early diagnosis is invasive which is Venography. Other modalities like compression ultrasound and duplex imaging are noninvasive but operator-dependent and not routinely available during evening and night shift hours. D-Dimer is a simple, noninvasive, cost-effective, and easily available diagnostic test that can be helpful in diagnosis. The objective of this study is to compare the sensitivity of D-Dimers with Doppler ultrasound for the diagnosis of deep venous thrombosis. Materials and Methods: A cross-sectional comparative (validation) study in which 65 admitted patients with a high risk of DVT on Well’s criteria for clinical diagnosis were enrolled by non-probability convenience sampling from March to August 2018. Both D-Dimer and Doppler ultrasounds were performed on all suspected patients of DVT. Results: Amongst 65 cases, 60 (92.3%) had positive Dimer results compared to 65 (100.0%) with the Doppler ultrasound. The difference between the two diagnostic modalities was statistically significant (p-value = 0.02). The sensitivity of D-dimers was 92.3% however specificity, PPV, and NPV were 0% as none of the cases was negative on ultrasound Doppler. Conclusion: The D-dimer test is a conveniently performed, cost-effective test with a sensitivity of more than 90% in the current study and can be useful for Emergency and critical care departments.

Background: The revolutionary technology of smartphone-based retinal imaging has been consistently improving over the years. Smartphone-based retinal image acquisition devices are designed to be portable, easy to use, and cost-efficient, which enables eye care to be more widely accessible especially in geographically remote areas. This enables early disease detection for those who are in low- and middle- income population or just in general has very limited access to eye care. This study investigates the limitation of smartphone compatibility of existing smartphone-based retinal image acquisition devices. Additionally, this study aims to propose a universal adapter design that is usable with an existing smartphone-based retinal image acquisition device known as the PanOptic ophthalmoscope. This study also aims to simulate the reliability, validity, and performance overall of the developed prototype. Methods: A literature review has been conducted that identifies the limitation of smartphone compatibility among existing smartphone-based retinal image acquisition devices. Designing and modeling of proposed adapter were performed using the software AutoCAD 3D. For the proposed performance evaluation, finite element analysis (FEA) in the software Autodesk Inventor and 5-point scale method were demonstrated. Results: Published studies demonstrate that most of the existing smartphone-based retinal imaging devices have compatibility limited to specific older smartphone models. This highlights the benefit of a universal adapter in broadening the usability of existing smartphone-based retinal image acquisition devices. A functional universal adapter design has been developed that demonstrates its compatibility with a variety of smartphones regardless of the smartphone dimension or the position of the smartphone's camera lens. The proposed performance evaluation method generates an efficient stress analysis of the proposed adapter design. The end-user survey results show a positive overall performance of the developed universal adapter. However, a significant difference between the expert's views on the developed adapter and the quality of images is observed. Conclusion: The compatibility of existing smartphone-based retinal imaging devices is still mostly limited to specific smartphone models. Besides this, the concept of a universal and suitable adapter for retinal imaging using the PanOptic ophthalmoscope was presented and validated in this paper.
