A SARS-CoV-2 megjelenésével a myeloma multiplexben szenvedők leggyakoribb halálozási okának számító fertőző ágensek köre tovább bővült. Az absztrakt szövegezésekor világszerte domináló SARS-CoV-2 omikron variáns (PANGO B.1.1.529) bár kisebb valószínűséggel okoz fatális kimenetelű fertőzést immunkompetensekben a korábbi delta variánshoz (PANGO B.1.617.2) képest, hatékonyabb transzmissziós képessége miatt az átfertőzöttek incidenciája valószínűleg nem alacsony. A COVID–19 súlyos vagy kritikus lefolyásának valószínűségét növeli a myeloma multiplexben szenvedő betegek esetén az alapbetegség, annak célzott hematológiai kezelése, valamint a betegséghez társuló egyéb komorbiditások (például veseelégtelenség) okozta komplex humorális és celluláris immunszuppresszió. A minél korábban megkezdett antivirális terápiák, a pre- vagy posztexpozíciós profilaxisként alkalmazott monoklonális antitestkészítmények, valamint a rekonvaleszcensplazma-terápia megakadályozhatja a fertőzés klinikai progresszióját. Míg az átlagpopulációban a COVID–19-et kísérő, közösségben szerzett koinfekciók incidenciája nem magas, myeloma multiplex esetén a légúti vírusbetegségeket követő Streptococcus pneumoniae fertőzés kb. 150-szer nagyobb eséllyel okozhat invazív betegséget. A modern onkohematológiai kezelések hatására a myeloma multiplex mára krónikus, többszöri relapsussal kísért entitássá vált, az ebben szenvedőket a fenti két kórokozóval szemben immunizálni szükséges. Cikkünkben citokinviharral komplikált COVID–19-ben, valamint invazív Streptococcus pneumoniae betegségben szenvedő, majd kórházi ellátása során de novo myeloma multiplexszel diagnosztizált felnőtt beteg esetét ismertetjük, végül röviden áttekintjük az ezzel kapcsolatos legfontosabb irodalmi adatokat. OBJECTIVE: Deaths due to suicide, substance use and violence/accident may reflect similar risk factors and overlap in their classification. This study aimed to investigate incidence and risk factors of mortality among people with HIV (PWH) due to these three related causes.DESIGN: Prospectively collected data from PWH at least 18 years old and under active follow-up in the EuroSIDA study from 2007 to 2019 were analysed.METHODS: Cause-specific Cox regression analysis was used to assess risk factors.RESULTS: A total of 17 881 participants were included, comprising 149 327 person-years of follow-up (PYFU). Forty participants died by suicide {incidence rate [IR] [95% confidence interval (CI)]: 0.3/1000 PYFU (0.2, 0.4)} 93 from substance use [IR (95% CI): 0.6/1000 PYFU (0.5, 0.8)], and 57 by violence/accident [IR (95% CI): 0.4/1000 PYFU (0.3, 0.5)]. An AIDS diagnosis within the last 12 months was associated with nine-fold increased risk of suicide vs. no history of AIDS [adjusted hazard ratio (aHR): 9.06; 95% CI: 2.07, 39.7]. Male gender was associated with double the risk of violent/accidental death (aHR: 2.28; 95% CI: 1.09, 4.78). PWH in Eastern Europe and those who acquired HIV by injection drug use (IDU) demonstrated a greater risk of death due to substance use or violence/accident.CONCLUSIONS: The association between a recent diagnosis of AIDS and suicide highlights a critical period for intervention. HIV infection acquired through IDU demonstrated an expected relationship with death due to substance use and violent/accidental deaths. Increased risk of death due to substance use and violence/accident in Eastern Europe demands investigation into specific differences that may drive that association.

