1. Seasonal snow–atmosphere modeling: let's do it

Abstract: Mountain snowpack forecasting relies on accurate mass and energy input information in relation to the snowpack. For this reason, coupled snow–atmosphere models, which downscale input fields to the snow model using atmospheric physics, have been developed. These coupled models are often limited in the spatial and temporal extents of their use by computational constraints. In addressing this challenge, we introduce HICARsnow, an intermediate-complexity coupled snow–atmosphere model. HICARsnow couples two physics-based models of intermediate complexity to enable basin-scale snow and atmospheric modeling at seasonal timescales. To showcase the efficacy and capability of HICARsnow, we present results from its application to a high-elevation basin in the Swiss Alps. The simulated snow depth is compared throughout the snow season to aerial lidar data. The model shows reasonable agreement with observations from peak accumulation through late-season melt-out, representing areas of high snow accumulation due to redistribution processes, as well as melt patterns caused by interactions between radiation and topography. HICARsnow is also found to resolve preferential deposition, with model outputs suggesting that parameterizations of the process using surface wind fields may only be inappropriate under certain atmospheric conditions. The two-way coupled model also improves surface air temperatures over late-season snow, demonstrating added value for the atmospheric model as well. Differences between observations and model outputs during the accumulation season indicate a poor representation of redistribution processes away from exposed ridges and steep terrain and a low bias in albedo at high elevations during the ablation season. Overall, HICARsnow shows great promise for applications in operational snow forecasting and in studying the representation of snow accumulation and ablation processes.

2. Efficacy of an app-based treatment for anxiety disorders including exposure in virtual reality – a randomized controlled trial

Abstract: Introduction Anxiety disorders are among the most prevalent mental disorders. However, only a minority of patients receives adequate psychotherapeutic treatment despite strong empirical evidence for the efficacy of CBT in anxiety disorders (Marcks et al. Psychiatr Serv 2009; 60 823-830). App-based psychotherapy can help to reduce this massive treatment gap. Objectives We aimed at evaluating the efficacy of an app-based treatment for anxiety disorders including exposure in virtual reality. Methods The randomized controlled trial was conducted in two university outpatient treatment centers in Northern Germany. Patients were diagnosed with agoraphobia (AP; with or without panic disorder; n=103), panic disorder (PD; n=84) or social anxiety disorder (SAD; n=110) and were randomly assigned to either the app-based intervention or treatment as usual (up to 6 sessions of supportive therapy). The app was developed based on evaluated CBT manuals and includes 14 hours of audio and video content and 15 disorder specific virtual reality exposure scenarios. Participants in the intervention groups also received two appointments with a therapist during the app-based treatment. Primary outcome was the change in Beck Anxiety Inventory (BAI) score pre to post (after 6 months). Mixed ANOVAs were conducted in intention to treat and completer analyses. Secondary outcomes were disorder specific questionnaires (Liebowitz Social Anxiety Scale LSAS for SAD and Panic and Agoraphobia Scale PAS for AP and PD) and health related quality of life measured with a single item (L-1). Results In the ITT analysis, the interaction effect between group and time was significant in patients with AP as well as in patients with PD (AP: p=.014, partial η²=.06; PD: p=.028, partial η²=.06). This indicates a stronger improvement of symptoms in the intervention group compared to the control group. In patients with SAD, there was no significant interaction effect (p=.101, partial η²=.03). The disorder specific measures LSAS and PAS showed a significantly stronger decrease in the intervention group than in the control group for each of the specific disorders. Concerning quality of life, a stronger improvement in the intervention group was only found in patients with PD. Conclusions A stronger symptom reduction in the app-based intervention group compared to the control group could be found in patients with AP (BAI/PAS), PD (BAI/PAS) and SAD (LSAS). This is particularly remarkable as the app was compared to an active control group with up to 6 sessions of psychotherapy. Effect sizes were comparable to those found in studies comparing face-to-face CBT to an active control group. The lack of an intervention-specific effect on BAI scores in patients with SAD might be due to the poor sensitivity of the BAI for the specific symptoms of SAD. Disclosure of InterestNone Declared

3. Far- and Near Range Measurements with a Synthetic Aperture Radar for Educational Purposes and Comparison of Two Different Signal Processing Algorithms

