Abstract

Abstract Smoldering multiple myeloma (SMM) precedes multiple myeloma (MM). The risk of progression of SMM patients is not uniform, thus different progression-risk models have been developed, although they are mainly based on clinical parameters. Recently, genomic predictors of progression have been defined for untreated SMM. However, the usefulness of such markers in the context of clinical trials evaluating upfront treatment in high-risk SMM (HR SMM) has not been explored yet, precluding the identification of baseline genomic alterations leading to drug resistance. For this reason, we carried out next-generation sequencing and fluorescent in-situ hybridization studies on 57 HR and ultra-high risk (UHR) SMM patients treated in the phase II GEM-CESAR clinical trial (NCT02415413). DIS3, FAM46C, and FGFR3 mutations, as well as t(4;14) and 1q alterations, were enriched in HR SMM. TRAF3 mutations were specifically associated with UHR SMM but identified cases with improved outcomes. Importantly, novel potential predictors of treatment resistance were identified: NRAS mutations and the co-occurrence of t(4;14) plus FGFR3 mutations were associated with an increased risk of biological progression. In conclusion, we have carried out for the first time a molecular characterization of HR SMM patients treated with an intensive regimen, identifying genomic predictors of poor outcomes in this setting.

Abstract

Histiocytic sarcoma (HS) is a neoplasm derived from histiocytes. Its diagnosis was not clear until its immunohistochemistry profile was correctly established. Not much is known about its genetic properties. We report a case of a 48-year-old male patient whose bone marrow was almost completely occupied by monomorphic medium size neoplastic cellularity. Its immunohistochemical profile was CD68+, CD4+, CD45+ with negativity of other dendritic cells, and other lineage markers. Cytogenetic study showed 4 related clones: one with trisomy 8 and extra material on the short arms of chromosome 4; a second line with tetrasomy of chromosome 8, add(4)(p16); the third clone had the same alterations as the previous and deletion of chromosome 3 at q11; the fourth line had tetrasomy 8 and translocation t(3;5)(q25;q35). To our knowledge this is the first HS case showing chromosome 8 trisomy and tetrasomy and the other described alterations.

Abstract

Summary Background Patients who are immunocompromised because of malignancy have an increased risk of herpes zoster and herpes zoster-related complications. We aimed to investigate the efficacy and safety of an inactivated varicella zoster virus (VZV) vaccine for herpes zoster prevention in patients with solid tumour or haematological malignancies. Methods This phase 3, two-arm, randomised, double-blind, placebo-controlled, multicentre trial with an adaptive design was done in 329 centres across 40 countries. The trial included adult patients with solid tumour malignancies receiving chemotherapy and those with haematological malignancies, either receiving or not receiving chemotherapy. Patients were randomly assigned (1:1) to receive four doses of VZV vaccine inactivated by γ irradiation or placebo approximately 30 days apart. The patients, investigators, trial site staff, clinical adjudication committee, and sponsor's clinical and laboratory personnel were masked to the group assignment. The primary efficacy endpoint was herpes zoster incidence in patients with solid tumour malignancies receiving chemotherapy, which was assessed in the modified intention-to-treat population (defined as all randomly assigned patients who received at least one dose of inactivated VZV vaccine or placebo). The primary safety endpoint was serious adverse events up to 28 days after the fourth dose in patients with solid tumour malignancies receiving chemotherapy. Safety endpoints were assessed in all patients who received at least one dose of inactivated VZV vaccine or placebo and had follow-up data. This trial is registered ( NCT01254630 and EudraCT 2010-023156-89). Findings Between June 27, 2011, and April 11, 2017, 5286 patients were randomly assigned to receive VZV vaccine inactivated by γ irradiation (n=2637) or placebo (n=2649). The haematological malignancy arm was terminated early because of evidence of futility at a planned interim analysis; therefore, all prespecified haematological malignancy endpoints were deemed exploratory. In patients with solid tumour malignancies in the modified intention-to-treat population, confirmed herpes zoster occurred in 22 of 1328 (6·7 per 1000 person-years) VZV vaccine recipients and in 61 of 1350 (18·5 per 1000 person-years) placebo recipients. Estimated vaccine efficacy against herpes zoster in patients with solid tumour malignancies was 63·6% (97·5% CI 36·4 to 79·1), meeting the prespecified success criterion. In patients with solid tumour malignancies, serious adverse events were similar in frequency across treatment groups, occurring in 298 (22·5%) of 1322 patients who received the vaccine and in 283 (21·0%) of 1346 patients who received placebo (risk difference 1·5%, 95% CI −1·7 to 4·6). Vaccine-related serious adverse events were less than 1% in each treatment group. Vaccine-related injection-site reactions were more common in the vaccine group than in the placebo group. In the haematological malignancy group, VZV vaccine was well tolerated and estimated vaccine efficacy against herpes zoster was 16·8% (95% CI −17·8 to 41·3). Interpretation The inactivated VZV vaccine was well tolerated and efficacious for herpes zoster prevention in patients with solid tumour malignancies receiving chemotherapy, but was not efficacious for herpes zoster prevention in patients with haematological malignancies. Abstract

