The seat characteristics have high relevance in overall comfort on any transportation means. In particular, the foam’s mechanical properties, interface pressure, and contact temperature play an important role in low- or no-vibration situations regarding static comfort. The present work presents the complete protocol for a static evaluation of different foams and seat covers to assess railway seats. Based on the evaluation of the foam’s mechanical properties and interface pressure profiles, it was concluded that higher-density foam (80 kg/m3) is the most favorable. Regarding the foam cover, a thermographic assessment demonstrated that the fabric cover that induces lower temperatures at passenger interface contact promotes higher comfort levels. It should be highlighted that experiments were conducted on real train seat cushions and environments using a thermographic camera and pressure map sensor.

Introduction The phenomenon of suicide and self-harm is one of the most intriguing and disturbing human behaviours. Suicide is global public health problem, with multiple and complex contributing factors. Global trend show a stabilizing or descending curves in the last years. The Portuguese atlantic archipelago of Azores has had an opposite trend that together with regional proctective and risk factors ought to be addressed for further tailored interventions. Objectives Review of the up-to-date literature on this topic and present the recent suicide-related data in the Azores. Methods Unsystematic review of the most recent and relevant literature. Results Epidemiology, risk and protective factors, preventive and treatment measures were described. The Azores region has a ascending trend in the suicide mortality rate, opposing the portuguese trend. The azorean suicide mortality rate has a bimodal distribution and has higher values in every age group, except for the +75yo, when compared with the portuguese rates. Suicide protective and risk factors mostly overlap with mental disorders’ factors and those of other health and socio-economic conditions. Azorean suicide rates are growing against the global and national descending trends. Prevention and treatment strategies to be implemented regionally must be fine tailored, accounting for the most relevant factors in place, in order to be most effective. Introduction The emergence of new psychoactive substances (NPS) has had a substantial impact worldwide. NPS mimic the effect of “existing” drugs and are specifically manufactured so that the new substances fall out of regulatory frameworks. Although the structural changes might be minimal, NPS often have marked differences in potency and subsequent harm. The population of the Azores archipelago has been particularly affected by the rapid growing and changing nature of this phenomenon, mainly caused by the introduction and spread of the “newly” synthetic cathinones as a inexpensive and easily available street drug. Before any educational, public health and socioeconomic policy changes be proposed to accurately tackle the problem, the basic step of knowing how these substances have been affecting their users, specially their neuropsychiatric effects, must be taken. Objectives This project aims to characterize the neuropsychiatric side effects caused by the acute intoxication of NPS with a psychostimulant profile, namely synthetic cathinones. Methods Description of the neuropsychiatric symptoms of intoxicated users of stimulant NPS that present to non-governmental organizations specialized in addictions and also to the psychiatry emergency department in Sao Miguel - Azores. Review of the forensic records of the deaths by suicide in 2021 in Sao Miguel, looking for evidence of recent abuse of NPS or previous history of “NPS use disorder”. Non-systematic review of the recent and relevant scientific literature on this topic. Results The desired effects are increased energy, mood enhancement, euphoria, mental clarity, improved concentration, improved sociability, increased talkativeness, empathy inducing effects, amplification of sound and colour and prosexual effects. Nevertheless, in the case of intoxication, the frequent neuropsychiatric side effects tend to be agitation, agressiveness, irritability, altered consciousness, brief psychosis with paranoid delusion, visual and auditive hallucinations, transient mania, enhanced sensorial experiences, headaches, dizzyness, seizures, confusion and amnesia. Usually, the acjute intoxication period tend to be followed by a “crash” with depression, craving, anxiety, panic, suicidal ideation and behaviours. A third of the deaths by suicides in Sao Miguel Island in 2021, 7 out of 21, were in stimulant NPS active or recent users. Conclusions Review of the neuropsychiatric effects of New Psychoactive Substances with a psychostimulant profile. Further studies of this population of synthetic cathinone users in the Azores are due, namely studying their socioeconomic background, looking for risk and protective factors, and also the long-term side effects. Introduction Delusional infestation (DI), also known as delusional parasitosis or Ekbom syndrome, is a rare disorder, characterised by fixed belief that the skin, body or immediate environment is infested by small pathogens, despite the lack of any medical evidence for it. Objectives To describe and discuss two clinical cases of DI, in order to show two different ways of presenting in this entity. Methods Two case report and non-systematic review. Results We present the case of a 76-year-old woman, without psychiatric history, with an DI with 5 years of evolution, referred to a psychiatric consultation by a dermatologist. The second case, is a 41-year-old woman with a history of multiple substance use disorder, with an DI with a month of evolution, who resorted to the emergency department. DI is not a single diagnostic entity. The classic form, as represented in the first case,is a primary form, which develops without any known cause or underlying disease, corresponding to a persistent delusional disorder. However, about 60% of patients have secondary forms of DI, in the context of substance misuse, some medications or in the course of physical or psychiatric diseases (e.g. stroke, delirium, dementia, depression, schizophrenia). Conclusions DI can occur as a primary delusional disorder or secondary to several other medical conditions. An in-depth clinical history is essential in order to make the correct diagnosis. A multidisciplinary approach is also important, to exclude any possible organic etiology, not forgetting that many patients may turn to other medical specialities first. Introduction Perinatal death includes losses such as ectopic pregnancies, miscarriages, stillbirths and neonatal deaths. Perinatal loss has well documented negative effects on the health of the bereaved parents. Early pregnancy loss (EPL) is the spontaneous death of a fetus within the first 20 weeks of gestation. Objectives To describe a clinical case of disenfranchised grief following EPL and to review the literature. Methods We reviewed the clinical file of a patient presenting to the psychiatry outpatient clinic with disenfranchised grief. We conducted a non-systematic review on PubMed and Google Scholar. Results A 29-years-old female patient presents to the outpatient clinic with depressive symptoms and thoughts of death. The symptoms had begun 4 months earlier, following the loss of pregnancy at 14 weeks. She felt her grief was not accepted by her family and social network. Progressively, her relationships deteriorated, and she felt more and more isolated. She experienced marked difficulty in caring for her older child. Compared to other types of mourning, the loss of a child is associated with grief experience that is particularly severe and complicated. Despite the high prevalence of EPL, many women suffer in silence due to the common belief these losses are insignificant and may develop complicated grief. Conclusions Perinatal loss of an infant has the potential to have a large impact on the mental health of the bereaved parents. Literature on the efficacy of different interventions is still scarce. Further studies are necessary on prevention strategies and interventions for parents already suffering from complicated grief or depressive disorders. Disclosure No significant relationships.

