There is limited evidence about how patients' initial preoperative psychological evaluation outcomes (require follow-up [RFU], no required follow-up [NFU], and place on hold [POH]) and current psychiatric diagnoses associate with postoperative outcomes.To test the hypotheses that patients who receive a clinical decision of RFU versus NFU from their initial psychological evaluation will be (1) more likely to experience postoperative complications, readmissions, and emergency room visits and (2) experience less weight loss over 12-months. Specific diagnoses (any psychiatric diagnosis, depression, and anxiety) are also examined for their association with weight loss over 12 months.Midwestern medical center, United States.The sample included 322 patients (81.1% female and 64.0% White) with completed psychological evaluations between August 2019 and December 2020. Patient demographics, psychological evaluation outcomes, current diagnoses, and postoperative outcomes were extracted from the health record. Bivariate analyses determined associations between NFU/RFU and postoperative complications (yes, no), readmissions (yes, no), and emergency room visits (yes, no). Mixed multilevel models were conducted with dichotomous variables NFU/RFU, any psychiatric diagnoses (yes, no), depression diagnoses (yes, no), or anxiety diagnoses (yes, no) as the main fixed within-group factors with weight loss (weight or percent total weight loss) used as the repeated measures. Insurance and surgical procedure were included as covariates.There were no significant differences in postoperative complications, readmissions, and emergency room visits between NFU and RFU groups. Patients who received a RFU versus an NFU had higher weights over 12 months (P = .001).Hypothesis 2 was only partially supported. Patients who received an RFU versus an NFU had higher weights over 12 months, but this association was not found for percent total weight loss or any of the psychiatric within-subjects variables (i.e., psychiatric diagnoses, depression, and anxiety).

Deep ocean passages are advantageous sites for long-term monitoring of deep transport and other physical properties relevant to climate. Rotating hydraulic theory provides potential for simplifying monitoring strategy by reducing the number of quantities that need to be measured. However, the applicability of these theories has been limited by idealizations such as restriction to zero or uniform potential vorticity (pv) and to channels with rectangular cross sections. Here the relationship between the flow characteristics in a canonical sea strait and its upstream condition is studied using uniform pv rotating hydraulic theory and a reduced-gravity shallow-water numerical model that allows for variation in pv. The paper is focused mainly on the sensitivity of the hydraulic solution to the strait geometry. We study the dynamics of channels with continuously varying (parabolic) cross sections to account for the rounded nature of sea-strait topographies and potentially improve monitoring strategies for realistic channel geometries. The results show that far enough from the channel entrance, the hydraulically controlled flow in the strait is insensitive to the basin circulation regardless of parabolic curvature. The controlled transport relation is derived for the case of uniform pv theory. Comparing the model to theory, we find that the measurement of the wetted edges of the interface height at the critical section can be used to estimate the volume flux. Based on this finding, we suggest three monitoring strategies for transport estimation and compare the estimates with the observed values at the Faroe Bank Channel. The results showed that the estimated transports are within the range of observed values. Significance Statement The paper investigates the relationship between the flow characteristics in an idealized sea strait and its upstream condition using rotating hydraulic theory and numerical modeling. This is significant, because the suggested monitoring strategies only require 1–3 measurements to estimate the transport at a given passage and can be used to guide observing systems.

OBJECTIVE To evaluate the effects of tirzepatide on body composition, appetite, and energy intake to address the potential mechanisms involved in body weight loss with tirzepatide. RESEARCH DESIGN AND METHODS In a secondary analysis of a randomized, double-blind, parallel-arm study, the effects of tirzepatide 15 mg (N = 45), semaglutide 1 mg (N = 44), and placebo (N = 28) on body weight and composition, appetite, and energy intake were assessed at baseline and week 28. RESULTS Tirzepatide treatment demonstrated significant reductions in body weight compared with placebo and semaglutide, resulting in greater fat mass reduction. Tirzepatide and semaglutide significantly reduced appetite versus placebo. Appetite scores and energy intake reductions did not differ between tirzepatide and semaglutide. CONCLUSIONS Differences in energy intake during ad libitum lunch were not sufficient to explain the different weight outcomes. Further evaluation is needed to assess mechanistic differences related to tirzepatide actions on 24-h energy intake, substrate utilization, and energy expenditure.

