1. Model-simulated hydroclimate in the East Asian summer monsoon region during past and future climate: a pilot study with a moisture source perspective

Abstract

Here we present a pilot study of the sensitivity of summer monsoon precipitation in the Yangtze River Valley (YRV; 110–122∘ E and 27–33∘ N, eastern China) to climatic boundary conditions from the Last Glacial Maximum (LGM), pre-industrial conditions, and the Representative Concentration Pathway 6 emission scenario from two different climate models. Using a quantitative Lagrangian moisture source diagnostic based on backward trajectories, we are able to interpret changes in precipitation amount and seasonality in terms of processes at the source regions and during transport that contribute to YRV precipitation. Thereby, we gain insight into influential processes and characteristics related to precipitation variability and the sensitivity of the summer monsoon hydroclimate in East Asia to boundary-condition changes in models. Comparing 10-year time slices similar to present-day conditions from the NorESM1-M and CAM5.1 models to reanalysis data reveals overall similar moisture source regions, albeit with a tendency for a more local precipitation origin in the climate models. The general characteristics of the moisture sources and moisture transport in the YRV are relatively stable across different climate forcings, both concerning the mean location of source regions, transport distance, and the relative contributions of moisture from land and ocean areas. Changes regarding regional precipitation contributions from the East Asian continent indicate that precipitation recycling responds to different climate forcings. We interpret these findings such that models to first order respond with a scaling rather than reorganisation of the hydroclimate to climatic forcing, while land–atmosphere interactions play an important, but secondary, role. If the model simulations are accurate, the moisture source regions and thus the general processes of precipitation in the YRV could remain relatively stable across different climates. However, some differences in moisture source conditions are larger between the different climate models than between different climatic boundary conditions in the same model. It may therefore be possible that current climate models underestimate the potential for non-linear responses to changing boundary conditions, for example due to precipitation recycling. Although limited by the relatively short analysis period, our findings demonstrate that the diagnosis of moisture sources provides a useful additional perspective for understanding and quantifying precipitation mechanisms and the hydroclimate simulated by models and enables more detailed evaluation of model simulations, for example using paleoclimate records.

2. Modeled Arctic sea ice evolution through 2300 in CMIP5 extended RCPs

Abstract

Almost all global climate models and Earth system models that participated in the Coupled Model Intercomparison Project 5 (CMIP5) show strong declines in Arctic sea ice extent and volume under the highest forcing scenario of the representative concentration pathways (RCPs) through 2100, including a transition from perennial to seasonal ice cover. Extended RCP simulations through 2300 were completed for a~subset of models, and here we examine the time evolution of Arctic sea ice in these simulations. In RCP2.6, the summer Arctic sea ice extent increases compared to its minimum following the peak radiative forcing in 2044 in all nine models. RCP4.5 demonstrates continued summer Arctic sea ice decline after the forcing stabilizes due to continued warming on longer timescales. Based on the analysis of these two scenarios, we suggest that Arctic summer sea ice extent could begin to recover if and when radiative forcing from greenhouse gas concentrations were to decrease. In RCP8.5 the Arctic Ocean reaches annually ice-free conditions in seven of nine models. The ensemble of simulations completed under the extended RCPs provide insight into the global temperature increase at which sea ice disappears in the Arctic and the reversibility of declines in seasonal sea ice extent.

What: Scientists from 13 countries involved with modeling and observing the coupled high-latitude weather and climate system discussed our current understanding and challenges in polar prediction, extreme events, and coupled processes on scales ranging from cloud and turbulent processes, from micrometers and a few hundred meters to processes on synoptic-scale weather phenomena and pan-Arctic energy budgets of hundreds to thousands of kilometers. Abstract

Background In 2014 a complex neurological syndrome with parasomnia, sleep breathing dysfunction and variable bulbar symptoms associated with antibodies to IgLON5, a neuronal cell-adhesion protein, was described (Sabater et al., 2016). Postmortal studies revealed tauopathy predominately involving the hypothalamus and tegmentum of the brainstem (Gelpi et al., 2016). To the best of our knowledge, there are 16 published cases so far. Methods (clinical presentation) We report a 72-year-old highly educated male Caucasian patient with a 6 year history of parasomnia with RBD in the beginning, following of multiple falls, parkinsonism, hypomania, frontal lobe disorder and impulse control disorder (ICD; leading to sentence of detention) with aggressive tendencies. By transcranial ultrasound hyperechogenicity of substantia nigra was demonstrated, FDG-PET imaging showed lower 18F-FDG activity in temporal lobes. Thus idiopathic Parkinson’s disease was assumed. Due to neuropsychiatric complications the antiparkinson medication was gradually reduced and eventually discontinued in 2014. Interestingly, there were no relevant changes in symptoms of parkinsonism thereafter. In 2016, after acute coronary syndrome the patient developed a sudden bilateral vocal cord paresis with stridor leading to intubation and following tracheotomy. Subsequently, he showed dysphagia, increasing rigidity, fluctuating level of consciousness with intermittent hypoventilation and hypercapnia, and autonomic dysregulation. Critical decompensation with need of short CPR occurred twice, continuous monitoring with mechanical ventilation was required. Results (workup and treatment, Table 1 ) Brain MRI showed iron accumulation in globus pallidus and dentate nucleus, and a small pineal cyst ( Fig. 1 a). The initial multiple EEGs were normal. Diverse EEGs from 2016 registered generalized slowing with intermittent hyperexcitability; nonconvulsive status epilepticus was captured once ( Fig. 1 b). Laboratory tests displayed neutrophil leukocytosis with MGUS IgG-kappa and -lambda. In serum (1:100) and in CSF (1:10) IgLON5 antibodies (IgG subclass) were detected. HLA-typing revealed HLA-DRB1∗10:01 and HLA-DQB1∗05:01 alleles. Tumor screening (thoracic and abdominal CT, bone marrow examination) did not detect malignancy. Because of suspected IgLON5-syndrome with autoimmune origin an immunosuppressive therapy with initial IV methylprednisolone pulse therapy following oral prednisolone (75 mg/d) and rituximab was initiated. Subsequently, after achieving better vigilance, it was possible to start weaning again. Conclusion Parasomnia, ICD, beginning frontal lobe syndrome with parkinsonism, multiple falls and bulbar symptoms is a new combination in IgLON5 antibody-linked syndrome. A connection to neurodegeneration with brain iron accumulation (NBIA) is possible and requires further investigation.

