Background Acute kidney injury is a common complication in solid organ transplants, notably liver transplantation. The MELD is a score validated to predict mortality of cirrhotic patients, which is also used for organ allocation, however the influence of this allocation criteria on AKI incidence and mortality after liver transplantation is still uncertain. Methods This is a retrospective single center study of a cohort of patients submitted to liver transplant in a tertiary Brazilian hospital: Jan/2002 to Dec/2013, divided in two groups, before and after MELD implementation (pre-MELD and post MELD). We evaluate the differences in AKI based on KDIGO stages and mortality rates between the two groups. Results Eight hundred seventy-four patients were included, 408 in pre-MELD and 466 in the post MELD era. The proportion of patients that developed AKI was lower in the post MELD era (p 0.04), although renal replacement therapy requirement was more frequent in this group (p p p Conclusion Liver transplants performed in the post MELD era had a lower incidence of AKI, although there were more cases requiring dialysis. 1-year mortality was lower in the post MELD era, suggesting that patient care was improved during this period.

INTRODUÇÃO: Simulação e discussão de caso são métodos de ensino efetivos com alta satisfação dos alunos. Nossa hipótese foi que o mesmo caso apresentado com ambos métodos traria ganho de conhecimento imediato similar em pós-teste, porém maior retenção de conhecimento em teste de retenção e maior satisfação com método de ensino com uso de simulação. MÉTODOS: Estudo prospectivo, não randomizado e controlado, usando um desenho cruzado para método de ensino, testes com questões de múltipla escolha e uma pesquisa de satisfação. Alunos de medicina do último ano fizeram o pré-teste e foram alocados em dois grupos. Grupo Enfermaria participou em simulação de anafilaxia (SIM-ANA) e discussão de caso de taquicardia supraventricular (DC-TSV). Grupo PS fez o oposto (SIM-TSV e DCANA). Alunos foram testados para cada tema no fim do seu estágio (pós-teste) e 4 a 6 meses após (teste de retenção). RESULTADOS: A maioria dos alunos (108- 66,3%) completaram todos testes. Nota média para anafilaxia do préteste foi 43,6% SIM-ANA e 46,6% DC-ANA; pós-teste 63,5% SIM-ANA e 67,8% CD-ANA e teste de retenção 61,5% SIM-ANA e 65,5% DC-ANA. Nota média do pré-teste para taquicardia supraventricular foi 33,9% SIM-TSV e 31,6% DCTSV; pós-teste 42,5% SIM-TSV e 47,7% DC-TSV e teste de retenção 41,5% SIM-TSV e 39,5% DC-TSV. Houve melhora significante entre pré e pós-testes (p < 0,05) e não houve diferença entre pós-teste e teste de retenção (p > 0,05) para ambos os temas. Não houve diferença estatisticamente significante entre simulação e discussão de caso para qualquer teste (todos com p > 0,05). Pesquisa de satisfação favoreceu simulação comparado com discussão de caso em seis das oito afirmativas pesquisadas (p < 0,001). CONCLUSÃO: Como intervenção única, simulação não apresenta diferença significante em relação a discussão de caso para aquisição e retenção de conhecimento. A maior satisfação dos alunos com método de ensino mostra um benefício mensurável da simulação em comparação com a discussão de caso INTRODUCTION: Simulation and case-based discussion are effective learning methods with high student satisfaction. Our hypothesis was that the same case presented by both methods would yield similar immediate knowledge gain in post-test, but higher retention of knowledge test and student satisfaction with method in simulation. METHODS: This was a prospective, non-randomized, controlled study using a crossover design for method, multiple choice questionnaires tests and a satisfaction survey. Final year medical students were pre-tested and allocated into two groups. Group Enfermaria participated in anaphylaxis simulation (SIM-ANA) and supraventricular tachycardia case-based discussion (CD-SVT); group PS did the opposite (SIM-TSV and CD-ANA). Students were tested for each theme at the end of their rotation (post-test) and 4-6 months later (retention). RESULTS: Most students (108 - 66.3%) completed all tests. Mean anaphylaxis pre-test score was 43.6% SIM-ANA and 46.6% CDANA; post-test 63.5% SIM-ANA and 67.8% CD-ANA and retention test 61.5% SIM-ANA and 65.5% CD-ANA. Mean supraventricular tachycardia pre-test score was 33.9% SIM-SVT and 31.6% CD-SVT; post-test 42.5% SIM-SVT and 47.7% CD-SVT and retention test 41.5% SIM-SVT and 39.5% CD-SVT. There was significant improvement between pre-tests and post-tests (p < 0.05) and no difference between post-tests and retention tests (p > 0.05) for both themes. No statistically significant differences between simulation and case discussion were found at any tests (all with p > 0.05). Satisfaction survey favored simulation compared with case discussion in six of the eight statements asked (p < 0.001). CONCLUSION: As a single teaching intervention, simulation does not significantly differ from case-based discussion for acquisition and retention of knowledge. Higher student satisfaction shows a measurable benefit of simulation compared to case-based discussion

