1. Spontaneous fission half-lives of actinides and super-heavy elements

Abstract

Spontaneous fission half-lives of actinide and super-heavy nuclei are calculated, using the least-action integral, through the WKB tunneling probability of the barrier that appears in the deformation landscape obtained in the macroscopic-microscopic potential-energy surface. This deformation-energy landscape is obtained using a Fourier shape parametrization with 4 deformation parameters, taking into account the nuclear elongation, left-right asymmetry, neck formation and non-axiality degrees of freedom. The collective inertia tensor entering the WKB half-life expression is given through the so-called irrotational flow approach, successfully used in nuclear fission to reproduce observables that characterize the nuclear system in the vicinity of the scission configurations, such as fragment mass or charge distributions. For a comparisons, we have also used the so-called phenomenological mass parameter depending only on the center-of-mass difference of the forming fission fragments. Our approach is shown to be able to reproduce empirical fission half-lives of all here considered nuclei to within 3 orders of magnitude.

Abstract

Landslides pose a significant threat worldwide, leading to numerous fatalities and severe economic losses. The city of Manizales, located in the Colombian Andes, is particularly vulnerable due to its steep topography and permeable volcanic ash-derived soils. This study aims to assess landslide hazards in Manizales by integrating shallow planar and deep-seated circular failure mechanisms using physics-based models (TRIGRS and Scoops3D). By combining hazard zonation maps with rainfall thresholds calibrated through historical data, we provide a refined approach for early warning systems (EWS) in the region. Our results underscore the significance of the landslide hazard maps, which combine shallow planar and deep-seated circular failure scenarios. By categorizing urban areas into high, medium, and low-risk zones, we offer a practical framework for urban planning. Moreover, we developed physics-based rainfall thresholds for early landslide warning, simplifying their application while aiming to enhance regional predictive accuracy. This comprehensive approach equips local authorities with essential tools to mitigate landslide risks, refine hazard zoning, and strengthen early warning systems, promoting safer urban development in the Andean region and beyond, as the physics-based methods used are well-established and implemented globally.

Abstract

The integration of distributed energy resources requires the implementation of control and automation functionalities in distribution networks, which allow them to operate in a more flexible, efficient, and reliable way. The operation of these functionalities causes topological changes on the network that must be identified since these affect protection, volt/var control, and state estimation, among others. This paper presents a data-driven topology detector for self-healing strategies in Active Distribution Networks (ADN). This approach uses machine learning (ML) techniques to obtain a trained model as a topology detector. The ML models are integrated into the Intelligent Electronic Device (IED) of ADN so that using the voltage and current signals measured locally determine the network’s topology. A features selection and tuning technique based on a multiverse optimizer are proposed to improve the ML model accuracy. This approach allows it to be implemented in decentralized architectures since each IED detects the system’s topology from local measurements and does not depend on the availability of the communication system. The proposed topology detector was validated on a modified IEEE 123 nodes test feeder considering six topology changes, five DER outages, and five load variations. The results obtained show accuracy values above 96%, which evidences a highlighted potential for real-life applications.

Abstract

Smart networks such as active distribution network (ADN) and microgrid (MG) play an important role in power system operation. The design and implementation of appropriate protection systems for MG and ADN must be addressed, which imposes new technical challenges. This paper presents the implementation and validation aspects of an adaptive fault detection strategy based on neural networks (NNs) and multiple sampling points for ADN and MG. The solution is implemented on an edge device. NNs are used to derive a data-driven model that uses only local measurements to detect fault states of the network without the need for communication infrastructure. Multiple sampling points are used to derive a data-driven model, which allows the generalization considering the implementation in physical systems. The adaptive fault detector model is implemented on a Jetson Nano system, which is a single-board computer (SBC) with a small graphic processing unit (GPU) intended to run machine learning loads at the edge. The proposed method is tested in a physical, real-life, low-voltage network located at Universidad del Norte, Colombia. This testing network is based on the IEEE 13-node test feeder scaled down to 220 V. The validation in a simulation environment shows the accuracy and dependability above 99.6%, while the real-time tests show the accuracy and dependability of 95.5% and 100%, respectively. Without hard-to-derive parameters, the easy-to-implement embedded model highlights the potential for real-life applications.

Abstract

The present research aimed to verify the effect of 10 weeks of structured FT grouped by muscular actions (GFT) or alternating actions (AFT) on scapular and lumbar-pelvic girdle stability. One hundred and twenty adults (60 men; 60 women) were allocated into three groups, GFT (n= 40) that performed the actions in sequence (squat - squat - pull - pull), AFT (n= 40) that performed alternate actions (squat - pull - squat - pull) and the control group (CG, n= 40). The shoulder girdle and pelvic girdle stability were assessed using the Octobalance Upper Body Test. The GFT increased stability after the intervention and compared to the CG (p= 0.003) as assessed by the relative range of the right (ES= 0.53) and left (ES= 0.57) hemispheres. Besides, most results were within the instrument's error value and the magnitude of the effect was moderate to trivial among the experimental groups. Conclusions: Therefore, ten weeks of functional training performed in a grouped sequence promoted improvements in scapular and lumbar-pelvic girdle stability.

