Abstract

Background: Growing epidemiological evidence suggests an adverse relationship between exposure to air pollutants and cognitive health, and this could be related to the effect of air pollution on vascular health. Objective: We aim to evaluate the association between air pollution exposure and a magnetic resonance imaging (MRI) marker of cerebral vascular burden, white matter hyperintensities (WMH). Methods: This cross-sectional analysis used data from the French Three-City Montpellier study. Randomly selected participants 65-80 years of age underwent an MRI examination to estimate their total and regional cerebral WMH volumes. Exposure to fine particulate matter (P[M.sub.2.5]), nitrogen dioxide (N[O.sub.2]), and black carbon (BC) at the participants' residential address during the 5 years before the MRI examination was estimated with land use regression models. Multinomial and binomial logistic regression assessed the associations between exposure to each of the three pollutants and categories of total and lobar WMH volumes. Results: Participants' (n = 582) median age at MRI was 70.7 years [interquartile range (IQR): 6.1], and 52% (n = 300) were women. Median exposure to air pollution over the 5 years before MRI acquisition was 24.3 (IQR: 1.7) [micro]g/[m.sup.3] for P[M.sub.2.5], 48.9 (14.6) [micro]g/[m.sup.3] for N[O.sub.2], and 2.66 (0.60) [10.sup.-5]/m for BC. We found no significant association between exposure to the three air pollutants and total WMH volume. We found that P[M.sub.2.5] exposure was significantly associated with higher risk of temporal lobe WMH burden [odds ratio (OR) for an IQR increase = 1.82 (95% confidence interval: 1.41, 2.36) for the second volume tercile, 2.04 (1.59, 2.61) for the third volume tercile, reference: first volume tercile]. Associations for other regional WMH volumes were inconsistent. Conclusion: In this population-based study in older adults, P[M.sub.2.5] exposure was associated with increased risk of high WMH volume in the temporal lobe, strengthening the evidence on P[M.sub.2.5] adverse effect on the brain. Abstract

Background: Black carbon (BC), a component of fine particulate matter [particles with an aerodynamic diameter [less than or equal to] 2.5 [micro]m ([PM.sub.2.5])], may contribute to carcinogenic effects of air pollution. Until recently however, there has been little evidence to evaluate this hypothesis. Objective: This study aimed to estimate the associations between long-term exposure to BC and risk of cancer. This study was conducted within the French Gazel cohort of 20,625 subjects. Methods: We assessed exposure to BC by linking subjects' histories of residential addresses to a map of European black carbon levels in 2010 with back- and forward-extrapolation between 1989 and 2015. We used extended Cox models, with attained age as time-scale and time-varying cumulative exposure to BC, adjusted for relevant sociodemographic and lifestyle variables. To consider latency between exposure and cancer diagnosis, we implemented a 10-y lag, and as a sensitivity analysis, a lag of 2 y. To isolate the effect of BC from that of total [PM.sub.2.5], we regressed BC on [PM.sub.2.5] and used the residuals as the exposure variable. Results: During the 26-y follow-up period, there were 3,711 incident cancer cases (all sites combined) and 349 incident lung cancers. Median baseline exposure in 1989 was 2.65 [10.sup.-5]/m [interquartile range (IQR): 2.23-3.33], which generally slightly decreased over time. Using 10 y as a lag-time in our models, the adjusted hazard ratio per each IQR increase of the natural log-transformed cumulative BC was 1.17 (95% confidence interval: 1.06, 1.29) for all-sites cancer combined and 1.31 (0.93, 1.83) for lung cancer. Associations with BC residuals were also positive for both outcomes. Using 2 y as a lag-time, the results were similar. Discussion: Our findings for a cohort of French adults suggest that BC may partly explain the association between [PM.sub.2.5] and lung cancer. Abstract

Background There is evidence to suggest that long term exposure to air pollution could be associated with decreased levels of fertility, although there is controversy as to how short term exposure may compromise fertility in IVF patients and what windows of exposure during the IVF process patients could be most vulnerable. Methods This prospective cohort study aimed to evaluate the impact of acute exposure that air pollution have on reproductive outcomes in different moments of the IVF process. Women undergoing IVF living in Barcelona were recruited. Abstract

