1. Modulation of the intraseasonal variability in early summer precipitation in eastern China by the Quasi-Biennial Oscillation and the Madden–Julian Oscillation

Abstract

Using the reanalysis and multiple observations, the possible impact of the Madden–Julian Oscillation (MJO) on early summer (June–July) rainfall in eastern China and its modulation by the Quasi-Biennial Oscillation (QBO) are examined. The composite results show that the suppressed (enhanced) convection anomalies for MJO phases 8–1 (4–5) are more concentrated over the maritime continent and the western Pacific during easterly QBO (EQBO). As a consequence, more significant wet (dry) anomalies develop in South (eastern) China during MJO phases 8–1 (4–5) configured with easterly (westerly) QBO. The enhancement and expansion of the anomalous tropical convection band do not necessarily correspond to enhancement of the extratropical circulation response to MJO phases 8–1 (4–5) configured with westerly (easterly) QBO. The anomalous high (low) over the maritime continent and western Pacific associated with MJO phases 8–1 (4–5) is intensified (deepened) during easterly (westerly) QBO, leading to large southwesterly (northeasterly) anomalies in South China and the coasts, carrying abundant (sparse) moisture. Two anomalous meridional circulation cells are observed for MJO phases 8–1 in the East Asia sector, with downwelling anomalies around 5–20∘ N, upwelling anomalies around 20–30∘ N, and another downwelling branch northward of 30∘ N, which are enhanced during easterly QBO. The anomalous meridional circulation cells are reversed for MJO phases 4–5, which are stronger during westerly QBO with the anomalous downwelling and dry anomalies covering eastern China. The combined impact of MJO phases 8–1 and easterly QBO on the early summer rainfall is noticeable in 1996, 2016, and 2020. The enormous rainfall amount appeared along the Yangtze River in 1996, 2016, and 2020 due to the extended period of MJO phases 8–1 under the background of the easterly QBO.

Abstract

Using European Centre for Medium-Range Weather Forecasts fifth-generation (ERA5) and second Modern-Era Retrospective analysis for Research and Applications (MERRA-2) reanalysis and surface meteorological observation data, this study explores the possible impact of sudden stratospheric warming (SSW) events on air quality in the Beijing–Tianjin–Hebei (BTH) region. Major SSW events are divided into polar vortex displacement SSW and polar vortex split SSW. As the duration of split SSW events is longer and the stratospheric signal pulses propagate further downward than displacement SSWs, subseasonal variability of the atmospheric particulates in the BTH is larger during split SSWs. The air particulate concentration is light before the SSW onset due to the enhanced perturbation in the troposphere associated with strengthened planetary waves. The air particulate concentration around the SSW onset dates begins to rise due to weakening of the tropospheric disturbance as the enhanced planetary waves enter the stratosphere. In the decaying period of the SSW, the air particulate concentration decreases as the stratospheric negative northern annular mode (NAM) signal propagates downward. Specifically, in the pre-SSW period of displacement (split) SSW events, a wavenumber-1-like (wavenumber-2-like) anomaly pattern is strengthened. The East Asian winter monsoon intensifies as the East Asian trough is deepened, especially before the split SSW event onset, leading to a cleaning period. Around the SSW onset period as the tropospheric perturbation diminishes and the East Asian winter monsoon weakens, a surge of air particulate concentration is observed. After the SSW onset, due to the downward propagation of the stratospheric negative NAM signal, cold anomalies form in northeastern East Asia, especially for split SSWs, corresponding to a cleaning period in the BHT region. The local meteorological conditions during the SSWs are also discussed.

3. Influence of the El Niño–Southern Oscillation on entry stratospheric water vapor in coupled chemistry–ocean CCMI and CMIP6 models

