Research was conducted to examine the fourfold coordinate nature of N in chemical vapor deposited (CVD) diamond and tetrahedral amorphous carbon (ta-C). Measurements were based on a combination of density-functional methods at several levels of approximation used on supercell and cluster models. Results reveal that when the Fermi level, Ef falls below A1, N doping charge is transferred to the available deeper lying states.

Males rats were fed diets containing wheat bran, oat bran, barley or malted barley without or with 10 g/kg cholesterol + 1 g/kg cholic acid (referred to as 'cholesterol'). Plasma total, HDL, and VLDL + LDL cholesterol concentrations were higher overall in rats fed cholesterol. There was a significant interaction between dietary cholesterol and cereal type. Higher concentrations of total and VLDL + LDL cholesterol were found in rats fed cholesterol with wheat bran than in those fed oat bran, barley or malted barley. HDL cholesterol concentrations were higher overall in rats fed wheat bran than in those fed oat bran or barley but not malted barley. Liver cholesterol pools were higher overall in rats fed cholesterol. In all animals fed oat bran, liver cholesterol was lower than in rats fed barley or malted barley. Hepatic HDL receptor activity was lower overall in rats fed cholesterol. There was no independent effect of cereal type on HDL receptor activity, but there was a significant interaction with dietary cholesterol. Activity tended to be higher in rats fed malted barley with cholesterol than in rats fed malted barley without cholesterol. LDL receptor activity was not affected by cereal type but was significantly lower overall in rats fed cholesterol. Aims Ketamine analgesia is limited by low intrinsic efficacy compounded by large interindividual variability in drug responses, possibly due to the heterogeneity in drug concentration. The CYP2B6*6 allele is associated with substantially reduced ketamine metabolism in vitro and, therefore, may affect ketamine clearance. Our aims were to examine the impact of the CYP2B6*6 allele on ketamine plasma clearance and on adverse effects in chronic pain patients. Methods CYP2B6 genotypes were identified in 49 chronic pain patients who received 24h continuous subcutaneous infusions of ketamine. Steady-state plasma concentrations of ketamine (Css,k) and norketamine (Css,nk) were determined using HPLC. Results The median plasma clearance of ketamine after 100mg 24h-1 dose was significantly lower in patients with the CYP2B6*6/*6 (21.6l h-1) and CYP2B6*1/*6 (40.6l h-1) genotypes compared with patients with the CYP2B6*1/*1 genotype (68.1l h-1, P<0.001). The ketamine: norketamine plasma metabolic ratio was significantly higher in patients with the CYP2B6*6/*6 genotype than in those with the CYP2B6*1/*6 and the CYP2B6*1/*1 genotypes (P<0.001). Patients who experienced adverse effects had lower plasma clearance (45.6l h-1) than those who did not (52.6l h-1, P=0.04). The CYP2B6*6 genotype and age, and their combined impact explained 40%, 30% and 60% of the variation in Css,k, respectively. Similar results were observed after higher doses. Conclusions The CYP2B6*6 allele is associated with a substantial decrease in steady-state ketamine plasma clearance in chronic pain patients. The decreased clearance and resultant higher plasma concentrations may be associated with a higher incidence of ketamine adverse effects.Copyright © 2015 The British Pharmacological Society.

Contains fulltext : 108210.pdf (Publisher’s version ) (Open Access) Many genes with crucial roles in zinc homeostasis in mammals respond to fluctuating zinc supply through unknown mechanisms, and uncovering these mechanisms is essential to understanding the process at cellular and systemic levels. We detected zinc-dependent binding of a zinc-induced protein to a specific sequence, the zinc transcriptional regulatory element (ZTRE), in the SLC30A5 (zinc transporter ZnT5) promoter and showed that substitution of the ZTRE abrogated the repression of a reporter gene in response to zinc. We identified the ZTRE in other genes, including (through an unbiased search) the CBWD genes and (through targeted analysis) in multiple members of the SLC30 family, including SLC30A10, which is repressed by zinc. The function of the CBWD genes is currently unknown, but roles for homologs in metal homeostasis are being uncovered in bacteria. We demonstrated that CBWD genes are repressed by zinc and that substitution of the ZTRE in SLC30A10 and CBWD promoter-reporter constructs abrogates this response. Other metals did not affect expression of the transcriptional regulator, binding to the ZTRE or promoter-driven reporter gene expression. These findings provide the basis for elucidating how regulation of a network of genes through this novel mechanism contributes to zinc homeostasis and how the cell orchestrates this response.

