1. Long-term follow-up of liver-directed, adeno-associated vector-mediated gene therapy in the canine model of hemophilia A

Abstract: Questions remain concerning the long-term efficacy, safety, and site(s) of transgene expression following adeno-associated vector (AAV) therapy. We report a long-term follow-up of 8 (male = 4, hemizygous, and female = 4, homozygous) dogs with severe hemophilia A treated with a single portal vein infusion of a B-domain–deleted (BDD)-canine FVIII (cFVIII) AAV vector (median dose = 1.25 × 1013 vg/kg, AAV2 = 4, AAV6 = 3, and AAV8 = 1). After a median follow-up of 10.8 years (8.2-12.0 years), persistent FVIII:C (median one-stage = 12.7%, chromogenic = 7.2%) was seen in all responding dogs (n = 6), with improvement in annualized bleed rates (pre = 3.9 vs post = 0.3 event per year; P = .003). Anti-AAV capsid neutralizing antibodies (nAbs) toward the dosed capsid were detected throughout the study, with limited cross-reactivity to other capsids. nAb titers for all capsid serotypes declined with time, although they remained at levels precluding redosing with the same capsid. AAV-BDD-cFVIII DNA was detected in the liver of all dogs (median = 0.15 vg per diploid genome), with lower levels in the spleen in 4 dogs (median = 0.005 vg per diploid genome). Consistent with the liver-specific promoter, BDD-cFVIII mRNA was only detected in the liver. Postmortem examination demonstrated no evidence of chronic liver disease or liver malignancy. Persistent FVIII expression and an improved bleeding phenotype was seen for more than a decade after vector delivery. This is the longest follow-up reported in a preclinical model supporting long-term efficacy and safety of AAV-mediated gene therapy.

Abstract: Background & aims: Genome-wide association studies in primary biliary cholangitis (PBC) have failed to find X chromosome (chrX) variants associated with the disease. Here, we specifically explore the chrX contribution to PBC, a sexually dimorphic complex autoimmune disease. Methods: We performed a chrX-wide association study, including genotype data from 5 genome-wide association studies (from Italy, United Kingdom, Canada, China, and Japan; 5244 case patients and 11,875 control individuals). Results: Single-marker association analyses found approximately 100 loci displaying P < 5 × 10-4, with the most significant being a signal within the OTUD5 gene (rs3027490; P = 4.80 × 10-6; odds ratio [OR], 1.39; 95% confidence interval [CI], 1.028-1.88; Japanese cohort). Although the transethnic meta-analysis evidenced only a suggestive signal (rs2239452, mapping within the PIM2 gene; OR, 1.17; 95% CI, 1.09-1.26; P = 9.93 × 10-8), the population-specific meta-analysis showed a genome-wide significant locus in East Asian individuals pointing to the same region (rs7059064, mapping within the GRIPAP1 gene; P = 6.2 × 10-9; OR, 1.33; 95% CI, 1.21-1.46). Indeed, rs7059064 tags a unique linkage disequilibrium block including 7 genes: TIMM17B, PQBP1, PIM2, SLC35A2, OTUD5, KCND1, and GRIPAP1, as well as a superenhancer (GH0XJ048933 within OTUD5) targeting all these genes. GH0XJ048933 is also predicted to target FOXP3, the main T-regulatory cell lineage specification factor. Consistently, OTUD5 and FOXP3 RNA levels were up-regulated in PBC case patients (1.75- and 1.64-fold, respectively). Conclusions: This work represents the first comprehensive study, to our knowledge, of the chrX contribution to the genetics of an autoimmune liver disease and shows a novel PBC-related genome-wide significant locus.

Abstract: Failure modes of a V-containing and a V-free AB 2 Laves phase-based metal hydride alloy were studied by the combination of X-ray diffractometer, scanning electron microscope, X-ray energy dispersive spectroscopy, inductively coupled plasma, Soxhlet extraction, and magnetic susceptibility measurement. Cells with the V-containing alloy exhibited less capacity degradation up until venting occurred in the cells, after which the capacity rapidly degraded. Cells with the V-free alloy remained linear in capacity degradation throughout the cycle life test. The failure mechanism for the V-containing alloy is related to the formation of an oxide layer that penetrates deeper into the alloy particles due to high V leaching and impedes gas recombination, while the failure mechanism for the V-free alloy is related to the continuous pulverization of the main AB 2 phase.

