1. Non-destructive multi-sensor core logging allows for rapid imaging and estimation of frozen bulk density and volumetric ice content in permafrost cores

Abstract: Permafrost cores provide physical samples that can be used to measure the characteristics of frozen ground. Measurements of core physical properties, however, are typically destructive and time intensive. In this study, multi-sensor core logging (MSCL) is used to provide a rapid (∼2–3 cm core depth per minute), high-resolution, non-destructive method to image and collect the physical properties of permafrost cores, allowing for the visualization of cryostructures and estimation of frozen bulk density, magnetic susceptibility, and volumetric ice content. Six permafrost cores with differing properties were analyzed using MSCL and compared with established destructive analyses to assess the potential of this instrument both in terms of accuracy and relative rate of data acquisition. A calibration procedure is presented for gamma ray attenuation from a 137Cs source that is specific to frozen-core materials. This accurately estimates frozen bulk density over the wide range of material densities found in permafrost. MSCL frozen bulk density data show agreement with destructive analyses based on discrete-sample measurements, with an RMSE of 0.067 g cm−3. Frozen bulk density data from the gamma attenuation, along with soil dry bulk density measurements for different sediment types, are used to estimate volumetric ice content. This approach does require an estimation of the soil dry bulk density and assumption of air content. However, the averaged results for this method show good agreement with an RMSE of 6.7 %, illustrating MSCL can provide non-destructive estimates of volumetric ice contents and a digital archive of permafrost cores for future applications.

2. Ultra-distal tephra deposits and Bayesian modelling constrain a variable marine radiocarbon offset in Placentia Bay, Newfoundland

Abstract: Radiocarbon dating marine sediments is complicated by the strongly heterogeneous age of ocean waters. Tephrochronology provides a well-established method to constrain the age of local radiocarbon reservoirs and more accurately calibrate dates. Numerous ultra-distal cryptotephra deposits (non-visible volcanic ash more than 3000 km from source) have been identified in peatlands and lake sediments across north-eastern North America and correlated with volcanic arcs in the Pacific north-west. Previously, however, these isochrons have not been identified in sediments from the north-west Atlantic Ocean. In this study, we report the presence of two ultra-distal cryptotephra deposits; Mazama Ash and White River Ash eastern lobe (WRAe), in Placentia Bay, North Atlantic Ocean. We use these well-dated isochrons to constrain the local marine radiocarbon reservoir offset (ΔR) and develop a robust Bayesian age–depth model with a ΔR that varies through time. Our results indicate that the marine radiocarbon offset in Placentia Bay was -126±151 years (relative to the Marine20 calibration curve) at the time of Mazama Ash deposition (7572 ± 18 yr BP) and −396 ± 144 years at the time of WRAe deposition (1098–1097 yr BP). Changes in ΔR appear to coincide with inferred shifts in relative influences of the inner Labrador Current and the Slopewater Current in the bay. An important conclusion is that single-offset models of ΔR are easiest to apply and often hard to disprove. However, such models may oversimplify reservoir effects in a core, even over relatively short timescales. Acknowledging potentially varying offsets is critical when ocean circulation and ventilation characteristics have differed over time. The addition of tephra isochrons permits the calculation of semi-independent reservoir corrections and verification of the single ΔR model.

3. The impact of COVID-19 restrictions on perceived health and wellbeing of adult Australian sport and physical activity participants

Abstract: Abstract Individuals’ access to sport and physical activity has been hampered due to COVID-19 lockdown restrictions. In Australia participation in community sport was cancelled during lockdowns. There is limited research on the impact of sport participation restrictions on the health and wellbeing of adults. Aim The aim of this study was to investigate the perceived health and wellbeing of a sample of predominantly active Australian adults, both during COVID-19 and in comparison with one year earlier (pre COVID-19). Methods A survey was conducted during the first COVID-19 restrictions and lockdowns in Australia in May–June 2020. It was distributed by national and state sporting organisations and through researchers’ social media accounts. This particular paper focuses on adults aged 18–59 years. The survey collected information on participant demographics, the sport and physical activity patterns pre- COVID-19, and health and wellbeing outcomes during COVID-19 lockdown and compared to one year earlier. The health measures were cross-tabulated against the demographic and sport and physical activity variables, and group profiles compared with chi-square tests. Scales were derived from three wellbeing questions, and group differences were analysed by t-tests and F-tests. Results The survey sample included 1279 men and 868 women aged 18–59 years. Most (67%) resided in metropolitan cities. The great majority (83%) were sport participants. During COVID-19 lockdown men were significantly more likely than women to report worse or much worse general (p = 0.014), physical (p = 0.015) and mental health (p = 0.038) and lower life satisfaction (p = 0.016). The inactive adults were significantly more likely to report poorer general health (p = 0.001) and physical health (p = 0.001) compared to active adults. 4. Interspecific and intraspecific foraging differentiation of neighbouring tropical seabirds