Abstract

The connection between the dominant mode of interannual variability in the tropical troposphere, the El Niño–Southern Oscillation (ENSO), and the entry of stratospheric water vapor is analyzed in a set of model simulations archived for the Chemistry-Climate Model Initiative (CCMI) project and for Phase 6 of the Coupled Model Intercomparison Project. While the models agree on the temperature response to ENSO in the tropical troposphere and lower stratosphere, and all models and observations also agree on the zonal structure of the temperature response in the tropical tropopause layer, the only aspect of the entry water vapor response with consensus in both models and observations is that La Niña leads to moistening in winter relative to neutral ENSO. For El Niño and for other seasons, there are significant differences among the models. For example, some models find that the enhanced water vapor for La Niña in the winter of the event reverses in spring and summer, some models find that this moistening persists, and some show a nonlinear response, with both El Niño and La Niña leading to enhanced water vapor in both winter, spring, and summer. A moistening in the spring following El Niño events, the signal focused on in much previous work, is simulated by only half of the models. Focusing on Central Pacific ENSO vs. East Pacific ENSO, or temperatures in the mid-troposphere compared with temperatures near the surface, does not narrow the inter-model discrepancies. Despite this diversity in response, the temperature response near the cold point can explain the response of water vapor when each model is considered separately. While the observational record is too short to fully constrain the response to ENSO, it is clear that most models suffer from biases in the magnitude of the interannual variability of entry water vapor. This bias could be due to biased cold-point temperatures in some models, but others appear to be missing forcing processes that contribute to observed variability near the cold point.

Abstract

Flushing blast furnace by manganese ore was one of the important methods to recover the hearth inactivity caused by hearth accumulation. In this work, the properties and mechanism of the slag with different MnO content were researched, including the viscosity and critical temperature measured by rotating cylinder method, the mineral phase and microstructure confirmed by X-ray diffraction (XRD), scanning electronic microscopy (SEM) and fourier transform infrared spectrometer (FTIR). The experiment results showed that during the process of flushing blast furnace, the appropriate and optimal MnO in the slag was 1,5 to 2,0 mas .%, the mechanism of flushing by MnO was that the tephroite whose melting point is low forms, the depolymerization of [SiO4]-tetrahedral structure caused by MnO addition also plays an important role in reducing the viscosity of the slag.

5. Cloud condensation nuclei characteristics during the Indian summer monsoon over a rain-shadow region

Abstract

Continuous aerosol and cloud condensation nuclei (CCN) measurements carried out at the ground observational facility situated in the rain-shadow region of the Indian subcontinent are illustrated. These observations were part of the Cloud Aerosol Interaction Precipitation Enhancement Experiment (CAIPEEX) during the Indian summer monsoon season (June to September) of 2018. Observations are classified as dry–continental (monsoon break) and wet–marine (monsoon active) according to the air mass history. CCN concentrations measured for a range of supersaturations (0.2 %–1.2 %) are parameterized using Twomey's empirical relationship. CCN concentrations at low (0.2 %) supersaturation (SS) were high (>1000cm-3) during continental conditions and observed together with high black carbon (BC∼2000ngm-3) and columnar aerosol loading. During the marine air mass conditions, CCN concentrations diminished to ∼350cm-3 at 0.3 % SS and low aerosol loading persisted (BC∼800ngm-3). High CCN activation fraction (AF) of ≅0.55 (at 0.3 % SS) was observed before the monsoon rainfall, which reduced to ≅0.15 during the marine air mass and enhanced to ≅0.32 after that. There was mostly monomodal aerosol number size distribution (NSD) with a mean geometric mean diameter (GMD) of ≅85 nm, with least (≅9 %) contribution from nucleation mode ( nm) particles persisted before the monsoon, while multimode NSD with ≅19 % of nucleation mode particles was found during the marine air mass. Critical activation diameters (dcri) for 0.3 % SS were found to be about 72, 169, and 121 nm prior to, during, and after the marine conditions, respectively. The better association of CCN with aerosol absorption, and the concurrent accumulation mode particles during continental conditions, points to the possibility of aged (oxygenated) carbonaceous aerosols enhancing the CCN activity prior to the marine conditions. An enhancement in CCN concentrations and k values during the daytime along with absorption Ångström exponent was observed during the marine conditions. Best closure obtained using measured critical diameter and ammonium sulfate composition during continental conditions emphasizes the role of aged aerosols contributing to the accumulation mode, enhancing the CCN efficiency. The overestimation of CCN and less hygroscopicity of accumulation mode aerosols during the marine air mass indicate the role of size-dependent aerosol composition in CCN activity during the period.