Emerging evidence suggests that remdesivir might improve clinical outcome of high-risk outpatients with coronavirus disease 2019 (COVID-19). Our aim was to evaluate characteristics and outcomes of nonhospitalised adults diagnosed with COVID-19 and treated with early remdesivir therapy during the omicron wave. A single-centre prospective cohort study was performed among adult patients between February and June 2022, during the circulation of phylogenetic assignment of named global outbreak (PANGO) subvariants BA.2, BA.4, and BA.5 in Hungary. Patients were enrolled based on pre-defined criteria. Clinical characteristics (demography, comorbidities, vaccination status, imaging, treatment, and disease course) and outcomes (COVID-19 related hospitalisation, oxygen supplementation, intensive care support, and all-cause death) were assessed at 28 days post-treatment. A subgroup analysis of patients with and without active haematological malignancies was also carried out. Altogether, 127 patients were enrolled: 51.2% (65/127) were female with a median age of 59 (IQR: 22, range: 21‒92) years, and 48.8% (62/127) had active haematological malignancy. At 28 days post-treatment, 7.1% (9/127) of patients required COVID-19-related hospitalisation, 2.4% (3/127) required oxygen supplementation, 1.6% (2/127) required intensive care, and 0.8% (1/127) died due to a non-COVID-19-related secondary infection at the intensive care unit, all with haematological malignancies. Early remdesivir treatment might be a feasible strategy among high-risk outpatients with COVID-19 during the omicron wave.

From March through December 2020, 100 autopsies were performed (Semmelweis University, Budapest, Hungary), with chart review, of patients with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection demonstrated by real-time reverse-transcription polymerase chain reaction testing (mean age, 74.73 years, range 40–102 years; 50 males, mean age 71.96 years, and 50 females, mean age 77.5 years). Classified by the date of death, 21 cases were from the pandemic’s “first wave” (March through July) and 79 from the “second wave” (August through December). Three mortality categories were defined by relevance of SARS-CoV-2 infection: (1) “strong” association (n=57), in which COVID-19 was primary responsible for death; (2) “contributive” association (n=27), in which a pre-existing condition independent of COVID-19 was primary responsible for death, albeit with substantial COVID-19 co-morbidity; (3) “weak” association (n=16), in which COVID-19 was minimally or not at all responsible for death. Distributions among categories differed between the first wave, in which the “contributive” association cases dominated (strong: 24%, contributive: 48%, weak: 28%), and the second wave, in which the “strong” association cases dominated (strong: 66%, contributive: 21%, weak: 13%). Charted co-morbidities included hypertension (85 %), cardiovascular diseases (71 %), diabetes (40 %), cerebrovascular diseases (31 %), chronic respiratory diseases (30 %), malignant tumors (20 %), renal diseases (19 %), diseases of the central nervous system (15 %), and liver diseases (6 %). Data suggests that favipiravir (FVP) could be used against SARS-CoV-2. Our aim was to investigate the role of FVP in COVID-19 treatment. A prospective sequential cohort study was performed among adults hospitalized at our center between March and August 2020 with moderate-to-severe, PCR-confirmed COVID-19. For diagnosis and severity, ECDC and WHO definitions were utilized. Patients were screened for inclusion by a priori criteria and included in the FVP cohort if standard-of-care (SOC) + FVP or the non-FVP cohort if SOC ± other antivirals without FVP were administered for > 48 h from diagnosis. Treatment allocation was done per national guidelines, based on severity and drug availability. Primary endpoint was disease progression, a composite of 14-day all-cause death, need for mechanical ventilation, or immunomodulatory therapy. The impact of FVP exposure on disease progression was analyzed by binomial logistic regression. In all, 150 patients were included, 75 in each cohort. Incidence of HIV-infection is increasing in Hungary, due to antiretroviral therapy AIDS related mortality remained low. Delayed diagnosis of HIV infection remains a challenge, co-infection of HIV and other STIs is common. Authors present here the prevalence of HIV-gonorhoea, HIV-syphilis, HIV-neurosyphilis and HIV-lymphogranuloma venereum coinfections in the Hungarian National STD Center among 01.01.2015-31.12.2020.

Since the establishment of highly active antiretroviral therapy, survival rates have improved among patients with human immunodeficiency virus infection giving them the possibility to become transplant candidates. Recent publications revealed that human immunodeficiency virus‐positive heart transplant recipients' survival is similar to non‐infected patients. We present the case of a 40‐year‐old human immunodeficiency virus infected patient, who was hospitalized due to severely decreased left ventricular function with a possible aetiology of acute myocarditis, that has later been confirmed by histological investigation of myocardial biopsy. Due to rapid progression to refractory cardiogenic shock, extracorporeal membrane oxygenation implantation had been initiated, which was upgraded to biventricular assist device later. On the 35th day of upgraded support, the patient underwent heart transplantation uneventfully. Our clinical experience confirms that implementation of temporary mechanical circulatory support and subsequent cardiac transplantation might be successful in human immunodeficiency virus‐positive patients even in case of new onset, irreversible acute heart failure.