In a series of experiments at the SISSI/LISE3 facility of GANIL conducted between 1999 and 2004, we have collected decay information for proton-rich nuclei between {sup 36}Ca and {sup 56}Zn. The data allowed us to study the decay properties of 26 nuclei. The main experimental information obtained for all nuclei is their {beta}-decay half-life and their total {beta}-delayed proton emission branching ratio. For many nuclei, individual proton groups and {gamma} rays were identified and allowed us to establish first partial decay schemes for some of the nuclei studied. In addition, mass-excess values have been determined for some of the nuclei by means of the isobaric multiplet mass equation. For {sup 50}Ni, the decay via {beta}-delayed two-proton emission could be tentatively identified. The decay of {sup 49}Ni allowed for the first time to identify the first 2{sup +} state in {sup 48}Fe. The experimental data are confronted to model predictions for the half-life and the mass-excess values.

Abstract

The structure of the A/Z = 3 nucleus 15B has been investigated using the in-beam $\gamma$ -spectroscopy technique with a fragmentation reaction of a 36S beam on a 9Be target at 77.5 ${\rm MeV}\cdot A$ . The fragments were identified and selected by their energy loss and time of flight using the SPEG spectrograph. $\gamma$ -ray energies and intensities have been measured in coincidence with the projectile-like fragments. From this information as well as from the $\gamma\gamma$ -coincidence relationships a level scheme is proposed for 15B up to the neutron separation energy. The experimental results have been interpreted using shell model calculations in the psd valence space. Effects of the weakly bound nature of the valence neutrons have been observed.

Abstract

In an experiment at the SISSI-LISE3 facility of GANIL, the decay of the proton drip-line nucleus Fe45 has been studied after projectile fragmentation of a Ni58 primary beam at 75 MeV/nucleon impinging on a natural nickel target. Fragment-implantation events have been correlated with radioactive decay events in a 16x16 pixel silicon strip detector on an event-by-event basis. The decay-energy spectrum of Fe45 implants shows a distinct peak at (1.06+/-0.04)MeV with a half-life of T1/2 = (4.7+3.4-1.4)ms. None of the events in this peak is in coincidence with beta particles which were searched for in a detector next to the implantation detector. For a longer correlation interval, daughter decays of the two-proton daughter Cr43 can be observed after Fe45 implantation. The decay energy for Fe45 agrees nicely with several theoretical predictions for two-proton emission. Barrier-penetration calculations slightly favour a di-proton emission picture over an emission of two individual protons and point thus to a He2 emission mode.

Abstract

The CONGA project concentrated on theoretical and experimental studies investigating the behaviour of advanced light water reactor containments containing passive containment heat removal systems and catalytic recombiners expected to be effectively operational during a hypothetical severe accident involving large quantities of aerosol particles and noncondensable gases. The central point of interest was the investigation of the effect of aerosol deposition on the condensation heat transfer of specially designed finned-type heat exchangers (HX) as well as the recombination efficiency of catalytic recombiners. A conceptual double-wall Italian PWR design and a SWR1000 design from Siemens were considered specifically as the reference Pressurized Water Reactor (PWR) and Boiling Water Reactor (BWR) designs. An assessment of selected accident scenarios was performed in order to define the range of boundary conditions necessary to perform the experimental studies of the other work packages. Experimental investigations indicated that aerosol deposition accounted for up to 37% loss in the heat removal capacity of the two-tube-layer BWR HX units. However, no significant heat transfer degradation could be observed for the PWR HX units. These results can be attributed to the important differences in the designs and operating conditions of the two units. The tests to study the effect of hydrogen (simulated by helium) on the heat transfer rate for heat exchanger units designed for BWR and PWR applications indicated a degradation less than 30% under various conditions. This was found to be acceptable within the over capacity designed for the heat exchangers or containment characteristics. The tests performed to study the long-term aerosol behaviour in the pressure suppression chamber of the current operating BWRs indicated that the water pool scrubs the aerosol particles effectively and reduces the ultimate aerosol load expected on the off-gas system. The efficiency of the catalytic recombiner system designed by Siemens for the off-gas system was found to be insensitive to the aerosol deposited in the recombiner. A computation code, HTCFOUL, was developed to predict the heat transfer rate of a finned-type heat exchanger subjected to a steam–noncondensable gas mixture containing airborne aerosol particles. The model predicts the non-aerosol part of two tests within a variation of 26% and the aerosol part within 32%.