Introduction Rates of cannabis use among pregnant women have been increasing. Psychiatrists may be required to provide counselling regarding marijuana use in pregnancy for their patients. Objectives To produce an up-to-date review of cannabis effects on pregnancy and the offspring. Methods We performed a non-systematic review of the literature apropos a clinical case. Results A 31-years-old, 22-weeks pregnant woman presented with severe anxiety, panic attacks and insomnia which she managed solely with cannabis. She had been previously treated with antidepressants and benzodiazepines with symptom remission but had suspended before her pregnancy without medical advice. She believed medication was more harmful to the baby than her cannabis use. There is little perception of risk concerning cannabis use in pregnant woman. Information on cannabis use is less likely to be obtained from healthcare providers than from anecdotal experiences, Internet searching and advice from friends and family. Prenatal use of cannabis has been associated with anaemia in the mother, whereas in the offspring it is associated with reduction in birth weight and greater likelihood of placement in intensive care units. There is insufficient evidence to support an association between marijuana use and any specific congenital abnormality, but also to demonstrate its safety. Conclusions It is essential for psychiatrists to have up-to-date knowledge of the effects of cannabis on the pregnancy and the offspring to properly counsel their patients. However, the effects of cannabis on maternal and foetal outcomes remain generally unknown. With rising numbers of female users, there is urgent need for further research. Introduction The mental health impact of the COVID-19 pandemic is well documented. Portugal entered the emergency state on 19th march due to rising numbers of infected patients. The emergency state introduced regulatory measures that restricted people’s movements, applied a curfew, and closed most non-essential spaces and activities, such as shops and religious celebrations. Objectives To evaluate the rates of suicides during the emergency state in Portugal. Methods We obtained the number of probable suicides during 19th march and 2nd may 2020, 2019 and 2018 from SICO/eVM (Real Time Mortality Electronic Surveillance). This system is used for health planning in Portugal and provides provisory data which is updated every 10 minutes. ExcelÒ was used for the statistical analysis. Results During the Emergency State in Portugal there were 57 probable suicides. Comparing to the same period in 2018 and 2019, there were 62 and 70 probable suicides, respectively. Social isolation, anxiety, fear of contagion, chronic stress, and economic difficulties may lead to the development or exacerbation of depressive, anxiety, substance use, and other psychiatric disorders. Literature on suicides due to COVID-19 mention not only fear of infection, but also social isolation and distancing and economic recession as causes for suicide attempts and completions. Conclusions During the emergency state there was not an increase of probable suicides, compared to previous years. The greater vigilance of people’s movements may have deterred many attempts. However, policymakers and health care providers must be alert as the current psychosocial predispose to an increase in suicide rates. Disclosure No significant relationships.

