Abstract

The nitroreductase (NR)/CB1954 enzyme prodrug system has given promising results in pre-clinical studies and is currently being assessed in phase I and II clinical trials in prostate cancer. Enhanced cell killing by apparent immune-mediated mechanisms has been shown in pancreatic and colorectal cancer models, by co-expressing murine granulocyte macrophage colony-stimulating factor (GM-CSF) with NR in a single replication deficient adenoviral vector. This consists of the CMV immediate early promotor driving expression of NR, with an internal ribosome entry site (IRES) and the gene for murine GM-CSF (mGM-CSF). To examine if similar enhancement of tumour cell killing could be produced in prostate cancer, the TRAMP model was chosen. Results illustrate that the combination of suicide gene therapy using NR and CB1954, with cytokine stimulation with mGM-CSF gives an improved response compared with either modality alone. The mechanism of this improved response is however likely to be non-immune based as it lacks a memory effect.

Abstract

Objective To summarize the practice of UK urologists with regard to nephrectomy for benign disease, documenting the indications, procedural techniques and outcomes. Methods All patients undergoing nephrectomy for a benign condition in 2012 were identified from the British Association of Urological Surgeons (BAUS) nephrectomy database. Recorded variables included the technique of surgery, the type of minimally invasive procedure, operating time, blood loss, transfusion rate, conversion rate, intra- and postoperative complications and mortality rate. Cases were also sub-analysed according to their pathologies to determine the differences in complication rate between stone disease, pyelonephritis, non-functioning kidney and other benign lesions. To contextualize procedural complexity, the simple nephrectomy data were compared with those obtained from the BAUS stage T1 radical nephrectomy audit. Results A total of 1 093 nephrectomies were performed (537 non-functioning kidneys, 142 stone disease, 129 nephrectomies secondary to pyelonephritis and 285 cases with other benign conditions). Of these, 76% were performed laparoscopically. Blood loss >500 mL was noted in 74 cases with a 4.8% blood transfusion rate. The intra- and postoperative complication rates were 5.2 and 11.9%, respectively. Of the 847 minimally invasive procedures, the conversion rate was 5.9%. Patients with stone disease have the highest intra- and postoperative complications (9.9 and 23.9%, respectively) compared with other benign pathologies. The total number of T1 radical nephrectomies performed was 1 095. In comparison with T1 radical nephrectomy, simple nephrectomy carries an increased risk of conversion to an open procedure (1.8 times), a higher rate of blood transfusion (4.8 vs 2.8%), and a higher risk of intra- and postoperative complications (5.2 vs 3.7% and 11.9 vs 10%, respectively). Conclusion The present study reports the largest series of nephrectomies performed for benign disease and the resultant data now support the bespoke preoperative counselling of patients. Furthermore, it confirms the commonly held view that simple nephrectomy can be more difficult than its radical counterpart. The authors suggest that the term ‘simple nephrectomy’ is changed to ‘benign nephrectomy’.

Abstract

To present the perioperative outcomes from the British Association of Urological Surgeons (BAUS) nephrectomy dataset for 2012, the first year of public reporting of individual surgeon outcomes in the UK.All nephrectomies performed in the year 2012 and recorded in the database were analysed. These were divided into simple nephrectomy (SN), partial nephrectomy (PN), radical nephrectomy (RN), and nephroureterectomy (NU). The estimated capture rate for nephrectomy was 80%. The outcomes measured were 30-day mortality (30-DM), Clavien-Dindo complications grade ≥III, blood transfusion, conversion to open, and length of stay.The overall 30-DM was 0.55% (SN 0.53%; PN 0.10%; RN 0.52%; NU 1.27%). Clavien-Dindo complications grade ≥III were recorded in 3.9% of nephrectomies (SN 4.3%; PN 5.4%; RN 3.1%; NU 4.5%). Blood transfusion was required during surgical admission for 8.4% of nephrectomies (SN 5.2%; PN 3.4%; RN 11.1%; NU 8.3%). Conversion to open was carried out in 5.5% of minimally invasive nephrectomies (SN 6.1%; PN 4.0%; RN 5.5%; NU 5.6%). Open nephrectomy patients remained in hospital for a median of 6 days (SN 7; PN 5; RN 7; NU 8 days), which was higher than the median 4-day stay (SN 3; PN 4; RN 4; NU 5 days) for minimally invasive surgery.Nephrectomy in 2012 was a safe procedure with morbidity and mortality rates comparable with or less than published series. The collection of surgeon-specific data should be iterative with further refinement of data categories, support for the collection process and independent validation of results.

