The presence of pesticides in fogwater plays a major role in accumulating relatively substantial levels of trace compounds due to their unique physico-chemical characteristics. The radiation wintertime fog in Alsace has been studied in the past few years (between 2015 and 2021) at four sites (Geispolsheim, Erstein, Strasbourg, and Cronenbourg). Fog samples are extracted using the liquid-liquid extraction (LLE) performed on the XTR Chromabond cartridge coupled with gas/liquid chromatography-tandem mass spectrometry (GC-MS/MS and LC-MS/MS). The samples are found to be contaminated by 25 semi- and non-volatile currently-used and previously-banned pesticides (like procymidone) and 16 organochlorine pesticides (OCPs) at notable levels and high detection frequency (DF). The analysis also reveals that Cronenbourg is the most contaminated site (31.5 ± 3.0 μg. L -1 ), followed by Erstein (23.1 ± 17.0 μg. L -1 ), Strasbourg (23.0 ± 3.5 μg. L -1 ), and Geispolsheim (22.8 ± 7.7 μg. L -1 ). Pearson and principal component analyses (PCA) prove the simultaneous application of fungicides, insecticides, and herbicides, and their atmospheric transport, mainly through west-southern air currents, from highly impacted sites to near-by urban and less impact sites (Strasbourg and Cronenbourg). The levels of OCPs are found at lower concentrations at all sites than other pesticides, of which dichlorodiphenyltrichloroethane (DDT) and its metabolites have the highest contribution (27%), while hexachlorobenzene (HCB) has the least contribution (3%). This paper assesses the concentrations, spatial distribution, compositional patterns, and sources of polycyclic aromatic hydrocarbons (PAHs) in the dissolved aqueous phase along the Abou Ali River course. The 16 priority PAHs, listed by the USEPA, were investigated in surface- and groundwater samples for 2 years starting August 2015 and ending in March 2017. Statistical analysis was done by using the ANOVA test at p < 0.05. The spatial distribution analysis and illustration were done using the ARC GIS software. The total PAHs concentration in surface and groundwater samples varied between not detected to 15.162 ng mL -1 and not detected to 0.635 ng mL -1 , respectively. The highest concentration of PAHs in surface water was observed at site S16, downstream of the river. However, the absence of PAHs was noticed at sites S1 and S2 of the upstream. The contamination levels of PAHs were found to be high in surface water samples, and low in groundwater samples. The 5- and 6-ring PAHs were the most abundant species among others in surface water samples, whereas the 3-ring PAHs were the most abundant in groundwater. Pyrogenic inputs deriving from fuel combustion, incineration, and miscellaneous burning were found to be the main PAH sources in surface and groundwater without ignoring the contribution of petrogenic inputs in some areas., (© 2021. The objective of this study is to assess the occurrence, spatial, and temporal distribution of forty-eight multiclass pesticides in surface and groundwater samples of the Abou Ali River, located in the North of Lebanon. A 3-year monitoring program (six batches from August 2015 to March 2017) was implemented, and thirty sampling points were selected along the river for analysis. The analysis was executed using a previously developed and optimized solid-phase micro-extraction (SPME) gas-chromatography-mass spectrometry (GC-MS) method. Statistical analysis, using Kolmogorov-Smirnov, Kruskal-Wallis, and Dunnet T3 multiple comparison tests, was applied to compare mean concentrations of pesticides between the different sampling sites and the batches taken. The pesticides that had the highest frequency of detection in the surface and groundwater samples were alachlor, α-endosulfan, and methomyl. For surface water samples, high mean concentrations were perceived for two stations in the upper stream (S5 and S7), two stations (S11 and S14) in the middle stream, and one station (S16) in the lower stream of the river. The highest mean concentrations were observed in October 2015 and August 2016, the time of the year which correlates with the period of pesticide application. Considering groundwater samples, high mean concentrations of pesticides were detected in sites G4, G9, G10, and G12 and during October 2015 and March 2016, following the rainy season. Ecotoxicological risk assessment using the risk quotient (RQ) methodology revealed high risk for five pesticides under average conditions and fourteen under extreme conditions. This study presents, for the first time, a statistical analysis showing the quantification of pesticides in the water resources of the Abou Ali River. In conclusion, it reveals the need to apply a complete pesticide monitoring program, not only for the Abou Ali River but for all the water resources in Lebanon.

