Abstract

International audience; Introduction: We aimed to describe patients with coexisting infective endocarditis (IE) and bacterial meningitis (BM).Methods: We merged two large prospective cohorts, an IE cohort and a BM cohort, with only cases of definite IE and community-acquired meningitis. We compared patients who had IE and BM concurrently to patients with IE only and BM only.Results: Among the 1030 included patients, we identified 42 patients with IE-BM (4.1%). Baseline characteristics of patients with IE-BM were mostly similar to those of patients with IE, but meningitis was the predominant presentation at admission (39/42, 92.3%). Causative pathogens were predominantly Streptococcus pneumoniae (18/42, 42.9%) and Staphylococcus aureus (14/42, 33.3%). All pneumococcal IE were associated with BM (18/18). BM due to oral and group D streptococci, Streptococcus agalactiae, and S. aureus were frequently associated with IE (14/30, 46.7%). Three-month mortality was 28.6% in patients with IE-BM, 20.5% in patients with IE, and 16.6% in patients with BM.Conclusions: Patients with pneumococcal IE or altered mental status during IE must be investigated for BM. Patients with S. aureus, oral and group D streptococcal or enterococcal BM, or unfavorable outcome in pneumococcal meningitis would benefit from an echocardiography. Patients with the dual infection have the worst prognosis. Their identification is mandatory to initiate appropriate treatment.

Abstract

Virtuel; International audience; Background: Each year in Europe, nearly 24,000 patients, adults and children of all ages, are transplanted, the last resort to treat and potentially cure serious blood diseases. While this intervention is saving more and more lives, it can lead to early or late complications in 50% of patients, very disabling in every day life and fatal in 25% of cases. Therefore, the chances of a successful transplant for these patients correlate directly with the non-occurrence of these complications.Recently, research in the field of transplantation and in particular psycho-neuro-immunology has highlighted the mutual influence of psychological and physical health on the chances of successful transplantation. Indeed, the simultaneous management of the mind and body of the patient would ensure better preparation for the transplant process for faster recovery after it by limiting the occurrence of complications. The establishment of an overall well-being throughout the transplant process, based on three inseparable pillars - nutritional, physical and psychological -, would contribute to very significantly increase the success of the transplant and the chances of recovery of the patient.Methods: Currently, there is limited comprehensive support program in the field of transplantation, based on clinical data and patient quality of life that can help them overcome the difficulties of preparing for transplant, the period of hospitalization in a protected environment as well as those related to the return to social and professional life.To answer patients’needs in terms of health support and quality of life improvement, the HTC Project has initiated the development of the digital solution NewSpringForMe. This tool intend to bring complementary therapeutic support for care, reinforcing it, and making each patient the main player in their quality of life.Results: The innovation of NewSpringForMe lies in the simultaneous management of transplanted patients in three dimensions: psychology, nutrition and physical activity. Designed around interconnected spaces, NewSpringForMe deploys recommendations, interactive tools and exercises adapted to respond to 1) each patient and their specific situation, 2) the different phases of their transplant protocol. NewSpringForMe aims at a better preparation and a more global care of the patient, condition of the best live towards its remission. A clinical study, in collaboration with the St Louis Hospital (AP-HP), is scheduled at the early 2022 in order to measure the impact of NewSpringForMe use on the psychological (ex. affect levels), nutritional (ex. level and quality of appetite) and physical (ex. effort and suppleness) parameters of almost 170 patients during one year. This study would also evaluate the impact on the post-transplant complications onset and the post-transplant recovery. The clinical study will be performed in accordance with the regulatory prerequisites.Conclusions: The data collected during the clinical protocol directly from the patients’experience will help in NewSpringForMe evaluation as a global support. A positive impact should be expected on patients daily quality of life, on the appearance of the post-transplant complications and the number of treatments. If this pilot phase is a success, NewSpringForMe could be proposed to the all French transplant units in order to lead a large-scale study.

Abstract

Objectives The neurological disorders in HIV-1-infected patients remain prevalent. The HIV-1 resistance in plasma and CSF was compared in patients with neurological disorders in a multicentre study. Methods Blood and CSF samples were collected at time of neurological disorders for 244 patients. The viral loads were >50 copies/mL in both compartments and bulk genotypic tests were realized. Results On 244 patients, 89 and 155 were antiretroviral (ARV) naive and ARV treated, respectively. In ARV-naive patients, detection of mutations in CSF and not in plasma were reported for the reverse transcriptase (RT) gene in 2/89 patients (2.2%) and for the protease gene in 1/89 patients (1.1%). In ARV-treated patients, 19/152 (12.5%) patients had HIV-1 mutations only in the CSF for the RT gene and 30/151 (19.8%) for the protease gene. Two mutations appeared statistically more prevalent in the CSF than in plasma: M41L (P = 0.0455) and T215Y (P = 0.0455). Conclusions In most cases, resistance mutations were present and similar in both studied compartments. However, in 3.4% of ARV-naive and 8.8% of ARV-treated patients, the virus was more resistant in CSF than in plasma. These results support the need for genotypic resistance testing when lumbar puncture is performed

