Abstract

Real-world data on the range and impact of comorbid health conditions that affect pediatric asthma are scant, especially from developing countries. Lack of data hinders effective diagnosis, treatment, and overall management of these complex cases. We, hereby, describe the common pediatric asthma comorbid conditions in terms of evidence for association, potential mechanisms of impact on asthma control, and treatment benefit. Obesity, upper airway allergies, dysfunctional breathing, multiple sensitizations, depressive disorders, food allergy, and gastro-esophageal reflux are common associations with difficult-to-treat asthma. On the other hand, asthma symptoms and/or management may negatively impact the well-being of children through drug adverse effects, worsening of anaphylaxis symptoms, and disturbing mental health.Awareness of these ailments may be crucial for designing the optimum care for each asthmatic child individually and may ultimately improve the quality of life of patients and their families. A multidisciplinary team of physicians is required to identify and manage such comorbidities aiming to mitigate the over-use of asthma pharmacotherapy. Asthma research should target relevant real-world difficulties encountered at clinical practice and focus on interventions that would mitigate the impact of such comorbidities. Finally, policymakers and global healthcare organizations are urged to recognize pediatric asthma control as a healthcare priority and allocate resources for research and clinical interventions. In other words, global asthma control needs support by compassionate scientific partnership.

Abstract

Background: Anaphylaxis is the most severe clinical presentation of acute systemic allergic reactions and can cause death. Given the prevalence of anaphylaxis within healthcare systems, it is a high priority public health issue. However, management of anaphylaxis – both acute and preventative – varies by region. Methods: The World Allergy Organization (WAO) Anaphylaxis Committee and the WAO Junior Members Steering Group undertook a global online survey to evaluate local practice in the diagnosis and management of anaphylaxis across regions. Results: Responses were received from WAO members in 66 countries. While intramuscular epinephrine (adrenaline) is first-line treatment for anaphylaxis, some countries continue to recommend alternative routes in contrast to guidelines. Epinephrine auto-injector (EAI) devices, prescribed to individuals at ongoing risk of anaphylaxis in the community setting, are only available in 60% of countries surveyed, mainly in high-income countries. Many countries in South America, Africa/Middle-East and Asian-Pacific regions do not have EAI available, or depend on individual importation. In countries where EAIs are commercially available, national policies regarding the availability of EAIs in public settings are limited to few countries (16%). There is no consensus regarding the time patients should be observed following emergency treatment of anaphylaxis. Conclusion: This survey provides a global snapshot view of the current management of anaphylaxis, and highlights key unmet needs including the global availability of epinephrine for self-injection as a key component of anaphylaxis management.

Abstract

Asthma imposes a heavy morbidity burden during childhood; it affects over 10% of children in Europe and North America and it is estimated to exceed 400 million people worldwide by the year 2025. In clinical practice, diagnosis of asthma in children is mostly based on clinical criteria; nevertheless, assessment of both physiological and pathological processes through biomarkers, support asthma diagnosis, aid monitoring, and further lead to better treatment outcomes and reduced morbidity. Recently, identification and validation of biomarkers in pediatric asthma has emerged as a top priority across leading experts, researchers, and clinicians. Moreover, the implementation of non-invasive biomarkers for the assessment and monitoring of paediatric patients with asthma, has been prioritized; however, only a proportion of them are currently included in the clinical practise. Although, the use of non-invasive biomarkers is highly supported in recent asthma guidelines for documenting diagnosis and supporting monitoring of asthmatic patients, data on the Pediatric population are limited.In the present report, the Pediatric Asthma Committee of the World Allergy Organization (WAO), aims to summarize and discuss available data for the implementation of non-invasive biomarkers in the diagnosis and monitoring in children with asthma. Information on the most studied biomarkers, including spirometry, oscillometry, markers of allergic sensitization, fractional exhaled nitric oxide, and the most recent exhaled breath markers and “omic” approaches, will be reviewed. Practical limitations and considerations based on both experts' opinion and critical review of the literature, on the utility of all “well-known” and newly introduced non-invasive biomarkers will be presented. A critical commentary on biomarkers’ use in diagnosing and monitoring asthma during the COVID-19 pandemic, cost and availability of biomarkers in different settings and in developing countries, the differences on the biomarkers use between Primary Practitioners, Pediatricians, and Specialists and their role on the longitudinal aspect of asthma is provided.

Abstract

This paper introduces the CLASSLA-Express workshop series as an innovative approach to disseminating linguistic resources and infrastructure provided by the CLASSLA Knowledge Centre for South Slavic languages and the Slovenian CLARIN.SI infrastructure. The workshop series employs two key strategies: (1) conducting workshops directly in countries with interested audiences, and (2) designing the series for easy expansion to new venues. The first iteration of the CLASSLA-Express workshop series encompasses 6 workshops in 5 countries. Its goal is to share knowledge on the use of corpus querying tools, as well as the recently-released CLASSLA-web corpora - the largest general corpora for South Slavic languages. 7. Gestational Diabetes and Risk Assessment of Adverse Perinatal Outcomes and Newborns Early Motoric Development

