1. Data-driven topology detector for self-healing strategies in Active Distribution Networks

Abstract: The integration of distributed energy resources requires the implementation of control and automation functionalities in distribution networks, which allow them to operate in a more flexible, efficient, and reliable way. The operation of these functionalities causes topological changes on the network that must be identified since these affect protection, volt/var control, and state estimation, among others. This paper presents a data-driven topology detector for self-healing strategies in Active Distribution Networks (ADN). This approach uses machine learning (ML) techniques to obtain a trained model as a topology detector. The ML models are integrated into the Intelligent Electronic Device (IED) of ADN so that using the voltage and current signals measured locally determine the network’s topology. A features selection and tuning technique based on a multiverse optimizer are proposed to improve the ML model accuracy. This approach allows it to be implemented in decentralized architectures since each IED detects the system’s topology from local measurements and does not depend on the availability of the communication system. The proposed topology detector was validated on a modified IEEE 123 nodes test feeder considering six topology changes, five DER outages, and five load variations. The results obtained show accuracy values above 96%, which evidences a highlighted potential for real-life applications.

2. Adaptive Fault Detection Based on Neural Networks and Multiple Sampling Points for Distribution Networks and Microgrids

Abstract: Smart networks such as active distribution network (ADN) and microgrid (MG) play an important role in power system operation. The design and implementation of appropriate protection systems for MG and ADN must be addressed, which imposes new technical challenges. This paper presents the implementation and validation aspects of an adaptive fault detection strategy based on neural networks (NNs) and multiple sampling points for ADN and MG. The solution is implemented on an edge device. NNs are used to derive a data-driven model that uses only local measurements to detect fault states of the network without the need for communication infrastructure. Multiple sampling points are used to derive a data-driven model, which allows the generalization considering the implementation in physical systems. The adaptive fault detector model is implemented on a Jetson Nano system, which is a single-board computer (SBC) with a small graphic processing unit (GPU) intended to run machine learning loads at the edge. The proposed method is tested in a physical, real-life, low-voltage network located at Universidad del Norte, Colombia. This testing network is based on the IEEE 13-node test feeder scaled down to 220 V. The validation in a simulation environment shows the accuracy and dependability above 99.6%, while the real-time tests show the accuracy and dependability of 95.5% and 100%, respectively. Without hard-to-derive parameters, the easy-to-implement embedded model highlights the potential for real-life applications.

3. Low dose anti-inflammatory radiotherapy for the treatment of pneumonia by covid-19: A proposal for a multi-centric prospective trial

Abstract: Background: COVID-19 is a highly contagious viral infection with high morbidity that is draining health resources. The biggest complication is pneumonia, which has a serious inflammatory component, with no standardized treatment. Low-dose radiation therapy (LD-RT) is non-invasive and has anti-inflammatory effects that can interfere with the inflammatory cascade, thus reducing the severity of associated cytokine release and might be useful in the treatment of respiratory complications caused by COVID-19. Study design and methods: This multicentric prospective clinical trial seeks to evaluate the efficacy of bilateral lung LD-RT therapy as a treatment for interstitial pneumonia in patients with COVID-19 for improving respiratory function.This prospective study will have 2 phases: I) an exploratory phase enrolling 10 patients, which will assess the feasibility and efficacy of low-dose lung irradiation, evaluated according to an increase in the PaO2/FiO2 ratio of at least 20% at 48–72 h with respect to the pre-irradiation value. If a minimum efficiency of 30% of the patients is not achieved, the study will not be continued. II) Non-randomized comparative phase in two groups: a control group, which will only receive pharmacological treatment, and an experimental arm with pharmacological treatment and LD-RT. It will include 96 patients, the allocation will be 1: 2, that is, 32 in the control arm and 64 in the experimental arm. The primary end-point will be the efficacy of LD-RT in patients with COVID-19 pneumonia according to an improvement in PaO2/FiO2. Secondary objectives will include the safety of bilateral lung LD-RT, an improvement in the radiology image, overall mortality rates at 15 and 30 days after irradiation and characterizing anti-inflammatory mechanisms of LD-RT by measuring the level of expression of adhesion molecules, anti-inflammatory cytokines and oxidative stress mediators.Trial registration: ClinicalTrial.gov NCT-04380818 https://clinicaltrials.gov/ct2/show/NCT04380818?term=RADIOTHERAPY&cond=COVID&draw=2&rank=4.

Abstract: Introducción: el cuestionario “Assessment of Spondyloarthritis International Society Health In- dex” (ASAS-HI) fue desarrollado para medir de manera global la funcionalidad y el estado de salud en pacientes con espondiloartritis (EspA). Se han propuesto puntos de corte para determinar diferentes estados de salud que fueron poco evaluados en pacientes de la vida real. Objetivos: describir el estado de salud medido por ASAS-HI en pacientes argentinos con EspA axial (EspAax) y periférica (EspAp) en la práctica diaria y evaluar los factores asociados al pobre estado de salud. Materiales y métodos: Estudio de corte transversal, analítico y multicéntrico. Se incluyeron consecutivamente pacientes con EspAax y EspAp según criterios ASAS, de 15 centros argentinos. Análisis estadístico: se realizó estadística descriptiva, análisis bivariado y multivariado (regresión logística múltiple) para evaluar los factores asociados al pobre estado de salud (ASAS-HI ≥12). Para analizar la validez de constructo de la herramienta se realizó correlación de Spearman entre el ASAS-HI y otros parámetros de evaluación de la enfermedad. Resultados: se incluyeron 274 pacientes con EspA, con una edad media de 49 (±14) años y una duración mediana de la enfermedad de 62 meses (p25-75: 24-135), 155 (56,6%) de los pacientes eran de sexo masculino, 129 pacientes (47%) con EspAax y 145 (52,9%) EspAp. Según el ASAS-HI 119 pacientes (43,4%) presentaban buen estado de salud, 117 (42,7%) tenían estado de salud moderado y 38 (13.9%) pobre estado de salud. 6. Effect of Liquefied Petroleum Gas (LPG) on Heavy Oil Recovery Process

