Abstract

Regulation of [K.sub.ATP] channels by [P.sub.2Y] purinoceptors coupled to PI[P.sub.2] metabolism in guinea pig ventricular cells. Am J Physiol Heart Circ Physiol 282: H757-H765, 2002. First published October 11, 2001; 10.1152/ ajpheart.00246.2001.--We used patch-clamp techniques to elucidate the regulatory mechanisms of ATP-sensitive [K.sup.+] ([K.sub.ATP]) channels by stimulation of [P.sub.2] purinoceptors in guinea pig ventricular myocytes. Extracellular ATP at 0.1 mM transiently inhibited by 90.5 [+ or -] 5.0% the whole cell [K.sub.ATP] channel current evoked by a reduction in intracellular ATP concentration to 0.5 mM and exposure to 30 [micro]M pinacidil. ADP and AMP (both 1 mM) also decreased the current by 42.8 [+ or -] 9.3% and 9.4 [+ or -] 4.8%, respectively, but adenosine did not, even at 10 mM. ATP-induced channel inhibition was hardly observed in the presence of 0.2 mM suramin, 0.2 mM guanosine 5'-O-(2-thiodiphosphate), or 0.1 mM compound 48/80, whereas it was not influenced by the presence of 0.1 [micro]M staurosporine or 10 mM 1,2-bis(2-aminophenoxy)ethane-N,N,N',N'-tetraacetic acid in the pipette. In the presence of 10 [micro]M wortmannin or the absence of ATP in the cytosol, the ATP-induced channel inhibition was irreversible. Phosphatidylinositol 4,5-bisphosphate (PI[P.sub.2]) at 0.1 mM in the outside-out patch pipette prevented ATP-induced channel inhibition. The half-maximal internal ATP concentrations for inhibition of channel activity determined in inside-out membrane patches were 13.8 [micro]M in the presence and 1.12 mM in the absence of 0.1 mM ATP at the external side. It is concluded that activity of [K.sub.ATP] channels is modulated by extracellular ATP by a mechanism involving [P.sub.2Y] purinoceptors coupled to GTP-binding proteins associated with reduction of the sarcolemmal PI[P.sub.2] concentration via stimulation of phospholipase C. phosphatidylinositol 4,5-bisphosphate, extracellular adenosine 5'-triphosphate; phosphatidylinositol turnover; phospholipase C

Abstract

To determine whether the administration of an angiotensin-converting enzyme inhibitor (ACEI) affects the renal hemodynamic changes and the renal handling of urinary protein excretion in response to a protein meal ingestion, we examined the changes in creatinine clearance (CCr) and urinary albumin excretion rate (AER) in response to the ingestion of a meal of tuna fish before and after the administration of enalapril 2.5 mg/day for one week in 7 healthy male volunteers. Before treatment with enalapril, CCr 0, 1, 2, and 3 (before and 1, 2, and 3 h after protein loading; mean±SD) were 118.8 ± 21.6, 136.8±21.7, 144.9^* ±23.2 and 135.9 ± 15.3 ml/min/1.73 m^2 (^*: p < 0.05 vs CCr 0), respectively. 20. Influence of Blood Pressure Level on the Suppression of Proteinuria by Dipyridamole in Diabetic Patients with Nephropathy

Abstract

To study the influence of blood pressure on the anti-proteinuric effect of dipyridamole, 14 diabetic patients with proteinuria were divided into two groups: Group A (n=7) in which the mean blood pressure was always >100 mmHg, and Group B (n=7) in which the mean blood pressure might be > 100 mmHg. During the observation period, protein excretions and serum creatinine (Cr) were examined every six months. In Group A, the urinary protein excretion increased during the control period and decreased during dipyridamole therapy. In Group B, the urinary protein excretion increased both during the control period and dipyridamole therapy. △1/Cr was lowered in Group A, but changed rather dramatically in Group B by dipyridamole therapy. We conclude that the anti-proteinuric effect of dipyridamole was achieved in patients with a mean blood pressure of < 100 mmHg.