Abstract

Objectives The neurological disorders in HIV-1-infected patients remain prevalent. The HIV-1 resistance in plasma and CSF was compared in patients with neurological disorders in a multicentre study. Methods Blood and CSF samples were collected at time of neurological disorders for 244 patients. The viral loads were >50 copies/mL in both compartments and bulk genotypic tests were realized. Results On 244 patients, 89 and 155 were antiretroviral (ARV) naive and ARV treated, respectively. In ARV-naive patients, detection of mutations in CSF and not in plasma were reported for the reverse transcriptase (RT) gene in 2/89 patients (2.2%) and for the protease gene in 1/89 patients (1.1%). In ARV-treated patients, 19/152 (12.5%) patients had HIV-1 mutations only in the CSF for the RT gene and 30/151 (19.8%) for the protease gene. Two mutations appeared statistically more prevalent in the CSF than in plasma: M41L (P = 0.0455) and T215Y (P = 0.0455). Conclusions In most cases, resistance mutations were present and similar in both studied compartments. However, in 3.4% of ARV-naive and 8.8% of ARV-treated patients, the virus was more resistant in CSF than in plasma. These results support the need for genotypic resistance testing when lumbar puncture is performed

Abstract

Cas clinique: Dermohypodermite compliquée d’une bactériémie àS. putrefaciens chez un homme de 71 ans, d’origine marocaine, s’étant présenté aux urgences pour altération de l’état général depuis un mois apparue au décours d’un voyage au Maroc (notion de baignade en mer pendant le séjour). À l’examen clinique on objectivait des œdèmes des membres inférieurs associés à une dermite ocre, des ulcérations superficielles bilatérales et un placard inflammatoire de la face interne de la cuisse gauche ; bilan biologique : syndrome inflammatoire, hypo albuminémie, bloc bêta-gamma, élévation isolée des gamma-GT. Le scanner abdominal mettait en évidence un nodule du segment III hépatique. L’élastométrie était en faveur d’un foie cirrhotique. Des hémocultures montraient l’existence d’une bactériémie à bacille Gram négatif mobile type «Pseudomonas-like ». Un traitement associant pipéracilline-tazobactam et ciprofloxacine était débuté. L’identification de la bactérie par les méthodes phénotypiques usuelles restait indéterminée et le recours à l’amplification du gène ADNr 16S suivi du séquençage permettait d’identifier l’espèce S. putrefaciens. L’antibiogramme montrait une sensibilité à tous les antibiotiques testés. L’évolution était rapidement favorable. Discussion: Schewanella est une bactérie ubiquitaire que l’on trouve surtout dans les eaux chaudes salées. L’identification du genre Shewanella peut aboutir à l’aide des caractères morphologiques et biochimiques. Cependant, les seuls critères phénotypiques sont insuffisants pour différencier les deux espèces pathogènes chez l’homme, S. putrefaciens et S. algae. Le recours au séquençage de l’ADNr 16S est donc indispensable pour une différentiation des deux espèces. L’arbre phylogénétique a permis de confirmer la position de la séquence ADNr 16S correspondant à l’isolat clinique dans le phylum S. putrefaciens (cf. iconographie). Il s’agit d’un pathogène opportuniste à l’origine d’infections rencontrées chez des patients fragiles. Parmi les cas de bactériémie décrits, on distingue d’une part les bactériémies survenant le plus souvent sur des lésions cutanées dues à des ulcères ou des traumatismes et d’autre part, les bactériémies d’évolution rapidement fatale survenant sur des terrains débilités (hépatopathie sévère ou pathologie maligne). Abstract

Background: This study aimed to compare the humoral responses to mRNA COVID-19 vaccination in people living with HIV (PWH) and HIV-negative individuals., Methods: We included PWH with an undetectable viral load under ART and HIV-negative participants from the French nationwide ANRS COV-POPART cohort who had received two doses of vaccine as a primary vaccination. We compared humoral response between controls and PWH, stratified by CD4 cell count (<200/mm 3 and ≥200/mm 3 CD4 cell counts) at 1, 6, and 12 months after primary vaccination., Results: A total of 1776 participants were included in this analysis, 684 PWH (99% were on ART, median CD4 counts 673 cells/mm 3 ) and 1092 controls. At 1 month, after adjustment on age, sex, and BMI, PWH had lower seroneutralization titers than controls, and PWH with <200 CD4 cell/mm 3 had lower anti-Spike SARS-CoV-2 IgG antibodies. Same results were found at 6 months. However, in participants who received a booster dose between 6 and 12 months postprimary vaccination, we did not observe differences between PWH and controls at 12 months., Conclusion: PWH had high responses to primary mRNA COVID-19 vaccination. Abstract

