1. A60 DOWN WITH THE SHP-1: CHRONIC HELMINTH INFECTION DISRUPTS BILE ACID HOMEOSTASIS AND SIGNALLING RECEPTOR ACTIVATION IN THE MURINE SMALL INTESTINE TO POTENTIALLY IMPACT GUT IMMUNITY

Abstract

Background Helminths (parasitic worms) use immunomodulatory mechanisms to maintain their presence in the host intestines. Understanding how helminths reduce inflammation in the guts may lead to new therapeutic options for inflammatory bowel diseases. Our lab uses a helminth which infects the proximal small intestine of mice to identify new, potentially immunomodulatory molecules. In this project, we examined the impact of helminth infection on bile acids (BA)s; a class of metabolites that can function as immunomodulatory signalling molecules. To our knowledge, how helminth infection alters BA homeostasis, and the potential consequences for proximal small intestine immunity have not yet been reported. Purpose Our aims were to determine whether helminth infection alters (1) the intestinal BA pool, (2) expression of BA transporters in the small intestine, and (3) expression and/or activation of the Farnesoid X receptor (FXR), a BA signalling receptor. Method We used a murine model system to study the effects of the mouse-specific helminth Heligmosomoides polygyrus on BA homeostasis, BA transporter gene expression, and BA signalling receptors in the small intestine. Male and female littermate mice were used. Targeted metabolomics was used to assess the composition of the BA pool in the small intestine. Reverse transcription qPCR was used to quantify gene expression of BA transporters/signalling molecules in tissues. All data were segregated by sex of mice for data analysis. Result(s) We found that helminth infection disrupts the BA pool of mice, with consistent reductions in T-CDCA and T-αMCA during helminth infection. In the proximal small intestine, expression of the genes that encode the apical sodium dependant BA transporter (ASBT) and organic solute transporter alpha (OSTα) were unaffected during helminth infection, however expression of the organic solute transporter β (OSTβ) gene, which encodes half of the basal BA transporter, was decreased. Expression of Shp-1, an indicator of activation of BA receptor FXR, and expression of the Fxr gene were significantly decreased in both the proximal and distal small intestine. Lastly, expression of the gene encoding Gpbar1 trended upwards in the proximal and distal small intestine of female mice. Conclusion(s) Infection of mice with a proximal small intestine-dwelling helminth impacts BA homeostasis in the small intestine. Helminth infection decreases expression of a BA transporter in the proximal small intestine, although whether this contributes to or is caused by perturbations in the BA pool during infection remains to be seen. The downregulation of BA signalling we see during helminth infection in the proximal small intestine points to a potential functional impact on the host immune response which we now intend to explore. Understanding the immunomodulatory mechanisms used by helminths to dampen the host immune response could lead to further advancements in treating intestinal inflammatory disorders such as Crohn’s and Colitis. 2. The Answer is Vitamin D! From Pediatrics to Geriatrics in Orthopaedics

Abstract

Vitamin D is necessary for the regulation of calcium and phosphate in the human body. Decreased vitamin D levels can alter the bone mineralization process. The prevalence of vitamin D deficiency in the general population is high, and low vitamin D levels are associated with disorders such as rickets and osteoporosis. As knowledge about vitamin D metabolism increases, physicians of all specialties are becoming more attentive to the vitamin D status of their patients. Similarly, orthopaedic surgeons, through various initiatives such as "Own the Bone," are making greater efforts to medically manage skeletal disorders. Unfortunately, universal guidelines for the optimization of vitamin D levels have not been adopted by orthopaedic surgeons, and, despite substantial efforts, vitamin D is not an integral part of most orthopaedic residency training programs. Although this may be partially attributed to attitudes among orthopaedic surgeons, the large number of vitamin D recommendations in the literature may be confusing and require substantial effort to synthesize into a viable approach for a given patient. Despite this confusion, orthopaedic surgeons should understand how to diagnose and manage disorders related to vitamin D and calcium deficiency.

