Abstract

Novel lipid-lowering therapies are being introduced. Few studies exist of the real-world effectiveness of adenosine-tri-phosphate citrate lyase inhibition with bempedoic acid. This study audited bempedoic acid therapy in 216 consecutive patients from three hospital centres – a university hospital (n = 77) and two district general hospitals (n = 106 and 33). Cardiovascular disease (CVD) risk factors, prescription qualification criteria, efficacy and adverse effects were assessed. The population was aged 65.9 ± 11.0 years, 42% were male, 25% had type 2 diabetes, and 31% had familial hypercholesterolaemia. CVD was present in 19% and multibed vascular disease in 8%. Statin intolerance was reported in 92%. Bempedoic acid reduced total cholesterol by 1.58 ± 1.44 mmol/L (20%), LDL-C by 1.37 ± 1.31 mmol/L (27%), triglycerides by 0.22 mmol/L (2%) with an 0.06 mmol/L (1%) increase in HDL-C after 22 ± 9 months follow-up. An LDL-C <2.5 mmol/L was achieved in 40% and <2 mmol/L in 20%. Efficacy (r2 =.33) was predicted by baseline LDL-C (β =.54; p <.001). No significant changes were seen in transaminases, creatinine, creatine kinase, urate or HbA1c. Treatment was discontinued by 33% of patients and occurred due to myalgia (43%), lack of efficacy (16%) and gastrointestinal adverse effects (15%). No cases of gout were observed. In a logistic regression only the number of previous drug classes not tolerated (β = 1.60; p =.009) was a contributing factor to discontinuation. This audit suggests that bempedoic acid therapy is effective but that adverse effects and discontinuation are common. This suggests nocebo effects might be generalizable to all lipid-lowering drug therapies in susceptible individuals. [ABSTRACT FROM AUTHOR]

Abstract

Due to the current ecological situation in the world, the demand for renewable energy is growing. Its use does not burden the environment because resources are renewed in a short time, unlike traditional fuels - coal or oil. It can be concluded that the main task of humanity is to choose the appropriate technology for obtaining renewable energy and adapt it to local conditions. The aim of the work is to present the technical, economic and environmental potential of using the wave energy of the Baltic Sea and the possibility of converting this energy into electricity. The theoretical and technical potential of sea wave energy that Poland could use was estimated. The most advanced methods of converting wave energy into electricity were presented, and the most promising technical solutions were selected for use in the Polish Exclusive Economic Zone and off the coast of our country. The selection of the most promising technologies - single and combined wave energy converters was based on reports on the operation of the first experimental wave power plants and wave converters operating in other waters. In terms of technology and economics, obtaining energy from sea waves is much more difficult than using solar or wind energy. However, along with the development of offshore wind farms and access to the infrastructure of the National Power System and the need to give up traditional, emission-related energy sources, wave power plants may have a chance for development in the Polish energy sector. 40. THE VALUE OF OCCUPATIONAL SAFETY – ESTIMATION OF LOST PRODUCTIVITY IN THE POLISH AGRICULTURAL INDUSTRY

Abstract

The study was to assess the level of lost productivity of human resources in the Polish agricultural industry determined by working conditions. The assumed analysis was conducted in a time frame by assessing the accident ratio and its economic consequences in Polish agriculture. The estimation of costs of lost productivity is based on the adoption of the so-called human capital approach. A comparison of the lost product value in the A section – “Agriculture, Forestry, Fishing”, with average results recorded in the economy indicates high divergence resulting both from existing differences in the scale of burden with consequences of incapacity for work and from the conducted valuation of working time against mean values in economy. Depending on the adopted criterion for valuation of the unit of working day, estimated values of lost productivity in the A section per one injured fluctuate between 22 and 150% of mean values obtained for economy in total, and between 30 and 212% per 1,000 persons working in the given section (year 2016). The obtained results, apart from indicating an economic aspect of relations between occupational safety and productivity, constitute the basis for discussion on the profitability of health “interventions”.

