Abstract

Purpose: The global cancer burden and mortality rates are increasing, with significant disparities in access to care in low- and middle-income countries (LMICs). This study aimed to identify radiology and radiation therapy needs in LMICs from the perspective of departmental and institutional leaders. Methods and Materials: A survey was developed and conducted by the American Association of Physicists in Medicine Global Needs Assessment Committee and the American Association of Physicists in Medicine International Council. The survey, organized into 5 sections (Introduction, Infrastructure Needs, Education Needs, Research Needs, and General Information), was open to respondents from March 1, to August 16, 2022. Results: A total of 175 responses were received from 6 global regions: Africa (31.4%), the Americas (17.7%), the Eastern Mediterranean (14.3%), Europe (9.1%), Southeast Asia (23.4%), and the Western Pacific (4.0%). The greatest reported need was for new or updated equipment, particularly positron emission tomography/computed tomography imaging technology. There was also a high demand for clinical and equipment training. Approximately 25% of institutions reported a lack of radiology-based cancer screening programs because of high health care costs and a shortage of specialized equipment. Many institutions that expressed interest in research face funding and grant challenges. Conclusions: The findings highlight critical areas where organizations can support LMICs in enhancing radiology and radiation therapy services to mitigate the growing cancer burden.

2. Reducing Preoperative Caregiver Anxiety With Virtual Reality: A Pragmatic, Randomized Controlled Study

Abstract

Pediatric patients and their caregivers often experience perioperative anxiety. Interventions reduce caregiver anxiety improve cooperation and contribute to an improved patient experience. This study seeks to evaluate the efficacy of virtual reality (VR)-assisted mindfulness on perioperative caregiver anxiety. Participants were randomized into a standard of care (SOC) group, which included snacks but no technology-based distractions, or a VR group, which included snacks and a VR-guided meditation. Caregiver anxiety was measured before and after the intervention using the Visual Analogue Scale for Anxiety (VAS-A). Secondary aims explored participants’ baseline anxiety with the State-Trait Anxiety Inventory (STAI). VR group participants completed a satisfaction survey. Linear regression models of VAS-A and STAI were used to compare group differences. Satisfaction survey results were reported with descriptive statistics. 26 participants were included, with 12 randomized to the SOC group and 14 to the VR group. VAS-A scores in the VR group were lower than those in the SOC group ( p = .002). The STAI found no change in participants’ state of anxiety in the SOC group ( p = .7108), compared to a significant reduction ( p = .014) in the VR group when controlling for anxiety traits. 12 of 14 caregivers in the VR group expressed satisfaction or strong satisfaction. This study supports the implementation of VR mindfulness as a method to reduce caregiver anxiety. VR use in the pediatric healthcare setting is safe and inexpensive, and the intervention had a high degree of participant satisfaction.

3. Outcomes of Primary Simultaneous Pancreas-kidney Transplants by Induction Agent in the United States

Abstract

Background. Long-term outcome data by induction type in simultaneous pancreas-kidney (SPK) is limited. Methods. Utilizing the Scientific Registry of Transplant Recipients, we examined all primary SPK transplants between 2000 and 2020, excluding crossmatch-positive recipients. We grouped recipients according to induction regimen into 3 groups: rabbit anti-thymocyte globulin (r-ATG) (n = 5678), alemtuzumab (n = 1199), and interleukin-2 receptor antagonist (IL-2RA; n = 1593). We analyzed the 10-y recipient and composite (kidney and pancreas) graft survival using the Kaplan-Meier survival function. Cox-proportion hazard models were generated to examine the association between induction type, the 10-y recipient, and graft survival. Models were adjusted for recipient age, sex, ethnicity, HLA-mismatch, diabetes type, dialysis dependency, cold-ischemia time, local versus imported organs, panel reactive antibody, steroid maintenance, and Pancreas Donor Risk Index. Results. r-ATG was associated with the lowest 1-y kidney and pancreas rejection rates compared with other agents (P < 0.001). In the univariable analysis, induction type was not associated with recipient (log-rank P = 0.11) or graft survival (log-rank P = 0.36). In the multivariable model for the composite graft survival, alemtuzumab use was associated with 22% increased kidney or pancreas graft loss compared with r-ATG (adjusted hazard ratio, 1.22; 95% confidence interval, 1.05–1.42), whereas IL-2RA use was not a predictor of graft survival. Induction type did not influence recipient survival in the adjusted model. Conclusions. r-ATG use was associated with the lowest SPK rejection rates. Compared with r-ATG, alemtuzumab but not IL-2RA was associated with worse long-term death-censored SPK graft outcome. Our analysis supports the common use of r-ATG for induction in US primary SPK recipients.