Abstract

Previously, it has been reported that a low protein diet prevents the progression of diabetic nephropathy. Low protein alimentation can reduce proteinuria in patients with moderate diabetic nephropathy (serum creatinine ≦ 2.0 mg/dl). However, dietary protein restriction may cause muscle wasting or malnutrition. In a previous report, we examined the influence of proteins from different sources on renal function in healthy volunteers and diabetic patients. Our study showed that ingestion of egg white, cheese or cooked soybean protein did not affect GFR or AER (albumin excretion rate). GFR rose significantly and AER did not change after ingestion of tuna fish. Either GFR or AER rose after ingestion of cooked pork or beef. Urinary 6-keto-PGF_1α excetion increased after ingestion of pork, beef or tuna fish. Urinary TXB_2 excretion increased after pork or beef loading. Our findings suggest that certain kinds of protein can be excepted from dietary portein restriction. Our study involved only acute loading, and prolonged clinical study is necessary to evaluate this possibility and safety.

Abstract

Although non-insulin-dependent diabetes mellitus (NIDDM) has been widely thought to be an inheritable disease, the identification of a major gene in NIDDM has not been demonstrated by the classical genetics. One of reasons for the failure to find any clear trend was due to the fact that NDDM is a multifactorial disease, in which several major genes are involved in the onset and progression. This fact was partly demonstrated from monozygotic twin studies. However, we now can have a cue to identify the major genes of NIDDM by virtue of molecular biology (recombinant DNA technology). There are at least three major approach to identify a disease susceptibility gene ; classical approach, reverse genetics (positional cloning) and candidate gene approach. Mutations in insulin gene in patients with abnormal insulinemia were identified by classical approach, in which abnormal insulin was firstly isolated from patients' blood, and secondly, the gene coding the abnormal insulin was isolated by molecular cloning. Positional cloning is a method to identify a disease susceptibility gene based on the linkage analysis between various well-known genetic markers and the disease. By this method, it has been demonstrated that gene for maturity-onset diabetes of the young (MODY), a from of NIDDM with an autosomal dominant inheritance is linked to adenosine deaminase gene locus on chromosome 20q. Candidate gene approach is the method to examine whether the genes which are thought to be involved in the abnormal phenotype of a disease, is related to the disease or not. In 1992, close linkage of glucokinase gene locus on chromosome 7p to MODY of French families has been revealed, and furthermore, non-sense mutation in the glucokinase gene has been identified in one of the families. Abstract

As to effect of glycemic control on diabetic complication, DCCT (Diabetes Control and Care Trial) research group reported that strict glycemic control could prevent and intervent diabetic retinopathy and nephropathy. Importance of blood pressure control is also recognized in prevention of retinopathy and nephropathy, although it has been well recognized in ischemic heart disease. We studied the mechanism of endothelial cell damage in diabetic patients using soluble thrombomodulin (sTM) concentration as endothelial cell marker. No correlation was found between sTM and HbA1c in diabetic patients without any complications, although high sTM concentrations were found in patients with HbA1c of more than 6 %. Short-term glycemic control could not reduce sTM concentration in the diabetic patients. Oral administration of aldose reductase inhibitor, Eparlestat, for four weeks could not reduce sTM in diabetic patients. In conclusin, hyperglycemia itself dose not seem to cause the endothelial cell damage directly in diabetes mellitus.

Abstract

To elucidate the clinical significance of autoantibodies found in Graves' disease, 56 patients with Graves' disease diagnosed between 1986 and 1990 were retrospectively studied. (1) Before medical treatment, 89.1% of patients had TSH binding inhibiting immunoglobulin (TBII), 72.7% had thyroid stimulating antibody (TSAb), 76.7% had antithyroidal microsomal antibody (MCHA) and 18.2% had antithyroglobulin antibody (TGHA). (2) Positive correlation was not found between level of TSAb and free thyroxin (FT_4) concentration, but between level of TBII and FT_4 concentration. (3) The efficacy of antithyroid drugs and the prognosis were not related to the TBII, TSAb and MCHA levels respectively. (4) 68.8% of patients negative for TBII and 72.7% of patients negative for TSAb were non-suppressible by T_3 supression test after long term (more than 1 year) maintenance therapy of anti thyroid drugs. In conclusion, the measurement of TBII and TSAb seems to be useful for diagnosis of Graves' disease, but not for the prediction of the clinical course or prognosis by medical therapy.
