Fifteen cases of acute renal failure following scorpion sting were studied. The onset of disease was characterized by the occurrence of hemoglobinuria within 24 h of the sting. Most of the patients developed oliguria, edema, hemolytic anemia, and hemolytic jaundice. Renal failure developed within a few days after the sting, and in five patients was severe enough to need dialysis. The onset of diuresis in oliguric patients occurred between 6 and 21 days following the sting. Renal biopsies were possible in four cases and showed mesangial proliferation, variable degrees of tubular changes, and mild interstitial infiltration. The pathogenesis of acute renal failure in these patients is discussed.

The diagnostic features and outcome of acute myocardial infarction in 100 very elderly (aged 85 years or more) hospitalized patients are reviewed and compared with those in a group of younger elderly (aged 65-84 years). The diagnostic triad of chest pain, sequential ECG changes and raised cardiac enzymes was present in only 24 very elderly patients and in 27 the diagnosis was not initially suspected. Presenting symptoms were often atypical and characteristic ECG changes could not be demonstrated in 25 patients. Very elderly patients had a higher mortality during the first few days in hospital, despite no greater incidence of cardiac failure and similar infarct size to the younger patients. Subsequent in-hospital mortality was similar in both groups. A higher index of suspicion of myocardial infarction in acutely ill very elderly patients should lead to earlier diagnosis, more appropriate management and may improve immediate prognosis.

It is commonly accepted that phosphatidylcholine, phosphatidylethanolamine and phosphatidylinositol, the three major phosphatide constituents of soya lecithin are insoluble in acetone, and that extraction of the soya lecithin with acetone dissolves out the triglyceride oil and leaves a precipitate of a mixture of these three phosphatides. Intimate molecular association between phosphatides and acetone appears to be responsible for this. This paper presents a mechanistic picture for this association, discusses the means for preventing this association, and thereby dissolving phosphatides into acetone. A prospect for dissolving phosphatidylethanolamine into acetone, preferentially over others from a mixture, is thus opened up. This is important since it has practical applications in industry. It has been exploited by the development of a process for the separation of phosphatidylcholine and phosphatidylinositol (mixture) from commercial soya lecithin. The process solves a long standing industrial problem for the preparation of phosphatidylethanolamine-free phosphatides. Verfahren zur Trennung von Phosphatid-Gemischen: Herstellung von phosphatidylathanolaminfreien Phosphatiden aus Sojalecithin Es ist bekannt, das Phosphatidylcholin, Phosphatidylathanolamin und Phosphatidylinosit, die drei Hauptphosphatidbestandteile von Sojalecithin, in Aceton unloslich sind; bei der Extraktion von Sojalecithin mit Aceton bleibt ein Ruckstand, der aus einer Mischung dieser drei Phosphatide besteht. Fur dieses Verhalten scheint eine molekulare Assoziation zwischen Phosphatiden und Aceton verantwortlich zu sein. In der vorliegenden Arbeit wird ein mechanistisches Bild fur diese Assoziation aufgezeigt; die Moglichkeiten zur Verhinderung der Assoziation und somit zum Losen von Phosphatiden in Aceton werden diskutiert. Es eroffnet sich damit die Aussicht, Phosphatidylathanolamin mit Aceton bevorzugt aus einem Gemisch herauszulosen. Dies ist fur die Industrie von praktischer Bedeutung. Das Verfahren ist bei der Entwicklung eines Prozesses fur die Abtrennung von Phosphatidylcholin und Phosphatidylinosit aus handelsublichem Sojalecithin erfolgreich angewendet worden. Der Prozes lost das seit langem bestehende Problem der industriellen Herstellung von phosphatidylathanolaminfreien Phosphatiden.

The clinical features and outcome were determined for 100 consecutive patients aged 65 years or older with a history of diabetes mellitus who presented to hospital with acute myocardial infarction. Each case was compared with an age- and sex-matched nondiabetic control also admitted to hospital with acute myocardial infarction. Chest pain was equally common in both groups and was the main presenting symptom. Cardiac failure was a more frequent accompaniment in the diabetics, despite the lack of evidence for greater infarct size in this group. The outcome was worst for female diabetics, of whom 46% died. Contrary to popular teaching, painless myocardial infarction is not a specific feature of elderly diabetics.
