Abstract

Much has been learned about the consequences of glucocorticoid receptor antagonism by studying steroidal active antagonists such as RU-38486 (1). In the liver glucocorticoid receptor antagonism suppresses hepatic glucose production decreasing plasma glucose levels; however, extrahepatic antagonism produces several undesirable side effects including activation of the hypothalamic pituitary adrenal axis. A series of nonsteroidal passive N-(3-dibenzylamino-2-alkyl-phenyl)-methanesulfonamide glucocorticoid receptor modulators was discovered. Liver selective and systemically available members of this series were found and characterized in diabetes and side effect rodent models. A highly liver selective member of this series, acid 14, shows efficacy in the ob/ob model of diabetes. It lowers plasma glucose, cholesterol, and free fatty acid concentrations and reduces the rate of body weight gain. The structurally related systemically available passive modulator 12 lowers glucose, HbA(1c), triglyceride, free fatty acid, and cholesterol levels. Interestingly, it did not acutely activate the hypothalamic pituitary adrenal axis in unstressed CD-1 mice or have the abortive effects observed with 1. These results indicate that passive GR antagonists may have utility as antidiabetic agents.

Abstract

A new air-sampling system, consisting of a mixture of magnesium silicate and an amine-trapping agent has been tested for the collection and concentration of airborne nitrosamines in atmospheres that also contain the precursors of these readily formed compounds. The new technique has been demonstrated to be resistant to in situ artifact formation and to quantitatively retain a variety of volatile nitrosamines. Two other solid sorbents, activated charcoal and Tenax-GC, were also compared for in situ formation of nitrosamines from added amines and air containing nitrogen oxides. The test conditions of preloaded amines and airborne nitrogen oxides were selected to simulate reported levels of these precursors in industrial atmospheres. Field data showing the reliability of the new technique in parallel studies with dilute (1 N potassium hydroxide) alkali impinger traps are also presented.

Abstract

Background: A questionnaire, modified from the posthospitalization behavioural questionnaire, was sent to all parents of children under 8 years of age who had elective surgery in the hospital of Lahr during the years 1997 and 1998. The parents were asked about long lasting changes (more than 1 week) in their child’s behaviour after surgery. Methods: The anaesthesia records of all these children were evaluated on the inhalational agent used (halothane or sevoflurane), the type of surgery, the sedative premedication used, the method of inducing anaesthesia and the use of opioids or nonopioids for pain therapy. Four hundred and fifty-eight of 863 parents responded (53.1%) to the questionnaire. Four hundred and twenty-seven children had minor ENT surgery and 31 minor urological surgery. Two hundred and eleven children received halothane and 214 sevoflurane as inhalational agent. Results: The parents of 34 of the 211 children (16.1%) receiving halothane and 55 of 214 children (25.7%) receiving sevoflurane reported postoperative behavioural changes. This difference was highly significant (P=0.015). Conclusions: We found that the children having sevoflurane anaesthesia were more likely to develop behavioural problems postoperatively than with halothane.The rate of postoperative behavioural changes was not influenced by the type of surgery, the sedative premedication used, the induction technique or the use of opioids or nonopioid pain therapy.

Abstract

Summary An oxygenated sterol was detected by chemical analysis of sheep thymus lipid extracts. It was characterized as 7β-hydroxycholesterol, which is well known for its biological activity. Using fluorescence microscopy, attempts were made to correlate this substance to particular histological structures of the thymus tissue. Microscopy revealed an intense yellow-green primary fluorescence distributed throughout the entire thymic parenchyma, which was predominant in the cortico-medullary junction, the vicinity of the HassalPs bodies and the subcapsular space. This indicated an accumulation of the fluorogenic substance in special tissue structures of sheep thymus. This result can be assumed as specific for the thymus, since such primary fluorescence was absent from other simultaneously investigated organs, such as the adrenal.

Abstract

The syntheses of 11- and 10-nitronoracronycine (10 and 11) from 1,3-dihydroxy-10-methyl-5-nitroacridinone (7) or 1,3-dihydroxy-10-methyl-6-nitroacridinone (8), respectively, and 2-chloro-2-methyl-3-butyne are reported. In screening tests with the transplantation tumor leukemia P 388 10, 11, 7 and 1,3-dimethoxy-10-methyl-6-nitroacridinone showed cytotoxicity. 8 and 1,3-dimethoxy-10-methyl-nitroacridinone (5) proved to be inactive.

The light sensitivity of corticosteroids in the solid state have been investigated and the photodecomposition rates of the compounds have been estimated by HPLC. After 48 h UV-irradiation desoxycortone acetate, hydrocortisone, hydrocortisone acetate, prednisolone, methylprednisolone, dexamethasone, betamethasone and triamcinolone acetonide decompose between 20 and 50%. Cortisone acetate, prednisone and fluocinolone acetonide show photostability in contrast to requirements of the Pharmacopeia for light protections.

