7. Comparison between left and right radial access for coronary angiography

Abstract

In the 1980s, Campeu and Kiemeneij introduced the radial approach for angiography and angioplasty, respectively, in 2017 Kiemeneij described the site of radial distal access to the snuffbox, as the viability and safety of the left approach, either conventional or distal, remains a concern and there are no studies evaluating comfort yet. We randomly assigned 55 patients (9 were excluded), to either left radial access or right radial access (27 pts vs 28 pts). The primary end point was patient´s comfort, contrast volume, distance between first operator and patient, number of angiography catheters, fluoroscopy time. As secondary safety endpoints we include radial spasm and procedure related bleeding. Statistical analysis was done with descriptive statistics, T student for quantitative variables and square chi for qualitative variables. From January 2019 to September 2019 we enrolled 64 patients, (9 were excluded) Stable coronary artery disease was the most common indication for angiography (60% left vs 43% right). Both access were perceived as comfortable (4.29 left vs 4.18 right P=0.549), the amount of contrast volume used was (103 + 85 ml vs 88+55 ml P=0.436, distance between intensifier-researcher (47.6 cm +4.6 right vs 47.7cms +4.8 left, p = 0.941). In safety endpoints the presence of bleeding was 7% vs 7% (P=0.99) and radial artery spasm 26% and 11% respectively (P=0.177). In terms of comfort and safety there is no difference between left and right radial access, both access sites can be done with femoral and radial dedicated catheters in stable coronary disease and acute coronary syndrome coronary.

Abstract

On April 24, 2001, Florida Shelf Lagrangian Experiment (FSLE) cruises were accompanied by PHILLS hyperspectral overflights at 30,000 ft altitude. Robert Steward, and Jim Ivey collected remote sensing reflectance and water samples for absorption during two FSLE experiments. The PHILLS was flown on several transects over the area with vicarious calibration measurements conducted by USF from the R/V LINK. The USF slow-drop package was also deployed to collect inherent and apparent optical properties as a function of depth for an evaluation of effects of vertical structure on remote sensing spectra. The PHILLS imagery has been successfully calibrated. We show the procedures, results, derived bathymetry, and some noise effects with a 3 s period.

18. Interpretation of Observations of Trans-Spectral Phenomena Acquired Using Hyperspectral Sensors Aboard a Remotely Operated Vehicle in Exuma Sound

Abstract

Hyper-spectral (512-channel) optical data acquired during a relatively deep (102m) dive of our ROSEBUD Remotely Operated Vehicle (ROV) in the clear waters of Exuma Sound, Bahamas provided the opportunity to investigate the trans-spectral shift of photonic energy (inelastic scattering) as a function of water depth. Results show a convolution of several spectral processes (e.g. absorption, scattering) involving water molecules, dissolved material and particulates as well as trans-spectral (inelastic) processes involving fluorescence by water molecules (Raman), dissolved material and chlorophyll. The spectral signatures of these convolved causes and effects allow deconvolution with a hyperspectral approach. Intrinsic to the convolution was the ability to position the vehicle at depths where Raman fluorescence dominated at red wavelengths. Results show that the calculated Raman absorption coefficients are generally consistent with historical values (i.e. 0.9 x 10(sup)-4 at 525 nm excitation) and that an angstrom exponent of 5 is more appropriate than the often cited value of 4.

High-frequency irreversible electroporation (H-FIRE) is a technique that uses pulsed electric fields that have been shown to ablate malignant cells. In order to evaluate the clinical potential of H-FIRE to treat glioblastoma (GBM), a primary brain tumor, we have studied the effects of high-frequency waveforms on therapy-resistant glioma stem-like cell (GSC) populations. We demonstrate that patient-derived GSCs are more susceptible to H-FIRE damage than primary normal astrocytes. This selectivity presents an opportunity for a degree of malignant cell targeting as bulk tumor cells and tumor stem cells are seen to exhibit similar lethal electric field thresholds, significantly lower than that of healthy astrocytes. However, neural stem cell (NSC) populations also exhibit a similar sensitivity to these pulses. This observation may suggest that different considerations be taken when applying these therapies in younger versus older patients, where the importance of preserving NSC populations may impose different restrictions on use.We also demonstrate variability in threshold among the three patient-derived GSC lines studied, suggesting the need for personalized cell-specific characterization in the development of potential clinical procedures. Future work may provide further useful insights regarding this patient-dependent variability observed that could inform targeted and personalized treatment.

29. Iatrogenic Radial Artery Injuries: Variable Injury Patterns, Treatment Times, and Outcomes.

Abstract

Background: The radial artery is commonly accessed for arterial blood sampling, invasive blood pressure monitoring, and vascular access for cardiac catheterization. Iatrogenic radial artery injury is a rare complication with potentially devastating outcomes. The purpose of our study was to identify the timing of these injuries and define a treatment algorithm. Methods: A retrospective chart review of all patients with iatrogenic radial artery injuries were identified between the years 2008 and 2018. Patient demographics, mechanism of injury, interventions, and outcomes were recorded. Results: A total of 18 patients were identified with iatrogenic radial artery injury over a 10-year period. Fifty percent of these resulted from arterial line cannulation, and 50% occurred after transradial cardiac catheterization. Thirty-three percent resulted in radial artery pseudoaneurysm (RAP), and 66% had acute radial artery thrombosis (RAT). Eleven of the 18 patients underwent operative intervention. Of the 12 patients with RAT, 4 were treated with systemic anticoagulation for 3 months. All patients with RAP who were surgically treated had resolution of symptoms on follow-up evaluation. Of the patients with RAT, 2 had persistent sensorimotor deficits after treatment, and 1 patient had multiple necrotic fingers requiring amputation. Conclusion: Radial artery injuries are an uncommon but potentially devastating complication of common invasive procedures resulting in thrombosis, pseudoaneurysm, or overt hand ischemia. 46. Fisheries imaging radar surveillance test /FIRST/ - Bering Sea test

Abstract

A joint NOAA, U.S. Coast Guard and NASA program is being conducted to determine if a synthetic aperture radar (SAR) system, such as planned for NASA's SEASAT, can be useful in monitoring fishing vessels within the newly established 200-mile fishing limit. As part of this program, data gathering field operations were conducted over concentrations of foreign fishing vessels in the Bering Sea off Alaska in April 1976. The Jet Propulsion Laboratory developed synthetic aperture L-band radar which was flown aboard the NASA Convair 990 aircraft, with a Coast Guard cutter and C-130 aircraft simultaneously gathering data to provide both radar imagery and sea truth information on the vessels being imaged. Results indicate that synthetic aperture radar systems have potential for all weather detection, enumeration and classification of fishing vessels.

49. Interrupted Identities: Autologous Stem Cell Transplant in Patients With Multiple Myeloma

Abstract

In the current study, we sought to examine whether patients incorporate the identity of a patient receiving autologous stem cell transplant (ASCT) for multiple myeloma (MM) into their daily lives. Multiple myeloma patients receive education before initiating the ASCT treatment process. In this ethnographic study using interpretative phenomenological analysis, we observed pretransplant education visits with 30 MM patients, followed by semistructured interviews in their hospital rooms during transplant. The experience of receiving ASCT for MM required effort by patients to not only maintain their past identity but also establish a new patient identity. Reconciling these 2 identities required deliberate and emotionally draining effort from the patient. Results were organized into 2 overarching themes of social relationships and aesthetics with subthemes for each. Understanding methods MM patients who are receiving ASCT use to negotiate normalcy during treatment may be helpful for developing interventions for alleviating distress during this difficult time.