A Fenntartható Fejlődési Célokat 2015-ben jelölte ki az ENSZ a szegénység leküzdésére, az egyenlőtlenségek csökkentésére és a környezeti fenntarthatóság megvalósítására. Mindezt úgy, hogy közben senki nem hagynak hátra és mindenki egyenlően részesül az elért eredményekből. Jelen tanulmányban az SDG7 és SDG13 kerül részletesen bemutatásra, melyek között rendkívül sok átfedés található. Az SDG7 a megfizethető és tiszta energia elérését, az SDG13 pedig a klímaváltozás elleni fellépést hangsúlyozza. A feltörekvő európai gazdaságokat vizsgáljuk 2008- 2020 közötti időtartamban, számításaink szerint a fenntartható fejlődési részcélok tekintetében konvergencia és divergencia egyaránt azonosítható. Látható eredmények és összetartás ott figyelhető meg, ahol az adott részcél egyben kiemelt Európai Uniós célkitűzés is az Európa 2020 alapján.

Absztrakt: Bevezetés: Jelenleg nem áll rendelkezésünkre specifikus kuratív kezelés vagy vakcina az új koronavírus-2 (COVID–19, SARS-CoV-2) kezelésére. A súlyos citokinvihar-szindrómával társuló COVID–19-fertőzés kezelésére sürgősen szükséges alternatív kezelési megoldást találni. Célkitűzés: A convalescens plazmaterápia alkalmazása más, súlyos vírusfertőzések kezelésében a túlélés javulását eredményezte, és ígéretes terápiás megközelítés lehet súlyos COVID–19-betegek számára. Módszer: Két kritikus állapotú, COVID–19-fertőzött beteg convalescens friss fagyasztott plazmával történő kezelését mutatjuk be. Eredmények: A plazmaterápia megkezdésekor mindkét beteg gépi lélegeztetésre szorult, és antivirális kezelésben, valamint maximális szupportív ellátásban részesült. A COVID–19-fertőzésből nemrég felépült és a súlyos akut légúti distressz szindrómát okozó koronavírus-2 elleni immunglobulin-G-t termelő donorokból 3 × 200 ml convalescens plazma transzfúzióját alkalmaztuk. A convalescens plazma transzfúzióját követően mindkét betegnél javult az oxigénszaturáció, és a gyulladásos értékeket jelző paraméterek csökkenést mutattak. A convalescens plazma adását megelőző paraméterekhez viszonyítva növekedett a lymphocytaszám, és csökkent az interleukin-6-szint. Mindkét betegnél 2 héten belül megszüntethető volt a gépi lélegeztetés. Nem észleltünk a convalescens plazma adása során súlyos mellékhatást. Következtetések: Megfigyelésünk alapján a convalescens plazmával történő terápia jól tolerálható volt, és javíthatja a betegek gyógyulási arányát. A convalescens plazmával való kezelés optimális dózisának, időpontjának és klinikai előnyeinek meghatározására további nagy, kontrollált vizsgálatok szükségesek. Közleményünk megírásának célja az első két, hazánkban convalescens plazmával sikeresen kezelt, kritikus állapotú COVID–19-fertőzött beteg kórtörténetének bemutatása. Absztrakt: A COVID–19 a SARS-CoV-2 vírus által okozott, járványosan terjedő, légúti kiindulású betegség. A kórokozó magas patogenitású, zoonotikus eredetű humán coronavírus, mely hatékonyan terjed emberről emberre cseppfertőzéssel és közeli kontaktussal. A vírusdiagnosztika a légutakból vett minta PCR-vizsgálatán alapul, melynek ismétlésére szükség lehet a fertőzés kizárására. A PCR-eredményt a klinikummal egybe kell vetni, mivel a preszimptomatikus beteg már vírust üríthet, a gyógyultak PCR-pozitivitása pedig hetekig elhúzódik. A terápiás stratégiák két ágát az antivirális gyógyszerek, valamint a hiperinflammációt gátló immunmodulánsok adják. Jelen összefoglalásunk a második azon két társközlemény közül, melyek célja a 2020. május 25-ig elérhető legfőbb nemzetközi és hazai betegséggel kapcsolatos eredmények ismertetése, elsősorban, de nem kizárólag hematológus kollégák számára.