Abstract

Background. Laparoscopic donor nephrectomy (LDN) offers advantages to the donor. The reported incidence of testicular pain after LDN varies in the literature ranging from 3% to 55%. Methods. A survey was sent to 322 male LDN patients who donated from February 5, 2009, to February 5, 2019. The survey assessed if the donor had testicular pain or saw an additional medical professional after donation. Results. Of the 322 surveyed, 147 (46%) responses were received. Of those who had a left nephrectomy, 39% had testicular pain; 23.8% of those patients had testicular swelling in addition. Of those who had pain, laterality of kidney donated did not impact if the patient had pain, pain onset, pain level, or pain duration. Of those who donated their right kidney, 35% had testicular pain, and 16.7% of those patients reported testicular swelling in addition. Twenty-seven symptomatic patients sought additional medical care for the testicular symptoms postdonation. Seven (25%) had hydroceles, 2 (7%) had testicular cysts, 1 had a urinary tract infection, and 16 (59%) had reassurance or no additional procedures provided. Conclusions. Our results suggest that orchialgia is not as uncommon as previously thought and may be one of the most common minor complications experienced by male donors.

Abstract

Hospital-based protocols to support pet care needs for hospitalized patients may have potential to benefit patient health and wellbeing, but must be informed by experiences of hospitalized pet owners. The aim of this study was to determine the scope and need for pet care services among hospitalized patients. A panel of prior inpatients and their family members at a tertiary care center were surveyed about preferences, experiences, and need for assistance with pet care during hospitalization. Respondents (n = 113) expressed interest in a low/no-cost pet-boarding or foster program for adult patients struggling to find pet care assistance. The majority of respondents (n = 71; 63%) reported challenges securing pet care during a prior hospitalization, and/or knew someone who encountered similar challenges. Respondents also indicated that these challenges had a negative impact on health, recovery, or their own decision to receive medical care. Pet care challenges during hospitalization are likely common and have the potential to hamper medical decision-making and health outcomes of inpatients.

Abstract

Recent accurate measurements of cosmic-ray (CR) species by ATIC-2, CREAM, and PAMELA reveal an unexpected hardening in the proton and He spectra above a few hundred GeV, a gradual softening of the spectra just below a few hundred GeV, and a harder spectrum of He compared to that of protons. These newly discovered features may offer a clue to the origin of high-energy CRs. We use the Fermi Large Area Telescope observations of the γ-ray emission from Earth's limb for an indirect measurement of the local spectrum of CR protons in the energy range ∼90  GeV-6  TeV (derived from a photon energy range 15 GeV-1 TeV). Our analysis shows that single power law and broken power law spectra fit the data equally well and yield a proton spectrum with index 2.68±0.04 and 2.61±0.08 above ∼200  GeV, respectively.

Abstract

Observations of occultations of bright γ-ray sources by the Sun may reveal predicted pair halos around blazars and/or new physics, such as, e.g., hypothetical light dark matter particles-axions. We use Fermi Gamma-Ray Space Telescope (Fermi) data to analyze four occultations of blazar 3C 279 by the Sun on October 8 each year from 2008 to 2011. A combined analysis of the observations of these occultations allows a point-like source at the position of 3C 279 to be detected with significance of ≈3σ, but does not reveal any significant excess over the flux expected from the quiescent Sun. The likelihood ratio test rules out complete transparency of the Sun to the blazar γ-ray emission at a 3σ confidence level.

