Abstract

Abstract Circular electron positron colliders, such as the CEPC and FCC-ee, have been proposed to measure Higgs boson properties precisely, test the Standard Model, search for physics beyond the Standard Model, and so on. One of the important goals of these colliders is to measure the W boson mass with great precision by taking data around the W-pair production threshold. In this paper, the data-taking scheme is investigated to maximize the achievable precisions of the W boson mass and width with a threshold scan, when various systematic uncertainties are taken into account. The study shows that an optimal and realistic data-taking scheme is to collect data at three center-of-mass energies and that precisions of 1.0 MeV and 3.4 MeV can be achieved for the mass and width of the W boson, respectively, with a total integrated luminosity of $$\mathscr {L}=3.2$$ L=3.2 ab$$^{-1}$$ -1 and several assumptions of the systematic uncertainty sources.

Abstract

A 56-year-old female was admitted to Department of Gastroenterology at Peking Union Medical College Hospital with diarrhea for seven months, and abnormal liver function for six months. She had a history of type 1 diabetes. The main clinical manifestations were recurrent fatty diarrhea and abnormal liver function, accompanied by abdominal and retroperitoneal lymphadenopathy, elevated CA19-9 and CEA. Progressive impairment of hepatic synthetic function and shrinkage of liver developed in a short period of time. The pathology of liver biopsy suggested that nodular regeneration of hepatocytes was followed by hyperplasia of thin bile ducts after submassive necrosis. Intestinal mucosa biopsies were performed twice. The pathology showed that the intestinal villi were completely blunt, accompanied with crypt hyperplasia. Goblet cells disappeared with reduced mucin. Paneth cells were barely seen without intraepithelial infiltration of lymphocytes. Rifaximin was not effective, while glucocorticoids improved clinical situation. The diagnosis of autoimmune enteropathy was finally confirmed by multidisciplinary team including departments of gastroenterology, pathology, endocrinology, hematology, infectious diseases, and rheumatology. With the administration of glucocorticoid and sirolimus, diarrhea relieved and liver function returned to normal.患者女性，56岁，既往1型糖尿病病史，本次因腹泻7个月，发现肝功能异常6个月收入北京协和医院消化内科。患者临床主要表现为反复发作的慢性腹泻和肝功能异常，同时伴有腹腔和腹膜后淋巴结肿大、CA19-9及癌胚抗原（CEA）升高。慢性腹泻，粪便苏丹Ⅲ染色阳性，病程中消耗症状重，整体符合脂肪泻特点。之后患者逐渐出现以肝脏合成指标异常为主的肝功能损害表现，肝脏影像提示在较短的时间内出现明显肝脏缩小，但肝脏形态改变与门脉高压不平行。经肝脏活检病理提示亚大块坏死后肝细胞结节状再生，伴细胆管增生。经两次小肠黏膜活检，病理显示小肠绒毛几乎完全萎缩伴隐窝增生、杯状细胞消失和黏蛋白减少，潘氏细胞几乎全部消失，未见上皮内淋巴细胞浸润。病程中利福昔明治疗未见改善，糖皮质激素治疗后症状缓解。经消化科、病理科、内分泌科、血液科、感染内科、风湿免疫科等多科协作，最终明确自身免疫肠病的诊断。患者经过糖皮质激素联合西罗莫司治疗及补充消化酶、肠道菌群调节等支持治疗后，目前腹泻缓解、肝功能恢复正常。.

Abstract

Long non-coding RNAs (lncRNAs) are highly involved in diverse biological processes of human malignancies. The expression profile and underlying mechanism of lncRNA growth arrest specific transcript 5 (GAS5) in colorectal cancer (CRC) is poorly understood. In this study, we found that GAS5 was commonly downregulated in CRC tissues, serum of CRC patients and CRC cell lines. Knockdown of GAS5 promoted CRC cell proliferation and colony formation, whereas overexpression of GAS5 produced the opposite result. We further demonstrated that knockdown of GAS5 increased the expression and secretion of interleukin-10 (IL-10) and vascular endothelial growth factor (VEGF-A) via NF-κB and Erk1/2 pathways. Neutralization of IL-10 and VEGF-A reduced tumour proli feration caused by GAS5 knockdown. Moreover, GAS5 expression showed a statistically significant correlation with the mRNA levels of IL-10 and VEGF-A in CRC tissues. We further illustrated that GAS5 was markedly downregulated and negatively correlated with the cytokine expression in a mouse model of colitis-associated cancer (CAC). These results delineate a novel mechanism of lncRNA GAS5 in suppressing colorectal carcinogenesis. The cytokines IL-10 and VEGF-A inhibited by GAS5 may provide targets for lncRNA-based therapies for CRC.