Abstract: Unconventional heavy oils are high viscosity and low API density which hinders process recovery from the wellbore. The injection of liquefied petroleum gas (LPG) could be an attractive alternative to improve process recovery. However, LPG dosing must be controlled in order to avoid asphaltenes precipitation. This study aimed to evaluate the feasibility of LPG injection to improve recovery of unconventional heavy oils. The study was based on the computer aided engineering using the software CMG (winprop). The physicochemical properties and composition of heavy oil and LPG were adapted from literature. Both fluids came from a Colombian oil field which had wells with average depth of 7900 ft and porosity of 17 %. The result showed that viscosity values decreased as increase in saturation pressure and temperature. It was convenient to keep the pressure under to 850 Kpa during process recovery in order to control asphaltenes precipitation. The minimum asphaltenes precipitation of 4.8 % (w/w) was obtained from heavy oil to GLP ratio of 0.64. The used of LPG showed as an attractive solvent to improve the process recovery for heavy oils. GLP injection throughout the well could be easily implemented since it is produced in situ during oil recovery.

7. Technical and Economic Analysis for Production of Bio- ammonia from African Palm Rachis

Abstract: The 95% of ammonia destined for fertilizer production via Haber-Bosch process comes from natural gas. This process is preceded by a series of stages, in which methane available in natural gas is reformed to syngas (a gas mixture consisting of H2, CO2, CO, CH4 and H2S). The stage of syngas production represents a bottleneck from the economic point of view. The natural gas availability and its commercial cost imply a high commercial price on fertilizer which in turn increases the cost per hectare of crop. Currently, gasification technology has caught attention as a waste treatment technology due to its capacity to transform large volumes of biomass into syngas with low retention time. Therefore, this study aimed to evaluate techno-economically the bio- ammonia production from syngas obtained by gasification of African palm Rachis. The study was conducted by process simulation using Aspen Tech software. The process was simulated from the collection of the raw material to conversion of synthesis gas into ammonia. The simulation comprises a pretreatment stage to obtain an optimum particle size for biomass, a stage of gasification to convert Palm rachis into Syngas, a stage of heat recovery for steam production, a gas shift (WGS) reactor to increase the concentration of hydrogen into syngas, an acid gas remover unit (AGR) to avoid poisoning of catalysts; and the final stage of Bio ammonia synthesis; this simulation was validated based on data reported in the literature. The technical analysis measured the complexity of the new process in terms of equipment and the efficiency of the proposed operations. On the other hand, economic analysis evaluated the profitability to adapt the process to the biomass. According to results, 1,630,000 metric tonnes (Mt) of palm Rachis per year produced a syngas with a concentration hydrogen of 99% which yielded 35,755 Mt yearly of Bio ammonia. It could be inferred that highest reactor conversion was 20% at a temperature of 360°C and 250 bar pressure. It was observed that the return of investment (ROI) of 13.2% was higher than the discount rate of 12% which it demonstrates the economic viability of the project. However, The ROI value was high sensitive to the cost and availability of raw materials.

8. Design of an Apparatus for Solar Drying of Farm Products

Abstract: A large part of crops is discarded due to their high production volume, and the small local market covered. Besides, commercialization of some crops is seasonal. Crops with a high content of water are perishable in a short time, so it is necessary to implement technological alternatives in order to preserve these products. One way to make better use of crops with high water content is by getting dehydrated products. The conversion of solar radiation into thermal energy for dehydration of crops is a widely known practice in agroindustry. Tropical regions receiving high solar radiation intensity are appropriate for the dehydration of crops in solar dryers. This process has proven effective for the upgrading of farm crops, and diversification of final products that may be destined for local and international market. The present work is developed as a response to the priority needs for small local farm crops that are not financially supported by government royalties, in a tropical country located in South America. The solar dehydrator described here is a modular unit to be used in non electrically interconnected zones (NIZ), and built with local materials, that makes use of convective heat to remove the moisture of previously sliced farm products. This way, this study covers the development of a simple agroindustrial process focused on the upgrading of crops, the broadening of marketing alternatives and the use of renewable energy sources. Two crops are considered as case studies for the solar dehydration: mangoes and tomatoes. Data required for the design calculations (density, moisture, solar radiation, temperature and humidity of air) were obtained experimentally, and from databases developed by governmental institutions. Microbiological analysis (total coliforms, fecal coliforms, enumeration of aerobic mesophilic microorganisms, molds and yeasts) were carried on samples before and after dehydration tests, to evaluate the adequacy of the dried products for human consumption. The results show that the solar dehydration is a feasible approach for preserving crops with a high content of water, in the region considered for this study; however, some crops require to be pre-treated before drying, due to the tendency of bacteria growth during or after the process.

