More severe vertebral fractures have more personal impact. In the European Prospective Osteoporosis Study, more severe vertebral collapse was predictable from prior fracture characteristics. Subjects with bi-concave or crush fractures at baseline had a 2-fold increase in incident fracture size and thus increased risk of a disabling future fracture. Introduction: According to Euler's buckling theory, loss of horizontal trabeculae in vertebrae increases the risk of fracture and suggests that the extent of vertebral collapse will be increased in proportion. We tested the hypothesis that the characteristics of a baseline deformity would influence the size of a subsequent deformity. Methods: In 207 subjects participating in the European Prospective Osteoporosis Study who suffered an incident spine fracture in a previously normal vertebra, we estimated loss of volume (fracture size) from plane film images of all vertebral bodies that were classified as having a new fracture. The sum of the three vertebral heights (anterior, mid-body, and posterior) obtained at follow-up was subtracted from the sum of the same measures at baseline. Each of the summed height loss for vertebrae with a McCloskey-Kanis deformity on the second film was expressed as a percentage. Results and Conclusions: In univariate models, the numbers of baseline deformities and the clinical category of the most severe baseline deformity were each significantly associated with the size of the most severe incident fracture and with the cumulated sum of all vertebral height losses. In multivariate modeling, age and the clinical category of the baseline deformity (crush > bi-concave > uni-concave > wedge) were the strongest determinants of both more severe and cumulative height loss. Baseline biconcave and crush fractures were associated at follow-up with new fractures that were approximately twice as large as those seen with other types of deformity or who previously had undeformed spines. In conclusion, the characteristics of a baseline vertebral deformity determines statistically the magnitude of vertebral body volume lost when a subsequent fracture occurs. Because severity of fracture and number of fractures are determinants of impact, the results should improve prediction of the future personal impact of osteoporosis once a baseline prevalent deformity has been identified.

Vertebral height ratios are used to define vertebral deformity in clinical and epidemiologic studies of vertebral osteoporosis. However, few data have been obtained on the variation in these ratios in different populations using standard methods. We examined vertebral morphometric measurements obtained in a population survey from three centers: Malmo (Sweden), Montceau-les-Mines (France), and Graz (Austria), to study the influence of sex and the population center on vertebral height ratios. Radiographs were obtained according to a standardized protocol, and morphometric measurements, anterior height Ha, central height Hc, and posterior height Hp, made in Berlin. The height ratios anterior, Ha/Hp, central, Hc/Hp, posterior I, Hp/Hp', and posterior II, Hp/Hp" (Hp' = posterior height of vertebrae above, Hp" = posterior height of vertebrae below) were calculated for each vertebra from T4 to L4. The mean and standard deviation of these ratios for each sex and each center were derived using a statistical trimming procedure to normalize the distribution. Threshold values for defining grade 1 and grade 2 deformities, wedge, biconcavity, and compression, were calculated using these parameters. Anterior and central vertebral height ratios were smaller in males than females (p < 0.01). There were significant differences between the three centers (p < 0.01) both in the trimmed mean values for anterior and central vertebral height ratios and in the thresholds derived using standard criteria for defining wedge and biconcavity deformity. The data confirm the impression from single-center studies that vertebral height ratios vary between populations and suggest that reference values for vertebral height ratios should be derived separately for males and females within individual populations whenever possible.