Abstract

Background Differentiating between ulcerative colitis (UC), Crohn’s disease (CD) and intestinal tuberculosis (ITB) is challenging under endoscopy. We aimed to realise automatic differential diagnosis among these diseases through machine learning algorithms. Methods A total of 6399 consecutive patients (5128 UC, 875 CD and 396 ITB) who had taken colonoscopy examinations in Peking Union Medical College Hospital from January 2008 to November 2018 was enrolled. The input was the description of the endoscopic image in the form of free-text. Word segmentation and key word infiltration were conducted as data pre-processing. Random forest (RF) and convolutional neural network (CNN) were applied to different disease entities. Three two-class classifiers (UC and CD, UC and ITB, CD and ITB) and a three-class classifier (UC, CD and ITB) were built. Sensitivity/specificity and precision/recall were applied to evaluate the performance of two-class classifiers and the three-class classifier, respectively. Results The classifiers built in this research were well-performed and the CNN had a better performance in general. The RF sensitivities/specificities of UC-CD, UC-ITB and CD-ITB were 0.89/0.84, 0.83/0.82 and 0.72/0.77, while the CNN of CD-ITB was 0.90/0.77. The precision/recall of UC-CD-ITB was 0.97/0.97, 0.65/0.53 and 0.68/0.76 by RF, respectively, and 0.99/0.97,0.87/0.83 and 0.52/0.81 by CNN, respectively. Conclusion Classifiers built by RF and CNN had an excellent performance when classifying UC with CD or ITB. For the differentiation of CD and ITB, high specificity and sensitivity were reached as well. Artificial intelligence through machine learning is very promising in helping unexperienced endoscopists differentiate inflammatory intestinal diseases.

Abstract

Background Inflammatory bowel disease (IBD) patients may experience a suboptimal response to anti-tumour necrosis factor (TNF) therapy. In China, these data are limited. The EXPLORE study aimed to assess the incidence and indicators of suboptimal response to first-line anti-TNF agents in IBD patients in real-world practice in the newly industrialised countries. Methods The EXPLORE study was a multinational, retrospective chart review study. In the China subgroup, adult patients from 10 centres diagnosed with ulcerative colitis (UC) or Crohn’s disease (CD) and who initiated first anti-TNF treatment between March 2010 and March 2015, were included. The cumulative incidence (CI) of suboptimal response was assessed over 24 months since the anti-TNF initiation. The indicators of suboptimal response included: IBD-related hospitalisation, dose-escalation, discontinuation including switch to another anti-TNF, non-biologic therapy augmentation, or IBD-related surgery. Primary non-response (PNR) and secondary loss of response (SLOR) were defined as any suboptimal response indicator at Results Overall, 287 first-line anti-TNF treated patients (35 UC; 252 CD) were included: male, UC 54.3% (n = 19), CD 74.6% (n = 188); mean (SD) age (years), UC 43.1 (14.2), CD 31.9 (11.3); median (min-max) disease duration (years), UC 1.0 (0–9), CD Conclusion Approximately three-quarters of UC and over half of CD patients experienced a suboptimal response to their first anti-TNF agent at 24 months. Given the high unmet needs observed with anti-TNF therapies in China, IBD patients may benefit from greater optimisation of care and new biologic options.