9. Global Pc5 pulsations during strong magnetic storms: excitation mechanisms and equatorward expansion

Abstract: The dynamics of global Pc5 waves during the magnetic storms on 29–31 October 2003 are considered using data from the trans-American and trans-Scandinavian networks of magnetometers in the morning and post-noon magnetic local time (MLT) sectors. We study the latitudinal distribution of Pc5 wave spectral characteristics to determine how deep into the magnetosphere these Pc5 waves can extend at different flanks of the magnetosphere. The wave energy transmission mechanisms are different during 29–30 October and 31 October wave events. Further, we examine whether the self-excited Kelvin–Helmholtz instability is sufficient as an excitation mechanism for the global Pc5 waves. We suggest that on 31 October a magnetospheric magnetohydrodynamic (MHD) waveguide was excited, and the rigid regime of its excitation was triggered by enhancements of the solar wind density. The described features of Pc5 wave activity during recovery phase of strong magnetic storm are to be taken into account during the modeling of the relativistic electron energization by ultra-low-frequency (ULF) waves.

10. Spontaneous fission half-lives of actinides and super-heavy elements

Abstract: Spontaneous fission half-lives of actinide and super-heavy nuclei are calculated, using the least-action integral, through the WKB tunneling probability of the barrier that appears in the deformation landscape obtained in the macroscopic-microscopic potential-energy surface. This deformation-energy landscape is obtained using a Fourier shape parametrization with 4 deformation parameters, taking into account the nuclear elongation, left-right asymmetry, neck formation and non-axiality degrees of freedom. The collective inertia tensor entering the WKB half-life expression is given through the so-called irrotational flow approach, successfully used in nuclear fission to reproduce observables that characterize the nuclear system in the vicinity of the scission configurations, such as fragment mass or charge distributions. For a comparisons, we have also used the so-called phenomenological mass parameter depending only on the center-of-mass difference of the forming fission fragments. Our approach is shown to be able to reproduce empirical fission half-lives of all here considered nuclei to within 3 orders of magnitude.

11. Prevalencia de la enfermedad de Pompe entre pacientes con elevación de creatinin-kinasa (CK)

Abstract: Introducción: La prevalencia exacta de la enfermedad de Pompe en la población general es desconocida, y la frecuencia estimada varía entre las diferentes formas clínicas y grupos étnicos. Existe un gran número de pacientes con elevación de enzimas musculares que no tienen un diagnóstico definitivo. Un porcentaje de ese grupo de pacientes podría tener enfermedad de Pompe. Objetivos: 1) Determinar la prevalencia de la enfermedad de Pompe entre pacientes mayores de un año de edad, con elevación persistente de creatin-kinasa (CK) de causa desconocida. 2) Describir las manifestaciones clínicas y demográficas de los pacientes con CPK elevada. Materiales y métodos: Se incluyeron a todos los pacientes con aumento de CK en sangre, definida como la detección de al menos un valor >500 UI/L y otro mayor de 200 UI/L, durante un período mínimo de 1 año, entre los años 2010-2013, tomados de la base de datos del laboratorio del Hospital Italiano de Buenos Aires. Se excluyeron a los pacientes bajo tratamiento actual con agentes hipolipemiantes; pacientes tratados con agentes hipolipemiantes, en quienes la CK no se normalizó luego de suspender el tratamiento; pacientes tratados con agentes hipolipemiantes que interrumpieron el tratamiento durante un tiempo menor de 9 meses antes del período de inclusión y pacientes con enfermedad muscular inflamatoria (pacientes con criterios diagnósticos probables o definidos de Bohan & Peter). Los pacientes con diagnóstico probable de miositis por cuerpo de inclusión (biopsia no típica) podían ser incluidos. Se les realizó evaluación clínica (medición de la fuerza muscular), cuestionario sobre síntomas musculares y se les extrajo sangre para test enzimático en papel de filtro para enfermedad de Pompe. Resultados: Se revisaron 849 historias clínicas; 199 pacientes cumplieron con los criterios de inclusión de los que 110 pacientes fueron evaluados. Se pudo realizar el test enzimático en papel de filtro para diagnóstico de enfermedad de Pompe en 93 de los 110 pacientes evaluados. A 21 pacientes se les realizó electromiograma y a 10 pacientes biopsia muscular. Se pudo establecer el diagnóstico del aumento de la CK sólo en el 34,4% de los pacientes. El diagnóstico de enfermedad de Pompe se realizó en 1 paciente, mostrando una prevalencia del 1,07%. Conclusiones: La prevalencia de enfermedad de Pompe fue similar a la publicada en la literatura. Se pudo hacer el diagnóstico del aumento de CK elevada en un bajo porcentaje de pacientes (34,4%).