Introduction Substance use disorders in adolescents are a growing problem worldwide. These disorders are often unrecognised, unvalued by families, society and clinicians and as a result underdiagnosed, with serious future consequences if improperly addressed. Objectives Updated review of the recent literature on this topic. Methods Unsystematic review of the most recent and relevant literature. Results Review of neurobiology, risk factors, co-morbidity, differential diagnosis, diagnostic criteria, evaluation and treatment of substance use disorders in adolescence. Conclusions Substance use disorder in adolescence includes a variety of behaviours related to the use of alcohol and/or drugs, for instance, inability to control substance use, impairment of function at school, home or work, interpersonal problems and hazardous use of substance. Further knowledge in identifying, early diagnosing and adequate intervention in adolescents’ substance use disorder may have paramount prognostic features. Disclosure No significant relationships.

AbstractIntroduction One of the main goals of periodontal treatment is the regeneration of lost tissues, in order to improve the prognosis of the involved teeth [1–3]. Guided tissue regeneration (GTR) involves the use of a barrier with or without the use of bone substitutes [4]. Given the great diversity and constant evolution of materials, it is necessary to maintain a permanent evaluation of their clinical effectiveness. This study intends to evaluate the success and predictability of GTR using a new membrane and bone substitute of equine origin (Heart® and Mix Granules® – Bioteck SpA, Turin, Italy), in the treatment of infra-bony defects with two or three walls [5].Materials and methods This study was approved by Egas Moniz Ethics Committee and all patients signed an informed consent form. Patients were selected among those referred to the Department of Periodontology/EMDC, with indication for surgical periodontal treatment, presence of 2–3-wall infra-bony defects, probing depth (PD) ≥ 5 mm, infra-bony component ≥4 mm deep (evaluated radiographically) and attached gingiva ≥2 mm. Exclusion criteria were: cases with PIaque Index (PI) >20%, smokers, extension of the defect to the furcation zone, mobility grade III, endodontic pathology, diabetes or other pathologies that interfere with bone remodelling and metabolism or contraindicate periodontal surgery. All surgeries were performed by the same operator. The clinicians involved in the evaluation of the results had no intervention in surgery. The following clinical and radiographic parameters were evaluated at one, three and six months after surgery: PI, Gingival Index (GI), Probing pocket depth (PPD), Bleeding on Probing (BoP); Gingival recession (GR), height of keratinised gingiva (KG) and radiographic angle of the defect (RAD). In addition, the presence of possible intra-surgical or post-operative complications was also evaluated. The assessment of the defect depth reduction was based on standardised periapical radiographs obtained through the use of an individualised device (parallelizer).Results A total of six participants (83% females, 17% males), with a mean age of 61.6 (± 8.1) years, were enrolled in the study. The number of defect walls was: 3 walls (50%), 2 walls (33.3%) and 2/3 walls (16.7%). After six months, an average clinical attachment level gain of 3.3 (± 1.4) mm, mean radiographic bone filling of 72 (± 29%) and a BoP reduction of 66.6% were observed. RAD average was 21.3 (± 1.2°) and GR changed from 2.2 to 2.7 mm. Membrane exposure was observed in 33.3% of the cases, however, there were no cases of infection or severe pain.Discussion and conclusions The use of collagen membranes and corticocancellous bone grafts of equine origin seems to be a viable alternative in the regeneration of infra-bony periodontal defects.