Abstract

Fever and abdominal pain are common symptoms and could be main manifestations in patients with autoinflammatory diseases. A 48-year-old female patient was admitted with recurrent fever and abdominal pain for 9 years. Serum level of inflammatory markers synchronously fluctuated with fever, and returned to normal when fever subsided. The periodic episodes of fever occurred every 1 to 4 months and failed to respond to empirical antibiotics. Whole exome sequencing showed heterozygous mutation of NOD2 gene q902k, leading to the final diagnosis of autoinflammatory disease. Corticosteroid and tripterygiumglycosides were effective for the disease remission.发热、腹痛为众多胃肠道受累疾病的常见表现，新近认识的自身炎症性疾病为周期性发热患者的鉴别诊断之一。本例患者48岁女性，因反复发热伴左上腹痛9年，加重3个月入院。发热时伴血炎症指标升高，发热间期炎症指标恢复正常。每1~4个月发作1次，应用抗菌药物治疗无效。最终全外显子测序回报NOD2基因q902k杂合突变，诊断得以明确，考虑自身炎症性疾病，经糖皮质激素及雷公藤治疗后患者病情缓解。.

Abstract

A 60-year-old man presented with severe watery diarrhea for 2 months complicated with weight loss and acute kidney injury. He did not respond well to antidiarrheal medicines, empirical antibiotics and dietary exclusion of gluten or even complete bowel rest. The final diagnosis of autoimmune enteropathy (AIE) was made based on histopathologic findings of endoscopic biopsy from duodenal mucosa after excluding neoplastic disease, inflammatory bowel disease, and infectious diarrhea, etc. Chronic diarrhea and oliguria alleviated after the administration of corticosteroids.患者男，60岁。因大量水样泻2个月余就诊，伴体重明显下降，并发急性肾损伤，经多种止泻药物、经验性抗感染及禁食禁水试验、去麦胶饮食等治疗均无明确疗效。在除外肿瘤性疾病、炎症性肠病及感染性腹泻等病因后，结合胃镜病理改变诊断为自身免疫性肠病，予糖皮质激素治疗后病情好转。.

A 32 year-old woman in the third trimester of pregnancy was admitted for severe acute pancreatitis due to hypertriglyceridemia. During hospitalization she developed multiorgan dysfunction, infected pancreatic necrosis, abdominal compartment syndrome and intrauterine fetal death. She was successfully treated by multidisciplinary team including department of emergency medicine, ICU, gastroenterology, obstetrics, endocrinology, ultrasonography, radiology, infectious disease, nutrition and surgery.一例32岁妊娠晚期患者因高甘油三酯血症诱发重症急性胰腺炎，造成多器官功能衰竭、胰腺坏死感染、腹腔间隔室综合征和胎死宫内。经包括急诊、重症医学、消化、产科、内分泌、超声、放射、感染、营养和外科共计10个专科的多学科协作，患者最终好转。.

Abstract

This meta-analysis assessed the value of interferon-γ release assays (IGRAs) in the differential diagnosis of intestinal tuberculosis (ITB) from Crohn's disease (CD).Systematic search without language restriction was conducted in the main computerized databases until June 2015. Studies that have evaluated the performance of IGRAs (QuantiFERON-TB Gold or T-SPOT.TB) in distinguishing ITB from CD were eligible. Main outcome measures included sensitivity and specificity. Area under the curve (AUC) of the summary receiver operating characteristic (sROC) curve was used to evaluate the accuracy of IGRAs.Twelve studies (all from Asia) were finally included. The pooled sensitivity and specificity of IGRAs for the differential diagnosis of ITB from CD were 82.8% (95%CI 78.4%-86.6%) and 86.7% (95%CI 83.2%-89.6%) respectively. The positive likelihood ratio (PLR) and negative likelihood ratio (NLR) were 6.870 (95%CI 5.345-8.830) and 0.171 (95%CI 0.105-0.279). The diagnostic odds ratio was 44.030 (95%CI 27.964-69.325). And the AUC of sROC was 0.939.IGRAs have a high sensitivity and specificity for the diagnosis of ITB, and specificity is consistent from study to study. IGRAs may be considered as a supplementary method in the differential diagnosis of ITB from CD.