Abstract: Ginsenoside Rg3 is a natural active ingredient that is extracted from Korean red ginseng root. It elevates the therapeutic effect of radiotherapy and chemotherapy, but previous studies found that the application of Rg3 is heavily limited by its low bioavailability and poor absorption via oral administration. To overcome these problems, Rg3-loaded PEG-PLGA-NPs (Rg3-NPs) were prepared by the modified spontaneous emulsification solvent diffusion (SESD) method, and the physicochemical characteristics of Rg3-NPs were investigated. We treated primary glioblastoma with 50 mM Rg3-NPs for 48h. We then used gene expression arrays (Illumina) for genome-wide expression analysis and validated the results for genes of interest by means of real-time PCR. Functional annotations were then performed using the DAVID and KEGG online tools. The results showed that the Rg3-NPs are slick and uniform, the average diameter of the nanoparticles is 75-90 nm, and their entrapment efficiency is 89.7+1.7%. MTT showed that the growth of cells can be significantly inhibited by Rg3-NPs in a dose-dependent manner. FCM testing showed Rg3-NPs can be released from the conjugate nanoparticle and react with the genes in the cell nuclei, causing changes in the gene molecules. We also found that cancer cells treated with Rg3-NPs undergo cell-cycle arrest at different checkpoints. This arrest was associated with a decrease in the mRNA levels of core regulatory genes BUB1, CDC20, TTK, and CENPE, as determined by microarray analysis and verified by real-time PCR. Furthermore, Rg3-NPs induced the expression of the apoptotic and antimigratory protein p53 in cell lines. The results of the present study, together with the results of earlier studies, show that Rg3-NPs target genes involved in theprogressionoftheM-phaseofthecellcycle.Itisassociatedwithseveralimportant pathways, which include apoptosis (p53). Rg3-NPs may be a potent cell-cycle regulation drug targeting the M-phase in glioblastoma cell lines.

Abstract: Mixed electron hole and oxide ion conducting perovskite-type oxides, La 0.8 Sr 0.2 (Ga 0.8 Mg 0.2 ) 1 − x Cr x O 3 − δ (0 ≤ x ≤ 1.0) , were prepared by solid state reaction. The phase stability and the oxygen permeation properties of the oxides were examined as a function of the content of Cr. La 0.8 Sr 0.2 (Ga 0.8 Mg 0.2 ) 1 − x Cr x O 3 − δ has a perovskite related tetragonal phase with x = 0.1 to 0.8. The total electrical conductivity of La 0.8 Sr 0.2 (Ga 0.8 Mg 0.2 ) 1 − x Cr x O 3 − δ increases with increasing x . The oxygen permeation flux across the La 0.8 Sr 0.2 (Ga 0.8 Mg 0.2 ) 1 − x Cr x O 3 − δ membranes at higher temperatures increases with x up to x = 04. The maximum oxygen permeation flux of 1.6 × 10 − 7 mol − 1 cm − 2 at 1100 °C in a oxygen activity gradient of air/10 − 2 Pa is observed in La 0.8 Sr 0.2 (Ga 0.8 Mg 0.2 ) 0.6 Cr 0.4 O 3 − δ . This perovskite-type oxide is stable under an oxygen partial pressure of 7 × 10 − 10 Pa at 1000 °C.

12. Fodder Supply in Cold Season in Gobi Nomadic Area, Mongolia

Abstract: Fodder supply condition was studied at 41 nomadic families in Gobi, Southern Semi-Desert area in Mongolia, from Dec. 1994 to Apr. 1995 to determine problems in cold, feed deficient season for establishment of sustainable livestock production system. The conditions of two family groups: those located in sparse vegetation (FG1), and those in comparably dense (FG2), were also compared. Commercial concentrate feed (concentrate), hay and Zoodoi were prepared for supplementation. Zoodoi was hand-made feed made mainly of Allium mongolicum and Allium polyrrhizum. Allium mongolicum tended to be used at FG1 frequently, and Allium polyrrhizum at FG2 depend on differences of micro vegetation. 44%, 90% and 39% of families prepared 165.6 kg of concentrate, 301.6 kg of hay and 6.8 kg of Zoodoi per sheep and goat (small livestock) on the average, respectively. The ratio of families that used concentrate at FG1 was smaller than those at FG2, though there were no significant differences on the amount. More hay was fed at FG1 than at FG2, and Zoodoi tended to be fed more in the FG1 group. Recipients were mostly restricted to young, female and sick small livestock that use the feeds effectively. More families gave concentrate and hay to the young than to the females and sick. They also gave more Zoodoi to young and sick animals than to females in this area. In the FG1 group, no differences were found between recipients on the concentrate supply. More families supplied hay to young animals than to sick ones, and Zoodoi was fed more to sick animals than to young and females. On the other hand, those in the FG2 found, more families fed fodders to young than to female and sick regardless of the kinds of feeds. The amount of fodder supplementation in the studied area was restricted, but accurate techniques of nomads to adapt the situation were clarified.