Abstract

Because orthopaedic surgeons focus on identifying serious potential complications, such as heart attack, stroke, and deep vein thrombosis, during the preoperative assessment, correctable factors, such as smoking, may be overlooked. Chronic exposure to nicotine has been correlated with perioperative complications that lead to worse outcomes, including decreased patient satisfaction, longer hospitalization periods, and an increased rate of hospital readmission. It has been proven that smoking is a negative risk factor for decreased bone mineral density, which leads to increased fracture risk, heightened pain, postoperative wound and bone healing complications, decreased fusion rates, and postoperative tendon and ligament healing complications. Physician-led preoperative smoking cessation programs that include, but are not limited to, pharmacotherapy plans have been shown to improve primary surgical outcomes and smoking cessation rates. Smoking has detrimental effects on specialty-specific physiology; however, there are many effective options for intervention that can improve primary outcomes.

Abstract

Vitamin D is a steroid hormone that affects not only bone metabolism and strength but also a variety of musculoskeletal health and surgical outcomes that are relevant to orthopaedic medicine. Risk factors for vitamin D deficiency include sex, age, skin pigmentation, obesity, and preexisting conditions such as nephritic syndrome and malabsorption syndrome. Furthermore, vitamin D deficiency is associated with the development of postoperative complications, such as an increased risk of infection, morbidity, and mortality. The standardization of vitamin D terminology as well as a thorough understanding of the medical considerations associated with vitamin D deficiency can improve preoperative planning and clearance, and, ultimately, patient outcomes and satisfaction.

Therefore, the clinical importance of a comprehensive preoperative evaluation and medical clearance is crucial and may substantially improve postoperative outcomes. A thorough knowledge of cardiovascular perioperative planning and management can enable healthcare professionals to identify patients who are potentially at risk for cardiovascular complications, and eventually improve both short- and long-term patient outcomes and satisfaction.

Abstract

We report a 32-year-old man who presented with a 2-year history of unexplained weight loss and symptoms of Raynaud's phenomenon. Clinical examination and investigations were consistent with a diagnosis of acquired partial lipodystrophy (Barraquer-Simons disease). This patient also exhibited an associated cutaneous vasculitis and peripheral perniotic changes. He had normal renal function and glucose tolerance and no immunological abnormality has been detected in his serum to date. The absence of C3-nephritic factor in the presence of overt lipodystrophy suggests that there may be another factor or immunological mediator responsible for the subcutaneous changes seen in patients with lipodystrophy.

Abstract

Background: By the age of 60 years, an estimated 33% of women will have undergone a hysterectomy. Approximately 85% of these hysterectomies are performed for benign disease. The object of this study was to evaluate cytologic findings from vaginal cuff smears in patients who have undergone hysterectomy for benign uterine conditions. Methods: We conducted a community-based retrospective study and foilow-up of women with vaginal cuff cytologic smears who had had a hysterectomy for benign uterine conditions. A total of 220 women were randomly selected who had one or more vaginal cuff smears. The main outcomes measures were invasive carcinoma, dysplastic lesions, and infections detected by vaginal cuff smear testing. The setting was a large inner-city hospital. Results: Ninety-seven percent of 220 women who underwent hysterectomy for benign uterine conditions and who were observed for an average of 89 months had no cytologic abnormalities on vaginal cuff smears. Cytologic evaluation found no invasive carcinomas. Dysplastic lesions were detected in 7 patients (3%). Seventy percent of patients (n = 154) had one or more infections; these infections included bacterial vaginosis (106), trichomoniasis (95), candidiasis (40), koilocytosis suggestive of human papilloma virus (HPV) infection (3), and cytopathic effect of herpes (4). The prevalence of koilocytosis was much higher in the patients with dysplasia (P = .0003). Conclusions: Most routine vaginal cuff cytology screening tests need not be performed in women who have had a hysterectomy for benign uterine conditions.

Abstract

The effect of source power, bias power, chamber pressure, flow rate, and feed gas composition on profile evolution during polycrystalline silicon etching with an oxide hardmask has been studied in a transformer-coupled plasma system. The large resultant data set provides a comprehensive look at feature evolution as plasma parameters are varied. This data set is valuable for evaluating the importance of several proposed mechanisms for feature evolution and for validating computational models. Microtrench formation was found to be a strong function of the plasma condition. A correlation between sidewall shape and microtrench development was observed. Profile development was found to be highly sensitive to feed gas composition. Results are consistent with a previous study on the etching of crystalline silicon under identical plasma conditions.