Abstract

Placental Site Trophoblastic Tumor (PSTT) is a rare malignancy that often presents with extensive disease and can be resistant to traditional treatments. We present the case of a woman with stage IV PSTT who was initially managed with neoadjuvant chemotherapy followed by tumor debulking. Adjuvant therapy was guided by further pathologic analysis that revealed high levels of staining for PD-L1 as well as the presence of tumor infiltrating lymphocytes (TILs). Subsequently, the patient was treated with traditional chemotherapy with the EP/EMA regimen with the addition of pembrolizumab. The patient’s treatment course was complicated by the development of pulmonary arteriovenous malformations, autoimmune thyroiditis thought to be secondary to immunotherapy, and significant tinnitus secondary to platinum agents. Currently the patient is in follow up and remains in a complete remission.

10. Effects of boron addition on microstructures and mechanical properties of Ti-6Al-4V manufactured by direct laser deposition

Large columnar grains with strong texture in direct laser deposited (DLDed) Ti-6Al-4V (Ti-64) is widely reported to cause issues like strong mechanical property anisotropy and poor tensile strength, which make this material unsuitable for many uses in the aerospace industry. In this study, a microstructure refinement investigation was carried out by adding various amounts of boron from 0.05 wt% to 1.0 wt%. The effects of boron additions, even with 0.05 wt%, are very strong on the microstructure refinement. The presence of large columnar grains was eliminated along with the α lath refinement, the grain boundary α elimination and the texture weakening. β reconstruction results showed that boron is very effective in the β grain refinement and the β texture weakening, which contributes to the transformed α lath refinement and the α texture weakening, respectively. Furthermore, the tensile strength of DLDed Ti-64 with different boron additions is improved. A good balance of the tensile strength and ductility, with considerably reduced anisotropy, was achieved by adding 0.05 wt% boron. A comprehensive understanding between the boron additions, microstructure refinement and tensile performance improvement was fully established, which could enable greater usage of DLDed Ti-64 in the aerospace industry. Abstract

Context: Aims: This study aims to compare the effectiveness, induction to abortion interval, blood loss, and side effect profile between administration of intraamniotic prostaglandin F2 alpha (PGF2α) with a combination of mifepristone and misoprostol for medical termination of pregnancy in mid-trimester. Prospective observational study. Subjects and Methods: The study was conducted by dividing the women with indicated mid-trimester abortions into two groups of 20 women each. Group-I received Tablet Mifepristone 200 mg orally on day 1 and then after 48 h, Tablet Misoprostol 400 μg given per vaginally every 4 h till abortion was completed or a maximum of 5 doses whichever was earlier. Group-II received a single injection of PGF2α (250 μg) intra-amniotically under ultrasonography guidance. Group comparisons were made using t-test/Mann–Whitney U-test for normally/nonnormally distributed continuous data, respectively, and Chi-square test for categorical variables. SPSS version 23 was used for analysis. P < 0.05 was taken as the cut-off for statistical significance. Results: The success rate was 100% in both the groups. There was a significant difference in the mean induction to abortion interval between Group-I, the mean being 12.92 ± 2.68 h compared to Group-II being 33.08 ± 6.35 h (P < 0.001). Group-I had a mean blood loss of 221 ± 55.15 mL, significantly more than Group-II 160 ± 25.13 mL (P < 0.001). Twenty-five percent of the Group-I woman had retained products of conception with none in Group-II (P = 0.029). The pain score was better in Group II (pain score 4.4 vs. 5.7, P < 0.001). Fewer women developed chills in Group II compared to Group I (0% vs. 50%, P < 0.001). Conclusions: PGF2α is equally effective as a combination of Mifepristone and Misoprostol in mid-trimester abortion. Although it has a longer induction to abortion interval, it appears to be better with respect to the completeness of evacuation, lesser blood loss, and better patient tolerance.