Abstract

The objective of the technology enhancement for passive safety systems (TEPSS) project TEPSS Final Report, INNO-TEPSS(99)-D21 (1999)) was to make significant additions to the technology base of the European simplified boiling water reactor (ESBWR). The project focused on mixing and stratification phenomena in large water pools, passive decay heat removal from containments, and effects of aerosol deposition inside a passive heat exchanger. The Paul Scherrer Institut (PSI) experimental facility—large-scale investigation of natural circulation and mixing—has been used to investigate venting of steam and steam/non-condensable gas mixtures into water pools. The test revealed that no significant steam bypass could be detected when injecting a mixture of steam or air and that mixing was very efficient. In addition to the tests, 3-D numerical computations and initial model development have been performed to study the behaviour of bubble plumes in water pools. The major part of the TEPSS project studied selective aspects of the response technology of modern pressure-suppression-type containment designs and of passive-type decay heat removal systems. The work included an experimental phase using the large-scale experimental facility—Passive Nachwarmeabbfuhr und DruckAbbau (PANDA), operated by PSI, where eight experiments successfully have been executed to test the performance of the ESBWR containment configuration. The PANDA tests have been analysed successfully using thermal–hydraulic system analysis codes and 3-D CFD codes. The aerosol impaction and deposition analysis experimental facility of PSI has been used to investigate the degradation of passive decay heat removal due to fission product aerosols deposited on the inside surfaces of the passive containment cooler (PCC) heat exchanger tubes. One test performed revealed that the degradation of the heat transfer in the PCC tubes due to the deposition of aerosols reached about 20%. The test has been analysed using the MELCOR severe accident analysis code.

Abstract

Isomeric low-lying states were identified and investigated in the {sup 75}Cu nucleus. Two states at 61.8(5)- and 128.3(7)-keV excitation energies with half-lives of 370(40)- and 170(15)-ns were assigned as {sup 75m1}Cu and {sup 75m2}Cu, respectively. The measured half-lives combined with the recent spin assignment of the ground state allow one to deduce tentatively spin and parity of the two isomers and the dominant multipolarities of the isomeric transitions with respect to the systematics of the Cu isotopes. Shell-model calculations using an up-to-date effective interaction reproduce the evolution of the 1/2{sup -}, 3/2{sup -}, and 5/2{sup -} states for the neutron-rich odd-mass Cu isotopes when filling the nug{sub 9/2}. The results indicate a significant change in the nuclear structure in this region, where a single-particle 5/2{sup -} state coexists with more and more collective 3/2{sup -} and 1/2{sup -} levels at low excitation energies.

Abstract

The structure of N19-22 nuclei was investigated by means of in-beam gamma-ray spectroscopic technique using fragmentation reactions of both stable and radioactive beams. Based on particle-gamma and particle-gamma gamma coincidence data, level schemes are constructed for the neutron-rich nitrogen nuclei. The experimental results are compared with shell model calculations. The strength of the N=14 and Z=8 shell closures and the weakening of the shell model interaction WBT are discussed.

Abstract

Nuclear structure of the neutron rich {sup 25-29}Ne nuclei has been investigated through the in-beam {gamma}-ray spectroscopy technique using fragmentation reactions of both stable and radioactive beams. Level schemes have been deduced for these Ne isotopes. In order to examine the importance of intruder fp configurations, they are compared to shell model calculations performed either in the restricted sd or in the larger sdpf valence space. The {sup 25,26}Ne and {sup 27}Ne nuclei were found to be in agreement with the sd shell model calculations, whereas {sup 28}Ne exhibits signatures of the intruder fp shell contribution.

Abstract

In an experiment performed at the Accelerator Laboratory of the University of Jyvaskyla, the beta-decay half-life of 62Ga has been studied with high precision using the IGISOL technique. A half-life of T1/2 = 116.09(17)ms was measured. Using beta-gamma coincidences, the gamma intensity of the 954keV transition and an upper limit of the beta-decay feeding of the 0+_2 state have been extracted. Abstract

In-beam gamma-ray spectroscopy using fragmentation reactions of both stable and radioactive beams has been performed in order to study the structure of excited states in neutron-rich oxygen isotopes with masses ranging from A=20 to 24. For the produced fragments, gamma-ray energies, intensities, and gamma-gamma coincidences have been measured. Based on this information new level schemes are proposed for O-21,O-22 up to the neutron separation energy. The nonobservation of any gamma-decay branch from O-23 and O-24 suggests that their excited states lie above the neutron decay thresholds. From this, as well as from the level schemes proposed for O-21 and O-22, the size of the N=14 and 16 shell gaps in oxygen isotopes is discussed in the light of shell-model calculations.