Abstract: In this paper two simple synthetic aperture radar (SAR) methods are applied on data from a 24 GHz FMCW radar implemented on a linear drive for educational purposes. The data of near and far range measurements are evaluated using two different SAR signal processing algorithms featuring 2D-FFT and frequency back projection (FBP) method (Moreira et al., 2013). A comparison of these two algorithms is performed concerning runtime, image pixel size, azimuth and range resolution. The far range measurements are executed in a range of 60 to 135 m by monitoring cars in a parking lot. The near range measurement from 0 to 5 m are realised in a measuring chamber equipped with absorber foam and nearly ideal targets like corner reflectors. The comparison of 2D-FFT and FBP algorithm shows that both deliver good and similar results for the far range measurements but the runtime of the FBP algorithm is up to 150 times longer as the 2D-FFT runtime. In the near range measurements the FBP algorithm displays a very good azimuth resolution and targets which are very close to each other can be separated easily. In contrast to that the 2D-FFT algorithm has a lower azimuth resolution in the near range, thus targets which are very close to each other, merge together and cannot be separated.

Abstract: Abstract Reduced ventricular longitudinal shortening measured by atrioventricular plane displacement (AVPD) and global longitudinal strain (GLS) are prognostic markers in heart disease. This study aims to determine if AVPD and GLS with cardiovascular magnetic resonance (CMR) are independent predictors of cardiovascular (CV) and all-cause death also in heart failure with reduced ejection fraction (HFrEF). Patients (n = 287) were examined with CMR and AVPD, GLS, ventricular volumes, myocardial fibrosis/scar were measured. Follow-up was 5 years with cause of death retrieved from a national registry. 5. Safety hazards in clinical practice of seclusion in psychiatric care

Abstract: Introduction Seclusion is part of the clinical practice in European psychiatric hospital care with the aim to maintain the safety of patients and staff. Adverse events and harm have been reported for patients and staff resulting from seclusion. Safety hazards, which are the prerequisite of adverse events, can be identified using video observation methods. Identifying safety hazards can be used to prevent adverse events and improve the quality of psychiatric care. Objectives To identify safety hazards during seclusion in psychiatric hospital care. Methods Descriptive design with non-participant video-observation of seclusion care practice. Data consisted of video recordings (n = 36) from six wards of one psychiatric hospital in Finland. The data were analysed with inductive thematic analysis. Results Clinical practice of seclusion included safety hazards stemming from the actions of patients and staff. Patients’ actions were as follows: aggressive behaviour, attempting to escape, precarious movements, preventing the visibility of staff, exposing themselves to contamination, and falls during seclusion. Staff actions included: leaving dangerous items to seclusion, issues in the administration of medication, performing physical and mechanical restraints in unsecure way, and precarious movements and postures. Conclusions According to our results, the use of seclusion has safety hazards that can result in harm for patients and staff. To improve the quality and safety of seclusion in clinical practice, the guidelines, practices, and staff training need to consider the various safety hazards. While the work in Europe to abolish the use of seclusion is still in progress, this topic requires attention in clinical practice, education, and policymaking. 6. Introducing mealworm as an ingredient in crisps and pâtés – sensory characterization and consumer liking

Abstract: The objective of this study was to evaluate sensory perception and consumers’ attitude and liking of products with different textures, crisps and pâtés, with added mealworm ingredient in different amounts. By addition of mealworm (Tenebrio molitor L.) in the crisps, the brittleness increased. However, an increase in mealworm also increased the darkness. Adding mealworms to pâté reduced the odour and flavour of vegetable and increased the flavour of cinnamon and pepper as well as the oily texture. There was no significant difference in total liking between 10 and 30% addition of mealworm, nor between 0 and 10% addition, in any of the two products. The crisps received higher liking score than the pâtés. Thus, there is also a much higher probability that a customer would buy insect crisps than insect pâté. Environmental sustainability and sensory properties were shown to be the main reasons for buying foods with added insect ingredients.

7. Heated Tobacco Product Marketing: A Mixed-Methods Study Examining Exposure and Perceptions among US and Israeli Adults

Abstract: The marketing of heated tobacco products (HTPs), like IQOS, influences consumers' perceptions. This mixed-methods study analyzed (i) survey data (2021) of 2222 US and Israeli adults comparing perceptions of 7 IQOS attributes (design, technology, colors, customization, flavors, cost and maintenance) and 10 marketing messages (e.g. 'Go smoke-free…') across tobacco use subgroups and (ii) qualitative interviews (n = 84) regarding IQOS perceptions. In initial bivariate analyses, those never using HTPs (86.2%) reported the least overall appeal; those currently using HTPs (7.7%) reported the greatest appeal. Notably, almost all (94.8%) currently using HTPs also currently used cigarettes (82.0%) and/or e-cigarettes (64.0%). Thus, multivariable linear regression accounted for current cigarette/e-cigarette use subgroup and HTP use separately; compared to neither cigarette/e-cigarette use (62.8%), cigarette/no e-cigarette use (17.1%) and e-cigarette/no cigarette use (6.5%), those with dual use (13.5%) indicated greater overall IQOS appeal (per composite index score); current HTP use was not associated. Qualitative data indicated varied perceptions regarding advantages (e.g. harm, addiction and complexity) of IQOS versus cigarettes and e-cigarettes, and perceived target markets included young people, those looking for cigarette alternatives and females. Given the perceived target markets and particular appeal to dual cigarette/e-cigarette use groups, IQOS marketing and population impact warrant ongoing monitoring to inform regulation.