Nonideal feature profile anomalies such as undercut, tapered, or bowed sidewalls and microtrenches at the base of trench sidewalls are often observed after etching masked silicon (Si) in chlorine (Cl2) plasmas. Off-normal impact with subsequent scattering and/or focusing of ions is believed to be the primary cause of these anomalies. Localized buildup of negative charge on the insulating mask sidewalls is one possible source of the ion deflection. Here we show that nearly identically shaped sidewalls and microtrenches were formed when Si features were etched in several Cl2 plasmas (the rf source and substrate bias powers and pressure were varied) regardless of whether the mask was insulating (SiO2) or conducting (Si or tungsten). Therefore, differential charging of the insulating mask material is not the fundamental cause of these profile characteristics. Abstract

A patient presented with features of olivopontocerebellar atrophy and was found to have marked hyperglycinaemia. Severe atrophy of the cerebellum and brain stem was found at post-mortem, with numerous glial cytoplasmic inclusions (GCIs) in atrophic areas, characteristic of multiple system atrophy. In situ hybridization studies of the spinal cord demonstrated a selective reduction in expression of glycine transporter mRNA. We suggest that the resulting impairment of regulation of glycine concentrations at synaptic level resulted in excitotoxic damage to neurons.

Abstract

Two patients with presumed encephalitis lethargica are presented with clinical features suggestive of two forms of the disease described by Von Economo: One patient had a psychosis and a mute-akinetic syndrome associated with myoclonus. The second patient presented with a psychosis and fever, developing severe dyskinesias involving the mouth, trunk and limbs, together with respiratory irregularities and presumed hypothalamic disturbance and disturbance of consciousness. In both cases, initial cerebrospinal fluid (CSF) examination revealed an elevated white cell count (predominantly lymphocytes), elevated protein in case 2, and oligoclonal bands in both cases. Computed tomography (CT) brain scan was normal but in both cases EEG revealed diffuse slow wave activity. A 18F-Dopa positron emission tomography (PET) scan in case 2 was normal. The medical management of both patients is discussed. In case 1, L-Dopa improved the akinesia, while the myoclonus responded to clonazepam. In case 2, the severe dyskinesias failed to respond to a number of drugs, and she ultimately required paralysis to relieve her almost continuous movements. Both patients responded rapidly and dramatically to intravenous methylprednisolone. We suggest that steroid treatment should be considered in the acute phase of patients with features suggestive of encephalitis lethargica.

Abstract

Three different epoxy resins, based on the diglycidylether of bisphenol A (DGEBA), triglycidyl-p-aminophenol (TGPAP) and tetra-glycidyldiaminodiphenylmethane (TGDDM), which are di-, tri- and tetrafunctional, respectively, were mixed in varying proportions and cured with both 3,3′-diaminodiphenylsulphone and 4,4′-[1,4-phenylene(1-methylethylidene)]bis(2,6-dimethylbenzenamine) (EPON 1062-M from Shell). All the blends could be satisfactorily cured and gave homogeneous materials. The dynamic mechanical and fracture properties of the cured materials were measured. It was found that the glass transition temperature varied with composition systematically, whereas values of the strain energy release rate (G1c) and the stress intensity factor (K1c) showed relatively small variations with the blend composition. Toughened epoxy resins were prepared by adding a polyetherimide (PEI), in varying proportions, to the resin mixture. The ‘toughenabilities’ of different resins, or resin mixtures, were compared. This showed that the 75/25 TGPAP/DGEBA resin mixture was the most toughenable. Adding 20% of PEI led to a more than three-fold increase of the G1c value. © of SCI.

Abstract

Mutations of SOD-1 have recently been associated with autosomal dominant familial amyotrophic lateral sclerosis (ALS). A patient is described with a 20 year duration of motor neuron disease, with clinical features of ALS, who was heterozygous for a point mutation ATT to ACT leading to substitution of isoleucine for threonine at codon 113 in exon 4 of SOD-1. This mutation has previously been described in two families with ALS and three apparently sporadic cases of ALS. The patient described here had a family history suggestive of autosomal dominant inheritance of this genetic mutation; other members of the family having a more typical disease duration. Unusual pathological features included neurofibrillary tangles in neurons of the globus pallidus, substantia nigra, locus coeruleus, and inferior olivary nuclei, and absence of ubiquitin immunoreactive inclusions in motor neurons. This may reflect the slow progression of the neurodegeneration associated with the SOD-1 mutation in this patient. The prolonged survival, of over 20 years, with other family members having a more typical survival of two to three years, has important implications for genetic counselling in families with ALS in addition to the fundamental biological questions concerning the influence of these mutations on disease expression.