Abstract

GANIL E400S, NIM; The excitation function for the elastic scattering reaction p($^{18}$Ne,p)$^{18}$Ne was measured with the first radioactive beam from the SPIRAL facility at the GANIL laboratory and with a solid cryogenic hydrogen target. Several broad resonances were observed, corresponding to new excited states in the unbound nucleus $^{19}$Na. In addition, the break-up reaction p($^{18}$Ne,2p)$^{17}$F was observed and is discussed.

Abstract

Experiment at the GANIL / LISE3 facility; International audience; According to most mass models, several of the best candidates for two‐proton radioactivity lie in the A = 50 mass region, at the proton drip‐line. Recent experiments using the fragmentation of a 58Ni beam allowed us to reach this region and observe some of these candidates, like 45Fe and 48Ni. During an experiment at the GANIL / LISE3 facility helded in 2000, we could identify 22 events of implantation of 45Fe nuclei. The energy distribution of the corresponding decay events shows a clear peak at (1.14 ± 0.04) MeV that could be assigned to the direct 2‐proton emission from the ground state. This energy is in very good agreement with several theoretical predictions, and no coincident β particle has been observed experimentally. A half‐life of (4.7−1.4+3.4) ms has been measured for the decay of 45Fe.

Abstract

In an experiment at the SISSI-LISE3 facility of GANIL, the decay of the proton drip line nucleus $^{\mathrm{45}}\mathrm{F}\mathrm{e}$ has been studied. Fragment-implantation events have been correlated with radioactive decay events in a $16\ifmmode\times\else\texttimes\fi{}16$ pixel silicon-strip detector. The decay-energy spectrum of $^{\mathrm{45}}\mathrm{F}\mathrm{e}$ implants shows a distinct peak at $(1.14\ifmmode\pm\else\textpm\fi{}0.04)\text{ }\text{ }\mathrm{M}\mathrm{e}\mathrm{V}$ with a half-life of ${T}_{1/2}=({4.7}_{\ensuremath{-}1.4}^{+3.4})\text{ }\text{ }\mathrm{m}\mathrm{s}$. None of the events in this peak is in coincidence with $\ensuremath{\beta}$ particles. For a longer correlation interval, daughter decays of the two-proton daughter $^{\mathrm{43}}\mathrm{C}\mathrm{r}$ can be observed after $^{\mathrm{45}}\mathrm{F}\mathrm{e}$ implantation. The decay energy for $^{\mathrm{45}}\mathrm{F}\mathrm{e}$ agrees nicely with several theoretical predictions for two-proton radioactivity.

Abstract

This paper describes the joint research project DABASCO which is supported by the European Community under a cost-shared contract and participated by nine European institutions. The main objective of the project is to provide a generic experimental data base for the development of physical models and correlations for containment thermalhydraulic analysis. The project consists of seven separate-effects experimental programs which deal with new innovative conceptual features, e.g. passive decay heat removal and spray systems. The results of the various stages of the test programs will be assessed by industrial partners in relation to their applicability to reactor conditions.

Abstract

The β-decay of the Tz = - 2 nucleus 36Ca was studied at the LISE3 magnetic spectrometer at GANIL. Two new proton-emitting states have been detected and the other nine known βp and βγ transitions have been remeasured with improved resolution. A simulation with the GEANT code has been applied to this experimental setup. A comparison with shell model calculations is given.

Abstract

We present a review of experimental studies on the properties of extremely neutron-rich nuclei (Z = 6-20, A = 20-60) near the neutron drip line. Recent data on mass measurements of neutron-rich nuclei at, GANIL, (Caen, France) and some characteristics of binding energies in this region are discussed. Being sensitive to the existence of nuclear shells separation energies S-2n together with the evidence of particle instability of doubly magic nuclei He-10 and O-28 could provide information on changes in shell closures. Particularly opening of an N = 16 shell and vanishing of the N = 20 shell for very neutron-rich isotopes is under discussion. Decay studies of these hardly accessible short-lived neutron-rich nuclei from oxygen to silicon using the in-beam gamma -ray spectroscopy are also reported.

Abstract

Isomeric ratios and momentum distributions of nuclei produced in the fragmentation of a 60A MeV ${}^{92}\mathrm{Mo}$ beam on a thin ${}^{27}\mathrm{Al}$ target have been studied in detail. A strong dependence of the isomeric ratio on the structure of the isomer and on the reaction mechanism has been observed for the first time at intermediate energies. The results are quantitatively reproduced in a framework of kinematical and statistical models of nuclear reactions.

