The primary and first goal of the DLCR was to improve survival and the overall clinical management of Danish lung cancer patients. Study population: All Danish primary lung cancer patients since 2000 are included into the registry and the database today contains information on more than 50,000 cases of lung cancer. Main variables: The database contains information on patient characteristics such as age, sex, diagnostic procedures, histology, tumor stage, lung function, performance, comorbidities, type of surgery, and/or oncological treatment and complications. Since November 2013, DLCR data on Patient -Reported Outcome Measures is also included. Descriptive data: Results are primarily reported as quality indicators, which are published online monthly, and in an annual report where the results are commented for local, regional, and national audits. Indicator results are supported by descriptive reports with details on diagnostics and treatment. Conclusion: DLCR has since its creation been used to improve the quality of treatment of lung cancer in Denmark and it is increasingly used as a source for research regarding lung cancer in Denmark and in comparisons with other countries. 2. $L^1$ contraction for bounded (non-integrable) solutions of degenerate parabolic equations

Abstract

We obtain new $L^1$ contraction results for bounded entropy solutions of Cauchy problems for degenerate parabolic equations. The equations we consider have possibly strongly degenerate local or non-local diffusion terms. As opposed to previous results, our results apply without any integrability assumption on the %(the positive part of the difference of) solutions. They take the form of partial Duhamel formulas and can be seen as quantitative extensions of finite speed of propagation local $L^1$ contraction results for scalar conservation laws. A key ingredient in the proofs is a new and non-trivial construction of a subsolution of a fully non-linear (dual) equation. Consequences of our results are maximum and comparison principles, new a priori estimates, and in the non-local case, new existence and uniqueness results.

Abstract

We derive and analyze monotone difference-quadrature schemes for Bellman equations of controlled Levy (jump-diffusion) processes. These equations are fully non-linear, degenerate parabolic integro-PDEs interpreted in the sense of viscosity solutions. We propose new ``direct'' discretizations of the non-local part of the equation that give rise to monotone schemes capable of handling singular Levy measures. Furthermore, we develop a new general theory for deriving error estimates for approximate solutions of integro-PDEs, which thereafter is applied to the proposed difference-quadrature schemes.

Abstract

Background With a growing population of cancer survivors in Denmark, the evaluation of health-related quality of life (HRQoL) has become increasingly important. We describe variations in HRQoL between educational groups in a national population of cancer survivors. Methods We conducted a cross-sectional questionnaire study among breast, prostate, lung, and colon cancer survivors diagnosed in 2010–2019 in Denmark. We used the EORTC QLQ-C30 to assess HRQoL including physical, role, emotional, cognitive, social functioning, and symptoms (fatigue, nausea and vomiting, pain, dyspnea, insomnia, appetite loss, constipation, diarrhea, and financial difficulties). Information on educational level and clinical data were extracted from national registers and clinical databases. Levels of impaired functioning and severe symptoms were identified using newly established thresholds for clinical importance. Multivariate logistic regression was used to examine associations between education and HRQoL. All statistical tests were 2-sided. Results In total, 27,857 (42%) participated in the study. Up to 72% and 75% of cancer survivors with short education (≤9 years) reported impaired functioning and severe symptoms, respectively. Cancer survivors with short compared to long education (>12 years) were more likely to report impaired functioning and severe symptoms, with for example significantly higher odds ratios (ORs) for impaired physical function (breast OR = 2.41, 99% CI = 2.01–2.89; prostate OR = 1.81, 99% CI = 1.48–2.21; lung OR = 2.97, 99% CI = 1.95–4.57; and colon cancer OR = 1.69, 99% CI = 1.28–2.24). Conclusions Cancer survivors with short education are at greater risk of impaired HRQoL than survivors with long education 2–12 years after diagnosis. This underscores the need for systematic screening and symptom management in cancer aftercare, in order to reach all cancer survivors, also cancer survivors with short education.

Abstract

Background: Lung cancer is the leading cause of cancer-related death in all Nordic countries which, though similar in demographics and healthcare systems, have noticeable differences in lung cancer survival. Historically, Denmark and Finland have had higher lung cancer incidences and lower survival than Norway and Sweden. All four countries have national cancer registries. Data in these registries are often compared, but their full potential as a source of learning across the Nordic countries is impeded by differences between the registries. In this paper, we describe and compare the Nordic registries on lung cancer–specific data and discuss how a more harmonized registration practice could increase their usefulness as a source for mutual learning and quality improvements. Methods: We describe and compare the characteristics of data on lung cancer cases from registries in Denmark, Finland, Norway and Sweden. Moreover, we compare the results from the latest annual reports and specify how data may be acquired from the registries for research. Results: Denmark has a separate clinical lung cancer registry with more detailed data than the other Nordic countries. Finland and Norway report lung cancer survival as relative survival, whereas Denmark and Sweden report overall survival. The Danish Lung Cancer Registry and the Swedish Cancer Registry do not receive data from the Cause of Death registries in contrast to the Finnish Cancer Registry and the Cancer Registry of Norway. Conclusion: The lung cancer registries in Denmark, Finland, Norway and Sweden have high level of completeness. However, several important differences between the registries may bias comparative analyses.