OBJETIVO: Describir un brote por Pseudomonas aeruginosa, en el área de atención ambulatoria de herida quirúrgica, en pacientes posmastectomizadas. MATERIAL Y MÉTODOS: Entre el 13 de marzo y el 18 de mayo de 2000 se definió como caso a las pacientes del Instituto Nacional de Cancerología de la Ciudad de México, mastectomizadas con infección quirúrgica por P aeruginosa resistente a ciprofloxacina y gentamicina. Se tomaron cultivos de los antisépticos, del material de curaciones, del agua y del personal de salud. Se efectuó un análisis de casos y controles. RESULTADOS: Se identificaron 13 infecciones tardías del sitio quirúrgico por P aeruginosa. La Pseudomona se aisló de las narinas de la enfermera y de las gasas que ella misma colocaba sobre la mesa de mayo, pero no de los paquetes de gasas cerrados del área de curaciones ambulatoria de tumores mamarios. El 14 de abril de 2000 se transfirió a la enfermera a otra área y se instauraron medidas estrictas para el control de infecciones. Después de esta fecha se diagnosticaron cuatro casos más. La radioterapia fue el único factor de riesgo asociado con la infección (RM=5.1, IC 95%=1.1-28.4). CONCLUSIONES: El brote probablemente estuvo causado en un inicio por una fuente común y se extendió por infección cruzada entre las pacientes. Las prácticas deficientes detectadas en el manejo de heridas y drenajes condujo a instaurar medidas preventivas específicas.

Abstract

Background: Very few studies have examined whether a long-term beneficial effect of physical activity on lung function can be influenced by living in polluted urban areas. Objective: We assessed whether annual average residential concentrations of nitrogen dioxide (NO2) and particulate matter with aerodynamic diameters < 2.5 mu m (PM2.5) and < 10 mu m (PM10) modify the effect of physical activity on lung function among never- (N = 2801) and current (N = 1719) smokers in the multi-center European Community Respiratory Health Survey. Methods: Associations between repeated assessments (at 27-57 and 39-67 years) of being physically active (physical activity: >= 2 times and >= 1 h per week) and forced expiratory volume in 1 s (FEV1) and forced vital capacity (FVC) were evaluated using adjusted mixed linear regression models. Models were conducted separately for never-and current smokers and stratified by residential long-term NO2, PM2.5 mass and PM10 mass concentrations (<= 75th percentile (low/medium) versus > 75th percentile (high)). Results: Among current smokers, physical activity and lung function were positively associated regardless of air pollution levels. Among never-smokers, physical activity was associated with lung function in areas with low/medium NO2, PM2.5 mass and PM10 mass concentrations (e.g. mean difference in FVC between active and non-active subjects was 43.0 mL (13.6, 72.5), 49.5 mL (20.1, 78.8) and 49.7 mL (18.6, 80.7), respectively), but these associations were attenuated in high air pollution areas. Only the interaction term of physical activity and PM10 mass for FEV1 among never-smokers was significant (p-value = 0.03). Conclusions: Physical activity has beneficial effects on adult lung function in current smokers, irrespective of residential air pollution levels in Western Europe. Trends among never-smokers living in high air pollution areas are less clear.

BACKGROUND: Evidence on the short-term effects of fine and coarse particles on morbidity in Europe is scarce and inconsistent. OBJECTIVES: We aimed to estimate the association between daily concentrations of fine and coarse particles with hospitalizations for cardiovascular and respiratory conditions in eight Southern European cities, within the MED-PARTICLES project. METHODS: City-specific Poisson models were fitted to estimate associations of daily concentrations of particulate matter with aerodynamic diameter ≤ 2.5 µm ([PM.sub.2.5]), [less than or equal to] 10 µm ([PM.sub.10]), and their difference ([PM.sub.2,5-10]) with daily counts of emergency hospitalizations for cardiovascular and respiratory diseases. We derived pooled estimates from random-effects meta-analysis and evaluated the robustness of results to co-pollutant exposure adjustment and model specification. Pooled concentration--response curves were estimated using a meta-smoothing approach. RESULTS: We found significant associations between all PM fractions and cardiovascular admissions. Increases of 10 µg/[m.sup.3] in [PM.sub.2.5], 6.3 µg/[m.sup.3] in [PM.sub.2.5-10], and 14.4 µg/[m.sup.3] in [PM.sub.10](lag 0-1 days) were associated with increases in cardiovascular admissions of 0.51% (95% CI: 0.12, 0.90%), 0.46% (95% CI: 0.10, 0.82%), and 0.53% (95% CI: 0.06, 1.00%), respectively. Stronger associations were estimated for respiratory hospitalizations, ranging from 1.15% (95% CI: 0.21, 2.11%) for [PM.sub.10] to 1.36% (95% CI: 0.23, 2.49) for [PM.sub.2.5] (lag 0-5 days). CONCLUSIONS: [PM.sub.2.5] and [PM.sub.2.5-10] were positively associated with cardiovascular and respiratory admissions in eight Mediterranean cities. Abstract