Abstract: Background: It is well known that obstructive jaundice causes biliary dilatation, but sonographic (US) findings of the cystic duct in patients with obstructive jaundice are rarely reported. Methods: We reviewed US findings in 25 such patients. Results: US findings of dilated cystic duct could be divided roughly into two patterns: tortuous and tubular. The former pattern was a tortuously dilated cystic duct arising from the gallbladder neck, running predominantly deeply, turning anteromedially, and then joining the posterior face of the common hepatic duct. The latter pattern was a straight dilated cystic duct running parallel with the common hepatic duct, giving the appearance of a septated bile duct. Conclusion: A better understanding of the US findings of dilated cystic duct translates into improved biliary US diagnosis.

Abstract: Background: Appendiceal mucocele (AM) is a relatively rare disease, and its sonograms (US) have not been sufficiently analyzed. Methods: We studied the US findings of five patients with AM, with special attention to AM size, shape, internal echoes, and the mode of back echoes. Results: All five cases showed an elongated mass in the lower right abdomen. Internal echoes were present in all cases and M-mode US confirmed the movement of those echoes. The echogenecity of the lesion changed according to the frequency of the transducer used. Only one case showed posterior echo enhancement, and no case showed lateral shadowing. Conclusion: AM appears as an elongated echo-poor mass without posterior echo enhancement. The cyst wall is less distinct than what one would expect for a cyst. When encountering such a mass in the lower right abdomen, one should strongly suspect an AM. In such cases, appropriate diagnostic and therapeutic strategies are especially necessary to prevent rupture that results in development of pseudomyxoma peritonei.

Abstract: Marked lymphadenopathy around the pancreas due to lymphoma (abdominal lymphoma) occasionally can mimic a total pancreatic carcinoma on ultrasonography (US). We investigated whether US and color Doppler US allowed differentiation between those pathologies.We analyzed the US and color Doppler results of 12 cases of abdominal peripancreatic lymphoma and 21 cases of total pancreatic carcinoma.With regard to shape, echogenicity of the lesion, and mode of vascular involvement, there was no difference between groups. With regard to maximal velocities and resistive indices of the involved vessels, there was no difference between groups. However, the presence of turbulent flows in the involved vessels was seen exclusively in the pancreatic carcinoma group.The presence or absence of turbulent flow in the involved vessels is a very important finding for differentiating abdominal lymphomas from total pancreatic carcinomas.

Abstract: Accessory hepatic ducts are relatively rare biliary anomalies. They have been reported only sporadically, and very few reports have described their sonographic (US) findings. Our experience with two such cases suggested that a diagnosis of an accessory hepatic duct should be considered when US shows a fine tubular structure or an anechoic or hypoechoic oval mass with acoustic shadowing at the level between the hepatic hilus and the common bile duct. A good knowledge of the US findings will determine the diagnostic and therapeutic strategies.

Abstract: 250 nm-thick PZT films were grown on (001)lr/(001)MgO substrates by metalorganic chemical vapor deposition (MOCVD). It was ascertained that PZT films on lr substrates are epitaxially grown without the formation of large amount of lrO 2 phase. Crystal structure and the electrical properties of Pt/PZT/(001)lr/(001)MgO structure was compared with Pt/PZT/ (001)c SrRuO 3 /(001)SrTiO 3 structure with the same Zr/(Zr+Ti) ratio of 0.50. X-ray diffraction pattern was almost the same to be identified as (001)- and (100)-oriented tetragonal PZT phase. P-E hysteresis loops and saturation properties of the remanent polarization ( P r ) and the coercive field ( E c ) were also the same, because of the same volume fraction of (001)-oriented grains that contributed to polarization. On the other hand, the leakage current density of Pt/PZT/lr/MgO structure was superior to that of Pt/PZT/SrRuO 3 /SrTiO 3 structure. As a result, lr/MgO substrate is suitable for epitaxial PZT film with low-leakage and good ferroelectricity.