11. GaitCopy: Disentangling Appearance for Gait Recognition by Signature Copy

Abstract

This paper addresses the problem of gait-based people identification by copying optical flow-based signatures. The proposed model, coined as GaitCopy, receives as input a stack of gray images and returns the gait signature of the represented subject. The novel property of this network is that it is not trained to only generate discriminative signatures, but to copy signatures generated by a Master network trained on optical flow inputs. Then, GaitCopy is enforced to extract signatures based on motion and not based on appearance, despite having been trained with pixel inputs. We implement two different versions of GaitCopy, one mainly composed of 3D convolutional layers to capture local temporal information; and a second one based on GaitSet which uses 2D convolutional layers under a temporal setup. We evaluate our approach on two public gait datasets: CASIA-B and TUM-GAID. We observe that compact networks, up to $\times 4.2$ smaller for TUM-GAID, can be obtained by using our approach, while keeping a competitive recognition accuracy with respect to the state of the art, and without the need of explicit optical flow computation. Even with such network compression, the results obtained in TUM-GAID are comparable to those of the state of the art, with an average accuracy of 97% on the test set. Abstract

Background: COVID-19 is a highly contagious viral infection with high morbidity that is draining health resources. The biggest complication is pneumonia, which has a serious inflammatory component, with no standardized treatment. Low-dose radiation therapy (LD-RT) is non-invasive and has anti-inflammatory effects that can interfere with the inflammatory cascade, thus reducing the severity of associated cytokine release and might be useful in the treatment of respiratory complications caused by COVID-19. Study design and methods: This multicentric prospective clinical trial seeks to evaluate the efficacy of bilateral lung LD-RT therapy as a treatment for interstitial pneumonia in patients with COVID-19 for improving respiratory function.This prospective study will have 2 phases: I) an exploratory phase enrolling 10 patients, which will assess the feasibility and efficacy of low-dose lung irradiation, evaluated according to an increase in the PaO2/FiO2 ratio of at least 20% at 48–72 h with respect to the pre-irradiation value. If a minimum efficiency of 30% of the patients is not achieved, the study will not be continued. II) Non-randomized comparative phase in two groups: a control group, which will only receive pharmacological treatment, and an experimental arm with pharmacological treatment and LD-RT. It will include 96 patients, the allocation will be 1: 2, that is, 32 in the control arm and 64 in the experimental arm. The primary end-point will be the efficacy of LD-RT in patients with COVID-19 pneumonia according to an improvement in PaO2/FiO2. Secondary objectives will include the safety of bilateral lung LD-RT, an improvement in the radiology image, overall mortality rates at 15 and 30 days after irradiation and characterizing anti-inflammatory mechanisms of LD-RT by measuring the level of expression of adhesion molecules, anti-inflammatory cytokines and oxidative stress mediators.Trial registration: ClinicalTrial.gov NCT-04380818 https://clinicaltrials.gov/ct2/show/NCT04380818?term=RADIOTHERAPY&cond=COVID&draw=2&rank=4.

Introducción: el cuestionario “Assessment of Spondyloarthritis International Society Health In- dex” (ASAS-HI) fue desarrollado para medir de manera global la funcionalidad y el estado de salud en pacientes con espondiloartritis (EspA). Se han propuesto puntos de corte para determinar diferentes estados de salud que fueron poco evaluados en pacientes de la vida real. Objetivos: describir el estado de salud medido por ASAS-HI en pacientes argentinos con EspA axial (EspAax) y periférica (EspAp) en la práctica diaria y evaluar los factores asociados al pobre estado de salud. Materiales y métodos: Estudio de corte transversal, analítico y multicéntrico. Se incluyeron consecutivamente pacientes con EspAax y EspAp según criterios ASAS, de 15 centros argentinos. Análisis estadístico: se realizó estadística descriptiva, análisis bivariado y multivariado (regresión logística múltiple) para evaluar los factores asociados al pobre estado de salud (ASAS-HI ≥12). Para analizar la validez de constructo de la herramienta se realizó correlación de Spearman entre el ASAS-HI y otros parámetros de evaluación de la enfermedad. Resultados: se incluyeron 274 pacientes con EspA, con una edad media de 49 (±14) años y una duración mediana de la enfermedad de 62 meses (p25-75: 24-135), 155 (56,6%) de los pacientes eran de sexo masculino, 129 pacientes (47%) con EspAax y 145 (52,9%) EspAp. Según el ASAS-HI 119 pacientes (43,4%) presentaban buen estado de salud, 117 (42,7%) tenían estado de salud moderado y 38 (13.9%) pobre estado de salud. Quantum and collisional effects on the unstable spectrum of a relativistic electron beam-plasma system are investigated through a two-fluids model. Application is made to the near target center interaction of the relativistic electron beam in the Fast Ignition Scenario. Partial degeneracy effects are found negligible while the most influential factors are the beam temperature and the electron-ion collision frequency of the plasma. The introduction of the latter triggers some oblique unstable modes of much larger wave length than the collisionless ones. Abstract