8. On the self-similarity of wind turbine wakes in a complex terrain using large eddy simulation

Abstract: We perform large eddy simulation of flow in a complex terrain under neutral atmospheric stratification. We study the self-similar behavior of a turbine wake as a function of varying terrain complexity and perform comparisons with a flat terrain. By plotting normalized velocity deficit profiles in different complex terrain cases, we verify that self-similarity is preserved as we move downstream from the turbine. We find that this preservation is valid for a shorter distance downstream compared to what is observed in a flat terrain. A larger spread of the profiles toward the tails due to varying levels of shear is also observed.

Abstract: Background: The Global Anticoagulant Registry in the FIELD–Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. Methods and results: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012–2016). In India, the mean age was 65.8 years at diagnosis of NVAF. Hypertension was the most prevalent risk factor for AF, present in 68.5% of patients from India and in 76.3% of patients globally (P

10. From wind to loads: wind turbine site-specific load estimation with surrogate models trained on high-fidelity load databases

Abstract: We define and demonstrate a procedure for quick assessment of site-specific lifetime fatigue loads using simplified load mapping functions (surrogate models), trained by means of a database with high-fidelity load simulations. The performance of five surrogate models is assessed by comparing site-specific lifetime fatigue load predictions at 10 sites using an aeroelastic model of the DTU 10 MW reference wind turbine. The surrogate methods are polynomial chaos expansion, quadratic response surface, universal Kriging, importance sampling, and nearest-neighbor interpolation. Practical bounds for the database and calibration are defined via nine environmental variables, and their relative effects on the fatigue loads are evaluated by means of Sobol sensitivity indices. Of the surrogate-model methods, polynomial chaos expansion provides an accurate and robust performance in prediction of the different site-specific loads. Although the Kriging approach showed slightly better accuracy, it also demanded more computational resources.

11. Effect of Reynolds number and inflow parameters on mean and turbulent flow over complex topography

Abstract: A characterization of mean and turbulent flow behaviour over complex topography was conducted using a large-scale (1 : 25) model in the WindEEE Dome at Western University. The specific topographic feature considered was the Bolund Hill escarpment facing westerly winds. A total of eight unique inflow conditions were tested in order to isolate the impact of key parameters such as Reynolds number, inflow shear profile, and effective roughness, on flow behaviour over the escarpment. The results show that the mean flow behaviour was generally not affected by the Reynolds number; however, a slight increase in speed-up over the escarpment was observed for cases with lower inflow roughness. The shape of the inflow wind shear profile also had a minor impact on the mean flow near the escarpment. More significant effects were observed in the turbulent flow behaviour, where the turbulent kinetic energy (TKE) over the escarpment was found be a strong function of inflow roughness and a weak function of the Reynolds number. The local change in the inflow wind shear was found to have the most significant influence on the TKE magnitude, which more closely approximated the full-scale TKE data, a result which had not been previously observed in wind tunnel modelling of this topography.

12. Comparison of OpenFOAM and EllipSys3D for neutral atmospheric flow over complex terrain

Abstract: The flow solvers OpenFOAM and EllipSys3D are compared in the case of neutral atmospheric flow over terrain using the test cases of Askervein and Bolund hills. Both solvers are run using the steady-state Reynolds-averaged Navier–Stokes k–ϵ turbulence model. One of the main modeling differences between the two solvers is the wall-function approach. The OpenFOAM v.1.7.1 uses a Nikuradse's sand roughness model, while EllipSys3D uses a model based on the atmospheric roughness length. It is found that Nikuradse's model introduces an error dependent on the near-wall cell height. To mitigate this error the near-wall cells should be at least 10 times larger than the surface roughness. It is nonetheless possible to obtain very similar results between EllipSys3D and OpenFOAM v.1.7.1. The more recent OpenFOAM v.2.2.1, which includes the atmospheric roughness length wall-function approach, has also been tested and compared to the results of OpenFOAM v.1.7.1 and EllipSys3D. The numerical results obtained using the same wall-modeling approach in both EllipSys3D and OpenFOAM v.2.1.1 proved to be almost identical. Two meshing strategies are investigated using HypGrid and SnappyHexMesh. The performance of OpenFOAM on SnappyHexMesh-based low-aspect-ratio unstructured meshes is found to be almost an order of magnitude faster than on HypGrid-based structured and high-aspect-ratio meshes. However, proper control of boundary layer resolution is found to be very difficult when the SnappyHexMesh tool is utilized for grid generation purposes. The OpenFOAM is generally found to be 2–6 times slower than EllipSys3D in achieving numerical results of the same order of accuracy on similar or identical computational meshes, when utilization of EllipSys3D default grid sequencing procedures is included.

