5. Biventricular versus Left ventricular pacing in cardiac Resynchronization therapy.

- Abstract
Cardiac resynchronization therapy (CRT) improves systolic function in heart failure patients with ventricular conduction delay by stimulating the left ventricle (LV) or both ventricles (biventricular, BIV). Optimal LV site selection is of major clinical interest for CRT device implantation. However, little is known about LV versus BIV pacing. Thus, the objective of this study was to compare electrical and echographic parameters before and after CRT and to compare BiV and LV pacing via echocardiographic parameters. LV and BIV pacing were tested sequentially in 20 patients (mean age 54 ± 13 years, 80% males) with pre-existing biventricular devices. Initial BIV configuration was switched to LV configuration or vice versa, and echocardiographic images were acquired for analysis. Regional and global LV longitudinal mechanics were assessed with 2D speckle-tracking echocardiography. A total of 20 patients (mean QRS interval before resynchronization: 161.17 ± 7, mean LV ejection fraction (LVEF) before resynchronization: 27.9 ± 8.9) enrolled in this study. CRT was administered with LV and BV stimulation in DDD mode. LV stimulation was at the lateral free wall, whereas right ventricular stimulation was fixed near the apex. Different echographic parameters were compared for LV to BIV pacing: LVEF, diastolic function, and LV global longitudinal strain. There was no significant difference in the echographic study of the left ventricle between LV pacing and BIV pacing (BIV LVEF: 43.33 ± 12.48% VS LV LVEF: 42.57 ± 14.32%, P = 0.64; BIV LGS: -11.00 ± 4.51% vs LV LGS: -11.05 ± 4.02, P =.925); time to peak between opposite walls (septal and lateral) (53.23 ± 68.89 vs 153.3 ± [MOU1] 158.7, P = 0.118). Understanding the nuanced disparities in clinical outcomes between male and female patients undergoing percutaneous coronary intervention (PCI) for unprotected left main coronary artery disease (CAD) is pivotal in refining patient care and treatment protocols. To determine the sex-specific outcomes in left main PCI. We analysed data from 213 patients (48 females; 165 males) with unprotected left main CAD who underwent PCI between January 2012 and January 2023 in our catheterization lab. The primary endpoint was a composite of all-cause mortality, myocardial infarction, or stroke. Median follow-up duration was 2.16 years. Although women tended to be older and had higher rates of diabetes mellitus and hypertension, these distinctions were not statistically significant. Men more frequently presented with left main bifurcation lesions and extensive CAD. Baseline findings revealed that wall motion abnormalities were more prevalent in men compared to women (68.3% vs. 50%, respectively; p = 0.020). Notably, the presence of wall motion abnormalities emerged as a significant predictor of adverse outcomes, including total mortality (19.2% vs. 5.4%; p = 0.007) and cardiac death (16.2% vs. 2.7%; p = 0.003). However, the primary endpoint did not exhibit a statistically significant difference between genders (20.8% vs. 24.8%, respectively; p = 0.569). Similarly, the requirement for target lesion revascularization was comparable in both groups (7.9% vs. 12.5%, respectively; p = 0.566). Despite variations in clinical and lesion characteristics, female and male patients demonstrated similar long-term outcomes post-PCI for left main CAD. [ABSTRACT FROM AUTHOR]