Abstract

To analyze the discrepancy and similarities of clinical characteristics in elderly patients with ulcerative colitis (UC) and ischemic colitis (IC).A total of 43 elderly patients (age≥60 yrs) with UC and 36 elderly patients with IC were enrolled from 2004 to 2015 at Peking Union Medical College Hospital. The clinical characteristics were retrospectively analyzed and compared between the two groups.Compared with IC group, the disease course was longer with lower incidence of cardiovascular comorbidities in UC patients (P0.05). In UC group, more patients presented with diarrhea, mucopurulent bloody stool [39(90.7%) vs 16(44.4%) and 34(79.1%) vs 2(5.6%) respectively, both P0.01]. Yet bloody stool as the only symptom was seen in more IC patients than UC patients [61.1%(22/36) vs 7.0%(3/43), P0.01]. The ratio of extra-intestinal manifestations was higher [18.6%(8/43) vs 0(0/36)] in UC patients, while complications were lower [11.6%(5/43) vs 30.6%(11/36), P0.05]. As to the laboratory parameters, median platelet count [(294.38±104.83)×10(9)/L vs (235.47±94.82)×10(9)/L, P0.05] was higher in UC group. In addition, more patients with UC had positive perinuclear antineutrophil cytoplasmic antibody (p-ANCA) [50.0%(15/30) vs 10.0%(2/20), P0.05]. The most commonly involved regions of IC were descending colon and sigmoid colon, in which the lesions were clearly demarcated with the normal mucosa. Lesions in patients with UC mainly originated from rectum and might spread to the whole colon. Vascular occlusion and micro thrombosis were characteristic pathological findings of IC. The crypt abscesses were frequently seen in the UC group.Even though UC and IC have some similar manifestations in the elderly patients, clinical and pathological discrepancy is still helpful to differentiate each other.

Abstract

Intestinal tuberculosis (ITB) and Crohn's disease (CD) have overlapping clinical, radiographic, endoscopic and histologic features, which makes the distinction between these two disease entities a great challenge in tuberculosis-endemic countries. The aim of the study was to investigate the value of in vitro interferonγ release assay (T-SPOT.TB) in differentiating ITB from CD.From June 2008 to February 2010, a total of 93 consecutive patients with undetermined ITB or CD were prospectively recruited. Clinical, endoscopic, histologic and therapeutic responses were longitudinally monitored at follow-up evaluation until the final definite diagnosis has been reached.After a median of 6 months' follow-up (interquartile range [IQR], 3.0 to 7.5 months), definitive diagnosis was achieved in 84 of the 93 patients (90%), with 19 having ITB and 65 having CD. On univariate analysis, a long duration of illness, chronic diarrhea, and anemia were significantly more common in CD (P0.05). While night sweat, ascites, pulmonary lesions, circumferential ulcer on endoscopy, ileo-cecal valve involvement and epithelioid granulomas were significantly more common in ITB (P0.05). On multivariate analysis, T-SPOT.TB (Hazard ratio 7.0, 95% confidence interval [CI] 1.9-25.7) was found to be a good predictor for ITB diagnosis. The sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) of T-SPOT.TB were 84.2%, 75.4%, 50.0%, and 94.2%, respectively.When differentiating ITB and CD in tuberculosis-endemic regions, T-SPOT.TB blood test may be a helpful and practical diagnostic tool for its high NPV to rule out ITB.

Abstract

Chromatin remodeling has been newly established as an important cancer genome characterization and recent exome and whole-genome sequencing studies of hepatocellular carcinoma (HCC) showed that recurrent inactivating mutations in SWI/SNF subunits involved in the molecular basis of hepatocarcinogenesis. To test the hypothesis that genetic variants in the key subunits of SWI/SNF complexes may contribute to HCC susceptibility, we systematically assessed associations of genetic variants in SWI/SNF complexes with HCC risk using a two-staged case-control study in Chinese population. A set of 24 single nucleotide polymorphisms (SNPs) in SWI/SNF complexes were examined in stage 1 with 502 HCC patients and 487 controls and three promising SNPs (SMARCA4 rs11879293, rs2072382 and SMARCB1 rs2267032) were further genotyped in stage 2 comprising 501 cases and 545 controls for validation. SMARCA4 rs11879293 presented consistently significant associations with the risk of HCC at both stages, with an OR of 0.73 (95% CI: 0.62–0.87) using additive model in combined analysis. Moreover, the decreased risk of HCC associated with SMARCA4 rs11879293 AG/AA was more evident among HBsAg positive individuals (OR = 0.47, 95% CI: 0.27–0.80) in combined analysis. The study highlighted the potential role of the SWI/SNF complexes in conferring susceptibility to HCC, especially modified HCC risk by HBV infection.