To determine the frequency of advanced clinical and radiological features of AS with reference to gender, onset of symptoms and disease duration. Fifty-seven patients diagnosed with AS were included in this study. Functional evaluation of the musculoskeletal system detected advanced clinical features: rubber-ball phenomenon, flattening of the chest anterior wall, diastasis of rectus abdominis muscle, steel back phenomenon, umbilical extrusion, skiing posture. Conventional radiographs of sacroiliac joints, pelvis and axial skeleton were obtained in order to analyze signs of sacroiliitis, syndesmophytes, vertebral squaring and ligamentous ossification. Statistical significance is found in the distribution of particular advanced clinical and radiological features of AS between men and women: rubber-ball phenomenon (p = 0.002), flat chest (p = 0.002), diastasis of rectus abdominis muscle (p = 0.002), skiing position (p = 0.000), syndesmophytes (p = 0.009) and ligamentous ossification (p = 0.030) in thoracic and lumbar spine. Onset of first disease symptoms (> 20 years of age) is significantly associated with radiological changes in thoracic spine (ligamentous ossification, p = 0.015) and cervical spine (vertebral squaring, p = 0.032). Longer disease duration (> 10 years) is significantly associated with the appearance of particular clinical features: rubber-ball phenomenon, p < 0.01; rectus abdominis diastasis, p=0.042) and radiological changes of sacroiliac joints (grade IV sacroileitis, p = 0.012), thoracic and lumbar spine (syndesmophytes, p = 0.015; ligamentous ossification, p = 0.027). Our study shows that the occurrence of clinical and some radiological features of AS appears to be gender dependent. Furthermore, onset of first disease symptoms (> 20 years of age) and longer disease duration (> 10 years) are associated with the higher risk of developing particular clinical signs and radiological features in sacroiliac joints and axial skeleton.

The Summary In 27 centres across Europe, the prevalence of deforming spinal Scheuermann's disease in age-stratified population-based samples of over 10,000 men and women aged 50+ averaged 8 % in each sex, but was highly variable between centres. Low DXA BMD was un-associated with Scheuermann's, helping the differential diagnosis from osteoporosis. Introduction This study aims to assess the prevalence of Scheuermann's disease of the spine across Europe in men and women over 50 years of age, to quantitate its association with bone mineral density (BMD) and to assess its role as a confounder for the radiographic diagnosis of osteoporotic fracture. Methods In 27 centres participating in the population-based European Vertebral Osteoporosis Study (EVOS), standardised lateral radiographs of the lumbar and of the thoracic spine from T4 to L4 were assessed in all those of adequate quality. The presence of Scheuermann's disease, a confounder for prevalent fracture in later life, was defined by the presence of at least one Schmorl's node or irregular endplate together with kyphosis (sagittal Cobb angle > 40A degrees between T4 and T12) or a wedged-shaped vertebral body. Alternatively, the (rare) Edgren-Vaino sign was taken as diagnostic. The 6-point-per-vertebral-body (13 vertebrae) method was used to assess osteoporotic vertebral shape and fracture caseness. DXA BMD of the L2-L4 and femoral neck regions was measured in subsets. We also assessed the presence of Scheuermann's by alternative published algorithms when these used the radiographic signs we assessed. Results Vertebral radiographic images from 4486 men and 5655 women passed all quality checks. Prevalence of Scheuermann's varied considerably between centres, and based on random effect modelling, the overall European prevalence using our method was 8 % with no significant difference between sexes. The highest prevalences were seen in Germany, Sweden, the UK and France and low prevalences were seen in Hungary, Poland and Slovakia. Centre-level prevalences in men and women were highly correlated. Scheuermann's was not associated with BMD of the spine or hip. Conclusions Since most of the variation in population impact of Scheuermann's was unaccounted for by the radiological and anthropometric data, the search for new genetic and environmental determinants of this disease is encouraged.

Lean body stature and tallness have both been identified as risk factors for hip fracture. In this study, height and weight data from a multinational multicenter study were used' to compare Oslo, which has some of the highest incidence rates of hip fracture ever reported, to other regions of Europe, with respect to height and body mass index. More than 17, 000 subjects in six age strata (50–54, 55–59, 60–64, 65–69, 70–74, 75+ years) from 36 centers in 19 European countries were enrolled in the European Vertebral Osteoporosis Study (EVOS), which included standardized height and weight measurements. We found that men in Oslo were 4.3 cm taller than men in western Europe, 5.0 cm taller than men in eastern Europe, and 8.6 cm taller than men in southern Europe. Oslo women were also taller, by 2.2 cm compared to women in western Europe, 2.7 cm compared to women in eastern Europe, and 5.2 cm compared to women in southern Europe. In all age groups, except women aged 55–59 years, mean body mass index (BMI) was lowest in Oslo. Nearly twice as many had a BMI less than 22.0 kg/m2 in Oslo compared to the other regions combined (11.1% vs. 6.6% in men and 19.2% vs. 9.9% in women). This study indicates that the people of Oslo are taller and leaner than people in other regions of Europe. This may in part explain the higher incidence of hip fracture in the population of Oslo.