At least 10% of patients with cancer have pain that is refractory to systemic analgesics. For most of these patients, interventional techniques may be of benefit but are often not considered or are difficult to access. Of these techniques, spinal analgesia is most commonly used in Australia and the United Kingdom, and neurosurgical procedures, such as open cordotomy with sectioning of the spinothalamic tract, are rarely used. We describe a case illustrating the successful use of bilateral open thoracic cordotomy in a patient with refractory mixed nociceptive and neuropathic pain secondary to a lumbosacral tumor. We discuss the various interventional options and review the recent literature regarding the use of both percutaneous and open cordotomy for cancer pain. © 2010 U.S. Cancer Pain Relief Committee.

Contains fulltext : 57294.pdf (publisher's version ) (Closed access), Breakthrough pain (BKP) is a transitory flare of pain that occurs on a background of relatively well controlled baseline pain. Previous surveys have found that BKP is highly prevalent among patients with cancer pain and predicts more severe pain, pain-related distress and functional impairment, and relatively poor quality of life. An international group of investigators assembled by a task force of the International Association for the Study of Pain (IASP) evaluated the prevalence and characteristics of BKP as part of a prospective, cross-sectional survey of cancer pain. Fifty-eight clinicians in 24 countries evaluated a total of 1095 patients with cancer pain using patient-rated items from the Brief Pain Inventory (BPI) and observer-rated measures. The observer-rated information included demographic and tumor-related data, the occurrence of BKP, and responses on checklists of pain syndromes and pathophysiologies. The clinicians reported BKP in 64.8% of patients. Physicians from English-speaking countries were significantly more likely to report BKP than other physicians. BKP was associated with higher pain scores and functional interference on the BPI. Multivariate analysis showed an independent association of BKP with the presence of more than one pain, a vertebral pain syndrome, pain due to plexopathy, and English-speaking country. These data confirm the high prevalence of BKP, its association with more severe pain and functional impairment, and its relationship to specific cancer pain syndromes. Further studies are needed to characterize subtypes of BKP. The uneven distribution of BKP reporting across pain specialists from different countries suggests that more standardized methods for diagnosing BKP are needed.

31. Opioid substitution to reduce adverse effects in cancer pain management.

Objective: To assess the effect of opioid substitution (substituting one member of the opioid class for another) on the incidence and severity of adverse effects in palliative-care patients who experience unacceptable, refractory adverse effects when taking an opioid drug. Design(s): Prospective audit of clinical records. Setting(s): Palliative Care Program, Monash Medical Centre, Melbourne, Victoria (comprising an acute 10-bed palliative care unit and a palliative care consultative service). Patient(s): All palliative care inpatients and patients referred to the consultative service who underwent opioid substitution between February 1996 and June 1997. Outcome measures: Adverse effects and pain control profiles before and after opioid substitution. Result(s): 55 opioid substitutions were undertaken in 49 patients. 49 substitutions were for adverse effects. The substitution produced partial or complete relief from confusion in 18 of 25 cases, from nausea and vomiting in 13 of 19, and from drowsiness in 8 of 15. Substitution also abolished myoclonus associated with renal failure in one patient. Conclusion(s): We found that the incidence and severity of adverse effects differed between opioids in the same patient. A trial of opioid substitution (usually in consultation with a specialist pain or palliative care unit and with auditing of outcomes) should be considered for patients with intolerable, refractory adverse effects of opioids.

We report on a case of listeriosis in a patient who probably consumed a prepackaged romaine lettuce–containing product recalled for Listeria monocytogenescontamination. Although definitive epidemiological information demonstrating exposure to the specific recalled product was lacking, the patient reported consumption of a prepackaged romaine lettuce–containing product of either the recalled brand or a different brand. A multinational investigation found that patient and food isolates from the recalled product were indistinguishable by pulsed-field gel electrophoresis and were highly related by whole genome sequencing, differing by four alleles by whole genome multilocus sequence typing and by five high-quality single nucleotide polymorphisms, suggesting a common source. To our knowledge, this is the first time prepackaged lettuce has been identified as a likely source for listeriosis. This investigation highlights the power of whole genome sequencing, as well as the continued need for timely and thorough epidemiological exposure data to identify sources of foodborne infections.