Abstract

A series of blends has been prepared by adding a polyetherimide, in varying proportions, to a trifunctional epoxy resin, triglycidylparaaminophenol, cured with 4,4′-diaminodiphenylsulphone. All the materials showed two-phase morphology when characterized by dynamic mechanical thermal analysis and scanning electron microscopy. Addition of the thermoplastic resulted in improved fracture properties (K1C and G1C), as measured by three-point bending experiments, although no obvious correlation with blend morphology was observed.

Abstract

We report a case of dermatomyositis occurring in association with transitional cell carcinoma of the bladder. The case illustrates the importance of a thorough search for neoplasms in elderly patients with dermatomyositis and is a reminder that bladder cancer may be a rare cause of dermatomyositis. The case also shows that successful treatment of an underlying tumour may lead to resolution of paraneoplastic dermatomyositis, and relapse of cutaneous and muscle symptoms and signs may indicate recurrence of tumour.

Abstract

A subgenomic restriction fragment from cDNA prepared from Coxsackie B2 virus (CVB2) RNA was subcloned into a riboprobe vector allowing the production of enteroviral group-specific RNA probes complementary to either the positive (genomic) or negative (template) strand of enteroviral RNA. These riboprobes were used to follow productive infection of cultured cells by CVB2; as expected, positive strand RNA was synthesized in approximately 100-fold excess over negative strand. RNA was extracted from muscle biopsy samples from patients with chronic fatigue syndrome and probed for the presence of enteroviral RNA. In cases where enteroviral RNA was detected the amounts of positive and negative strands of enteroviral RNA were approximately equal, in contrast to the situation in lytic infection of cultured cells. This suggests that enterovirus persistence in muscle is due to a defect in control of viral RNA synthesis.

Abstract

The Smallpox Eradication Program, initiated by the WHO in 1966, was originally based on mass vaccination. The program emphasized surveillance from the beginning, largely to track the success of the program and further our understanding of the epidemiology of the disease. Early observations in West Africa, bolstered by later data from Indonesia and the Asian subcontinent, showed that smallpox did not spread rapidly, and outbreaks could be quickly controlled by isolation of patients and vaccination of their contacts. Contacts were usually easy to find because transmission of smallpox usually required prolonged face-to-face contact. The emphasis therefore shifted to active searches to find cases, coupled with contact tracing, rigorous isolation of patients, and vaccination and surveillance of contacts to contain outbreaks. This shift away from mass vaccination resulted in an acceleration of the program’s success.

Abstract

With the aging international population, osteoporosis has become an epidemic. This painless disease is characterized by a decreased bone mass, resulting in decreased structural integrity of bone, and often goes undiagnosed. Typical osteoporotic fractures include vertebrae, hip, and wrist fractures, and these may have a dramatic impact on quality of life, even if the fracture is successfully treated. Many antiresorptive agents have demonstrated the ability to reduce the risk of osteoporotic fractures, and newer anabolic agents may further reduce risk. Non-medical treatments, such as external hip protectors and balance and low-impact strength training, are also very effective in preventing fractures. Before specific treatments can be addressed, however, osteoporosis must first be considered as a diagnosis in any patient with a low-energy fracture. This requires continued public health initiatives involving patient and physician education regarding the necessity for bone mass measurement and the merits of antiresorptive therapy.

They can cause significant morbidity and mortality, including physical, functional, and psychosocial impairments. Screening using fracture risk assessments can identify those at greatest risk. Medical and non-medical comprise appropriate, comprehensive treatment programs. The minimally invasive spine procedures vertebroplasty and kyphoplasty can help to significantly reduce pain and deformity in patients with severe pain and deformity from vertebral fracture.

