Objectives: The aim of this study was to evaluate dizziness in patients with sleep disorders, objectively identify vestibular function through the vestibulo-ocular reflex (VOR) using the video Head Impulse Test (vHIT), and evaluate the association between these findings.Methods: Among the patients who visited the sleep clinic from June to October 2021, 69 who underwent both polysomnography (PSG) and vHIT were included. Participants completed questionnaires including the Dizziness Handicap Inventory (DHI), Beck Anxiety Inventory, Korean-Beck Depression Inventory-II, Epworth Sleepiness Scale, Insomnia Severity Index (ISI), and Pittsburgh Sleep Quality Index (PSQI). The subjects were classified into four groups: insomnia (n=4), rapid eye movement sleep behavior disorder (RBD) (n=13), obstructive sleep apnea syndrome (OSAS) (n=34), and RBD and OSAS (n=18). Moderate to severe OSAS (n=49) was compared with no OSAS and mild OSAS (n=20).Results: In comparison of the four groups according to sleep disorders, the OSAS patients showed the highest DHI scores and the lowest VOR gain, but statistical significance was not found. Although all VOR gains were within the normal range, the VOR gain of the left posterior semicircular canal was significantly lower in the moderate to severe OSA group than in the no OSA and mild OSA groups (1.02±0.18 vs. 0.94±0.10, p=0.019). DHI total scores showed no correlation with VOR gain but showed a positive correlation with ISI (r=0.422, p=0.001) and PSQI (r=0.287, p=0.022). Among PSG parameters, lowest oxygen saturation (SaO2) and percentage of time with SaO2 less than 90% were correlated with the emotional score of DHI (r=-0.245, p=0.043 and r=0.311, p=0.010, respectively).Conclusions: Although our study could not objectively confirm vestibular dysfunction in patients with sleep disorders, we found that subjective sleep complaints were associated with dizziness and hypoxic conditions during sleep were associated with emotional aspects of dizziness. This suggests that the treatment of concomitant sleep disorders may improve dizziness.

Obstructive sleep apnea (OSA) and dizziness are common conditions observed in the general population, and several epidemiological studies have reported an association between OSA and dizziness. Vestibular dysfunction, autonomic instability, and cerebellar degeneration secondary to recurrent hypoxia are implicated as mechanisms underlying dizziness in patients with OSA. Moreover, OSA is a risk factor for many diseases associated with dizziness, including Meniere disease, stroke, and psychiatric conditions. A dizziness questionnaire, vestibular function tests, and tests for autonomic function are useful for evaluation of OSA and concomitant dizziness. A growing body of evidence has shown that effective treatment of OSA including continuous positive airway pressure therapy reduces dizziness in these patients. Greater attention to dizziness is warranted in patients with OSA.

Objectives: Insomnia is a disorder that involves poor sleep quality at nighttime and various symptoms in the daytime. Although it is known to increase cardiovascular risk as well as psychiatric comorbidities such as depression and anxiety, less attention has been paid to daytime sleepiness in patients with insomnia. This study aimed to investigate clinical and polysomnographic characteristics and identify factors influencing excessive daytime sleepiness in patients with insomnia.Methods: Between 2014 and 2022, 1,241 patients with insomnia as the primary symptom who underwent polysomnography were included in the study. Patients with an Epworth sleepiness scale ≥10 were classified as having insomnia accompanied by daytime sleepiness, and sleep-related questionnaire responses and polysomnography findings were utilized for analysis.Results: A total of 327 patients (26.3%) had daytime sleepiness, and a higher prevalence was observed in younger and male individuals. Poor sleep quality, severe depression, and insomnia were identified through the questionnaire. Polysomnography showed longer total sleep times, a higher proportion of stage N3 sleep, and higher sleep efficiency. The variables related to the Epworth sleepiness scale identified in multiple regression analysis were age, the Korean Beck Depression Inventory-II, sleep latency, and the apnea-hypopnea index on polysomnography.Conclusions: Insomnia with excessive daytime sleepiness was more affected by depression than indicated by polysomnography findings. The pathophysiology is presumed to be related to sleep misperception, and further studies are needed to elucidate the discrepancy between excessive daytime sleepiness and polysomnography findings.