BACKGROUND The European Vertebral Osteoporosis Study Group (EVOS) developed a questionnaire, back translated into 14 different European languages, for use in a multinational epidemiological study of vertebral osteoporosis. We investigated the reproducibility of this questionnaire in four of the participating study centres. METHODS In all 151 men and women, aged 50-85 years, from Lubeck (Germany), Malmo (Sweden), Warsaw (Poland) and Oviedo (Northern Spain), were retested with the questionnaire on two occasions using a different observer within a 28-day period. RESULTS Questions relating to personal or medical history were more reproducible than questions concerning subjective symptoms or aspects of lifestyle. The level of agreement for the non-ordinal categorical variables, as estimated by kappa, varied from 0.38 to 1.00 across the four centres. Agreement for the multicategory ordinal, mainly lifestyle, questions was in general poorer though improved when a weighted analysis was performed. For continuous data the 95% limits of agreement were narrow, and there was no evidence of bias between interviewers. There were no important differences in reproducibility across the four centres for either categorical or continuous data. CONCLUSION The study indicates that the questionnaire may produce useful and comparable information concerning risk factors for osteoporosis across different countries and in different languages. It also highlights that questionnaire instruments designed for use in multinational population-based studies may provide data of comparable quality across a range of settings.

Quantitative dermatoglyphic analysis of digitopalmar ridge count was used to research psoriatic symmetrical polyarthritis in fifty women. Analyzed were 25 dermatoglyphics traits: number of epidermal ridges on all ten fingers, their sum for five and ten fingers, four traits on both palms, i.e. between a-b, b-c, c-d and a-d triradii, and atd angles and their bilateral sum. The data obtained were compared with those recorded in a control group of 200 pairs of imprints of phenotypically healthy females from Zagreb area. Statistically significant differences were found in 13 variables in decreased ridge count in all ten fingers, their sum in five and ten fingers separately. Accordingly, a polygenetic system identical in some loci to polygenic system predisposing to women psoriatic symmetrical polyarthritis susceptibility might be found responsible for the dermatoglyphic pattern development., U radu se kvantitativnom analizom digitopalmarnog kompleksa istražio broj grebenova u 50 žena sa simetričnim psorijatičnim poliartritisom. Analizirano je 25 varijabli, broj grebenova na svih deset prstiju šaka, zatim, sveukupno na pet i deset prstiju, te između triradijusa a-b, b-c, c-d i a-d na oba dlana, zatim, njihov ukupan broj na jednom i oba dlana, te atd kutovi na oba dlana i njihov ukupni broj u stupnjevima. Dobiveni podaci uspoređeni su sa kontrolnom skupinom - 200 pari otisaka odraslih i fenotipski zdravih žena Zagrebačke regije. Statistički značajne razlike prema kontroli nađene su u 13 varijabli u smislu smanjenja broja kožnih grebenova na svih deset prstiju, i njihovu zbroju na pet i deset prstiju zasebno. Stoga se dade zaključiti kako je poligeneski sustav u razvoju dermatoglifa identičan s nekim lokusima za razvoj simetričnog psorijatičnog poliartritisa u žena.

Childhood fractures are common. Their clinical relevance to osteoporosis and fractures in later life is unclear. The aim of this study was to determine the predictive risk of childhood fracture on the risk of fracture in later life. Men and women ≥50 yr of age were recruited from population registers for participation in the European Prospective Osteoporosis Study (EPOS). Subjects completed an interviewer administered questionnaire that included questions about previous fractures and the age at which the first of these fractures occurred. Lateral spine radiographs were performed to ascertain prevalent vertebral deformities. Subjects were followed prospectively by postal questionnaire to determine the occurrence of clinical fractures. A subsample of subjects had BMD measurements performed. Cox proportional hazards model was used to determine the predictive risk of childhood fracture between the ages of 8 and 18 yr on the risk of future limb fracture and logistic regression was used to determine the association between reported childhood fractures and prevalent vertebral deformity. A total of 6451 men (mean age, 63.8 yr) and 6936 women (mean age, 63.1 yr) were included in the analysis. Mean follow-up time was 3 yr. Of these, 574 (8.9%) men and 313 (4.5%) women reported a first fracture (any site) between the ages of 8 and 18 yr. A recalled history of any childhood fracture or forearm fracture was not associated with an increased risk of future limb fracture or prevalent vertebral deformity in either men or women. Among the 4807 subjects who had DXA measurements, there was no difference in bone mass among those subjects who had reported a childhood fracture and those who did not. Our data suggest that self-reported previous childhood fracture is not associated with an increased risk of future fracture in men or women.