Abstract Background Chronic angiogenesis is a hallmark of most tumors and takes place in a hostile tumor microenvironment (TME) characterized by hypoxia, low nutrient and glucose levels, elevated lactate and low pH. Despite this, most studies addressing angiogenic signaling use hypoxia as a proxy for tumor conditions. Here, we compared the effects of hypoxia and TME conditions on regulation of the Na+/H+ exchanger NHE1, Ser/Thr kinases Akt1–3, and downstream effectors in endothelial cells. Methods Human umbilical vein endothelial cells (HUVEC) and Ea.hy926 endothelial cells were exposed to simulated TME (1% hypoxia, low serum, glucose, pH, high lactate) or 1% hypoxia for 24 or 48 h, with or without NHE1 inhibition or siRNA-mediated knockdown. mRNA and protein levels of NHE1, Akt1–3, and downstream effectors were assessed by qPCR and Western blotting, vascular endothelial growth factor (VEGF) release by ELISA, and motility by scratch assay. Results Within 24 h, HIF-1α level and VEGF mRNA level were increased robustly by TME and modestly by hypoxia alone. The NHE1 mRNA level was decreased by both hypoxia and TME, and NHE1 protein was reduced by TME in Ea.hy926 cells. Akt1–3 mRNA was detected in HUVEC and Ea.hy926 cells, Akt1 most abundantly. Akt1 protein expression was reduced by TME yet unaffected by hypoxia, while Akt phosphorylation was increased by TME. The Akt loss was partly reversed by MCF-7 human breast cancer cell conditioned medium, suggesting that in vivo, the cancer cell secretome may compensate for adverse effects of TME on endothelial cells. TME, yet not hypoxia, reduced p70S6 kinase activity and ribosomal protein S6 phosphorylation and increased eIF2α phosphorylation, consistent with inhibition of protein translation. Finally, TME reduced Retinoblastoma protein phosphorylation and induced poly-ADP-ribose polymerase (PARP) cleavage consistent with inhibition of proliferation and induction of apoptosis. NHE1 knockdown, mimicking the effect of TME on NHE1 expression, reduced Ea.hy926 migration. TME effects on HIF-1α, VEGF, Akt, translation, proliferation or apoptosis markers were unaffected by NHE1 knockdown/inhibition. Conclusions NHE1 and Akt are downregulated by TME conditions, more potently than by hypoxia alone. This inhibits endothelial cell migration and growth in a manner likely modulated by the cancer cell secretome.

We intend to contribute towards a better understanding of the variables that affect the Schematic Investment and Self-consciousness of the appearance of those who suffer from acquired facial disfigurement. The sample consisted of 67 individuals who have been submitted to plastic and reconstructive surgery and completed a questionnaire evaluating the influence of personality traits, optimism, self-concept, emotions and perception of satisfaction with social support, and with the perception of appearance during their admittance to the hospital, and 12 months after the surgery. There were meaningful statistical differences in both evaluation moments for the variables Neuroticism, Extraversion, Openness, Agreeableness, Conscientiousness, Optimism, Positive and Negative Affect, Self-Concept, and Social Support Satisfaction. In both evaluations, Self-conscientiousness of Appearance reveals a positive relationship with Neuroticism and negative relation with Self-concept. Results show that Schematic Investment and Self-conscientiousness of appearance vary in time and that there are variants that influence the psychological adjustment to acquired facial disfigurement.