Abstract Introduction: Persistence in physician-scientist careers has been suboptimal, particularly among women. There is a gender gap in self-confidence in medicine. We measured the impact of our physician-scientist training programs on trainee’s confidence in professional, personal, and scientific competencies, using a survey measuring self-rated confidence in 36 competencies across two timepoints. Methods: Results were analyzed for the full survey and for thematic subscales identified through exploratory factor analysis (EFA). A mixed effects linear model and a difference in differences (DID) design were used to assess the differential impact of the programing by gender and career level. Results: Analysis included 100 MD-PhD or MD-only medical student or resident/fellow trainees enrolled between 2020 and 2023. Five subscales were identified through EFA; career sustainability, science productivity, grant management, goal setting, and goal alignment (Cronbach’s alpha 0.85–0.94). Overall, mean scores increased significantly for all five subscales. Women significantly increased their confidence levels in all five areas, whereas men increased only in science productivity and grant management. Mixed effects models showed significant increases over time for women compared to men in career sustainability and goal alignment. Residents and fellows had greater increases than medical students across all subscales. Conclusion: Physician-scientist trainees fellows increased their confidence in personal, professional, and scientific skills during training. Training had a greater impact on women than men in building confidence in sustaining careers and aligning their goals with professional and institutional priorities. The magnitude of increased confidence among residents and fellows exceeded that in medical students.

Abstract

ImportanceCytokine storm due to COVID-19 can cause high morbidity and mortality and may be more common in patients with cancer treated with immunotherapy (IO) due to immune system activation.ObjectiveTo determine the association of baseline immunosuppression and/or IO-based therapies with COVID-19 severity and cytokine storm in patients with cancer.Design, Setting, and ParticipantsThis registry-based retrospective cohort study included 12 046 patients reported to the COVID-19 and Cancer Consortium (CCC19) registry from March 2020 to May 2022. The CCC19 registry is a centralized international multi-institutional registry of patients with COVID-19 with a current or past diagnosis of cancer. Records analyzed included patients with active or previous cancer who had a laboratory-confirmed infection with SARS-CoV-2 by polymerase chain reaction and/or serologic findings.ExposuresImmunosuppression due to therapy; systemic anticancer therapy (IO or non-IO).Main Outcomes and MeasuresThe primary outcome was a 5-level ordinal scale of COVID-19 severity: no complications; hospitalized without requiring oxygen; hospitalized and required oxygen; intensive care unit admission and/or mechanical ventilation; death. The secondary outcome was the occurrence of cytokine storm.ResultsThe median age of the entire cohort was 65 years (interquartile range [IQR], 54-74) years and 6359 patients were female (52.8%) and 6598 (54.8%) were non-Hispanic White. A total of 599 (5.0%) patients received IO, whereas 4327 (35.9%) received non-IO systemic anticancer therapies, and 7120 (59.1%) did not receive any antineoplastic regimen within 3 months prior to COVID-19 diagnosis. Although no difference in COVID-19 severity and cytokine storm was found in the IO group compared with the untreated group in the total cohort (adjusted odds ratio [aOR], 0.80; 95% CI, 0.56-1.13, and aOR, 0.89; 95% CI, 0.41-1.93, respectively), patients with baseline immunosuppression treated with IO (vs untreated) had worse COVID-19 severity and cytokine storm (aOR, 3.33; 95% CI, 1.38-8.01, and aOR, 4.41; 95% CI, 1.71-11.38, respectively). Patients with immunosuppression receiving non-IO therapies (vs untreated) also had worse COVID-19 severity (aOR, 1.79; 95% CI, 1.36-2.35) and cytokine storm (aOR, 2.32; 95% CI, 1.42-3.79).Conclusions and RelevanceThis cohort study found that in patients with cancer and COVID-19, administration of systemic anticancer therapies, especially IO, in the context of baseline immunosuppression was associated with severe clinical outcomes and the development of cytokine storm.Trial RegistrationClinicalTrials.gov Identifier: NCT04354701

Abstract

Classroom assessment is the process of documenting the knowledge, skills, attitudes and beliefs of learners. It provides essential feedback to both instructors and students to improve their teaching methods for guiding and motivating students to be actively involved in their learning. Assessment drives learning. Formative assessments enable the instructor to guide the students to learn well. Summative assessments enable the measurement of levels of attainment of course outcomes and act as feedback to course design and curriculum improvement. This article presents the underlying principles of assessment through a discussion of assessment approaches and their purposes, types of assessment items, quality of assessment and summative assessment plans. Quality assessment instruments can be developed through an understanding of the quality attributes of assessment items, the process of designing assessment instruments, designing a variety of assessment items, and devising plans to evaluate them through rubrics. An approach is presented for creating a summative assessment plan that can also lead to the attainment of outcomes as per the requirements of programme accreditation.

