Objective: To evaluate the relationship between magnetic resonance imaging (MRI) findings and histopathologic findings, including histologic type, histologic grade, extensive intraductal component (EIC), estrogen receptor (ER), progesterone receptor (PR), Ki-67, P53, and molecular subtypes in breast cancer. Materials and Methods: The present data was a retrospective review of the patients that underwent breast MRI at King Chulalongkorn Memorial Hospital between January 1, 2014 and December 31, 2016. MRI was assessed for shape, margin, and internal enhancement of the mass lesion, distribution and internal enhancement of the non-mass enhancement lesions, and kinetic curve type and restricted diffusion of both mass and non-mass enhancement lesions. The association between MRI findings and histologic type, histologic grade, EIC, ER, PR, human epidermal growth factor receptor 2 (HER2), Ki-67, P53, and molecular subtypes were evaluated. Results: One hundred seven patients with invasive breast cancers were included in the present study with 99 mass lesions and 8 non-mass enhancement lesions. Rim enhancement of the mass was a significant independent predictor of grade III (high) histologic grade (p=0.001, odd ratio 4.552), negative ER (p=0.001, odd ratio 4.644), negative PR (p=0.021, odd ratio 2.679), and positive Ki-67 status (p=0.022, odd ratio 3.373). Internal enhancement was significantly associated with molecular subtypes such as luminal A subtype appearing as heterogeneous enhancement for 78.3%, HER2-overexpressed and triple negative subtypes dominantly presented with rim enhancement for 76.9% and 75%, respectively. The authors found that spiculated tumor margin of the mass was significantly associated with negative EIC (p=0.006). Restricted diffusion of all lesions was also a significant independent predictor of negative EIC (p=0.022, odd ratio 7.417) and Ki-67 positive (p=0.028, odd ratio 7.182). Conclusion: The consistency of the association between rim enhancement and high histologic grade, negative ER/PR, positive Ki-67 statuses in the present study may help to determine that rim enhancement was likely to predict poor pathologic prognosis of breast cancer. Rim enhancement may also predict poor molecular subtypes including HER2-overexpressed and triple negative subtypes. These results suggest that rim enhancement is the most useful MRI finding to predict histopathology and molecular subtypes of breast cancer. [ABSTRACT FROM AUTHOR]

Purpose: To determine the diagnostic value of breast magnetic resonance imaging (MRI) in detecting axillary metastatic node in newly diagnosed breast cancer, we assessed the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of breast MRI., Material and Methods: Data were collected retrospectively from January 2011 to December 2018. Preoperative breast MRI of women with newly diagnosed breast cancer were reviewed to evaluate the axillary nodal staging by using the morphological characteristic of metastatic axillary lymph node. Statistical analysis was performed to assess the performance of breast MRI in diagnosing metastatic axillary lymph nodes based on the pathological result., Results: A total of 131 women with breast cancer were analysed. Two hundred and twenty-seven axillary lymph nodes from preoperative breast MRIs were considered to be metastasis. 65.65% (86 patients) of the breast MRI results matched with the pathological results: 37 patients with N0 stage, 39 patients with N1 stage, eight patients with N2 stage, and two patients with N3 stage. Sensitivity of breast MRI for axillary nodal staging was 98.5% (95% CI: 92-100%), and the negative predictive value was 96.4% (86.2-99.9%). Specificity of breast MRI for axillary nodal staging was 57.8% (44.8-70.1%) and the positive predictive value was 71% (60.6-79.9%)., Conclusions: Our study showed that the breast MRI had a high sensitivity (98.5%) and high NPV (96.4%) in detecting metastatic axillary lymph nodes, but its specificity was only fair (57.8%). We reported a case of an African American woman who went to the hospital with palpable right breast lump with bloody nipple discharge at University of Texas Medical Branch at Galveston. The modalities of breast imagings included mammography and ultrasonography. The method used for viral identification was Linear Array HPV genotyping test. Intraductal papilloma revealed as high density tubular or rounded lobular masses with partially circumscribed, obscured margins and clustered punctate microcalcifications on mammograms. Ultrasound showed as intraductal masses with dilated ducts. The core biopsy demonstrated duct filled with papillary lesion and post excision revealed intraductal papilloma. HPV DNA types 16, 33, 58 and 71 were detected after use of Linear Array HPV genotyping test.

Objectives: To assess the proportions of response to neoadjuvant chemotherapy of breast cancer according to color Doppler ultrasound vascularity patterns., Material and Method: Prospective evaluation using gray scale and color Doppler ultrasound for number of vessels and feeding appearances of tumors as well as the changes in tumor sizes at before and after chemotherapy in 69 breast cancer patients., Results: The overall response rate in 70 breast cancers was 20%. Twenty-nine lesions (41%) showed hypervascularity and 41 lesions (59%) revealed hypovascularity. There were 5 vascularity patterns and each pattern had the proportion of responders as follows; 33.3% for hypovascularity with single-vessel feeding into the tumor, 25% for hypovascularity with single-vessel feeding at periphery of the tumor, 25% for no vascular feeding to the tumor, 16.7% for hypervascularity with vascular feeding at the periphery of the tumor and 13% for hypervascularity with vascular feeding into the tumor. The highest percentage of responsive group was the pattern of hypovascularity with single-vessel feeding into the tumor (33.3%)., Conclusion: There is a trend that tumor vascularity and patterns of vascular feeding by color Doppler ultrasound can predict the responsiveness of breast cancer to neoadjuvant chemotherapy.