Abstract: The observation of ultrahigh energy neutrinos (UHEνs) has become a priority in experimental astroparticle physics. UHEνs can be detected with a variety of techniques. In particular, neutrinos can interact in the atmosphere (downward-going ν) or in the Earth crust (Earth-skimming ν), producing air showers that can be observed with arrays of detectors at the ground. With the surface detector array of the Pierre Auger Observatory we can detect these types of cascades. The distinguishing signature for neutrino events is the presence of very inclined showers produced close to the ground (i.e., after having traversed a large amount of atmosphere). In this work we review the procedure and criteria established to search for UHEνs in the data collected with the ground array of the Pierre Auger Observatory. This includes Earth-skimming as well as downward-going neutrinos. No neutrino candidates have been found, which allows us to place competitive limits to the diffuse flux of UHEνs in the EeV range and above.

Abstract: RESUMO: O estudo foi realizado entre 1997 e 2002 em 3 paÃses (EUA, Espanha e Venezuela) num primeiro grupo de 207 doentes com DPOC, e em que existiu uma mortalidade de 12%.Dos vÃ¡rios parÃ¢metros analisados e comparados entre sobreviventes e mortes, os que apresentaram maior valor preditivo de mortalidade foram: Ãndice de massa corporal (B de BMI), o FEV1 em % de referÃªncia (O de obstruÃ§Ã£o), grau de dispneia pela escala do MRC-Medical Research Council (D de dispneia) e distÃ¢ncia em metros no teste de 6 minutos de marcha (E de exercÃcio). Cada variÃ¡vel tem uma pontuaÃ§Ã£o e peso diferente, mas Ã© referido que a diferente estruturaÃ§Ã£o do Ãndice nÃ£o correspondeu diferente valor preditivo.

Abstract: Quantum and collisional effects on the unstable spectrum of a relativistic electron beam-plasma system are investigated through a two-fluids model. Application is made to the near target center interaction of the relativistic electron beam in the Fast Ignition Scenario. Partial degeneracy effects are found negligible while the most influential factors are the beam temperature and the electron-ion collision frequency of the plasma. The introduction of the latter triggers some oblique unstable modes of much larger wave length than the collisionless ones. Abstract: Background The primary aim of our study was to investigate the association between intubation timing and hospital mortality in critically ill patients with coronavirus disease 2019 (COVID-19)-associated respiratory failure. We also analysed both the impact of such timing throughout the first four pandemic waves and the influence of prior noninvasive respiratory support on outcomes.Methods This is a secondary analysis of a multicentre, observational and prospective cohort study that included all consecutive patients undergoing invasive mechanical ventilation due to COVID-19 from across 58 Spanish intensive care units (ICUs) participating in the CIBERESUCICOVID project. The study period was between 29 February 2020 and 31 August 2021. Early intubation was defined as that occurring within the first 24 h of ICU admission. Propensity score matching was used to achieve a balance across baseline variables between the early intubation cohort and those patients who were intubated after the first 24 h of ICU admission. Differences in outcomes between early and delayed intubation were also assessed. We performed sensitivity analyses to consider a different time-point (48 h from ICU admission) for early and delayed intubation.Results Of the 2725 patients who received invasive mechanical ventilation, a total of 614 matched patients were included in the analysis (307 for each group). In the unmatched population, there were no differences in mortality between the early and delayed groups. After propensity score matching, patients with delayed intubation presented higher hospital mortality (27.3% versus 37.1%; p=0.01), ICU mortality (25.7% versus 36.1%; p=0.007) and 90-day mortality (30.9% versus 40.2%; p=0.02) compared with the early intubation group. Abstract: The concern for the amount of end-of-life tires generated each year has arisen from constant research directed to their valorisation. Herein we propose a new material, which is constituted by GTR with a binder, as acoustic and also as a thermal insulator for civil engineering. The insulator can also include the fibre mat present in the tire, seldomly considered as a recyclable sub-product. To provide insight into the insulating behaviour of these materials, four mathematical models have been tested and compared with the experimental results of thermal conductivity. The Lewis-Nielsen modelization presented good accuracy with deviations of less than 3%. A statistical analysis has also been conducted on the experimental data showing that the parameter with more effect on thermal conductivity is thickness (differences up to 43%) being particle size, less important (ca 6%). In acoustic properties, different effects can be observed depending on the frequency range, being the density the most relevant. From the mathematical, statistical and experimental analysis can be deduced that good insulation properties would be achieved in materials with: low density, porous; including mat and thick. The effect of these parameters causes variations of thermal conductivity from 0.189 to 0.117 W/m·K and in sound absorption coefficient from 0.06 to 0.6.

Abstract: Background Pain is one of the most common and most difficult symptoms to manage in adult burn patients in the Intensive Care Unit (ICU). Insufficient or unsuccessful pain management can negatively affect physiological, psychological and social health in burn patients, both during and after hospitalization. Music therapy and music medicine interventions have been shown to positively affect pain and mental health in this population. This systematic review and meta-analysis provide an update of Randomized Controlled Trials (RCTs) using music therapy or music medicine interventions in adult burn patients. Methods A variety of databases were searched from their beginning to June 2020, including PsycINFO and PsycArticles (via APAsycNET), PubMed and MEDLINE (via OvidSP), Scopus, Web of Science, and the Cochrane Library. Data of all articles meeting the inclusion criteria were extracted, organized and processed according to the PRISMA guidelines. Statistical analysis was performed using Q-test and I2 statistics. Results 10 RCTs with a total of 1,061 participants were included. The results of the meta-analysis showed a statistically significant reduction of pain (I2 = 96.03%, P Conclusions This review provides preliminary evidence for the effectiveness of music interventions for adult burn patients. However, more high-quality RCTs are needed to safely establish guidelines for music therapists and other health care professionals in using music for health purposes with this population.