13. Disparities in Cannabis Use Outcomes, Perceived Risks and Social Norms across Sexual Orientation Groups of US Young Adult Women and Men

Abstract: Little research has examined mechanisms driving cannabis use disparities among sexual minority (SM; versus heterosexual) young adults (YAs). Participants were US YA women (N = 1345; 24.8% bisexual, 6.1% lesbian) and men (N = 998: 8.9% bisexual, 13.8% gay). Bivariate analyses examined associations between sexual orientation (heterosexual, gay/lesbian, bisexual) and cannabis use outcomes (past 30-day [current] use, use frequency, cannabis-tobacco dual use, use intentions), perceived risks (harm, addictiveness) and social norms (social acceptability, peer use). Regressions examined sexual orientation, perceived risks and social norms as correlates of cannabis outcomes. Bisexual and lesbian (versus heterosexual) women reported lower cannabis-related perceived risks, greater social norms and higher odds of current use. Bisexual (versus heterosexual) women reported more days of use, higher odds of cannabis-tobacco dual use and greater use intentions. Gay (versus heterosexual) men reported lower perceived addictiveness, greater social norms and higher odds of current use. Lower perceived risks and greater social norms predicted greater use outcomes for women and men. Bisexual women displayed higher odds of several cannabis use outcomes, whereas lesbian women and gay men displayed higher odds of current cannabis use only. Cannabis-related perceptions and social norms may be important targets for public health messaging for SMYAs.

14. Development of A Cervical Cancer Prevention Text-Messaging Program For Women Living with HIV

Abstract: Cervical cancer screening and human papillomavirus (HPV) vaccination are critical for those immunocompromised due to human immunodeficiency virus (HIV). Health education programs, including text messaging, can effectively improve knowledge of cervical cancer and recommended screening. This paper describes the data-driven development of a 4-week text-messaging intervention to improve HPV and cervical cancer knowledge among women living with HIV (WLH). This study reports data from surveys (n = 81; January 2020 to September 2021) and focus group discussions (FGDs, n = 39; April-June 2020) conducted among WLH in the DC area. While most WLH revealed that their usual sources of health information were through in-person group sessions, they pointed out that these were impractical options due to the coronavirus 2019 pandemic. They noted that a text-messaging intervention was feasible and acceptable. FGD participants' responses structured around the Protection Motivation Theory constructs informed the text-messaging library, covering topics such as (I) understanding of cervical cancer and HPV, (II) cervical cancer prevention and (III) HPV self-sampling. The utilization of low-cost and easily accessible health education interventions, such as mobile-based text messaging, can effectively increase knowledge and awareness of cervical cancer in populations that have been historically difficult to access and during times when health services are disrupted such as during a global pandemic or public health emergency.

