Prior to survey development, small focus groups with anti-VEGF-treated patients identified five treatment-related “attributes” considered important to those with nAMD or DME: vision outcomes, cost to the insurance provider, cost to the patient, frequency of treatment, and drug label status. Attributes were described using two to three “levels”, and hypothetical treatment profiles were generated by assigning one level to each attribute. Surveyed patients were asked to indicate their preference between two given treatment profiles for a total of eight pairwise comparisons. Discrete choice conjoint analysis was performed to estimate the relative importance of each attribute for the overall patient cohort, and for subgroups stratified by age and highest education level.Results: Among 300 respondents, 54% were female, 78% were aged ≥ 65 years, and 67% indicated that high school was their highest level of education. Achieving good vision was the most important factor associated with anti-VEGF therapy for nAMD or DME (relative importance, 40.4%), followed by low cost to the patient, on-label drug status, less frequent treatment intervals, and low cost to the insurance provider (23.1%, 21.3%, 12.2%, and 3.0%, respectively). When patients were stratified by age group or highest education level, preference trends across subgroups were generally comparable with the overall cohort.Conclusion: Our data suggest that treatment decisions regarding anti-VEGF therapies for nAMD or DME are most likely driven by their efficacy, and that patients may be willing to accept less desirable treatment attributes, such as increased cost and/or injection frequency, in order to achieve superior vision outcomes.Keywords: anti-VEGF therapy, conjoint analysis, diabetic macular edema, neovascular age-related macular degeneration, patient preferences

In this paper we present the observations performed by the BeppoSAX Gamma-Ray Burst Monitor (GRBM) and Wide Field Cameras (WFC) of GB960720. We derive a precise localization (3 arcmin radius) and fast broad band (2-700 keV) spectral evolution of the event. A search in the catalogues at all wavelengths in the error box yields a unique outstanding source: the bright radio quasar 4C 49.29. Although the probability of finding such a source by chance is very low (0.0002), the absence of similar counterparts in other small error boxes suggests a chance occurrence. We also find that the duration-energy relationship for bursts previously observed above 25keV (Fenimore et al. 1995) extends down to 1.5 keV. This result suggests that the same radiation mechanism is operating from X-rays to gamma-rays and is in agreement with radiative cooling by synchotron emission. A fast evolution of the spectrum is found, in which the ratio of X- to gamma-ray intensities varies over three orders of magnitude. Furthermore, the spectrum in the initial phase of the event betrays the presence of an optically thick source rapidly evolving in a thin configuration.No other class of sources in the universe shows such a fast and extreme evolution. Abstract

Here we report the discovery in the X-ray band of the first afterglow of a gamma-ray burst. It was detected and quickly positioned by the Beppo-SAX satellite on 1997 February 28 (GRB970228). The X-ray afterglow source was detected with the X-ray telescopes aboard the same satellite about eight hours after the burst and faded away in a few days with a power law decay function. The energetic content of the X-ray afterglow results to be a significant fraction of gamma-ray burst energetics. The Beppo-SAX detection and fast imaging of GRB970228 started a multiwavelength campaign that lead to the identification of a fading optical source in a position consistent with the X-ray source., Comment: 10 pages, LaTeX, 3 postscript figures; Nature, issue of 19 June 1997

Abstract

We report on a comprehensive radio monitoring program of the bright gamma-ray burster GRB970111. These VLA observations were made at a frequency of 1.4 GHz and span a range of post-burst timescales between 28 hours and one month. Despite extensive sampling at sub-milliJansky sensitivities, no radio source was detected above 0.5 mJy in the current best error box (~14 arcmin^2) for GRB970111. A highly unusual radio source, VLA J1528.7+1945, was seen to drop in flux density by a factor of two in our monitoring period but it lies outside the error box and thus it is unlikely to be related to GRB970111. Cosmological fireball models of gamma-ray bursts make predictions of late-time emission occurring at longer wavelengths. The absence of a flaring or fading radio counterpart to GRB970111 provides strong constraints on these models., Comment: ApJ Let (accepted)