Abstract

Background: Reducing the burden of death due to infection is an urgent global public health priority. Previous studies have estimated the number of deaths associated with drug-resistant infections and sepsis and found that infections remain a leading cause of death globally. Understanding the global burden of common bacterial pathogens (both susceptible and resistant to antimicrobials) is essential to identify the greatest threats to public health. To our knowledge, this is the first study to present global comprehensive estimates of deaths associated with 33 bacterial pathogens across 11 major infectious syndromes. Methods: We estimated deaths associated with 33 bacterial genera or species across 11 infectious syndromes in 2019 using methods from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, in addition to a subset of the input data described in the Global Burden of Antimicrobial Resistance 2019 study. This study included 343 million individual records or isolates covering 11 361 study-location-years. We used three modelling steps to estimate the number of deaths associated with each pathogen: deaths in which infection had a role, the fraction of deaths due to infection that are attributable to a given infectious syndrome, and the fraction of deaths due to an infectious syndrome that are attributable to a given pathogen. Estimates were produced for all ages and for males and females across 204 countries and territories in 2019. 95% uncertainty intervals (UIs) were calculated for final estimates of deaths and infections associated with the 33 bacterial pathogens following standard GBD methods by taking the 2·5th and 97·5th percentiles across 1000 posterior draws for each quantity of interest. Findings: From an estimated 13·7 million (95% UI 10·9–17·1) infection-related deaths in 2019, there were 7·7 million deaths (5·7–10·2) associated with the 33 bacterial pathogens (both resistant and susceptible to antimicrobials) across the 11 infectious syndromes estimated in this study. We estimated deaths associated with the 33 bacterial pathogens to comprise 13·6% (10·2–18·1) of all global deaths and 56·2% (52·1–60·1) of all sepsis-related deaths in 2019. Five leading pathogens—Staphylococcus aureus, Escherichia coli, Streptococcus pneumoniae, Klebsiella pneumoniae, and Pseudomonas aeruginosa—were responsible for 54·9% (52·9–56·9) of deaths among the investigated bacteria. The deadliest infectious syndromes and pathogens varied by location and age. The age-standardised mortality rate associated with these bacterial pathogens was highest in the sub-Saharan Africa super-region, with 230 deaths (185–285) per 100 000 population, and lowest in the high-income super-region, with 52·2 deaths (37·4–71·5) per 100 000 population. S aureus was the leading bacterial cause of death in 135 countries and was also associated with the most deaths in individuals older than 15 years, globally. Among children younger than 5 years, S pneumoniae was the pathogen associated with the most deaths. In 2019, more than 6 million deaths occurred as a result of three bacterial infectious syndromes, with lower respiratory infections and bloodstream infections each causing more than 2 million deaths and peritoneal and intra-abdominal infections causing more than 1 million deaths. Interpretation: The 33 bacterial pathogens that we investigated in this study are a substantial source of health loss globally, with considerable variation in their distribution across infectious syndromes and locations. Compared with GBD Level 3 underlying causes of death, deaths associated with these bacteria would rank as the second leading cause of death globally in 2019; hence, they should be considered an urgent priority for intervention within the global health community. Strategies to address the burden of bacterial infections include infection prevention, optimised use of antibiotics, improved capacity for microbiological analysis, vaccine development, and improved and more pervasive use of available vaccines. These estimates can be used to help set priorities for vaccine need, demand, and development. Abstract