Abstract

Background and Objectives: The aim of this study was to analyze the presence of gestational diabetes mellitus (GDM) on maternal and fetal perinatal parameters, as well to evaluate the influence of GDM on neonatal early motoric development. Materials and Methods: In this prospective study, we evaluated 203 eligible participants that were admitted to obstetrics department for a labor. GDM was assessed by evaluation of maternal parameters, fetal parameters, as well its impact on infants early motoric development (Alberta Infant Motor Scale—AIMS). Results: Presence of GDM was significantly positively associated with: pre-pregnancy weight, obesity degree, weight at delivery, gestational weight gain (GWG), body mass index (BMI) at delivery, GWG and increased pre-pregnancy BMI, glucose levels in mother’s venous blood after the delivery, positive family history for cardiovascular disease, pregnancy-related hypertension, congenital thrombophilia, drug use in pregnancy, large for gestational age (LGA), mode of delivery (Cesarean section and instrumental delivery). Likewise, GDM association was detected for tested ultrasound parameters (biparietal diameter (BPD), head circumference (HC), abdominal circumference (AC), femoral length (FL)), length at birth, birth weight, newborn’s head circumference, newborn’s chest circumference, AIMS supination and pronation at three months, AIMS supination, pronation, sitting and standing at six months. Only Amniotic Fluid Index and AIMS supination at three months of infant’s age remained significantly associated in multivariate regression model. Conclusions: The presence of significant positive association of numerous tested parameters in our study on perinatal outcomes and early motoric development, points to the necessity of establishing appropriate clinical decision-making strategies for all pregnant woman at risk and emphasize the importance of providing adequate glycaemia control options and further regular follow ups during the pregnancy.

10. Modeling convective thin-layer drying of carrot slices and quality parameters

Abstract

The influence of thin layer convective dehydration parameters on drying kinetics parameters, chemical composition, and color parameters of carrot slices were investigated, and corresponding mathematical models were developed. In the carrot slices, convective dehydration process hot air temperature and the sample slice thickness were varied, while measured, calculated, and modeled responses were: time of dehydration, effective moisture diffusivity, the energy of activation, proteins and cellulose contents, lightness, redness, and yellowness. The obtained results showed that varied convective dehydration process parameters statistically significantly affected all investigated responses except activation energy. The most efficient drying model with the minimum thickness (3 mm) and the maximum drying temperature (70?C) had the shortest drying time (231 minutes). This model had the minimum resistance to mass transfer (the minimum effective moisture diffusivity, 2.04 ? 10?08 ? 7.12 ? 10?08 [m2s?1]), and the average maximum energy of activation (31.31 kJ/mol). As far as the carrot slices? chemical composition and color parameters were concerned, the model with the maximum thickness (9 mm) and the minimum drying temperature (35?C) was the optimal one. This model had the longest dehydration time (934 minutes), the maximum resistance to the mass transfer (8.87 ? 10?08 [m2s?1]), the minimum total protein content (5.26 %), and the darkest color (49.70). The highest protein content (7.91%) was found for the samples subjected to the highest drying temperatures and the lowest carrot slice thickness. In contrast, the process of convective dehydration had led to the lighter, reddish, and yellowish carrot slices. All developed mathematical models were statistically significant.

Abstract

In this research, chicken meat was inoculated with selected microorganisms and subjected to the osmotic dehydration process in two osmotic solutions, in an effort to investigate the effect of storage time duration on its microbiological and chemical characteristics. Total viable counts, numbers of Enterobacteriaceae, Salmonella spp., Listeria monocytogenes, Escherichia coli, proteolytic bacteria, psychotropic bacteria, of microbiological, and biogenic amine content, TBARS and DPPH, and chemical analyses were conducted on meat samples stored at 22?C, during 14 days. During storage, the number of all tested microorganisms on meat samples decreased. The highest reduction occurred in the first 4 days. The meat dehydrated in molasses achieved better results of microbiological profile during storage. Results of TBARS and DPPH analyses indicated lipid oxidation after 14 days of storage, while the results after 10 days were satisfactory. Developed mathematical models allows good prediction of microbiological and chemical responses of dehydrated chicken meat during the investigated storage duration.

Abstract

Background and Objectives: The aim of this study was to analyze the presence of gestational diabetes mellitus (GDM) on maternal and fetal perinatal parameters, as well to evaluate the influence of GDM on neonatal early motoric development. Materials and Methods: In this prospective study, we evaluated 203 eligible participants that were admitted to obstetrics department for a labor. GDM was assessed by evaluation of maternal parameters, fetal parameters, as well its impact on infants early motoric development (Alberta Infant Motor Scale—AIMS). Results: Presence of GDM was significantly positively associated with: pre-pregnancy weight, obesity degree, weight at delivery, gestational weight gain (GWG), body mass index (BMI) at delivery, GWG and increased pre-pregnancy BMI, glucose levels in mother’s venous blood after the delivery, positive family history for cardiovascular disease, pregnancy-related hypertension, congenital thrombophilia, drug use in pregnancy, large for gestational age (LGA), mode of delivery (Cesarean section and instrumental delivery). Likewise, GDM association was detected for tested ultrasound parameters (biparietal diameter (BPD), head circumference (HC), abdominal circumference (AC), femoral length (FL)), length at birth, birth weight, newborn’s head circumference, newborn’s chest circumference, AIMS supination and pronation at three months, AIMS supination, pronation, sitting and standing at six months. Only Amniotic Fluid Index and AIMS supination at three months of infant’s age remained significantly associated in multivariate regression model. Conclusions: The presence of significant positive association of numerous tested parameters in our study on perinatal outcomes and early motoric development, points to the necessity of establishing appropriate clinical decision-making strategies for all pregnant woman at risk and emphasize the importance of providing adequate glycaemia control options and further regular follow ups during the pregnancy.