Abstract

De aanleiding voor dit onderzoek betreft een voorstel voor een Nederlandse norm voor een minimumaandeel niet-fossiele kunststoffen, waarvan de uitwerking momenteel wordt besproken. Deze Nationale Norm Circulaire Plastics verplicht kunststof- en rubber verwerkers in Nederland om minstens 25%-30% kunststof uit recyclaat en/of biogebaseerd kunststof (en mogelijk rubber) te verwerken voor alle kunststoffen die zij op de Nederlandse markt verkopen in 2030. De verplichting start in 2027 op een nog vast te stellen niveau (bijvoorbeeld 15%) en loopt op naar 25%-30% in 2030. De exacte scope en de percentages van de norm moeten nog worden vastgesteld. De overheid werkt op dit moment de (financiële) ondersteuning voor de implementatie van de norm verder uit. Hiervoor heeft de Rijksdienst voor Ondernemend Nederland (RVO) in samenwerking met de Federatie Nederlandse Rubber- en Kunststofindustrie (NRK), het ministerie van Economische Zaken en Klimaat (EZK), en het ministerie van Infrastructuur en Waterstaat (IenW) opdracht verstrekt aan Partners for Innovation om een onderzoek uit te voeren onder kunststof- en rubberverwerkers. Het doel van dit onderzoek is om te begrijpen welke knelpunten kunststof- en rubberverwerkers verwachten tegen te komen bij implementatie van deze norm. De studie is bedoeld als een eerste onderbouwing voor de financiële instrumenten die ingezet kunnen worden bij invoering van de norm en is niet bedoeld om de haalbaarheid van de norm of haar impact op de markt te onderzoeken.

Abstract

Objective: To investigate the safety and effectiveness of direct oral anticoagulants (DOAC) versus vitamin K antagonists (VKA) after recent stroke in patients with atrial fibrillation (AF) aged ≥85 years. Methods: Individual patient data analysis from seven prospective stroke cohorts. We compared DOAC versus VKA treatment among patients with AF and recent stroke (≥85y = 0.65, 95%-CI [0.52, 0.81]) and < 85 years (HR = 0.79, 95%-CI [0.66, 0.95]) in simple (p interaction = 0.129), adjusted (p interaction = 0.094) or weighted (p interaction = 0.512) models. Analyses on recurrent stroke, ICH and death separately were consistent with the primary analysis, as were sensitivity analyses using age dichotomized at 90 years and as a continuous variable. DOAC had a similar net clinical benefit in patients aged ≥85 (+1.73 to +2.66) and < 85 years (+1.90 to +3.36 events/100 patient-years for ICH-weights 1.5 to 3.1). Interpretation: The favorable profile of DOAC over VKA in patients with AF and recent stroke was maintained in the oldest old. ANN NEUROL 2021.

Several nutritive factors such as vitamin concentrations and bioavailability of trace elements and minerals increase during germination. The objective of this work was to study the enrichment of various essential trace elements during germination of wheat (Triticum aestivum), buckwheat (Fagopyrum esculentum), and quinoa (Chenopodium quinoa) seeds in order to improve their nutritional role as a source of bioavailable trace elements. Seeds were sprouted either in distilled- or tap-water and in five different electrolyte solutions to investigate the concentration-dependent uptake. The time-dependence was investigated by analyzing aliquots of the sprouts after certain germination periods. Samples were analyzed after freeze drying for their Li, V, Cr, Fe, Mn, Co, Cu, Zn, Sr, Mo, As and Se concentrations with inductively-coupled plasma mass-spectrometry (ICP-MS). As a control for possible changes in the biochemical metabolism of the sprouts, the biosynthesis of vitamin C was also determined by using reversed-phase ion-pair HPLC. It was shown that quinoa was the most resistant to the applied electrolyte solutions and had the highest uptake rates for almost all elements, followed by buckwheat and wheat. Greatest increases were observed for Co, Sr, and Li. No significant changes in vitamin C biosynthesis were observed between sprouts grown in different electrolyte solutions. The time-dependent uptake for most elements was characterized by a significant absorption during soaking of the seeds, followed by a lag phase during the first day of germination and an increased uptake during the second and third day. Se and As showed distinctly different uptake behaviors. 34. Cú Chulainn as a National Hero