Abstract Background Ovarian cancer is the leading cause of gynecologic cancer-related death, due in part to a late diagnosis and a high rate of recurrence. Primary and acquired platinum resistance is related to a low response probability to subsequent lines of treatment and to a poor survival. Therefore, a comprehensive understanding of the mechanisms that drive platinum resistance is urgently needed. Methods We used bioinformatics analysis of public databases and RT-qPCR to quantitate the relative gene expression profiles of ovarian tumors. Many of the dysregulated genes were cancer stem cell (CSC) factors, and we analyzed its relation to therapeutic resistance in human primary tumors. We also performed clustering and in vitro analyses of therapy cytotoxicity in tumorspheres. Results Using bioinformatics analysis, we identified transcriptional targets that are common endpoints of genetic alterations linked to platinum resistance in ovarian tumors. Most of these genes are grouped into 4 main clusters related to the CSC phenotype, including the DNA damage, Notch and C-KIT/MAPK/MEK pathways. The relative expression of these genes, either alone or in combination, is related to prognosis and provide a connection between platinum resistance and the CSC phenotype. However, the expression of the CSC-related markers was heterogeneous in the resistant tumors, most likely because there were different CSC pools. Furthermore, our in vitro results showed that the inhibition of the CSC-related targets lying at the intersection of the DNA damage, Notch and C-KIT/MAPK/MEK pathways sensitize CSC-enriched tumorspheres to platinum therapies, suggesting a new option for the treatment of patients with platinum-resistant ovarian cancer. Conclusions The current study presents a new approach to target the physiology of resistant ovarian tumor cells through the identification of core biomarkers. We hypothesize that the identified mutations confer platinum resistance by converging to activate a few pathways and to induce the expression of a few common, measurable and targetable essential genes. These pathways include the DNA damage, Notch and C-KIT/MAPK/MEK pathways. Finally, the combined inhibition of one of these pathways with platinum treatment increases the sensitivity of CSC-enriched tumorspheres to low doses of platinum, suggesting a new treatment for ovarian cancer.

Abstract

People identification using gait information (i.e., the way a person walks) obtained from inertial sensors is a robust approach that can be used in multiple situations where vision-based systems are not applicable. Typically, previous methods use hand-crafted features or deep learning approaches with pre-processed features as input. In contrast, we present a new deep learning-based end-to-end approach that employs raw inertial data as input. By this way, our approach is able to automatically learn the best representations without any constraint introduced by the pre-processed features. Moreover, we study the fusion of information from multiple inertial sensors and multi-task learning from multiple labels per sample. Our proposal is experimentally validated on the challenging dataset OU-ISIR, which is the largest available dataset for gait recognition using inertial information. After conducting an extensive set of experiments to obtain the best hyper-parameters, our approach is able to achieve state-of-the-art results. Specifically, we improve both the identification accuracy (from 83.8% to 94.8%) and the authentication equal-error-rate (from 5.6 to 1.1). Our experimental results suggest that: 1) the use of hand-crafted features is not necessary for this task as deep learning approaches using raw data achieve better results; 2) the fusion of information from multiple sensors allows to improve the results; and, 3) multi-task learning is able to produce a single model that obtains similar or even better results in multiple tasks than the corresponding models trained for a single task.

22. MCQ-Balance: a method to monitor patients with balance disorders and improve clinical interpretation of posturography

Abstract

Background An estimated 20% to 30% of the global population has suffered a vertiginous episode. Among them, 20% do not receive a clear diagnosis. Improved methods, indicators and metrics are necessary to assess the sensory systems related to balance, especially when patients are undergoing treatment for vertiginous episodes. Patients with balance disorders should be monitored for changes at the individual level to gather objective information. In this study, we evaluate the use of the MCQ-Balance (Measure, Classify and Qualify) assessment for examining a patient’s balance progression using tests to measure static balance control and dynamic postural balance with a stabilometric platform. Materials and Methods The MCQ-Balance assessment comprises three stages: (i) measuring the progression of each variable between two separate and consecutive days (called sessions) using the Magnitude-Based Decision analysis; (ii) classifying the progression of the patient’s balance with a score; and (iii) qualifying the progression of the patient’s balance from the resulting scores using a set of rules. This method was applied to 42 patients with balance disorders of peripheral or central origin characterised by vertigo as the cardinal symptom. Balance progression was measured using the MCQ-Balance assessment over the course of three months, and these results were compared with the assessment of a clinical expert. Results The MCQ-Balance assessment showed an accuracy of 83.4% and a Cohen’s Kappa coefficient of 0.752 compared to the assessment of a clinical expert. Conclusion The MCQ-Balance assessment facilitates the monitoring of patient balance and provides objective information that has the potential to improve medical decision making and the adjustment of individual treatment.

Abstract

BackgroundWhole-body vibration (WBV) is a method utilizing vibrating platforms to expose individuals to mechanical vibration. In its various applications, it has been linked to improved muscular, skeletal, metabolic, or cognitive functioning, quality of life, and physiological parameters such as blood pressure. Most evidence concerning WBV is inconclusive and meta-analytical reviews may not readily produce insights since the research has a risk of misunderstandings of vibration parameters and incomplete reporting occurs. This study aims at laying an empirical foundation for reporting guidelines for human WBV studies to improve the quality of reporting and the currently limited comparability between studies.MethodThe Delphi methodology is employed to exploit the integrated knowledge of WBV experts to distil the specific aspects of WBV methodology that should be included in such guidelines. Over three rounds of completing online questionnaires, the expert panel (round 1/2/3: 51/40/37 experts respectively from 17 countries with an average of 19.4 years of WBV research experience) rated candidate items.ResultsA 40-item list was established based on the ratings of the individual items from the expert panel with a large final consensus (94.6%).ConclusionThe final consensus indicates comprehensiveness and valuableness of the list. The results are in line with previous guidelines but expand these extensively. The present results may therefore serve as a foundation for updated guidelines for reporting human WBV studies in order to improve the quality of reporting of WBV studies, improve comparability of studies and facilitate the development of WBV study designs.