Abstract

We have measured the time integrated B0 B 0 mixing parameter and the forward-backward asymmetry in the process e+e− → b b using hadronic events containing muons or electrons. The data sample corresponds to 1,044,000 hadronic decays of the Z. From a fit to the momentum and transverse momentum distributions for single lepton and dilepton events, we have determined the B0 B 0 mixing parameter to be χB = 0.123 ± 0.012 (stat.) ± 0.008 (sys.), and the b b forward-backward asymmetry at the effective center-of-mass energy √s = 91.30 GeV to be A b b = 0.087 ± 0.011 ( stat. ) ± 0.004 ( sys. ) This measurement corresponds to a value of the effective electroweak mixing angle of sin 2 θ w = 0.2335 ± 0.0021 .

Abstract

Using the L3 detector, the branching ratio BR(b --> taunuX) has been measured using a sample of Z --> bbBAR events tagged by high momentum and high transverse momentum leptons in one hemisphere and with missing energy in the opposite hemisphere. From a sample of 948000 hadronic events we find BR(b --> taunuX) = (2.4 +/- 0.7 (stat.) +/- 0.8 (syst.))%.

Abstract

We have measured the time integrated B0-BBAR0 mixing parameter and the forward-backward asymmetry in the process e+e- --> bbBAR using hadronic events containing muons or electrons. The data sample corresponds to 1,044,000 hadronic decays of the Z. From a fit to the momentum and transverse momentum distributions for single lepton and dilepton events, we have determined the B0-BBAR0 mixing parameter to be chi(B) = 0.123 +/- 0.012 (stat.) +/- 0.008 (sys.), and the bbBAR forward-backward asymmetry at the effective center-of-mass energy square-root s = 91.30 GeV to be A(bbBAR) = 0.087 +/- 0.011 (stat.) +/- 0.004 (sys.) This measurement corresponds to a value of the effective electroweak mixing angle of sin2thetaBAR(w) = 0.2335 +/- 0.0021.

Abstract

To clarify the relationship between TT virus (TTV) infection and the development of hepatocellular carcinoma (HCC).TTV was examined in sera of 112 patients with HCC and 95 patients with chronic liver disease (CLD) without HCC. 30 patients without liver disease were examined as control. TTV DNA was detected by the second generation of TTV PCR primers (NG059, NG061 and NG063) after extraction of DNA from 50 microliters serum. The relationship between TTV infection and clinical features of each group patients was analyzed.TTV DNA was detected in 17.86% (20/112) of HCC patients, and this prevalence in HCC patients was not significantly higher than that of TTV DNA in CLD patients (15.78%, 15/95). There were no significant differences in age, gender, tumor factors (size, TNM classification and markers) and other hepatitis virus markers between TTV-positive and TTV-negative in HCC and CLD patients, except history of blood transfusion [(80.00% vs 35.87% (P0.05) in HCC patients and 73.33% vs 27.50% (P0.05) in CLD patients)] and ALT [(93 +/- 27) IU/L vs (68 +/- 31) IU/L (P0.05) in HCC patients]. It seemed that TTV did not do any harm to the liver function of patients who infected with HBV or HCV, except for elevating ALT level in such a kind of HCC patients.Although TTV could do harm to the liver function of HCC patients, it does not contribute to the development of HCC from CLD.

Abstract

Background Repeated qualitative fecal immunochemical test (qlFIT) is a clinical strategy widely used to detect lower gastrointestinal lesions, but its diagnostic power has not been assessed in opportunistic screening for colorectal neoplasia. Objective This study aimed to determine the performance of three-sample qlFIT in screening for colorectal cancer and its precursors in high-risk participants. Methods 513 gastrointestinal outpatients yielded three qlFITs before a standard colonoscopy. We evaluated the diagnostic value of one, two, and three positive qlFITs serving as the positivity threshold. The risk factors of colorectal neoplasia to yield positive qlFITs were also determined. Results 52 patients were diagnosed with colorectal cancer and 70 with advanced adenomatous polyp. For colorectal cancer, the sensitivity and specificity of one positive qlFIT were 90.4% and 53.8%, of two were 80.8% and 75.1%, and of three were 53.9% and 88.5%, respectively. For advanced adenomatous polyp, the sensitivity and specificity of one positive qlFIT were 81.4% and 54.2%, of two were 50.0% and 72.5%, and of three were 28.6% and 86.2%. Left-sided location (OR 2.50, 95%CI 1.26–4.95) and advanced histology of tumors (OR 3.08, 95%CI 1.58–6.01) were independently associated with positive qlFITs. Conclusions Three-sample qlFIT is a reasonably good method to detect colorectal neoplasia in high-risk population. Tumors in the left side or with advanced pathological features are more likely to produce positive qlFITs.