Abstract

Under greenhouse warming, climate models simulate a weakening of the Atlantic Meridional Overturning Circulation and the associated ocean heat transport at mid-latitudes, but an increase in the ocean heat transport to the Arctic Ocean. These opposing trends lead to what could appear to be a discrepancy in the reported ocean contribution to Arctic amplification. This study clarifies how ocean heat transport affects Arctic climate under strong greenhouse warming using a set of 21st century simulations performed within the Coupled Model Intercomparison Project (CMIP5). The results suggest that a future reduction in subpolar ocean heat loss enhances ocean heat transport to the Arctic Ocean, driving an increase in Arctic Ocean heat content and contributing to the intermodel spread in AA. The results caution against extrapolating the forced oceanic signal from the mid-latitudes to the Arctic.

Abstract

Myelodysplastic syndromes (MDS) are a heterogenous group of clonal stem cell disorders which generally occur in older adults but may also affect children. Primary MDS should be distinguished from secondary MDS associated with antineoplastic or immunosuppressive therapy (t-MDS), exposure to toxic compounds, or genetic disorders. The establishment of a neoplastic clone is reflected by dysplastic features and impaired function which may affect all three hematopoietic cell lineages. The ineffective hematopoiesis which causes bone marrow failure is accompanied by peripheral blood cytopenia and is considered to result from increased apoptosis, at least in the less advanced MDS stages. The elucidation of the molecular pathogenesis of MDS has provided evidence that chromosomal abnormalities are present in about 50% of patients with primary MDS. They include numerical aberrations such as monosomy 5 or 7, trisomy 8, loss of the Y-chromosome and structural abnormalities such as deletion of the long arm of chromosome 5 (5q-syndrome), 7, or 8. Based on the percentage of blasts (5%, 5-20%, 20-30%) and the presence of15% ringed sideroblasts for marrows with5% blasts, the French-American-British (FAB) classifies MDS into 4 morphologic categories: refractory anemia (RA), refractory anemia with excess of blasts (RAEB), refractory anemia with excess of blasts in transformation (RAEB-t), and refractory anemia with ringed sideroblasts. The fifth morphologic type is chronic myelomonocytic leukemia characterized by peripheral blood monocytosis (1x10(9)/l). However, a modification of this classification will be proposed by the World Health Organization, with the intention of lowering the threshold for the diagnosis of AML from 30% to 20% blast cells. In patients presenting with cytopenias suggesting impaired hematopoiesis, the initial diagnosis depends mainly on the cytological evaluation of bone marrow and blood smears and the histological findings of trephine bone marrow biopsy. In a retrospective analysis we evaluated the occurrence of the distinct FAB-categories as percentage of the total number of MDS cases diagnosed at the Institute of Pathology of the University of Freiburg. A total of 63% fullfilled the criteria of RA/RARS, 17% of RAEB, 14% of RAEB-t, and 6% of CMML. A fibrotic variant of MDS was observed in 7.67% of all cases, ranging from 2.34% in RA up to 15. 42-15.84% in the categories which did not show significant differences with regard to myelofibrosis. The histologic evaluation of a trephine bone marrow biopsy is of critical importance for the evaluation of fibrotic or hypocellular MDS since these patterns are not reflected by the cytological examination. The combined cytological and histological diagnosis of bone marrow and peripheral blood is a reliable tool for the initial diagnosis of MDS. In addition, cytogenetic and molecular analysis should be performed. Presently, the risk of leukemic transformation is evaluated using the International Prognostic Scoring System for MDS, which is the sum of the scores of bone marrow blasts, karyotypes and cytopenia. In the context of clinical trials therapeutic modalities should be considerd according to the age and the general performance state and the prognostic scores of individual patients.