8. Association Between Hyperoxia, Supplemental Oxygen, and Mortality in Critically Injured Patients

Abstract

OBJECTIVES:. Hyperoxia is common among critically ill patients and may increase morbidity and mortality. However, limited evidence exists for critically injured patients. The objective of this study was to determine the association between hyperoxia and in-hospital mortality in adult trauma patients requiring ICU admission. DESIGN, SETTING, AND PARTICIPANTS:. This multicenter, retrospective cohort study was conducted at two level I trauma centers and one level II trauma center in CO between October 2015 and June 2018. All adult trauma patients requiring ICU admission within 24 hours of emergency department arrival were eligible. The primary exposure was oxygenation during the first 7 days of hospitalization. INTERVENTIONS:. None. MEASUREMENTS AND MAIN RESULTS:. Primary outcome was in-hospital mortality. Secondary outcomes were hospital-free days and ventilator-free days. We included 3,464 critically injured patients with a mean age of 52.6 years. Sixty-five percent were male, and 66% had blunt trauma mechanism of injury. The primary outcome of in-hospital mortality occurred in 264 patients (7.6%). Of 226,057 patient-hours, 46% were spent in hyperoxia (oxygen saturation > 96%) and 52% in normoxia (oxygen saturation 90–96%). During periods of hyperoxia, the adjusted risk for mortality was higher with greater oxygen administration. At oxygen saturation of 100%, the adjusted risk scores for mortality (95% CI) at Fio2 of 100%, 80%, 60%, and 50% were 6.4 (3.5–11.8), 5.4 (3.4–8.6), 2.7 (1.7–4.1), and 1.5 (1.1–2.2), respectively. At oxygen saturation of 98%, the adjusted risk scores for mortality (95% CI) at Fio2 of 100%, 80%, 60%, and 50% were 7.7 (4.3–13.5), 6.3 (4.1–9.7), 3.2 (2.2–4.8), and 1.9 (1.4–2.7), respectively. CONCLUSIONS:. During hyperoxia, higher oxygen administration was independently associated with a greater risk of mortality among critically injured patients. Abstract

This catalog summarizes 117 high-confidence ≥0.1 GeV gamma-ray pulsar detections using three years of data acquired by the Large Area Telescope (LAT) on the Fermi satellite. Half are neutron stars discovered using LAT data through periodicity searches in gamma-ray and radio data around LAT unassociated source positions. The 117 pulsars are evenly divided into three groups: millisecond pulsars, young radio-loud pulsars, and young radio-quiet pulsars. We characterize the pulse profiles and energy spectra and derive luminosities when distance information exists. Spectral analysis of the off-peak phase intervals indicates probable pulsar wind nebula emission for four pulsars, and off-peak magnetospheric emission for several young and millisecond pulsars. We compare the gamma-ray properties with those in the radio, optical, and X-ray bands. We provide flux limits for pulsars with no observed gamma-ray emission, highlighting a small number of gamma-faint, radio-loud pulsars. The large, varied gamma-ray pulsar sample constrains emission models. Fermi's selection biases complement those of radio surveys, enhancing comparisons with predicted population distributions. © 2013. The American Astronomical Society. All rights reserved..

Abstract

Pulsar wind nebulae (PWNe) have been established as the most populous class of TeV γ-ray emitters. Since launch, the Fermi Large Area Telescope (LAT) has identified five high-energy (100 MeV < E < 100 GeV) γ-ray sources as PWNe and detected a large number of PWN candidates, all powered by young and energetic pulsars. The wealth of multi-wavelength data available and the new results provided by Fermi-LAT give us an opportunity to find new PWNe and to explore the radiative processes taking place in known ones. The TeV γ-ray unidentified (UNID) sources are the best candidates for finding new PWNe. Using 45 months of Fermi-LAT data for energies above 10 GeV, an analysis was performed near the position of 58 TeV PWNe and UNIDs within 5° of the Galactic plane to establish new constraints on PWN properties and find new clues on the nature of UNIDs. Of the 58 sources, 30 were detected, and this work provides their γ-ray fluxes for energies above 10 GeV. The spectral energy distributions and upper limits, in the multi-wavelength context, also provide new information on the source nature and can help distinguish between emission scenarios, i.e., between classification as a pulsar candidate or as a PWN candidate. Six new GeV PWN candidates are described in detail and compared with existing models. A population study of GeV PWN candidates as a function of the pulsar/PWN system characteristics is presented. © 2013. The American Astronomical Society. All rights reserved.

12. Community Conversations: Toward Shared Understandings of American Identity.

Abstract

This document describes a program called Community Conversations, a joint effort between Phi Theta Kappa, the international honor society of the two-year college, and the Community College Humanities Association. The program consisted of a series of national and regional conversations among students and community members, addressing what it means to be an American. Highlights of the conversations, which took place at two-year institutions in Colorado, Indiana, Michigan, Mississippi, Missouri, New York, Ohio, Oregon, North and South Carolina, Texas, and Washington, include: (1) one theme that emerged repeatedly was the keen interaction and interplay between national issues and local issues; (2) participants defined themselves and their communities by a sense of place, yet noted the difficulties of overcoming isolation; (3) the continuing tragedy of racism provoked strong reactions from African Americans, and questions of racial difference and treatment occasionally brought conversations to a halt; (4) the conversations augmented people's sense of community and clarified their understanding of questions of American identity. Each conversation is described in detail. This paper also presents guidelines for conversation leaders--step-by-step suggestions on how to plan and organize these types of conversations. 13. Preexposure Prophylaxis and Patient Centeredness