Abstract

Introduction Treatment targeting the KRAS p.G12C (G12C) mutation was approved by both the U.S. Food and Drug Administration and the European Medicines Agency in the past year as second line of treatment (LOT2) for advanced non-small-cell lung cancer (aNSCLC). However, the treatment-eligible G12C mutant NSCLC population has not been well characterized, nor has its outcomes with current treatment approaches. The aim of this study was to characterize a nationwide real-world G12C mutant NSCLC population and to estimate its overall survival (OS) in Denmark. Section snippets Methods Adult aNSCLC patients diagnosed between January 1st, 2018 and October 31st, 2020 were identified in the Danish Lung Cancer Register. The register has captured data on all Danish lung cancer patients since 2003. Additional information was gathered from the following registers: the Danish National Patient Register, the Danish Pathology Register, the Danish Civil Registration System, the Danish National Prescription Register, the Danish Education Register, and the Income Statistics Register. Results We identified 6,058 patients with aNSCLC. A total of 36.5% (n=2,214) were tested with next-generation sequencing (NGS) or polymerase chain reaction (PCR) tests for a KRAS mutation prior to LOT1. Among those tested, 40.5% (n=896) were KRAS mutated, 9.3% (n=206) were G12C mutated, and 39.0% (n=864) were KRAS/EGFR/ALK wild-type (Triple WT). Co-mutation with EGFR was present in 11% of G12C-mutated patients. Key characteristics and outcomes are presented in Table 1. TTNT was similar among Conclusions This Danish real-world study evaluating aNSCLC patients after the implementation of targeted therapy and immunotherapy showed that patients with a KRAS G12C mutation had a longer OS from LOT1 and LOT2 compared with Triple WT patients. Abstract

PURPOSE Given the poor results using hypofractionated radiotherapy for early breast cancer, a dose of 50 Gy in 25 fractions (fr) has been the standard regimen used by the Danish Breast Cancer Group (DBCG) since 1982. Results from more recent trials have stimulated a renewed interest in hypofractionation, and the noninferiority DBCG HYPO trial (ClincalTrials.gov identifier: NCT00909818) was designed to determine whether a dose of 40 Gy in 15 fr does not increase the occurrence of breast induration at 3 years compared with a dose of 50 Gy in 25 fr. PATIENTS AND METHODS One thousand eight hundred eighty-two patients .40 years of age who underwent breast-conserving surgery for node-negative breast cancer or ductal carcinoma in situ (DCIS) were randomly assigned to radiotherapy at a dose of either 50 Gy in 25 fr or 40 Gy in 15 fr. The primary end point was 3-year grade 2-3 breast induration assuming noninferiority regarding locoregional recurrence. RESULTS A total of 1,854 consenting patients (50 Gy, n 5937; 40 Gy, n 5917) were enrolled from 2009-2014 from eight centers. There were 1,608 patients with adenocarcinoma and 246 patients with DCIS. The 3-year rates of induration were 11.8% (95% CI, 9.7% to 14.1%) in the 50-Gy group and 9.0% (95% CI, 7.2% to 11.1%) in the 40-Gy group (risk difference, 22.7%; 95% CI, 25.6% to 0.2%; P 5 .07). Systemic therapies and radiotherapy boost did not increase the risk of induration. Telangiectasia, dyspigmentation, scar appearance, edema, and pain were detected at low rates, and cosmetic outcome and patient satisfaction with breast appearance were high with either no difference or better outcome in the 40-Gy cohort compared with the 50-Gy cohort. The 9-year risk of locoregional recurrence was 3.3% (95% CI, 2.0% to 5.0%) in the 50-Gy group and 3.0% (95% CI, 1.9% to 4.5%) in the 40-Gy group (risk difference, 20.3%; 95% CI, 22.3% to 1.7%). Abstract

The past 20 years have resulted in unprecedented progress in understanding brain energy metabolism and its role in health and disease. In this review, which was initiated at the 14th International Society for Neurochemistry Advanced School, we address the basic concepts of brain energy metabolism and approach the question of why the brain has high energy expenditure. Our review illustrates that the vertebrate brain has a high need for energy because of the high number of neurons and the need to maintain a delicate interplay between energy metabolism, neurotransmission, and plasticity. Disturbances to the energetic balance, to mitochondria quality control or to glia-neuron metabolic interaction may lead to brain circuit malfunction or even severe disorders of the central nervous system (CNS). We cover neuronal energy consumption in neural transmission and basic ('housekeeping') cellular processes. Additionally, we describe the most common (glucose) and alternative sources of energy namely glutamate, lactate, ketone bodies and medium chain fatty acids. We discuss the multifaceted role of non-neuronal cells in the transport of energy substrates from circulation (pericytes and astrocytes) and in the supply (astrocytes and microglia) and usage of different energy fuels.