Abstract

The techniques of pulse radiolysis and emission spectroscopy have been used to determine absolute total emission yields for the XeBr* exciplex. By application of known formation and quenching rate constants and mechanisms, individual fluorescence yields have been determined for the unquenched three-body ionic recombination reaction, Xe2+ + Br- + Xe → XeBr* + 2Xe, Go+ = 3.40 ± 0.18, and for the reaction of electronically excited xenon atoms with CF3Br, Xe* + CF3Br → XeBr* + CF3, Go* = 0.68 ± 0.10. These experimental yields are compared to the predictions of current theoretical models, which allow calculation of the efficiency for each ion-pair recombination event and excited state reaction to give the fluorescent XeBr* exciplex as 75% and 36%, respectively.

Abstract

The deposition of small amounts of radioactive material on the surface of oil and gas processing equipment creates, in time, a scale that can potentially pose a safety, health and environmental risk. Release criteria are usually defined in terms of the activity concentration of the radionuclides in a material; however this constrains the measurement of NORM in equipment to an impractical and prolonged testing process. This paper evaluates field data to establish a practical empirical relationship between the activity concentration present in a NORM deposit and measurements obtained with a hand-held Geiger-Müller radiation monitor. This has been done for varying circular diameters to account for geometrical variations introduced by measuring a curved surface with a flat probe.

Abstract

The use of dedicated PET scanners is becoming more widespread throughout Australia and the world. PET imaging utilizes short-lived (approximately 108 min), high-energy (511 keV) gamma-ray emitters that could result in a high radiation dose being received by staff. As part of a larger staff and area monitoring project, this paper discusses the personal dose equivalent, H(p)(10), received by PET staff working in a dedicated PET center. The typical H(p)(10) received by staff was approximately 1 microSv per minute of close contact with patients, which resulted in an average daily dose for nuclear medicine technologists of approximately 31 microSv. The average daily administered activity to patients at Austin Health was 1,280 MBq.

Abstract

Case reviews based on autopsy studies have shown that motor vehicle collisions cause between 50 and 90% of traumatic aortic ruptures. Very few studies have analyzed the nature and severity of the collision forces associated with this injury. Our passenger car study (1984-1991) examined 36 collisions in which 39 fatally injured victims sustained aortic trauma. In this injury group, a disproportionate number of heavy truck and roadside fixed-object impacts occurred. Vehicle crash forces were generally severe and were either perpendicular or oblique to the vehicle surface. Intrusion into the occupant compartment was a significant factor in most of these fatal injuries. Occupant contact with vehicle interior surfaces was identified in most cases, and occupant restraints were often ineffective, especially in side collisions. The more elderly victims were seen in the least severe collisions. The most frequent site of aortic rupture was at the isthmus. A majority of victims had rib/sternal fractures indicating significant chest compression. Of the various traumatic aortic injury mechanisms proposed in motor vehicle impacts, the favored theories in the literature combine features of rapid deceleration and chest compression. This study supports that predominant impression, concluding that rapid chest deceleration/compression induces torsional and shearing forces that result in transverse laceration and rupture of the aorta, most commonly in the inherently vulnerable isthmus region.

Abstract

Thermoluminescence dosimetry (TLD) using LiF:Mg,Ti and electron spin resonance dosimetry using alanine (ALD) have been used to map the dose distribution in the sample chamber of a Gammacell-1000 Irradiator. The results showed that the dose rate to air varied by approximately 20% over the volume of the chamber. The mean ratio of the dose rates at various locations throughout the chamber, TLD/ALD was 1.020 +/- 0.025 (1 sd). The dose range around 30 Gy is regarded as being towards the upper limit of reliability for TLD measurements and towards the lower limit for ALD measurements.

Abstract

This paper describes the historical origins, modern-day operations and complexities of the Ontario coroners system. Both the Office of the Chief Coroner and Statistics Canada publish death statistics for the province of Ontario. The time required by the coroner's office to produce accurate cause-of-death data may significantly exceed the time constraints faced by Statistics Canada in producing its own annual cause-of-death statistics. As a result, substantial discrepancies arise in the number of deaths attributed to accidents, suicides and homicides that are published by these two agencies.

4572 Background: We report a phase I clinical trial in PCa, using intraprostatic (ip) injection of a replication defective adenovirus encoding ntr (CTL102) +/- iv prodrug CB1954. Ntr converts the weak monofunctional alkylating agent CB1954 into a highly cytotoxic bifunctional alkylating agent effective in replicating and quiescent cells. The ease of direct access to the prostate makes locally targeted therapy for PCa a viable option. Methods: Patients underwent TRUS-guided ip injection of CTL102 in escalating doses, from 1010 to 5x1011 particles, with subsequent prostatectomy (gene expression study) or iv CB1954 24mg/m2 (therapeutic group) 48–72h post injection. Ntr expression in resected tissue was assessed by immunohistochemistry. Tissue to air ratios of absorbed dose have been measured for certain organs in an Alderson Rando radiotherapy phantom irradiated by simulated circular plane sources of degraded gamma rays from radium-226. The absorbed dose measurements were performed using thermoluminescent dosemeters. Factors were determined which relate the absorbed dose received by the organ of interest to (i) the absorbed dose in air at the height of the organ, (ii) the adsorbed dose measurement by a personal monitor worn at the chest and waist, and (iii) the absorbed dose in air at 1 m above the source. Effective dose equivalents have been calculated using the above factors in conjunction with ICRP 26 weighting factors. The results indicate that a good estimate of the effective dose equivalent can be obtained irrespective of the wearing position of the personal monitor on the trunk.