Abstract: Abstract Background Social interactions, reproductive demands and intrinsic constraints all influence foraging decisions in animals. Understanding the relative importance of these factors in shaping the way that coexisting species within communities use and partition resources is central to knowledge of ecological and evolutionary processes. However, in marine environments, our understanding of the mechanisms that lead to and allow coexistence is limited, particularly in the tropics. Methods Using simultaneous data from a suite of animal-borne data loggers (GPS, depth recorders, immersion and video), dietary samples and stable isotopes, we investigated interspecific and intraspecific differences in foraging of two closely-related seabird species (the red-footed booby and brown booby) from neighbouring colonies on the Cayman Islands in the Caribbean. Results The two species employed notably different foraging strategies, with marked spatial segregation, but limited evidence of interspecific dietary partitioning. The larger-bodied brown booby foraged within neritic waters, with the smaller-bodied red-footed booby travelling further offshore. Almost no sex differences were detected in foraging behaviour of red-footed boobies, while male and female brown boobies differed in their habitat use, foraging characteristics and dietary contributions. We suggest that these behavioural differences may relate to size dimorphism and competition: In the small brown booby population (n 2000 individuals) and avoidance of kleptoparasitism may be more prevalent in driving movements for both sexes. Conclusions Our results reveal how, in an environment where opportunities for prey diversification are limited, neighbouring seabird species segregate at-sea, while exhibiting differing degrees of sexual differentiation. While the mechanisms underlying observed patterns remain unclear, our data are consistent with the idea that multiple factors involving both conspecifics and heterospecifics, as well as reproductive pressures, may combine to influence foraging differences in these neighbouring tropical species.

5. Sport participation settings: where and ‘how’ do Australians play sport?

Abstract: Abstract Background Leisure-time physical activity and sport participation trends are often reported, both in aggregate and by specific activity. Recently there has been a rise in overall leisure-time physical activity, but little change in the prevalence of organised sport. It is important that the development of sport policy, infrastructure and strategic developments meet the changing landscape of participation. However, there has been relatively little research into the settings in which people participate. The aim of this study is to investigate the settings of participation of children and adults in 12 major Australian sports. Methods This study utilised data about participation in sport and recreational physical activity collected in the AusPlay survey from a representative sample of adults and children in the Australian state of Victoria. For each type of physical activity, the settings of participation are identified. Respondents can report participation in a particular activity in more than one setting. Therefore we use the term “instance of participation” to refer to a person playing a particular sport in a particular setting. Participation and settings across 12 major sports were investigated for children and adults. Results For children, the most popular sport was swimming with a weighted estimate of 323,565 (30.3%) instances of participation in the Victorian population, followed by Australian football (n = 180,459; 16.9%), and basketball (n = 137,169; 12.9%). For adults the most popular sports were swimming (n = 703,950; 30.9%) followed by golf (n = 274,729; 12.1%), and tennis (n = 260,814; 11.4%). There were considerable differences between the profiles of settings of participation for the 12 sports. Across the 12 sports, the majority of participation by children took place within a sports club or association setting, representing 63% of all instances of sport participation. For adults, sports clubs and associations was also the most popular setting, but it represented only 37% of instances of participation. Conclusions Traditionally, community clubs and inter-club competitions provided the main setting for sport participation, but this is no longer the case, particularly for adults. If the community sport sector is to continue to flourish, it must consider new strategies and participation options more attractive to other segments of its potential market.

