- Abstract
We propose a new test to evaluate the impact of horizontal mergers on competition in the banking industry. The test is designed to be applied ex ante to potential mergers while being parsimonious in terms of data, as it only uses information on branches in local markets. The test is a counterfactual exercise based on a two-stage model where banks compete in branching and interest rates and requires comparing the estimated degree of competition in the status quo, where branching networks by banks are those actually observed, with a counterfactual scenario, where the branching network of the new entity is the sum of the branches of the banks involved in the horizontal merger. The statistical difference between the two estimated measures of competition quantifies the impact of the merger. We apply our test to French and Italian mergers.

Background There are no Italian data regarding the strategies for preventing neonatal group B streptococcal (GBS) infection. We conducted a national survey in order to explore obstetrical, neonatal and microbiological practices for the GBS prevention. Methods Three distinct questionnaires were sent to obstetricians, neonatologists and microbiologists. Questionnaires included data on prenatal GBS screening, maternal risk factors, intrapartum antibiotic prophylaxis, microbiological information concerning specimen processing and GBS antimicrobial susceptibility. Results All respondent obstetrical units used the culture-based screening approach to identify women who should receive intrapartum antibiotic prophylaxis, and more than half of the microbiological laboratories (58%) reported using specimen processing consistent with CDC guidelines. Most neonatal units (89 out of 107, 82%) reported using protocols for preventing GBS early-onset sepsis consistent with CDC guidelines. Conclusions The screening-based strategy is largely prevalent in Italy, and most protocols for preventing GBS early-onset sepsis are consistent with CDC guidelines. However, we found discrepancies in practices among centers that may reflect the lack of Italian guidelines issued by public health organizations.

Background: Accurate measurement devices are required to objectively quantify physical activity. Wearable activity monitors, such as pedometers, may serve as affordable and feasible instruments for measuring physical activity levels in older adults during their normal activities of daily living. Currently few available accelerometer-based steps counting devices have been shown to be accurate at slow walking speeds, therefore there is still lacking appropriate devices tailored for slow speed ambulation, typical of older adults.\ud \ud This study aimed to assess the validity of step counting using the pedometer function of the ADAMO Care Watch, containing an embedded algorithm for measuring physical activity in older adults.\ud \ud Methods: Twenty older adults aged ≥ 65 years (mean ± SD, 75±7 years; range, 68–91) and 20 young adults (25±5 years, range 20–40), wore a care watch on each wrist and performed a number of randomly ordered tasks: walking at slow, normal and fast self-paced speeds; a Timed Up and Go test (TUG); a step test and ascending/descending stairs. The criterion measure was the actual number of steps observed, counted with a manual tally counter. Absolute percentage error scores, Intraclass Correlation Coefficients (ICC), and Bland–Altman plots were used to assess validity.\ud \ud Results: ADAMO Care Watch demonstrated high validity during slow and normal speeds (range 0.5–1.5 m/s) showing an absolute error from 1.3% to 1.9% in the older adult group and from 0.7% to 2.7% in the young adult group. The percentage error for the 30-metre walking tasks increased with faster pace in both young adult (17%) and older adult groups (6%). In the TUG test, there was less error in the steps recorded for older adults (1.3% to 2.2%) than the young adults (6.6% to 7.2%). For the total sample, the ICCs for the ADAMO Care Watch for the 30-metre walking tasks at each speed and for the TUG test were ranged between 0.931 to 0.985.\ud \ud Conclusion: These findings provide evidence that the ADAMO Care Watch demonstrated highly accurate measurements of the steps count in all activities, particularly walking at normal and slow speeds. Therefore, these data support the inclusion of the ADAMO Care Watch in clinical applications for measuring the number of steps taken by older adults at normal, slow walking speeds.

In this study branching costs and competitiveness of European banks are measured by fitting a monopolistic competition model to a representative sample drawn from nine EEC banking industries in the period from 1990 to 1996. In the theoretical model, banks decide strategically the size of their branching network anticipating the degree of competition faced on interest rates. From the structural equations of the model an econometric test is derived in order to measure branching costs and degree of competition in banking services. The empirical analysis captures their changing over time together with the impact of various European directives aiming at deregulating the banking industry. This paper first discusses how the market is delineated in some recent antitrust cases in the printed media industry. It evaluates the extent to which the main features of the industry are incorporated into the analysis and affect market definition. In addition we argue that an econometric analysis that does not incorporate these features can lead to biased estimates of elasticities. As demand substitution is a crucial element for defining market, bad estimates of elasticities could blur the boundaries of relevant markets. Second we propose a simple model that encompasses these features and in particular the two-sidedness of the market. Thirdly, we review some empirical papers that analyse the issue of demand estimation in printed media. Finally, we perform a statistical estimation on a dataset of magazines in order to provide a measure of the possible bias that could arise in the estimation of elasticities when one does not use a proper model.