18. Solar desalination of saline soil for afforestation in arid areas: numerical and experimental investigation

Abstract: Afforestation in deserts and arid lands has immense potential for global carbon balance and climate change. The key to desert afforestation is efficient use of scarce water resources and requires innovative, and cost effective, as well as maintenance free technologies for desalination. In this paper, one such methodology is presented. A state of the art solar distillation system is constructed using waste material, which works as an “evaporation accelerator,” to remove salt from the root zone. The process is optimized with reference to a host of species, conventionally thought to be suitable for deserts and arid areas. The process is numerically simulated and experimentally tested under similar environments of Western Australia. The results obtained from one dimensional and two dimensional column experiments to determine the salt and water movement under desert like conditions clearly indicate that the objective of salt leaching in two weeks can be met cost effectively for a plantation without any outside supply of energy sources. After two weeks, proper drip irrigation would be sufficient to maintain the soil moisture for the survival of the plant.

Abstract: Our study of color (seven cases) and contrast (three cases) Doppler results of seven cases with gallbladder carcinoma localized in the gallbladder wall (TNM stage T1) showed that the presence or absence of blood flow signals distinguishes gallbladder carcinoma in stage T1b (muscular involvement) from tumefactive biliary sludge and that injection of contrast medium markedly increased diagnostic confidence. Thus, when color Doppler sonography is ambiguous, contrast-enhanced Doppler sonography is the next line of investigation. However, actual color Doppler sonography is still not fully capable of displaying fine blood flow signals from gallbladder carcinoma in stage T1a (mucosal involvement), and greater Doppler sensitivity is mandatory for this purpose.

Abstract: We reviewed medical and radiologic records of five patients with bone metastasis from hepatocellular carcinoma (HCC) to evaluate the role of sonography (US) in the diagnosis of this bone metastasis.The metastatic lesions were clearly visualized by US as expansive, homogeneous, soft tissue masses with bony destruction, and color Doppler US showed fine vessels within the lesions. After treatment, US showed a decrease in tumor size with an increase in echogenicity and a decrease in blood flow. Hepatic US at the time of bone metastasis showed a portal tumor thrombus in all cases.When US detects a portal venous tumor thrombus in HCC patients, attention should be directed not only to the liver but also to bone to improve patient care. US is useful not only in detecting the metastatic lesion but also in evaluating the treatment effect.

Abstract: Background: Pancreatic carcinoma arising from the uncinate process (pancreatic uncinate carcinoma) is relatively rare. We wished to define its clinical manifestations and sonographic findings. Methods: Clinical and sonographic data of eight cases were reviewed. Results: The common bile duct and the pancreatic duct were not dilated until a very late stage. The lesion mimicked a mesenteric tumor in two cases. The superior mesenteric vessels were compressed anteriorly. Computed tomography was useful, not only for confirming the pancreatic uncinate origin of the lesion but also for determining precisely the mode of mesenteric vascular involvement. Conclusion: Knowledge of these unusual sonographic findings can determine the diagnostic strategy in pancreatic uncinate carcinoma.

Abstract: Macronodular hepatic deformity on normal liver is very rare. We present nine such cases and try to define the characteristic clinical and ultrasound (US) findings.In the left lobe, the lateral segment was replaced by multiple nodules and the medial segment was very atrophied and irregularly shaped. Compared with the left lobe, the right lobe showed very few abnormalities except for segment 6, which showed a macronodular deformity. These nodules, regardless of diameter or lobe, showed a relatively homogeneous internal structure and were isoechoic relative to the surrounding hepatic parenchyma. These multinodular changes on US corresponded to multiple regenerative nodules identified at laparoscopic evaluation. On color Doppler US and angiography, the major intrahepatic vessels were patent in all cases.Although relatively rare, the macronodular hepatic deformity on normal liver collected in our series showed a characteristic appearance by US. A good understanding of its characteristics may help sonographers in differentiating it from other, more common hepatic deformities.