Abstract

We have determined the interaction second virial coefficients (B12) for the CO2 C2H6 system between 250 and 300K using a Burnett-isochoric apparatus. The technique follows a procedure suggested by Hall and Eubank (1973) and extended by Holste et al. (1980). Combined errors for B12 are within 1.2 cm3/mol. In addition, values for the binary interaction parameter k12 calculated from our B12 values show difinite temperature dependence.

Densities of pure carbon dioxide were measured using Burnett, Burnett-isochoric, and isochoric techniques. The measurements were made at temperatures from 217.01 to 448.15 K and pressures up to 47.7 MPa. Eleven vapor-pressure measurements were also made. Second and third virial coefficients were derived. Three saturated-liquid and five saturated-vapor densities were also derived from the measurements.

Abstract

Most process plant and a great deal of structural steelwork for the nuclear, petrochemical, power generation and gas industries is fabricated with the use of fusion welding. Imperfections occur in such welds, due to problems with materials, procedures and techniques, and non-destructive testing is employed to detect such imperfections. The two principal reasons for the use of non-destructive testing are ( a ) to monitor and control the quality of weld workmanship and ( b ) to assess fitness for purpose and to ensure that failure will not occur from a weld fault within the design life of the fabrication. In both cases it is necessary to be able to detect, identify and measure weld defects. The results are compared with quality control levels of defect acceptance in the former circumstance and used in fracture mechanics analyses in the latter to ensure that defects present are not critical. A further important application of non-destructive testing is to assess deterioration of plant and structures in service or undergoing maintenance.

Abstract

• Serum creatine phosphokinase (CPK) levels were studied in 110 individuals: 40 psychotic children suffering from childhood autism, atypical personality development, and childhood schizophrenia; five children with childhood aphasia; 22 children with severe personality disorders; 29 normal children and normal siblings of psychotic children; and 14 normal parents of psychotic children. Creatine phosphokinase levels from the entire population of adults and children were normally distributed, and the mean CPK levels for the eight diagnostic groups were within normal limits. Those 22 children with personality disorders had significantly higher CPK levels than the other diagnostic groups. This relatively higher level of CPK may be related to vulnerability to later development of schizophrenic spectrum disorders. There was no apparent relationship between CPK levels and motor activity, nor was there any change in the level of CPK during a trial of psychoactive medication. Creatine phosphokinase levels remained relatively stable on test-retest determination.

Published reports of toxic hallucinosis during treatment with methylphenidate are rare. Two cases are described and it is suggested that methylphenidate-induced hallucinosis is more common than has been appreciated. Symptoms may be understood in terms of dose-related effects at pre- and postsynaptic receptors. Two other hypotheses that may clarify underlying mechanisms of action that produce toxic effects relate to (1) noradrenergic facilitation transmission of information in the visual pathway, and (2) interactions among brain monoamine systems.

Free norepinephrine, epinephrine, and dopamine are determined in urine after purification on a small ion-exchange column and concentration on alumina. The catecholamines are separated by reversed-phase ion-pair chromatography and are quantitated by measuring their native fluorescence (lambda ex 285 nm, lambda em 305 nm). The internal standard is dihydroxybenzylamine. Within-day CVs for the compounds ranged from 1.6 to 5.6%; between-day CVs were 6.6 to 9.8%.

Abstract

There are conflicting data concerning the efficacy of clonidine in TS. Some TS patients, probably 50% or more, experience substantial, long-term symptomatic improvement with minimal side effects. However, their profile of response is often variable, with behavioral symptoms appearing to show the most consistent improvement. Maximal benefit may not be evident for 4 to 6 months. A minority of patients do not respond, and a few worsen on clonidine. The need for additional double-blind trials is clear. Additional metabolic and pharmacologic investigations are needed to understand the determinants of the response of TS patients to clonidine. The response to acute doses of clonidine on sedation, growth hormone release, blood pressure, and plasma MHPG levels may be predictive of eventual therapeutic response. The variable response to clonidine, however, suggests that noradrenergic mechanisms in TS may not be of primary pathogenic importance. The study of interactions between neurochemical systems may help illuminate the pathophysiology of TS and lead the way to improved treatment of this disabling condition.