U izdanju Birotiska izašla je 2008. godine knjiga, u hrvatskom i engleskom izdanju, posvećena reumatologiji od začetaka postojanja pojma reume u staroj Grčkoj do njenog razvitka kao grane medicine te razvojnim spoznajama u području reumatologije do 2000. godine, knjiga posvećena prikazu spoznaja ovog područja medicine tijekom dva tisućljeća, pod nazivom RAZVOJ REUMATOLOGIJE TIJEKOM DVA TISUĆLJEĆA (hrvatsko izdanje) i THE DEVELOPMENT OF RHEUMATOLOGY THROUGH TWO MILLENIA (englesko izdanje). Autori knjige Ivo Jajić i Zrinka Jajić u suradnji s nacionalnim i sveučilišnim knjižnicama Europe i SAD, kolegama diljem svijeta zaljubljenicima ove struke koji su posebnim žarom i voljom od samog početka ovog projekta sudjelovali u prikupljanju materijala i informacija , uz veliku zahvalnost na svesrdnoj pomoći, osmislili su ovu publikaciju, jedinstveno djelo ovoga tipa u svijetu, kao više sadržajnih cjelina posvećenih razvoju reumatologije u svjetskim razmjerima. U dijelu knjige pod nazivom Prikaz povijesnih zbivanja, autori su dali prikaz povijesnih zbivanja u svjetskoj reumatologiji od rane povijesti i značenja grčke riječi „reuma“- strujanje, uvođenja pojma reumatologije i reumatologa, razvitka specijalizacije iz reumatologije, osnivanje nacionalnih reumatoloških društava u Europi i SAD-u, do prve održane stručne konferencije liječnika o reumatskim bolestima 1932. godine u New Orleansu, potom kongresa reumatologa pojedinih zemalja te međunarodnih skupova. Značajno mjesto zauzima prikaz osnivanja Međunarodne liga protiv reumatizma koju čine četiri regionalne lige: Europska (EULAR), Američka (PANLAR), Azijsko-australska (APLAR) i Afrička (AFLAR) a potom i zbivanja i aktivnosti vezano uz pojedine regionalne lige. Publikacije iz područja reumatologije autori ovog djela prikazali su kroz pregled najvažnijih svjetskih reumatoloških časopisa od početka osnivanja. U povijesnom pregledu značajno mjesto zauzimaju i kronološki pregledi klasifikacija i nomenklatura reumatskih bolesti, dijagnostičkih kriterija za reumatske bolesti, prvim profesorima reumatologije na svjetskim sveučilištima i prikaz Nobelovih nagrada za medicinu važnih za reumatologiju. Drugi najopširniji dio knjige pod nazivom Autori koji su pridonijeli razvoju reumatologije, na 372 bogato ilustrirane stranice sa više od 1000 slika, abecedni je prikaz autora koji su svojim radom doprinijeli razvoju reumatologije u svijetu, njih ukupno 2343 i to ne samo reumatologa već i epidemiologa, patologa, imunologa, biokemičara, okulista, dermatologa, povjesničara medicine, pedijatara, specijalista nuklearne medicine, gastroenterologa, radiologa te liječnika drugih specijalnosti. Prezentirani su biografskim podacima, svojim užim područjem rada (specijalizacija), kratkim prikazom publikacije sa izvornim podacima iz literature te je za većinu autora objavljena i fotografija. U trećem dijelu knjige pod nazivom Iz povijesti razvoja hrvatske reumatologije dat je kratki prikaz na osam stranica o povijesnih zbivanja iz razvoja reumatologije u Hrvatskoj. Na kraju knjige nalaze se detaljno obrađeni Kazalo imena i Kazalo pojmova. Knjigu su recenzirali akademik Zvonko Kusić i akademik Dragan Dekaris. Ovo potpuno originalno osmišljeno djelo u domaćoj i svjetskoj literaturi, valja svakako preporučiti reumatolozima ali i ostalim liječnicima srodnih struka, povjesničarima medicinskih znanosti, koji će u njemu naći mnoštvo interesantnih spoznaja i predstavljati poticaj za daljnji razvoj ove struke kod nas i u svijetu.