Background: Acute lymphoblastic leukemia (ALL) is the most frequent childhood malignancy. Treatment has been unified in the middle of 1980 in the Czech Republic. In 2002-2007 children and adolescents with acute lymphoblastic leukemia were treated in an international randomized trial ALL-IC BFM 2002 in the Czech Republic. 291 patients aged 1-18 years were enrolled; infants below 1 year entered a separate trial., Methods and Results: Patients were stratified into three risk groups according to their age, initial leukocyte count, prednisone response, presence of fusion genes BCR/ABL or MLL/AF4, bone marrow D+15 and remission status D+33. The whole therapy took 24 months. Randomized late intensification compared standard BFM therapy with extended, usually more intensive experimental treatment. The median follow-up was 8.7 years. Complete remission was achieved in 97.9% patients, 1% died in remission. 11% of children relapsed, 1.7% with CNS involvement. Six children (2.1%) developed secondary malignancy. Event free survival (EFS) 8 years from diagnosis was 83.5%, overall survival (OS) 91.4%. EFS and OS of the risk groups were: standard risk: 89.4%; 98.1%; intermediate risk: 82.6%; 89.6%; high risk: 68.8%; 78.1%. Male sex and age above 10 years were adverse prognostic factors., Conclusions: In comparison with the previous trial ALL-BFM 95, significant improvement was achieved.

Purpose: From 2002 to 2007, the International Berlin-Frankfurt-Münster Study Group conducted a prospective randomized clinical trial (ALL IC-BFM 2002) for the management of childhood acute lymphoblastic leukemia (ALL) in 15 countries on three continents. The aim of this trial was to explore the impact of differential delayed intensification (DI) on outcome in all risk groups., Patients and Methods: For this trial, 5,060 eligible patients were divided into three risk groups according to age, WBC, early treatment response, and unfavorable genetic aberrations. DI was randomized as follows: standard risk (SR), two 4-week intensive elements (protocol III) versus one 7-week protocol II; intermediate risk (IR), protocol III × 3 versus protocol II × 1; high risk (HR), protocol III × 3 versus either protocol II × 2 (Associazione Italiana Ematologia Oncologia Pediatrica [AIEOP] option), or 3 HR blocks plus single protocol II (Berlin-Frankfurt-Münster [BFM] option)., Results: At 5 years, the probabilities of event-free survival and survival were 74% (± 1%) and 82% (± 1%) for all 5,060 eligible patients, 81% and 90% for the SR (n = 1,564), 75% and 83% for the IR (n = 2,650), and 55% and 62% for the HR (n = 846) groups, respectively. No improvement was accomplished by more intense and/or prolonged DI., Conclusion: The ALL IC-BFM 2002 trial is a good example of international collaboration in pediatric oncology. A wide platform of countries able to run randomized studies in ALL has been established. Although the alternative DI did not improve outcome compared with standard treatment and the overall results are worse than those achieved by longer established leukemia groups, the national results have generally improved.