Abstract

Allergic diseases are the most common chronic diseases in children and no complete agreement on effective measures for primary prevention is available. Atopic family history is one of the most important risk factors for the development of asthma. A decline in microbial diversity due to modern lifestyle particularly in urban areas was proposed to have an important role in allergic epidemic. Recent studies are more focused on the specific mode of prevention such as probiotic usage in early pregnancy and infants period. It is well known that the composition of the gastrointestinal microbiota has been postulated to play a role in the development of allergies because it promotes potentially antiallergenic processes: TH1-type immunity, generation of TGF (which has an essential role in suppression of TH2-induced allergic inflammation and induction of oral tolerance), and IgA production, an essential component of mucosal immune defenses. Probiotic interventions administered during pregnancy and breastfeeding offer a unique opportunity to influence a range of important maternal and infant outcomes.

Abstract

Background: The aim of our study was to assess the influence of prepregnancy Body Mass Index (BMI) changes as well as excessive gestational weight gain (GWG) on maternal and fetal perinatal parameters. Furthermore, we aimed to analyze the influence of increased prepregnancy BMI values and excessive GWG on neonatal early motoric development. Methods: The 203 eligible female participants were evaluated. Prepregnancy Body Mass Index (BMI) and excessive gestational weight gain (GWG) defined according to Institute of Medicine 2009 guidelines in the USA were assessed with tested maternal and fetal perinatal parameters and infants early motoric development (Alberta Infant Motor Scale&mdash, AIMS). Results: Significant predictors of increased prepregnancy BMI in perinatal period include: weight at delivery (p = 0.001), GWG (p = 0.002) and BMI at delivery (p &lt, 0.001), while significant predictors of excessive GWG in perinatal period are: prepregnancy BMI (p = 0.029) and BMI at delivery (p &lt, 0.001). In the group of participants with both increased prepregnancy BMI and excessive GWG versus others, significant predictors were: hypertension (HTA) (p = 0.019), amniotic fluid index (AFI) (p = 0.047), Pronation (AIMS) (p = 0.028) and Supination (AIMS) (p = 0.029). Conclusion: Increased prepregnancy BMI and excessive GWG are significantly associated with numerous perinatal factors that could alter the pregnancy course, pregnancy outcome and early motoric development of newborn. Moreover, increased prepregnancy BMI is shown to be a significant predictor of excessive GWG, thus, early selection of pregnant women for close monitoring of weight gain during pregnancy will have positive effects on reducing the risk of less favorable pregnancy course and early motoric development of newborn.

Abstract

Objectives: To estimate clinical outcomes and cost-effectiveness of ombitasvir/paritaprevir/ritonavir and dasabuvir ± ribavirin (OMB/PTV/r + DSV ± RBV) compared with treatment regimens including pegylated interferon (PegIFN) for patients with chronic genotype 1 hepatitis C virus (HCV) infection. Methods: An Excel spreadsheet Markov model tracking progression through stages of liver disease was developed. Costs and patient utilities for liver disease stages were taken from published studies. Rates of disease progression were based on studies of untreated HCV infection and long-term follow-up of those achieving sustained virologic response (SVR) after drug treatment. Impact of OMB/PTV/r + DSV ± RBV and other drug regimens on progression was estimated through SVR rates from clinical trials. Analyses were performed for treatment-naive and treatment-experienced patients. Impact of alternative scenarios and input parameter uncertainty on the results were tested. Results: For genotype 1 treatment-naive HCV patients, for OMB/PTV/r + DSV ± RBV, PegIFN + ribavirin (PegIFN/RBV), sofosbuvir + PegIFN/RBV, telaprevir + PegIFN/RBV, boceprevir + PegIFN/RBV, lifetime risk of decompensated liver disease was 5.6%, 18.9%, 7.4%, 11.7%, and 14.9%; hepatocellular carcinoma was 5.4%, 9.2%, 5.7%, 7.0%, and 7.4%; and death from liver disease was 8.7%, 22.2%, 10.4%, 14.8%, and 17.6%, respectively. Estimates of the cost-effectiveness of OMB/PTV/r + DSV ± RBV for treatment-naive and treatment-experienced patients indicated that it dominated all other regimens except PegIFN/RBV. Compared with PegIFN/RBV, the incremental cost-effectiveness ratios were £13,864 and £10,258 per quality-adjusted life-year (QALY) for treatment-naive and treatment-experienced patients, respectively. The results were similar for alternative scenarios and uncertainty analyses. Limitations: A mixed-treatment comparison for SVR rates for the different treatment regimens was not feasible, because many regimens did not have comparator arms; instead SVR rates were based on those from recent trials. Conclusions: OMB/PTV/r + DSV ± RBV is a cost-effective oral treatment regimen for chronic genotype 1 HCV infection compared with standard treatment regimens and is estimated to reduce the lifetime risks of advanced liver disease.