A book entitled The Development of Rheumatology through Two Millennia (English edition), dedicated to rheumatology from the initial concepts of rheuma in ancient Greece through its development as a field of medicine and accumulation of knowledge in the field of rheumatology to the year 2000, bringing an overview of this medical profession over two millennia, was published by Birotisak, Zagreb, Croatia, in 2008. Ivo Jajić and Zrinka Jajić, professors of rheumatology at School of Medicine University of Zagreb, Croatia, the authors of the book, in collaboration with national and university libraries from Europe and USA, and with renowned experts from all over the world, true lovers of the profession, having whole- heartedly taken part in the material and information collection, have designed this book as a unique work of the type in the world, consisting of a number of sections dedicated to the global development of rheumatology. The part of the book on historical events provides an overview of historical events in international rheumatology from ancient times and the Greek word rheuma meaning flux, introducing the terms rheumatology and rheumatologist, development of residency in rheumatology, establishment of national societies of rheumatology in Europe and the USA, through the first professional medical conference on rheumatic diseases, held in 1932 in New Orleans, congresses of rheumatologists in particular countries and international meetings. Foundation of the International League Against Rheumatism including four regional leagues, i.e. European (EULAR), American (PANLAR), Asian–Pacific (APLAR) and African (AFLAR), and all major events and activities related to particular leagues are given due emphasis. Publications in the field of rheumatology are presented through an overview of the most important international rheumatology journals from their launching. This historical overview bring a chronologic survey of classifications and nomenclature of rheumatic diseases, diagnostic criteria for rheumatic diseases, first professors of rheumatology at international universities and Nobel Prize winners in medicine relevant for rheumatology. The second, most extensive section of the book is dedicated to the authors worldwide that have contributed most to the development of rheumatology. On 372 pages with more than 1000 illustrations, this section presents 2343 authors, in alphabetical order, not only rheumatologists but also epidemiologists, pathologists, immunologists, biochemists, ophthalmologists, dermatologists, medical historians, paediatricians, nuclear medicine specialists, gastroenterologists, radiologists and other specialists engaged in rheumatology. They all are presented by their biographic data, special field of work (specialty), brief presentation of respective publications with original literature data, and a photo (most of them). The third section entitled From the History of Croatian Rheumatology, brings a short survey of historical events in the development of rheumatology in Croatia. Cilj istraživanja je bio utvrditi neka klinička obilježja ankilozantnog spondilitisa (AS) u epidemiološkom istraživanju putem poštom odaslanog upitnika. U analiziranom uzorku bolesnika (N=92) bilo je 60 muškaraca i 32 žene. U većine bolesnika (69 ili 75%) bolest traje više od 15 godina. Dijagnoza AS je postavljena prosječno 6, 5 godina nakon početka tegoba. Glavni simptomi bolesti su zakočenost i bol. Bolnu oteklinu jednog ili više zglobova imalo je 58 (63, 7%) bolesnika. Najčešće su aficirani kukovi (75: 81, 5%) i ramena (68: 73, 9%). Upalu očiju navodi 53 bolesnika (57, 6%), a upalu crijeva u smislu enterokolitisa nespecificirane geneze njih 16 (17, 6%).