24. A Systematic Review of the Prospective Relationship between Child Maltreatment and Chronic Pain

Abstract

Objective: The present systematic review aimed to evaluate the association between childhood maltreatment and chronic pain, with specific attention to the temporal nature of the relationship and putative moderators, including, the nature (type), timing of occurrence, and magnitude of maltreatment; whether physical harm or injury occurred; and whether post-traumatic stress disorder (PTSD) subsequently developed. Method: We included studies that measured the prospective relationship between child maltreatment and pain. Medline, EMBASE, PsycINFO, and CINAHL were searched electronically up to 28 July 2019. We used accepted methodological procedures common to prognosis studies and preregistered our review (PROSPERO record ID 142169) as per Cochrane review recommendations. Results: Nine studies (17,340 participants) were included in the present review. Baseline participant age ranged from 2 years to more than 65 years. Follow-up intervals ranged from one year to 16 years. Of the nine studies included, three were deemed to have a high risk of bias. With the exception of one meta-analysis of three studies, results were combined using narrative synthesis. Results showed low to very low quality and conflicting evidence across the various types of maltreatment, with the higher quality studies pointing to the absence of direct (non-moderated and non-mediated) associations between maltreatment and pain. PTSD was revealed to be a potential mediator and/or moderator. Evidence was not found for other proposed moderators. Conclusions: Overall, there is an absence of evidence from high quality studies of an association between maltreatment and pain. Our results are limited by the small number of studies reporting the relationship between child maltreatment and pain using a prospective design. High quality studies, including prospective cohort studies and those that assess and report on the moderators described above, are needed to advance the literature.

Abstract

Poisoning results from the ingestion of or contact with harmful substances including overdose or incorrect use of any drug or medication. Decontamination measures prevent the absorption of the substance from the gastrointestinal tract to minimize systemic effects. Activated charcoal is the intervention most frequently used in the initial management of patients with acute intoxications. The use of activated charcoal has decreased over time, but there may be a subgroup of patients who would benefit from its use. In this review we describe the epidemiology of intoxications, the composition and pharmacology of activated charcoal, indications and dosing for use of single dose and multiple doses of charcoal, contraindications, complications and a summary for recommended use of this measure based on published studies.

Abstract

Objective: The aim of this study was to review the literature about the relevance of the whole body vibration (WBV) in decreasing the number of fractures in osteoporotic women. Methods: Searches were performed by three independent researchers through the PubMed and PEDro databases. Results: Only 0.1% of the publications with “Fracture and osteoporosis” have a relation with WBV exercise. The achievements have revealed a positive effect of this exercise in patients with risk factors for fractures like osteoporosis. Protocols were performed two to three times a week, from 6 up to 18 months, and with 12.6 up to 40 Hz as frequencies. Different tools were used to evaluate the effects of the WBV exercise in conditions that could cause fractures in postmenopausal women. Conclusions: Although the paucity of research regarding direct effects of WBV in decreasing fractures, WBV could be a feasible and effective way to modify well-recognized risk factors for falls and fractures, improvements in some aspects of neuromuscular function and balance. More studies have to be performed establish protocols with well controlled parameters.

Abstract

Background The primary aim of our study was to investigate the association between intubation timing and hospital mortality in critically ill patients with coronavirus disease 2019 (COVID-19)-associated respiratory failure. We also analysed both the impact of such timing throughout the first four pandemic waves and the influence of prior noninvasive respiratory support on outcomes.Methods This is a secondary analysis of a multicentre, observational and prospective cohort study that included all consecutive patients undergoing invasive mechanical ventilation due to COVID-19 from across 58 Spanish intensive care units (ICUs) participating in the CIBERESUCICOVID project. The study period was between 29 February 2020 and 31 August 2021. Early intubation was defined as that occurring within the first 24 h of ICU admission. Propensity score matching was used to achieve a balance across baseline variables between the early intubation cohort and those patients who were intubated after the first 24 h of ICU admission. Differences in outcomes between early and delayed intubation were also assessed. We performed sensitivity analyses to consider a different time-point (48 h from ICU admission) for early and delayed intubation.Results Of the 2725 patients who received invasive mechanical ventilation, a total of 614 matched patients were included in the analysis (307 for each group). In the unmatched population, there were no differences in mortality between the early and delayed groups. After propensity score matching, patients with delayed intubation presented higher hospital mortality (27.3% versus 37.1%; p=0.01), ICU mortality (25.7% versus 36.1%; p=0.007) and 90-day mortality (30.9% versus 40.2%; p=0.02) compared with the early intubation group. Abstract

The concern for the amount of end-of-life tires generated each year has arisen from constant research directed to their valorisation. Herein we propose a new material, which is constituted by GTR with a binder, as acoustic and also as a thermal insulator for civil engineering. The insulator can also include the fibre mat present in the tire, seldomly considered as a recyclable sub-product. To provide insight into the insulating behaviour of these materials, four mathematical models have been tested and compared with the experimental results of thermal conductivity. The Lewis-Nielsen modelization presented good accuracy with deviations of less than 3%. A statistical analysis has also been conducted on the experimental data showing that the parameter with more effect on thermal conductivity is thickness (differences up to 43%) being particle size, less important (ca 6%). In acoustic properties, different effects can be observed depending on the frequency range, being the density the most relevant. From the mathematical, statistical and experimental analysis can be deduced that good insulation properties would be achieved in materials with: low density, porous; including mat and thick. The effect of these parameters causes variations of thermal conductivity from 0.189 to 0.117 W/m·K and in sound absorption coefficient from 0.06 to 0.6.