Abstract: BACKGROUND: Mindfulness-based, in-person programs are effective at reducing stress and enhancing resilience in military and civilian samples, yet few studies have examined or compared training offered via real-time, interactive social media. Such a program would have a wider-reach and could include those unable to attend in-person. There is also interest in resolving ambiguity about the effects of mindfulness training on individual difference variables, such as self-compassion. OBJECTIVE: The purpose of this research was to compare pre/post self-compassion for three interventions; Mindfulness-based Stress Reduction delivered in-person (IP), mindfulness meditation training delivered via a Virtual World (VW), and a wait-list Control Group (CG) among active duty and veteran U.S. military. METHODS: A 2 (pre/post)×3 (group) factorial design was conducted with 250 active duty and veteran U.S. Military service members, with self-compassion measures as dependent variables. RESULTS: Self Compassion improved 10% for the IP group and 14% for the VW group, while the CG group did not improve. Combined treatment groups yielded a 10.3% improvement in self-compassion compared with no change in the CG and with a group×time interaction effect (p < 0.01). Participants with lower initial self-compassion experienced greater benefits than those with higher baseline self-compassion (p < 0.01). CONCLUSION: IP and VW Mindfulness Meditation training were equally effective in increasing self-compassion. Adding effective on-line mindfulness delivery will promote self-compassion among a more extensive audience, likely yielding improved coping, confidence, connectedness, cheerfulness, steadiness, and self-satisfaction, while lessening anxiety, fear-of-failure, and stress among participants. Abstract: Desert springs contain many endemic taxa and are of conservation concern due to anthropogenic activities that are expected to increase environmental salinity. Understanding the nature of osmoregulation is necessary to predict how non-vagile aquatic organisms will respond to changes. In the Chihuahuan Desert, the amphipod genus Gammarus Fabricius, 1775 is composed of two lineages. These lineages have species that currently inhabit springs ranging from 0.4 to 7.8 parts per thousand (ppt). All Gammarus in this region are of conservation concern because each is endemic to a single spring system. We exposed individuals of Gammarus colei Walters, Cannizzaro, and Berg in Walters, Cannizzaro, Trujillo and Berg, 2020 and Gammarus seideli Cannizzaro, Walters and Berg, 2018 species occupying low-salinity springs, to the range of salinities found in the Chihuahuan Desert. We measured metabolic rates as [Formula: see text] to examine the energetic cost of osmoregulation. We also measured the hemolymph osmolality of G. colei and compared that with the isosmotic line to determine the degree of osmoregulation. Neither species increased its metabolic rate across increasing salinities. However, G. colei showed an increase in hemolymph osmolality. Despite the divergence (∼66 million years ago) between the two lineages, it appears their physiological tolerances have converged, suggesting that both lineages may be able to persist when exposed to moderate salinity changes.

Abstract: Human immunodeficiency virus (HIV) infection is associated with subclinical cardiomyopathy, diastolic dysfunction, and increased risk of cardiovascular death. However, the relationship between left atrial (LA) mechanics and left ventricular (LV) diastolic function has not been evaluated in people living with HIV (PLWH) relative to HIV-uninfected (HIV-) controls. This is a multicenter, cross-sectional cohort analysis using the HIV Cardiovascular Disease substudy of the Veterans Aging Cohort Study database, which aimed to examine a cohort of PLWH and HIV- veterans without known cardiovascular disease. A total of 277 subjects (180 PLWH, 97 HIV-) with echocardiograms were identified. LV and LA phasic strain were derived and diastolic function was evaluated. Relationship between LA strain, LV strain, and the degree of diastolic dysfunction were assessed using analysis of variance and ordinal logistic regression with propensity weighting. In the PLWH cohort, 91.7% were on antiretroviral therapy and 86.1% had HIV viral loads500 copies/ml. The mean (± SD) duration of infection was 9.7 ± 4.9 years. Relative to HIV- veterans, PLWH did not differ in LA mechanics and proportion of diastolic dysfunction (p = 0.31). Using logistic regression with propensity weighting, we found no association between HIV status and degree of diastolic dysfunction. In both cohorts, LA reservoir strain and LA conduit strain were inversely and independently associated with the degree of diastolic dysfunction. Compared with HIV- veterans, PLWH who are primarily virally suppressed and antiretroviral-treated did not differ in LA strain or LV diastolic dysfunction. If confirmed in other cohorts, HIV viral suppression may curtail adverse alterations in cardiac structure and function.

Abstract: Female sex workers (FSW) have worse HIV outcomes in part due to lower anti-retroviral therapy (ART) adherence. Substance use and depression are important barriers to ART adherence, yet few studies have assessed these relationships among FSW in longitudinal studies. Cross-Lagged Panel Models and autoregressive mediation analyses assessed substance use (illicit drug use and alcohol use disorders) in relation to ART non-adherence and the mediation role of depressive symptoms among 240 FSW living with HIV in the Dominican Republic. In annual visits (T1, T2, T3), the majority (70%, 66%, and 53%) reported at-risk drinking and 15%, 13% and 9% used illicit drug during the past 6 months. Most FSW (70%, 62% and 46%) had mild-to-severe depression. Illicit drug use predicted later ART non-adherence. This relationship was not mediated via depressive symptoms. Integrated substance use and HIV care interventions are needed to promote ART adherence and viral suppression among FSW.Las trabajadoras sexuales (TRSX) tienen peores resultados de VIH debido en parte a la menor adherencia a la terapia antirretroviral (TAR). El uso de sustancias y la depresión son barreras importantes para la adherencia; sin embargo, pocos estudios longitudinales han evaluado estas relaciones entre las TRSX. Utilizamos modelos de panel y análisis de mediación para evaluar el uso de sustancias en relación a la falta de adherencia al TAR y el papel de mediación de los síntomas depresivos entre 240 TRSX con VIH en la República Dominicana. En visitas anuales (T1, T2, T3), la mayoría (70%, 66%, and 53%) reportó consumo riesgoso de alcohol y 15%, 13% y 9% consumieron alguna droga ilícita durante los últimos 6 meses. La mayoría (70%, 62% y 46%) tenían depresión leve a grave. El uso de drogas ilícitas predijo la falta de adherencia al TAR. Esta relación no fue mediada por síntomas depresivos. Se necesitan intervenciones integradas de atención del VIH y el uso de sustancias para promover la adherencia al TAR y la supresión viral entre TRSX.