Abstract

A new isomeric state in 78Zn has been identified and studied in the fragmentation of a 60.5 AMeV 86Kr beam on a natNi target. The measured energies of the transitions and the half-life, T1/2=319(9) ns, of the isomeric decay suggest a spin and parity assignment F=S+. The deduced B(E2,S+ -+ 6+) value equals 1.21(5) W.u. is well reproduced in large scale shell model calculations using realistic interactions in the proton-neutron 2p3/2, 1f5/2' 2p1/2' 199 / 2 model space. Standard E2 polarisation charges of D.5e for both protons and neutrons were used. This result consists the first experimental evidence of the persistance of the N=50 shell gap in the vicinity f 78N' o 28 150·

Abstract

The structure of nucleus 25F was studied by use of in-beam -ray spectroscopy of the fragmentation of a 36S beam. The emitted rays were detected by BaF2 detectors. In the -ray spectrum obtained a wide bump between 3 and 4.5 MeV energy was observed corresponding to a set of rays, including their first and second escape peaks. In order to resolve the peaks and to determine the high-energy structure of 25F, the experimental spectra were compared to Geant4 simulations taking the complex line shape into account. The observed decomposition of the bump is in good agreement with the results of the two-step fragmentation experiment [Zs. Vajta et al., submitted to Phys. Rev. C]. Furthermore, an additional transition was observed at 2140(30) keV.

Abstract

We evaluated the role of high-dose granulocyte colony stimulating factor (G-CSF) in vitro, in inducing the generation of high-proliferative potential colony forming cells (HPP-CFC), from either mononuclear cells or purified CD34+ cells. Both normal controls and patients undergoing peripheral blood stem cell (PBSC) mobilization and transplantation were studied. In serum-driven agar cultures, G-CSF stimulated the proliferation of HPP-CFC in a dose dependent manner (r = 0.92). The number of HPP-CFC was four-fold greater in mobilized patients than in normal controls. Purified CD34+ cells yielded 11-fold more colonies than mononuclear cells. HPP-CFC from mobilized patients showed replating capacity, giving rise to secondary colonies of more mature appearance. In serum-free cultures, the effect of G-CSF appeared to be mediated by synergistic interaction with stem cell factor. Our results suggest that G-CSF stimulates primitive hematopoietic cells that are detectable in increased amounts in patients receiving mobilization therapy. Therefore, determination of G-CSF induced HPP-CFC could be a useful tool in the evaluation of mobilization strategies.

Abstract

Introduction: Scarce real-life data exists for COVID-19 management in hematologic disease (HD) patients in the Omicron era., Purpose: To assess the current clinical management and outcome of SARS-CoV-2 infection diagnosed, identify the risk factors for severe outcomes according to the HD characteristics and cell therapy procedures in a real-world setting., Methods: A retrospective observational registry led by the Spanish Transplant Group (GETH-TC) with 692 consecutive patients with HD from December 2021 to May 2023 was analyzed., Results: Nearly one-third of patients (31%) remained untreated and presented low COVID-19-related mortality (0.9%). Nirmatrelvir/ritonavir was used mainly in mild COVID-19 cases in the outpatient setting (32%) with a low mortality (1%), while treatment with remdesivir was preferentially administered in moderate-to-severe SARS-CoV-2 infection cases during hospitalization (35%) with a mortality rate of 8.6%. The hospital admission rate was 23%, while 18% developed pneumonia. COVID-19-related mortality in admitted patients was 14%. Older age, autologous hematopoietic stem cell transplantation (SCT), chimeric antigen receptor T-cell therapy, corticosteroids and incomplete vaccination were factors independently associated with COVID-19 severity and significantly related with higher rates of hospital admission and pneumonia. Incomplete vaccination status, treatment with prior anti-CD20 monoclonal antibodies, and comorbid cardiomyopathy were identified as independent risk factors for COVID-19 mortality., Conclusions: The results support that, albeit to a lower extent, COVID-19 in the Omicron era remains a significant problem in HD patients. Complete vaccination (3 doses) should be prioritized in these immunocompromised patients. Abstract

Smoldering multiple myeloma (SMM) precedes multiple myeloma (MM). The risk of progression of SMM patients is not uniform, thus different progression-risk models have been developed, although they are mainly based on clinical parameters. Recently, genomic predictors of progression have been defined for untreated SMM. However, the usefulness of such markers in the context of clinical trials evaluating upfront treatment in high-risk SMM (HR SMM) has not been explored yet, precluding the identification of baseline genomic alterations leading to drug resistance. For this reason, we carried out next-generation sequencing and fluorescent in-situ hybridization studies on 57 HR and ultra-high risk (UHR) SMM patients treated in the phase II GEM-CESAR clinical trial (NCT02415413). DIS3, FAM46C, and FGFR3 mutations, as well as t(4;14) and 1q alterations, were enriched in HR SMM. TRAF3 mutations were specifically associated with UHR SMM but identified cases with improved outcomes. Importantly, novel potential predictors of treatment resistance were identified: NRAS mutations and the co-occurrence of t(4;14) plus FGFR3 mutations were associated with an increased risk of biological progression. In conclusion, we have carried out for the first time a molecular characterization of HR SMM patients treated with an intensive regimen, identifying genomic predictors of poor outcomes in this setting., (© 2024. Abstract