Background: Human metapneumovirus (hMPV) is one of the leading respiratory viruses. This prospective observational study aimed to describe the clinical features and the outcomes of hMPV-associated lower respiratory tract infections in adult inpatients., Methods: Consecutive adult patients admitted to one of the 31 participating centers with an acute lower respiratory tract infection and a respiratory multiplex PCR positive for hMPV were included. A primary composite end point of complicated course (hospital death and/or the need for invasive mechanical ventilation) was used., Results: Between March 2018 and May 2019, 208 patients were included. The median age was 74 [62-84] years. Ninety-seven (47 %) patients were men, 187 (90 %) had at least one coexisting illness, and 67 (31 %) were immunocompromised. Median time between first symptoms and hospital admission was 3 [2-7] days. The two most frequent symptoms were dyspnea (86 %) and cough (85 %). The three most frequent clinical diagnoses were pneumonia (42 %), acute bronchitis (20 %) and acute exacerbation of chronic obstructive pulmonary disease (16 %). Among the 52 (25 %) patients who had a lung CT-scan, the most frequent abnormality was ground glass opacity (41 %). While over four-fifths of patients (81 %) received empirical antibiotic therapy, a bacterial coinfection was diagnosed in 61 (29 %) patients. Mixed flora (16 %) and enterobacteria (5 %) were the predominant documentations. The composite criterion of complicated course was assessable in 202 (97 %) patients, and present in 37 (18 %) of them. In the subpopulation of pneumonia patients (42 %), we observed a more complicated course in those with a bacterial coinfection (8/24, 33 %) as compared to those without (5/60, 8 %) (p = 0.02). Sixty (29 %) patients were admitted to the intensive care unit. Among them, 23 (38 %) patients required invasive mechanical ventilation. In multivariable analysis, tachycardia and alteration of consciousness were identified as risk factors for complicated course., Conclusion: hMPV-associated lower respiratory tract infections in adult inpatients mostly involved elderly people with pre-existing conditions. Bacterial coinfection was present in nearly 30 % of the patients. 40. Mid-term health-related quality of life in community-acquired bacterial meningitis survivors; the COMBAT study.

Abstract

Background: Community Acute Bacterial Meningitis (CABM) is a rare infectious disease leading to important impairments. Our aim was to describe CABM survivors' quality of life (QOL) 12 months post-CABM and to assess its associations with CABM sequelae., Methods: Patients included in the CABM COMBAT cohort were evaluated one year after the CABM episode. Data were collected by questionnaire, via phone calls with the patients. The WHOQOL-BREF was used to measure CABM survivors' QOL. Hierarchical multivariate linear regressions were performed., Results: Study population was composed of 284 patients. At 12 months, 53.9% (153/284) reported at least incident headache/worsening headache intensity at 12 months post-CABM, and/or incident hearing impairment, and/or unfavourable disability outcome (GOS). Unfavourable disability outcome was associated with lower physical health QOL (B = -30.35, p<0.001), lower mental health QOL (B = -15.31, p<0.001), lower environmental QOL (B = -11.08, p<0.001) and lower social relationships QOL (B = -9.62, p<0.001). Incident headache/worsening headache since meningitis onset was associated with lower psychological health (B = -5.62, p = 0.010). Incident hearing impairment was associated with lower physical QOL (B = -5.34, p = 0.030). Hierarchical regressions showed that CABM impairments significantly increase explanatory power of multivariate models (for physical health R2 change = 0.42, p<0.001, for psychological health R2 change = 0.23, p<0.001, for social relationships R2 change = 0.06, p<0.001 and for environment domain R2 change was 0.15, p<0.001)., Conclusions: 12 month-CABM burden is heavy. Early detection and management of CABM impairments should be performed in clinical practice as early as possible to optimize patients' psychological and psychosocial functioning., Clinicaltrial. 41. Plasticity of Sorghum Stem Biomass Accumulation in Response to Water Deficit: A Multiscale Analysis from Internode Tissue to Plant Level.