6. Enhancing group‐based internet obesity treatment: A pilot RCT comparing video and text‐based chat

Abstract: Summary Objective Internet delivery of behavioural weight control interventions offers potential for broad geographic reach and accessibility, but weight losses online fall short of those produced with the same programme delivered in‐person. This pilot study examined feasibility and preliminary efficacy of a video‐based platform for delivering weekly chat as part of a 6‐month, 24‐session online group behavioural weight control programme compared with the established text‐based format, which has produced the best online weight losses to date. Method Women with obesity (N = 32) were randomized to either (a) weekly video group chat sessions and provided with a cellular‐enabled scale (Video) or (b) Text‐based weekly chat sessions and given a digital scale (Text) and followed for 6 months to determine weight loss and treatment engagement. Results Women randomized to the ideo condition lost more weight than those in the Text condition (−5.0 ± 6.0% vs. −3.0 ± 4.1%, respectively) at 6 months, although the difference was not statistically significant. However, women in the Video condition had significantly greater treatment engagement, with greater self‐monitoring and website utilization than those in the Text condition. Conclusions Videoconference delivery of group‐based online weight control accompanied by a cellular‐connected scale may promote greater treatment engagement and weight loss than text‐based chat. A larger, adequately powered study is warranted to determine which elements drive these enhanced treatment outcomes.

7. Efficacy and Haematologic Toxicity of Palliative Radioligand Therapy of Metastatic Castrate-Resistant Prostate Cancer with Lutetium-177-Labeled Prostate-Specific Membrane Antigen in Heavily Pre-Treated Patients

Abstract: Background: Metastatic castration-resistant prostate cancer (mCRPC) remains a significant contributor to the global cancer burden. lutetium-177-prostate-specific membrane antigen radioligand therapy (177Lu-PSMA RLT) is an effective salvage treatment. However, studies have highlighted haematologic toxicity as an adverse event of concern. We report our single-centre experience of compassionate access palliative 177Lu-DOTAGA-(I-y)fk(Sub-KuE) (177Lu-PSMA I&T) with respect to efficacy and haematologic safety. Methods: Patients with mCRPC and adequate bone marrow/liver function were included. All patients included underwent baseline and response assessment by Gallium-68-PSMA-11 positron emission tomography/computed tomography (68Ga-PSMA-11 PET/CT). Prescribed activity of therapy was a median 6.24 GBq per patient per cycle (IQR1.29 GBq), administered in 8-week intervals, up to four cycles. Response was assessed by prostate specific antigen (PSA) and a week-12 PET/CT. Incidence of grade ≥ 3 haematologic toxicity, including association with risk factors (age ≥ 70 years, prior/concurrent therapy, presence of metastases, and number of cycles completed), was analysed. Results: One hundred patients completed one cycle of 177Lu PSMA I&T and underwent response assessment by both PSA and PET/CT. Two patients had an uninterpretable week-12 PET/CT. Median age was 70 (50–89), median number of prior therapies was three (1–6), and median follow up was 12-months. Fifty-four percent achieved a PSA response. Disease control rate (DCR) by PET/CT was 64% (29% SD, 34% PR, and 1% CR). Disease control by PET/CT was associated with an improved one-year overall survival (OS) compared to non-responders, median OS not-reached vs 10-months (p < 0.0001; 95% CI: 0.08–0.44). Regarding haematologic toxicity, 11% experienced a grade ≥ 3 cytopenia (self-limiting). No cases of myelodysplasia/acute leukaemia (MDS/AL) have been recorded. No association with risk factors was demonstrated. Conclusion: 177Lu-PSMA I&T is a safe and effective palliative outpatient treatment for mCRPC. 68Ga-PSMA-11 PET/CT response is associated with an improved one-year OS and may be used to adapt therapy.

8. Children's Perception of Biodiversity in Their School Grounds and Its Influence on Their Wellbeing and Resilience

Abstract: Evidence increasingly demonstrates nature engagement benefits. However, little evidence shows nature engagement provides children with a reflective perception of biodiversity, nor whether perception influences children's wellbeing and resilience. We explored the impact of weekly nature engagement in schoolgrounds on 509 eight-to-eleven-year-olds' biodiversity perception, wellbeing, and resilience. At the start and end of the academic year, wellbeing and resilience questionnaires were completed, and biodiversity perception was assessed by children drawing what they thought was in their schoolgrounds. Children initially perceived few organisms within easily visible taxa, and perceived more vertebrates compared to invertebrate species. After, children were more aware of taxa, resulting in a more reflective biodiversity perception. Children with initially low scores in wellbeing and resilience increased in these measures, and a positive association was found between increased invertebrates and vertebrates perception and improved resilience. Nature engagement within curriculum could reduce children's extinction of experience and improve wellbeing and resilience.