Abstract

Preexposure prophylaxis has transformed HIV prevention, becoming widespread in communities of gay and bisexual men in the developed world in a short time. There is a broad concern that preexposure prophylaxis will discourage condom use among gay men (i.e., “risk compensation”). This commentary argues for broadening the focus on gay men’s health beyond sexual health to address the holistic health and well-being of gay men. Gay men may benefit from being offered candid, nonjudgmental health promotion/HIV prevention messages not requiring condom use for anal sex. Lessons can be drawn from the family planning movement, which has undergone a similar shift in focus. The principle of patient centeredness supports such a shift in gay men’s health toward the goal of providing men with the knowledge to evaluate various prevention approaches according to the specifics of their life circumstances and health needs. Bringing more nuance to discussions of sexual risk and sexual pleasure could facilitate more universally healthy attitudes regarding sex among gay men, in turn enabling healthier decisions more compatible with men’s own values and preferences.

15. The Extent and Distribution of Poverty in Mississippi.

Abstract

Statistical and descriptive data are used to analyze the location, incidence, characteristics and general dimensions of poverty in Mississippi. The following areas are examined: Mississippi's population structure, the development of public concern with anti-poverty measures and the State's economic structure in terms of its income sources and its labor force. The paper identifies major characteristics and problems of the poor which are apparent from the study of the census data and other available materials. These include family composition, family size, nutrition, health, income, and education. Extensive statistical detail is found in the two appendices. (Author/AM)

16. Considerations Concerning Kindergarten Education: A Kindergarten Report and Position Paper.

Abstract

This committee report and position paper recommends that a system of state public kindergartens should be established in Mississippi. Justification for public kindergartens is based on two major premises: (1) a belief that every child can benefit from quality kindergartens as they are described in this paper, and (2) a belief that kindergartens based on a child development philosophy could influence the traditional primary schools to provide more appropriate experiences for six-, seven-, and eight-year-olds. Specific recommendations for implementing public kindergartens are suggested: (1) passage of enabling legislation, (2) appropriation of funds for a selected number of kindergarten programs, (3) designation of responsibility for selection of initial kindergarten programs to the State Department of Education, and (4) appointment of a Supervisor of Early Childhood Education to act as kindergarten consultant to school districts. Appendixes include a listing of the kindergarten legislation from every state through 1974, and an outline of proposed kindergarten legislation in Mississippi from 1971 through 1974.

17. New Orleans Effective Schools Project. An Interim Report to the Orleans Parish School Board.

Abstract

The primary goal of the Southern Coalition for Educational Equity's New Orleans Effective Schools Project is to improve academic achievement at one middle school, Martin Behrman, in ways that can be replicated by schools facing similar problems. The Project is based on research findings about school improvement from the school effectiveness literature and focuses resources--primarily in-person assistance--on one school for a 5-year period. Martin Behrman Middle School was selected as the first Project School from among twelve "high risk" schools. In the fall of 1982, Behrman began its planning year, to be followed by three years of implementation. The goal of the planning year was to establish a Site Council representing all relevant constituencies and to develop a plan that would increase academic achievement over a three to five year period. During the first implementation year, at the request of the Site Council, project staff conducted a full day workshop on positive discipline and helped a committee of teachers develop a system of rules and consequences that included both preventive and punitive components. A management team was created to support the school administrative staff. Additionally, the Behrman Special Summer Program was implemented and achieved great success. The Summer Program provided five weeks of daily reading instruction to over 90 students. Final test results of the summer students demonstrated larger than expected gains in reading comprehension. In the Project's second year of implementation, it continued to build on changes begun during the first year. The College Board's reading comprehension test, Degrees of Reading Power (DRP) was administered to insure measurement of progress towards the goal of improved comprehension, and an outside expert prepared a report on the barriers to effective use of instructional time at Behrman. As the project enters the third year of implementation, it is ready to be replicated in similar "high-risk" schools. A network between Behrman and other schools will be developed, and the replication schools will be selected by the end of 1985. (KH)

Abstract

This annual report summarizes the work and progress of programs which promote effective education in 11 Southern states. Working cooperatively with other groups, this advocacy organization has developed a model for putting effective schools research into practice. The Effective Schools Model has been accepted for use by the New Orleans Public Schools. The model includes methods for improving instruction, increasing parent involvement, developing staff competence, and evaluating and documenting the program. Other ongoing projects are: (1) a special summer program which has improved children's reading skills and teachers' classroom skills; and (2) a project which brings innovative uses of microcomputers to minority children. The shared educational philosophy of these programs is that poverty and discrimination are not insurmountable barriers; poor and minority children can and will be well educated. Information is provided about the supporters, directors and staff of the organization. (VM)

