Purpose: To investigate the molecular basis of recessively inherited congenital cataract, microcornea, and corneal opacification with or without coloboma and microphthalmia in two consanguineous families. Methods: Conventional autozygosity mapping was performed using single nucleotide polymorphism (SNP) microarrays. Whole-exome sequencing was completed on genomic DNA from one affected member of each family. Exome sequence data were also used for homozygosity mapping and copy number variation analysis. PCR and Sanger sequencing were used to confirm the identification of mutations and to screen further patients. Evolutionary conservation of protein sequences was assessed using CLUSTALW, and protein structures were modeled using PyMol. Results: In family MEP68, a novel homozygous nucleotide substitution in SIX6 was found, c.547G>C, that converts the evolutionarily conserved aspartic acid residue at the 183rd amino acid in the protein to a histidine, p.(Asp183His). This residue mapped to the third helix of the DNA-binding homeobox domain in SIX6, which interacts with the major groove of double-stranded DNA. This interaction is likely to be disrupted by the mutation. In family F1332, a novel homozygous 1034 bp deletion that encompasses the first exon of SIX6 was identified, chr14:g.60975890_60976923del. Both mutations segregated with the disease phenotype as expected for a recessive condition and were absent from publicly available variant databases. Conclusions: Our findings expand the mutation spectrum in this form of inherited eye disease and confirm that homozygous human SIX6 mutations cause a developmental spectrum of ocular phenotypes that includes not only the previously described features of microphthalmia, coloboma, and congenital cataract but also corneal abnormalities.

Background: Antimicrobial development for children remains challenging due to multiple barriers to conducting randomised clinical trials (CTs). There is currently considerable heterogeneity in the design and conduct of paediatric antibiotic studies, hampering comparison and meta-analytic approaches. The board of the European networks for paediatric research at the European Medicines Agency (EnprEMA), in collaboration with the Paediatric European Network for Treatments of AIDS – Infectious Diseases (PENTA-ID) network (www.penta-id.org), recently developed a Working Group (WG) on paediatric antibiotic CT design, involving academic, regulatory and industry representatives. Objectives: The evidence base for any specific criteria for the design and conduct of efficacy and safety antibiotic trials for children is very limited and will evolve over time as further studies are conducted. The suggestions being put forward here are based on the adult EMA guidance, adapted for neonates and children. In particular, this document provides suggested guidance on the general principles of harmonisation between regulatory and strategic trials, including (I) standardised key inclusion/exclusion criteria and widely applicable outcome measures for specific clinical infectious syndromes (CIS) to be used in CTs on efficacy of antibiotic in children; (II) key components of safety that should be reported in paediatric antibiotic CTs; (III) standardised sample sizes for safety studies. Results: Summarising views from a range of key stakeholders, specific criteria for the design and conduct of efficacy and safety antibiotic trials in specific CIS for children have been suggested. Conclusion: The recommended criteria are intended to be applicable to both regulatory and clinical investigator-led strategic trials and could be the basis for harmonisation in the design and conduct of CTs on antibiotics in children. The next step is further discussion internationally with investigators, paediatric clinical trials networks and regulators.

Objective\ud \ud To produce a strategic roadmap for supporting the development of dementia research in Pakistan.\ud \ud \ud \ud Background\ud \ud While global research strategies for dementia research already exist, none is tailored to the specific needs and challenges of low- and middle-income countries (LMIC) like Pakistan.\ud \ud \ud \ud Methods\ud \ud We undertook an iterative consensus process with lay and professional experts to develop a Theory of Change-based strategy for dementia research in Pakistan. This included Expert Reference Groups (ERGs), strategic planning techniques, a “research question” priority survey, and consultations with Key Opinion Leaders.\ud \ud \ud \ud Results\ud \ud We agreed on ten principles to guide dementia research in Pakistan, emphasizing pragmatic, resource sparing, real-world approaches to support people with dementia, both locally and internationally. Goals included capacity/capability building. Priority research topics included raising awareness and understanding of dementia, and improving quality of life.\ud \ud \ud \ud Conclusion\ud \ud This roadmap may be a model for other LMIC health ecosystems with emerging dementia research cultures.

