Abstract

The effectiveness of nitrosourea fotemustine in treating malignant melanoma, which is an aggressive tumor of melanocytes, pigmented skin cells, was assessed in 169 afflicted patients. The cancer was disseminated throughout the body, including the brain. Nitrosourea fotemustine was given in a dose of 100 milligrams per square meter of body area by intravenous infusion over a one-hour period, every week for three weeks. This was followed by a rest period of four to five weeks. Therapy was maintained at the same dose every three weeks in patients who responded or were stabilized by initial treatment, until their disease progressed. Of 169 patients treated, 153 were assessed for complete response, as indicated by the elimination of the tumor. Treatment with nitrosourea fotemustine resulted in complete responses in three cases and partial responses in 34 patients, thereby producing a response rate of 24.2 percent. The response rates at specific sites were 25 percent within the brain; 19.2 percent in the viscera (or abdominal region); and 31.8 percent at nonvisceral sites. In most cases, the duration of response was 22 weeks, and the response rate in previously untreated patients was 30.7 percent. The main toxic effects of nitrosourea fotemustine were blood-related, and included leukopenia (an abnormal decrease in white blood cells) and thrombocytopenia (an abnormal decrease in platelets, cells involved in blood clotting). Abstract

Fotemustine is a new nitrosourea which has shown some efficacy on disseminated malignant melanoma (DMM) (24.2% response rate (RR) among 153 patients in a Phase II trial) but little activity on hepatic metastasis (8.8% RR). In order to improve those poor results, hepatic intra-arterial infusion (HIAI) of fotemustine was performed. After two years, thirteen patients, all in good general condition, were evaluable. Seven were pretreated and six had extrahepatic metastasis on entry into the study. All patients had a surgically implanted intra-arterial catheter. The induction cycle consisted of 100 mg/m2/week for 3-4 weeks, followed by 5 weeks rest and maintenance therapy of 100 mg/m2 every 3 weeks for stabilized or responding patients. Two complete responses (CR) (72+ and 145+ weeks) and six partial responses (PR) (7-18.5 weeks) were observed. The hepatic RR reached 61.5%. A RR of 42.8% was registered on preexisting EHM (one CR and one PR on cerebral lesions). Nevertheless, this treatment is limited by the high progression rate of 46.1% in extrahepatic disease. Toxicity was mainly hematologic (grade III-IV), comprising 36% neutropenia and 15% thrombopenia. Hepatic intra-arterial infusion of fotemustine is efficient therapy for liver metastases of DMM, but combination schedules (IV + HIA) are warranted.

Abstract

Over the past decade, with the important development of neo-adjuvant chemotherapy associating cisplatin and 5 FU (CF), a major change in the indication of chemotherapy in head and neck cancer has taken place. There is still an ongoing discussion regarding the results in term of survical. Despite these controversies, some facts appear to be obvious: prognosis is better for patients with complete clinical response (CCR); rate of CCR is better in small tumors (T1 – T2) than in large tumors (T3 – T4).

33. A survey of the anthracycline derivatives in hematology.

Abstract

The anthracyclines, of which daunorubin (DNR), rubidazone (RBD), and adriamycin (ADR) are the most commonly used in cancer chemotherapy, belong to the group of intercalating agents capable of inhibiting the replication and transcription of DNA, and of fragmenting DNA and inhibiting the mitochondrial respiratory chain. Their experimental antitumoral activity varies according to the product, the type of tumor, and the route of administration. Following intravenous (IV) injection, they are distributed throughout all body tissues except those of the central nervous system and are metabolized principally in the liver, 90% of the metabolites being excreted in the bile. Excretion is slow, permitting intermittent high-dose therapy. Their principal toxicity is cardiac, presenting as congestive cardiac failure, which occurs when a specific cumulative dose of anthracycline is exceeded. This effect may be due to the formation of free semiquinone radicals from the anthracycline nucleus by the capture of electrons from the mitochondrial respiratory chain. Only a few of the most active derivatives have been studied in man and the search continues for active agents devoid of cardiotoxicity. The therapeutic indications vary according to the derivative. DNR, which we have utilized since 1967, is remarkably effective in induction therapy for acute myeloblastic leukemia, is the only agent which is effective in acute promyelocytic leukemia, and increases the number of complete remissions in acute lymphoblastic leukemia in the adult and in severe childhood forms of this disorder. ADR is active against solid tumors (thyroid, breast, osteosarcoma) and also against lymphomas. RBD induces a complete remission in two-thirds of the cases of acute monoblastic leukemia, is equally as effective as DNR in acute myeloblastic leukemia, and is significantly more manipulable. This agent may similarly induce a remission in the severe lymphomas (lymphosarcoma and Hodgkin's disease). A new semisynthetic compound, DEA.14.DNR, appears promising. Abstract