Abstract

Myasthenia gravis is usually a chronic disorder, although remission rates of 25–30% can be expected with judicious interventions such as thymectomy and immunosuppression in appropriate cases. Spontaneous remission occurs rarely. Oosterhuis found a remission rate of only 1% per annum over 17 years among 180 patients with generalised myasthenia gravis treated with anticholinesterases alone.1 We describe a case of ocular and bulbar myasthenia in which there was complete recovery over a matter of weeks without immunosuppressive agents. The patient, a 78 year old woman, developed acute, painless left sided ptosis in October 2002, while on holiday abroad. There was no diplopia. She had developed hypothyroidism one year before but was euthyroid on replacement therapy. The ptosis resolved gradually over the next few days. There was no family history of autoimmune diseases. Contemporary orthopedic literature on therapeutic options for osteosarcoma patients is reviewed. Background information on the basic principles of kinesiology, with emphasis on studies of gait pattern differences among patients treated with limb salvage versus amputation, is presented. Finally, several studies of the psychologic profiles of patients after these two procedures for osteosarcoma are reviewed. Trends in contemporary orthopedic literature suggest that functional outcomes, in terms of kinesiologic parameters, are comparable for patients treated with either limb salvage or amputation. Both sets of patients reported quality-of-life problems, including difficulty retaining health insurance and finding appropriate employment, social isolation, and poor self-esteem. The management of patients with osteosarcoma includes not only an individualized surgical plan for each patient but also includes awareness of the patients' psychologic and social needs after surgery.

33. Interleukin-4 transgenic mice develop glomerulosclerosis independent of immunoglobulin deposition

Abstract

Mice with constitutive transgenic (tg) expression of IL-4 develop autoimmune-type disorders resembling human lupus nephritis. The kidneys show progressive glomerulosclerosis with immunoglobulin (Ig) and complement deposition. This study investigated the roles of renal IL-4 expression and glomerular Ig deposition in the pathogenesis of glomerulosclerosis in IL-4 tg mice. Treatment of these mice with IL-4 neutralizing antibody prevented renal disease. IL-4 tg mice treated with methylprednisolone (MP) showed increased mesangial collagen deposition with only trace amounts of glomerular Ig. To analyze the relevance of Ig deposition in the development of the renal lesions, IL-4 tg mice were cross-bred with mu chain-deficient mice (muMT-/-), which are unable to produce Ig. IL-4 tg/muMT-/- mice developed progressive glomerulosclerosis with mesangial accumulation of collagen types I, IV and V despite complete absence of glomerular Ig deposits. Renal IL-4 expression was observed in both anti-IL-4- and MP-treated IL-4 tg mice as well as in IL-4 tg/muMT-/- mice. No statistical difference in the number of glomerular T cells and macrophages between any of the groups was evident. Our data demonstrate that in this model glomerulosclerosis can develop independently of and prior to Ig deposition, and suggest that the initial accumulation of glomerular extracellular matrix is due to renal IL-4 expression. Our results point to a novel mechanism for the development of glomerulosclerosis which may have implications for human disease.

Abstract

Distraction osteogenesis is a novel technique for the biological restoration of segmental bone defects. Definitive treatment of musculoskeletal tumors often requires large bony resections that can leave patients with significant osseous defects. Limb salvage using distraction osteogenesis is an attractive reconstructive alternative that may, in fact, offer advantages over other conventional techniques. We present our initial experience with the use of distraction osteogenesis in limb salvage.

Abstract

The decreased bone mass associated with osteoporosis reduces the load-bearing capacity of both cancellous and cortical bone resulting in an increased fracture risk among the affected population. This disease affects over 28 million men and women in the United States [1], which underscores the importance of understanding both the treatment of this common metabolic bone disease and the augmentation of normal fracture healing. Osteoporosis does not impair fracture healing, but diminished bone mass causes a decrease in surface contact between bone surfaces and may result in increased time restoring normal bone strength. The diminished bone mass also reduces the strength and stability between the bone and fixation devices, which can lead to delayed fracture healing and nonunion [2]. Thus, treatment of osteoporotic fractures may benefit from the enhancement and acceleration of the healing process. Other conditions implicated as risk factors for impaired fracture healing, delayed union, and nonunion include infection, distraction, instability, age, obesity, soft tissue trauma and fracture site.

41. Traumatic lumbar pseudomeningocele occurring with spina bifida occulta

Abstract

Pseudomeningocele in the lumbar spine due to spinal fractures or surgical procedures are well-recognized complications. We treated a 24-year-old man who fell from a horse, landing on his buttocks, and developed headaches, back pain, and a large lumbar subcutaneous fluid collection. Plain radiographs and magnetic resonance (MR) imaging revealed a midline defect in the sacral lamina and a large fluid collection in the subcutaneous space consistent with cerebrospinal fluid (CSF). He was treated conservatively, his symptoms resolved, and follow-up MR imaging showed resolution of the CSF accumulation. To our knowledge, there have been no cases of traumatic pseudomeningocele related to spina bifida occulta reported in the literature. The authors postulate that congenital defects in the neural arch contribute to the occurrence of this entity, and nonsurgical treatment should be the initial course.