Abstract

Introduction: Sublingual allergen specific immunotherapy (SLIT) constitutes a prefered route of administration of AIT for respiraotory allergies, especially in children population. The aim of our study was to investigate sustained efficacy of SLIT in a 4-yeras observational case control study trial, including 2 yeras of follow up after completion of a 2 - yeras period of treatment. Methodes: A case control observational study inculded children with history of mild to moderate allergen induced AR and asthma, inadequately controlled by symptomatic medications. 59 patients completed the trail. End points included nasal and asthma symptom and medication scores. Results: The mean nasal and asthma symptoms was reduced by 75% (x2=37,783, p Conclusions: The results confirm positive clinical outcomes and long lasting effects of SLIT.

Abstract

Introduction: With the prevalence of between 10-20% atopic dermatitis (AD) is one of the most common chronic diseases in infants and children. AD is characterized by itching and recurrent lesion. Mixtures of probiotics, including at the first place Lactobacillus rhamnosus LGG strain strains significantly decreased the risk of AD if given pre- and postnatally. Objective: The aim of the current study was to assess the effect of oral supplementation with LGG in infancy on eczema development and sensitization during the first 2 years of life. Material and methods: The study was a real life controlled observational study that included 96 patients (52 infants were treated with Lactobacillus rhamnosus with vitamin D3 and zinc, while the rest were advised to use only symptomatic treatment). Results: At the baseline the infants in the experiment group had higher values for SCORAD. During the follow up period a significant reduction in SCORAD was observed only in the group of participants who was treated with probiotics. They used also less topical treatments and antihistamines. Conclusions: This study has found that Lactobacillus rhamnosus GG (LGG) formulation with Zn and vitamin D3 supplementation during the postnatal period (in infancy and early childhood) reduce the severity of atopic dermatitis. Based on the findings it is recommended to introduce probiotic supplementation as well as vitamin D3 and zinc in infants and children with atopic dermatitis and positive family history.

Abstract

Electronic lexicography is a branch of lexicography that deals with theoretical and practical aspects of online dictionaries, additionally encompassing encyclopedias, lexicons, glossaries, terminological repositories, and other online information tools. From the user’s perspective, electronic dictionaries offer fast access, entries enriched with new types of information such as sounds and images as well as links to other software featuring embedded teaching and learning, games, etc. From the lexicographic perspective, e-dictionaries overcome limitations of alphabetical ordering as well as those of linear text by supplying entries with links and the show less/more functionality. However, despite these advantages, not all national lexicographies offer a wide range of born-digital dictionaries. In response to this, contemporary e-lexicography seeks to offer solutions to the challenges arising from the seemingly ideal digital conditions, including the danger of overwhelming the user with an abundance of unstructured data leading to the loss of information. E-lexicography also seeks to answer the question of what lexicographers can do to induce users to obtain information from dictionaries rather than from online databases or fast search engines. This article provides an overview of the current state-of-the-art in electronic lexicography from a theoretical and technological perspective. It starts by presenting the main theoretical postulates related to the features of e-dictionaries and electronic lexicography theory. On the one hand, electronic lexicography is based on existing lexicographic theory with its linguistic underpinnings, where dictionary microstructure crucially depends on the lexicographer’s practice. On the other hand, it involves technological, functional and aesthetic aspects of creating a digital dictionary. The overview is followed by examples from electronic dictionaries of Croatian and other languages illustrating how the challenges of the information age and of balancing all the features of e-lexicography with user needs may be addressed. The article concludes that current lexicographic practice in creating dictionaries and other lexicographic tools is turned towards keeping up with rapid technological developments and social needs.

Abstract

Background: Allergen-specific immunotherapy (AIT) holds a great promise in the management of allergic conditions, as the only with the capability to change the natural causes of allergic diseases. Most studies evaluated clinical scores as the main parameters, whereas immunological and-or inflammatory factors were studied only occasionally. The aim of our study was to investigate the efficacy of AIT drops on FeNO as the main non-invasive parameters of eosinophil airway inflammation. We also compared the relation of asthma and medication scores and parameters of lung function. Methods: 59 asthmatic children were included in the study. 34 patients were received AIT drops plus standard pharmacotherapy while 29 children received only standard pharmacotherapy according to the GINA guideline. Efficacy was evaluated using FeNO, asthma and medication symptom score and lung function tests. The results of the test were compared at baseline, during the first year of follow up period and at the end of the observational period. Results: Both asthma and rhinitis scores were decreased during the first year of treatment, with the tendency of further decrease in the second year of follow up period, but only in the experimental group. Abstract

Our previous studies showed that zinc (II), cadmium (II) and nickel (II) complexes with 2-formylpyridine selenose-micarbazone induce apoptosis in cancer cells via activation of mitochondrial pathway. Herein, we reported their antimetastatic properties. Nickel (II), and zinc (II) complexes exhibited the strongest inhibitory potential towards MMP-2/9, while all investigated compounds significantly decreased proteolytic activity of MMP-2/9 in human breast cancer MDA-MB-361 cells. As shown by in vitro transmembrane assays, nickel (II) complex was the most effective in inhibiting invasion of MDA-MB-361 cells, while the cadmium (II) complex was the most active in inhibiting HeLa cells invasion. In malignant cells, the complexes inhibited intracellular accumulation of reactive oxygen species, known for its pro-angiogenic properties via VEGF signaling, but no reduction in total cellular amount of VEGF was found. Furthermore, tubulogenesis test showed anti-angiogenic effect of the complexes in treated endothelial cells. Data indicate multiple mechanisms of the complexes' anti-angiogenic properties. In addition, they could modulate metastatic phenotype of tumor cells. Nickel (II) complex with 2-formylpyridine selenosemicarbazone revealed to be the most potent.