Abstract

Rationale: Megalophthalmos anterior is a rare, bilateral, nonprogressive, hereditary, congenital disorder, characterized by the enlargement of all anterior segment structures of the eye, with megalocornea, iris atrophy, and zonular abnormalities commonly found. Usually almost asymptomatic in young patients, with most complaints concerning blurred vision due to the common corneal astigmatism, it might in time lead to several complications including premature cataract formation and pigmentary glaucoma. Patient concerns: This review presents the case of a 47-year old patient referred to our clinic for traumatic cataract surgery, with striking bilateral megalocornea, somehow overlooked during previous ophthalmic examinations in his local outpatient clinic. Diagnosis: We noticed markedly enlarged corneas and deepened anterior chambers of his both eyes, accompanied by intumescent, white cataract of the right eye, and incipient cortical cataract of the left eye. Best corrected visual acuity (BCVA) was counting fingers in the right eye and 20/25 in the left eye. Additional examination revealed multiple abnormalities of the anterior segment, leading to the diagnosis of anterior megalophthalmos. It is astounding the patient remained undiagnosed during previous examinations, with his megalocornea and remarkably deep anterior chamber so apparent. Interventions and outcomes: We performed standard phacoemulsification procedure, with 3 piece posterior chamber intraocular lens (PCIOL) implantation into the lens capsule. The surgery was uneventful, with postoperative BCVA of 20/20 in the right eye, and no dislocation of the lens in 9-month observation period. Lessons: Complicated cataract in patients with anterior megalophthalmos can be successfully treated with standard phacoemulsification procedure followed by the bag PCIOL implantation.Care needs to be taken during cataract surgery in these patients, as zonular abnormalities and lens enlargement are common, resulting in increased rate of intra- and postoperative complications. As patients with anterior megalophthalmos require a more careful follow-up.

Abstract

This paper discusses Jarosław Kaczyński’s public utterances from 2007 to 2010. It explains the typical features of a language, political language and the language of opposition. J. Kaczyński’s political language was the language of a leading oppositionist. Populism, performative speech, thematic persistence, labeling, verbosity, prolixity, rhetorical tricks, neologisms, sense of efficacy, arbitrary description of reality are all the examples of his public utterances. The statesman denied many elements of Polish political system However, the democratic foundations of the III RP as well as the legitimacy of PO-PSL coalition were not questioned.

Celem artykułu jest ocena nierówności występujących na poziomie krajów Unii Europejskiej w obszarze zdrowia. Analizy dokonano w ujęciu miasto – wieś, w kontekście ich oddziaływania na potencjał kapitału ludzkiego. Analizie poddano zróżnicowanie zmiennej opisującej poziom samooceny stanu zdrowia mieszkańców obszarów wiejskich i miejskich krajów UE. Wykorzystując dane statystyczne na poziomie NTS-1, wskazano na charakter badanych procesów w ujęciu państw członkowskich. Zidentyfikowano odrębność zachowań mieszkańców obszarów wsi i miast. Na potrzeby badania wyodrębniono populację kobiet i mężczyzn w dwóch grupach wiekowych. Wykazano znacząco wyższy poziom nierówności w obszarze zdrowia w relacji miasto – wieś, występujący w grupie krajów członkowskich Środkowej i Wschodniej Europy.

Abstract

Celem artykułu jest ocena stopnia zagrożenia obszarów wiejskich krajów UE zjawiskiem ekskluzji społecznej. Analizie poddano zmienne opisujące poziom ubóstwa, bezrobocia oraz wykształcenia mieszkańców wsi. W celu kwantyfikacji badanego zjawiska wykorzystano miernik syntetyczny pozwalający wskazać obszary wiejskie krajów UE o najwyższym stopniu zagrożenia. Podjęto próbę określania specyfiki obszarów wiejskich krajów „starej” i „nowej” UE w kontekście zróżnicowania przyjętych w badaniu czynników ekskluzji. Uzyskane wyniki sugerują, że w przypadku badanego zjawiska nie występuje klasyczny podział na problem krajów UE-15 i UE-CEE, jednak dokonana ocena różnic w rozkładzie poszczególnych czynników ekskluzji w obu grupach referencyjnych wskazała, że kraje Europy Środkowej i Wschodniej, przy stosunkowo korzystnej strukturze wykształcenia mieszkańców wsi, są nadal w dużo większym stopniu zagrożone ryzykiem ubóstwa.