Abstract

Background Pain is one of the most common and most difficult symptoms to manage in adult burn patients in the Intensive Care Unit (ICU). Insufficient or unsuccessful pain management can negatively affect physiological, psychological and social health in burn patients, both during and after hospitalization. Music therapy and music medicine interventions have been shown to positively affect pain and mental health in this population. This systematic review and meta-analysis provide an update of Randomized Controlled Trials (RCTs) using music therapy or music medicine interventions in adult burn patients. Methods A variety of databases were searched from their beginning to June 2020, including PsycINFO and PsycArticles (via APAsycNET), PubMed and MEDLINE (via OvidSP), Scopus, Web of Science, and the Cochrane Library. Data of all articles meeting the inclusion criteria were extracted, organized and processed according to the PRISMA guidelines. Statistical analysis was performed using Q-test and I2 statistics. Results 10 RCTs with a total of 1,061 participants were included. The results of the meta-analysis showed a statistically significant reduction of pain (I2 = 96.03%, P Conclusions This review provides preliminary evidence for the effectiveness of music interventions for adult burn patients. However, more high-quality RCTs are needed to safely establish guidelines for music therapists and other health care professionals in using music for health purposes with this population.

Abstract

Background During the first wave of the COVID-19 pandemic, shortages of ventilators and ICU beds overwhelmed health-care systems. Whether early tracheostomy reduces the duration of mechanical ventilation and ICU stay is controversial. Research Question Can failure-free day outcomes focused on ICU resources help to decide the optimal timing of tracheostomy in overburdened health-care systems during viral epidemics? Study Design and Methods This retrospective cohort study included consecutive patients with COVID-19 pneumonia who had undergone tracheostomy in 15 Spanish ICUs during the surge, when ICU occupancy modified clinician criteria to perform tracheostomy in Patients with COVID-19. We compared ventilator-free days at 28 and 60 days and ICU- and hospital bed-free days at 28 and 60 days in propensity score-matched cohorts who underwent tracheostomy at different timings (≤ 7 days, 8-10 days, and 11-14 days after intubation). Results Of 1,939 patients admitted with COVID-19 pneumonia, 682 (35.2%) underwent tracheostomy, 382 (56%) within 14 days. Earlier tracheostomy was associated with more ventilator-free days at 28 days (≤ 7 days vs > 7 days [116 patients included in the analysis]: median, 9 days [interquartile range (IQR), 0-15 days] vs 3 days [IQR, 0-7 days]; difference between groups, 4.5 days; 95% CI, 2.3-6.7 days; 8-10 days vs > 10 days [222 patients analyzed]: 6 days [IQR, 0-10 days] vs 0 days [IQR, 0-6 days]; difference, 3.1 days; 95% CI, 1.7-4.5 days; 11-14 days vs > 14 days [318 patients analyzed]: 4 days [IQR, 0-9 days] vs 0 days [IQR, 0-2 days]; difference, 3 days; 95% CI, 2.1-3.9 days). Except hospital bed-free days at 28 days, all other end points were better with early tracheostomy. Interpretation Optimal timing of tracheostomy may improve patient outcomes and may alleviate ICU capacity strain during the COVID-19 pandemic without increasing mortality. Tracheostomy within the first work on a ventilator in particular may improve ICU availability.

Abstract

In the rehabilitation field, clinicians are continually struggling to assess improvements in patients following interventions. In this paper, we propose an approach to use gait analysis based on inertial motion capture (MoCap) to monitor individuals during rehabilitation. Gait is a cyclical movement that generates a sufficiently large data sample in each capture session to statistically compare two different sessions from a single patient. Using this crucial idea, 21 heterogeneous patients with hemiplegic spasticity were assessed using gait analysis before and after receiving treatment with botulinum toxin injections. Afterwards, the two sessions for each patient were compared using the magnitude-based decision statistical method. Due to the challenge of classifying changes in gait variables such as improvements or impairments, assessing each patient’s progress required an interpretative process. After completing this process, we determined that 10 patients showed overall improvement, five patients showed overall impairment, and six patients did not show any overall change. Finally, the interpretation process was summarized by developing guidelines to aid in future assessments. In this manner, our approach provides graphical information about the patients’ progress to assess improvement following intervention and to support decision-making. This research contributes to integrating MoCap-based gait analysis into rehabilitation.

Abstract

Given the exponential availability of data in health centers and the massive sensorization that is expected, there is an increasing need to manage and analyze these data in an effective way. For this purpose, data mining (DM) and machine learning (ML) techniques would be helpful. However, due to the specific characteristics of the field of healthcare, a suitable DM and ML methodology adapted to these particularities is required. The applied methodology must structure the different stages needed for data-driven healthcare, from the acquisition of raw data to decision-making by clinicians, considering the specific requirements of this field. In this paper, we focus on a case study of cervical assessment, where the goal is to predict the potential presence of cervical pain in patients affected with whiplash diseases, which is important for example in insurance-related investigations. By analyzing in detail this case study in a real scenario, we show how taking care of those particularities enables the generation of reliable predictive models in the field of healthcare. Using a database of 302 samples, we have generated several predictive models, including logistic regression, support vector machines, k-nearest neighbors, gradient boosting, decision trees, random forest, and neural network algorithms. The results show that it is possible to reliably predict the presence of cervical pain (accuracy, precision, and recall above 90%). We expect that the procedure proposed to apply ML techniques in the field of healthcare will help technologists, researchers, and clinicians to create more objective systems that provide support to objectify the diagnosis, improve test treatment efficacy, and save resources.