Abstract

The human forkhead box A1 (FOXA1) and A2 (FOXA2) transcription factors have been found to control estrogen and androgen signaling through co-regulating target genes with sex hormone receptors. Here we used an integrative strategy to examine the hypothesis that genetic variants at FOXA1/2 binding elements may be associated with sexual dimorphism of hepatocellular carcinoma (HCC) risk. Firstly we extracted chromatin immunoprecipitation-sequencing (ChIP-seq) data of FOXA1, FOXA2 and estrogen receptor 1(ERα) from ENCODE database to obtain dual target regions of FOXA/ERα, and further intersected these regions with genes' promoters. Then we used MATCH program to predict FOXA binding elements, in which genetic variants were retrieved by dbSNP database (NCBI, build 134). A total of 15 candidate variants were identified in this stage. Secondly we performed a case-control study with 1,081 HCC patients and 2,008 matched controls and found a significant association of SERPINA6-rs1998056 with female HCC risk under common genetic models (e.g. GG versus CC: OR = 2.03, 95% CI = 1.26-3.27, P = 0.004). Moreover, results from our real-time quantitative polymerase chain reaction (qPCR) using 72 normal liver tissues adjacent to the tumors showed that SERPINA6 expression was significantly different among different genotypes of this variant (GG versus CC: P = 0.032; Group test: P = 0.060). In summary, our study suggested that SERPINA6-rs1998056 regulated by FOXA/ERα might be associated with female HCC risk.

Abstract

The production of B* mesons in Z decays has been measured at LEP with the L3 detector. A sample of Z --> b ($) over bar b events was obtained by tagging muons in 1.6 million hadronic Z decays collected in 1991, 1992 and 1993. A signal with a peak value of E(y) = 46.3+/-1.9 (stat) MeV in the B rest frame energy spectrum was interpreted to come from the decay B* --> gamma B. The inclusive production ratio of B* mesons relative to B mesons was determined from a fit to the spectrum to be N-B*/(N-B* + N-B) = 0.76+/-0.08+/-0.06, where the first error is statistical and the second is systematic.

Abstract

With a data sample of 86 000 Z → τ+τ−(γ) events collected in 1990 through 1993 we have measured the polarization of τ-leptons as a function of the production polar angle using the following 1-prong τ decay modes: r- ~ e-~evT, r- --+/.t-Vuv~, r- ---+ 7r-(K-)v~, r- ~ p-v, and r- ---+ a~'v, We obtain for the ratio of vector to axial-vector weak neutral couplings for electrons gVe/gAe=0.0791±0.0099(stat)±0.0025 (syst) and taus gVτ/gAτ=0.0752±0.0063 (stat)±0.0045 (syst) consistent with the hypothesis of e - τ universality. Assuming universality of the e - τ neutral current we determine the effective electroweak mixing angle to be sin2 θeffw = 0.2309 ± 0.0016.., Facultad de Ciencias Exactas

Abstract

We present a study of the inclusive production of 7r °, r I, K ° and A based on 929,000 hadronic Z decays recorded with the L3 detector at LEP. The measured inclusive momentum distributions have been compared with predictions from parton shower models as well as an analytical Quantum Chromodynamics calculation. Comparing to low energy e+e - data, we find that QCD describes the energy evolution of the hadron spectrum., Facultad de Ciencias Exactas

Abstract

A study of twelve distinct decay channels of the eta(c) has been performed with the L3 detector at LEP, for an integrated luminosity of 30 pb-1. Summing all channels, 28 candidate events have been identified, with an estimated background of 11 events. The two-photon radiative width is evaluated to be GAMMA(gammagamma) (eta(c)) = 8.0 +/- 2.3 +/- 2.4 keV.