Background This multicenter, prospective, randomized, controlled, parallel trial compares the efficacy of biventricular (BIV) versus right ventricular (RV) antitachycardia pacing (ATP) in terminating all kinds of ventricular tachycardia (VT). Methods Five hundred twenty-six patients implanted with a cardiac resynchronization therapy defibrillator (CRT-D) device were enrolled and randomized 1:1 to either BIV (266) or RV (260) ATP (single burst 8 pulse, 88% coupling interval) and were followed up for 12 months. Results During 12 months' follow-up, 1,077 ventricular episodes in 180 patients were detected and classified: 634 true VTs divided into 69 ventricular fibrillation (VF) (11%), 202 fast ventricular tachycardia (FVT) (32%), and 363 VT (57%). A comparable first ATP efficacy (BIV 65% vs RV 68%, P = .59) was observed in FVT + VT, in VT zone (BIV 62% vs RV 71%, P = .25), and in FVT zone (BIV 71% vs RV 61%, P = .34). A trend toward lower accelerations during ATP applied to FVT was observed in the BIV group (3.5% BIV vs 10.2% RV, P = .163). No syncope/presyncope occurred during ATP for FVT in the BIV group versus 4 events (3.2%) in the RV group ( P = .016). biventricular ATP was more effective in treating FVT in coronary artery disease (CAD) patients ( P = .032), whereas both modalities presented similar efficacy in patients with non-CAD etiology ( P = .549). Conclusions Antitachycardia pacing is effective in patients implanted with a CRT-D device. No significant differences in efficacy emerged between BIV- and RV-delivered ATP in the general population, whereas BIV ATP seems to present a safer profile in ischemic patients.

Aim: To describe the duration and type, as classified by World Health Organization (WHO) criteria, of breastfeeding in Ligurian newborns up to 1 year of age, and to identify possible related factors. Methods: A prospective, observational study on a cohort of 757 women recruited in 10 hospitals in Northern Italy. Women were followed up by means of a questionnaire at discharge and by telephone interviews in the 4th, 12th, 24th and 48th weeks postpartum using the ‘recall period’ method. Results: At discharge, 97.3% of mothers had started breastfeeding; regional breastfeeding rates at the 4th, 12th, 24th and 48th weeks were respectively 84.4%, 71.2%, 54.4% and 26.3%. Socio-demographic factors did not significantly influence these percentages. Median duration of breastfeeding was 6.5 months. No statistically significant differences were observed in duration of breastfeeding between women who started exclusive breastfeeding (EBF) and those who did predominant breastfeeding (PBF), while the difference was statistically significant when comparing complementary breastfeeding (CBF) with EBF and with PBF. Conclusions: Our study is the first regional experience in the Liguria region and represents a starting point for the diffusion of the culture of breastfeeding. Our results are satisfactory with regards to the overall duration of breastfeeding until the first year of life (26.3%), while duration of EBF (9.5% at the 24th week) is still unsatisfactory according to WHO objectives that include EBF for all children up to the sixth month of life.

Epidemiologic data suggest strong links between hospitalisation with bronchiolitis in infancy and subsequent higher risk of developing lower respiratory tract infections (LRTI) and/or hyperreactive airway diseases. The aim of this study was to evaluate in an Italian population the natural history of respiratory diseases in children hospitalised for LRTI when they were2 years.An observational, perspective, longitudinal study was performed through telephone interviews. Nine pediatric tertiary care centres participated to the study evaluating a population of 187 children, hospitalised in the previous year (November 1999-April 2000) for bronchiolitis or pneumonia when they were2 years of age and participated to a previous study on the prevalence of infant LRTI in Italy (RADAR).Twenty-three (12.3%) children had a gestational age36 weeks. In the 12 months following the first hospitalisation, an elevated frequency of respiratory symptoms was found. Indeed, 152 (81.3%) children suffered from not-requiring-hospital-admission respiratory infections and 21 (11.2%) were hospitalized again for LRTI: 11.6% had bronchiolitis, 23.5% bronchitis and 35.2% pneumonia. In addition, 1.2% had gs;3 infectious episodes and 21.4% gs;6: 68 (36.4%) showed wheezy bronchitis and 17 (9.1%) were reported to have asthma; 132 children (71%) took antibiotics during the last year, 19.4%3 times; 111 (59.4%) bronchodilators and 49 (26.2%) oral corticosteroids. One year after the first hospitalisation, 19 subjects (10.2%) were found to be positive to at least one class of allergens by prick test or RAST.Thus, the demonstration of a high morbidity rate for LRTI, wheezing and asthma in this study group during the first year follow-up after hospital admission further support the need for prophylactic interventions to reduce the morbidity and severity of sequelae of LRTI, in particularly in premature children and/or with additional risk factors.