Abstract: Background: Neurofibromatosis 1 (NF1) has been studied from many viewpoints, but its abdominal involvement has rarely been reported. Sonography (US) is now the initial diagnostic tool for abdominal exploration, which prompted us to determine the clinical manifestations and US findings of abdominal involvement in NF1. Methods: We analyzed the US findings and clinical data of eight NF1 cases with abdominal involvement. Results: Abdominal involvement included neurofibromatous tumor growth in the liver, mesentery, and retroperitoneum, in addition to mesenteric leiomyomatosis and gastric carcinoma. Color Doppler US was useful not only in detecting blood flows in the lesions but also in preventing hazardous vascular injury during tumor biopsy. Conclusion: A better understanding of the clinical manifestations and US findings of abdominal involvement in NF1 translates into improved NF1 patient care.

Abstract: The term for re-entrainment to a new light-dark cycle in streptozotocin (STZ)-induced diabetic rats was significantly longer than that in control rats. In STZ-induced diabetic rats, the same level of phase delay in the suprachiasmatic nucleus neuronal firing rhythm was observed in control rats after glutamate application. Therefore, 5-HT function in the hypothalamus is thought to be decreased in STZ-induced diabetic rats. These results suggest that postsynaptic neuronal function is still maintained in the suprachiasmatic nucleus of STZ-induced diabetic rats. Therefore, 5-HT mechanisms may play an important role in the maintenance of this function.

Abstract: Background and methods: We tried to determine the role and problems of gray-scale sonography (US), computed tomography (CT), and color Doppler sonography in the diagnosis of splenic lymphangioma on the basis of our experience with seven adult cases with this relatively rare tumor. Results: (1) The whole spleen was replaced by a collection of cysts of different sizes with or without calcifications in six patients. In these patients, color Doppler sonography showed the intrasplenic arteries and veins running along the cyst walls. (2) The enlarged spleen occupied the whole upper abdomen and contained numerous small cysts in one patient. The patient was initially diagnosed as having a pancreatic tumor because of the location, but color Doppler sonography clearly demonstrated two vessels (artery and vein) running parallel from the center of the mass. This characteristic vascular structure led to the determination that the mass was the markedly enlarged spleen. (3) The splenic lesion was isolated in six patients but was associated with mesenteric and pleural lymphangioma in one symptomatic patient. Conclusions: (1) When US shows multiple cysts of different sizes in the spleen, the diagnosis of splenic lymphangioma is not difficult to make with US and CT alone. (2) Color Doppler sonography is a very useful tool to increase diagnostic confidence because it demonstrates the vasculature of the mass. (3) When examining patients with splenic lymphangioma, one should consider the possibility of multiorgan involvement.

Abstract: This study was undertaken to evaluate the role of color Doppler sonography in the preoperative assessment of vascular involvement in patients with pancreatic carcinoma. Twenty-six pancreatic carcinomas were investigated with color Doppler sonography and angiography, and the results of these examinations were compared with those of surgical findings. Color Doppler sonography was more sensitive than angiography in depicting vascular involvement of carcinoma. Thus, it seems rational to perform a preoperative assessment in suspected pancreatic carcinoma patients initially with color Doppler sonography to improve patient management.

28. Effectiveness of Photodynamic Therapy and Nd-YAG Laser Treatment for Obstructed Tracheobronchial Malignancies

Abstract: Since 1980, advanced lung carcinomas were treated with palliative laser therapy for the purpose of opening the endobronchial stenosis and obstruction by either photodynamic therapy (PDT) or Nd-YAG laser treatment at Tokyo Medical University. A total of 258 lesions were treated, 81 by PDT and 177 by Nd-YAG laser treatment. PDT achieved effective results in 61 (75%) of 81 lesions. In the Nd-YAG laser group, 143 (81%) of 177 lesions showed effective results. When the tumor was located in the trachea or main bronchi, effective results were obtained in 73% (19 of 26) of cases treated by PDT and in 93% of cases (64 of 69) treated by Nd-YAG laser. However, in cases in which the tumor was located in lobar or segmental bronchi, the tumor response was effective in 76% (42 of 55) of PDT-treated patients and 73% (79 of 108) of Nd-YAG laser-treated patients. With a mortality rate of 0%, the greatest advantage of PDT over Nd-YAG treatment was safety. Considering complications, PDT seems to be useful for obstruction of lobar and segmental bronchus. Nevertheless, when deciding among alternative therapies, physicians treating patients with advanced lung carcinoma should give careful consideration to the benefit and complications of both laser therapies and decide the most suitable modality.