13. Prognostic value of lung ultrasound in acute heart failure.

- Abstract
Clinical congestion rather than low cardiac output, is the most frequent cause in patients hospitalized for heart failure and accounts for high rates of death. Persisting symptoms and signs of congestion at discharge or amongst out-patients are strong predictors of an adverse outcome. Lung ultrasound (LUS) has emerged as a simple, non-invasive and semi-quantitative tool for the detection and the assessment of pulmonary congestion. Assess the prognostic importance of pulmonary congestion detected with LUS on short-term adverse events and to describe the dynamic changes of LUS findings. In a bi-centric prospective observational study, we included consecutive patients hospitalized for acute heart failure. Each patient underwent a thorough clinical examination, a biological evaluation, a chest X-ray, LUS and echocardiography. The operators performing LUS were blinded to clinical data and examined 8 thoracic zones.The main clinical outcomes were a composite of urgent visit, HF hospitalization for acute decompensation of HF or cardiac death. A total of 116 individuals were included (median, 69 years of age; 53% men; mean ejection fraction 40%) The mean number of B-lines at discharge was 6.6 ± 3.3. During a mean follow-up of 6 months, we detected 72 events: 52 patients were admitted due to severe HF symptoms and 20 patients died from cardiac cause. By multivariate analysis, ≥ 3 B-lines Bilaterally (11.024 HR, 95% CI 5.542–21.926, P < 0.001) was retained as a predictor for the risk of the combined endpoint at 180-day follow-up. The number of B-lines significantly decreased from 31.9 ± 12.7 when patients were admitted at the hospital to 6.6 ± 3.1 when discharged from it (P = 0). A simplified 8-Zone LUS method is useful to assess severity and monitor the resolution of lung congestion. Patients with persistent pulmonary congestion at discharge assessed by LUS have worse prognosis. [ABSTRACT FROM AUTHOR]

Climate change caused drought stress to become a critical challenge in maize (Zea mays L.) production, surpassing other environmental stresses. Understanding drought resilience mechanisms in maize is essential for breeding drought-tolerant varieties. This study evaluated the drought tolerance of two maize hybrids, Sancia and Agrister, under greenhouse conditions with four irrigation regimes: 90%, 75%, 50%, and 25% of field capacity (FC). Under severe drought (25% FC), the hybrid Agrister showed reductions in shoot length, leaf area, biomass, and chlorophyll content by 13.9%, 29.86%, 30.76%, and 13.7%, respectively, compared with well-watered conditions (90% FC). In both hybrids, electrolytic conductivity increased under drought, indicating membrane stress. Agrister, however, maintained lower levels of hydrogen peroxide (H2O2) than Sancia, signaling a better antioxidative balance. A higher catalase and peroxidase enzyme activity marked Agrister's response to drought, alongside increased phenolic compounds, strengthening its antioxidant defense system. The study concludes that Agrister's superior drought tolerance makes it a promising candidate for cultivation in Tunisia's arid regions, highlighting its potential in breeding programs focused on resilience to water scarcity. 24. Transposition of great arteries in developing world: Early mortality and predictive factors.

- Abstract
Transposition of great arteries (TGA) is a prevalent congenital heart defect presenting a diagnostic and therapeutic emergency. Its immediate prognosis is currently much improved by initial palliative treatment. However, in the absence of early complementary surgical repair, the prognosis remains extremely poor, with a high rate of mortality. In this study, we sought to investigate the preoperative in-hospital mortality of newborns with TGA born in Sfax in Tunisia, a developing country with limited resources where surgical repair is not possible and most children undergo surgery abroad. Monocentric retrospective analysis of all newborns with TGA hospitalized in CHU Hedi Chaker of Sfax in Tunisia during 2014–2022. Infants with simple TGA (± small VSD) and with complex TGA (large VSD, pulmonary stenosis or aortic coarctation) were included. Factors associated with preoperative in-hospital mortality were analyzed. The study included 51 patients divided into two groups: group I, 42 cases of simple TGA and group II, 9 cases with complex TGA. The overall preoperative mortality was 19.6% (10 of 51 patients) and it did not differ significantly between the two groups (group I 21.4% vs. group II 11%, P = 0.667). The mean age of death was 19 days (IQR = [8–44]). The mean time of death after diagnosis of TGA was 11 days with the earliest death occurring within the first 24 hours after diagnosis and the latest 36 days later. Major hypoxemia and infections were main causes of death, responsible of 90% of all deaths. Early mortality was significantly associated with a lower birth weight (2885 vs. 3317; P = 0.014) with a higher rate in female patients. On the other hand, age of discovery (P = 0.429) and antenatal diagnosis of TGA (P = 0.537) were not significantly associated with preoperative mortality. However, this is statistically insignificant result due to the limited prenatal detection (6%). No ultrasound predictive factor was identified with a higher rate of early death in the presence of incongruence between the pulmonary artery and the aorta (27.3% vs. 17.5%, P = 0.47) and in cases of banana-like LV shape (33.3% vs. 18.8%, P = 0.537). Failed balloon atrial septostomy was significantly associated with early mortality (75% vs. 16%; P = 0.028). In our study, lower birth weight and insufficient interatrial shunt were independent factors predicting preoperative early death in TGA patients. [ABSTRACT FROM AUTHOR]