Background: Systemic inflammation has been implicated in tumor growth, symptoms of cancer including cachexia and pain, as well as shortened survival. Advanced Lung Cancer Inflammation Index (ALI) is a simple tool for assessing systemic inflammation and has been shown to predict outcome in variety of thoracic malignancies including NSCLC. The purpose of this study was to assess if ALI is a predictive biomarker for checkpoint inhibitors in patients with metastatic NSCLC. Methods: We evaluated all patients with metastatic NSCLC treated with checkpoint inhibitors (CPI) at our institute between Jan 1st 2015 and June 30th 2019. ALI was calculated (BMI x Alb / NLR) at the time of initiating CPI using patients Body Mass Index (BMI), serum Albumin (Alb), and Neutrophil Lymphocyte Ratio (NLR) to quantify the degree of systemic inflammation. Patients were divided into two groups, based on ALI: ALI < 15 (indicating a relatively high degree of systemic inflammation), and ALI > 15 (indicating a relatively low degree of systemic inflammation). Primary end point was progression-free survival (PFS). Secondary endpoint included overall survival (OS), response rates, and adverse events (AE) from immunotherapy. Chi-squared test was performed to compare clinical characteristics in different groups. Kaplan-Meier and Cox regression model were used to determine independent prognostic factors. Results: A total of 79 patients were included in the final analysis. The median age was 66. There were 39 males and 40 females. Ethnicities included White (n=40), African American (n=17), and Asian (n=6). Sixty-six had adenocarcinoma, 9 had Squamous cell carcinoma, 2 had neuroendocrine carcinoma, and 11 were NSCLC, Not otherwise specified NOS. Pembrolizumab was used in 43 patients with or without chemotherapy, nivolumab in 34 with and without chemotherapy and durvalumab in 2 patients. PDL-1 expression was available in forty-seven (59%) patients,14 had >50% PDL-1 expression, 27 had (1 - 49%), and 6 had < 1% or none. Thirteen patients had complete response (CR) on first assessment after immunotherapy, 23 had partial response (PR), 9 had stable disease or mixed response, 28 had progression, and 6 could not be assessed due to lack of follow up scan. Thirty-two had an ALI < 15, and 47 had ALI > 15. Patients with ALI > 15 had a significantly longer PFS compared to those with ALI < 15 (7.57 vs 3.17 months, p = 0.0018). Similarly, Patients with ALI > 15 had a significantly longer OS (17.60 vs 6.37 months, p = 0.002) than those with ALI < 15. Of the 43 patients who were treated with Pembrolizumab +/- chemotherapy, 18 had an ALI < 15 and 25 had an ALI > 15, there was no difference in PFS or OS based on ALI. In contrast, amongst those who received nivolumab +/- chemotherapy (n=34), patients with ALI > 15 (n=?) had significantly longer PFS (14.7 vs 6.22 months, p = 0.01), as well as OS (19.68 vs 9.75 months, p = 0.004). Thirty-seven patients had reported AE from immunotherapy. Most common was fatigue (n=17, 21.52%). Others included anorexia (n=4, 5.06%), elevated Liver Function Tests (n=2, 2.53%), and Thrombocytopenia (n=2, 2.53%). Conclusion: ALI is easy to calculate clinical tool which can help predict response to checkpoint inhibitors in patients with metastatic lung cancer especially in those receiving nivolumab. Herpesvirus colitis is a known cause of morbidity and mortality amongst immunosuppressed individuals. We present a case of HSV colitis following a diagnosis of Crohn’s Disease and methylprednisolone therapy. Diagnosis was confirmed by immunohistochemical staining and supported by polymerase chain reaction (PCR) of cutaneous vesicles. The patient recovered following three weeks of acyclovir.

Objectives Stakeholders' views are essential for informing implementation strategies for non-invasive prenatal testing (NIPT). Little is known about such views in developing countries. We explored attitudes towards NIPT among obstetricians in Pakistan, a developing, Islamic country. Methods A 35-item questionnaire was distributed and collected at eight events (a national conference and seven workshops in five cities) for obstetric professionals on advances in fetal medicine. Results Responses from 113 obstetrician show positive attitudes towards implementation of NIPT: 95% agreed prevention of genetic conditions was a necessity, and 97% agreed public hospitals should provide prenatal screening tests. However, participants also agreed the availability of NIPT would increase social pressure on women to have prenatal screening tests and to terminate an affected pregnancy (53% and 63%, respectively). Most participants would not offer NIPT for sex determination (55%), although 31% would. The most valued aspects of NIPT were its safety, followed by its utility and then accuracy. Conclusion Participants generally supported the implementation of NIPT but raised concerns about social implications. Therefore, national policy is needed to regulate the implementation of NIPT, and pretest information and post-test genetic counselling are needed to mitigate social pressure and support parents to make informed decisions.

Amelogenesis is the process of dental enamel formation, leading to the deposition of the hardest tissue in the human body. This process requires the intricate regulation of ion transport and controlled changes to the pH of the developing enamel matrix. The means by which the enamel organ regulates pH during amelogenesis is largely unknown. We identified rare homozygous variants in GPR68 in three families with amelogenesis imperfecta, a genetically and phenotypically heterogeneous group of inherited conditions associated with abnormal enamel formation. Each of these homozygous variants (a large in-frame deletion, a frameshift deletion, and a missense variant) were predicted to result in loss of function. GPR68 encodes a proton-sensing G-protein-coupled receptor with sensitivity in the pH range that occurs in the developing enamel matrix during amelogenesis. Immunohistochemistry of rat mandibles confirmed localization of GPR68 in the enamel organ at all stages of amelogenesis. Our data identify a role for GPR68 as a proton sensor that is required for proper enamel formation.