Absztrakt: A COVID–19 a SARS-CoV-2 vírus által okozott, járványosan terjedő, légúti kiindulású betegség. A kórokozó magas patogenitású, zoonotikus eredetű humán coronavírus, mely hatékonyan terjed emberről emberre cseppfertőzéssel és közeli kontaktussal. A medián lappangási idő 5 nap, a maximum 14 nap. A COVID–19 kardinális tünetei a láz, dyspnoe és száraz köhögés. Az esetek 80%-a enyhe lefolyású, 5%-ban azonban intenzív terápiás ellátás és gépi lélegeztetés válik szükségessé. A COVID–19 akár súlyos, az egész szervezetre kiterjedő citokinvihart is kiválthat, mely gyors beavatkozás nélkül végzetes kimenetelű lehet. Jelen összefoglaló – melyet a diagnosztikai és terápiás szempontokat tárgyaló társközlemény követ – a betegséggel kapcsolatban 2020. május 25-ig elérhető legfőbb nemzetközi és hazai irodalmi eredményeket ismerteti; elsősorban, de nem kizárólag hematológus kollégák számára.

Absztrakt: Az elmúlt hónapokban pandémiához vezető, Kínából induló, új koronavírus okozta megbetegedés (COVID–19) rámutatott az emberiség kiszolgáltatottságára. Aktuálisan a COVID–19 kezelésére randomizált kontrollált vizsgálattal igazolt hatékonyságú, törzskönyvezett gyógyszer nem áll rendelkezésre. A súlyos akut légúti tünetegyüttest okozó koronavírus-2 (SARS-CoV-2) a jelen munka írásáig – a legóvatosabb becslések szerint is – közel 4 millió fertőzésért és megközelítőleg 300 000 halálesetért felelős. A patomechanizmus mélyebb megismerése fedte fel, hogy a fatális esetek hátterében az immunválasz diszregulációja, a citokinvihar-szindróma (CRS) áll. Eddigi ismereteink alapján a COVID–19-ben kialakuló citokinvihar sarkalatos szereplője az interleukin-6 (IL6). Esetleírások és kis betegcsoportokon végzett vizsgálatok eredményei alapján az IL6 monoklonális antitest gátló terápia (tocilizumab) eredményesnek mutatkozik a citokinvihar kezelésében. A szerzők egy eset leírása kapcsán foglalják össze a legfrissebb ismereteket a COVID–19 terápiájával kapcsolatosan. A jelen esetismertetés súlyos COVID–19-ben szenvedő betegben kialakult CRS sikeres terápiáját mutatja be, melynek során – a szerzők tudomása szerint – hazánkban elsőként került sor tocilizumab alkalmazására infektológiai osztályon. A COVID–19 kapcsán tapasztalható jelenség a szérummájenzim-aktivitásnak a laboratóriumi vizsgálat során észlelt emelkedése. Ezen változások hátterében feltehetően három patomechanizmus áll: 1) a SARS-CoV-2 direkt citotoxikus és indirekt sejtszintű és szisztémás következményei, 2) a COVID–19-ben alkalmazott gyógyszeres kezeléseknek (remdesivir, favipiravir, tocilizumab, baricitinib, szisztémás kortikoszteroid) a farmakológiai hatásai, illetve 3) a betegekben korábban fennálló krónikus májbetegség(ek) progressziója. A májenzim-emelkedés kiváltó okának differenciáldiagnosztikája és a terápiával kapcsolatos döntéshozatal kihívást jelenthet a klinikus számára, mivel a nem megfelelő kezelés a COVID–19 vagy a májkárosodás progresszióját okozhatja. A jelen összefoglaló a COVID–19-betegeket ellátó klinikusok számára kíván alapvető támpontot nyújtani a helyes döntéshez. Orv Hetil. 2022; 163(36): 1415–1421.