Abstract: Laser endoscopic surgery, especially the effectiveness of photodynamic therapy (PDT) using Photofrin as a photosensitizer, has now achieved a status as effective treatment modality for lung cancer. Twenty-six lung cancer patients received the preoperative PDT for the purpose of either reducing the extent of resection or increasing operability. Bronchoscopical PDT is performed with topical anesthesia approximately 48 h after the intravenous injection of 2.0 mg/kg body weight of Photofrin. Operation was performed 2–9 weeks after initial PDT. The initial purpose of PDT, i.e. either to reduce the extent of resection or convert inoperable disease to operable status, was achieved in 22 out of 26 patients treated. The survival rate of T3 (main bronchus invasion) cases treated by surgery alone increased significantly from 50.9% to 60.0% with the application of preoperative PDT. This remarkable result may imply that this new option of PDT as preoperative laser irradiation may contribute to the management of advanced lung malignancy.

Abstract: Aim and Methods : In 1850 patients, high-frequency (HF: 7.5–10 MHz) annular array US examination was performed to evaluate the value of HF annular US in the detection of pathologies of the anterior abdominal wall. Results : HF annular US revealed pathological lesions (metastasis, benign tumor, lymphoma, hematoma, fistula, hernia, abscess) in 37 patients. The lesions were detected by conventional medium-frequency (3.5–3.75 MHz) US in only 8 of 37 cases (21.6%). HF annular US added information that altered clinical management in 19 cases (51.4%), predominantly in cases with tumors and fistulas. Conclusion : The addition of HF annular US to a conventional US examination increases the detectability of small, but clinically important pathologies in the anterior abdominal wall.

Abstract: Two cases of small bowel (S-B) varices associated with portal hypertension, one with liver cirrhosis and one with portal thrombus, are reported. Detection of S-B varices has been a challenging task and several invasive diagnostic techniques have been used for this purpose. However, in our cases, color Doppler sonography revealed the S-B varices supplied by the superior mesenteric vein and draining to the iliac (one case) or ovarian vein (other case), which helped to establish an early appropriate diagnostic and treatment plan.

Abstract: The aim of this study was to reevaluate the causes and sites of intrahepatic venous collaterals and to determine the role of color Doppler sonography in the diagnosis of this relatively rare vascular abnormality.Real-time color Doppler sonography was used to study 21 patients with intrahepatic venous collaterals. The cause, distribution, and clinical manifestations of collaterals were determined, and Doppler waveforms obtained from the collaterals were also analyzed.First, the causes of intrahepatic venous collaterals were divided roughly into two groups according to the presence or absence of veno-occlusions. The former group included liver tumors (six cases), primary Budd-Chiari syndrome (five cases), and metastatic adrenal tumors invading the inferior vena cava (two cases). The latter group consisted of diaphragmatic hernia (three cases), Osler-Weber-Rendu disease (two cases), and congestive liver (one case). The cause was not determined in two cases. Second, venous collaterals were distributed throughout the entire liver in primary Budd-Chiari syndrome but localized in the other cases. Third, Doppler waveforms of the collaterals were divided into two patterns: flat flow and multiphasic flow. Flat flow pattern was seen in patients with veno-occlusive diseases, and multiphasic flow pattern was seen in patients without veno-occlusive disease.The relationship between intrahepatic venous collaterals and veno-occlusive diseases has been emphasized in the literature, but the results of our series showed that they occurred under a wide variety of conditions, even without veno-occlusive diseases, including diaphragmatic hernia and Osler-Weber-Rendu disease. The analysis of the Doppler waveforms of the collaterals was useful in differentiating those due to veno-occlusive diseases and those not.

Abstract: Anomalous position of the gallbladder is relatively rare and has been reported only in isolated case reports. We tried to determine its ultrasound (US) findings on the basis of 18 such patients. In the left-side gallbladder group (nine patients), the gallbladder was imaged as an oval cystic mass in front of the pancreas. In all patients, the narrow neck of the gallbladder was clearly detected by US in the usual location before the main portal vein. Four of nine patients had small gallbladder stones. The retrohepatic gallbladder group (four patients) showed marked atrophy of the right lobe of the liver. Two patients had multiple gallstones in the bile ducts of the right lobe. All patients in the suprahepatic retrohepatic gallbladder group (four patients) were cirrhotic, and the anterior segment of the right lobe was markedly atrophied. In the intercostal scan, the gallbladder mimicked a perihepatic fluid. In the floating gallbladder group (one patient), the gallbladder was imaged as a cystic mass in the anterior abdominal wall. Surgical intervention showed a severely inflamed gallbladder, with small stones adhered into the anterior abdominal wall and partially ruptured. Knowledge of the wide range of US findings of malposition of the gallbladder helps in avoiding misdiagnosis.