31. Examining Multilevel Correlates of Geographic Mobility in a Sample of US Young Adults Before and During the COVID-19 Pandemic

Abstract: Before the COVID-19 pandemic, geographic mobility, previously viewed as an indicator of economic stability, was declining among young adults. Yet, these trends shifted during the COVID-19 pandemic; young adults were more likely to move during COVID-19 for reasons related to reducing disease transmission and fewer educational and job opportunities. Few studies have documented the individual and neighborhood characteristics of young adults who moved before and during the pandemic. We used data from a cohort of young adults aged 18-34 in six metropolitan areas to examine individual- and neighborhood-level predictors of mobility before and during the COVID-19 pandemic. The sample was majority female, white, and educated with a bachelor's degree or more. Residents in neighborhoods they lived in were mostly White, US-born, employed, and lived above the poverty level. Before the pandemic, identifying as a sexual minority was significantly related to mobility. During the pandemic, being younger, single, and non-Hispanic were significantly related to mobility. Higher neighborhood poverty was significantly related to mobility before and during the COVID-19 pandemic. Future studies that examine young adult populations who moved during the pandemic are needed to determine whether COVID-19 related moves increase economic instability and subsequent health-related outcomes.

Abstract: U.S. Vietnamese experience HPV-related cancer disparities and HPV vaccine underutilization. We explored practice-, provider-, and patient-level influences on U.S. Vietnamese mothers' HPV vaccine decision-making for their female and male adolescent children. Using purposive sampling, we conducted 32 interviews among U.S. Vietnamese mothers. Data were analyzed using a hybrid thematic analysis approach. Findings indicated that practice-level barriers included limited clinic-based HPV promotion materials in Vietnamese and challenges in appointment scheduling. While provider recommendation emerged as a key facilitator of vaccine uptake, several mothers received either no recommendation or a low-quality recommendation. We found diverging patterns of vaccine acceptance following recommendation receipt. Patient-level barriers included misconceptions regarding the vaccine (particularly eligibility for males to get the vaccine), lack of health care utilization, perceived sexual inactivity, and safety concerns. We discuss the need for interventions addressing modifiable multilevel barriers to HPV vaccine acceptance and uptake among U.S. Vietnamese.

Abstract: Cannabidiol (CBD) product regulatory efforts must be informed by research regarding consumer perceptions. This mixed-methods study examined CBD product information sources, knowledge, perceptions, use and use intentions among young adults. This study analyzed (i) Fall 2020 survey data from 2464 US young adults (Mage = 24.67, 51.4% ever users, 32.0% past 6-month users) and (ii) Spring 2021 qualitative interviews among 40 survey participants (27.5% past-month users). Overall, 97.9% of survey participants reported having heard of CBD, 51.4% ever/lifetime use and 32.0% past 6-month use. Survey participants learned about CBD from friends/family (58.9%), products/ads at retailers (36.4%), online content/ads (34.8%), CBD stores (27.5%) and social media (26.7%). One-fourth believed that CBD products were required to be US Food and Drug Administration-approved (24.9%), tested for safety (28.8%) and proven effective to be marketed for pain, anxiety, sleep, etc. (27.2%). Survey and interview participants perceived CBD as safe, socially acceptable and effective for addressing pain, anxiety and sleep. Interview findings expanded on prominent sources of marketing and product exposure, including online and specialty retailers (e.g. vape shops), and on participants’ concerns regarding limited regulation and/or evidence regarding CBD’s effectiveness/risks. Given young adults’ misperceptions about CBD, surveillance of CBD knowledge, perceptions and use is critical as the CBD market expands.