Abstract

Objective. In light of the large number of recent studies and systematic reviews investigating the cost of RA, this article examines the methods used to assess the impact of RA on employment and work productivity, and provides an overview of the issues surrounding work productivity loss in the RA population. Methods. A review of the published literature was conducted in order to identify relevant articles. These articles were then reviewed and their methodologies compared. The various methods used to calculate economic loss were then explained and discussed. Results. We found that although methods of lost productivity and associated costs varied between studies, all suggest that RA is associated with significant burden of illness. Economic analyses that exclude indirect costs will therefore underestimate the full economic impact of RA. However, the methods used to calculate productivity loss have a significant impact on the results of indirect cost analyses, and should be selected carefully when designing such studies. Several factors relating to the disease, the job and socio-demographics have been found to predict work disability. Conclusions. Consideration of these factors is vital when measuring the extent of both absenteeism and presenteeism, and will allow for more accurate estimation of the impact of RA on work productivity. This information may also guide interventions aiming to prevent or postpone work disability and job loss.

The antitumour action of three polyoxometallate compounds of Keggin's type: 12-molibdophosphoric acid (MoPA), 12-tungstophosphoric acid (WPA) and Mg salt of WPA (MgHWPA) was studied in vitro. For human cervix carcinoma (HeLa) cells survival, as well as for nonstimulated and stimulated peripheral blood mononuclear cells (PBMC), MTT test was applied and IC50 values of POMs were determinated. Index selectivity for WPA and MgHWPA are 1.9 and 1.8, calculated for nonstimulated, as well as 2.5 and 2.0, calculated for stimulated PBMS. Combination of studied POMs do not contribute to their lower IC50 values. Apoptosis detection implies mild cytotoxic effect of WPA and more cytostatical effect of MgHWPA. Combination of each of the studied POMs with caffeine decreases HeLa survival in dose dependent way. None of the studied POMs in the used concentrations (up to 100 mmol/L) damages blood cells and/or decreases their number. Ispitivano je in vitro antitumorsko dejstvo tri jedinjenja iz grupe polioksometalata (POM-a) Keginovog tipa: 12-fosformolibdenska kiselina (MoPA), 12-fosforvolframova kiselina (WPA) i Mg so 12-fosforvolframove kiseline (MgHWPA). Primenom MTT testa na ćelije karcinoma cerviksa (HeLa ćelije), kao i na nestimulisane i stimulisane mononuklearne ćelije iz periferne krvi (PBMC), određene su IC50 vrednosti ispitivanih POM-a. Indeksi selektivnosti za WPA i MgHWPA su 1,9 i 1,8 računati za nestimulisane, odnosno 2,5 i 2,0 za stimulisane PBMC. Kombinacijom ispitivanih POMa ne postiže se smanjenje njihovih IC50 vrednosti. Detekcijom apoptoze utvrđeno je da WPA pokazuje blagi citotoksični efekat, dok MgHWPA ima blagi citotostatski efekat. U kombinaciji sa kofeinom, POM jedinjenja još više smanjuju preživljavanje HeLa ćelija. U ispitivanim koncentracijama (do 100 mmol/L), nijedno od POM jedinjenja ne utiče na broj i izgled krvnih ćelija pune krvi zdravih davalaca.

The European Commission, through the Directorate General for Health and Consumer Affairs, funded the study on Health Effects of School Environment held in different European countries. Levels of air pollutants can be several folds higher exposures are prolonged. Since children spend a large part of the day in school environment, nationwide initiatives to evaluate such indoor air quality (IAQ) were developed.

Objectives: To estimate clinical outcomes and cost-effectiveness of ombitasvir/paritaprevir/ritonavir and dasabuvir ± ribavirin (OMB/PTV/r + DSV ± RBV) compared with treatment regimens including pegylated interferon (PegIFN) for patients with chronic genotype 1 hepatitis C virus (HCV) infection. Methods: An Excel spreadsheet Markov model tracking progression through stages of liver disease was developed. Costs and patient utilities for liver disease stages were taken from published studies. Rates of disease progression were based on studies of untreated HCV infection and long-term follow-up of those achieving sustained virologic response (SVR) after drug treatment. Impact of OMB/PTV/r + DSV ± RBV and other drug regimens on progression was estimated through SVR rates from clinical trials. Analyses were performed for treatment-naive and treatment-experienced patients. Impact of alternative scenarios and input parameter uncertainty on the results were tested. Results: For genotype 1 treatment-naive HCV patients, for OMB/PTV/r + DSV ± RBV, PegIFN + ribavirin (PegIFN/RBV), sofosbuvir + PegIFN/RBV, telaprevir + PegIFN/RBV, boceprevir + PegIFN/RBV, lifetime risk of decompensated liver disease was 5.6%, 18.9%, 7.4%, 11.7%, and 14.9%; hepatocellular carcinoma was 5.4%, 9.2%, 5.7%, 7.0%, and 7.4%; and death from liver disease was 8.7%, 22.2%, 10.4%, 14.8%, and 17.6%, respectively. Estimates of the cost-effectiveness of OMB/PTV/r + DSV ± RBV for treatment-naive and treatment-experienced patients indicated that it dominated all other regimens except PegIFN/RBV. Compared with PegIFN/RBV, the incremental cost-effectiveness ratios were £13,864 and £10,258 per quality-adjusted life-year (QALY) for treatment-naive and treatment-experienced patients, respectively. Abstract