Abstract: A simple, selective and sensitive high-performance liquid chromatographic method with fluorescence detection is described for the simultaneous determination of catecholamines and 5-hydroxyindoleamines. The method is based on the derivatization of the amines with benzylamine in the presence of potassium hexacyanoferrate(III) under mild conditions. The resulting fluorescent derivatives are separated on a reversed-phase column, Cosmosil 5C18, using a mixture of acetonitrile and 10 mmol l-1 acetate buffer (pH 6.0) (35 + 65 v/v) as mobile phase, and are detected spectrofluorimetrically at 480 nm with excitation at 345 nm. The detection limits (signal-to-noise ratio = 3) for the amines are 2.8-236.0 fmol for an injection volume of 50 microliters. The method was successfully applied to the determination of 5-hydroxyindol-3-ylacetic acid, serotonin, norepinephrine and epinephrine in human urine. The method is simple and does not require clean-up of urine samples.

Abstract: A highly sensitive and rapid high-performance liquid chromatographic method for the determination of glyoxal, methylglyoxaJ. diacethyl, and 2, 3-pentanedione in fermented foods is described. After extraction of the compounds with methanol, the compounds in the extract are converted into the corresponding fluorescent derivatives by reaction with 1.2-diamino-4.5-methylenedioxybenzene. a fluorogenic reagent for α-dicarbonyl compounds. The derivatives are separated on a reversed-phase column (L-column ODS) with isocratic elution using acetonitrile −0.5M ammonium acetate, and are detected fluorimetrically. The detection limits are 11.6–13.8 fmol per 10-μl injection for all the compounds at a signal to noise ratio of 3.

Abstract: Mesenteric lipoma (ML) is a relatively rare disease that has been very infrequently reported. We present the clinical data and medical imaging results of an asymptomatic case with ML incidentally detected by sonography (US). On US the lesion was imaged as a well-demarcated hypoechoic mass with multiple linear echoes. The mass also changed location under probe compression. The lesion was imaged as a clearly demarcated non-enhanced homogeneous fat-density mass on CT. Color Doppler US and angiography confirmed the avascular nature of the mass. Knowledge of the US findings in this case is useful for the diagnosis of ML.

Abstract: Mycotic abdominal aneurysm is a relatively rare disease and its sonographic (US) findings have rarely been reported. We present a case of mycotic aneurysm of the common iliac artery in which the initial US showed a slightly wall-thickened abdominal aorta with gas echoes in the wall, a second US 6 days later showed a rapid increase in aortic size, and an emergent surgical intervention showed a mycotic aneurysm. This interesting observation suggested that the presence of gas echoes is a useful US finding indicating the beginning of a mycotic aneurysm and, when encountering this US finding, that emergent treatment should be considered to prevent delay in patient management.

Abstract: We present five cases of chronic hepatic porphyria (CHP) without skin lesions, in whom a multinodular fatty change in the liver was incidentally detected by ultrasound. Discovery prompted further examinations, leading to the diagnosis of CHP. Although relatively rare, the possibility of CHP must be considered when encountering patients with irregular (multinodular) fatty infiltration of the liver of unknown cause. We also discuss the differential diagnosis.

Abstract: We investigated packet transmission schemes for TDMA and CDMA systems under a condition that all systems assume the same bandwidth. In CDMA systems, packet dropping probabilities are improved by introducing carrier sensing and dividing frames into slots. The performance is improved further with a dynamic channel allocation method where packet transmission slot is allocated by base station. In TDMA systems, the performance without TPC is superior to that with TPC due to the capture effect. TDMA system shows better performance than CDMA system under the assumed conditions.

Abstract: Integrated voice and data transmission enhances the efficiency of channel usage. When it is applied to a multi-cell system, we need a channel assignment method that decreases interference between cells. Voice calls require real time transmission with higher priority over data calls. We investigate a method that decreases the interference between cells by transmitting data packets with priority for the silent periods of circuit-switched-voice communication. Computer simulation results show characteristics such as interference rates, blocking rates, forced terminating rates, and hand-off rates are improved with this method.