Introdução: É provável que o tratamento precoce da sepse grave e do choque séptico possa melhorar o desfecho dos pacientes. Objetivo: O objetivo deste estudo foi avaliar como o atraso no início da ressuscitação da sepse influencia a gravidade da doença, a intensidade das medidas de ressuscitação necessárias para atingir estabilidade hemodinâmica, o desenvolvimento da disfunção orgânica e a função mitocondrial. Métodos: Estudo experimental, prospectivo, randomizado e controlado, realizado em um laboratório experimental de um hospital universitário. Trinta e dois porcos submetidos à anestesia geral e ventilados mecanicamente foram randomizados (8 animais por grupo) em um grupo controle sadio ou para um de três grupos em que induziu-se peritonite fecal (instilação peritoneal de 2,0 g/kg de fezes autólogas) e, após 6 (deltaT-6h), 12 (deltaT-12h) ou 24 (deltaT-24h) horas, iniciou-se um período de 48 horas de ressuscitação protocolada. Resultados: O retardo no início da ressuscitação da sepse foi associada a sinais progressivos de hipovolemia e ao aumento dos níveis plasmáticos de interleucina-6 e do fator de necrose tumoral alfa. O atraso no início do tratamento da sepse resultou em balanço hídrico progressivamente positivo (2,1 ± 0,5 mL/kg/h, 2,8 ± 0,7 mL/kg/h e 3,2 ± 1,5 mL/kg/h, respectivamente, para os grupos deltaT-6h, deltaT-12h, e deltaT-24h, p < 0,01), maior necessidade de administração de noradrenalina durante as 48 horas de ressuscitação (0,02 ± 0,04 mcg/kg/min, 0,06 ± 0,09 mcg/kg/min e 0,13 ± 0,15 mcg/kg/min, p=0,059), redução da capacidade máxima de respiração mitocondrial cerebral dependente do Complexo II (p=0,048) e tendência a aumento da mortalidade (p=0,08). Houve redução do trifosfato de adenosina (ATP) na musculatura esquelética em todos os grupos estudados (p < 0,01), com os valores mais baixos nos grupos deltaT-12h e deltaT-24h. Conclusões: O aumento do tempo entre o início da sepse e o início das manobras de ressuscitação resultou no aumento da gravidade da doença, na maior intensidade das manobras de ressuscitação e na disfunção mitocondrial cerebral associada à sepse. Nossos resultados suportam o conceito da existência de uma janela crítica de oportunidade para ressuscitação da sepse Introduction: Early treatment in sepsis may improve outcome. Objective: The aim of this study was to evaluate the impact of delays in resuscitation on disease severity, need for resuscitation, and the development of sepsis-associated organ and mitochondrial dysfunction. Methods: Prospective, randomized, controlled experimental study performed at an experimental laboratory in a university hospital. Thirty-two anesthetized and mechanically ventilated pig were randomly assigned (n = 8 per group) to a nonseptic control group or one of three groups in which fecal peritonitis (peritoneal instillation of 2 g/kg autologous feces) was induced, and a 48 hour period of protocolized resuscitation started 6 (deltaT-6 hrs), 12 (deltaT-12 hrs), or 24 (deltaT-24 hrs) hours later. Results: Any delay in starting resuscitation was associated with progressive signs of hypovolemia and increased plasma levels of interleukin-6 and tumor necrosis factor-alfa prior to resuscitation. Delaying resuscitation increased cumulative net fluid balances (2.1 ± 0.5 mL/kg/hr, 2.8 ± 0.7 mL/kg/ hr, and 3.2 ± 1.5 mL/kg/hr, respectively, for groups deltaT-6 h rs, delta T-12 hrs, and ?T-24 hrs; p < 0.01) and norepinephrine requirements during the 48-hr resuscitation protocol (0.02 ± 0.04 mcg/kg/min, 0.06 ± 0.09 mcg /kg/min, and 0.13 ± 0.15 mcg/kg/min; p=0.059), decreased maximal brain mitochondrial Complex II respiration (p=0.048), and tended to increase mortality (p=0.08). Muscle tissue adenosine triphosphate decreased in all groups (p < 0.01), with lowest values at the end in groups deltaT-12 hrs and deltaT-24 hrs. Conclusions: Increasing the delay between sepsis initiation and resuscitation increases disease severity, need for resuscitation, and sepsis-associated brain mitochondrial dysfunction. Our results support the concept of a critical window of opportunity in sepsis resuscitation