Diffuse large B-cell lymphoma (DLBCL) requires a complete staging at diagnosis that may have prognostic and therapeutic implications. The role of bone marrow (BM) biopsy (BMB) is controversial in the era of nuclear imaging techniques. We performed a comparative review of 25 studies focused on BM evaluation at DLBCL diagnosis, including at least two of the following techniques: BMB, flow cytometry, and positron emission tomography (PET-FDG). The report about BM involvement (BMi), diagnostic accuracy, and prognostic significance was collected and compared among techniques. A concordance analysis between BMB, FCM, and PET was also performed, and we deeply evaluated the implications of the different types of BMi: concordant by LBCL or discordant by low-grade B-cell lymphoma for both BMB and FCM, and focal or diffuse uptake pattern for PET. As a main conclusion, BMB, FCM, and PET are complementary tools that provide different and clinically relevant information in the assessment of BMi in newly diagnosed DLBCL.

Abstract

Mitoviruses were initially known for their presence in the mitochondria of fungi and were considered exclusive to these organisms. However, recent studies have shown that they are also present in a large number of plant species. Despite the potential impact that mitoviruses might have on the mitochondria of plant cells, there is a lack of information about these ancient RNA viruses, especially within the Cannabaceae family. Cannabis sativa has been in the spotlight in recent years due to the growing industrial applications of plant derivatives, such as fiber and secondary metabolites. Given the importance of Cannabis in today's agriculture, our study aimed to expand the knowledge frontier of Mitoviruses in C. sativa by increasing the number of reference genomes of CasaMV1 available in public databases and representing a larger number of crops in countries where its industrial-scale growth is legalized. To achieve this goal, we used transcriptomics to sequence the first mitoviral genomes of Colombian crops and analyzed RNA-seq datasets available in the SRA databank. Additionally, the evolutionary analysis performed using the mitovirus genomes revealed two main lineages of CasaMV1, termed CasaMV1&#95;L1 and CasaMV1&#95;L2. Abstract

Background: Scarce data exist that analyze the outcomes of hematological patients with SARS-CoV-2 infection during the Omicron variant period who received treatment with remdesivir or nirmatrelvir/ritonavir., Methods: This study aims to address this issue by using a retrospective observational registry, created by the Spanish Hematopoietic Stem Cell Transplantation and Cell Therapy Group, spanning from 27 December 2021 to 30 April 2023., Results: This study included 466 patients, 243 (52%) who were treated with remdesivir and 223 (48%) with nirmatrelvir/ritonavir. Nirmatrelvir/ritonavir was primarily used for mild cases, resulting in a lower COVID-19-related mortality rate (1.3%), while remdesivir was preferred for moderate to severe cases (40%), exhibiting a higher mortality rate (9%). A multivariate analysis in the remdesivir cohort showed that male gender (odds ratio (OR) 0.35, p = 0.042) correlated with a lower mortality risk, while corticosteroid use (OR 9.4, p < 0.001) and co-infection (OR 2.8, p = 0.047) were linked to a higher mortality risk. Prolonged virus shedding was common, with 52% of patients shedding the virus for more than 25 days. In patients treated with remdesivir, factors associated with prolonged shedding included B-cell malignancy as well as underlying disease, severe disease, a later onset of and shorter duration of remdesivir treatment and a higher baseline viral load. Nirmatrelvir/ritonavir demonstrated a comparable safety profile to remdesivir, despite a higher risk of drug interactions., Conclusions: Nirmatrelvir/ritonavir proved to be a safe and effective option for treating mild cases in the outpatient setting, while remdesivir was preferred for severe cases, where corticosteroids and co-infection significantly predicted worse outcomes. Despite antiviral therapy, prolonged shedding remains a matter of concern.

Abstract

The long-term clinical efficacy of SARS-CoV-2 vaccines according to antibody response in immunosuppressed patients such as hematological patients has been little explored. A prospective multicenter registry-based cohort study conducted from December 2020 to July 2022 by the Spanish Transplant and Cell Therapy group, was used to analyze the relationship of antibody response over time after full vaccination (at 3-6 weeks, 3, 6 and 12 months) (2 doses) and of booster doses with breakthrough SARS-CoV-2 infection in 1551 patients with hematological disorders. At a median follow-up of 388 days after complete immunization, 266 out of 1551 (17%) developed breakthrough SARS-CoV-2 infection at median of 86 days (range 7-391) after full vaccination. The cumulative incidence was 18% [95% confidence interval (C.I.), 16-20%]. Multivariate analysis identified higher incidence in chronic lymphocytic leukemia patients (29%) and with the use of corticosteroids (24.5%), whereas female sex (15.5%) and more than 1 year after last therapy (14%) were associated with a lower incidence (p < 0.05 for all comparisons). Median antibody titers at different time points were significantly lower in breakthrough cases than in non-cases. A serological titer cut-off of 250 BAU/mL was predictive of breakthrough infection and its severity. SARS-CoV-2 infection-related mortality was encouragingly low (1.9%) in our series. Our study describes the incidence of and risk factors for COVID-19 breakthrough infections during the initial vaccination and booster doses in the 2021 to mid-2022 period. The level of antibody titers at any time after 2-dose vaccination is strongly linked with protection against both breakthrough infection and severe disease, even with the Omicron SARS-CoV-2 variant., (© 2023. Abstract

