BACKGROUND Transcatheter Aortic Valve Implantation (TAVI) had revolutionized the treatment and outcome of symptomatic severe aortic stenosis (AS) since its introduction. In 2009, our center performed the first TAVI in Malaysia and since then it has provided an alternative treatment for severe AS patients. The objective of the study is to evaluate the clinical outcome of severe AS patients who had undergone TAVI in our center. METHODS A retrospective observational study was conducted. All TAVIs performed in our center between 01 Jan 2009 and 31 December 2020 were included. Patients’ baseline characteristic, clinical outcomes post procedure at 1-year and 3-year mortality were studied. Data were collected by using case report form and reviewing patient’s case note. RESULTS A total of 107 patients had TAVI performed during this period of time. The mean age (±SD) was 77.4 ± 5 (range 61-90 years). 50.5% of the patients were male (n = 54) and 45.8% were Malay ethnicity (n = 49). Majority of the patients had hypertension (86.9%, n = 93), dyslipidaemia (84.1%, n = 90) and ischemic heart disease (60.7%, n = 65). 26.2% of patients (n = 28) had previous coronary artery bypass surgery and 2.8% of patients (n = 3) had previous valvular operation. 85.9% of patient was in NYHA II – IV (n = 92). Mean EuroScore II and STS score were 4.82 and 4.564 respectively. Average hospitalization stay was 5 days. Total of 60 (56.1%) CoreValve were deployed followed by 46 Edwards Sapien (43%) and 1 MyVal (0.9%). 89.7% of procedure was performed via transfemoral access (n = 96) followed by transapical (6.5%, n = 7), transaortic (2.8%, n = 3) and subclavian (0.9%, n = 1). 98.1% (n = 105) procedure was successfully performed. There were 6 in-hospital mortality (5.6%) during indexed TAVI admission, of which 5 out of 6 were TAVI related. Kaplan Meier analysis of predicted survival rate with freedom from all-causes mortality at 1 year was 86.9% and 73.7% at 3 years. CONCLUSIONS Untreated severe AS was associated with poor outcome. High success rate of TAVI had provided another option of treatment to severe AS patient who are not the suitable candidate for surgical intervention.

This study addresses the improvement of first pass yield on the production floor of ABC Semiconductor. ABC is a company running a multiple volume complex product in aligning with memory business in the operation. First pass yield refers to the proportion of fully built product that pass testing without the need for additional rework. The first pass yield (FPY) project showed steady progression for its first five years. But over the following one year, the primary yield metric which is first pass yield has stayed stagnant. The goal of the paper presented here is to analyze the reasons for the current performance and propose novel ways of improving the metric again using quality management tools. The most common is using 7QC tools method by categorizing failure pareto that will lead to targeted corrective action to reduce the recurrence of pre-identified failure modes. A DMAIC methodology is establish on how to define an efficient corrective action, along with a top three major defect on data acquired from ABC organization. From the exercise, the yield loss was able to be reduced from 17.4% to 3.52%.

Electrical injuries are potentially devastating with high degree of morbidity and mortality. Deaths due to electrocution need to be thoroughly investigated and documented for reasons of compensation and for instituting future measures in relation to safety and prevention. In the present case, the deceased sustained electrocution while working at a function pandal and died on the spot. An attempt is made to discuss about electrocution and related injuries.

Aluminium phosphide is a solid fumigant pesticide, insecticide and rodenticide. Aluminium phosphide is used as a poison either by inhalation or ingestion. Poisoning is usually suicidal, occasionally accidental and rarely homicidal. It is commonly known as an agent of sure death in the medical profession. Starvation is regular and continuous deprivation of food and drink, both necessary for the maintenance of sound health and normal active life of a person. A case is presented wherein an elderly male was reported to have died of starvation.

A study was conducted to ascertain how much variation in allergenic potency of ragweed pollen occurs among individual plants and different plant populations in a restricted geographic area. Seeds of the short ragweed (Ambrosia artemisiifolia) were collected from 38 different sites in Champaign County, Ill. Plants from these seeds were grown under similar conditions, and antigen E (AgE) was determined on pollen samples harvested from individual plants. The populations from various sites in Champaign County differed by as much as severalfold with respect to mean AgE values. There were statistically significant differences among population means, although large variations occurred among plants within populations. Further research was conducted to learn whether the level of AgE was determined by genetic factors within the plants. Genetic regulation was indicated by the finding that 3 plants which had unusually low levels of pollen AgE all produced offspring with significantly lower mean AgE levels than those of control plants.