Abstract

The title compounds were prepared by reacting [(4,5-diphenyl-1H-imidazol-2-yl)thio]acetic acid hydrazides 6 and 7 with aromatic aldehydes to give the corresponding hydrazones 6a-m and 7a-d or with isothiocyanates to afford the corresponding thiosemicarbazides 8a-c and 9. 8a-c and 9 were cyclized into triazoline-5-thiones 10a-c and 11 in the presence of sodium hydroxide or into 1,3,4-thiadiazoles 14a-c and 15 in the presence of sulfuric acid. On the other hand, 10c and 11 were also reacted with alkyl halides to obtain their thioether derivatives, 12a,b and 13. All the synthesized compounds were characterized by their elementary analysis and by using UV, IR, NMR and mass spectrometry. 6b, c, f, h-k, m, 7, 7d, 8c, 9, 10c, 11, 12a-b, 13, 14 and 15 were tested for antimicrobial activity, but no significant activity was observed.

Abstract

A sample of 19 spasmodic dysphonia (SD) patients was selected from a larger population of such patients to undergo magnetic resonance imaging (MRI), auditory brain stem response (ABR) testing, speech analysis, and extensive physical examination. Six patients had abnormal spin-echo MRI findings, ranging from infarcts within the basal ganglia to demyelinating lesions within the supralateral angles of the lateral ventricles. A weakly positive correlation was noted between the abnormal MRI findings and an abnormal ABR. The lack of a significant correlation between the MRI findings and other predictors of brain stem and midbrain disease, and the current spatial resolution limitations of MRI, suggest that we are visualizing the associated lesions rather than the actual foci of SD. The range of MRI findings is consistent with the concept that SD is a voice disorder in a heterogeneous patient population.

Volatile nitrosamines, N-nitrosodimethylamine (NDMA), N-nitrosomorpholine (NMOR), N-nitrosodiethylamine (NDEA) and possibly N-nitrosodibutylamine (NDBA), have been found as airborne contaminants inside new (1979 model) motor-cars. Abstract

(−)-Cyclopenine (I) and (−)-cyclopenol (II) are benzodiazepine derivatives formed by Penicillium viridicatum Westling and P. cyclopium Westling. They are easily transformed to the quinoline alkaloids viridicatine (III) and viridicatol (IV) respectively by a general acid catalysis or an enzyme of P. viridicatum called cyclopenase. Mass spectrometric investigations suggest that both benzodiazepine compounds may be converted to viridicatine and viridicatol by thermic degradation. In fact, by heating (−)-cyclopenine and (−)-cyclopenol in various solvents to 220–240°C they readily rearrange by loss of methyl isocyanate to viridicatine and viridicatol. Besides this major route of degradation there is a minor one giving rise to 3-methyl-3,4-dihydroquinazolone-(4), carbonmonoxide and benzaldehyde or m-hydroxybenzaldehyde. The pyrolysis presumably starts with the homolytic fission of the epoxide ring between the oxygen and either C-atom 10 or 3 and follows the route shown in Schemes 1 and 2. Degradation of biosynthetically labelled (−)-cyclopenol-2-14C, -3-14C and -5-14C supports the mechanisms proposed. It was possible to show that the carbonyl group of the methyl isocyanate derives from C (5) and the carbon monoxide from C (2) of the benzodiazepine ring. In the 3-methyl-3,4-dihydroquinazolone-(4) C-atoms 3 and 5, in the viridicatol C-atoms 2 and 3 of this ring remain (Table 1). By mass spectrometric degradation of (−)-cyclopenine and (−)-cyclopenol a set of peaks is obtained which corresponds to the molecular ions of viridicatine or viridicatol and their fragmentation products (Figs. 1–4). In addition there seem to be other fragmentation routes probably connected in part with a hydrogen shift from the side chain to the benzodiazepine moiety (Schemes 3 and 4), In spite of this 3-methyl-3,4-dihydroquinazolon-(4) may be formed in small quantities. Conversion of cyclopenine and cyclopenol to 3-methyl-1,2,3,4-tetrahydroquinazolindione-(2,4) was not detectable.

Analysis of human thymus steroids by liquid-gel chromatography and gas chromatography-mass spectrometry revealed in dependence of age 4-pregnene-3,20-dione, 11 beta-hydroxy-4-pregnene-3,20-dione, 7 beta-hydroxycholesterol, and 7-ketocholesterol. 11 beta-Hydroxy-pregnene-3,20-dione should be the inhibitor of immune maturation.