Introduction: Both exercise and pre-meal metformin could lower postprandial glucose and lipid profiles. Aims: To explore whether pre-meal metformin administration is superior to metformin administration with the meal in reducing postprandial lipid and glucose metabolism, and whether its combination with exercise confer superior benefits in metabolic syndrome patients. Materials and Methods: In a randomized crossover design, 15 metabolic syndrome patients were assigned to 6 sequences including 3 experimental conditions: metformin administration with a test meal (met-meal), metformin administration 30 min prior to a test meal (pre-meal-met) with or without an exercise bout designed to expend 700 Kcal at 60% VO 2 peak performed the evening just before pre-meal-met condition. Only 13 participants (3 males, 10 females; age: 46 ± 9.86, HbA1c: 6.23 ± 0.36) were included in the final analysis. Results: Postprandial triglyceridemia was unaffected by any condition (all P > .05). However, both pre-meal-met (−7.1%, P = .009) and pre-meal-metx (−8.2%, P = .013) significantly reduced total cholesterol AUC with no significant differences between the two latter condition ( P = .616). Similarly, LDL-cholesterol levels were significantly lower during both pre-meal-met (−10.1%, P = .013) and pre-meal-metx (−10.7%, P = .021) compared to met-meal with no difference between latter conditions ( P = .822). Plasma glucose AUC was significantly reduced by pre-meal-metx compared to both pre-meal-met (−7.5%, P = .045) and met-meal (−8%, P = .03). Insulin AUC was significantly lower during pre-meal-metx compared to met-meal (−36.4%, P = .044). Conclusions: Metformin administration 30 minutes prior to meal seems to exert favorable effects on postprandial TC and LDL-Cholesterol levels compared to its administration with meal. Addition of one exercise bout only improved postprandial glycemia and insulinemia. Trial registry: Pan African clinical trial registry, Identifier PACTR202203690920424.

Neurofibromatosis type 1 (NF1) is a neurocutaneous condition with an autosomal dominant pattern of inheritance. This congenital disease is characterized by a wide spectrum of clinical manifestations and degree of severity. This case report describes a female patient in her early 20s who presented with a complaint of lumbosciatica-like pain evolving for several months. The condition initially escaped the attention of clinicians until a lumbar computed tomography scan and spinal magnetic resonance imaging were performed. The patient was then transferred to the general surgery department, where a clinical diagnosis of NF1 was established. The clinical manifestations were specific for this disease, including café-au-lait macules, plexiform neurofibroma, and a history of neurofibromatosis in her mother. The patient underwent surgical resection of the neurofibroma, which resulted in a favorable outcome. However, 2 years later, a new mass attached to the second lumbar spinal nerve was revealed by a follow-up computed tomography scan. Long-term and close follow-up of NF1 is required because of the high risk of malignancy and recurrence in NF1 patients.

