Naoya Masumori,1 Taiji Tsukamoto,1 Akihiko Shibuya,2 Noriomi Miyao,3 Yasuharu Kunishima,4 Akihiko Iwasawa5 1Department of Urology, Sapporo Medical University School of Medicine, Sapporo, 2Kaguraoka Urological Clinic, Asahikawa, 3Division of Urology, Muroran City General Hospital, Muroran, 4Hokkaido Social Work Association Obihiro Hospital, Obihiro, 5Iwasawa Clinic, Sapporo, Japan Purpose: Our aim was to prospectively analyze the 3-year outcomes of naftopidil treatment for patients with benign prostatic hyperplasia (BPH), including those who dropped out during follow-up and had retreatment for BPH after termination of the drug within 3 years.Patients and methods: Naftopidil, 50 mg/d or 75 mg/d, was given to 117 patients having BPH aged 50 years and older who had international prostate symptom scores (IPSS) ≥8. They were prospectively followed for 3 years with periodic evaluation. If naftopidil was terminated, the reason was determined. For patients with termination, an outcome survey was done to evaluate the status of retreatment for BPH at 3 years.Results: Twenty-five patients (21.4%) continued the same medication for 3 years. The total IPSS, quality of life index, BPH problem index, and maximum flow rate were significantly improved during 3 years. Treatment failure defined as symptomatic progression (an increase in the IPSS of ≥4 points compared to the baseline value), development of acute urinary retention, conversion to other α1-blockers, add-on of a 5α-reductase inhibitor, or conversion to surgery was observed in 41 patients (35.0%). In the univariate analysis, age, prostate volume, and serum prostate-specific antigen were predictors of treatment failure. Of the 50 patients who discontinued naftopidil during the follow-up, only 13 (26%) patients reported that they needed retreatment with α1-blockers and/or surgery within 3 years.Conclusion

Abstract

In urogenital lesions, human papillomavirus （HPV） has been known as the cause of condyloma acuminatum, which has long been considered one of the most commonly diagnosed sex-ually transmitted diseases （STD）. HPV has received much attention clinically because HPV has also been detected at a high rate in genital cancers, such as penile cancer and uterine cervical cancer. Accordingly, since it has been shown that HPV is related to the occurrence of not only condyloma acuminatum but also genital cancers, the author studied HPV in urogenital lesions. 1） HPV has been detected at quite a high incidence even from genital organs without any abnormal findings. In subjects in their twenties, an age of high sexual activity, the HPV-positive rate was 8.0% （9/113） in males and 5.3% （10/189） in females. After several weeks, follow-up studies were conducted on HPV-positive cases. HPV was detected in one of 17 males and two of nine females, and the HPV DNA was the same type as at the first examination. However, after 3-4 months, all three cases had become negative for HPV DNA. It was thus speculated that HPV disaappears spontaneously. A multistep process must be necessary for HPV to attach to epithelial cells and form tumors. 2） The HPV-positive rate of condyloma acuminatum tissues was 95.0% （19/20） in males, while it was 100% （2/2） in females. All of these cases were positive for HPV types 6/11, and two cases were also positive for HPV 16. Similar high rates of HPV detection were recorded for the smear specimens of condyloma acuminatum lesions : HPV types 6/11 were found in 95.8% （23/24） of males and 100% （7/7） of females'. HPV can thus be detected using only smears of lesions. It is notewor-thy that four male cases of condyloma acuminatum underwent malignant changes after a long time, and HPV 16 was detected in the tissues of all of the resultant penile cancers. 3） HPV was detected in 63.1% （70/111） of untreated penile cancers and there was a clear ten-dency for HPV 16 to be the most prevalent type. HPV 18 was detected in two cases （1.8%）, but HPV 33 could not be detected. The HPV DNA in the penile cancer tissues was located in the nuclei of the cancer cells. HPV 16 was also detected in six lymph node metastatic tissues of penile cancer, and the HPV type was the same as the HPV DNA type in the primary cancer. In cases treated with radiation and chemotherapy, the HPV-positive rate was reduced to 16.7% （2/12）. 4） One case of condyloma acuminatum of the urinary bladder was positive for HPV 6/11. It is considered a possibility that HPV entered the urinary bladder by the transurethral route and caused the condyloma acuminatum. HPV was not detected in cases of prostate cancer, benign prostatic hyperplasia, bladder cancer or vulvar cancer.

