Hidradenitis suppurativa is a chronic, recurrent, inflammatory skin disease that affects hair follicles, typically developing after puberty. It is characterized by painful, deep-seated inflammatory lesions that appear in anatomical regions where apocrine glands are located, mainly in the axillar, inguinal, and anogenital areas. The pathogenesis of hidradenitis suppurativa is multifactorial, with follicular occlusion representing the initial stage of the disease and subsequently leading to the formation of neutrophilic abscesses and infiltration by various inflammatory cells. The treatment of hidradenitis suppurativa requires a multidisciplinary approach, involving dermatologists and dermatologic surgeons, as well as physicians of other medical specialties. The primary pharmacological treatments for hidradenitis suppurativa involve antibiotics and biologic disease-modifying drugs, which can be used either in monotherapy or in combination with surgical interventions. The following guidelines provide up-to-date information on the management of patients with hidradenitis suppurativa.

Primary cutaneous lymphomas are a heterogeneous group of rare lymphoproliferative diseases that are initially limited to the skin but potentially also involve the lymph nodes, blood, and occasionally visceral organs. They develop from mature T cells, B cells, and NK cells. The classification includes a wide range of pathologies, each characterized by distinct clinical symptoms and variations in disease course. Cutaneous lymphomas are typically chronic in nature, and a complete cure is rarely achievable. Since primary cutaneous lymphomas are uncommon and present in many variants, they pose diagnostic and therapeutic challenges that require multidisciplinary collaboration between dermatologists, pathologists, hematologists, and oncologists. We present revised guidelines for the diagnosis and treatment of primary cutaneous lymphomas, with the goal of simplifying the diagnostic and therapeutic procedure in routine clinical practice. The guidelines cover the clinical, histopathological, and dermoscopic characteristics of skin lesions, along with recommendations for the diagnostic and therapeutic management of primary cutaneous lymphomas. When selecting the most suitable therapeutic approach for individual patients diagnosed with primary cutaneous lymphoma, the main factor to consider should be the clinical stage of the disease. However, other factors, such as the patient’s age, overall health, and medical history, should also be taken into account. The choice of therapy should be guided by finding optimal efficacy and safety. The guidelines also include globally recognized therapeutic approaches that are currently not accessible in Poland. The recommendations rely on the existing Polish and global guidelines, and the knowledge and experience of experts.

Acne vulgaris is the most common dermatosis among adolescents, but it can also affect adults. Acne lesions located on the face cause discomfort and lead to impaired quality of life for many patients. Acne is a dermatological condition with multiple causes and clinical manifestations, as well as varying degrees of severity. Dermatological treatment alone does not always bring good therapeutic effects, and occasionally it may result in symptoms of skin irritation. Therefore, using appropriate dermocosmetics is crucial in managing the condition. Dermocosmetics play a potentially important role in minimizing the adverse effects of acne medications, providing a synergistic effect by addressing other acne-causing factors and/or enhancing the efficacy of other treatment modalities, and protecting the skin against the adverse impacts of the exposome. In cases of mild acne, dermocosmetics can be used in monotherapy as an alternative to acne medications or for maintenance therapy. It is also advisable to combine dermocosmetics with topical or systemic therapies to achieve a synergistic effect or to prevent and reduce drug-induced adverse reactions. Dermocosmetics have keratolytic and anti-inflammatory properties, regulate sebum production, and help maintain the balance of the skin’s natural microbiome.

Dermocosmetics (active cosmetics, functional cosmetics, cosmeceuticals) are preparations for external use, supporting pharmacotherapy of skin diseases during the active phase of the disease and in remission. They are also used for healthy skin (preventive) care. Historically, topical skin care products have been divided into cosmetics and drugs, as defined by the Federal Food, Drug and Cosmetic Act of 1938. A cosmetic was defined as ”articles intended to cleanse, care, beautify and improve the appearance of the skin”. Drugs were defined as ”products intended to affect the structure or any function of the body, or products intended for use in the diagnosis, treatment, mitigation, treatment or prevention of human diseases”. Dermocosmetics (cosmeceuticals) remain cosmetics from a legal point of view, but they fill the gap between cosmetics and drugs. These products achieve a cosmetic effect with active substances however are not subject to drug regulations.