We have recently demonstrated the occurrence of functional circulating soluble cell-free Fcγ2b/γ1 receptor in normal mouse serum by means of the 2.4G2 rat monoclonal antibody. With the solid phase radioimmunoassay described previously, we report here a dramatic increase in the level of circulating soluble cell-free Fcγ2b/γ1R in Schistosoma mansoni-infected mice individually tested before and on days 22, 41,62, and 82 after infection. This increase was statistically highly significant when the five measurements from each mouse were compared, and also when the entire group of infected mice was compared with the stable level of Cf-Fcγ2b/γlR measured in strain-, sex-, and age-matched control mice individually tested on the same days. This increase was commensurate with a rise in the amount of IgG detected in the sera of the S. mansoni-infected mice. Thus, infection seems to be one of the factors that modulate the expression of such soluble Cf-Fcγ2b/γlR in mouse serum. Furthermore, the experimental device reported here for the production of high levels of Cf-Fcγ2b/γ1R by infection (in this case by parasitic infection) could serve as a model for obtaining large quantities of serum Cf-Fcγ2b/γ1R for further purification. Lastly, we point out that, by establishing a functional relationship with circulating IgG, such soluble Cf-Fcγ2b/γ1R may modulate some of the functions in which the Fc portion of immunoglobulins is involved. Abstract

A total of 42 patients with cerebral metastases of malignant melanoma were included in this study of the nitrosourea fotemustine. The treatment plan consisted of a l-h i.v. infusion of 100 mg/m2 fotemustine every week for 3-4 weeks, followed by a 4- to 5-week rest period. Responding or stabilised patients then received 100 mg/m2 fotemustine every 3 weeks. Among the 39 evaluable patients, 2 complete responses and 9 partial responses were documented, leading to an overall response rate of 28.2%. Most of the responses were obtained in previously untreated patients and/or those presenting with a single cerebral metastasis. Toxicity was mild and mainly hematological, especially in patients previously treated by polychemotherapeutic regimen. Abstract

Ninety-eight patients with locally advanced breast cancer (Stage IIIA-IIIB) were entered into a pilot study combining intensive induction (neoadjuvant) chemotherapy (VTMFAP) with or without hormonochemotherapy, external and interstitial radiotherapy, and consolidation chemotherapy with or without hormonochemotherapy. Tumor regression over 50% was observed in 91% patients after chemotherapy, and complete clinical remission occurred in 100% patients after irradiation. The rate of local relapse is 13%. The 3-year disease-free survival is 62% and 3-year global survival is 77%. Initial chemotherapeutic tumor regression greater than 75% is the main predictive factor for disease-free survival.

Abstract

The immunologic status of 146 patients with acute leukemia has been explored at different stages of the disease, before and during various treatments, and compared with clinical and hematologic status. Preestablished delayed hypersensitivity (D.H.) was studied by intradermal injection of 4 antigens; antibody response by injection of polio vaccine. D.H. reactions were found to be slightly impaired before treatment. During induction therapy, the immunodepression involves mainly antibody production, while D.H. is preserved in most cases. Abolition of D.H. reactions occur during drug-induced aplasias, and strongly correlates the severity of the aplasia. Maintenance treatments tested did not significantly affect immunologic reactivity of patients in remission.