Association for SNP rs6983267 in the CCAT2 gene only was noteworthy after multiple test correction (P = 0.001). Site-specific analysis showed association between SNPs rs1799853 and rs2302615 with reduced colon cancer risk only (P = 0.01 and P = 0.004, respectively), however neither reached significance threshold following multiple test correction. Meta-analysis of SNPs rs2070959 and rs1105879 in UGT1A6 gene showed interaction between aspirin use and CRC risk (Pinteraction = 0.01 and 0.02, respectively); stratification by aspirin use showed an association for decreased CRC risk for aspirin users having a wild-type genotype (rs2070959 OR = 0.77, 95% CI = 0.68-0.86; rs1105879 OR = 0.77 95% CI = 0.69-0.86) compared to variant allele cariers. The direction of the interaction however is in contrast to that published in studies on colorectal adenomas. 22. The Status of Minimum Competency Programs in Twelve Southern States: A Report of the Southeastern Public Education Program.

Abstract

The legislative history and provisions, implementation, and intended use of 12 state-mandated programs are described. Florida's program was mandated in 1971. The other states (Alabama, Arkansas, Georgia, Kentucky, Louisiana, Mississippi, North Carolina, South Carolina, Tennessee, Texas and Virginia) initiated programs after 1975; most are in their initial stages with full scale operation expected in 1982 or 1983. The programs share these features: (1) performance objectives are established; (2) state developed or state contracted tests are administered to three or four elementary and secondary grade levels; (3) specific consequences are mandated for students who fail. Six states, for example, require high school students to pass minimum competency tests before graduating. Although these programs attract citizens and policy makers they raise many problems: funds are needed for remedial programs; states devote more effort to assessing than to satisfying needs of disadvantaged students; management and reporting systems are complex; mandates do not always apply to private schools or handicapped students; grade retention and diploma denial have social, educational, and financial costs; and school districts may lose autonomy. In the final analysis, teaching, not testing, can best assure the attainment of competencies. (CP)

23. A Demographic Analysis of Poverty in Mississippi.

Abstract

One of the functions of the Governor's Office of Human Resources is that of gathering, analyzing, and distributing information on the extent, distribution and characteristics of the poverty population in Mississippi and the social, economic and demographic conditions which affect the poor. This lengthy document disburses that kind of information as it sets forth a great deal of material on the statistical descriptors of the state, its economy, population, parameters of cultural and social change and prospects which some of the changes may present for the future. Data on epidomology, familial composition, labor force characteristics, population processes and many other features of the state and her people are also included. The book is divided into three major sections: Part A presents resource material for program planning with chapters on conceptualization and measurement of poverty, county demographic data, and a Community Action Agency user guide and index. Part B centers on a demographic and economic profile of the state. Part C provides a demographic portrait of the Mississippi poor with chapters on the distribution and characteristics of the poor and agricultural and rural poverty. Appendices deal with poverty as it relates to inflation and present auxilliary statistical data. A specific objective of this document was to provide Community Action Agencies with readily available data required for their annual funding package. (DS)

24. Poverty in Mississippi: A Statistical Analysis.

Abstract

This study addresses major identifying characteristics of the poor in Mississippi. These include problems which are apparent from a study of census data and other secondary materials, such as family composition, family size and occupation, and socioeconomic and demographic attributes. Mississippi has changed from a one crop economy to a diversified economy. Mississippi has the largest proportion of poor families and persons of any State and poverty is more frequent among persons over the age of 65, persons living alone, black families, female headed families, and rural residents. Mississippi also has the largest proportion of school age children in poverty. Poverty is more severe among blacks than whites. Mississippi has the lowest per capita income, but it has been increasing in both relative and absolute terms. Levels of formal education attained by the population have rapidly improved except for blacks. Much remains to be accomplished in the provision of adequate housing. Ameliorative steps suggested include policies relating to migration, selective placement programs, taxation, educational opportunity, employment opportunities, retirement plans, and minimum wage legislation. Three analytical models are presented as a means of assisting action agencies to identify and interpret data in terms of the role of particular agencies and what this means with respect to their resources. Abstract