The aim of the study was to assess the acute effect of whole-body vibration (WBV) exercise, with low frequency (5 Hz), on the pain level (PL), trunk flexibility, and cardiovascular responses (blood pressure [BP] and heart rate [HR]) in individuals with metabolic syndrome (MetS). Forty-four individuals were included in the study (control: 15) or in (WBV exercise: 29) groups. They were submitted to 3 bouts (1 minute each) of WBV exercise (5 Hz and peak-to-peak displacements of 2.5, 5.0, and 7.5 mm, corresponding to peak accelerations of 0.12, 0.25, and 0.35 g, respectively, sitting in a chair with the feet on the platform with knees flexed, followed by 1 minute of interset rest. The Control Group performed the same protocol, but the platform was turned off. The PL was measured through the visual analog pain scale, and the flexibility was measured through the anterior trunk flexion test. Significant improvements on PL ( P = .031) and flexibility ( P = .004) were found only in the WBV exercise group. The BP and HR remained at physiological levels. In conclusion, the WBV exercise would lead to physiological response decreasing PL and increasing flexibility as well as maintaining the cardiovascular responses in individuals with MetS.

Background & aimsSerum interferon-gamma-inducible protein-10 (IP-10) is elevated in cholestatic liver diseases and predicts response to antiviral therapy in patients with chronic hepatitis C virus (HCV) infection. Dipeptidylpeptidase 4 (DPPIV) cleaves active IP-10 into an inactive form, which inhibits recruitment of CXCR3+ T cells to the liver. In this study the link between IP-10 levels, DPPIV activity in serum and CXCR3+ T cells is analysed in cholestatic and non-cholestatic liver patients.MethodsIn serum DPPIV activity (by enzymatic assay), IP-10 (by ELISA) and bile acids (BA) (by enzymatic assay) were analysed in 229 naive HCV genotype (GT) 1 patients and in 16 patients with cholestatic liver disease. In a prospective follow-up (FU) cohort of 27 HCV GT 1 patients peripheral CD3+CXCR3+, CD4+CXCR3+ and CD8+CXCR3+ cells were measured by FACS.ResultsIn 229 HCV patients serum IP-10 levels correlated positively to DPPIV serum activity. Higher IP-10 levels and DPPIV activity were detected in cholestatic and in cirrhotic HCV patients. Increased IP-10 serum levels were associated with therapeutic non-response to antiviral treatment with pegylated-interferon and ribavirin. In the HCV FU cohort elevated IP-10 serum levels and increased BA were associated with higher frequencies of peripheral CD3+CXCR3+, CD4+CXCR3+ and CD8+CXCR3+ T cells. Positive correlation between serum IP-10 levels and DPPIV activity was likewise validated in patients with cholestatic liver diseases.ConclusionsA strong correlation between elevated serum levels of IP-10 and DPPIV activity was seen in different cholestatic patient groups. Furthermore, in cholestatic HCV patients a functional link to increased numbers of peripheral CXCR3+ immune cells could be observed. The source of DPPIV release in cholestatic patients remains open.

Abstract

The incidence of human papillomavirus (HPV)-associated cancers, especially those from the head and neck region, has increased. The relatively early age of presentation of HPV-positive head and neck cancer (HNC) indicates that viral infection might be acquired early in life. Persistent HPV infection has been recognized as the main risk factor for cancer development, but most studies have focused on evaluating HPV persistence in the genital region. Thus, in this work, we aimed to evaluate the prevalence of HPV in oral cavity and oropharynx in a young population, as well as the possible persistence of the infection after 12 months. Our results indicate that almost half (46.8%) of the analyzed population harbors an HPV infection either in the oral cavity or in the oropharynx. Furthermore, after 1 year of initial identification, half of them eliminated the infection, and only one person (5.26%) exhibited persistence. Interestingly, 50% of the individuals who successfully eliminated the infection acquired a new viral type, indicating that even when the primary infection is effectively eliminated by the immune system, there is a dynamic circulation of HR-HPV types that produce reinfection. This dynamic HPV infection among young individuals could influence the future establishment of cancer in some proportion of the cases. Abstract