Abstract

腎癌症例においてIAPとISSを測定し, その腫瘍マーカーとしての意義を検討した.1) IAP, ISSとも各stage別の平均値は正常上限値を上回り, 特にM1群で高値を示した.また両者の実測値は非常によく相関した.2)両者ともstageの進行に伴ってその陽性率は上昇した.3)両者とも臨床経過とある程度相関して変動した.4)有転移症例において, 正常上限値の2倍以上の高値群は2倍以下の群に比して, 予後不良な傾向が両者ともに認められた.以上より, IAP, ISSは腎癌における腫瘍マーカーとして有用である.5) IAPもしくはISSとTPA, ferritinの三者でのcombination assayは臨床的に有意義である, To clarify their usefulness as markers for renal cell carcinoma, serum immunosuppressive acidic protein (IAP) and serum immunosuppressive substance (ISS) were evaluated by TIA (turbidometric immunoassay) for IAP and by SRID (single radial immunodiffusion) for ISS. The mean level of IAP and ISS was beyond each upper normal limit (500, 700 micrograms/ml) in every stage, and especially high in the M1 group. The levels of IAP and ISS were significantly correlated with each other. The determination of IAP and ISS levels after treatment showed a good correlation to the clinical course of the disease. The positive rates of IAP and ISS increased as the stages progressed, respectively. When the influences of pretreatment IAP and ISS level on survival period were investigated, the low IAP or ISS level group (less than two times of the upper normal limit) tended to have a better prognosis than the high level group (more than two times of the upper normal limit) in the M1 patients. These findings suggested that IAP and ISS could be used as markers for monitoring a disease and predicting the prognosis in patients with renal cell carcinoma. As for the positive rate in the combination assay for IAP, TPA and ferritin, or ISS, TPA and ferritin, more than 80% of the patients with low stage renal cell carcinoma had at least one positive marker. This suggested that the combination assay of these three markers was clinically valuable as a disease monitor in patients with renal cell carcinoma.

Abstract

Objectives: To clarify Japanese real-world clinical data on the use of desmopressin 25 and 50 μg orally disintegrating tablets (ODT) for male patients with nocturia and evaluate the predictive factors to improve nighttime frequency., Methods: We retrospectively accumulated real-world clinical data from 27 institutions in Japan. Male patients with two or more episodes of nocturia who received desmopressin ODT for nocturnal polyuria (NP) from 2019 through 2021 were included. The primary endpoint was the change of nighttime frequency until 3 months after desmopressin administration. The secondary endpoints were to clarify the persistence rate, adverse events, and predictive factors of decreasing nighttime frequency., Results: A total of 118 patients were eligible to participate in this study. The persistence rate of desmopressin on the Kaplan-Meier curve at week 12 was 51.3. The reason for discontinuation was mainly the occurrence of adverse events in 67 patients (56.8%), particularly hyponatremia in 7 patients (5.9%). Nighttime frequencies at baseline, - 1 month and 1 - 3 months after desmopressin administration were 4.1 ± 1.3, 2.9 ± 1.4 (P < .01), and 2.6 ± 1.3 (P < .01), respectively. The mean nighttime urine volume voided at baseline was significantly larger in patients whose nighttime frequency decreased by two or more times than in those with a decrease of less than two times., Conclusions: Desmopressin 25 and 50 μg ODT treatments are feasible for male patients with NP in Japanese real-world clinical practice. Patients with higher voided volumes, particularly in the nighttime, may have great benefit from desmopressin., (© 2022 John Wiley & Sons Australia, Ltd.)