Bevezetés és célkitűzés: Az Európai Unióban a hepatitis C-vírussal (HCV) fertőzött betegek kétharmada nem tud a fertőzöttségéről. A WHO célja 2030-ra az új krónikus hepatitises esetek számának 90%-os csökkentése. A börtönökben a HCV-fertőzöttek aránya a lakosságénak a tízszeresét is elérheti. A cikk a magyar büntetés-végrehajtási intézetekben 2007 és 2017 között zajlott HCV-szűrő vizsgálat eredményeinek összefoglalása. Módszer: Az önkéntes alapú anti-HCV-szűrő vizsgálatot követően a pozitív eseteknél HCV-PCR- és -genotípus-meghatározás történt. A betegek írásos beleegyezése után kezdődött el az interferonalapú kezelés, mely a hepatológusok irányításával és a büntetés-végrehajtási intézetek egészségügyi személyzetének együttműködésével történt. Eredmények: A magyar börtönök 84%-ában folyik HCV-szűrő program és -kezelés. Összesen 25 384 betegnél történt meg ezen időszak alatt az anti-HCV-szűrő vizsgálat. A betegek 6,6%-ában anti-HCV-pozitivitást és 3,8%-ában HCV-PCR-pozitivitást találtunk. A HCV-PCR-pozitív betegek 55,2%-ában kezdődött el a kezelés. Teljes időtartamú volt a kezelés 143 betegnél, idő előtt fejeződött be 162 betegnél (42,6%), míg 75 betegnél nem ismert a kezelés időtartama. A rendelkezésre álló, a kezelés vége utáni 24. heti eredmény alapján a tartós terápiás válasz 88% volt. Megbeszélés: A büntetés-végrehajtási intézetekben folyó HCV-szűrő és -kezelési program egy jól működő rendszer. A sikeres kezelésben nagy szerepe van a betegek felvilágosításának, a hepatológus kollégák és a börtönök egészségügyi dolgozói közötti együttműködésnek. Következtetés: Eredményeink azt mutatják, hogy a „test and treat” gyakorlat a HCV mikroeliminációjának kivitelezhető módja a börtönökben. Orv Hetil. 2022; 163(22): 871–878.

Összefoglaló. A mély molekuláris remisszióban lévő CML-es betegnél láz, izomfájdalmak alakultak ki, melyet jelentős kétvonalas cytopenia kísért. Alapbetegségének blasztos transzfomációja mellett, az utazási anamnézisre tekintettel a differenciáldiagnosztika trópusi betegségekre is kiterjedt. Dengue-lázat diagnosztizáltunk, melyből a beteg szövődmény nélkül felépült. Summary. A CML patient in deep molecular remission was admitted with fever, muscle pain, followed by pronounced bilineage cytopenia. In addition to possible blastic transformation, tropical diseases were also included in the differential diagnosis, due to travel history. Dengue fever was diagnosed, and the patient recovered without any complications.

Large randomized clinical trials in severe Coronavirus Disease 2019 (COVID-19) patients have proven efficacy of intravenous tocilizumab. Our aim was to describe the laboratory parameters predicting in-hospital mortality of patients with tocilizumab administration in COVID-19 associated cytokine release syndrome (CRS).We evaluated high-dose (8 mg/kg) intravenous tocilizumab administration in severe and critically ill COVID-19 adult patients fulfilling predefined strict CRS criteria. A single-centre, prospective, observational cohort study was carried out among consecutive adult (≥18 years of age) in-patients with COVID-19 between April 1 and December 31, 2020. The primary endpoint was 28-day all-cause mortality. The changes in laboratory parameters from baseline on day 7 and 14 after administration of tocilizumab were analysed.In total, 1801 patients were admitted to our centre during the study period. One hundred and six patients received tocilizumab, and among them 62 (58.5%) required intensive care unit admittance while 25 (23.6%) deceased. At day 7 after tocilizumab administration, inflammatory markers (CRP, IL-6, ferritin) and lactate dehydrogenase (LDH) values were significantly lower among survivors. Subsequently, at day 14, differences of IL-6 and LDH levels has become more pronounced between subgroups. Restoration of absolute lymphocyte count (ALC) by day 7 and 14 was insufficient among patients who died.In our cohort, administration of high-dose tocilizumab for COVID-19 patients with CRS demonstrated clinical and sustained biochemical parameter improvement in 76.4%. In this patient population high and increasing LDH, IL-6, and low ALC levels had a predictive role for mortality.