In 2016, the WHO included haemoglobin values within normal ranges as a diagnostic criterion for Polycythaemia Vera (PV). Since then, concerns have arisen that a large number of patients are undergoing unnecessary screening for PV. To address this issue, we estimated the prevalence of JAK2 p.V617F in individuals with elevated haemoglobin or haematocrit and developed and validated a screening algorithm for PV. A total of 15,366 blood counts performed in seven non-consecutive days were reviewed, of which 1001 were selected for subsequent JAK2 p.V617F mutation screening. Eight (0.8%) new JAK2 p.V617F-mutated cases were detected. From ROC curves, a two-step algorithm was developed based on the optimal cut-off for the detection of the JAK2 p.V617F mutation. The algorithm was prospectively validated in an independent cohort of 15,298 blood counts. A total of 1595 (10.4%) cases met the criterion for haemoglobin or haematocrit, of whom 581 passed to step 2 (3.8% of the total). The JAK2 p.V617F mutation was detected in 7 of the 501 patients tested, which accounts for 0.04% of the total cohort and 0.4% of patients with erythrocytosis. In conclusion, this data show that our two-step algorithm improves the selection of candidates for JAK2 p.V617F testing.

Abstract

We investigated the clinicopathological features and prognostic factors of patients with peripheral T-cell lymphoma (PTCL) in 13 sites across Spain. Relevant clinical antecedents, CD30 expression and staining pattern, prognostic indices using the International Prognostic Index and the Intergruppo Italiano Linfomi system, treatments, and clinical outcomes were examined. A sizeable proportion of 175 patients had a history of immune-related disorders (autoimmune 16%, viral infections 17%, chemo/radiotherapy-treated carcinomas 19%). The median progression-free survival (PFS) and overall survival (OS) were 7·9 and 15·8 months, respectively. Prognostic indices influenced PFS and OS, with a higher number of adverse factors resulting in shorter survival (P < 0·001). Complete response (CR) to treatment was associated with better PFS (62·6 vs. 4 months; P < 0·001) and longer OS (67·0 vs. 7·3 months; P < 0·001) compared to no CR. CD30 was expressed across all subtypes; >15% of cells were positive in anaplastic lymphoma kinase-positive and -negative anaplastic large-cell lymphoma and extranodal natural killer PTCL groups. We observed PTCL distribution across subtypes based on haematopathological re-evaluation. Poor prognosis, effect of specific prognostic indices, relevance of histopathological sub-classification, and response level to first-line treatment on outcomes were confirmed. Abstract

Background: Information is needed on the safety of adalimumab when used in pregnancy for the treatment of certain autoimmune diseases., Methods and Findings: Between 2004 and 2016, the Organization of Teratology Information Specialists Research Center at the University of California San Diego conducted a prospective controlled observational cohort study in 602 pregnant women who had or had not taken adalimumab. Women in the adalimumab-exposed cohort had received at least one dose of the drug in the first trimester for the treatment of rheumatoid arthritis or Crohn's Disease (N = 257). Women in the disease comparison cohort had not used adalimumab in pregnancy (N = 120). Women in the healthy comparison cohort had no rheumatic or inflammatory bowel diseases (N = 225). Women and their infants were followed to one year postpartum with maternal interviews, medical records abstraction, and physical examinations. Study outcomes were major structural birth defects, minor defects, spontaneous abortion, preterm delivery, pre and post-natal growth deficiency, serious or opportunistic infections and malignancies. 42/602 (7.0%) of pregnancies were lost-to-follow-up. 22/221 (10.0%) in the adalimumab-exposed cohort had a live born infant with a major birth defect compared to 8/106 (7.5%) in the diseased unexposed cohort (adjusted odds ratio 1.10, 95% confidence interval [CI] 0.45 to 2.73). Women in the adalimumab-exposed cohort were more likely to deliver preterm compared to the healthy cohort (adjusted hazard ratio [aHR] 2.59, 95% CI 1.22 to 5.50), but not compared to the diseased unexposed cohort (aHR 0.82, 95% CI 0.66 to 7.20). No significant increased risks were noted with adalimumab exposure for any other study outcomes., Conclusions: Adalimumab exposure in pregnancy compared to diseased unexposed pregnancies was not associated with an increased risk for any of the adverse outcomes examined. Abstract