Aims To understand the mode of action of thyme essential oil as an alternative biofungicide. Methods and Results The chemical composition of thyme essential oil isolated by hydrodistillation from the aerial parts of Thymus vulgaris was analyzed. The main constituents of thyme essential oil were thymol (76·96%), ρ-cymene (9·89%), γ-terpinene (1·92%) and caryophyllene oxide (1·69%). The antifungal activity of the oil and its pure major component (thymol) was assessed by the in vitro assay against Mycosphaerella graminicola. Thyme oil exhibited higher antifungal activity than thymol. The expression pattern of genes involved in fungal development and detoxification acting in M. graminicola under thyme oil and thymol treatment was analyzed. Thyme oil overexpressed, more than thymol, the genes encoding for the efflux pump ( MgMfs1, MgAtr4), the regulatory subunit of protein kinase A ( PKA) ( MgBcy1) and the MAPK MgHog1. Thyme oil repressed the expression of the genes encoding for the efflux pump MgAtr4, the MAPK ( MgSlt2) and the regulatory subunit of PKA ( MgBcy1). However, thymol repressed only MgAtr4 and MgSlt2 expression. Conclusions These data highlight the ability of thyme oil to target genes involved in fungal development and virulence of the yeast-like fungi M. graminicola, which explain its higher antifungal activity. Significance and Impact of the Study These findings will probably be useful to design an alternative biofungicide which will not lead to pathogen multidrug resistance. [ABSTRACT FROM AUTHOR]

- Abstract
Introduction: The increase of left auricular volume (LAV) is a robust cardiovascular event predictor. Despite that echochardiography is more often used, cardiac MRI is considered more accurate. Our objetives are to validate "fast" LAV measures by MRI vs the considered gold standard (GS) and to compare Echo and MRI in a wide spectrum of patients. Methods: In a non-selected popullation with MRI study previously realized, we measured LAV by biplane method (BPMR) and by area-length in 4 chamber view (ALMR) and compared them with biplane (BPe) and discs method (MDDe) in 4 chamber view in echo. To validate MRI measurements, we measured LAV in short axis slices (Simpson Method, SM) in a group of patients and considered it the GS. Results: 186 patients were included (mean age 51 ± 17 age; 123 male; 14 in AF) with clinical indication of cardiac MRI (Philips 1,5 T). In 24 patients SM was calculated. 29% of cardiac MRI were considered normal. Mean underlying pathologies were myocardiopathy (27%), Ischemic myocardiopathy (17%), myopericarditis (10%), prior to AF ablation (4%), valvular disease (6%) and miscellaneous (7%). Excellent correlation was obtained between "fast" MRI measurements and SM in MRI (SM vs BPMR interclass correlation coefficient ICC=0.965 and SM vs ALMR, ICC=0.958; P<0.05) with low interobserver variability (ICC=0.983 for SM; ICC=0.949 for BPMR; ICC=0.931 for ALMR). "Fast" measurements by MRI showed stadistical correlation between them (CCI=0.910) (Figure). Correlation between Echo and MRI measures was only moderate. (BPRM vs BPe CCI=0,469 mean difference -30 ml; ALMR vs MDDe ICC=0,456 mean difference -24 mL). Conclusions: ‘fast’ LAV measures by MRI are comparable with the MRI GS and also between them. Echo values seem to underestimate compared to MRI, so its use may not be suitable.

- Abstract
Purpose: We have previously demonstrated that multi-line transmit (MLT) beam forming can provide high quality full field-of-view (90° sector) B-mode images at very high frame rates, i.e. up to 500 fps. The purpose of this study was to test the feasibility of this technique in imaging the mechanical intraventricular waves such as the one associated with activation of the left ventricle. Methods: A dedicated pulse sequence using MLT was implemented on the ULA-OP research scanner equipped with a 2.0 MHz phased array to obtain 90° sector images at a frame rate of 436 fps. The left ventricle of a healthy volunteer was imaged from the apical 4 chamber view and the RF data was acquired. Subsequently, the strain rate was extracted from the RF data using a normalized cross-correlation method. Results: As expected, during the early filling phase, myocardium lengthening (positive strain rate) was observed propagating from the base of the septum to the apex and back (Figure a). A similar wave was detected in the lateral wall, although a brief shortening (negative strain rate) was detected in the mid-wall which could be the result of reverberations (Figure b). During isovolumetric contraction, the septal wall shortened before the lateral wall (as expected) - moreover - there seemed to be an intra-wall base-apex shortening gradient (Figure c and d). Conclusions: Our preliminary results show that visualization of the cardiac mechanical activation could be feasible using MLT based high frame rate imaging. Further research is required to examine this in depth, which is the topic of on-going work.