Cryptococcal meningitis, caused by the fungal pathogen

Many writers have sought to outline Wesley’s conception of how human beings obtain revelational knowledge. In this regard, the use of what has been dubbed the Wesleyan quadrilateral continues to remain widespread in both the academy and the pulpit. However, this presentation of Wesley’s thought has received severe criticism from the late William Abraham, philosopher of religion and Methodist commentator. He has proposed the creation of a new subdiscipline in epistemology for examining theology. This view has prompted a handful of attempts to extrapolate John Wesley’s epistemology of theology from his various writings. The present essay will contribute to this discussion by examining Wesley’s approach to these questions. In doing so, it explores mainly the function and place of nature in Wesley’s theological reflections. Drawing mainly upon Wesley’sA View of the Wisdom of God in Creation(1763), this essay shows that the natural world (read: creation) played an important, neglected role in Wesley’s epistemology of theology. Contribution:This article contributes to a new subdiscipline in epistemology for examining theology by presenting a new perspective on John Wesley by exploring the role of nature in his thought. Offering an epistemological–theological interpretation of John Wesley, this article fits well within the scope ofHTS Teologiese Studies/Theological Studies, particularly this subsection.

Introduction Polo-like kinase 1 (Plk1) is a Ser/Thr protein kinase that plays a role in cell cycle regulation and is required for arterial structure organization. It has also been shown to be required for RhoA activation and vasoconstriction. We aimed to investigate whether Plk1 is involved in erectile dysfunction playing a role in the contraction of the pudendal artery (PA) and corpus cavernosum (CC). Objective We aimed to test the hypothesis that agonist-mediated contraction is dependent on activation of Plk1 in the PA and CC. Methods Male wild type mice were used in this study. The PA and the CC were isolated and mounted in a wire- and strip- myograph, respectively. In the PA, contractions were obtained by concentration-response curves to phenylephrine (PE), serotonin (5-HT), thromboxane A2 analogue U46619 and angiotensin II (Ang-II). In the CC, we performed concentration-response curves to PE, 5-HT and electrical field stimulation (EFS), and relaxation was tested by EFS as well. Concentration-response curve to the same agonist was repeated after incubation with Plk1 inhibitor Volasertib (100 nM). Contractions in the presence of Volasertib were calculated as percentage of the maximal contraction evoked by its respective agonist in control conditions, which was considered 100%. Expression of Plk1 was evaluated by western blot using a pool of resistance arteries in control conditions and after incubation with Ang-II, with or without the Plk1 inhibitor Volasertib. Curves to Ang-II were also repeated in the absence of any inhibitor to rule out tachyphylaxis to the peptide in the second curve. Relaxation is given as % of contraction induced by PE. Paired Student’s t-test was used for data analysis. Data are expressed as mean ± S.E.M of 3-5 mice. Results In the PA, Volasertib treatment did not affect the contraction induced by PE, 5-HT and U46619. On the other hand, the contraction induced by Ang-II was virtually abolished by Volasertib (100 nM) reaching a maximal response of only EMAX: 3.86 ± 0.97%. In the CC, the contractions induced by PE, 5-HT and electrical field stimulation were not affected by Plk1 inhibition (95 ± 6%, 99 ± 5% and 102 ± 0.4%, respectively). On the other hand, the relaxation induced by EFS 16 Hz (73 ± 3%) was significantly facilitated by Volasertib treatment (84 ± 2%). Protein expression of total Plk1 was not affected in the pool of arteries treated with Ang-II (100 nM) and/or Volasertib (100 nM). However, Ang-II incubation increased the phosphorylation of PLK1, which was inhibited when the arteries were incubated with Volasertib. Conclusions Our findings are the first to demonstrate that Plk1 plays a role in Ang-II-induced contraction of the PA and Volasertib treatment can improve relaxation in the CC of mice. More studies are necessary to elucidate the process of Plk1 activation and whether Plk1 plays a role in the development of sexual dysfunction in pathological conditions such as diabetes. It is a privilege to be invited to contribute to the Festschrift dedicated to Professor Johan Buitendag, Emeritus Dean, Faculty of Theology and Religion at the University of Pretoria, South Africa. While his own work often examined the relationship between theology and natural science, he was also passionate about ecumenism and, in that spirit, the present essay utilised what might be described as an inductive approach to an important ecumenical question, the unity between Methodists and Catholics. Ecumenical dialogue is often undertaken from a theological and dogmatic perspective. This essay took a different approach in that it sought to inductively examine what might be described as the inner logic of two ecclesial systems (Methodist and Catholic). In doing so, it was premised upon an