Widespread introduction of HIV integrase inhibitors into clinical care may result in appearance of drug resistance mutations affecting treatment outcome. The aim of our study was to monitor the resistance patterns of integrase inhibitors beside protease and reverse transcriptase inhibitors in newly diagnosed therapy-naive HIV-positive patients in Hungary between 2017 and 2019.Genotype-based resistance testing of HIV integrase, protease and reverse transcriptase was performed by amplification and Sanger population sequencing from plasma samples. Drug resistance mutations were identified by the algorithm of Stanford HIV Drug Resistance Database.Potentially transmitted, non-polymorphic integrase major mutation was detected in 1 out of 249 samples, while accessory mutations were observed in further 31 patients (12.4%). The overall prevalence of transmitted drug resistance (TDR) mutations related to protease and reverse transcriptase inhibitors was 5.8% (10/173) between the end of 2017 and 2019. Nucleoside reverse transcriptase inhibitor associated resistance mutations were the most frequent indicators of TDR (6/173; 3.5%), followed by resistance mutations associated with protease (3/173; 1.7%) and non-nucleoside reverse transcriptase inhibitors (2/173, 1.2%).The first detection of integrase major mutation and the changing patterns of other resistance mutations in Hungarian untreated HIV-positive population indicate the necessity of continuous molecular surveillance of Hungarian HIV epidemic.

Primary composite end-point was disease progression (14-day all-cause death, need for mechanical ventilation, or immunomodulatory therapy). At present, neither specific curative treatment nor vaccines for novel coronavirus 2019 (COVID-19) are available. There is an urgent need to look for alternative strategies for COVID-19 treatment especially in the case of severe and/or critically ill patients with cytokine release syndrome (CRS).Convalescent plasma proved to increase survival rates in other severe viral infections. Therefore, convalescent plasma could be a promising treatment option for severe COVID-19 patients.In our article, we present the first two critically ill Hungarian patients with COVID-19 infection treated with convalescent fresh frozen plasma.At the time of plasma therapy both patients were on mechanical ventilation and received antiviral agents and a full scale of supportive care. Each patient received 3 × 200 mL of convalescent plasma of recently recovered donors with sufficient novel anti-coronavirus IgG titers. Subsequent to convalescent plasma infusion, oxygenization improved and inflammatory markers decreased in both individuals. As compared to pretransfusion, lymphocyte counts increased and interleukin-6 level lessened. Both patients were weaned from mechanical ventilation within 2 weeks of treatment. No severe adverse effects were observed.Our experience indicates that convalescent plasma therapy is well tolerated and could potentially improve clinical outcomes. Optimal dose and timing as well as precise assessment of clinical benefit of convalescent plasma therapy will need further investigation in larger, well-controlled trials. During the past few months, a pandemic originating from China named new coronavirus disease (COVID-19) has shown how vulnerable the world is. To date, no medication supported by randomized clinical trials has been approved for the treatment of COVID-19. At the time of writing of this paper, severe acute respiratory syndrome caused by coronavirus-2 (SARS-CoV-2) has been responsible - according to modest estimations - for around 4 million of infections and 300 000 deaths. Unveiling details of patomechanism, in fatal cases the role of immune dysregulation, namely cytokine release syndrome (CRS) has been discovered. Based on the current knowledge, interleukin-6 (IL6) plays a pivotal role in COVID-19 associated CRS. Case reports and result of small case series suggest efficacy of an IL6 inhibitor monoclonal antibody (tocilizumab) in treating CRS. Authors describe a case and review recent knowledge on the treatment of COVID-19. To our knowledge, the first case of severe COVID-19-associated cytokine storm syndrome - treated succesfully with IL6 monoclocal antibody at a Hungarian department of infectology - is presented here. A rendszerváltást követően a visegrádi négyek gazdaságának energiaintenzitása csökkenő tendenciát mutat. Hosszú távon ezen országok gazdaságai az energia csökkenő vagy csökkenő növekedési ütemű felhasználása mellett fejlődhetnek, ami hozzájárul a gazdasági és környezeti fenntarthatósághoz és közvetve a környezeti terhelés csökkenéséhez. A technológiai fejlődés, a gazdasági struktúra változása lehetővé teszi, hogy a termelés és a lakosság igényeinek a kielégítése kevésbé energiaintenzív módon történjen. Tanulmányunkban kétféle szétválási tényező számításával vizsgáljuk a visegrádi négyekben (Lengyelország, Csehország, Szlovákia és Magyarország) a végső energiafelhasználás és a gazdasági növekedés (erőforrás-)szétválását 1990 és 2015 között. Következtetéseket vonunk le a hatásszétválásról, felhasználva a vizsgált országok ökológiai lábnyomát és biokapacitását. Az elemzésben kiemelt figyelmet szentelünk azon időszakoknak, melyek során a gazdaság recesszióba került. Eredményeink szerint a rendszerváltást követő mintegy két és fél évtized során többségben voltak azok az évek, amikor megvalósult az erőforrás-szétválás abszolút vagy relatív formája. Ugyanakkor a 2009–2013-as időszak strukturális törésként értelmezhető, ezekben az években a vizsgált országokban a gazdasági növekedés és az energiafelhasználás szétválása nem valósult meg. Ebben a periódusban a gyenge negatív, az erős negatív és az expanzív negatív irányú szétválásra találunk példát. Elemzésünk alapján kijelenthető, hogy a szétválás nem egy állandósult folyamat, akár másfél évtizednyi szétválást követően is megfordulhat a pozitív tendencia. Az ökológiai lábnyomok elemzéséből arra következtethetünk, hogy az energiatermelést vizsgálva a termelésszétválás mellett érzékelhető hatásszétválás is.