Background The disease presentation of the severe acute respiratory syndrome coronavirus 2 infection (COVID-19) ranges from asymptomatic to fatal. COVID-19 patients with pre-existing coronary artery disease (CAD) risk factors or overt cardiovascular disease more often develop severe COVID-19, which are also related to thrombotic, inflammatory, and to viral infectivity response. We hypothesised that despite some genetic predisposition, especially in COVID-19 severity, the main determinants of fatal complications in COVID-19 patients are related to comorbidity. Purpose To determine the role of genetics and cardiovascular comorbidity in mortality from COVID-19. Methods We conducted a retrospective cohort study including 3,120 patients with positive COVID-19 test from several hospitals and primary care between February 2020 and June 2021. Among them 1,096 required hospitalization, and 121 died within 3 months after symptom onset. Standard parametric and non-parametric methods, as required, were used to compare patient characteristics by vital status. Individual genotypes for 32 CAD, 14 thrombosis, 19 inflammation, and 11 viral infectivity single nucleotide variants (SNV), as well as, 2 COVID-19 SNVs already published were tested for association with mortality with Cochran-Armitage statistics and p-values corrected for multiple comparisons. The mutually-adjusted odds ratio (OR) and 95% confidence interval (95% CI) of fatal COVID-19 was analysed for SNVs significantly associated to case-fatality, with their adverse alleles count (0, 1 or 2), and for comorbidity factors with logistic regression adjusted for age and sex. The discrimination of the models was also estimated by the area under the curve (AUC). Results Fatal and non-fatal cases' characteristics are compared in Table 1. Fatal cases had a more adverse cardiovascular and anthropometric risk profile. After correcting for multiple testing by Benjamini-Hochberg method, we observed the inflammation-related rs6993770 SNV to be significantly associated with COVID-19 fatality (p-value = 0.04). The CAD-related rs9982601 and rs2505083 SNVs, and the thrombosis-related rs7853989 SNV were moderately associated with COVID-19 fatality (p-value ≤0.1). On Figure 1 we show the adjusted OR for rs6993770 (OR: 1.02; 95% CI 1.01–1.03 per risk allele) and that for clinical factors related to COVID-19 case-fatality. The AUC of the model was 0.85 (95% CI 0.81–0.88), which not improved that of a model with clinical risk factors alone (AUC: 0.84; 95% CI 0.81–0.87). Conclusion The rs6993770 inflammation (interleukin measurement trait)-related SNV was independently associated to case fatality; however the outcome was mainly driven by age, male sex, diabetes, and glomerular filtration rate. Abstract

This is a randomized phase-2 trial aimed to compare consolidation vs. maintenance in untreated patients with follicular lymphoma (FL) responding to induction. 146 patients were enrolled from 25 Spanish institutions (ZAR2007; ClinicalTrials.gov #NCT00662948). Patients in PR or CR/CR[u] after R-CHOP were randomized 1:1 to 90Y-ibritumomab-tiuxetan 0.4 mCi/kg (arm A) vs. rituximab 375 mg/m2 every 8 weeks for 2 years (arm B). After a median follow-up of 10.55 years, 53 patients eventually progressed with a 10-year PFS of 50% vs. 56% for patients in arm A and B, respectively (HR = 1.42; p > 0.1). No significant differences were seen in OS (10-year OS 78% vs. 84.5%; HR = 1.39, p > .1). Patients receiving 90Y-ibritumomab-tiuxetan showed higher incidence of second neoplasms than those in arm B (10-year cumulative incidence 18.5 vs. 2%, respectively; p = .038). In conclusion, in FL patients responding to R-CHOP, no significant differences were found between consolidation and maintenance, although with higher late toxicity for consolidation.

Introduction: the “Assessment of Spondyloarthritis International Society Health Index” (ASASHI) questionnaire was developed to globally measure function and health status in patients with spondyloarthritis (SpA). Cut-off points have been proposed to determine different health states that were poorly evaluated in real-life patients. Objectives: to describe the health status measured by ASAS-HI in Argentine patients with axial SpA (AxSpA) and peripheral SpA (SpAp) in daily practice and to evaluate the factors associated with poor health. Materials and methods: cross-sectional, analytical and multicenter study. Patients with SpAax and SpAp were consecutively included according to ASAS criteria, from 15 Argentine centers. Statistical analysis: descriptive statistics, bivariate and multivariate analysis (multiple logistic regression) were performed to evaluate the factors associated with poor health status (ASAS-HI≥12). To analyze the construct validity of the tool, Spearman correlation was performed between the ASAS-HI and other disease evaluation parameters. Results: 274 patients with SpA were included, with a mean age of 49 (± 14) years and a median duration of the disease of 62 months (p25-75: 24-135), 155 (56.6%) were male, 129 patients (47%) with AxSpA and 145 (52.9%) SpAp. According to the ASAS-HI, 119 patients (43.4%) had good health, 117 (42.7%) had moderate health and 38 (13.9%) had poor health. In patients with SpAp, the mean ASAS-HI value was 7 (p25-75: 3-10). Abstract

Unconventional heavy oils are high viscosity and low API density which hinders process recovery from the wellbore. The injection of liquefied petroleum gas (LPG) could be an attractive alternative to improve process recovery. However, LPG dosing must be controlled in order to avoid asphaltenes precipitation. This study aimed to evaluate the feasibility of LPG injection to improve recovery of unconventional heavy oils. The study was based on the computer aided engineering using the software CMG (winprop). The physicochemical properties and composition of heavy oil and LPG were adapted from literature. Both fluids came from a Colombian oil field which had wells with average depth of 7900 ft and porosity of 17 %. The result showed that viscosity values decreased as increase in saturation pressure and temperature. It was convenient to keep the pressure under to 850 Kpa during process recovery in order to control asphaltenes precipitation. The minimum asphaltenes precipitation of 4.8 % (w/w) was obtained from heavy oil to GLP ratio of 0.64. The used of LPG showed as an attractive solvent to improve the process recovery for heavy oils. GLP injection throughout the well could be easily implemented since it is produced in situ during oil recovery.