21. Scintigrafija u ranoj dijagnostici sindroma sekundarne hipertrofične osteoartropatije

Obrađivali smo 23 bolesnika sa SHO. AHO se razvila u 21 slučaju s karcinomom bronha. U ostalih bolesnika ona je bila povezana s fibrozom pluća, bronhiektazijama, pneumonijom i malignim tumorom medijastinuma. Uzeta je iscrpna anamneza, status, učinjena rendgenska pretraga i scintigrafija skeleta. Scintigrafija je učinjena tako da je u kubitalnu venu apliciran spoj disfonata obilježen s 99 mTc-TMDP-"Osteocis" u dozi od 740 MBq. U 5 bolesnika nađena je blaga periostoza na tibiji i fibuli i pozitivno vezivanje radiofarmaka. U ostalih bolesnika otkriven je vrlo intenzivno nakupljen radioizotop na više mjesta na skeletu, a rendgenski se nije otkrila periostoza. U zaključku možemo kazati da je scintigrafija osjetljiva metoda kojom se otkriva povećana funkcija koštanog tkiva.

26. Effects of Smoking and Alcohol Consumption on Vertebral Deformity in the Elderly –An Epidemiological Study

The aim of the study was to assess the role of smoking and alcohol consumption as possible risk factors for vertebral deformities in an elderly Croatian population sample. Data on smoking habit, alcohol consumption, body mass index, and overall life activity were collected in 425 randomly chosen community dwelling subjects. Radiographic morphometric method was used to assess vertebral deformities of thoracic and lumbar spine. Men smoked and drank significantly more than women. There was no association of either smoking status or number of cigarettes, or frequency of alcohol intake with prevalent vertebral deformities. There was a tendency of an increased risk of vertebral deformities in heavy drinkers (OR=1.69 ; 95% CI=0.98–2.91), and a reduced risk of these deformities in female regular drinkers (OR=0.72 ; 95% CI=0.14–3.66). Further studies in the Croatian population are needed to establish the association of smoking and alcohol consumption with vertebral deformities.

Autori Ivo Jajić i Zrinka Jajić, profesori Medicinskog fakulteta Sveučilišta u Zagrebu objavili su 2005. godine u izdanju Medicinske naklade knjigu Izvanzglobni reumatizam i srodna stanja. Udžbenik je namijenjen reumatolozima, fizijatrima, ortopedima, internistima te liječnicima medicine rada i opće medicine, broji 322 stranice te obiluje slikovnim prikazima izvanzglobnih bolesti i sindroma (249 slika) i tabličnim prikazima (l3 tablica). Tvrdog je uveza i formata 17x24 cm. Građa je podijeljena na trinaest poglavlja: Izvanzglobne reumatske bolesti, Sistemne i lokalizirane izvanzglobne reumatske bolesti, Bolesti uslijed odlaganja kalcijevih soli, Sistemne i lokalizirane bolesti kostiju, Reumatski aspekti nekih endokrinopatija i metaboličkih bolesti, Izvanzglobne manifestacije upalnih reumatskih bolesti, Nekrotizirajući sindromi, Sindromi temporomandibularnog područja, Intermitentni i periodični sindromi i Tumori mekih tkiva u okolici zglobova. Vezano uz pojedina poglavlja dat je prikaz svjetski priznatih autora koji su svojim radom doprinjeli razvoju reumatologije. Na kraju svake tematske jedinice opisa bolesti odnosno sindroma navedena je izdašna suvremena literatura koja omogućava daljnje proširenje znanja. Ovo djelo predstavlja novost u domaćoj literaturi te kao prvijenac sustavno upotpunjava poznavanje ovih stanja koja predstavljaju jedan od najčešćih reumatskih sindroma i bolesti suvremenog načina života. Anamneza i fizikalni pregled dva su osnovna elementa u dijagnostičkom postupku, koja ni danas, u doba razvijene tehnologije i sofisticiranih dijagnostičkih tehnika, nisu izgubili ništa od svoje nekadašnje vrijednosti i dalje ostaju vrlo važni i gotovo nezamjenljivi. Prvo izdanje pojavilo se prije nekolko godina, a ovo, drugo dopunjeno je novim spoznajama opisujući nove testove i nove činjenice iz područja anamneze i fizikalnog kliničkog pregleda lokomotornog sustava. Udžbenik je namijenjen ne samo specijalistima reumatologije, fizikalne medicine i rehabilitacije već i ostalim graničnim strukama : ortopedija, neurologije, medicina rada, primarna zdravstvena zaštita, kao i specijalizantima i studentima, svima koji žele produbiti znanje iz problematike lokomotornog sustava. polimijalgija reumatika ,arteritis divovskih stanica - Abstract
Autori monografije Ivo Jajić i Zrinka Jajić, profesori Medicinskog fakulteta Sveučilišta u Zagrebu, objavili su 2003. monografiju Polimijalgija reumatika i arteritis divovskih stanica, prvijenac u hrvatskoj literaturi. Monografija o polimijalgiji reumatici i arteritisu divovskih stanica cjelovito je djelo koje se prvi put pojavljuje u našoj literaturi i predstavlja novinu u spoznaji ovog entiteta. Namijenjena je liječnicima reumatolozima, fizijatrima, internistima, ortopedima, okulistima, patohistolozima, liječnicima medicine rada, obiteljskim liječnicima te polaznicima poslijediplomske nastave iz gore navednih struka, sa namjerom ranog otkrivanja i adekvatnog liječenja. Sastoji se od 17 poglavlja i to: uvoda, definicije, nomenklature, povijesti, epidemiologije, etiologije, patogeneze, patoanatomske i kliničke slika, odnosa polimialgije reumatike i arteritisa divovskih stanica, laboratorijskih nalaza, dijagnostike, diferencijalne dijagnoze, liječenja, tijeka i prognoze. Na kraju udžbenika nalazi se sadržaj i kazalo, a iza svakog poglavlja napisana je bogata literatura koje je vezan uz tekst i predstavlja mogućnost daljnje izvora podataka. Poglavlja su bogato dokumentirana i ilustrirana, sadržajno obuhvaća brojne tablice, grafičke prikaze te slike patoanatomskih nalaza i kliničkih manifestacija , kako bi čitava građa bila lako razumljiva u ranom postavljanju dijagnoze i u poduzimanju adektvatnoga suvremenog liječenja. Recenzenti djela su eminentni stručnjaci prof. dr. sc. Zdravko Mandić i prof. dr. sc. Mladen Belicza.