Introduction Data suggests that invasive fungal infections (IFI) might complicate COVID-19. Our goal was to describe characteristics of IFI among critically ill COVID-19 adults. Methods A retrospective observational case-series analysis was done between March–July 2020. Consecutive patients with critical COVID-19 were eligible, and have been included when proven or putative/probable IFI could be confirmed during their course. For COVID-19 diagnosis, ECDC definitions and WHO severity criteria were followed. Candidaemia was diagnosed according to the ESCMID 2012 guideline. Invasive pulmonary aspergillosis (IPA) was defined following EORTC/MSG, ECMM/ISHAM and modified AspICU criteria. Outcome variables were rates of IFIs, in-hospital all-cause mortality, rate and time to negative respiratory SARS-CoV-2 PCR. Results From 90 eligible patients, 20 (22.2%) fulfilled criteria for IFI. Incidence rate for IFI was 2.02 per 100 patient-days at ICU. Patients were mostly elderly males with significant comorbidities, requiring mechanical ventilation because of ARDS. IFI could be classified as candidaemia in 7/20 (40%), putative/probable IPA in 16/20 (80.0%). Isolated species of candidaemia episodes were Candida albicans (4/9, 44.4%), Candida glabrata (3/9, 33.3%), Candida parapsilosis (1/9, 11.1%), Candida metapsilosis (1/9, 11.1%). Mold isolates from lower respiratory tract were Aspergillus fumigatus, BAL galactomannan positivity was prevalent (16/20, 80.0%). Mortality was 12/20 (60.0%) with a median time to death of 31.0±37.0 (5–89) days. Only 9/20 (45.0%) patients reached SARS-CoV-2 PCR negativity after a median time of 20.0±12.0 (3–38) days. Conclusion In this small cohort of critically ill COVID-19 adults, morbidity and mortality related to invasive fungal infections proved to be significant.