9. The prevalence of refractive error in three communities of Cape Town, South Africa

Abstract: The prevalence, distribution and demographic associations of refractive error in three communities in Cape Town, South Africa were assessed. In this cross-sectional study, a clustered random sampling procedure was used to recruit participants (n=176; age=40.6±14.7 years; males=76, females=96) from Khayelitsha, Milnerton, and Mitchell’s Plain. From March to May 2010, participants underwent autore-fraction and subjective refraction eye examinations.A structured interview was used to collect data on sociodemographics, age, gender, level of education, employment and race. Participants younger than 15 years, non-residents, or residents for less than six months, who declined signing the informed consent forms were excluded from the study. In this study myopia was defined as the spherical equivalent value in the better eye of −1.00D or worse and hyperopia as the spherical equivalent value in the better eye of ≥1.00D. Astigmatism was defined as −0.50 cylinder or worse in the better eye. The prevalence of myopia was 17.4% with a 90% confidence interval (CI) of 12.65-22.15, hyperopia was 13.4% (90% CI 9.13-17.67), and astigmatism was 60% (90% CI 53.86-66.14). Myopia was found to be significantly associated with race and age; while hyperopia was significantly associated with age, employment and race. Abstract: Abstract A search for a doubly-charged Higgs boson in pp collisions at $\sqrt{s} = 7 \ \mbox{TeV}$ is presented. The data correspond to an integrated luminosity of 4.9 fb−1, collected by the CMS experiment at the LHC. The search is performed using events with three or more isolated charged leptons of any flavor, giving sensitivity to the decays of pair-produced triplet components Φ ++ Φ −−, and Φ ++ Φ − from associated production. No excess is observed compared to the background prediction, and upper limits at the 95 % confidence level are set on the Φ ++ production cross section, under specific assumptions on its branching fractions. Lower bounds on the Φ ++ mass are reported, providing significantly more stringent constraints than previously published limits.

Abstract: Abstract Measurements from the CMS experiment at the LHC of dihadron correlations for charged particles produced in PbPb collisions at a nucleon–nucleon centre-of-mass energy of 2.76 TeV are presented. The results are reported as a function of the particle transverse momenta (p T) and collision centrality over a broad range in relative pseudorapidity (Δη) and the full range of relative azimuthal angle (Δϕ). The observed two-dimensional correlation structure in Δη and Δϕ is characterised by a narrow peak at (Δη,Δϕ)≈(0,0) from jet-like correlations and a long-range structure that persists up to at least |Δη|=4. An enhancement of the magnitude of the short-range jet peak is observed with increasing centrality, especially for particles of p T around 1–2 GeV/c. The long-range azimuthal dihadron correlations are extensively studied using a Fourier decomposition analysis. The extracted Fourier coefficients are found to factorise into a product of single-particle azimuthal anisotropies up to p T≈3–3.5 GeV/c for at least one particle from each pair, except for the second-order harmonics in the most central PbPb events. Various orders of the single-particle azimuthal anisotropy harmonics are extracted for associated particle p T of 1–3 GeV/c, as a function of the trigger particle p T up to 20 GeV/c and over the full centrality range.

Abstract: Over 90% of prostate cancers over-express prostate specific membrane antigen (PSMA) and these tumor cells may be accurately targeted for diagnosis by 68Ga-PSMA-positron emission tomography/computed tomography (68Ga-PSMA-PET/CT) imaging. This novel molecular imaging modality appears clinically to have superseded CT, and appears superior to MR imaging, for the detection of metastatic disease. 68Ga-PSMA PET/CT has the ability to reliably stage prostate cancer at presentation and can help inform an optimal treatment approach. Novel diagnostic applications of 68Ga-PSMA PET/CT include guiding biopsy to improve sampling accuracy, and guiding surgery and radiotherapy. In addition to facilitating the management of metastatic castrate resistant prostate cancer (mCRPC), 68Ga-PSMA can select patients who may benefit from targeted systemic radionuclide therapy. 68Ga-PSMA is the diagnostic positron-emitting theranostic pair with the beta emitter Lutetium-177 PSMA (177Lu-PSMA) and alpha-emitter Actinium-225 PSMA (225Ac-PSMA) which can both be used to treat PSMA-avid metastases of prostate cancer in the molecular tumor-targeted approach of theranostic nuclear oncology.