Abstract

Klinisches Merkmal gastrointestinaler Stromatumoren (GIST) ist das variable biologische Verhalten, welches von benignen Zufallsbefunden bis zu metastasierenden Tumoren reicht. In dieser Arbeit wurden klinischer Verlauf und pathohistologische Parameter von 26 Patienten retrospektiv untersucht mit dem Ziel, Prognosefaktoren zu definieren. Das Risiko eines Tumorrezidives war bei Tumoren > 4 cm signifikant (p = 0,024) erhoht. Ein erhohter Proliferationsindex gemessen mit dem Anti-Ki-67-Antikorper MIB-1 war bei MIB-1 > 3 auch mit einer signifikant (p = 0,0031) gesteigerten Rezidivrate assoziiert. Der MIB-1 Index erscheint daher zur Abschatzung der Prognose geeignet.

Abstract

Head mounted displays (HMD) provide one means of displaying virtual environments. This paper assesses the state of HMD technology, with respect to the Virtual Reality (VR) goal of creating an environment which matches user's experience with the real world. We find that current HMDs fall short in important characteristics, such as resolution, field of view, and physical attributes. A system designer, familiar with these limitations, should be able to provide a suitable immersive virtual environment with the inclusion of appropriate graphics rendering techniques and virtual environment input/output devices.

Abstract

Fifty-five percutaneous nephrostomies in 40 patients under ultrasound guidance are reported. Twenty-six procedures were performed using a compound scanner and 29 were performed under real-time guidance. Fifty of the 55 procedures were successful. The puncture modalities of both techniques are compared and it was concluded that only real-time guidance permits the continuous two-dimensional visual control of the procedure. It is, therefore, the safer and more precise technique for establishing a percutaneous nephrostomy.

Abstract

A report is given on the method and results of ultrasound-guided nephropyelostomy which has been performed here for several years without open surgical intervention. The method has a low rate of complications and can be applied in severely ill patients, too. The problems of ureteral obstruction are quickly eliminated, even if the tumor has not been removed by chemotherapy, radiotherapy, or surgery. The authors report on 47 interventions performed in 36 patients.

Abstract

99 ultrasonographically-guided percutaneous nephropyelostomies (PNP) were done in 77 patients. Average age at operation was 60 years, most often PNP was done in cancer patients. In 26 cases compound-scanning technique and in 73 cases real-time technique was used. Real-time ultrasound imaging of the kidney is superior to compound-scan technique and allows dynamical imaging of percutaneously introduced canulas. Thus guidance of puncture technique and security of reaching the calix system have been optimized. Correct placement of nephrostomy catheter was achieved in 88.5% using compound-scanning technique compared to 92% using real-time ultrasonography. Moreover puncture of the calix system by needle was successful in every case using real-time ultrasonography. Complication rate was 2%. In one patient the calix system was perforated and one with macrohematuria required open surgery. Real-time ultrasound technique enables us to perform controlled percutaneous nephropyelostomy easily without hazardous complications.

Abstract

Ten patients with presacral tumors proved by histologic and computed-tomographic investigations underwent ultrasound scanning. The sound beam was directed through the ischial foramen along longitudinal, transverse, and oblique planes. Nine of 10 tumors localized in the sacral fossa were identified sonographically. In six of 10 instances, tumor demarcation was possible. Sonographic evaluation was impaired because of distance between the transducer and the target due to a patient's large volume and by the cranial location of the tumor. The transgluteal approach is suggested for sonographically guided percutaneous biopsy.

Pemphigus vulgaris is an autoimmune disease characterized by suprabasal blisters with acantholysis, which has a fatal course in a large number of untreated patients. Systemic corticosteroid therapy is considered first-line therapy. Adjuvant treatment with the goal of sparing corticosteroids include, among others, dapsone. This drug is not without side effects and its use requires clinical and laboratory control. We present a patient with PV initially managed with suboptimal dose of prednisone, evolving into drug-induced hepatitis after introduction of dapsone.

Abstract

We introduce a new, not yet described cord-like structure of the knee of children associated with discoid or enlarged menisci. This structure was responsible for knee pathology (loss of extension) in 3 cases. The patients (5 to 8 years of age) complained of knee pain without skeletal abnormality or trauma. Magnetic resonance imaging revealed enlarged or discoid lateral menisci. The patients showed increasing limp with limited range of motion. Lack of extension was between 10 degrees and 45 degrees and continued under anesthesia. During arthroscopy, the menisci and the anterior and posterior cruciate ligaments (ACL, PCL) showed no abnormality other than the variant of the lateral meniscus described above. A tight cord-like structure was imposed, running laterally along the ACL. This cord-like structure was attached to the lateral femoral intercondylar area and the posterior horn region of the lateral meniscus in a sail-like shape. Two knees showed abnormal mobility of the lateral meniscus, tending to luxate. The ligamentous structure was cut stepwise lateral to the ACL, leaving the entire lateral meniscus undisturbed. Finally, full extension was achieved. Examination 7 to 27 months after surgery showed asymptomatic knees. This ligamentous structure is an important differential diagnosis to symptoms usually referred to as meniscus pathology. The appropriate treatment involves dissection. A loss of meniscal tissue followed by development of osteoarthritis can be prevented.