Rosacea is a chronic inflammatory skin disease characterised by periods of remission and subsequent relapses typically provoked by environmental factors. Eruptions located on the face are easily visible and may be difficult to conceal, which leads to a decrease in patients’ self-esteem, quality of life, and general well-being. Since the aetiology of rosacea is multifactorial and not entirely understood, and the clinical characteristics and course of the disease are heterogeneous, therapy may be challenging. While a complete cure is not possible, in the majority of cases clinical improvement and long-term remission are satisfying both to the patient and physician. In recent years, the therapeutic approach has been significantly modified. Currently, the focus is placed on the need to personalise the treatment process and adapt it to the patients’ phenotypic characteristics. Combination therapies and new variants of existing topical and systemic drugs are increasingly being recommended. The mainstays of therapy are antiinflammatory agents. Proper skin care – along with the identification and avoidance of factors known to trigger consecutive episodes of rosacea – are also essential.

Rosacea is a chronic, relapsing, inflammatory, but not infectious, skin disease. The prevalence of rosacea is relatively high in the adult population of Northern Europe. Its characteristic symptoms include recurrent flushing, erythema, telangiectasia, papules and pustules, localized on cheeks, nose, chin and forehead. At present, four, not mutually exclusive subtypes of acne are classified: erythematotelangiectatic, maculopapular, hypertrophic and ocular. The clinical presentation of this dermatosis is heterogeneous. Clinical symptoms and their severity may vary individually. For this reason, treatment should be individualized and take into consideration the type of cutaneous lesions, presence of comorbidities, patient’s occupation, impact of the disease on the quality of life, and patient’s preferences. In addition to topical and systemic medications, adequate skin care is important.

Psoriasis is a chronic inflammatory skin disease which concerns approx. 1-3% population worldwide. In children psoriasis is less common, with the peak of morbidity in teenagers. Because of its more common prevelance in children in last decade, especially the disease with moderate and severe course, it is necessary to remind its clinical features and comorbidities. The aim of the recommendation of Polish Dermatological Society was to give the newest data on the psoriasis treatment in pediatric population which may be extremely important for everyday dermatological treatment. We described both topical and systemic treatments in details, including biological therapies.

Psoriasis vulgaris is a chronic, immune-mediated disorder, which has a substantial impact on all aspects of patients’ quality of life. In most patients, the disease is mild to moderate and is successfully treated with topical agents. The most common therapy involves a vitamin D3 analogue (calcipotriene) in combination with a synthetic corticosteroid (betamethasone dipropionate). The aerosol vehicle (foam) with softening properties is another formulation of this combination drug, apart from ointment and gel, expanding the therapeutic options available to patients with psoriasis. The article describes the pharmacokinetic and pharmacodynamic properties of calcipotriene/betamethasone dipropionate foam. The results of the key randomised clinical studies investigating the efficacy, including patients’ quality of life and safety of the foam versus ointment, gel and either active ingredient in foam vehicle are presented. In addition, the results of a study on maintenance treatment with calcipotriene/betamethasone dipropionate foam as well as reports on real-world use of this medicine in patients with psoriasis, are discussed.

Introduction Therapeutic recommendations for hidradenitis suppurativa (HS) have recently shifted towards non-invasive pharmacological options. Recent evidence has shown promising efficacy for specific treatments. However, data regarding the comparative efficacy of these treatments in patients with HS are still limited. Therefore, we plan to conduct a systematic review and network meta-analysis (NMA) to summarise the benefits and harms of different pharmacological interventions for treating people living with HS.Methods and analysis We will search electronic databases, including Medline, Embase, PubMed, Web of Science, Scopus, CINAHL and Cochrane Library beginning from their inception dates with no language restrictions. A grey literature search will be performed to supplement the electronic databases. Both randomised trials and non-randomised studies using validated measurement tools that investigated the benefits and harms of pharmacological interventions among people living with HS will be included. The predefined primary outcomes will include treatment responses that reflect the patient’s perspective and all-cause discontinuation. Screening, selection, extraction, assessment of the risk of bias and analysis of the strength of the evidence will be performed independently by a pair of reviewers. A two-step approach of traditional pairwise and NMA will be performed. Based on a random-effects model, standardised weighted mean differences and ORs with corresponding 95% CIs will be pooled as effect estimates for the continuous and categorical endpoints, respectively. Statistical and methodological heterogeneities will be assessed. Preplanned subgroup analyses and univariate meta-regression will be conducted to quantify the potential sources of heterogeneity. Evidence-based synthesis will be based on the magnitudes of effect size, evidence certainty and the surface under the cumulative ranking curve values.Ethics and dissemination Ethical approval is not required because this study is based on existing published data. These findings will be disseminated through scientific meetings and publications in peer-reviewed journals.PROSPERO registration number CRD42022302795.