Abstract: Background During the first wave of the COVID-19 pandemic, shortages of ventilators and ICU beds overwhelmed health-care systems. Whether early tracheostomy reduces the duration of mechanical ventilation and ICU stay is controversial. Research Question Can failure-free day outcomes focused on ICU resources help to decide the optimal timing of tracheostomy in overburdened health-care systems during viral epidemics? Study Design and Methods This retrospective cohort study included consecutive patients with COVID-19 pneumonia who had undergone tracheostomy in 15 Spanish ICUs during the surge, when ICU occupancy modified clinician criteria to perform tracheostomy in Patients with COVID-19. We compared ventilator-free days at 28 and 60 days and ICU- and hospital bed-free days at 28 and 60 days in propensity score-matched cohorts who underwent tracheostomy at different timings (≤ 7 days, 8-10 days, and 11-14 days after intubation). Results Of 1,939 patients admitted with COVID-19 pneumonia, 682 (35.2%) underwent tracheostomy, 382 (56%) within 14 days. Earlier tracheostomy was associated with more ventilator-free days at 28 days (≤ 7 days vs > 7 days [116 patients included in the analysis]: median, 9 days [interquartile range (IQR), 0-15 days] vs 3 days [IQR, 0-7 days]; difference between groups, 4.5 days; 95% CI, 2.3-6.7 days; 8-10 days vs > 10 days [222 patients analyzed]: 6 days [IQR, 0-10 days] vs 0 days [IQR, 0-6 days]; difference, 3.1 days; 95% CI, 1.7-4.5 days; 11-14 days vs > 14 days [318 patients analyzed]: 4 days [IQR, 0-9 days] vs 0 days [IQR, 0-2 days]; difference, 3 days; 95% CI, 2.1-3.9 days). Except hospital bed-free days at 28 days, all other end points were better with early tracheostomy. Interpretation Optimal timing of tracheostomy may improve patient outcomes and may alleviate ICU capacity strain during the COVID-19 pandemic without increasing mortality. Tracheostomy within the first work on a ventilator in particular may improve ICU availability.

Persistent HPV infection has been recognized as the main risk factor for cancer development, but most studies have focused on evaluating HPV persistence in the genital region. Thus, in this work, we aimed to evaluate the prevalence of HPV in oral cavity and oropharynx in a young population, as well as the possible persistence of the infection after 12 months. Our results indicate that almost half (46.8%) of the analyzed population harbors an HPV infection either in the oral cavity or in the oropharynx. Furthermore, after 1 year of initial identification, half of them eliminated the infection, and only one person (5.26%) exhibited persistence. Interestingly, 50% of the individuals who successfully eliminated the infection acquired a new viral type, indicating that even when the primary infection is effectively eliminated by the immune system, there is a dynamic circulation of HR-HPV types that produce reinfection. This dynamic HPV infection among young individuals could influence the future establishment of cancer in some proportion of the cases. Abstract: Background The disease presentation of the severe acute respiratory syndrome coronavirus 2 infection (COVID-19) ranges from asymptomatic to fatal. COVID-19 patients with pre-existing coronary artery disease (CAD) risk factors or overt cardiovascular disease more often develop severe COVID-19, which are also related to thrombotic, inflammatory, and to viral infectivity response. We hypothesised that despite some genetic predisposition, especially in COVID-19 severity, the main determinants of fatal complications in COVID-19 patients are related to comorbidity. Purpose To determine the role of genetics and cardiovascular comorbidity in mortality from COVID-19. Methods We conducted a retrospective cohort study including 3,120 patients with positive COVID-19 test from several hospitals and primary care between February 2020 and June 2021. Among them 1,096 required hospitalization, and 121 died within 3 months after symptom onset. Standard parametric and non-parametric methods, as required, were used to compare patient characteristics by vital status. Individual genotypes for 32 CAD, 14 thrombosis, 19 inflammation, and 11 viral infectivity single nucleotide variants (SNV), as well as, 2 COVID-19 SNVs already published were tested for association with mortality with Cochran-Armitage statistics and p-values corrected for multiple comparisons. The mutually-adjusted odds ratio (OR) and 95% confidence interval (95% CI) of fatal COVID-19 was analysed for SNVs significantly associated to case-fatality, with their adverse alleles count (0, 1 or 2), and for comorbidity factors with logistic regression adjusted for age and sex. The discrimination of the models was also estimated by the area under the curve (AUC). Results Fatal and non-fatal cases' characteristics are compared in Table 1. Fatal cases had a more adverse cardiovascular and anthropometric risk profile. After correcting for multiple testing by Benjamini-Hochberg method, we observed the inflammation-related rs6993770 SNV to be significantly associated with COVID-19 fatality (p-value = 0.04). The CAD-related rs9982601 and rs2505083 SNVs, and the thrombosis-related rs7853989 SNV were moderately associated with COVID-19 fatality (p-value ≤0.1). On Figure 1 we show the adjusted OR for rs6993770 (OR: 1.02; 95% CI 1.01–1.03 per risk allele) and that for clinical factors related to COVID-19 case-fatality. The AUC of the model was 0.85 (95% CI 0.81–0.88), which not improved that of a model with clinical risk factors alone (AUC: 0.84; 95% CI 0.81–0.87). Conclusion The rs6993770 inflammation (interleukin measurement trait)-related SNV was independently associated to case fatality; however the outcome was mainly driven by age, male sex, diabetes, and glomerular filtration rate. Abstract: This is a randomized phase-2 trial aimed to compare consolidation vs. maintenance in untreated patients with follicular lymphoma (FL) responding to induction. 146 patients were enrolled from 25 Spanish institutions (ZAR2007; ClinicalTrials.gov #NCT00662948). Patients in PR or CR/CR[u] after R-CHOP were randomized 1:1 to 90Y-ibritumomab-tiuxetan 0.4 mCi/kg (arm A) vs. rituximab 375 mg/m2 every 8 weeks for 2 years (arm B). After a median follow-up of 10.55 years, 53 patients eventually progressed with a 10-year PFS of 50% vs. 56% for patients in arm A and B, respectively (HR = 1.42; p > 0.1). No significant differences were seen in OS (10-year OS 78% vs. 84.5%; HR = 1.39, p > .1). Patients receiving 90Y-ibritumomab-tiuxetan showed higher incidence of second neoplasms than those in arm B (10-year cumulative incidence 18.5 vs. 2%, respectively; p = .038). In conclusion, in FL patients responding to R-CHOP, no significant differences were found between consolidation and maintenance, although with higher late toxicity for consolidation.