34. ‘It’s like if a vape pen and a cigarette had a baby’: a mixed methods study of perceptions and use of IQOS among US young adults

Abstract: Young adults’ perceptions and use of heated tobacco products (HTPs) are understudied. This mixed methods study analyzed (i) Fall 2020 survey data from 2470 US young adults (meanage = 24.67; 19.5% and 25.2% past-month cigarette and e-cigarette use; 4.1% ever HTP use) assessing HTP use intentions and perceptions (1 = not at all to 7 = extremely) and (ii) Spring 2021 interview data regarding IQOS (most widely available HTP) in a subset of 40 e-cigarette users. Among survey participants, HTPs versus cigarettes and e-cigarettes showed lower use intentions (mean = 1.27 vs. mean = 1.73, mean = 2.16) but were perceived as less addictive (mean = 5.11 vs. mean = 6.28, mean = 5.82) and harmful (mean = 5.37 vs. mean = 6.65, mean = 5.62). HTPs were perceived more socially acceptable than cigarettes but less than e-cigarettes (mean = 3.39 vs. mean = 3.13, mean = 4.37). Among interviewees, most reported limited HTP knowledge. A few perceived IQOS as a hybrid of traditional cigarettes and e-cigarettes. Most perceived IQOS as harmful but less harmful than cigarettes and were uncertain in relation to e-cigarettes. Over half reported minimal interest in trying IQOS; common reasons included IQOS containing tobacco, limited flavors and use complexity. The varied perceptions of IQOS versus cigarettes and e-cigarettes underscore the need for continued surveillance of perceptions, use and marketing of IQOS to inform regulatory oversight and potential interventions.

36. The Rhetoric of Narcissism: Trump's Tweets on Writing

Abstract: Donald Trump's tweets on writing, whether his own or the print media's, typically employ an extreme form of rhetoric involving the manipulation of meaning and construction of self-serving arguments. The practice of close reading suggests that these tweets often display several types of rhetorical operation, which distort the message through the amalgamation, expansion, contraction, and reversal of meaning to create expressions like “the Fake News.” These polysemous expressions are then combined to form word groups, all centered on the self but each designed to meet a particular narcissistic need, from self-promotion and self-proclaimed victory to self-defense and self-casting as the Messiah. Trump's tweets often take the form of a triangular configuration, composed of the writer to proclaim, an adversary to be conquered, and a witness to validate the victory. By putting at least two of the three actors into play within its reduced space, the tweet becomes a miniature psychodrama—scripted, cast, and staged by the narcissist for an audience of kindred spirit.

Abstract: Autophagy is markedly impaired in Alzheimer’s disease (AD). Here we reveal unique autophagy dysregulation within neurons in five AD mouse models in vivo and identify its basis using a neuron-specific transgenic mRFP-eGFP-LC3 probe of autophagy and pH, multiplex confocal imaging and correlative light electron microscopy. Autolysosome acidification declines in neurons well before extracellular amyloid deposition, associated with markedly lowered vATPase activity and build-up of Aβ/APP-βCTF selectively within enlarged de-acidified autolysosomes. In more compromised yet still intact neurons, profuse Aβ-positive autophagic vacuoles (AVs) pack into large membrane blebs forming flower-like perikaryal rosettes. This unique pattern, termed PANTHOS (poisonous anthos (flower)), is also present in AD brains. Additional AVs coalesce into peri-nuclear networks of membrane tubules where fibrillar β-amyloid accumulates intraluminally. Lysosomal membrane permeabilization, cathepsin release and lysosomal cell death ensue, accompanied by microglial invasion. Quantitative analyses confirm that individual neurons exhibiting PANTHOS are the principal source of senile plaques in amyloid precursor protein AD models.

Abstract: BACKGROUND AND OBJECTIVES Alarm fatigue is exacerbated by frequent, nonactionable physiologic monitor alarms. Overutilization of pulse oximetry (SpO2) compounds this alarm burden. Narrow default alarm limits and overutilization of continuous (CSpO2) rather than intermittent monitoring contribute to nonactionable alarms. There were 1.12 million SpO2 alarms on included units during the baseline period, of which 41.0% were for SpO2 ≥ 88%. We aimed to decrease SpO2 alarms per patient day by 20% within 12 months. METHODS This quality improvement study included patients admitted January 2019 to June 2022. Intensive care and cardiology units were excluded. Interventions included (1) changing default alarm SpO2 limits on monitors from RESULTS Our study included 120 408 patient days with 2.98 million SpO2 alarms. Total SpO2 alarms and alarms for SpO2 ≥ 88% per patient day decreased by 5.48 (30.57 to 25.09; 17.9%) and 4.48 (12.50 to 8.02; 35.8%), respectively. Special cause improvement was associated with changing default monitor alarm parameters. Balancing measures remained stable. CONCLUSIONS SpO2 monitors alarm frequently at our children’s hospital. Widening default alarm limits was associated with decreased SpO2 alarms, particularly nonactionable alarms (≥88%). This high-reliability intervention may be applied, when appropriate, to other monitor alarm parameters to further mitigate alarm burden.