This phase II study evaluated YM155, a novel small-molecule survivin suppressant, in combination with rituximab in patients with relapsed aggressive B-cell non-Hodgkin lymphoma (NHL) who failed or were not candidates for autologous stem cell transplant (ASCT). During 14-day cycles, 41 patients received YM155 (5mg/m(2)/d) by continuous intravenous (IV) infusion for 168 hours (day 1-7), and rituximab (375mg/m(2)) IV on days 1 and 8 during cycles 1-4 and repeated for 4 cycles every 10 cycles. Forty patients (97.6%) had prior rituximab and 15 patients (36.6%) prior ASCT. Most frequent grade 3-4 adverse events were neutropenia (19.5%) and thrombocytopenia (12.2%). In the per-protocol set (n = 34), objective response rate was 50% and median progression-free survival 17.9 months. Median overall survival was not reached at study termination (median follow-up, 23 months). YM155 in combination with rituximab was tolerable with encouraging antitumor activity and durable responses in relapsed aggressive B-cell NHL patients.

41. Cytogenetics findings in a histiocytic sarcoma case.

Abstract

Histiocytic sarcoma (HS) is a neoplasm derived from histiocytes. Its diagnosis was not clear until its immunohistochemistry profile was correctly established. Not much is known about its genetic properties. We report a case of a 48-year-old male patient whose bone marrow was almost completely occupied by monomorphic medium size neoplastic cellularity. Its immunohistochemical profile was CD68(+), CD4(+), CD45(+) with negativity of other dendritic cells, and other lineage markers. Cytogenetic study showed 4 related clones: one with trisomy 8 and extra material on the short arms of chromosome 4; a second line with tetrasomy of chromosome 8, add(4)(p16); the third clone had the same alterations as the previous and deletion of chromosome 3 at q11; the fourth line had tetrasomy 8 and translocation t(3;5)(q25;q35). To our knowledge this is the first HS case showing chromosome 8 trisomy and tetrasomy and the other described alterations.

Abstract

Objective: In preclinical reproductive studies, leflunomide was found to be embryotoxic and teratogenic. Women treated with leflunomide are advised to avoid pregnancy; those who become pregnant are advised to reduce fetal exposure through a cholestyramine drug elimination procedure. The present study was undertaken to investigate pregnancy outcomes in women who received leflunomide and were treated with cholestyramine during pregnancy., Methods: Sixty-four pregnant women with rheumatoid arthritis (RA) who were treated with leflunomide during pregnancy (95.3% of whom received cholestyramine), 108 pregnant women with RA not treated with leflunomide, and 78 healthy pregnant women were enrolled in a prospective cohort study between 1999 and 2009. Information was collected via interview of the mothers, review of medical records, and specialized physical examination of infants., Results: There were no significant differences in the overall rate of major structural defects in the exposed group (3 of 56 live births [5.4%]) relative to either comparison group (each 4.2%)(P = 0.13). The rate was similar to the 3-4% expected in the general population. There was no specific pattern of major or minor anomalies. Infants in both the leflunomide-exposed and non-leflunomide-exposed RA groups were born smaller and earlier relative to infants of healthy mothers; however, after adjustment for confounding factors, there were no significant differences between the leflunomide-exposed and non-leflunomide-exposed RA groups., Conclusion: Although the sample size is small, these data do not support the notion that there is a substantial increased risk of adverse pregnancy outcomes due to leflunomide exposure among women who undergo cholestyramine elimination procedure early in pregnancy. These findings can provide some reassurance to women who inadvertently become pregnant while taking leflunomide and undergo the washout procedure.

Abstract

We evaluated the role of high-dose granulocyte colony stimulating factor (G-CSF) in vitro, in inducing the generation of high-proliferative potential colony forming cells (HPP-CFC), from either mononuclear cells or purified CD34+ cells. Both normal controls and patients undergoing peripheral blood stem cell (PBSC) mobilization and transplantation were studied. In serum-driven agar cultures, G-CSF stimulated the proliferation of HPP-CFC in a dose dependent manner (r = 0.92). The number of HPP-CFC was four-fold greater in mobilized patients than in normal controls. Purified CD34+ cells yielded 11-fold more colonies than mononuclear cells. HPP-CFC from mobilized patients showed replating capacity, giving rise to secondary colonies of more mature appearance. In serum-free cultures, the effect of G-CSF appeared to be mediated by synergistic interaction with stem cell factor. Our results suggest that G-CSF stimulates primitive hematopoietic cells that are detectable in increased amounts in patients receiving mobilization therapy. Therefore, determination of G-CSF induced HPP-CFC could be a useful tool in the evaluation of mobilization strategies.