Abstract: On stage, and in real life, timing is critical for success. Theranostic cancer care epitomizes the central role of timing in the evolution of efficacious molecular targeted radioligand therapy and its incorporation into routine clinical practice of oncology. Nuclear medicine has returned to its therapeutic roots, having been founded as a medical specialty, over three-quarters of a century ago, with radioiodine therapy of thyroid cancer. The very recent oncologist acceptance of 68Ga/177Lu/225Ac-PSMA effectiveness in treating prostate cancer has re-established the role of the physician in nuclear medicine. This article addresses various important issues in respect of timing related to this resurgence. Training of the required new workforce in technical -omics expertise and physicianly virtues is an urgent priority. Precision in radioligand therapy requires definition of individual radiation absorbed dose (Gy) to tumor and to critical normal organs, preferably prospectively. It is time to abandon one-size-fits-all administration of fixed activities (GBq) in arbitrary cycle intervals and duration. The time has also come to design combination sequenced theranostic-immuno-chemotherapeutic approaches to metastatic cancer to address unmet needs, particularly in pancreatic carcinoma; exploiting the potential of new fibroblast activation protein inhibitor radioligands targeting the tumor microenvironment. Public perception of all things "nuclear," including nuclear medicine, has recently recovered from the general opprobrium and radiophobia of the last half-century. Nuclear is the new green. Abstract: Patients with relapsed or refractory metastatic cancer unresponsive to standard therapies have motivated nuclear physicians to develop innovative radioligands, precisely targeted to tumor molecular receptors, for effective treatment of specific advanced malignancies. Individual practitioners in departments of nuclear medicine across the world have performed first-in-human studies on compassionate patient usage N-of-One protocols. These physician-sponsored studies then evolved into early-phase clinical trials and obtained real-world data to demonstrate real-world evidence of effectiveness in prolonging survival and enhancing quality of life of many so-called "End-Stage" cancer patients. Virtually all the therapeutic radiopharmaceuticals in current clinical oncology have been discovered and developed into effective specific treatments of targetable cancers by individual doctors in the course of their hospital practice. Pharma industry was not involved until many years later when performance of mandated Phase 3 randomized controlled trials became necessary to achieve regulatory agency approval. This article traces the history of several novel theranostic agents developed from compassionate N-of-One studies by hospital physicians over the past 36 years. Abstract: Multiple sclerosis (MS) is a debilitating disease that requires prolonged treatment with often severe side effects. One experimental MS therapeutic currently under development is a single amino acid mutant of a plant peptide termed kalata B1, of the cyclotide family. Like all cyclotides, the therapeutic candidate [T20K]kB1 is highly stable as it contains a cyclic backbone that is cross-linked by three disulfide bonds in a knot-like structure. This stability is much sought after for peptide drugs, which despite exquisite selectivity for their targets, are prone to rapid degradation in human serum. In preliminary investigations, it was found that [T20K]kB1 retains oral activity in experimental autoimmune encephalomyelitis, a model of MS in mice, thus opening up opportunities for oral dosing of the peptide. Although [T20K]kB1 can be synthetically produced, a recombinant production system provides advantages, specifically for reduced scale-up costs and reductions in chemical waste. In this study, we demonstrate the capacity of the Australian native Nicotiana benthamiana plant to produce a structurally identical [T20K]kB1 to that of the synthetic peptide. By optimizing the co-expressed cyclizing enzyme, precursor peptide arrangements, and transgene regulatory regions, we demonstrate a [T20K]kB1 yield in crude peptide extracts of ~ 0.3 mg/g dry mass) in whole plants and close to 1.0 mg/g dry mass in isolated infiltrated leaves. With large-scale plant production facilities coming on-line across the world, the sustainable and cost-effective production of cyclotide-based therapeutics is now within reach.