Abstract: Introduction: the “Assessment of Spondyloarthritis International Society Health Index” (ASASHI) questionnaire was developed to globally measure function and health status in patients with spondyloarthritis (SpA). Cut-off points have been proposed to determine different health states that were poorly evaluated in real-life patients. Objectives: to describe the health status measured by ASAS-HI in Argentine patients with axial SpA (AxSpA) and peripheral SpA (SpAp) in daily practice and to evaluate the factors associated with poor health. Materials and methods: cross-sectional, analytical and multicenter study. Patients with SpAax and SpAp were consecutively included according to ASAS criteria, from 15 Argentine centers. Statistical analysis: descriptive statistics, bivariate and multivariate analysis (multiple logistic regression) were performed to evaluate the factors associated with poor health status (ASAS-HI≥12). To analyze the construct validity of the tool, Spearman correlation was performed between the ASAS-HI and other disease evaluation parameters. Results: 274 patients with SpA were included, with a mean age of 49 (± 14) years and a median duration of the disease of 62 months (p25-75: 24-135), 155 (56.6%) were male, 129 patients (47%) with AxSpA and 145 (52.9%) SpAp. According to the ASAS-HI, 119 patients (43.4%) had good health, 117 (42.7%) had moderate health and 38 (13.9%) had poor health. In patients with SpAp, the mean ASAS-HI value was 7 (p25-75: 3-10). Abstract: Elevated blood pressure remains the single biggest risk factor contributing to the global burden of disease and mortality. May Measurement Month is an annual global screening campaign aiming to improve awareness of blood pressure at the individual and population level. Adults (≥18 years) recruited through opportunistic sampling were screened at sites in 92 countries during May 2019. Ideally, 3 blood pressure readings were measured for each participant, and data on lifestyle factors and comorbidities were collected. Hypertension was defined as a systolic blood pressure ≥140 mm Hg, or a diastolic blood pressure ≥90 mm Hg (mean of the second and third readings) or taking antihypertensive medication. When necessary, multiple imputation was used to estimate participants' mean blood pressure. Mixed-effects models were used to evaluate associations between blood pressure and participant characteristics. Of 1 508 130 screenees 482 273 (32.0%) had never had a blood pressure measurement before and 513 337 (34.0%) had hypertension, of whom 58.7% were aware, and 54.7% were on antihypertensive medication. Of those on medication, 57.8% were controlled to <140/90 mm Hg, and 28.9% to <130/80 mm Hg. Of all those with hypertension, 31.7% were controlled to <140/90 mm Hg, and 350 825 (23.3%) participants had untreated or inadequately treated hypertension. Of those taking antihypertensive medication, half were taking only a single drug, and 25% reported using aspirin inappropriately. This survey is the largest ever synchronized and standardized contemporary compilation of global blood pressure data. This campaign is needed as a temporary substitute for systematic blood pressure screening in many countries worldwide.

Finalmente, a partir de estos resultados ofrecemos estrategias de redaccion cientifica que podrian ser utiles a investigadores expertos o en formacion EnglishThe goal of this study was to identify some of the writing processes that the members of the National System of Researchers (SNI) implement while preparing scientific papers for their further publication. We applied a questionnaire, validated by an expert committee, to 12 subjects. Some of the results we can report, for example, include that the methodology section is the first to be written; and while writing down the paper is often an individual task, reviewing the paper, prior to journal submission, is a shared task. Finally, given these results, we offer some strategies for scientific writing that may be of interest for researchers both established and beginners