Abstract

Although recent in vivo studies indicate that basic fibroblast growth factor hastens the healing and strength of the medical collateral ligament after injury, in vitro studies with epidermal growth factor and basic fibroblast growth factor have shown increased fibroblast proliferation with the exogenous administration of these growth factors. Using an established spontaneously healing rabbit injury model, the surgical transection of the medial collateral ligament was undertaken in 12 anesthetized male adult rabbits. Immunohistochemical localization using monoclonal antibodies to the basic fibroblast growth factor receptor and epidermal growth factor receptor were used to identify the distribution and relative concentrations of the individual receptors at 3, 7, 14, and 28 days after surgery. Realizing that the trophic effects of basic fibroblast growth factor and epidermal growth factor are determined exclusively by their individual receptors, this study confirms the hypothesis that basic fibroblast growth factor and epidermal growth factor receptor proteins are present and increase during the early stages of medial collateral ligament healing. The understanding of this rabbit ligament injury model has far reaching implications to ligament healing seen in humans. By elucidating the spatial and temporal regulation of the basic fibroblast growth factor and epidermal growth factor receptor proteins, exogenous growth factor therapy, once approved for human use, potentially can be synchronized with maximal protein receptor levels.

Abstract

Article abstract-Mutations of the gene SOD-1, which encodes the enzyme copper-zinc superoxide dismutase, occur in patients with a familial form of amyotrophic lateral sclerosis (ALS). We investigated 71 families with more than one individual affected by ALS for clinical features and SOD-1 mutations. Mutations were identified in 14 families, indicating the presence of SOD-1 mutations in around 20% of this population. There were 10 different heterozygote missense point mutations in eight different codons, and a novel two-base frameshift insertion (132insTT), which leads to substitution of aspartic acid for glutamic acid at codon 132, and a premature stop codon at 133, with predicted truncation of the protein. SOD enzyme activity was reduced to around 50% of normal in individuals with SOD-1 mutations, and may be a useful predictor for the presence of these mutations. A predilection for disease onset in the lower limbs appears to be a distinguishing feature of familial ALS with SOD-1 mutations, and accords with findings in transgenic mouse models. Abstract

The treatment of Ewing's sarcoma consists of chemotherapy for systemic and local disease. However, the role of radiation therapy, and/or surgical resection for definitive local treatment has yet to be determined.A retrospective review of 32 patients (24 males and 8 females) treated for femoral Ewing's sarcoma between 1970 and 1985 was performed. Patients were divided into 3 treatment groups: chemotherapy and radiotherapy (CR) (10); chemotherapy and surgery (CS) (9); and chemotherapy, surgery, and radiotherapy (CSR) (13). Patients in the CR group received a mean of 5320 centigray (cGy) of radiation and patients in the CSR group received a mean of 3590 cGy. Multiagent cyclophosphamide/doxorubicin based chemotherapy was used in all cases. Surgery consisted of wide resection or amputation.Patients in the CR group had a higher risk of local recurrence than patients in the CS and CSR groups (P=0.02, log rank). The combination of local recurrences and treatment complications necessitated surgery for 7 of 10 CR patients, whereas 1 of 9 and 4 of 13 in the CS and CSR groups required additional surgery. The median survival for the entire group was 39 months. Minimum follow-up for surviving patients was 45 months. Five-year survival consisted of 1 of 10 patients in the CR group, 2 of 9 in the CS group, and 7 of 13 in the CSR group. There were no statistically significant differences among the three survival curves. Tumor location within the femur was a significant prognostic variable. Distal femoral location had a survival advantage compared with proximal and mid-femur locations (P = 0.049, log rank).Femoral Ewing's sarcoma remains a disease with a poor prognosis. Radiation alone for local treatment results in a high rate of local recurrence and complications. Our current local treatment strategy for femoral Ewing's sarcoma includes surgery in all and adjuvant radiotherapy in many of the patients.