Abstract: Acute compartment syndrome is a devastating condition associated with lasting consequences or even death if not treated in a timely fashion. Current pre-clinical modeling is inadequate. Ideally a model should mimic human disease. There should be a trauma induced reperfusion or direct muscle event that causes gradual increased pressure and is amenable to release with fasciotomy. We have attempted to reproduce this mechanism and outcome in a porcine model.Anterior tibial musculature was injured with either vascular occlusion plus exterior tourniquet crush or direct intra-compartmental crush through balloon inflation. The injury was maintained for over 5 hours. At that time the tourniquet or balloon was removed. The injuries were continuously monitored with an intramuscular continuous pressure sensor. Pressure changes were recorded and after two hours of post-injury observation a fasciotomy was performed for the muscle compartment.Pressures were brought to 100 mmHg during the injury phase. During the two-hour observation period, the balloon catheter technique achieved an average pressure of 25.1 ± SD 8.8 mmHg with a maximum reading of 38.2 mmHg and minimum reading of 14.1 mmHg. During this same period, the ischemia-reperfusion + direct crush technique achieved an average pressure of 33.7 ± SD 7.3 mmHg, with a maximum reading of 43.5 mmHg and minimum reading of 23.5 mmHg. Average pressure post-fasciotomy for the balloon catheter technique was 2.4 ± SD 2.5 mmHg; and for the crush technique average value post-fasciotomy was 4.9 ± SD 3.7 mmHg - both representing a return to physiological levels.This is the first pre-clinical model that shows the same response to injury and treatment as is seen in human physiology. Both surgical and non-surgical therapies for compartment syndrome can now be tested reliably.

Abstract: The recurrent nova RS Oph underwent a new outburst on August 8, 2021, reaching a visible brightness of V = 4.8 mag. Observations of the 2021 outburst made with the high resolution UVES spectrograph at the Kueyen-UT2 telescope of ESO-VLT in Paranal enabled detection of the possible presence of 7Be freshly made in the thermonuclear runaway reactions. The 7Be yields can be estimated in N(Be)/N(H) = 5.7 x 10^(-6), which are close to the lowest yields measured in classical novae so far. 7Be is short-lived and decays only into 7Li. By means of a spectrum taken during the nebular phase we estimated an ejected mass of about 1.1 x 10^(-5) Msun, providing an amount of about 4.4x 10^(-10) Msun of 7Li created in the 2021 event. Recurrent novae of the kind of RS Oph may synthesize slightly lower amount of 7Li per event as classical novae, but occur 10^3 times more frequently. The recurrent novae fraction is in the range of 10-30% and they could have contributed to the making of 7Li we observe today. The detection of 7Be in RS Oph provides further support to the recent suggestion that novae are the most effective source of 7Li in the Galaxy., Accepted 2022 September 17. Abstract: The Composite Burn Index (CBI) is commonly linked to remotely sensed data to understand spatial and temporal patterns of burn severity. However, a comprehensive understanding of the tradeoffs between different methods used to model CBI with remotely sensed data is lacking. To help understand the current state of the science, provide a blueprint towards conducting broad-scale meta-analyses, and identify key decision points and potential rationale, we conducted a review of studies that linked remotely sensed data to continuous estimates of burn severity measured with the CBI and related methods. We provide a roadmap of the different methodologies applied and examine potential rationales used to justify them. Our findings largely reflect methods applied in North America – particularly in the western USA – due to the high number of studies in that region. We find the use of different methods across studies introduces variations that make it difficult to compare outcomes. Additionally, the existing suite of comparative studies focuses on one or few of many possible sources of uncertainty. Thus, compounding error and propagation throughout the many decisions made during analysis is not well understood. Finally, we suggest a broad set of methodological information and key rationales for decision-making that could facilitate future reviews.

Abstract: Approximately 5% to 10% of all breast cancers are hereditary; many of which are caused by pathogenic variants in genes required for homologous recombination, including BRCA1 and BRCA2. Here we discuss systemic treatment for such breast cancers, including approved chemotherapeutic approaches and also targeted treatment approaches using poly-(ADP ribose) polymerase inhibitors. We also discuss experimental approaches to treating hereditary breast cancer, including new small molecule DNA repair inhibitors and also immunomodulatory agents. Finally, we discuss how drug resistance emerges in patients with hereditary breast cancer, how this might be delayed or prevented, and how biomarker-adapted treatment is molding the future management of hereditary breast cancer.