Összefoglaló: A szülészeti ellátás terén viszonylag kevés a HIV-pozitív várandósok aránya, a gondozásukban részt vevő szakembereknek azonban ismerniük kell a legújabb kezelési módokat, vizsgálati eredményeket. Jelenleg sem a szülésznő, sem a védőnő nem gondozhat önálló hatáskörön belül HIV-vel élő várandós anyát, hiszen ez az állapot a nagy kockázatú kategóriába tartozik, azonban a várandósgondozást végző szakemberek kompetenciájának folyamatos bővülése miatt elengedhetetlen az aktuális kutatási eredmények megismerése. A kutatások alapján jelentős javulások érhetők el a HIV-pozitív várandósok körében szakszerű ápolással, gondozással: a szülés, a szoptatás, ugyanolyan szép élmény lesz az anya számára, mint az egészséges várandósok esetén. Tanulmányunk célja az volt, hogy eloszlassuk a még meglévő sztereotípiákat a HIV-pozitív várandósok gondozásával, ellátásával kapcsolatban, valamint hogy felhívjuk a figyelmet az egészségügy szakemberek ismereteinek folyamatos fejlesztésére ezen a területen. Summary: In the field of obstetrics, there is a relatively low rate of HIV prevalence among pregnant women, however, we still believe that the professionals involved in their care need to know the latest treatment methods and test results. Currently, midwives, and nurses can not care for mothers with HIV independently, as it belongs to the high risk category but, and we still think that due to the constantly changing competency of professionals in the field of pre-screening, it is essential to get acquainted with current research findings. Based on current research, significant improvements can be found among HIV-positive pregnant women in relation to nursing and nursing care; giving birth will be as beautiful and elevated experience for the mothers as for healthy pregnant women. The basic aim of sustainable spatial development is to develop and achieve a program which is based on the principle of sustainable development with the active collaboration of local governments and the support of the population. In our paper we present the main criteria of sustainable spatial development and we examine the main requirements created by the European Union for spatial planning. In our publication we discuss the presence of aspects of sustainability in the spatial development frameworks of the Heves and Borsod-Abauj-Zemplen Counties. We highlight the main similarities and differences between these two approaches.

Available data on the prevalence of hepatic steatosis in an unselected HIV-infected population are limited. The aim of this study was to determine the prevalence of hepatic steatosis and assess the associated factors in HIV-infected individuals.One hundred and thirty-six HIV-infected individuals were enrolled in this cross-sectional study. Patients underwent transient elastography and controlled attenuation parameter (CAP) measurements. We analyzed the associations between the CAP value and demographic, metabolic, and immunologic parameters. For the first time, in HIV-infected individuals, we used a continuous scale of CAP values to identify significant covariates of hepatic fat accumulation. As a result and compared with other methods, one of the main advantages of CAP was that the quantitative measurement of liver steatosis could be used for analysis.Using univariate analysis, CAP was significantly correlated with the following continuous variables: CD4 percentage (P=0.035), CD8 percentage (P=0.016), age (P0.001), CD4/8 ratio (P=0.002), BMI (P0.001), serum triglyceride (P0.001), and serum cholesterol (P=0.004) levels, the length of known HIV positivity (P0.001), and liver stiffness (P=0.041). With respect to categorical variables, a significant association was found for the presence of diabetes (P=0.006), hypertension (P0.001), facial lipodystrophy (P=0.031), and the use of lopinavir (P=0.042). In multivariate analysis using linear regression, BMI (P0.001), presence of diabetes (P=0.026), and hypertension (P=0.040) were identified as independent significant correlates. Darunavir therapy was associated negatively with the CAP value (P=0.032).Our findings reflect the importance of metabolic factors in hepatic steatosis. The strongest independent covariate was BMI.

Antibodies to solid phase C1q (C1qAb) were determined in 295 serum samples from 132 HIV-infected subjects and in sera from 140 HIV-seronegative healthy individuals as control. An ELISA method applied for the determination of C1qAb in other diseases was used. In part of these sera, other autoantibodies (antibodies reacting with 60-kDa human heat shock protein (hsp60) or mycobacterial hsp65; IgA and IgG class antibodies against the Fab and F(ab′)2 moieties of IgG) as well as complement-mediated antibody-dependent enhancement/neutralization (C′-ADE) were also determined. Increased amount of C1qAb was found in HIV-infected subjects as compared with HIV-seronegative controls (P= 0.0138). In 17 of 132 (13.0%) seropositive individuals but only in 7/140 (5.0%) samples from the controls, the amount of C1qAb exceeded the upper limit (95th percentile) of the normal values (P= 0.031). The amount of C1qAb significantly decreased during a follow-up period of 65 months. C1qAb levels were found to strongly correlate to hsp60/65 autoantibodies but did not correlate or only weakly correlated to the amount of anti-Fab or anti-F(ab′)2autoantibodies measured in the same serum samples. Anti-C1q antibodies recognized the solid phase hsp60/65. Three predicted epitope regions ofM. paratuberculosishsp65 were able to bind efficiently C1q antibodies. An inverse correlation was found between C1qAb and C′-ADE, neutralization was more frequent in the sera with detectable C1qAb, whereas sera without C1qAb more likely enhanced HIV infectionin vitro.