Abstract: The tumor microenvironment plays an essential role in supporting glioma stemness and radioresistance. Following radiotherapy, recurrent gliomas form in an irradiated microenvironment. Here we report that astrocytes, when pre-irradiated, increase stemness and survival of cocultured glioma cells. Tumor-naïve brains increased reactive astrocytes in response to radiation, and mice subjected to radiation prior to implantation of glioma cells developed more aggressive tumors. Extracellular matrix derived from irradiated astrocytes were found to be a major driver of this phenotype and astrocyte-derived transglutaminase 2 (TGM2) was identified as a promoter of glioma stemness and radioresistance. TGM2 levels increased after radiation in vivo and in recurrent human glioma, and TGM2 inhibitors abrogated glioma stemness and survival. These data suggest that irradiation of the brain results in the formation of a tumor-supportive microenvironment. Therapeutic targeting of radiation-induced, astrocyte-derived extracellular matrix proteins may enhance the efficacy of standard-of-care radiotherapy by reducing stemness in glioma.Significance:These findings presented here indicate that radiotherapy can result in a tumor-supportive microenvironment, the targeting of which may be necessary to overcome tumor cell therapeutic resistance and recurrence.

Abstract: Chemoradiation therapy is the mainstay for treatment of locally advanced, borderline resectable pancreatic cancer. Pharmacologic ascorbate (P-AscH−, i.e., intravenous infusions of ascorbic acid, vitamin C), but not oral ascorbate, produces high plasma concentrations capable of selective cytotoxicity to tumor cells. In doses achievable in humans, P-AscH− decreases the viability and proliferative capacity of pancreatic cancer via a hydrogen peroxide (H2O2)-mediated mechanism. In this study, we demonstrate that P-AscH− radiosensitizes pancreatic cancer cells but inhibits radiation-induced damage to normal cells. Specifically, radiation-induced decreases in clonogenic survival and double-stranded DNA breaks in tumor cells, but not in normal cells, were enhanced by P-AscH−, while radiation-induced intestinal damage, collagen deposition, and oxidative stress were also reduced with P-AscH− in normal tissue. We also report on our first-in-human phase I trial that infused P-AscH− during the radiotherapy “beam on.” Specifically, treatment with P-AscH− increased median overall survival compared with our institutional average (21.7 vs. 12.7 months, P = 0.08) and the E4201 trial (21.7 vs. 11.1 months). Progression-free survival in P-AscH−–treated subjects was also greater than our institutional average (13.7 vs. 4.6 months, P < 0.05) and the E4201 trial (6.0 months). Results indicated that P-AscH− in combination with gemcitabine and radiotherapy for locally advanced pancreatic adenocarcinoma is safe and well tolerated with suggestions of efficacy. Because of the potential effect size and minimal toxicity, our findings suggest that investigation of P-AscH− efficacy is warranted in a phase II clinical trial.Significance:These findings demonstrate that pharmacologic ascorbate enhances pancreatic tumor cell radiation cytotoxicity in addition to offering potential protection from radiation damage in normal surrounding tissue, making it an optimal agent for improving treatment of locally advanced pancreatic adenocarcinoma.

Abstract: Background: Smoking is detrimental to recovery and survival from cancer, but many cancer survivors continue to smoke. Information is lacking on smoking patterns of survivors many years after diagnosis and correlates of smoking status and patterns, likelihood of quitting, and intentions to quit.Methods: Cross-sectional analyses were conducted among survivors of 10 cancers recruited by stratified random sampling from cancer registries in a nationwide, longitudinal, quality-of-life study (n = 2,938).Results: Approximately 9 years after diagnosis, 9.3% of all survivors were current (past 30-day) smokers. Smoking prevalence was highest among survivors of bladder (17.2%), lung (14.9%), and ovarian (11.6%) cancers. Most current smokers (83%) smoked daily, averaging 14.7 cigarettes per day (cpd). Forty percent of daily smokers smoked more than 15 cpd. Nondaily smokers smoked a mean of 10.9 days in the last 30 days and averaged 5.7 cpd on smoking days. Current smoking was associated with younger age, lower education and income, and greater alcohol consumption. Quitting after diagnosis was associated with having a smoking-related cancer. Roughly, a third of current smokers intended to quit, 40% within the next month. The odds of intending to quit were lower if survivors were married, older, or smoked more.Conclusions: This population-based study indicated that smoking can persist long after initial diagnosis and at high levels and identified characteristics associated with quitting and intentions to quit.Impact: Findings can be used to identify survivors most at risk for continued smoking and to inform tailoring of cessation treatments for survivors.