This study presents 3 case reports of patients who experienced anginous pain during treatment with capecitabine. The interruption of capecitabine and sublingual or intravenous nitroglycerine treatment lead to recovery. Rechallenge of capecitabine with dose reduction of 30% lead to repeated anginous pain in 2 patients. Treatment with capecitabine had been replaced with weekly bolus 5FU-LV, without further cardiotoxicity. The literature contains data from about 50 patients who experienced cardiotoxicity during capecitabine treatment. The most frequent manifestations of capecitabine cardiotoxicity included: anginous pain in 38/53 (71.7%), arrhythmia in 6/53 (11.3%), myocardial infarction in 6/53 (11.3%). Cardiotoxicity of capecitabine lead to death in 6/53 (11.3%) patients. Risk factors for cardiotoxicity are associated with the grade 4 and the fatal outcome of cardiotoxicity (p = 0.035, p = 0.015), but not with the symptom recurrence upon capecitabine rechallenge (p = 0.18). The combination chemotherapy regimens are associated with the grade 4 of cardiotoxicity (p = 0.048), but not with the fatal outcome (p = 0.3). Rechallenge of capecitabine lead to symptoms recurrence in 10/16 patients. Neither the dose reduction of capecitabine (p = 0.18) nor the additional medical prophylaxis (p = 0.37) were important for the outcome of capecitabine rechallenge.

Abstract

Immune response against Helicobacter pylori is important for the course and outcome of infection. We conducted study looking for the difference in anti H. pylori IgG and IgA between patients with intestinal type of gastric cancer, superficial and atrophic gastritis.For this study, 133 patients infected with H. pylori were enrolled: 50 with superficial gastritis, 42 with atrophic gastritis and 41 with gastric cancer. Anti H. pylori IgG and IgA ELISA tests were performed. The difference in antibody titers of IgG and IgA, frequency of IgAIgG ratio and combination of low IgG and IgAIgG ratio were analyzed.The patients with gastritis had higher titer of IgG that the patients with gastric cancer (p0.01). The patients with superficial gastritis had higher titer of IgA than the patients with gastric cancer (p0.05). IgAIgG ratio is more frequent in patients with gastric cancer than in the patients with superficial gastritis (p0.01). Low IgG and IgAIgG is more frequent in the patients with gastric cancer than in the patients with gastritis (p0.01).The patients with gastric cancer elicit different anti H. pylori IgG and IgA response than the patients with superficial and atrophic gastritis. Low IgG and IgA predominance seems characteristic for gastric cancer.

Abstract

Opća načela leksikografske obrade traže da objašnjenje značenja bude kratko i jasno, dovoljno općenito da može obuhvatiti veliki broj primjera ili gotovo svaki kontekst te dovoljno razlikovno da informacija koja dolazi do korisnika bude razumljiva i korisna. Uzorom se dugo smatrala logička definicija koja uključuje najbliži viši rodni pojam i specifičnu razliku, ali su leksikografi tijekom povijesti i iskustva rada na različitim rječnicima tražili i iznalazili i druge tehnike opisa značenja. Jedna je od njih leksikografska definicija koja, osim što dopušta različite pristupe pri oblikovanju samog objašnjenja značenja, ovisno o jezičnim obilježjima pojedine riječi, u širem smislu uključuje i druge elemente natuknice, poput specifičnih formalnih rješenja kao što su veličina slova ili zasebne kućice s bilješkama o upotrebi, zatim oznake registra, konceptualna povezanost s drugim natuknicama te primjeri upotrebe. U kontekstu suvremene leksikografije koja je posve usmjerena na korisničke potrebe, sve su to elementi koji sudjeluju u opisu značenja riječi, odnosno pomažu korisniku razumjeti ga i potencijalno upotrebljavati. U ovome radu, na primjeru metoda prikupljanja i oblikovanja objašnjenja značenja frazema u Frazeološkome rječniku hrvatskoga jezika koji je u izradi, ispituju se utjecaj i uloga jezičnih i izvanjezičnih podataka koje o frazemima daje računalni korpus na kojem se temelji rječnička građa i zatim pokazuje način na koji se ti podaci integriraju u natuknicu s ciljem da rječnik što bolje odgovori idolu suvremene leksikografije – korisničkim potrebama.