Abstract

Bone morphogenetic protein-2 is a low molecular weight glycoprotein, classified as a morphogen. The sine qua non of bone morphogenetic protein is consistently reproducible induction of bone development in heterotopic sites. Bone morphogenetic proteins belong to the expanding transforming growth factor-beta superfamily. Bone morphogenetic protein-2 has pleiotropic functions that range from extraskeletal and skeletal organogenesis to bone generation and regeneration. Bone morphogenetic protein induced bone formation in postfetal life recapitulates the process of embryonic and endochondral ossification. Through recombinant gene technology, human bone morphogenetic protein-2 is available in almost unlimited amounts for basic research and clinical trials. Human bone morphogenetic protein-2 induces structurally sound orthotopic bone in a variety of experimental systems, including femoral defects in rats, tibial and ulnar defects in rabbits, femoral defects in sheep, mandibular defects in dogs, spinal fusion in dogs, and porous ingrowth in rats. Human bone morphogenetic protein-2 research extends to the fields of developmental biology, genetics, and evolution. Bone morphogenetic protein has been used successfully at the authors' institution to heal clinical nonunions and to achieve spinal fusion. This report reviews the current understanding of bone morphogenetic proteins in general and BMP-2 in particular and summarizes their potential applications.

Abstract

High grade soft tissue sarcoma arising in the popliteal space, axilla, and antecubital fossae (flexor fossae tumors) have by convention been classified as extracompartmental tumors by the accepted staging and grading criteria of the Musculoskeletal Tumor Society (MSTS). Advances in neoadjuvant chemotherapy and radiation therapy have made surgical resection more feasible. The hypothesis to be tested is that compartmental status may not be of prognostic significance if the tumor is adjusted for size, histologic grade, and distant metastasis after undergoing adjuvant chemotherapy and radiation.From June 1976 to December 1992, 22 patients with high grade soft tissue sarcomas of the flexor fossae (Group A) were treated at UCLA Medical Center. The histologic subtypes were liposarcoma (five), synovial cell sarcoma (eight), malignant fibrous histiocytoma (four), leiomyosarcoma (two), angiosarcoma (two), and rhabdomyosarcoma (one). The popliteal fossa was the location in 11, the axilla in 10, and the antecubital fossa in 1. Wide resection was attempted in all patients after preoperative chemotherapy and radiation therapy. Amputation was performed in 5 patients because of repeated or extensive recurrent tumor. A group of 77 patients (Group B) with high grade soft tissue sarcoma located within an extremity compartment were chosen to test the hypothesis that survival of patients with tumors in the flexor fossae is equal to that of patients with intracompartmental tumors of similar size and grade if both are given adjuvant therapy. This group was chosen so that histologic subtype, size, sex, and location would be similar in the two groups. The authors selected thigh and calf tumors for comparison with popliteal fossa tumors and periscapular, deltoid, and arm tumors for comparison with axilla and antecubital fossae tumors. All of these patients had similar treatment and follow-up protocols. The median follow-up of survivors in Group A was 104 months and for patients in Group B was 79 months.The 5-year cumulative survival rate (Kaplan-Meier method) of patients in Group A was 76%, and 67% for those in Group B. The difference was not significant. Three patients in Group A (14%) and 17 (22%) in Group B had local tumor recurrence. Eight patients in Group A (36%) and 27 (35%) in Group B had lung metastases. Age, sex, histologic subtype, and surgical margins did not affect survival outcomes, lung metastasis, and local recurrence. However, patients with larger tumors (maximum dimensionor = 8 cm or cross-sectional areaor = 40 cm2) had significantly poorer survival, more metastases, and local recurrences.Flexor fossae sarcomas do not have a poorer prognosis than extremity intracompartmental tumors when adjusted for size, distant metastasis, and histologic grade when they are treated with adjuvant radiation therapy, chemotherapy, and surgery.

Abstract

Saccadic eye movements, fixation, and smooth pursuit were recorded in 17 subjects with amyotrophic lateral sclerosis (ALS) and 11 age-matched controls using a magnetic scleral search coil. Reflexive, remembered and antisaccades, and smooth pursuit at four target velocities were studied. Subjects with ALS showed significantly elevated error rates (distractibility) and latency in the antisaccade and remembered saccade paradigms but no abnormality of reflexive saccades. The frequency of small saccades that intruded on steady fixation (square-wave jerks) was also increased in ALS subjects. Peak velocity gain of smooth pursuit and performance on the Wisconsin Card Sort Test did not differ significantly between the two groups. These findings are consistent with prefrontal dysfunction in ALS and provide an independent source of support for the thesis that the pathology of this condition invades frontal cortex.