This paper starts from a recent case to study how merger analysis in Europe may potentially be improved through simulation analysis. Starting from the product and geographic market definition in the Merger Decision, we formulate and estimate an oligopoly model with differentiated products. The model is simulated to account for the changed multiproduct ownership structure after the merger. We show how our first two tests, a potential and an actual market power test, produce useful information, complementary to the traditional dominance principle adopted in the European Union. A drastic revision of current merger principles is thus not required. We also show how simulation analysis can provide useful additional information that goes beyond the traditional dominance principle. This is illustrated through two examples. This paper analyses the relation between competition and concentration in the banking sector. The empirical answer is given by testing a monopolistic competition model of bank branching behaviour on individual bank data at county level (départements and provinces) in France and Italy. We propose a measure of the degree of competiveness in each local market that is function also of market structure indicators. We then use the econometric model to evaluate the impact of horizontal mergers among incumbent banks on competition and discuss when, depending on the pre-merger structure of the market and geographic distribution of branches, the merger is anti-competitive. The paper has implications for competition policy as it suggests an applied tool to evaluate the potential anti-competitive impact of mergers.

The effect of hand muscle mechanical vibration on the somatosensory potential (SEP) evoked by median nerve stimulation, was investigated in 10 healthy subjects. A marked decrease in the amplitude of the N17, N20 and P25 components of the cerebral SEP was observed, while the S11 and S13 components of the cervical response did not change. The amplitude reduction of the SEP components was larger when low frequency vibration was used. Recordings performed after cooling the hand further suggest that the reduction of the amplitude of the SEP components induced by vibration is likely to depend on activation of muscle receptors. These findings could reflect an interaction between limniscal and spino-cerebellar inputs, possibly occurring at the thalamo-cortical levels, a concept compatible with the hypothesis that muscle spindle afferents do contribute to kinaesthesia or position-sense. In six normal subjects cerebral somatosensory potentials evoked by median nerve stimulation at the wrist have been recorded in order to assess whether N20 and P25 components can be related to a single generator dipole. It was found that, at variance with the current literature, such components are due to separate generator dipoles, possibly related to the activity of different cortical structures. Moreover an additional component, chronologically occurring between N20 and P25, was consistently observed.

- Abstract
Initial energy (IE) of 200 J or greater is recommended for external electrical cardioversion (EC) of Atrial Fibrillation (AF) with monophasic waveform (MW). Devices that deliver current with a biphasic waveform (BW) appear to achieve EC at lower energy levels than that use a MW. The aim of the present study was to evaluate the influence of several factors (age, body-weight, ejection fraction, left atrial diameter, AF duration) on the results of EC with BW. 105 patients (pts) (59 male and 46 female, mean age 70 ± 8 yrs) underwent EC. Before EC, all pts were treated with oral anticoagulant for 4 weeks until the INR value achieved between 2.5 and 3, and with different antiarrhythmic drugs (sotalol, propafenone, flecainide, amiodarone). EC was done under general anaesthesia, in the presence of anaesthetist. The initial energy used for the first shock was 50 J/100 J, increasing up to 150-200 J, if the first shock unsuccessful (protocol 50/100J to 150-200 J). Reversion of AF to sinus rhythm was obtained in 92 % of the pts. Correlated with shock energy of 50 J, 100 J, 150 J, 200 J, the successful CE was found for the body-weight 69 ± 5, 70 ± 12, 78 ± 4 and 80 ± 9 kg, respectively. There was no statistical different between the four groups, regarding age, left atrial diameter, ventricular systolic function, duration of arrhythmia. In conclusion, an IE of 50 J could be used in pts weighing ⊂ 75 kg, an IE of 100 J could be used in pts with weight ⊇ 75 but ⊆ 82 kg, while for pts with a weight ⊃ 82 kg an IE of 150 J should be recommended. We propose a new test to evaluate the impact of horizontal mergers on competition in the banking industry. The test is designed to be applied ex ante to potential mergers while being parsimonious in terms of data, as it only uses information on branches in local markets. The test is a counterfactual exercise based on a two‐stage model where banks compete in branching and interest rates and requires comparing the estimated degree of competition in the status quo, where branching networks by banks are those actually observed, with a counterfactual scenario, where the branching network of the new entity is the sum of the branches of the banks involved in the horizontal merger. The statistical difference between the two estimated measures of competition quantifies the impact of the merger. We apply our test to French and Italian mergers.