Abstract

U ovome se radu prikazuje leksikografska obrada varijantnih frazema u mrežnome Frazeološkom rječniku hrvatskoga jezika koji je u izradi, a utemeljen je na podatcima iz računalnoga korpusa hrWaC. U tome se rječniku varijante različitih vrsta (leksičke, tvorbene i sintaktičke) bilježe u istoj natuknici jer imaju zajedničku konceptualnu osnovu. To odražava kognitivnolingvistički pristup frazemima, u kojem se varijante promatraju kao jezične manifestacije različitih konceptualizacija iste situacije, a među kojima zbog toga mogu postojati razlike u leksičkom sastavu, strukturi, značenju i upotrebi. Također, varijantni oblici istoga frazema mogu se razlikovati po stupnju konvencionaliziranosti, pa se redoslijed njihova navođenja u tome e-rječniku temelji na frekvenciji upotrebe prema podatcima iz hrWaC-a. Takva leksikografska obrada frazeološke varijantnosti odražava stvarnu upotrebu, a korisnici dobivaju potpunu i preciznu informaciju o različitim oblicima frazema i njihovim značenjima u suvremenome hrvatskome jeziku.

41. Microbiological quality of soft, semi-hard and hard cheeses during the shelf-life

Abstract

Cheeses as ready-to-eat food should be considered as a potential source of foodborne pathogens, primarily Listeria monocytogenes. The aim of present study was to determine the microbiological quality of soft, semi-hard and hard cheeses during the shelf-life, with particular reference to L. monocytogenes. Five types of cheeses were sampled at different timepoints during the cold storage and analyzed for presence of Salmonella and L. monocytogenes, as well as lactic acid bacteria, Escherichia coli, coagulase-positive staphylococci, yeasts, molds, sulfite-reducing clostridia and L. monocytogenes counts. Water activity, pH and NaCl content were monitored in order to evaluate the possibility of L. monocytogenes growth. Challenge test for L. monocytogenes was performed in soft whey cheese, to determine the growth potential of pathogen during the shelf-life of product. All analyzed cheeses were compliant with microbiological criteria during the shelf-life. In soft cheeses, lactic acid bacteria increased in the course of the shelf-life period (1.2-2.6 log increase), while in semi-hard and hard cheeses it decreased (1.6 and 5.2 log decrease, respectively). Soft cheeses support the growth of L. monocytogenes according to determined pH values (5.8-6.5), water activity (0.99-0.94), and NaCl content (0.3-1.2%). Challenge test showed that L. monocytogenes growth potential in selected soft cheese was 0.43 log10 cfu/g during 8 days at 4°C. Water activity in semi-hard and hard cheeses was a limiting factor for Listeria growth during the shelf-life. Soft, semi-hard and hard cheeses were microbiologically stable during their defined shelf-life. Good manufacturing and hygienic practices must be strictly followed in the production of soft cheeses as Listeria-supporting food and be focused on preventing (re)contamination.

The antimicrobial susceptibility of coagulase-negative staphylococci isolated from spontaneously fermented sausages was assessed using both traditional and molecular methods. Isolates were tested for sensitivity to vancomycin, ampicillin, erythromycin, tetracycline, gentamycin and oxacillin by the disk diffusion method and quantitative-qualitative epsilometer test. PCR was used for the detection of resistance genes mecA, ermB, tetK and tetM. The identified coagulase-negative staphylococci were Staphylococcus epidermidis (69 %), S. capitis (5 %) and S. warneri (2.5 %). S. epidermidis showed a high rate of phenotypical resistance to tetracycline and erythromycin (44.4 % of strains). Molecular evaluation of resistance determinants revealed tetK or tetM genes in eight S. epidermidis strains. Although S. epidermidis is not classical food poisoning bacteria, its presence in food could be of public health significance due to the possible spread of antimicrobial resistance determinants. Our findings implicate that spontaneous meat fermentation could result in products with a potential hazard to consumers.

Abstract

Two flavonoids, rutin and hesperidin, were investigated in vitro for anticoagulant activity through coagulation tests: activated partial thromboplastin time (aPTT), prothrombin time (PT) and thrombin time (TT). Only an ethanolic solution of rutin at the concentration of 830 µM prolonged aPTT, while TT and PT were unaffected. In order to evaluate whether the prolongation of aPTT was due to the decrease of coagulation factors, the experiment with deficient plasma was performed, showing the effects on factors VIII and IX. Since pharmacological activity of flavonoids is believed to increase when they are coordinated with metal ions, complexes of these flavonoids with Al(III) and Cu(II) ions were also tested. The results showed that complexes significantly prolonged aPTT and had no effects on PT and TT. Assay with deficient plasma (plasma having the investigated factor at less then 1%) revealed that complexes could bind to the coagulation factors, what may lead to a non-specific inhibition and aPTT prolongation. An effort was made to correlate stability of complexes with their anticoagulant properties.

U radu se na temelju dostupnih izvora i literature s tvorbeno-motivacijskog aspekta analiziraju prezimena zabilježena u najstarijoj matičnoj knjizi rođenih župe sv. Lovre u Petrinji. Početci oblikovanja naselja Petrinje sežu u 17. stoljeće, a matične se knjige u crkvi sv. Lovre vode od 1677. godine. Od sedamdeset tri prezimena čiji zapis seže i do tristo godina u prošlost, šezdeset tri su se zadržala u Petrinji i danas. Cilj je rada analizom prezimena dati povijesnu antroponimijsku sliku jezično općenito neistraženoga petrinjskog područja.