Aristotelian philosophical principle, epistemic fit, in that it presupposed that both these ecclesial bodies possess an intrinsic logic that can be brought to the surface in order to exhibit parallel contours in the wider ecclesial terrain. CONTRIBUTION: This article fits well within the scope of HTS Teologiese Studies/Theological Studies in that it contributed to a new subdiscipline in epistemology, which explores the conceptual and epistemological dimensions of ecclesiology. Background: The scale of COVID-19 and its well documented long-term sequelae support a need to understand long-term outcomes including frailty. Methods: This prospective cohort study recruited adults who had survived hospitalisation with clinically diagnosed COVID-19 across 35 sites in the UK (PHOSP-COVID). The burden of frailty was objectively measured using Fried's Frailty Phenotype (FFP). The primary outcome was the prevalence of each FFP group—robust (no FFP criteria), pre-frail (one or two FFP criteria) and frail (three or more FFP criteria)—at 5 months and 1 year after discharge from hospital. For inclusion in the primary analysis, participants required complete outcome data for three of the five FFP criteria. Longitudinal changes across frailty domains are reported at 5 months and 1 year post-hospitalisation, along with risk factors for frailty status. Patient-perceived recovery and health-related quality of life (HRQoL) were retrospectively rated for pre-COVID-19 and prospectively rated at the 5 month and 1 year visits. This study is registered with ISRCTN, number ISRCTN10980107. Findings: Between March 5, 2020, and March 31, 2021, 2419 participants were enrolled with FFP data. Mean age was 57.9 (SD 12.6) years, 933 (38.6%) were female, and 429 (17.7%) had received invasive mechanical ventilation. 1785 had measures at both timepoints, of which 240 (13.4%), 1138 (63.8%) and 407 (22.8%) were frail, pre-frail and robust, respectively, at 5 months compared with 123 (6.9%), 1046 (58.6%) and 616 (34.5%) at 1 year. Factors associated with pre-frailty or frailty were invasive mechanical ventilation, older age, female sex, and greater social deprivation. Frail participants had a larger reduction in HRQoL compared with before their COVID-19 illness and were less likely to describe themselves as recovered. Interpretation: Physical frailty and pre-frailty are common following hospitalisation with COVID-19. Improvement in frailty was seen between 5 and 12 months although two-thirds of the population remained pre-frail or frail. This suggests comprehensive assessment and interventions targeting pre-frailty and frailty beyond the initial illness are required. Copyright © 2023 The Author(s). Background: Sleep disturbance is common following hospital admission both for COVID-19 and other causes. The clinical associations of this for recovery after hospital admission are poorly understood despite sleep disturbance contributing to morbidity in other scenarios. We aimed to investigate the prevalence and nature of sleep disturbance after discharge following hospital admission for COVID-19 and to assess whether this was associated with dyspnoea. Methods: CircCOVID was a prospective multicentre cohort substudy designed to investigate the effects of circadian disruption and sleep disturbance on recovery after COVID-19 in a cohort of participants aged 18 years or older, admitted to hospital for COVID-19 in the UK, and discharged between March, 2020, and October, 2021. Participants were recruited from the Post-hospitalisation COVID-19 study (PHOSP-COVID). Follow-up data were collected at two timepoints: an early time point 2–7 months after hospital discharge and a later time point 10–14 months after hospital discharge. Sleep quality was assessed subjectively using the Pittsburgh Sleep Quality Index questionnaire and a numerical rating scale. Sleep quality was also assessed with an accelerometer worn on the wrist (actigraphy) for 14 days. Participants were also clinically phenotyped, including assessment of symptoms (ie, anxiety [Generalised Anxiety Disorder 7-item scale questionnaire], muscle function [SARC-F questionnaire], dyspnoea [Dyspnoea-12 questionnaire] and measurement of lung function), at the early timepoint after discharge. Actigraphy results were also compared to a matched UK Biobank cohort (non-hospitalised individuals and recently hospitalised individuals). Multivariable linear regression was used to define associations of sleep disturbance with the primary outcome of breathlessness and the other clinical symptoms. PHOSP-COVID is registered on the ISRCTN Registry (ISRCTN10980107). Findings: 2320 of 2468 participants in the PHOSP-COVID study attended an early timepoint research visit a median of 5 months (IQR 4–6) following discharge from 83 hospitals in the UK. Data for sleep quality were assessed by subjective measures (the Pittsburgh Sleep Quality Index questionnaire and the numerical rating scale) for 638 participants at the early time point. Sleep quality was also assessed using device-based measures (actigraphy) a median of 7 months (IQR 5–8 months) after discharge from hospital for 729 participants. After discharge from hospital, the majority (396 [62%] of 638) of participants who