- Abstract
Purpose: Although delayed-enhancement magnetic resonance imaging (DEMRI) is essential for diagnosis of cardiac sarcoidosis (CS), the test was not available when pacemaker was implamted. Recently, MR-conditional pacemaker has become avilable and we hypothesized that this device would be useful for diagnosis and management of CS. The aim of this study was to assess the diagnostic ability of MR-conditional pacemaker about CS in patients with advanced A-V nodal block (AAVB). Methods: Twenty-seven AAVB patients (14 men, 13 women; mean age, 69 ± 11 years) who were implanted MR-conditional pacemaker were studied. DEMRI was performed 6 weeks after implantation of permanent pacemaker. In patients with positive for DE, additional examinations like echocardiography, radioisotope imaging, biopsy, and coronary computed-tomography were performed due to confirm the diagnosis of CS and exclude coronary artery disease. Results: DE was observed in 12 patients (44 %). Out of 12 patients, 2 patients were excluded for having prior myocardial infarction. Seven of 10 (70 %) patients were diagnosed of CS by the consensus criteria. Compared with non-CS group, CS group had significantly lower age (61 ± 12 years vs. 72 ± 9 years p = 0.017). There was no significant difference about sex, angiotensin-converting enzyme, brain natriuretic peptide, and left ventricular ejection fraction between 2 groups. Six patients had started corticosteroid therapy and 5 patients (83%) recovered A-V nodal conduction. Conclusion: MR-conditional pacemaker was useful for diagnosis and management of patients with AAVB caused by CS.

42. Clinical Profile and Management of Wolf-Parkinson White Syndrome and Asymptomatic Pre-Excitation Carriers in Africa: A Multicenter Pan African Registry From 17 Countries.