We report an analysis of the interstellar γ-ray emission from the Chamaeleon, R Coronae Australis (R CrA), and Cepheus and Polaris flare regions with the Fermi Large Area Telescope. They are among the nearest molecular cloud complexes, within ~300 pc from the solar system. The γ-ray emission produced by interactions of cosmic rays (CRs) and interstellar gas in those molecular clouds is useful to study the CR densities and distributions of molecular gas close to the solar system. The obtained γ-ray emissivities above 250 MeV are (5.9 ± 0.1stat +0.9 –1.0sys) × 10^–27 photons s^–1 sr^–1 H-atom–1, (10.2 ± 0.4stat +1.2 –1.7sys) × 10^–27 photons s^–1 sr^–1 H-atom^–1, and (9.1 ± 0.3stat +1.5 –0.6sys) × 10^–27 photons s^–1 sr^–1 H-atom^–1 for the Chamaeleon, R CrA, and Cepheus and Polaris flare regions, respectively. Whereas the energy dependences of the emissivities agree well with that predicted from direct CR observations at the Earth, the measured emissivities from 250 MeV to 10 GeV indicate a variation of the CR density by ~20% in the neighborhood of the solar system, even if we consider systematic uncertainties. The molecular mass calibrating ratio, X CO = N(H2)/W CO, is found to be (0.96 ± 0.06stat +0.15 –0.12sys) × 10^20 H2-molecule cm^–2 (K km s^–1)^–1, (0.99 ± 0.08stat +0.18 –0.10sys) × 10^20 H2-molecule cm^–2 (K km s^–1)^–1, and (0.63 ± 0.02stat +0.09 –0.07sys) × 10^20 H2-molecule cm^–2 (K km s^–1)^–1 for the Chamaeleon, R CrA, and Cepheus and Polaris flare regions, respectively, suggesting a variation of X CO in the vicinity of the solar system. From the obtained values of X CO, the masses of molecular gas traced by W CO in the Chamaeleon, R CrA, and Cepheus and Polaris flare regions are estimated to be ~5 × 10^3 M ☉, ~10^3 M ☉, and ~3.3 × 10^4 M ☉, respectively. A comparable amount of gas not traced well by standard H I and CO surveys is found in the regions investigated.

Abstract

An efficient approach for construction of physically justified STO(II)-3Gmag family basis sets for calculation of molecules magnetic properties has been proposed. The procedure of construction based upon the taken into account the second order of perturbation theory in the magnetic field case. Analytical form of correction functions has been obtained using the closed representation of the Green functions by the solution of nonhomogeneous Schrödinger equation for the model problem of "one-electron atom in the external uniform magnetic field". Their performance has been evaluated for the DFT level calculations carried out with a number of functionals. The test calculations of magnetic susceptibility and 1H nuclear magnetic shielding tensors demonstrated a good agreement of the calculated values with the experimental data., Предложен эффективный способ построения физически адаптированных базисных наборов STO(II)-3Gmag для расчета магнитных свойств молекул. Способ построения основан на учете явных выражений для поправочных функций второго порядка теории возмущений по магнитному полю к базисным атомным орбиталям. Аналитический вид поправочных функций получен путем использования замкнутого представления функции Грина при решении неоднородного уравнения Шредингера для модельной задачи "одноэлектронный атом во внешнем однородном магнитном поле". Эффективность этих базисов продемонстрирована в расчетах магнитной восприимчивости и тензоров магнитного экранирования ядер 1H для ряда молекул. Расчеты проведены в рамках DFT с применением различных функционалов., Запропоновано ефективний спосіб побудови фізично адаптованих базисних наборів STO(II)-3Gmag для розрахунків магнітних властивостей молекул. Спосіб побудови ґрунтується на врахуванні явних виразів для корекційних функцій другого порядку теорії збуджень по магнітному полю до базисних атомних орбіталей. Аналітичний вигляд корекційних функцій отримано шляхом застосування замкнутої форми функції Гріна у ході розв’язання неоднорідного рівняння Шредінгера для модельної задачі "одноелектронний атом у зовнішньому однорідному магнітному полі". Ефективність цих базисів продемонстровано на прикладі розрахунків магнітної сприйнятливості й тензорів магнітного екранування ядер 1Н для ряду молекул. Розрахунки здійснено в межах DFT із застосуванням різних функціоналів.

Abstract

Microquasar Spotted Microquasars are binary star systems where a normal star sheds matter onto a neutron star or a black hole, generating x-ray radiation and jets of material moving at relativistic speeds. Microquasars have proved difficult to detect in high-energy gamma rays (> 100 megaelectron volts). Using the Fermi Large Area Telescope, Abdo et al. (p. 1512 , published online 26 November; see the Perspective by Bignami ) now report the detection of variable gamma-ray emission from the microquasar Cygnus X-3. The gamma-ray flux is modulated at the orbital period of Cygnus X-3, and its variation is correlated with the radio emission originating from the microquasar's relativistic jets.