In this study branching costs and competitiveness of European banks are measured by fitting a monopolistic competition model to a representative sample drawn from nine EEC banking industries in the period from 1990 to 1996. In the theoretical model, banks decide strategically the size of their branching network anticipating the degree of competition faced on interest rates. From the structural equations of the model an econometric test is derived in order to measure branching costs and degree of competition in banking services. The empirical analysis captures their changing over time together with the impact of various European directives aiming at deregulating the banking industry. Furthermore the study shows persistence of segmentation acoss EEC banking industries.

- Abstract
The history of safety science has seen the flourishing of several models and metaphors aimed at describing organizational accidents' dynamics. Metaphors and their graphical representations are powerful tools to frame risks and adverse events in socio-technical systems; they help in coping with systemic complexity but can also become a constraint and even bias the understanding of our environment. This paper aims to investigate how metaphors and their graphical representations influence the comprehension of organizational accidents, how they could be misinterpreted, and, as a result, generate misunderstandings of events. To address these questions, we analyze three paradigmatic accident causation models, typical of three phases in the evolution of models in the last century, describing how the related metaphors and depictions could influence the perception and understanding of risk factors. In addition, we present some possible misunderstandings that could be produced by the metaphor and graphical features of representations, with a particular focus on safety outcomes. Eventually, we provide a framework with the basic characteristics of an effective model and metaphor for the description and analysis of organizational accidents in modern complex socio-technical systems. This framework could be used as a guide for proposing new and more effective models in safety science.

46. Competitive impact of the air ticket levy on the European airline market.

- Abstract
In tribute to Jules Dupuit, this study analyzes how the innovative financing tax for development has influenced the competition among airline companies and to what extent it affected their sales. To do so, we specify an econometric model where the representative consumer chooses the utility maximizing alternative among the differentiated products offered by the airlines which compete in terms of prices. It is a fairly reasonable and well accepted representation of the competition in the airline industry, assuming that the structure of their networks is given in the short run. The present solidarity tax has increased the average price for the European airlines by only 0.08 % and could increase average prices by 1 % if it were applied to all European airlines. Implemented by all countries it would lead to a 0.17 % increase in Air France prices while the price increase would be 1.05 % for the other airlines in our sample. These numbers compared to the growth rate of the air traffic show that the impact of the solidarity tax would be very small. Overall the air ticket levy applied by all European airlines would not affect significantly the degree of competition among airlines., (© 2018 Published by Elsevier Ltd.)

- Abstract
Background: Accurate measurement devices are required to objectively quantify physical activity. Wearable activity monitors, such as pedometers, may serve as affordable and feasible instruments for measuring physical activity levels in older adults during their normal activities of daily living. Currently few available accelerometer-based steps counting devices have been shown to be accurate at slow walking speeds, therefore there is still lacking appropriate devices tailored for slow speed ambulation, typical of older adults. This study aimed to assess the validity of step counting using the pedometer function of the ADAMO Care Watch, containing an embedded algorithm for measuring physical activity in older adults., Methods: Twenty older adults aged ≥ 65 years (mean ± SD, 75±7 years; range, 68-91) and 20 young adults (25±5 years, range 20-40), wore a care watch on each wrist and performed a number of randomly ordered tasks: walking at slow, normal and fast self-paced speeds; a Timed Up and Go test (TUG); a step test and ascending/descending stairs. The criterion measure was the actual number of steps observed, counted with a manual tally counter. Absolute percentage error scores, Intraclass Correlation Coefficients (ICC), and Bland-Altman plots were used to assess validity., Results: ADAMO Care Watch demonstrated high validity during slow and normal speeds (range 0.5-1.5 m/s) showing an absolute error from 1.3% to 1.9% in the older adult group and from 0.7% to 2.7% in the young adult group. The percentage error for the 30-metre walking tasks increased with faster pace in both young adult (17%) and older adult groups (6%). In the TUG test, there was less error in the steps recorded for older adults (1.3% to 2.2%) than the young adults (6.6% to 7.2%). For the total sample, the ICCs for the ADAMO Care Watch for the 30-metre walking tasks at each speed and for the TUG test were ranged between 0.931 to 0.985., Conclusion: These findings provide evidence that the ADAMO Care Watch demonstrated highly accurate measurements of the steps count in all activities, particularly walking at normal and slow speeds. Therefore, these data support the inclusion of the ADAMO Care Watch in clinical applications for measuring the number of steps taken by older adults at normal, slow walking speeds.