Abstract

In the late nineteenth and early twentieth century nationalism was on the rise all around Europe. Ireland was no exception. It, however, creates an interesting situation because it was still part of the Commonwealth at the time. The Irish national identity was promoted by movements like the GAA and the Gaelic League, who eventually turned to a political agenda and according to some scholars helped make Irish Independence a possibility. This thesis will study signs of nationalism in two adaptations of the Irish national hero Cú Chulainn: The Coming of Cuculain (1894) by Standish O’Grady and Cuchulainn of Muirthemne (1902) by Lady Augusta Gregory. It will answer how these texts could be used by political movements. Furthermore, a great part of this thesis will focus on children’s literature since these works have been appropriated by children and have also been used for educational purposes. It will expand on the phenomenon of nationalism in children’s literature and on the discussion surrounding the definition of children’s literature in the late nineteenth and early twentieth century. The thesis will end with a close consideration of the signs of nationalism in both works and an explanation of strategies used by the writers when adapting these Early Irish stories. In short, this thesis will look at the works of O’Grady and Gregory in connection to politics, children’s literature and nationalism to show the different forces that influenced (children’s) literature in Ireland on the eve of Irish Independence.

Abstract

The chariot or carpat in Old Irish Literature poses an interesting problem, since the chariots are described in the literature but are poorly represented in other sources. There is no direct archaeological evidence for military use of the chariot in Early Ireland , nor are there concrete references in the Law Texts or in any other art except for the literature. Several theories exist on the origin of the chariot as a motif in Early Irish myth. Some claim that the chariot from the Táin Bó Cúailnge is a representation of a chariot in existence in Iron Age Ireland . The Iron Age chariot would have been a remnant from the La Tène period, where princes on the mainland were buried with chariots and must have travelled with the tribes north-westwards to eventually end up in Ireland . Others believe the chariot to be a purely literary motif . Besides the many theories the question remains: did carpat ever exist in Early Irish society? And if yes, when did it exist? Archaeology has no answers, neither have the Laws. The only real evidence of carpat remains hidden in the Old Irish tales.

Abstract

Background: Oesophageal carcinoma is a rare disease with often dismal prognosis. Despite multiple trials addressing specific issues, currently, many questions in management remain unanswered. This work aimed to specifically address areas in the management of oesophageal cancer where high level evidence is not available, performing trials is very demanding and for many questions high-level evidence will not be available in the forseeable future. Methods: Two experts of each national, oesophageal cancer research group from Austria, France, Germany, the Netherlands and Switzerland were asked to provide statements to controversial issues. After an initial survey, further questions were formulated and answered by all experts. The answers were then discussed and qualitatively analysed for consensus and controversy. Results: Topics such as indications for PET-CT, reasons for induction chemotherapy, radiotherapy dose, the choice of definitive chemo-radiotherapy versus surgery in squamous cell cancer, the role of radiotherapy in adenocarcinoma and selected surgical issues were identified as topics of interest and discussed. Conclusion: Areas of significant controversy exist in the management of oesophageal cancer, mostly due to high-level evidence. This is not expected to change in the upcoming years.

Een membraanbioreactor (MBR) is, door de vergaande verwijdering van bacteriën en zwevende stof, een interessante optie voor verbetering van de effluentkwaliteit, met name in gebieden met een kwetsbare waterhuishouding of daar waar sprake is van hergebruik van effluent. Toch blijft grootschalige toepassing van MBR voor het upgraden van rwzi’s in Nederland uit. Is bredere toepassing van dit beproefde concept haalbaar en zo ja, onder welke voorwaarden dan?