Abstract: Retroperitoneal cyst (RC) is a relatively rare disease, and its sonographic (US) findings have not been analyzed sufficiently.We studied US findings of five patients with RC, with special attention to location, size, shape, internal echoes, the presence or absence of lateral shadowing, and the mode of back echoes.In all cases, the cyst was situated behind (four cases) or lateral to (one case) the pancreas: behind or lateral to the pancreatic head in two cases, behind the pancreatic body in one case, and behind the pancreatic tail in two cases. Four cases showed a round mass (three cases) or multiple round mass (one case). Internal echoes were present in those cases and showed a "pseudo-solid" pattern. In those cases, M-mode US confirmed the movement of those internal echoes. In the remaining case, the lesion was imaged as an irregularly shaped multilocular mass. No case showed posterior echo enhancement, and no case showed lateral shadowing. No blood flow signals from the lesion were seen.Unlike ordinary cysts, RC usually is imaged as a round mass behind the pancreas, with dense internal echoes without lateral shadowing or posterior echo enhancement, which presents a "pseudo-solid" pattern. A diagnosis of RC should be considered when encountering a mass with such US findings.

Abstract: Background and Methods: Renal arteriovenous malformation (RAVM) is a relatively rare congenital disease. Although sonography (US) currently is the first diagnostic tool for examining the kidney, its US and color Doppler findings have seldom been reported. We reviewed the clinical manifestations and US results of five cases of RAVM to clarify the role and limitations of US in the diagnosis. Results: The lesions were solitary in all cases, and the affected side was the right in four cases and the left in one case. In four cases, the patients complained of hematuria, but the remaining case had no symptoms. US did not detect the lesion, but in all cases color Doppler US showed a focal vascular lesion with posterior color spots. US reexamination with knowledge of the Doppler results did not show any focal lesion. Conclusion: US was not diagnostic for RAVM, and color Doppler US should be performed immediately in patients with hematuria.

Abstract: Background Exercise in a hot environment can cause loss of water and electrolytes, which conveys tress on homeostasis. Objective To maintain appropriate hydration status in such situation, it expects to be suitable for fluid replacement to drink oral rehydration solutions (ORS) which have higher sodium concentration and higher absorptive of water in the intestine than general purpose sports drinks (SPD). Design Randomized crossover study. Setting University SUMO League. Participants 10 members of a university SUMO club voluntarily participated after providing written informed consent. Interventions Participants ingested SPD or ORS with an equalized exercise load in a randomized crossover manner at a wet bulb globe temperature of 27–30°C. Main outcome measurements Body weight, water distribution with bioelectrical impedance analysis, vital signs, blood analysis and visual analogue scale (VAS) for fatigue and thirst were evaluated between the two drinks. Values are expressed as mean±SD (SPD vs. ORS). Results Differences were observed 15 min after training in the changes from the baseline of serum total protein (0.6±0.3 vs. 0.3±0.2 g/dL, P=.02), serum sodium (–1.4±2.1 vs. 0.2±2.0 mEq/L, P Conclusions The ORS rather than the SPD were shown to be more suitable to maintain hydration status in a hot environment of SUMO training. The intake of ORS should be considered upon exercise accompanied with marked perspiration.

Abstract: Background: Adult cases of choledochal cyst due to anomalous pancreaticobiliary duct junction have been rarely reported. At present, sonography (US) is the first tool for diagnosing biliary disorders. The aim of this study was to reevaluate the US findings of choledochal cysts due to anomalous pancreaticobiliary duct junction in adults. Methods: We reviewed the clinical manifestations and US findings of 12 such adult cases confirmed by endoscopic retrograde cholangiopancreatic ductography (ERCP). Patients were assigned to three groups: (a) associated with biliary carcinoma (two cases), (b) associated with choledocholithiasis (one case), and (c) not associated with other abdominal diseases (nine cases). Results: Patients in group c were asymptomatic, and the lesions were detected incidentally detected by US. In contrast, patients in group a sought medical care because of symptoms such as jaundice and those in group b sought medical attention because of abdominal pain. The diameter of a dilated bile duct on US was considerably less than that of ERCP (ERCP: 26–58 mm, mean = 37.6 mm; US: 13–32 mm, mean = 21.8 mm). Its diameter changed significantly under probe compression when the dilated bile duct took a purely cystic form but changed very little when it took a tubular form. Conclusion: Unlike cases in children, adult cases of choledochal cyst are generally asymptomatic. Careful US observation of the bile duct is thus expected to detect asymptomatic adult choledochal cysts cases. Cyst diameter can change significantly under probe compression, so it is important not to compress the bile duct during routine US examination.