Abstract: Does the ketone acetoacetate (AcAc) alone, or combined with β-hydroxybutyrate (βOHB), impact mouse embryo development, metabolism, histone acetylation and viability?Pronucleate mouse oocytes were cultured in vitro in G1/G2 media supplemented with ketones (AcAc or AcAc + βOHB) at concentrations representing those in maternal serum during pregnancy (0.04 mmol/l AcAc, 0.1 mmol/l βOHB), standard diet consumption (0.1 mmol/l AcAc, 0.25 mmol/l βOHB), ketogenic diet consumption (0.8 mmol/l AcAc, 2 mmol/l βOHB) and diabetic ketoacidosis (2 mmol/l AcAc, 4 mmol/l βOHB). Day 5 blastocysts were assessed for cell allocation, glucose metabolism and histone acetylation. Day 4 blastocysts exposed to 0.8 mmol/l AcAc + 2 mmol/l βOHB were transferred to standard-fed recipient females, and E14.5 fetal and placental development assessed.Exposure to 2 mmol/l AcAc or 0.8 mmol/l AcAc + 2 mmol/l βOHB did not impair blastocyst development, but significantly increased glucose consumption (P = 0.001 each), lowered glycolytic flux (P = 0.01, P 0.001) and elevated trophectoderm (TE) histone 3 lysine 27 acetylation (H3K27ac; P 0.001 each) compared with unexposed controls. Preimplantation AcAc + βOHB exposure reduced post-implantation fetal development by 25% (P = 0.037), and delayed female-specific fetal limb development (P = 0.019) and estimated fetal age (P = 0.019) compared with controls.Preimplantation exposure to ketones affects underlying metabolism and histone acetylation in blastocysts that are associated with persistent, female-specific perturbations in fetal development. A periconceptional diet that elevates ketone concentrations may impair human embryonic viability.

47. Predictors of Foot Acute Compartment Syndrome: Big Data analysis

Abstract: Acute compartment syndrome (ACS) in the foot is a challenging diagnosis and can lead to significant disabilities to patients. The present study aims to investigate the incidence, risk factors, demographics and association in the analysis of acute compartment syndrome (ACS) of the foot. We performed a retrospective review of the Trauma Quality Programs data from the American College of Surgeons including 70,525 patients who sustained a fracture of the foot from 2015 to 2018 (4 calendar years). Fasciotomies were performed in 0.7% of all foot fractures. Open fractures, crush injuries and multiple foot fractures were the strongest predictors of fasciotomies, with odds ratios of 2.38, 2.38 and 2.33 respectively. Being a male was associated with an increased likelihood of fasciotomies of 64% (p.0001 O.R. = [1.42-1.90]), while a dislocation in the foot increased likelihood of fasciotomies by 48% (p = .0008 O.R. = [1.18-1.86]). Trauma centre level III had higher rate of fasciotomy than Tertiary Trauma centers. Multiple other factors were addressed while controlling for cofounders. This big data analysis provided information not previously reported on the risk factors, demographics, and clinical association of ACS in the foot.

49. Prelacrimal Approach to Maxillary Sinus Pathology

Abstract: Background Access to the anterior, lateral, inferior, and inferomedial maxillary sinus has been a limitation of the middle meatal antrostomy. Expanded techniques such as the modified medial maxillectomy provide access to many of these areas but require remucosalization, and crusting can occur during the recovery phase. The prelacrimal approach (PLA) offers direct 0° endoscope access to these areas. Additionally, PLA can preserve the nasolacrimal duct and mucosal coverage. Objectives We describe the current surgical technique and outcomes of PLA patients. Methods Consecutive adult patients with pathology addressed by PLA to the maxillary sinus were assessed. The primary outcome was the restoration of the lateral wall, and the secondary outcomes were early (< 90 days) and late morbidity (> 90 days). Results Forty patients (52.8 ± 17 years, 62.5% female) were assessed. All patients had successful restoration of the lateral nasal wall (100% [95CI: 91.2%-100%]). The complications reported were primarily dysesthesia (early 10% and late 2.5%). Conclusion The PLA provides robust access to the anterior, lateral, inferior, and inferomedial maxilla. PLA offers rapid mucosal recovery while preserving the normal physiology and the lacrimal systems with low morbidity.