Abstract

Hemangiomas involving the spleen are rare and seldom symptomatic. Treatment options for large lesions usually consist of splenectomy, embolization, or both. Antiangiogenic treatment has not been reported previously as an effective alternative for this type of lesion. We report our experience of successfully using glucocorticoids in an infant with a large hemangioma of the spleen.

37. High prevalence of non-albicans yeasts and detection of anti-fungal resistance in the oral flora of patients with advanced cancer.

Abstract

Oral fungal infections frequently develop in individuals with advanced cancer. This study examined the oral mycological flora of 207 patients receiving palliative care for advanced malignant disease. Demographic details and a clinical history were documented from each participant. A tongue swab was collected and cultured on CHROMAgar Candida (CHROMAgar Paris, France). All yeasts were identified by germ tube test, API ID 32C profiles and, for Candida dubliniensis, by species-specific PCR. Susceptibility to fluconazole and itraconazole was determined by a broth microdilution assay according to the National Committee for Clinical Laboratory Standards (NCCLS). At time of sampling, 54 (26%) of the 207 subjects had clinical evidence of a fungal infection and yeasts were isolated from 139 (67%) individuals. In total, 194 yeasts were isolated, of which 95 (49%) were Candida albicans. There was a high prevalence of Candidia glabrata (47 isolates) of which 34 (72%) were resistant to both fluconazole and itraconazole. All nine isolates of C. dubliniensis recovered were susceptible to both azoles. No relationship was established between anti-fungal usage in the preceding three months and the presence of azole resistant yeasts. This study of patients with advanced cancer has demonstrated a high incidence of oral colonization with non-C. albicans yeasts, many of which had reduced susceptibility to fluconazole and itraconazole. The role of improved oral care regimes and novel anti-fungal drugs merits further attention, to reduce the occurrence of fungal infection in these patients. [ABSTRACT FROM AUTHOR]

Abstract

With the increasing prevalence of short-term psychiatric hospitalization, brought about in part by reduced inpatient insurance coverage, decisions must be made concerning both the nature of treatment and who will provide services. The impracticality of traditional rehabilitative services in these settings has serious implications for the future role of occupational therapists in psychiatric health care. It is proposed that necessary modifications to treatment approaches must and can be made without abandoning fundamental theories of practice. Recommendations are made to upgrade the current level of practice via improved communication of information concerning short-term methodologies, to develop long-range plans to strengthen professionalism, and to make greater use of community-based alternatives for the delivery of services no longer practical in short-term treatment settings.

Topics covered include the decreasing length of inpatient hospitalization; the possibility of a shifting emphasis lo the treatmcnt of the severely and chronically ill; increasing utilization of rehabilitative services; and suggestions to facilitate greater interprofessional collaboration. Thc establishment of measurable outcomes for continued or improved reimbursement of occupational therapy services is stressed, and the potential for multiprofession group practices as a cost-effective approach to reduced business and industry health care costs is suggested.

Abstract

An acoustic scheme for buried object detection is thought to involve a sound source above the ground and a microphone as a receiver. In the simplest scenario, an airborne acoustic pulse would be transmitted, strike the ground surface, propagate through the pores and reflect off the surface of a non-porous object. The microphone would then be used to detect the pore-fluid echo. These ideas were first considered in a sound tube with spherical beads used to porous medium. Experimental measurements of the pore-fluid propagation constants were compared to rigid framed model calculations for model verification. These experimental and theoretical results were then used to make predictions of the sound levels required to detect objects buried a few centimeters in outdoor soils. In addition, we chose a detection criteria which required that the pore- fluid echo be at least 1/2 cycle behind the pulse reflected from the ground surface. This criteria is quite arbitrary and conservative, but it allows for source specifications and from an experimental point of view appears as a realistic criteria for detection. The models allowed for prediction of reflection losses, propagation losses, phase velocities and source levels based on burial depths of a few centimeters. Using these calculations various off-the- shelf acoustic sources were considered and it was realized that none were available to use existing sound sources to demonstrate that non-porous objects could be detected a few centimeters below the surface of institutional or grass soils. Research to develop a sound source which meets the original design criteria is on-going.

Abstract

Fort Chaffee, Arkansas, includes approximately one thousand three hundred (1,300) buildings, comprising approximately 5,000,000 square feet. Of this number, eighty-seven buildings are permanently heated and are used twelve months a year. There are an additional eighty-seven buildings utilized a portion of the year. It is extremely difficult to estimate current energy costs and anticipated savings for buildings where use changes so radically. We have developed a model utilizing all one hundred seventy-four buildings used during the year. Those buildings that are not used twelve months a year have an ill-determined occupancy schedule. We have estimated the hours of occupancy for each of these buildings and have included estimates of electric and natural gas consumption for each. Heat gain/heat loss calculation for each of the one hundred seventy-four buildings were provided earlier under separate cover.