Verificou-se a ação dos reguladores de crescimento de insetos (IGR), diflubenzuron e methoprene, sobre o besouro rola-bosta africano, Digitonthophagus gazella (Fabricius), um inimigo natural da mosca-dos-chifres, Haematobia irritans irritans (Linnaeus). Casais de besouros foram colocados em baldes contendo terra úmida e alimentados com fezes bovinas contendo 1, 0,5 ou 0,2ppm de diflubenzuron e 0,2ppm de methoprene. Os insetos e sua prole foram recuperados com o auxílio de armadilhas pitfall. Diflubenzuron e methoprene não afetaram a sobrevivência dos adultos inicialmente expostos, mas interferiram na produção de descendentes. Diflubenzuron, nas concentrações de 1 e 0,5ppm, também afetou a duração do ciclo de vida dos besouros. Nenhum dos IGRs alterou a razão sexual dos descendentes obtidos. As concentrações testadas de diflubenzuron mostraram-se moderadamente nocivas ao besouro enquanto methoprene a 0,2ppm mostrou ser pouco nocivo, segundo os critérios da International Organization for Biological Control.The effects of insect growth regulators (IGR), diflubenzuron and methoprene, on African dung beetle, Digitonthophagus gazella (Fabricius), a natural enemy of the horn fly, Haematobia irritans irritans (Linnaeus), was studied. Beetles were placed in buckets partially filled with humid soil and were fed bovine feces containing 1, 0.5, or 0.2ppm diflubenzuron and 0.2ppm methoprene. Insects and their progenies were recovered by pitfall traps. Diflubenzuron and methoprene did not affect the survival of the adults but reduced their progenies. Diflubenzuron 1 and 0.5ppm also affected the life cicle of the beetles. None of the IGR modified the gender ratio of the progenies. According to the IOBC criteria, diflubenzuron tested concentrations showed to be moderately harmful to the beetles, whereas methoprene 0.2ppm was slightly harmful.

Neste trabalho procedeu-se à avaliação de um distribuidor centrífugo de adubo com vista à sua utilização no âmbito de um projecto de agricultura de precisão. Foram realizadas regulações diversas e ensaios estáticos e dinâmicos. Para além da garantia da homogeneidade de distribuição, entre discos e transversal, ficou demonstrada a capacidade do equipamento para se auto-regular perante variações na velocidade de avanço e em função da posição geo-referenciada na parcela, aspectos fundamentais para garantirem a aplicação diferenciada de fertilizante numa parcela.In this article a fertilizer spreader was evaluated considering its use in precision agriculture production. Several regulations and several static and dynamic trials were performed. It was demonstrated that the fertilizer spreader had good distribution homogeneity, considering different working velocities and different positions in the parcel. With these results one could confirm that the spreader can be used to spread any fertilizer differentially in space.

A Doença de Dupuytren observa-se com frequência nesta região. Dada a sua incidência, a diminuição da capacidade de trabalho que pode provocar e o elevado número de casos tratados cirurgicamente, decidimos reavaliar os pacientes, tentando identificar factores etiológicos e o resultado do tratamento. Foi efectuado um estudo de revisão clínico e de registos em processo de 100 casos, operados entre Fevereiro de 2002 e Dezembro de 2005 no Serviço de Ortopedia do Hospital do Vale do Sousa. Após discussão com os ortopedistas assistentes, todos os dados relevantes recolhidos foram tratados estatisticamente em colaboração com o Serviço de Bioestatística da Faculdade de Medicina do Porto. Os resultados são precedidos de uma perspectiva histórica e um sumário da etiologia, evolução, estadiamento, classificação e opções de tratamento da doença de Dupuytren.

O experimento foi realizado em um Latossolo Vermelho-Escuro distrófico textura média, em Ponta Grossa (PR), com o objetivo de avaliar a produção de milho, trigo e soja em função das alterações das características químicas do solo, pela aplicação de calcário e gesso na superfície, em sistema plantio direto. O delineamento experimental empregado foi o de blocos ao acaso em parcela subdividida, com três repetições. Foram utilizadas quatro doses de calcário dolomítico, com 84% de PRNT: 0, 2, 4 e 6 t ha-1 e quatro doses de gesso agrícola: 0, 4, 8 e 12 t ha-1. A calagem foi realizada em julho, e a aplicação de gesso feita em novembro de 1993. A produção da cultura de milho foi avaliada no ano agrícola de 1994/95, a de trigo no inverno de 1996 e a de soja em 1996/97. A aplicação de calcário na superfície não influenciou a produção de milho, trigo e soja. Houve ação da calagem na correção da acidez de camadas superficiais e do subsolo, porém seu efeito em camadas profundas não foi observado após quarenta meses. O gesso foi eficiente na melhoria do ambiente radicular do subsolo, embora tenha causado lixiviação de magnésio trocável do solo. Das três culturas avaliadas, somente o milho apresentou resposta à aplicação de gesso em decorrência do fornecimento de enxofre, da melhoria do teor de cálcio trocável, da redução da saturação por alumínio e do aumento da relação Ca/Mg do solo.