had been admitted to hospital for COVID-19 reported poor sleep quality in response to the Pittsburgh Sleep Quality Index questionnaire. A comparable proportion (338 [53%] of 638) of participants felt their sleep quality had deteriorated following discharge after COVID-19 admission, as assessed by the numerical rating scale. Device-based measurements were compared to an age-matched, sex-matched, BMI-matched, and time from discharge-matched UK Biobank cohort who had recently been admitted to hospital. Compared to the recently hospitalised matched UK Biobank cohort, participants in our study slept on average 65 min (95% CI 59 to 71) longer, had a lower sleep regularity index (–19%; 95% CI –20 to –16), and a lower sleep efficiency (3·83 percentage points; 95% CI 3·40 to 4·26). Similar results were obtained when comparisons were made with the non-hospitalised UK Biobank cohort. Overall sleep quality (unadjusted effect estimate 3·94; 95% CI 2·78 to 5·10), deterioration in sleep quality following hospital admission (3·00; 1·82 to 4·28), and sleep regularity (4·38; 2·10 to 6·65) were associated with higher dyspnoea scores. Poor sleep quality, deterioration in sleep quality, and sleep regularity were also associated with impaired lung function, as assessed by forced vital capacity. Depending on the sleep metric, anxiety mediated 18–39% of the effect of sleep disturbance on dyspnoea, while muscle weakness mediated 27–41% of this effect. Interpretation: Sleep disturbance following hospital admission for COVID-19 is associated with dyspnoea, anxiety, and muscle weakness. Due to the association with multiple symptoms, targeting sleep disturbance might be beneficial in treating the post-COVID-19 condition. Stellar convection is a non-local process responsible for the transport of heat and chemical species. It can lead to enhanced mixing through convective overshooting and excitation of internal gravity waves (IGWs) at convective boundaries. The relationship between these processes is still not well understood and requires global hydrodynamic simulations to capture the important large-scale dynamics. The steep stratification in stellar interiors suggests that the radial extent of such simulations can affect the convection dynamics, the IGWs in the stably stratified radiative zone, and the depth of the overshooting layer. We investigate these effects using two-dimensional global simulations performed with the fully compressible stellar hydrodynamics code MUSIC. We compare eight different radial truncations of the same solar-like stellar model evolved over approximately 400 convective turnover times. We find that the location of the inner boundary has an insignificant effect on the convection dynamics, the convective overshooting and the travelling IGWs. We relate this to the background conditions at the lower convective boundary which are unaffected by the truncation, as long as a significantly deep radiative layer is included in the simulation domain. However, we find that extending the outer boundary by only a few percent of the stellar radius significantly increases the velocity and temperature perturbations in the convection zone, the overshooting depth, the power and the spectral slope of the IGWs. Impaired family functioning has been associated with obesity in children and adolescents, but few longitudinal studies exist. We examined whether family functioning from early to mid-childhood is associated with overweight and obesity in later childhood and adolescence.We examined data from the Quebec Longitudinal Study of Child Development (QLSCD), a birth cohort (N = 2120), collected between 1998 and 2011. Parent-reported family functioning was assessed at 4 time points between ages 0.5 and 8 years using the McMaster Family Assessment Device with established cut-offs for impaired family functioning. Participants were classified as having experienced: 1) early-childhood impaired functioning, 2) mid-childhood impaired functioning, 3) both early and mid-childhood impaired functioning, or 4) always healthy family functioning. Overweight and obesity were determined at 10- and 13-years using WHO criteria. Covariate adjusted multinomial logistic regressions were fitted to the data to examine associations between longitudinal family functioning groups (using the always healthy functioning as reference category) and the likelihood of having overweight and obesity (vs normal weight) at ages 10 (n = 1251) and 13 years (n = 1226).In the 10- and 13-year sub-samples, respectively 10.2% and 12.5% of participants had experienced both early and mid-childhood impaired family functioning. Participants in this group had an increased likelihood of having obesity (vs normal weight) at age 10 years [OR = 2.63 (95% CI: 1.36; 5.08)] and at age 13 years [OR = 1.94 (95% CI: 0.99; 3.80] compared to those in the always healthy functioning group. No associations were found for other family functioning categories or for overweight status.Approximately one in ten children experienced impaired family functioning throughout early and mid-childhood. Findings suggest a link between impaired functioning across childhood and the development of obesity at 10 years of age and possibly at 13 years of age.