Background Pulmonary arterial hypertension (PAH) is a complex, fatal disease where disease severity has been associated with the single nucleotide polymorphism (SNP) rs2856830, located near the human leukocyte antigen DPA1 (HLA‐DPA1) gene. We aimed to define the genetic architecture of functional variants associated with PAH disease severity by identifying allele‐specific binding transcription factors and downstream targets that control endothelial pathophenotypes and PAH. Methods and Results Electrophoretic mobility shift assays of oligonucleotides containing SNP rs2856830 and 8 SNPs in linkage disequilibrium revealed functional SNPs via allele‐imbalanced binding to human pulmonary arterial endothelial cell nuclear proteins. DNA pulldown proteomics identified SNP‐binding proteins. SNP genotyping and clinical correlation analysis were performed in 84 patients with PAH at University of Pittsburgh Medical Center and in 679 patients with PAH in the All of Us database. SNP rs9277336 was identified as a functional SNP in linkage disequilibrium ( r 2 >0.8) defined by rs2856830, and the minor allele was associated with decreased hospitalizations and improved cardiac output in patients with PAH, an index of disease severity. SNP pulldown proteomics showed allele‐specific binding of nuclear ACTN4 (alpha actinin 4) protein to rs9277336 minor allele. Both ACTN4 and HLA‐DPA1 were downregulated in pulmonary endothelium in human patients and rodent models of PAH. Via transcriptomic and phenotypic analyses, knockdown of HLA‐DPA1 phenocopied knockdown of ACTN4, both similarly controlling cell structure pathways, immune pathways, and endothelial dysfunction. Conclusions We defined the pathogenic activity of functional SNP rs9277336, entailing the allele‐specific binding of ACTN4 and controlling expression of the neighboring HLA‐DPA1 gene. Through inflammatory or genetic means, downregulation of this ACTN4‐HLA‐DPA1 regulatory axis promotes endothelial pathophenotypes, providing a mechanistic explanation for the association between this SNP and PAH outcomes.

40. Cooled radiofrequency ablation of the sacroiliac joint a retrospective case series

Abstract

Background Sacroiliac (SI) joint dysfunction is a common source of back pain. Recent evidence from different parts of the world suggest that cooled radiofrequency ablation of sacral nerves supplying the SI joints has superior pain alleviating properties than currently available treatment options for SI joint dysfunction. Patients and methods After obtaining institutional review board approval, the medical records of 81 patients who underwent cooled radiofrequency ablation in a single institution and by a single surgeon were analyzed retrospectively. The recurrence of pain, progression to fusion and functional outcomes were noted. The patients were operated on between June 2020 and December 2021, they include 59 females and 22 males, the average age was 55.4 ± 17.3. Follow up was at least 6 months postoperative. Results 22 of the patients had previously underwent lumbar fusions. Follow up period ranged from 6 to 18 months. After radiofrequency ablation, 7 patients progressed to fusions, and 6 patients had to have the procedure done again to relieve their pain. Student t-test was used to compare between preoperative and postoperative values of NPRS (numerical pain rating score) and ODI (Oswestry disability index). It showed significance with P-value Conclusions Sacroiliac joint radiofrequency ablation is a good option in the treatment of SI joint pain showing good results in the short term follow up period. It is a simple procedure that can be done in less than 30 min and is capable of providing significant pain relief for patients with sacroiliac joint dysfunction.

45. Incidence, Mortality, and Treatment Patterns of Synchronous Lower Genital Tract Squamous Cell Carcinoma

Abstract

The aim of the study was to define the incidence, prognosis, and treatment patterns associated with synchronous lower genital tract squamous cell carcinoma (LGTSCC) among women diagnosed with any LGTSCC.Surveillance, Epidemiology, and End Results (SEER) database patients diagnosed with synchronous cervical, vaginal, vulvar, and/or anal SCC from 2000 to 2016 were included. Incidence and mortality were evaluated using Kaplan-Meier curves. Multivariable Cox proportional hazards regression was used to identify treatment patterns, risk factors, and mortality associated with LGTSCC.Among 15,424 women with LGTSCC, 138 had synchronous LGTSCC (0.89%). Vulvar and vaginal SCC was the most common combination (49.3%), and cervical with anal SCC was the least common (1.4%). Only one patient had 3 or more primary LGTSCC. Synchronous LGTSCC was independently associated with higher mortality compared with single-site LGTSCC (adjusted hazards ratio [aHR] = 1.67; p.001). Synchronous LGTSCC was significantly associated with older age (63 vs. 58 years, p.001) and lower stage (adjusted odds ratio [aOR] = 0.38; p.001) and grade of disease (aOR = 0.32; p.001). Patients with synchronous LGTSCC were more likely to receive radiation (aOR = 1.74; p = .005) and were more likely to receive adjuvant radiation after primary surgical resection compared with single-site LGTSCC (aOR = 1.88; p = .007). Receipt of any treatment including radiation (aHR = 0.85; p.001), chemotherapy (aHR = 0.50; p.001), and surgery (aHR = 0.70; p.001) was independently associated with decreased mortality hazard.Synchronous LGTSCC is rare but is associated with increased mortality and higher rates of adjuvant radiation compared with single-site LGTSCC despite lower stage and grade at diagnosis. More research is needed to define optimal therapy for these patients.