- Abstract
Background and Aim: Data are scarce regarding Wolf-Parkinson White Syndrome (WPW) syndrome and asymptomatic pre-excitation in Africa. This study tried to understand the current approaches utilized in Africa for the diagnosis and management of both symptomatic and asymptomatic ventricular pre-excitation., Methods: The current prospective study was conducted in 20 centers located in 17 countries spanning all areas of Africa. Participants included had ventricular pre-excitation patterns. The data collected included symptoms, locations of accessory pathways (AP) and tachyarrhythmia, risk stratification, as well as acute and long-term management. In addition, we assessed the clinical effectiveness of the centers and the impact of socioeconomic and health metrics on these treatment approaches., Results: Among 541 participants, 93% were diagnosed with WPW syndrome, with orthodromic atrioventricular reciprocating tachycardia (AVRT) being the most prevalent arrhythmia, affecting 55% of the cases. Approximately 30% of patients in Africa, except for the southern region, received intravenous amiodarone as the first-choice drug for orthodromic AVRT while adenosine was the drug of choice in the Southern region. Electrical cardioversion was the first-line treatment for pre-excited atrial fibrillation and antidromic AVRT across all of Africa. Radiofrequency ablation (RF) was the first long-term therapy option for 88% of patients in all African regions, unlike western and central Africa where it was implemented for less than 30% of patients (p < 0.001). The rates of RF success, long-term recurrence, and complications were 93%, 4.1%, and 3.8%, respectively., Conclusion: There are significant variations between African regions in terms of diagnostic workup, accessory pathway localizations, and volume of invasive treatment. Countries in Northern and Southern Africa are more advanced than other African countries in terms of modern management strategy. - Abstract
Background: One of the most prevalent causes of emergency room visits is acute dyspnea. Several etiologies, including cardiac, pulmonary, metabolic, psychogenic etc… may be involved. Acute heart failure (AHF) is among the most common causes. This study aims to evaluate, in patients presenting with acute dyspnea to the emergency departement (ED), the accuracy of a diagnostic approach combining Lung ultrasonography (LUS) and clinical assessment as compared to the traditional AHF diagnostic work-up., Methods: This is a bi-centric cross-sectional observational study, conducted at the Emergency and Cardiology Department of both the Hedi Chaker Hospital in Sfax and the Habib Thameur Hospital in Tunis for the period extending from 01/07/2022 to 30/09/2023. The diagnostic performance of pulmonary ultrasonography was studied and the sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were compared with those of clinical examination, chest X-Ray, NT-pro -BNP and the Transthoracic echocardiography (TTE) which was the reference exam., Results: The most common cause of acute dyspnea is acute heart failure (79.3 %). LUS had a sensitivity of 94,2 % in diagnosing AHF and a specificity of 77,5 %. Its PPV and NPV were respectively 92 % and 81 %. The area under curbe (AUC) of B-Lines required for the diagnosis of interstitial pulmonary syndrome was excellent (92 %). There was a moderate significant positive correlation between the number of B-Lines and NT-Pro-BNP levels r = 0.51, P < 0.001. Also, there was a very strong significant positive relationship between the pulmonary congestion assessed by LUS and Left atrium - pressure r = 0.788, P < 0.001 CONCLUSION: LUS is an excellent test both to confirm and exclude the diagnosis of AHF in patients consulting the emergency room for acute dyspnea and therefore deserves to be performed systematically., Competing Interests: Declaration of competing interest The authors declare that they have no known competing financial interests or personal relationships that could have appeared to influence the work reported in this paper., (Copyright © 2024 Elsevier Inc. 44. Rethinking the Terminology: A Perspective on Renaming Polycystic Ovary Syndrome for an Enhanced Pathophysiological Understanding.

- Abstract
Polycystic Ovary Syndrome (PCOS) is a complex endocrine disorder that affects women at various stages of life, presenting a wide range of symptoms and health implications. The term "Polycystic Ovary Syndrome" can be misleading, prompting many within the medical community and advocacy groups to advocate for a name change. Critics argue that this terminology can complicate understanding and awareness of the disease among patients. The primary concern is that PCOS emphasizes the ovarian aspect, fostering the misconception that PCOS is merely a gynecological disorder. In reality, PCOS impacts multiple organ systems, particularly metabolic health. Patients frequently experience insulin resistance, weight gain, irregular menstrual cycles, and hirsutism-symptoms that extend beyond ovarian dysfunction. In light of these issues, there is increasing support for renaming PCOS to better reflect its systemic implications and minimize confusion. The current name may hinder understanding and potentially lead to inadequate disease management. Alternative names have been proposed, including "Ovarian Dysmetabolic Syndrome," which our team supports, as well as "Metabolic Reproductive Syndrome" and "Hyperandrogenic Persistent Ovulatory Dysfunction Syndrome." These alternatives aim to highlight the hormonal imbalances and metabolic disturbances associated with the condition, fostering inclusivity and reducing stigma for all affected individuals. This narrative review provides a historical overview of PCOS, tracing its recognition from early descriptions to contemporary guidelines. We discuss the evolving understanding of its pathophysiology and the rationale behind the proposed name change. - Abstract
Long COVID-19, also known as post-acute sequelae of SARS-CoV-2 infection, is a condition where individuals who have recovered from the acute phase of COVID-19 continue to experience a range of symptoms for weeks or even months afterward. While it was initially thought to primarily affect the respiratory system, it has become clear that Long COVID-19 can involve various organs and systems, including the endocrine system, which includes the pituitary gland. In the context of Long COVID-19, there is a growing understanding of the potential implications for the pituitary gland. The virus can directly affect the pituitary gland, leading to abnormalities in hormone production and regulation. This can result in symptoms such as fatigue, changes in appetite, and mood disturbances. Long COVID-19, the persistent and often debilitating condition following acute COVID-19 infection, may be explained by deficiencies in ACTH and Growth hormone production from the pituitary gland. Corticotropin insufficiency can result in the dysregulation of the body's stress response and can lead to prolonged feelings of stress, fatigue, and mood disturbances in Long COVID-19 patients. Simultaneously, somatotropin insufficiency can affect growth, muscle function, and energy metabolism, potentially causing symptoms such as muscle weakness, exercise intolerance, and changes in body composition. Recently, some authors have suggested the involvement of the pituitary gland in Post COVID-19 Syndrome. The exact mechanisms of viral action on infected cells remain under discussion, but inflammatory and autoimmune mechanisms are primarily implicated. The aim of our study will be to review the main pituitary complications following COVID-19 infection. Moreover, we will explain the possible involvement of the pituitary gland in the persistence of Post COVID-19 Syndrome.