Abstract

The 95% of ammonia destined for fertilizer production via Haber-Bosch process comes from natural gas. This process is preceded by a series of stages, in which methane available in natural gas is reformed to syngas (a gas mixture consisting of H2, CO2, CO, CH4 and H2S). The stage of syngas production represents a bottleneck from the economic point of view. The natural gas availability and its commercial cost imply a high commercial price on fertilizer which in turn increases the cost per hectare of crop. Currently, gasification technology has caught attention as a waste treatment technology due to its capacity to transform large volumes of biomass into syngas with low retention time. Therefore, this study aimed to evaluate techno-economically the bio- ammonia production from syngas obtained by gasification of African palm Rachis. The study was conducted by process simulation using Aspen Tech software. The process was simulated from the collection of the raw material to conversion of synthesis gas into ammonia. The simulation comprises a pretreatment stage to obtain an optimum particle size for biomass, a stage of gasification to convert Palm rachis into Syngas, a stage of heat recovery for steam production, a gas shift (WGS) reactor to increase the concentration of hydrogen into syngas, an acid gas remover unit (AGR) to avoid poisoning of catalysts; and the final stage of Bio ammonia synthesis; this simulation was validated based on data reported in the literature. The technical analysis measured the complexity of the new process in terms of equipment and the efficiency of the proposed operations. On the other hand, economic analysis evaluated the profitability to adapt the process to the biomass. According to results, 1,630,000 metric tonnes (Mt) of palm Rachis per year produced a syngas with a concentration hydrogen of 99% which yielded 35,755 Mt yearly of Bio ammonia. It could be inferred that highest reactor conversion was 20% at a temperature of 360°C and 250 bar pressure. It was observed that the return of investment (ROI) of 13.2% was higher than the discount rate of 12% which it demonstrates the economic viability of the project. However, The ROI value was high sensitive to the cost and availability of raw materials.

Abstract

A large part of crops is discarded due to their high production volume, and the small local market covered. Besides, commercialization of some crops is seasonal. Crops with a high content of water are perishable in a short time, so it is necessary to implement technological alternatives in order to preserve these products. One way to make better use of crops with high water content is by getting dehydrated products. The conversion of solar radiation into thermal energy for dehydration of crops is a widely known practice in agroindustry. Tropical regions receiving high solar radiation intensity are appropriate for the dehydration of crops in solar dryers. This process has proven effective for the upgrading of farm crops, and diversification of final products that may be destined for local and international market. The present work is developed as a response to the priority needs for small local farm crops that are not financially supported by government royalties, in a tropical country located in South America. The solar dehydrator described here is a modular unit to be used in non electrically interconnected zones (NIZ), and built with local materials, that makes use of convective heat to remove the moisture of previously sliced farm products. This way, this study covers the development of a simple agroindustrial process focused on the upgrading of crops, the broadening of marketing alternatives and the use of renewable energy sources. Two crops are considered as case studies for the solar dehydration: mangoes and tomatoes. Data required for the design calculations (density, moisture, solar radiation, temperature and humidity of air) were obtained experimentally, and from databases developed by governmental institutions. Microbiological analysis (total coliforms, fecal coliforms, enumeration of aerobic mesophilic microorganisms, molds and yeasts) were carried on samples before and after dehydration tests, to evaluate the adequacy of the dried products for human consumption. The results show that the solar dehydration is a feasible approach for preserving crops with a high content of water, in the region considered for this study; however, some crops require to be pre-treated before drying, due to the tendency of bacteria growth during or after the process.

Abstract

The dynamics of global Pc5 waves during the magnetic storms on 29–31 October 2003 are considered using data from the trans-American and trans-Scandinavian networks of magnetometers in the morning and post-noon magnetic local time (MLT) sectors. We study the latitudinal distribution of Pc5 wave spectral characteristics to determine how deep into the magnetosphere these Pc5 waves can extend at different flanks of the magnetosphere. The wave energy transmission mechanisms are different during 29–30 October and 31 October wave events. Further, we examine whether the self-excited Kelvin–Helmholtz instability is sufficient as an excitation mechanism for the global Pc5 waves. We suggest that on 31 October a magnetospheric magnetohydrodynamic (MHD) waveguide was excited, and the rigid regime of its excitation was triggered by enhancements of the solar wind density. The described features of Pc5 wave activity during recovery phase of strong magnetic storm are to be taken into account during the modeling of the relativistic electron energization by ultra-low-frequency (ULF) waves.

Abstract

Elevated blood pressure remains the single biggest risk factor contributing to the global burden of disease and mortality. May Measurement Month is an annual global screening campaign aiming to improve awareness of blood pressure at the individual and population level. Adults (≥18 years) recruited through opportunistic sampling were screened at sites in 92 countries during May 2019. Ideally, 3 blood pressure readings were measured for each participant, and data on lifestyle factors and comorbidities were collected. Hypertension was defined as a systolic blood pressure ≥140 mm Hg, or a diastolic blood pressure ≥90 mm Hg (mean of the second and third readings) or taking antihypertensive medication. When necessary, multiple imputation was used to estimate participants' mean blood pressure. Mixed-effects models were used to evaluate associations between blood pressure and participant characteristics. Of 1 508 130 screenees 482 273 (32.0%) had never had a blood pressure measurement before and 513 337 (34.0%) had hypertension, of whom 58.7% were aware, and 54.7% were on antihypertensive medication. Of those on medication, 57.8% were controlled to <140/90 mm Hg, and 28.9% to <130/80 mm Hg. Of all those with hypertension, 31.7% were controlled to <140/90 mm Hg, and 350 825 (23.3%) participants had untreated or inadequately treated hypertension. Of those taking antihypertensive medication, half were taking only a single drug, and 25% reported using aspirin inappropriately. This survey is the largest ever synchronized and standardized contemporary compilation of global blood pressure data. This campaign is needed as a temporary substitute for systematic blood pressure screening in many countries worldwide.