The present investigation is concerned with the preparation and characterization of sol-gel derived lead zircon ate titanate (PZT) powders and films. Particular emphasis is placed on the synthesis, processing and characterization of thin films. The chemistry employed involves the use of alkoxides of Ti/Zr and lead acetate trihydrate. PZT gels were prepared and characterized insights gained were used as the basis for the synthesis and characterization of thin films. A model was also proposed to augment further understanding of the PZT capacitors obtained. PZT thin films were successfully prepared on various substrates such as platinized Si wafers and Corning 7059 glass. Numerous electrical and optical characterizations were performed, namely dielectric constant and loss, hysteresis loop, switching, fatigue, aging, leakage currents, refractive index, UV transmission spectroscopy, second harmonic generation (SHG) and waveguide loss. These electrical and optical properties are discussed in conjunction with film microstructure and phase assemblage. Very high quality films were obtained (e.g., dielectric constant as high as 3000 at 1kHz, and fatigue-free beyond 10⁸ cycles and optical loss as low as 1.1 dB/em). Aging of these films can be kept as low as l%/decade. It was found that the domains play an important role in determining the dielectric properties. A model of the Pt-PZT-Pt capacitor was successfully developed based on totally depleted back-to-back Schottky barriers and the model predictions agree extremely well with measured device characteristics. This model also explains the different dielectric behaviours of FE films compared to those of bulk ceramics. By tailoring the chemistry and controlling the post-deposition processing conditions and hence microstructures, the properties (both electrical and optical) of the PZT films can be effectively engineered.

45. Multistate Outbreak of Listeria monocytogenesAssociated with Mexican-Style Cheese Made from Pasteurized Milk among Pregnant, Hispanic Women

- Abstract
Listeriosis is a severe infection caused by Listeria monocytogenes. Since 2004, the Centers for Disease Control and Prevention has requested that listeriosis patients be interviewed using a standardized Listeria Initiative (LI) questionnaire. In January 2009, states and the Centers for Disease Control and Prevention began investigating a multistate outbreak of listeriosis among pregnant, Hispanic women. We defined a case as an illness occurring between October 2008 and March 2009 with an L. monocytogenesisolate indistinguishable from the outbreak strain by pulsed-field gel electrophoresis. We conducted a multistate case-control study using controls that were selected from L. monocytogenesillnesses in non-outbreak-related pregnant, Hispanic women that were reported to the LI during 2004 to 2008. Eight cases in five states were identified. Seven of these were pregnant, Hispanic females aged 21 to 43 years, and one was a 3-year-old Hispanic girl, who was excluded from the study. Seven (100%) cases but only 26 (60%) of 43 controls had consumed Mexican-style cheese in the month before illness (odds ratio, 5.89; 95% confidence interval, 1.07 to ∞; P= 0.04). Cultures of asadero cheese made from pasteurized milk collected at a manufacturing facility during routine sampling by the Michigan Department of Agriculture on 23 February 2009 yielded the outbreak strain, leading to a recall of cheeses produced in the plant. Recalled product was traced to stores where at least three of the women had purchased cheese. This investigation highlights the usefulness of routine product sampling for identifying contaminated foods, of pulsed-field gel electrophoresis analysis to detect multistate outbreaks, and of the LI for providing timely exposure information for case-control analyses. Recalls of contaminated cheeses likely prevented additional illnesses.

8 páginas, 3 figuras, We show the distribution of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) genetic clades over time and between countries and outline potential genomic surveillance objectives. We applied three genomic nomenclature systems to all sequence data from the World Health Organization European Region available until 10 July 2020. We highlight the importance of real-time sequencing and data dissemination in a pandemic situation, compare the nomenclatures and lay a foundation for future European genomic surveillance of SARS-CoV-2., We gratefully acknowledge the authors, originating and submitting laboratories of the sequences from GISAID’s EpiCoV Database used in the phylogenetic analysis.