We have evaluates the genotoxicity of H2 in vivo in rats after 72 h exposure, following the International Council for Harmonization guidelines ICH S2 (R1). The study was conducted on three groups of male Wistar rats: a negative control group, a positive control group receiving methyl methanesulfonate, and a H2 -treated group receiving a 3.1% H2 gas mixture for 72 h. Alkaline comet, formamidopyrimidine DNA glycosylase (Fpg)-modified comet and bone marrow micronucleus assays were performed. H2 exposure increased neither comet-tail DNA intensity (DNA damage) nor frequency of "hedgehogs" in blood, liver, lungs, or bronchoalveolar lavage fluid. No increase in Fpg-sensitive sites in lungs, no induction of micronucleus formation, and no imbalance of immature erythrocyte to total erythrocyte ratio (IME%) was observed in rats exposed to H2 . - Abstract
Celiacomesenteric trunk (CMT) refers to the common origin of celiac trunk and superior mesenteric artery which is a very rare anatomical variation. CMT is incidentally diagnosed during angiography or abdominal computed tomography scanning. The diagnosis of CMT may inform surgical practice and prevent damage during invasive radiologic procedures, lowering thus the rate of iatrogenic errors. Complete common mesentery is in its turn a rarer congenital anomaly that arises from an abnormal rotation of primitive small intestine during embryonic development. We report a case of a 48-year-old man, suffering from chronic abdominal pain, and postprandial discomfort. The patient underwent an abdominal contrast-enhanced computed tomography that detected a CMT associated with common complete mesentery. According to our review of bibliography, this is the first case report to simultaneously report both congenital anomalies (CMT and common complete mesentery). Furthermore, the CMT described here has not been described in previous classifications and represent a novel anatomical variation of CMT.

Background: Radiofrequency catheter ablation is considered to be a relatively safe procedure. This is an unusual case report in which severe mitral regurgitation was occurred after left lateral accessory pathway radiofrequency catheter ablation., Case Presentation: A 15-year-old man without structural heart disease was referred for ablation of a left lateral accessory pathway. He was a rugby player who had lived with Wolff-Parkinson-White syndrome since 2017. In 2017, two failed extensive radiofrequency catheter ablations of a left lateral accessory pathway had been performed in another center. In June 2018, he underwent a third radiofrequency catheter ablation of a left lateral accessory pathway using an anterograde transseptal approach with an early recurrence one month later. A successful fourth procedure was performed in August 2018 using a retrograde aortic approach. Three months later, the patient presented to the hospital with atypical chest pain and dyspnea on exertion. Transthoracic echocardiography revealed severe mitral regurgitation caused by a perforation of the posterior leaflet. Given the symptoms and the severity of the mitral valve regurgitation, the decision was taken to proceed with surgical intervention. Posterior mitral leaflet perforation was confirmed intraoperatively. The patient underwent video-assisted mitral valve repair via Minithoracotomy approach., Conclusion: This case demonstrates a very rare complication of Wolff-Parkinson-White radiofrequency ablation., (© 2022. 49. European experience with a first totally leadless cardiac resynchronization therapy pacemaker system.