This presentation talks about endangered species legislation. Because of habitat changes such as dams, agriculture, logging, and pollution in the rivers of the United States, many species of freshwater mussels have become extinct or are threatened with extinction. These factors bring the Endangered Species Act and freshwater mussels together. Presently there are 25 species of mussels listed as endangered under the Act. There are an additional 48 mussel species in the Southeast alone that are being considered as candidates for listing. Abstract

Fourteen sites on Mill, Sevenmile, and Indian creeks in Davidson and Williamson Counties, Tenn., were surveyed for crayfish in July 1985. Orconectes shoupi, considered for listing as Endangered by the US Fish and Wildlife Service, numerically dominated at 8 of the 14 locations and comprised 78 percent of the crayfish community. The remaining 22 percent of the crayfish community consisted of two species, O. minus and O. palacidus. At sites where quantitative collections were made, O. shoupi densities ranged for 0.6/sq m to 11.9 sq m. Orconectes shoupi was numerically dominant at the majority of sites in Mill Creek, whereas the other two species of orconectes were most common in Sevenmile Creek, a major tributary of Mill Creek. When O. shoupi dominates the community, it had a greater proportion of larger sized individuals than when it was subdominant to the other two species of Orconectes. Length-frequency analysis for this species indicated an early cohort (17 to 25 mm), a late chohort (maximum size 15 mm), and a group larger than 25 mm, which probably had overwintered from the previous year Orconectes shoupi is a nocturnal, aggressive crayfish that is densest at sites with sufficient cover consisting of slab rock for large individuals or cobble or gravel for juveniles. Results of this survey indicated that, although this species has been reported only from well-oxygenated, sediment-free water, it is tolerant of suspended and settled sediment and eutrophic conditions.

Abstract

Two copies of a photolithograph, dated April 17, 1906, of an 1834 Mississippi land survey. The area of land depicted is proximal to the Pearl River, and was part of the Choctaw Land District. The map is gridlined and subdivided into 36 sections, each of which represents approximately one square mile. Copy 2 of the map contains a section in the bottom-left quadrant that has been shaded in red and labelled "#4"., In 1830, the Choctaw Nation and the US Government signed the Treaty of Dancing Rabbit Creek, which was the first treaty made under the Indian Removal Act. The Choctaw agreed to forfeit all of their remaining land in the state of Mississippi in exchange for land in the Oklahoma Territory. Under the terms of the treaty, the Choctaw who removed to Oklahoma would retain their sovereign status, while those who stayed in Mississippi could become US citizens and be granted 640 acres of land.

To identify common variants contributing to normal variation in two specific domains of cognitive functioning, we conducted a genome-wide association study (GWAS) of executive functioning and information processing speed in non-demented older adults from the CHARGE (Cohorts for Heart and Aging Research in Genomic Epidemiology) consortium. Neuropsychological testing was available for 5429-32 070 subjects of European ancestry aged 45 years or older, free of dementia and clinical stroke at the time of cognitive testing from 20 cohorts in the discovery phase. We analyzed performance on the Trail Making Test parts A and B, the Letter Digit Substitution Test (LDST), the Digit Symbol Substitution Task (DSST), semantic and phonemic fluency tests, and the Stroop Color and Word Test. Replication was sought in 1311-21860 subjects from 20 independent cohorts. A significant association was observed in the discovery cohorts for the single-nucleotide polymorphism (SNP) rs17518584 (discovery P-value=3.12 × 10(-8)) and in the joint discovery and replication meta-analysis (P-value=3.28 × 10(-9) after adjustment for age, gender and education) in an intron of the gene cell adhesion molecule 2 (CADM2) for performance on the LDST/DSST. Rs17518584 is located about 170 kb upstream of the transcription start site of the major transcript for the CADM2 gene, but is within an intron of a variant transcript that includes an alternative first exon. The variant is associated with expression of CADM2 in the cingulate cortex (P-value=4 × 10(-4)). The protein encoded by CADM2 is involved in glutamate signaling (P-value=7.22 × 10(-15)), gamma-aminobutyric acid (GABA) transport (P-value=1.36 × 10(-11)) and neuron cell-cell adhesion (P-value=1.48 × 10(-13)). Our findings suggest that genetic variation in the CADM2 gene is associated with individual differences in information processing speed.