Autori knjige Ivo Jajić i Zrinka Jajić , profesori Medicinskog fakulteta Sveučilišta u Zagrebu objavili su 2003. godine u izdanju Medicinske naklade, monografiju Algodistrofični sindrom, koja je prvijenac u našoj literaturi. Algodistrofični sindrom predstavlja komplikaciju cijeljenja traumatskih, upalnih ili nekih drugih procesa na lokomotorom sustavu i nerazjašnjenih drugih etioloških čimbenika. Njegova je definicija, za razliku, od drugih bolesti, neobično komplicirana i teška a nomenklatura nije jedinstvena. Pristup takvim bolesnicima je stoga insuficijentan i polipragmatičan. To je bio razlog pisanju ove monografije, koja zasigurno popunjava prazninu u domaćoj literaturi i pružit će čitateljima brojne odgovore na pitanja vezana uz ovaj entitet. Preporučuje se fizijatrima, reumatolozima, ortopedima, traumatolozima, krurzima, neurolozima, liječnicima medicine rada. Sastoji se od 17 poglavlja i to: definicije, nomenklature, povijesnog pregleda, epidemiologije, etiologije i patogeneze, patološkoanatomske slike i patofizioloških zbivanja , kliničke slike, topografskih oblika algodistrofičnog sindroma, radioloških i laboratorijskih nalaza i ostale dijagnostike, diferencijalne dijagnoze, liječenja, kliničkog tijeka i prognoze algodistrofičnog sindroma. Udžbenik ima sadržaj i kazalo, a iza svakog poglavlja nalazi se najnovija literatura vezana uz tekst koja omogućava daljne proširenje znanja. Većina je poglavlja bogato dokumentirana i ilustrirana (povjesna zbivanja, kliničke slike, scintigrafske i rentgenske slike , elektroneurografski nalaza) kako bi čitava građa bila lako razumljiva u ranom postavljanju dijagnoze i u poduzimanju adektvatnoga suvremenoga liječenja. Djelo su recenzirali eminentni stručnjaci : prof. dr. sc. Krešimir Koržinek, prof. dr. sc. Šime Mihatov i dr. sc. Simeon Grazio.