O presente trabalho busca caracterizar os impactos causados ao meioambiente, particularmente aos recursos hídricos subterrâneos da cidade de Fortaleza, em função da disposição final de resíduos sólidos urbanos (RSU) no aterro do Jangurussu. As atenções ficam potencializadas em função de que uma parcela da população não dispõe de saneamento básico no bairro, obrigando-se a que esta comunidade se abasteça através de poços cuja a água é utilizada para todos os fins. As condutividades hidráulicas relativas a formação Barreiras e aluviões foram avaliadas através de “slug test”. Registrou-se em campo o pH, temperatura e condutividade elétrica (CE), sendo realizadas análises para Cl, Na, Mg, K, Ca, Ba, Fe, Al, Mn, Ni, Co, Cr, Cu, Cd, B, P, Pb e Zn, das distintas surgências de chorume e das águas subterrâneas locais em dois poços, considerados como “background”, oito piezômetros e um poço de monitoramento. Foram realizadas leituras seqüenciais (cada 40 metros) com condutivímetro eletromagnético Geonics EM-34-XL, com bobinas espaçadas de 10 metros e com leituras em HD (7,5m) e VD (15m), ao longo dos 2500 metros de perímetro do aterro. Mapeou-se com um condutivímetro Geonics EM-31, de 10 em 10 metros (258 estações), com leituras HD (3m) e VD (6m), cobrindo-se aproximadamente, uma área de 29 ha., ao norte do aterro.

Este trabalho relata a primeira ocorrência do parasitóide Paraganapis egeria Diaz & Gallardo (Hymenoptera: Eucoilidae) parasitando pupas de Haematobia irritans L. (Diptera: Muscidae) na região Neotropical. As fezes bovinas foram coletadas nas pastagens da Fazenda Canchim da Embrapa de São Carlos-SP, de abril de 1993 a abril de 1994. As pupas foram separadas das fezes bovinas por flutuação em baldes com água. As recolhidas foram acondicionadas individualmente em cápsulas de gelatina até a emergência dos dípteros ou dos seus parasitóides. Foram obtidas 718 pupas de H. irritans, das quais duas emergiram parasitóides. Constatou-se parasitismo de 0,26%.

Os Autores propõem a padronização de métodos para se avaliarem os resultados de ensaios de terapêutica específica da doença de Chagas, a fim de se firmarem critérios seguros de interpretação. Discutem as experiências terapêuticas, com controle estatístico, no ser humano, sobretudo o problema ético, e concluem serem elas indispensáveis para se estabelecer a eficácia do medicamento, definir-lhe o modo de emprego, a posologia e os perigos, mas precisam reger-se por normas bem definidas. Relatam uma metodologia para o ensaio clínico na tripanossomíase americana, partindo do princípio de que só devem ser selecionados pacientes de "alta parasitemia". Submetendo chagásicos crônicos a xenodiagnósticos mensais consecutivos, por períodos até de três anos, verificaram ser variável a positividade: enquanto uns exibem todos os exames positivos (100%), outros mostram graus menores de positividade, havendo até os que nunca apresentam xeno positivo (0%). Explicam o fenômeno admitindo a existência de níveis diferentes de parasitemia e recomendam evitarem-se os pacientes de "baixa parasitemia", pela dificuldade de interpretação, ligada ao fato de eles apresentarem, espontâneamente, sem qualquer tratamento, xenodiagnósticos negativos durante anos. Descrevem os procedimentos correspondentes à fase pré-tratamento, para selecionar os pacientes que apresentem positivos pelo menos 60% de uma série de, no mínimo, seis xenodiagnósticos sêxtuplos quinzenais, cuja padronização é dada. Na fase de tratamento os doentes são internados no Hospital e examinados diariamente, para se verificar a tomada do medicamento e a tolerância, que também se aprecia por exames de laboratório prévios, e coincidentes com a última quinzena. A fase pós-tratamento, em regime ambulatório, é prevista para um ano, submetendo-se o paciente, de 15 em 15 dias, ao xenodiagnóstico e às reações imunológicas. Finalmente, discutem e definem o eventual critério de cura da tripanossomiase americana humana.

A case is reported in which a leiomyoma of the small bowel was revealed by an ileovesical fistula, complication of a benign tumor believed to be the first one in the literature. The mechanism of fistula formation is commented upon.