Abstract

The paper presents technological parameters, basic chemical composition and sensory evaluation, as well as the yield of 146 samples of Trappist cheese. In terms of water content in non-fat matter (56.36-58.31 %), Trappist cheese belongs to the group of semi-hard cheeses, and in terms of content of fat in dry matter (46.06-48.63 %), it belongs to the group of full-fat cheeses. In four cheese samples E. coli was determined within microbiological parameters, whereas other researched bacteria were not determined. Cheese has a shape of a symmetrical ring and it has a homogenous yellow color of protective coating. Mean mass value of one cheese ring is from 2.50 to 2.55 kg. The body of cheese has a homogenous light yellow color, which is brighter along the edges. The cheese holes have the size of a pea; they are bright and patterned over the whole surface of cross section. Consistency of cheese body is elastic, soft and interconnected. The cheese has mild, pure lactic and acidic taste and odor, and it is moderately salty. The results of sensory score show that even 91.78 % of cheese samples from experimental production can be assigned to the first quality class, whereas 8.22 % of samples belong to the second quality class.

Lactic acid bacteria are the constituent part of milk microbial flora that could influence the safety of dairy products due production of organic acids, hydrogen peroxide, carbon dioxide and bacteriocins. Taking this in consideration, the objective of this study was to investigate the composition of lactic acid bacteria population in fresh cow cheeses taken from local markets, as well as their antimicrobial capacity. Lactic acid bacteria counts were determined according to ISO 1524:1998 method, biochemical determination using API 50 CHL system, and inhibitory activity against L. monocytogenes NCTC 10527 by agar well diffusion assay. Lactic acid bacteria count in fresh cow cheeses (n=10) ranged from 5.87 to 8.38 log10 CFU g-1. Among 52 MRS isolates collected, 61.54 % were assigned to the Lactococcus lactis subsp. Lactis species, 23.07 % Lactobacillus helveticus, 11.54 % Leuconostoc mesenteroides subsp. cremoris and 3.85 % Leuconostoc mesenteroides subsp. mesenteroides. Antilisterial activity was found in 18 isolates.

Abstract

Various chemical agents are used during the whole production chain of milk and dairy products. Production of feedingstuffs is accompanied with pesticide usage, which may remain in environment, thus are transported through feeding into animals, animal products and finally in human organism. Preparation procedure and storage conditions of feed also influence on milk safety in the sense of mycotoxins entering into the food chain. Chemical agents are, on daily basis, used on dairy farms either as detergents or disinfections. The residuals of cleaning agents might remain in milk if the cleaning agents and its dosage are not performed adequately. Besides already mentioned agents, a great influence in milk production can bee seen through veterinary drugs usage, particularly antibacterial drugs (mastitis). Proper application of drugs and by following legal recommendation, a by-reactions can be avoided such as allergic reaction in humans, development of resisting bacteria or even undesirable influence on starter cultures in dairy products manufacture. The maximum residue limits, monitoring plan as well as sampling procedures are set up within the harmonization of Croatian and European legislation, in order to provide official control of residues in foodstuffs of animal origin.

Aims and background: Mining and energy complexes in Serbia are recognized as a major source of a large number of pollutants. Serbia’s environmental performance reports clearly indicate that large mining and energy complexes are the dominant source of air pollution. It is difficult to determine which of them remarkably threaten the quality of the environment, as all the basic elements of the environment (air, water, and soil) are threatened, not just one. Mining and energy complexes significantly reduce the quality of the immediate environment, as the distance areas throughout water and air pollution propagation. Additional motivation for this study lies in the fact that large mining and energy complexes are particularly interesting because they are located in the immediate vicinity or large rivers (mostly the Danube), which are protected in many national and international legislation acts (particularly the Danube). The basis for the preservation of environmental quality is an effective environmental management system (EMS) in mining and energy complexes. The aim of this study is to promote and elaborate the possibility for improving the mining and energy complex environmental protection/management system by applying the basic principles of sustainable development. Methodology: Project management methodology is selected as a tool. Project management is based on the application of a network planning technique (Critical Path Method), because of its suitability for representing the logical structure of environmental protection system. The survey was conducted in the area of the city of Kostolac. The examined area included mining and energy complexes in Kostolac, which incorporate three surface mines (Ćirikovac, Klenovik, and Drmno) and two thermal power stations (TE “Kostolac A” (100 MW) and TE “Kostolac B” (2 × 348.5 MW)). The PSR model (Pressures, State, Response) was used for the organization of the interactions within “society–environment” system as the basis for indicator selection. Results: Procedures to identify and determine the significance of the environmental aspects should be established by executive staff in mining and energy plants and complexes. Identification of environmental aspects is the first part of the planning system of environmental management and the activities related to them. The accomplishment of critical path activities represents the basis for improving the environmental protection system in mining and energy complexes. Discussion: Application of sustainable development principles depends on the country’s energy potential and the application of the adopted environmental policy regarding mining and energy complexes. Strict enforcement of existing laws and regulations presents an opportunity to rectify many shortcomings while providing economic benefits, restoring the usable value of ravaged land, and preserving air and water quality.