Cilj je istraživanja bio na uzorku od 31 bolesnika odrediti učinkovitost terapijskog ultrazvuka (UZV) u liječenju bolesnika s kroničnom križoboljom. Rezultati su pokazali da je primjena terapijskog UZV-a bila učinkovita u smanjenju boli, ali ne i u poboljšanju funkcijskog statusa slabinske kralješnice u bolesnika s kroničnom križoboljom.

Cilj je istraživanja bio utvrditi prevalenciju osteoporotskih fraktura kralješnice i odrediti njihov utjecaj na simptome i aktivnosti svakodnevnog života na randomiziranome uzorku starije populacije grada Zagreba. U 425 ispitanika (156 muškaraca, 269 žena), dobi 50 i više godina, neposrednim su intervjuom dobiveni podatci o simptomima i mogućnostima obavljanja aktivnosti svakodnevnog života. Kod svakog je bolesnika standardiziranim postupkom rendgenski snimljena grudna i slabinska kralješnica. Za definiranje fraktura kralješaka upotrebljena je morfometrijska metoda po McCloskeyju i sur., a za određivanje normalnih vrijednosti omjera dimenzija nefrakturiranih kralježaka iz radiografije uzorka opće populacije metoda po Meltonu i sur. Prevalencija ispitanika s frakturom kralježaka bila je 11, 8% (muškarci 15, 4%, žene 9, 7%). Ne postoji značajna povezanost nalaza osteoporotskih deformacija kralježaka i boli u leđima. Iako postoji tendencija povećanog rizika nalaza fraktura kralježaka u ispitanika koji su imali ikad doživljenu bol u leđima, kao i onih s bolovima posljednjih godinu dana, te onih s jačom intenzivnošću posljednje atake boli u leđima, rezultati nisu dostigli statističku značajnost. Osobe s frakturama kralježaka nemaju bitno smanjene funkcijske sposobnosti u odnosu na osobe istoga spola i dobi bez fraktura.

Cilj je istraživanja bio utvrditi učestalost promjene boje kože iznad aficiranih zglobova u psorijatičnome artritisu, kao jednome od mogućih elemenata za jednostavnije i ranije postavljanje dijagnoze. Ispitanici su bili 650 bolesnika s psorijatičnim artritisom i 250 bolesnika s reumatoidnim artritisom koji su promatrani u razdoblju od 1975. do 1998. U 598 bolesnika s psorijatičnim artritisom (92%) nađena je plava boja kože iznad aficiranih zglobova, a samo u 63 bolesnika s reumatoidnim artritisom (5%). Na temelju tih podataka moguće je zaključiti da je plava boja kože u psorijatičnome artritisu pouzdan znak koji može značajno pomoći u postavljanju dijagnoze.

Lupus profundus je zriedkavá choroba. Vyskytuje sa samostatne alebo zriedkavo aj spolu s diskoidným alebo systémovým lupus erythemtosus. Vo väčšine prípadov postihuje táto choroba ženy vo veku 30—60 rokov. V podkožnom tukovom tkanive sa objavia bolestivé, modrasté uzlíky veľkosti 1—2 cm. Po mesiaci sa vyvinie atrofia kože a podkožného tukového tkaniva a pozoruje sa priľnavosť kože k svalom a kostiam. V typických prípadoch sú patohistologické nálezy bohaté na lymfocyty a histiocyty. Terapia pozostáva z podávania antimalarík buď samostatne, alebo v kombinácii s kortikosteroidmi a príležitostne cytostatikami. Uvádzame kazuistiku ženy s 20-ročným trvaním tejto choroby, kontinuálnou progresiou, nevratnými zmenami a rezistenciou proti leičbe.

The objective of the paper was to emphasize the meaning of involvement of ischial tuberosity in ankylosing spondylitis. The classification of changes is useful for the assessment of disease range and progression as well as for the diagnosis when sacroiliitis is obscurely developed and the other radiologic signs are not present.

Rentgenska dijagnostika reumatskih bolesti prvijenac je u hrvatskoj literaturi.