Granuloma annulare (GA) is a fairly common granulomatous disease characterized by painless, erythematous, annular papules with necrobiosis and granuloma formation in histopathology. There are four clinically distinct subtypes: localized, subcutaneous, generalized and perforating. The first two subtypes primarily occur in childhood, particularly in the first 5–6 years of life [1].

The basis for development of atopic dermatitis are genetic, immunological and microbiological abnormalities as well as epidermal barrier defects. The course of the disease includes periodic exacerbations and remissions. Its development and course are influenced by numerous environmental and individual factors. In recent decades, in industrialized countries, a threefold increase in the incidence of atopic dermatitis was observed. There also is an increasing number of cases resistant to topical treatment. Effective treatment of atopic dermatitis should provide control of clinical symptoms, prevent exacerbations and improve the quality of life of patients. The multifactorial etiopathogenesis and various endotypes and phenotypes of the disease justify the tendency to optimize and personalize the therapy. Currently, we recommend the use of dupilumab for the treatment of patients from 12 years of age with moderate and severe atopic dermatitis, who do not respond to topical treatment.

Abstract Recombinant interferon beta‐1b is one of the treatment options of multiple sclerosis (MS). Insertional biologics that are used in the treatment of MS may lead to skin adverse effects, for example, morphea.

The treatment goal in atopic dermatitis is eliminating clinical symptoms of the disease, preventing exacerbations and complications, as well as improving patients’ quality of life. In cases of severe atopic dermatitis and lack of response it is recommended to introduce systemic therapy. Patients ofter require multi-specialist consultations, and occasionally hospitalization. It is not recommended to use acupuncture, acupressure, bioresonance, homeopathy, or Chinese herbs in the treatment of atopic dermatitis.

Psoriasis is one of the most common chronic inflammatory dermatoses. We present the second part of the diagnostic and therapeutic guidelines on psoriasis. The second part discusses the principles of treatment of plaque psoriasis, with a particular emphasis on systemic drugs. The second part of the guidelines constitutes an integral whole with the first part which addresses the general principles of diagnostic management, prevention of cardiovascular complications, therapeutic goals, and selected specific forms of psoriasis. The guidelines have been updated to take account of access to new therapies which have recently been approved in Poland, ongoing improvements in the efficacy of available therapeutic methods, and changing needs of patients. We hope that the current guidelines will assist physicians in making diagnostic and therapeutic decisions, and contribute to the improvement of medical care provided to patients with psoriasis.

Psoriasis is a chronic inflammatory disease affecting approximately 1–3% of the general population. Although skin lesions are the primary clinical manifestation of the condition, psoriasis – especially moderate to severe – is currently regarded as a systemic disease, not only associated with the development of psoriatic arthritis but also linked to a range of other health consequences including cardiovascular complications. In view of new data on the pathogenesis of psoriasis, very rapid progress in the development of new therapeutic approaches, and in particular the registration of new drugs, a need arose to update the guidelines for the diagnosis and treatment of this nosological entity. The aim of the recommendations of the Polish Dermatological Society is to summarise the most recent literature and formulate guidelines for the treatment of psoriasis that may be useful in daily dermatological practice. However, the final decision regarding the diagnostic and therapeutic procedure should always be made individually for each patient based on the patient’s current clinical status and the most up-to-date scientific reports.

Atopic dermatitis-patients are predisposed to have bacterial, viral and fungal skin infections; they also suffer from an increased risk of developing food allergies (especially, at an infantile age), allergic rhinitis, or bronchial asthma (a so-called atopic march). Currently, an increasing atopic dermatitis incidence constitutes a serious medical problem that regards not only dermatology and allergology, but also paediatrics, and family medicine. The basis for atopic dermatitis treatment and prophylaxis is restoration of epidermal barrier functions by means of tailored emollients. Atopic dermatitis therapies should effectively eliminate clinical symptoms of the disease, prevent exacerbations as well as complications, and improve patients’ quality of life.

Classic dermatomyositis (DM) is an idiopathic inflammatory myopathy characterized by symmetric, proximal muscle weakness and a wide range of cutaneous findings. Its pathogenesis is not fully understood, however, it is believed that immunogenetic and probably other factors have a significant impact on the manifestation of the disease. Classically, patients exhibit proximal skeletal muscle weakness accompanied by cutaneous symptoms i.e. heliotrope eruption on the eyelids, shawl sign, Gottron sign. Scrotum involvement is an unusual finding in DM. To the best of our knowledge, scrotal ulcerations accompanied by severe pain have been extremely rarely reported as the first dominant manifestation of the disease.