Abstract: Objective. To establish the diagnostic value of lung ultrasound (LUS) in patients with rheumatoid arthritis (RA) for the detection of interstitial lung disease (ILD). Methods. A cross-sectional study was performed. Consecutive patients with RA (American College of Rheumatology/European League Against Rheumatism 2010 criteria) who had a chest high-resolution computed tomography (HRCT) performed within 12 months before inclusion, regardless of symptomatology, were included. Demographic, clinical, laboratory, and pharmacological data were recorded. Each patient underwent a LUS with assessment of B-lines (BL) and pleural irregularities (PI). HRCT was considered the gold standard for the confirmatory diagnosis of ILD. Receiver operating characteristic (ROC) curves were calculated to test the ability of LUS findings (BL and PI) in discriminating patients with ILD. Results. Abstract: Some physically based landslide models analyse pore water pressure changes due to rainfall infiltration and its effects on slope stability. The physically-based model TRIGRS has been successfully used in rainfall-induced shallow landslide assessments in different studies around the world; nevertheless, evaluating its performance in tropical mountain terrains, such as the Colombian Andes, is necessary. In this study, the TRIGRS model was applied to the La Arenosa basin (San Carlos, Colombia) and ROC (receiver operating characteristic) analysis was used to assess its effectiveness (performance) at predicting areas susceptible to shallow landslides in this tropical mountainous area. The results were compared with those obtained using the SHIA_Landslide (Simulacion Hidrologica Abierta, or SHIA, in Spanish) and SHALSTAB models in the same case study. The three models performed well, especially TRIGRS and SHIA_Landslide. The predictive results using TRIGRS were thoroughly analysed, describing the effect of the slope angle and its relationship with the estimated soil depth on the variation of the pressure head and the factor of safety (FS) during the simulated rainfall event. The high dependence of FS on soil thickness demonstrated that defining this variable must be carefully accomplished. The results suggest that TRIGRS can be a valuable tool in tropical mountain terrains, such as the Colombian Andes basins, and it can be useful despite the lack of data and the high parameter uncertainty that is common in many study areas.

46. Physically based and distributed rainfall intensity and duration thresholds for shallow landslides

Abstract: For landslide assessment in a specific small area of interest, regional thresholds may not be applicable and the application of empirical-statistical methodologies could be limited due to a lack of required databases (landslide inventories and triggering rainfall events). Rainfall thresholds for landslides have been recently derived from the implementation of physically based methodologies that usually provide a power-law equation for the study area (e.g. region or a specific basin). In this work, a new methodology for defining rainfall intensity and duration thresholds using a physically based model (TRIGRS) is presented and implemented in a tropical mountain basin of the Colombian Andes. Multiple calculations of the factor of safety for different (synthetic) rainfall events permitted the fitting of power-law equations to the sets of critical intensity and duration conditions, causing failure in each grid cell. Maps of the equation parameters (scale and shape) and the range of applicability (initial and final duration) of the grid cell thresholds were analysed. The results show that the hydraulic and mechanical properties of the soil affected the threshold curve characteristics, comparing sandy soil with silty loam; the higher strength characteristics of sandy soil meant that most extreme rainfall conditions are required to reach their thresholds. This methodology could improve landslide early warning systems in terms of analysing the specific (local) areas that are highly susceptible to landslide occurrence.

Abstract: Laboratory tests are an integral part of the diagnosis and management of patients; however, these tests are far from perfect. Their imperfections can be due to patient health condition, specimen collection, and/or technological difficulty with performing the assay and/or interpretation. To be useful clinically, testing requires calculation of positive predictive values (PPVs) and negative predictive values (NPVs). During the current global pandemic of COVID-19 (coronavirus disease 2019), multiple assays with unknown clinical sensitivity and specificity have been rapidly developed to aid in the diagnosis of the disease. Due to a lack of surveillance testing, the prevalence of COVID-19 remains unknown. Hence, using this situation as an clinical example, the goal of this article is to clarify the key factors that influence the PPV and NPV yielded by diagnostic testing, By doing so, we hope to offer health-care providers information that will help them better understand the potential implications of utilizing these test results in clinical patient management.

Abstract: In geotechnical engineering, bored-pile wall stability is evaluated using deterministic design methods based on safety factors to establish a margin against failure. In recent years, reliability-based design methods have been adopted to include uncertainty in the assessment of bored-pile wall stability as well as in the calculation of the feasible embedment depth of the walls. In this study, an expanded reliability-based design approach, along with finite element analysis, was applied to conduct parametric analyses of bored-pile wall stability. In serviceability limit state design framework, the results indicate that cohesion and groundwater level are factors that significantly affect bored-pile wall stability. Moreover, high variability in the cohesion range causes great uncertainty to determine the embedment depth of bored-pile wall. The feasible embedment depth can reach 4 times the free height considering the maximum coefficient of variation (50 %) of the cohesion. In turn, when the groundwater level is located at the retained ground surface, the horizontal displacement of the upper end of the wall reaches 15.2 mm, i.e., 0.0038 times the free height of the wall, for which the soil mobilizes active earth pressures. It was also found that the resolution of probabilistic results is highly influenced by the number of iterations in Monte Carlo simulations. Resumen En ingeniería geotécnica, la estabilidad de muros de pilas excavadas es evaluada mediante métodos de diseño determinísticos que se basan en el uso de factores de seguridad para establecer un margen para la falla. En los últimos años, se han adoptado métodos de diseño basados en la confiabilidad para involucrar la incertidumbre en la evaluación de la estabilidad de los muros, así como para el cálculo de la profundidad de empotramiento factible para los muros. En este estudio, se aplica un enfoque de diseño basado en la confiabilidad ampliada para desarrollar análisis paramétricos de la estabilidad de un muro de pilas excavadas, junto con un análisis de elementos finitos. En el marco del diseño por estado límite de servicio, los resultados indican que la cohesión del suelo y el nivel freático son factores que afectan significativamente la estabilidad del muro. Una alta variabilidad en el rango de cohesión causa una gran variabilidad en la incertidumbre para determinar la profundidad de empotramiento del muro. La profundidad de empotramiento factible puede alcanzar 4 veces la altura libre considerando el coeficiente de variación máximo (50 %) de la cohesión del suelo. Por otro lado, cuando el nivel freático se ubica en la superficie del terreno retenido, el desplazamiento horizontal del extremo superior del muro alcanza 15.2 mm, equivalente a 0.0038 veces la altura libre del muro, para el cual el suelo alcanza a movilizar los empujes activos. También se encontró que la resolución de los resultados probabilísticos está altamente influenciada por el número de iteraciones en las simulaciones de Monte Carlo.