- Abstract
Aims: Totally leadless cardiac resynchronization therapy (CRT) can be delivered with a combination of Micra and WiSE-CRT systems. We describe the technical feasibility and first insights into the safety and efficacy of this combination in European experience., Methods and Results: Patients enrolled had indication for both Micra and WiSE-CRT systems because of heart failure related to high burden of pacing by a Micra necessitating system upgrade or inability to implant a conventional CRT system because of infectious or anatomical conditions. The endpoints of the study were technical success of WiSE-CRT implantation with right ventricle-synchonized CRT delivery, acute QRS duration reduction, and freedom from procedure-related major adverse events. All eight WiSE-CRT devices were able to detect the Micra pacing output and to be trained to deliver synchronous LV endocardial pacing. Acute QRS reduction following WiSE-CRT implantation was observed in all eight patients (mean QRS 204.38 ± 30.26 vs. 137.5 ± 24.75 mS, P = 0.012). Seven patients reached 6 months of follow-up. At 6 months after WiSE-CRT implantation, there was a significant increase in LV ejection fraction (28.43 ± 8.01% vs. 39.71 ± 11.89%; P = 0.018) but no evidence of LV reverse remodelling or improvement in New York Heart Association class., Conclusion: The Micra and the WiSE-CRT systems can successfully operate together to deliver total leadless CRT to a patient. Moreover, the WiSE-CRT system provides the only means to upgrade the large population of Micra patients to CRT capability without replacing the Micra. The range of application of this combination could broaden in the future with the upcoming developments of leadless cardiac pacing., (© The Author(s) 2020. 50. A Novel Aspect of Essential Oils: Coating Seeds with Thyme Essential Oil induces Drought Resistance in Wheat.

Coating seeds with biostimulants is among the promising approaches in crop production to increase crop tolerance to drought stress. In this study, we evaluated the potential of coating durum wheat seeds of the cultivar 'Karim' with thyme essential oil on enhancing seed germination and seedling growth, and on plant growth promotion and induction of drought resistance. Coated seeds were pre-germinated, grown in hydroponics, and grown in pots under controlled well-watered and progressive water/nutrient stress conditions. Seed coating with thyme oil increased germination rate and enhanced seedling growth development in hydroponics. In the pot experiment, thyme oil increased, when well watered, root and shoot development, chlorophyll, nitrogen balance index (NBI), abscisic acid (ABA), anthocyanins and flavonoids in leaves, decreased nitrogen isotope composition (δ
15 N) and increased carbon isotope composition (δ13 C) of shoots. Increasing water/nutrient stress in control plants induced higher accumulation of ABA and anthocyanins coupled with a transient decrease in chlorophyll and NBI, a decrease in shoot and root development, the Normalized Difference Vegetation Index (NDVI), shoot C content, δ15 N, and an increase in δ13 C, revealing the avoidance strategy adopted by the cultivar. Thyme oil had the potential to enhance the avoidance strategy by inducing roots elongation, reducing the loss of shoot and roots dry matter and chlorophyll, maintaining balanced NBI, an decreasing anthocyanins, flavonoids, and δ13 C via maintaining lower ABA-mediated-stomatal closure. Thyme oil increased shoot N content and δ15 N indicating preferential uptake of the15 N enriched NH4 + . Coating seeds with thyme oil is suggested as a promising alternative approach to improve plant's water and nutrient status and to enhance drought resistance.- Published