Epilepsy’s impact on cardiovascular function and autonomic regulation, including heart-rate variability, is complex and may contribute to sudden unexpected death in epilepsy (SUDEP). Lateralization of autonomic control in the brain remains the subject of debate; nevertheless, ultra-short-term heart-rate variability (HRV) analysis is a useful tool for understanding the pathophysiology of autonomic dysfunction in epilepsy patients. A retrospective study reviewed medical records of patients with temporal lobe epilepsy who underwent presurgical evaluations. Data from 75 patients were analyzed and HRV indices were extracted from electrocardiogram recordings of preictal, ictal, and postictal intervals. Various HRV indices were calculated, including time domain, frequency domain, and nonlinear indices, to assess autonomic function during different seizure intervals. The study found significant differences in HRV indices based on hemispheric laterality, language dominancy, hippocampal atrophy, amygdala enlargement, sustained theta activity, and seizure frequency. HRV indices such as the root mean square of successive differences between heartbeats, pNN50, normalized low-frequency, normalized high-frequency, and the low-frequency/high-frequency ratio exhibited significant differences during the ictal period. Language dominancy, hippocampal atrophy, amygdala enlargement, and sustained theta activity were also found to affect HRV. Seizure frequency was correlated with HRV indices, suggesting a potential relationship with the risk of SUDEP.

Introduction Insomnia and obstructive sleep apnea(OSA) may co-exit, and it is now named comorbid insomnia and OSA(COMISA). Such combined condition is associated with increased morbidity, and impairment, which requires treatment for both insomnia and OSA. Bioelectrical impedance analysis(BIA) is a widely used convenient method of body composition. In OSA patients, muscle and fat distribution are different among different sex, and severity of OSA. We hypothesized that the muscle distributions in COMISA were different from OSA, which can help early identification COMISA to optimize treatment. Methods All the patients aged 18 or older with symptoms of sleep disturbances who visited single tertiary sleep clinic in Korea were enrolled from December 2017 to September 2022. All patients underwent structured sleep evaluation, insomnia severity index (ISI), overnight polysomnography and BIA. All patients with apnea-hypopnea index (AHI) 15 or greater were enrolled. The COMISA were classified by insomnia symptoms based on the ISI score greater than 15. Results Total of 3328 subjects were finally enrolled(79.7% males and 20.3% females), and among them 29.5%(n=981) was COMISA. Compare to OSA without insomnia symptoms COMISA were older(COMISA,56.6 ± 12.9, OSA,54.3 ± 13.3; p< 0.001), and were more likely to be female(COMISA,27.5%, OSA,17.3%; p< 0.001). Although AHI had no significance, apnea was lower, and hypopnea was higher in COMISA than OSA. The muscle distribution in COMISA was different compared to OSA in non-obese groups(BMI< 25). In both male and female, total muscle mass(MM), skeletal muscle mass(SMM), Rt and Lt arm(RAMM, LAMM), Rt and Lt leg(RLMM, LLMM), and trunk(TMM) muscle mass, and fat free mass(FFM) were significantly different between non-obese COMISA and OSA. The correlation analysis in non-obese COMISA group revealed that ISI is negative correlated with muscle masses in both sex (M:F); MM(M,F:rho:-0.09*,-0.16*), SMM(rho:-0.01*,-0.17*), RAMM(rho:-0.07*,-0.15*) LAMM(rho:-0.07*,-0.16*), RLMM(rho:-0.09*,-0,15*), LLMM(rho:-0.09*,-0.16*), TMM(rho:-0.08*,-0.17*), FFM (rho: -0.1*,-0.16*)[*p< 0.05]. Conclusion This study revealed that the muscle distribution is different in non-obese, male and female, moderate to severe COMISA population. And insomnia symptom severity has negative relation with trunk and limb muscle masses. Our study suggests possible insomnia symptoms among OSA patients have different pathomechanisms in relation to body muscle distribution. Support (if any)

Neurocysticercosis (NCC) is the most common central nervous system parasite infection, frequently produces seizure, headache, or hydrocephalus as clinical manifestations. Cerebral vasculitis is an infrequent complication of the clinical phenotype of NCC. Moreover, NCC involving basal cerebral arteries, including distal internal carotid artery or middle cerebral artery, has rarely been reported. Therefore, we present a case of NCC with moyamoya-like basal cerebral arterial steno-occlusive disease with an emphasis on the differential diagnosis.

BACKGROUND AND PURPOSE Obesity is known of one of the risk factors for obstructive sleep apnea (OSA). Although body mass index (BMI) can be an indicator for obesity, it does not represent the actual body composition of fat or muscle. We hypothesized that bioelectrical impedance analysis (BIA) can help analyze the fat and muscle distributions in males and females with OSA. METHODS This study screened subjects who visited the Department of Neurology, Samsung Medical Center, Seoul, Korea due to sleep disturbances with symptoms suggestive of OSA from December 2017 to December 2019. All subjects underwent overnight type I polysomnography (PSG) and BIA. RESULTS PSG and BIA were completed in 2,064 OSA patients who had an apnea-hypopnea index (AHI) of ≥5/hour (77.1% males and 22.9% females).