49. Análisis y evaluación del riesgo de deslizamientos superficiales en un terreno montañoso tropical: implementación de modelos físicos simples

Abstract: El riesgo de deslizamientos superficiales expresa las pérdidas o daños causados por la ocurrencia de este fenómeno y depende de la magnitud, relacionada con su probabilidad de ocurrencia, y su efecto en los elementos expuestos, determinado por su grado de vulnerabilidad. En esta investigación se analizan y evalúan la amenaza, vulnerabilidad y riesgo de deslizamientos superficiales en un terreno montañoso tropical. Se utilizó el modelo físico SHALSTAB para el análisis de la amenaza y otros modelos de fácil implementación para calcular la vulnerabilidad física de ocho viviendas y de personas dentro de las mismas. Se calcula el riesgo de las estructuras, en términos económicos, y el riesgo de pérdidas de vidas humanas, para personas dentro de las viviendas, mediante un análisis de las consecuencias de estos elementos expuestos dada la ocurrencia de deslizamientos superficiales definidos en la evaluación de la amenaza. Esta metodología representa una herramienta que puede ser útil en planes de ordenamiento territorial y gestión del riesgo de movimientos en masa.

50. Understanding the sensitivity to the soil properties and rainfall conditions of two physically-based slope stability models

Abstract: Physically-based models have been used to assess landslide susceptibility, hazard, and risk in many regions worldwide. They have also been regarded as valuable tools for landslide prediction and the development or improvement of landslide early warning systems. They are usually validated to demonstrate their predictive capacity, but they are not deeply studied regularly to understand the sensitivity of the input variables and the behavior of the models under many different rainfall scenarios. In this research paper, we studied two distributed physically-based models for shallow landslides: SLIP and Iverson. For this, the first-order second-moment (FOSM) method was used to calculate the contribution of random input variables (soil strength, unit weight, and permeability parameters) to the variance of the factor of safety. Different intensity and duration rainfall events were simulated to assess the response of the models to those rainfall conditions in terms of the factor of safety and failure probability. The results showed that the shear strength (cohesion and friction angle, in order of significance) parameters have the largest contribution to the variance in both models, but they vary depending on geological, geotechnical, and topographic conditions. The Iverson and SLIP models respond in different ways to the variation of rainfall conditions: for shorter durations (e.g. ≤ 8 h), increasing the intensity caused more unstable areas in the SLIP model, while for longer durations the unstable areas were considerably higher for the Iverson model. Understanding those behaviors can be useful for practical and appropriate implementation of the models in landslide assessment projects. Resumen Se han implementado modelos basados en la física para evaluar la susceptibilidad, la amenaza y el riesgo de movimientos en masa en muchas regiones del mundo. También se han considerado herramientas valiosas para la predicción de movimientos en masa y el desarrollo o mejora de sistemas de alerta temprana. Por lo general, se validan para demostrar su capacidad predictiva, pero pocas veces se estudian en profundidad para comprender la sensibilidad de las variables de entrada y el comportamiento de los modelos en diversos escenarios de lluvias. En este artículo de investigación se utilizaron dos modelos distribuidos de base física para deslizamientos superficiales: Iverson y SLIP. Para ello, se utiliza el método de first-order second moment (FOSM) para calcular la contribución de las variables de entrada aleatorias (resistencia del suelo, peso unitario y parámetros de permeabilidad) a la varianza del factor de seguridad. Se simularon eventos de lluvia de diferente intensidad y duración para evaluar la respuesta de los modelos a esas condiciones de lluvia en términos del factor de seguridad y probabilidad de falla. Los resultados mostraron que los parámetros de resistencia al corte (cohesión y ángulo de fricción, en orden de importancia) tienen la mayor contribución a la varianza en ambos modelos, pero varían según las condiciones geológicas, geotécnicas y topográficas. Los modelos Iverson y SLIP responden de diferentes maneras a la variación de las condiciones de lluvia: para duraciones más cortas (por ejemplo, ≤ 8 h), el aumento de la intensidad provocó más áreas inestables en el modelo SLIP; mientras que, para duraciones más largas, las áreas inestables fueron considerablemente mayores para el modelo de Iverson. Comprender esos comportamientos puede ser útil para una implementación práctica y adecuada de los modelos en proyectos de evaluación de deslizamientos de tierra.