Abstract

Sorghum is increasingly used as a biomass crop worldwide. Its genetic diversity provides a large range of stem biochemical composition suitable for various end-uses as bioenergy or forage. Its drought tolerance enables it to reasonably sustain biomass production under water limited conditions. However, drought effect on the accumulation of sorghum stem biomass remains poorly understood which limits progress in crop improvement and management. This study aimed at identifying the morphological, biochemical and histological traits underlying biomass accumulation in the sorghum stem and its plasticity in response to water deficit. Two hybrids (G1, G4) different in stem biochemical composition (G4, more lignified, less sweet) were evaluated during 2 years in the field in Southern France, under two water treatments differentiated during stem elongation (irrigated; 1 month dry-down until an average soil water deficit of -8.85 bars). Plant phenology was observed weekly. At the end of the water treatment and at final harvest, plant height, stem and leaf dry-weight and the size, biochemical composition and tissue histology of internodes at 2-4 positions along the stem were measured. Stem biomass accumulation was significantly reduced by drought (in average 42% at the end of the dry-down). This was due to the reduction of the length, but not diameter, of the internodes expanded during water deficit. These internodes had more soluble sugar but lower lignin and cellulose contents. This was associated with a decrease of the areal proportion of lignified cell wall in internode outer zone whereas the areal proportion of this zone was not affected. All internodes for a given genotype and environment followed a common histochemical dynamics. Hemicellulose content and the areal proportion of inner vs. outer internode tissues were set up early during internode growth and were not drought responsive. G4 exhibited a higher drought sensitivity than G1 for plant height only. At final harvest, the stem dry weight was only 18% lower in water deficit (re-watered) compared to well-watered treatment and internodes growing during re-watering were similar to those on the well-watered plants. These results are being valorized to refine the phenotyping of sorghum diversity panels and breeding populations.

Abstract

In this study, Pneumocystis jirovecii was detected and characterized in the air surrounding patients with Pneumocystis pulmonary colonization. Air samples were collected in the rooms of 10 colonized patients using Coriolis® μ air sampler at 1m and 5m from the patient's head. P. jirovecii DNA was amplified and genotyped in pulmonary and air samples at the mitochondrial large subunit ribosomal RNA gene. P. jirovecii DNA was detected in 5 of the 10 air samples collected at 1m and in 5 of the 10 other air samples collected at 5m. P. jirovecii genotyping was successful in 4 pairs or triplets of air and pulmonary samples. Full genotype matches were observed in 3 of the 4 pairs or triplets of air and pulmonary samples. These results provide original data supporting P. jirovecii exhalation from colonized patients and emphasize the risk of P. jirovecii nosocomial transmission from this patient population., (Copyright © 2015 Elsevier Inc. Abstract

Aim: To describe the main characteristics and the treatment of cryptococcosis in patients with sarcoidosis., Design: Multicenter study including all patients notified at the French National Reference Center for Invasive Mycoses and Antifungals., Methods: Retrospective chart review. Each case was compared with two controls without opportunistic infections., Results: Eighteen cases of cryptococcosis complicating sarcoidosis were analyzed (13 men and 5 women). With 2749 cases of cryptococcosis registered in France during the inclusion period of this study, sarcoidosis accounted for 0.6% of all the cryptococcosis patients and for 2.9% of the cryptococcosis HIV-seronegative patients. Cryptococcosis and sarcoidosis were diagnosed concomitantly in four cases; while sarcoidosis was previously known in 14/18 patients, including 12 patients (67%) treated with steroids. The median rate of CD4 T cells was 145 per mm(3) (range: 55-1300) and not related to steroid treatment. Thirteen patients had cryptococcal meningitis (72%), three osteoarticular (17%) and four disseminated infections (22%). Sixteen patients (89%) presented a complete response to antifungal therapy. After a mean follow-up of 6 years, no death was attributable to cryptococcosis. Extra-thoracic sarcoidosis and steroids were independent risk factors of cryptococcosis in a logistic regression model adjusted with the sex of the patients., Conclusions: Cryptococcosis is a significant opportunistic infection during extra-thoracic sarcoidosis, which occurs in one-third of the cases in patients without any treatment; it is not associated to severe CD4 lymphocytopenia and has a good prognosis.

Abstract

Background and Aims: Tillering is an essential factor when estimating yield, but investigations rarely include both the temporal and spatial changes that occur in tillers. This study analyses the morphology and development dynamics of each tiller, based on its topological location, the timing of appearance and main stem development stage., Methods: An indica cultivar of rice, 'Ir64', glasshouse-grown (25/20 degrees C, 12 h photoperiod), was used to examine the emergence, phenology and morphology of each axis starting at the third leaf stage up to heading., Key Results: Little variability was observed in the structural and morphological characteristics of the tillers, and the rice population appeared to be hierarchical. Blade length initially increased with leaf rank and then decreased sharply for the last three leaves. The number of phytomers per axis decreased with branching order and rank. An analysis of plant dynamics showed synchronous emergence of the leaves on the main stem and on the tillers up to flowering. Axillary bud development into tillers depended on their topological location and plant developmental stage., Conclusions: The timing and frequency of flowering tillers complied with rules of priority depending on their order, rank and emergence time. Precise description of plant topology in grasses is a useful tool that can be used to quantify growth events and predict tillering in terms of location, structure and fate according to growing conditions.