BACKGROUND: Errors in address geocodes may affect estimates of the effects of air pollution on health. OBJECTIVE: We investigated the impact of four geocoding techniques on the association between urban air pollution estimated with a fine-scale (10 m x 10 m) dispersion model and lung function in adults. METHODS: We measured forced expiratory volume in 1 sec ([FEV.sub.1]) and forced vital capacity (FVC) in 354 adult residents of Grenoble, France, who were participants in two well-characterized studies, the Epidemiological Study on the Genetics and Environment on Asthma (EGEA) and the European Community Respiratory Health Survey (ECRHS). Home addresses were geocoded using individual building matching as the reference approach and three spatial interpolation approaches. We used a dispersion model to estimate mean [PM.sub.10] and nitrogen dioxide concentrations at each participant's address during the 12 months preceding their lung function measurements. Associations between exposures and lung function parameters were adjusted for individual confounders and same-day exposure to air pollutants. The geocoding techniques were compared with regard to geographical distances between coordinates, exposure estimates, and associations between the estimated exposures and health effects. RESULTS: Median distances between coordinates estimated using the building matching and the three interpolation techniques were 26.4, 27.9, and 35.6 m. Compared with exposure estimates based on building matching, [PM.sub.10] concentrations based on the three interpolation techniques tended to be overestimated. When building matching was used to estimate exposures, a one-interquartile range increase in [PM.sub.10] (3.0 µg/[m.sup.3]) was associated with a 3.72-point decrease in FVC% predicted (95% CI: -0.56, -6.88) and a 3.86-point decrease in [FEV.sup.1]% predicted (95% CI: -0.14, -3.24). The magnitude of associations decreased when other geocoding approaches were used [e.g., for FVC% predicted -2.81 (95% CI: -0.26, -5.35) using NavTEQ, or 2.08 (95% CI -4.63, 0.47, p = 0.11) using Google Maps]. Abstract

Satellite-derived (SAT) and chemical transport model (CTM) estimates of PM2.5 and NO2 are increasingly used in combination with Land Use Regression (LUR) models. We aimed to compare the contribution of SAT and CTM data to the performance of LUR PM2.5 and NO2 models for Europe. Four sets of models, all including local traffic and land use variables, were compared (LUR without SAT or CTM, with SAT only, with CTM only, and with both SAT and CTM). LUR models were developed using two monitoring data sets: PM2.5 and NO2 ground level measurements from the European Study of Cohorts for Air Pollution Effects (ESCAPE) and from the European AIRBASE network. LUR PM2.5 models including SAT and SAT+CTM explained similar to 60% of spatial variation in measured PM2.5 concentrations, substantially more than the LUR model without SAT and CTM (adjR(2): 0.33-0.38). For NO2 CTM improved prediction modestly (adjR2: 0.58) compared to models without SAT and CTM (adjR2: 0.47-0.51). Both monitoring networks are capable of producing models explaining the spatial variance over a large study area. SAT and CTM estimates of PM2.5 and NO2 significantly improved the performance of high spatial resolution LUR models at the European scale for use in large epidemiological studies. Abstract

Occupational exposure is a well-recognised modifiable risk factor for asthma, but the relationship between occupational exposure and asthma control has not been studied. We aimed to study this association among working-age adults from the European Community Respiratory Health Survey (ECRHS). Data were available for 7077 participants (mean age 43 years, 45% never-smokers, 5867 without asthma and 1210 with current asthma). Associations between occupational exposure to specific asthmagens and asthma control status (33% with uncontrolled asthma, based on the Global Initiative for Asthma guidelines) were evaluated using logistic and multinomial regressions, adjusted for age, sex and smoking status, with study areas included as a random effect. Statistically significant positive associations were observed between uncontrolled adult-onset asthma and both past 12-month and 10-year exposure to any occupational asthmagens (OR (95% CI) 1.6 (1.0-2.40) and 1.7 (1.2-2.5), respectively); high (1.7 (1.0-2.8) and 1.9 (1.3-2.9), respectively) and low (1.6 (1.0-2.7) and 1.8 (1.2-2.7), respectively) molecular weight agents; and cleaning agents (2.0 (1.1-3.6) and 2.3 (1.4-3.6), respectively), with stronger associations for long-term exposures. These associations were mainly explained by the exacerbation domain of asthma control and no associations were observed between asthmagens and partly controlled asthma. These findings suggest that occupational exposure to asthmagens is associated with uncontrolled adult-onset asthma. Occupational risk factors should be quickly identified to prevent uncontrolled asthma.