WENSING, L. A. Regulation of TBX3 gene by TGF-β1 in mesangial cells. 2012. 91 p. Ph. D. thesis (Human Physiology) Instituto de Ciencias Biomedicas, Universidade de Sao Paulo, Sao Paulo, 2013. Mesangial cells (MC) synthesize extracellular matrix (ECM), providing structural support to the glomerular capillaries, and participate in the regulation of the glomerular filtration rate. During the progression of nephropathies, MC assume a myofibroblastic phenotype, characterized by changes in proliferation, apoptosis and ECM production, which reflect modifications in their gene expression pattern. Using DNA microarrays, we identified TBX3 as a gene upregulated in cells stimulated with fetal bovine serum (FBS), condition in which MC display a phenotype similar to that observed in nephropathies. TBX3 is a transcriptional repressor containing a DNA binding domain (T-box), which binds to the consensus T-site present in the promoter region of target genes. This gene encodes for two isoforms, TBX3.1 and TBX3 + 2α, generated by alternative splicing, in which the exon 2α is inserted into the middle of the sequence corresponding to its T-box domain. In CM, TBX3 was upregulated by low concentrations of TGF-β1 preferentially through a post-transcriptional mechanism. TBX3 isoforms, when overexpressed separately by adenoviral transduction, showed distinct subcellular localizations: TBX3.1 was localized in the nucleus and TBX3 + 2α was detected in the cytoplasm. Overexpression of the isoforms did not affect proliferation or matrix production in MC. However, both forms of the gene decreased MC apoptosis induced by FBS deprivation. TBX3.1, in particular, reduced the level of cell death next to that observed in cells maintained in medium supplemented with FBS. Moreover, TBX3 gene silencing sensitized MC to the proapoptotic stimuli caused by FBS removal, and increase, although modestly, apoptosis rates of CM, even under the pro-survival condition induced by the presence of FBS. Finally, we observed an increase in TBX3 protein expression both in glomerular and tubular regions in a model of diabetic nephropathy (5/6 nephrectomy), temporally related to increased expression of TGF-β1, collagen IV and fibronectin. Our results indicate that TBX3 acts as an antiapoptotic factor in MC in vitro and may be involved in the mechanism by which TGF-β1 induces glomerulosclerosis and tubular fibrosis during the progression of nephropathies.

Um problema importante na área de Biologia Sistêmica é o de inferência de redes de regulação gênica. Os avanços científicos e tecnológicos nos permitem analisar a expressão gênica de milhares de genes simultaneamente. Por \"expressão gênica\'\', estamos nos referindo ao nível de mRNA dentro de uma célula. Devido a esta grande quantidade de dados, métodos matemáticos, estatísticos e computacionais têm sido desenvolvidos com o objetivo de elucidar os mecanismos de regulação gênica presentes nos organismos vivos. Para isso, modelos matemáticos de redes de regulação gênica têm sido propostos, assim como algoritmos para inferir estas redes. Neste trabalho, focamos nestes dois aspectos: modelagem e inferência. Com relação à modelagem, estudamos modelos existentes para o ciclo celular da levedura (Saccharomyces cerevisiae). Após este estudo, propomos um modelo baseado em redes Booleanas probabilísticas sensíveis ao contexto, e em seguida, um aprimoramento deste modelo, utilizando cadeias de Markov não homogêneas. Mostramos os resultados, comparando os nossos modelos com os modelos estudados. Com relação à inferência, propomos um novo algoritmo utilizando o paradigma de crescimento de semente de genes. Neste contexto, uma semente é um pequeno subconjunto de genes de interesse. Nosso algoritmo é baseado em dois passos: passo de crescimento de semente e passo de amostragem. No primeiro passo, o algoritmo adiciona outros genes à esta semente, seguindo algum critério. No segundo, o algoritmo realiza uma amostragem de redes, definindo como saída um conjunto de redes potencialmente interessantes. Aplicamos o algoritmo em dados artificiais e dados biológicos de células HeLa, mostrando resultados satisfatórios. A key problem in Systems Biology is the inference of gene regulatory networks. The scientific and technological advancement allow us to analyze the gene expression of thousands of genes, simultaneously. By \"gene expression\'\' we refer to the mRNA concentration level inside a cell. Due to this large amount of data, mathematical, statistical and computational methods have been developed in order to elucidate the gene regulatory mechanisms that take part of every living organism. To this end, mathematical models of gene regulatory networks have been proposed, along with algorithms to infer these networks. In this work, we focus in two aspects: modeling and inference. Regarding the modeling, we studied existing models for the yeast (Saccharomyces cerevisiae) cell cycle. After that, we proposed a model based on context sensitive probabilistic Boolean networks, and then, an improvement of this model, using nonhomogeneous Markov chain. We show the results, comparing our models against the studied models. Regarding the inference, we proposed a new algorithm using the seed growing paradigm. In this context, a seed is a small subset of genes. Our algorithm is based in two main steps: seed growing step and sampling step. In the first step, the algorithm adds genes into the seed, according to some criterion. In the second step, the algorithm performs a sampling process on the space of networks, defining as its output a set of potentially interesting networks. We applied the algorithm on artificial and biological HeLa cells data, showing satisfactory results.