Diamond-Blackfan anemia (DBA) is a congenital red blood cell aplasia that is usually diagnosed during early infancy. Apart from defects in red blood cell maturation, the disorder is also associated with various physical anomalies in 40% of patients. Mutations in the ribosomal protein (RP) S19 are found in 25% of patients, while mutations in other proteins of the small ribosomal subunit--RPS17 and RPS24--have been found in a fraction of patients. Recently, mutations in RPL5, RPL11, and RPL35a of the large ribosomal subunit have also been reported in several DBA patients. Here, we present the identification of mutations in the RPL5 and RPL11 genes in patients from the Czech DBA Registry. Mutations in RPL5 were identified in eight patients from 6 out of 28 families (21.4%), and mutations in RPL11 in two patients from 2 out of 28 families (7.1%). Interestingly, all 10 patients with either an RPL5 or RPL11 mutation exhibited one or more physical anomalies; specifically, thumb anomalies (flat thenar) were always present, while no such anomaly was observed in seven patients with an RPS19 mutation. Moreover, 9 out of 10 patients with either an RPL5 or RPL11 mutation were born small for gestational age (SGA) compared to 3 out of 7 patients from the RPS19-mutated group. These observations may suggest that mutations, at least in RPL5, seem to generally have more profound impact on fetal development than mutations in RPS19. Since RPL5 and RPL11, together with RPL23, are also involved in the MDM2-mediated p53 pathway regulation, we also screened the RPL23 gene for mutations; however, no mutations were identified., (2009 Wiley-Liss, Inc.)

Background: Acute myeloid leukemia (AML) in children is rare. Although more resistant to chemotherapy than acute lymphoblastic leukemia, its responsiveness and survival rates have considerably improved during the last 15 years by virtue of intensification of chemotherapy and due to the better supportive care. Relapses still remain the main cause of treatment failure. Management of children with AML was unified in the Czech Republic in 1993 according to AML-BFM 93 Study protocol., Methods and Results: Treatment results were evaluated in 61 patients, of whom 45 (73.8%) achieved complete remission. Five-year event-free-survival (EFS) was found in 42.3%, and overall survival was 45.3%. Prognosis of the standard-risk patients was significantly better than in the high-risk group (EFS 62.5% vs. 29.7%, p = 0.03). The most important prognostic factor was the early treatment response. Compared to chemotherapy, allogeneic stem-cell transplantation did not significantly improve the outcome of high-risk patients., Conclusions: Treatment results of children with AML in the Czech Republic are comparable to those achieved by leading leukemia study groups in the world. The aim of the next study is to increase the complete-remission rate by reducing early deaths.

Background: Prognosis of children with acute lymphoblastic leukaemia (ALL)--the most common cancer in childhood, has improved remarkably over the last 40 years. The authors report the treatment outcome in children with ALL cured according to ALL-BFM 90 Study protocol in the Czech Republic during the first half of nineties., Methods and Results: Children aged 0-18 years were included into the study in 10 centers between 1990 to 1996. Patients were classified into standard-risk (SR), medium-risk (MR) and high-risk (HR) group according to initial leukaemic burden, early treatment response, and genotype of leukaemia. Duration of the chemotherapy was two years. Treatment results were evaluated in 352 children. With a median follow-up of 7.3 years, event-free-survival (EFS) was 71.3% and overall survival 76.4%. EFS was 80.3%, 74% and 28.2% in SR, MR and HR group, respectively. Relapse was diagnosed in 17.8% of the patients., Conclusions: The treatment outcome of children with ALL improved significantly (p = 0.0045) compared to the previous study ALL-BFM 83 (EFS 62%). These results are comparable to those achieved by leading leukaemia study groups in the world.

The authors describe the disease of a 22-year-old woman treated from the age of 13 years on account of Hodgkin's lymphoma by irradiation and cytostatic treatment. On account of a relapse of lymphoma at the age of 14 years megachemotherapy with subsequent transplantation of autologous bone marrow. In the course of eight years of the follow up gradual development of constrictive pericarditis with exsudate. Concurrently progression of mitral insufficiency based on valvular prolapse resulting from radiation. During the last two years refractory systemic hypertension resistant to treatment. At the peak of the disease development of cardiac tamponade and cardiac cachexia with anasarca. After anamnestic, clinical and haemodynamic analysis total pericardetomy was indicated and mitral valve replacement performed. The operation led to improvement of the patient's condition, systemic hypertension receded completely. The patient is in permanent remission.