46. Surveillance for endemic infectious disease outbreaks: Adaptive sampling using profile likelihood estimation

Abstract

Outbreaks of an endemic infectious disease can occur when the disease is introduced into a highly susceptible subpopulation or when the disease enters a network of connected individuals. For example, significant HIV outbreaks among people who inject drugs have occurred in at least half a dozen US states in recent years. This motivates the current study: how can limited testing resources be allocated across geographic regions to rapidly detect outbreaks of an endemic infectious disease? We develop an adaptive sampling algorithm that uses profile likelihood to estimate the distribution of the number of positive tests that would occur for each location in a future time period if that location were sampled. Sampling is performed in the location with the highest estimated probability of triggering an outbreak alarm in the next time period. The alarm function is determined by a semiparametric likelihood ratio test. We compare the profile likelihood sampling (PLS) method numerically to uniform random sampling (URS) and Thompson sampling (TS). TS was worse than URS when the outbreak occurred in a location with lower initial prevalence than other locations. PLS had lower time to outbreak detection than TS in some but not all scenarios, but was always better than URS even when the outbreak occurred in a location with a lower initial prevalence than other locations. PLS provides an effective and reliable method for rapidly detecting endemic disease outbreaks that is robust to this uncertainty.

47. Traumatic temporal bone fracture with middle ear effusion: A case report

Abstract

Temporal bone fracture is a relatively rare but significant complication of traumatic head injury. We present a rare and unique case of traumatic temporal bone fracture with middle ear effusion, in a 76-year-old woman, following a fall. Physical examination on presentation was remarkable for a superficial scalp hematoma in the occipital region, without any focal neurological deficits. An initial non-contrast head CT revealed a large posterior scalp hematoma and subtle changes suggestive of artifact vs. hemorrhage within the right temporal lobe. Over two days, she developed a worsening headache, with new hearing impairment and reduced right sided bone-conduction on auditory testing. A repeat head CT confirmed a right hemorrhagic temporal lobe contusion as well as a right mastoid and middle ear effusion. A dedicated temporal bone CT scan was performed, which revealed an acute longitudinal fracture through the right mastoid bone without extension into the middle ear cavity. She was evaluated by neurosurgery, managed symptomatically, and observed closely. Her neurological status remained stable, and she was discharged with planned outpatient follow-up with her primary care provider and the consulting neurosurgeon. This case illustrates sequalae of traumatic temporal bone fracture, as well as the value of clinical history and heightened clinical concern for an occult, easily overlooked region during imaging.

BackgroundInformation on reproductive experiences and awareness of adverse pregnancy outcomes (APOs) and cardiovascular disease (CVD) risk among pregnancy-capable and post-menopausal individuals has not been well described. We sought to evaluate preconception health and APO awareness in a large population-based registry.MethodsData from the Fertility and Pregnancy Survey of the American Heart Association Research Goes Red Registry (AHA-RGR) were used. Responses to questions pertaining to prenatal health care experiences, postpartum health, and awareness of the association of APOs with CVD risk were used. We summarized responses using proportions for the overall sample and by stratifications, and we tested differences using the Chi-squared test.ResultsOf 4,651individuals in the AHA-RGR registry, 3,176 were of reproductive age, and 1,475 were postmenopausal. Among postmenopausal individuals, 37% were unaware that APOs were associated with long-term CVD risk. This varied by different racial/ethnic groups (non-Hispanic White: 38%, non-Hispanic Black: 29%, Asian: 18%, Hispanic: 41%, Other: 46%;P= 0.03). Fifty-nine percent of the participants were not educated regarding the association of APOs with long-term CVD risk by their providers. Thirty percent of the participants reported that their providers did not assess pregnancy history during current visits; this varied by race-ethnicity (P= 0.02), income (P= 0.01), and access to care (P= 0.02). Only 37.1% of the respondents were aware that CVD was the leading cause of maternal mortality.ConclusionsConsiderable knowledge gaps exist in the association of APOs with CVD risk, with disparities by race/ethnicity, and most patients are not educated on this association by their health care professionals.