48. Higher Neuromuscular Manifestations of Fatigue in Dynamic than Isometric Pull-Up Tasks in Rock Climbers.

- Abstract
Neuromuscular assessment of rock climbers has been mainly focused on forearm muscles in the literature. We aimed to extend the body of knowledge investigating on two other upper limb muscles during sport-specific activities in nine male rock climbers. We assessed neuromuscular manifestations of fatigue recording surface electromyographic signals from brachioradialis and teres major muscles, using multi-channel electrode arrays. Participants performed two tasks until volitional exhaustion: a sequence of dynamic pull-ups and an isometric contraction sustaining the body at half-way of a pull-up (with the elbows flexed at 90°). The tasks were performed in randomized order with 10 minutes of rest in between. The normalized rate of change of muscle fiber conduction velocity was calculated as the index of fatigue. The time-to-task failure was significantly shorter in the dynamic (31 ±10 s) than isometric contraction (59 ±19 s). The rate of decrease of muscle fiber conduction velocity was found steeper in the dynamic than isometric task both in brachioradialis (isometric: -0.2 ±0.1%/s; dynamic: -1.2 ±0.6%/s) and teres major muscles (isometric: -0.4±0.3%/s; dynamic: -1.8±0.7%/s). The main finding was that a sequence of dynamic pull-ups lead to higher fatigue than sustaining the body weight in an isometric condition at half-way of a pull-up. Furthermore, we confirmed the possibility to properly record physiological CV estimates from two muscles, which had never been studied before in rock climbing, in highly dynamic contractions.

- Abstract
In order to investigate the behaviour of athletes in choosing sports, we analyse data from part of the We-Sport database, a vertical social network that links athletes through sports. In particular, we explore connections between people sharing common sports and the role of age and gender by applying "network science" approaches and methods. The results show a disassortative tendency of athletes in choosing sports, a negative correlation between age and number of chosen sports and a positive correlation between age of connected athletes. Some interesting patterns of connection between age classes are depicted. In addition, we propose a method to classify sports, based on the analyses of the behaviour of people practising them. Thanks to this brand new classifications, we highlight the links of class of sports and their unexpected features. We emphasise some gender dependency affinity in choosing sport classes.

- Abstract
Aim: To describe the duration and type, as classified by World Health Organization (WHO) criteria, of breastfeeding in Ligurian newborns up to 1 year of age, and to identify possible related factors., Methods: A prospective, observational study on a cohort of 757 women recruited in 10 hospitals in Northern Italy. Women were followed up by means of a questionnaire at discharge and by telephone interviews in the 4th, 12th, 24th and 48th weeks postpartum using the 'recall period' method., Results: At discharge, 97.3% of mothers had started breastfeeding; regional breastfeeding rates at the 4th, 12th, 24th and 48th weeks were respectively 84.4%, 71.2%, 54.4% and 26.3%. Socio-demographic factors did not significantly influence these percentages. Median duration of breastfeeding was 6.5 months. No statistically significant differences were observed in duration of breastfeeding between women who started exclusive breastfeeding (EBF) and those who did predominant breastfeeding (PBF), while the difference was statistically significant when comparing complementary breastfeeding (CBF) with EBF and with PBF., Conclusions: Our study is the first regional experience in the Liguria region and represents a starting point for the diffusion of the culture of breastfeeding. Our results are satisfactory with regards to the overall duration of breastfeeding until the first year of life (26.3%), while duration of EBF (9.5% at the 24th week) is still unsatisfactory according to WHO objectives that include EBF for all children up to the sixth month of life.