The weighted ensemble (WE) family of methods is one of several statistical-mechanics based path sampling strategies that can provide estimates of key observables (rate constants, pathways) using a fraction of the time required by direct simulation methods such as molecular dynamics or discrete-state stochastic algorithms. WE methods oversee numerous parallel trajectories using intermittent overhead operations at fixed time intervals, enabling facile interoperability with any dynamics engine. Here, we report on major upgrades to the WESTPA software package, an open-source, high-performance framework that implements both basic and recently developed WE methods. These upgrades offer substantial improvements over traditional WE. Key features of the new WESTPA 2.0 software enhance efficiency and ease of use: an adaptive binning scheme for more efficient surmounting of large free energy barriers, streamlined handling of large simulation datasets, exponentially improved analysis of kinetics, and developer-friendly tools for creating new WE methods, including a Python API and resampler module for implementing both binned and “binless” WE strategies.Table of Contents/Abstract ImageFor the manuscript “WESTPA 2.0: High-performance upgrades for weighted ensemble simulations and analysis of longer-timescale applications” by Russo et al.

The energy and momentum balance of an abyssal overflow across a major sill in the Samoan Passage is estimated from two highly resolved towed sections, set 16 months apart, and results from a two-dimensional numerical simulation. Driven by the density anomaly across the sill, the flow is relatively steady. The system gains energy from divergence of horizontal pressure work O(5) kWm−1 and flux of available potential energy O(2) kWm−1. Approximately half of these gains are transferred into kinetic energy while the other half is lost to turbulent dissipation, bottom drag, and divergence in vertical pressure work. Small-scale internal waves emanating downstream of the sill within the overflow layer radiate O(1) kWm−1 upwards but dissipate most of their energy within the dense overflow layer and at its upper interface. The strongly sheared and highly stratified upper interface acts as a critical layer inhibiting any appreciable upward radiation of energy via topographically generated lee waves. Form drag of O(2) Nm−2, estimated from the pressure drop across the sill, is consistent with energy lost to dissipation and internal wave fluxes. The topographic drag removes momentum from the mean flow, slowing it down and feeding a counter current aloft. The processes discussed in this study combine to convert about one third of the energy released from the cross-sill density difference into turbulent mixing within the overflow and at its upper interface. The observed and modeled vertical momentum flux divergence sustains gradients in shear and stratification, thereby maintaining an efficient route for abyssal water mass transformation downstream of this Samoan Passage sill.

The anaerobic gut fungi (AGF) represent a coherent phylogenetic clade within the Mycota. Twenty genera have been described so far. Currently, the phylogenetic and evolutionary relationships between AGF genera remain poorly understood. Here, we utilized 52 transcriptomic datasets from 14 genera to resolve AGF inter-genus relationships using phylogenomics, and to provide a quantitative estimate (amino acid identity, AAI) for intermediate rank assignments. We identify four distinct supra-genus clades, encompassing all genera producing polyflagellated zoospores, bulbous rhizoids, the broadly circumscribed genus Piromyces, and the Anaeromyces and affiliated genera. We also identify the genus Khoyollomyces as the earliest evolving AGF genus. Concordance between phylogenomic outputs and RPB1 and D1/D2 LSU, but not RPB2, MCM7, EF1α or ITS1, phylogenies was observed. We combine phylogenomic analysis and AAI outputs with informative phenotypic traits to propose accommodating 14/20 AGF genera into four families: Caecomycetaceae fam. nov. (encompassing the genera Caecomyces and Cyllamyces), Piromycetaceae fam. nov. (encompassing the genus Piromyces), emend the description of the family Neocallimastigaceae to encompass the genera Neocallimastix, Orpinomyces, Pecoramyces, Feramyces, Ghazallomyces, Aestipascuomyces and Paucimyces, as well as the family Anaeromycetaceae to include the genera Oontomyces, Liebetanzomyces and Capellomyces in addition to Anaeromyces. We refrain from proposing families for the deeply branching genus Khoyollomyces and for genera with uncertain position (Buwchfawromyces, Joblinomyces, Tahromyces, Agriosomyces and Aklioshbomyces) pending availability of additional isolates and sequence data; and these genera are designated as ‘genera incertae sedis’ in the order Neocallimastigales. Our results establish an evolutionary-grounded Linnaean taxonomic framework for the AGF, provide quantitative estimates for rank assignments, and demonstrate the utility of RPB1 as an additional informative marker in Neocallimastigomycota taxonomy.