Contains fulltext : 88112.pdf (Publisher’s version ) (Closed access) BACKGROUND: Stents are an alternative treatment to carotid endarterectomy for symptomatic carotid stenosis, but previous trials have not established equivalent safety and efficacy. We compared the safety of carotid artery stenting with that of carotid endarterectomy. METHODS: The International Carotid Stenting Study (ICSS) is a multicentre, international, randomised controlled trial with blinded adjudication of outcomes. Patients with recently symptomatic carotid artery stenosis were randomly assigned in a 1:1 ratio to receive carotid artery stenting or carotid endarterectomy. Randomisation was by telephone call or fax to a central computerised service and was stratified by centre with minimisation for sex, age, contralateral occlusion, and side of the randomised artery. Patients and investigators were not masked to treatment assignment. Patients were followed up by independent clinicians not directly involved in delivering the randomised treatment. The primary outcome measure of the trial is the 3-year rate of fatal or disabling stroke in any territory, which has not been analysed yet. The main outcome measure for the interim safety analysis was the 120-day rate of stroke, death, or procedural myocardial infarction. Analysis was by intention to treat (ITT). This study is registered, number ISRCTN25337470. FINDINGS: The trial enrolled 1713 patients (stenting group, n=855; endarterectomy group, n=858). Two patients in the stenting group and one in the endarterectomy group withdrew immediately after randomisation, and were not included in the ITT analysis. Between randomisation and 120 days, there were 34 (Kaplan-Meier estimate 4.0%) events of disabling stroke or death in the stenting group compared with 27 (3.2%) events in the endarterectomy group (hazard ratio [HR] 1.28, 95% CI 0.77-2.11). The incidence of stroke, death, or procedural myocardial infarction was 8.5% in the stenting group compared with 5.2% in the endarterectomy group (72 vs 44 events; HR 1.69, 1.16-2.45, p=0.006). Risks of any stroke (65 vs 35 events; HR 1.92, 1.27-2.89) and all-cause death (19 vs seven events; HR 2.76, 1.16-6.56) were higher in the stenting group than in the endarterectomy group. Three procedural myocardial infarctions were recorded in the stenting group, all of which were fatal, compared with four, all non-fatal, in the endarterectomy group. There was one event of cranial nerve palsy in the stenting group compared with 45 in the endarterectomy group. There were also fewer haematomas of any severity in the stenting group than in the endarterectomy group (31 vs 50 events; p=0.0197). INTERPRETATION: Completion of long-term follow-up is needed to establish the efficacy of carotid artery stenting compared with endarterectomy. In the meantime, carotid endarterectomy should remain the treatment of choice for patients suitable for surgery. Abstract

MAPPER Lithography is developing a maskless lithography technology. The technology combines massively-parallel electron-beam writing with high speed optical data transport used in the telecommunication industry. The electron optics generates 13,000 electron beams that are focused on the wafer by electrostatic lens arrays which are manufactured by using MEMS manufacturing techniques. Each beam has its own optical column to avoid a central cross-over. This secures high throughput (> 10 wafers per hour) at high resolution (< 45 nm half pitch). The 13,000 e-beams are generated by splitting up a single electron beam that originates from a single electron source and are finally accelerated to 5 kV to expose the resist on the wafer. The e-beams are arranged in such a way that they form a rectangular slit with a width of 26 mm, the same width of a field in an optical stepper. During exposure the e-beams are deflected over 2 ?m perpendicular to the wafer stage movement. This means that with one scan of the wafer a full field of 26 mm x 33 mm can be exposed. During the simultaneous scanning of the wafer and deflection of the electron beams the beams are switched on and off by 13,000 light signals, one for each e-beam. The light beams are generated in a data system that contains the chip patterns in a bitmap format. This bitmap is divided over 13,000 data channels and streamed to the ebeams at 1-10 GHz. This paper will explain the design drivers behind the system and provide more detail on the current design. Finally, results of our technology Demonstrator are presented, showing the viability of MAPPER’s concept.
