1. Momentum flux fluctuations in wall turbulence formulated along the distance from the wall

Abstract

Wall turbulence has a sublayer where the mean wall-normal flux of the streamwise momentum is constant. Via the law of the wall, this mean flux is related to the wall-normal profile of the mean streamwise velocity. However, the momentum flux has large fluctuations, for which the corresponding law is yet unknown. To formulate such a law, we decompose fluctuations of the streamwise and the wall-normal velocities. These are smoothed to single out a component that would dominate the momentum flux fluctuations. It is dependent on the wall-normal distance. We relate this dependence to the wall-normal profile of the streamwise velocity variance. The resultant law is consistent with laboratory and field data across a wide range of that distance and applies readily to wall modeling of a numerical simulation., Comment: 10 pages, to appear in Physics of Fluids (see https://pubs.aip.org/aip/pof )

However, there is a lack of a comprehensive analysis, including CT evaluation, of clinical outcomes in Japanese patients who have undergone BT for severe asthma. This study aimed to evaluate the impact of BT in Japanese asthma patients, with a focus on the CT metric “WA at Pi10” to assess airway disease.Methods: Twelve patients with severe persistent asthma who underwent BT were assessed using ACQ6, AQLQ, pulmonary function tests, FeNO measurement, blood sampling, and chest CT before BT and one year after the third procedure for the upper lobes.Results: The median age of the patient was 62.0 years, 7/12 (58.3%) were male, 4/12 (33.3%) used regular oral corticosteroids, and 8/12 (66.7%) received biologics. Median FEV1% was 73.6%, and median peripheral eosinophil count was 163.8/μL. After one year of BT, ACQ6 scores improved from 2.4 to 0.8 points (p = 0.007), and AQLQ scores improved from 4.3 to 5.8 points (p < 0.001). Significant improvements were also observed in asthma exacerbations, unscheduled visits due to exacerbations, FeNO, and √WA at Pi10 (p < 0.05). The baseline mucus score on the CT findings was negatively correlated with FEV1 (r = − 0.688, p = 0.013) and with the maximum mid-expiratory flow rate (r = − 0.631, p = 0.028), and positively correlated with the peripheral blood eosinophil count (r = − 0.719, p = 0.008). Changes in √WA at Pi10 after one year were positively correlated with changes in the mucus score (r = 0.742, p = 0.007).Conclusion: This study has limitations, including its single-arm observational design and the small sample size. However, BT led to a symptomatic improvement in patients with severe asthma. Abstract

We report an improved measurement of the degree-scale cosmic microwave background $B$-mode angular-power spectrum over 670 square-degree sky area at 150 GHz with POLARBEAR. In the original analysis of the data, errors in the angle measurement of the continuously rotating half-wave plate, a polarization modulator, caused significant data loss. By introducing an angle-correction algorithm, the data volume is increased by a factor of 1.8. We report a new analysis using the larger data set. We find the measured $B$-mode spectrum is consistent with the $\Lambda$CDM model with Galactic dust foregrounds. We estimate the contamination of the foreground by cross-correlating our data and Planck 143, 217, and 353 GHz measurements, where its spectrum is modeled as a power law in angular scale and a modified blackbody in frequency. Abstract

Within wall turbulence, there is a sublayer where the mean wall-normal flux of the streamwise momentum is constant and related to the logarithmic wall-normal profile of the mean streamwise velocity. This relation, i.e., the law of the wall, has been used to estimate the mean stress at the wall surface. However, the momentum flux exhibits large temporal fluctuations. To relate them theoretically to those of the streamwise velocity at the same position from the wall, we consider an orthogonal decomposition of the fluctuations on a plane of the streamwise and wall-normal velocities. Since a large timescale is expected for the component that would dominate the momentum flux, it is singled out by temporal smoothing. The resultant formula is consistent with time-series data of a boundary layer in a wind tunnel. No specific safety issues were found and no cases of concern for drug dependence identified. Efficacy (itch improvement) was demonstrated in 73.16% (815/1114 cases; 12-week analysis set) and in 85.67% (520/607; general assessment of itch improvement at 1-year analysis set). A significant difference was found in 4 items of itch improvement at 12 weeks and 8 items of itch improvement at 1 year. No noteworthy issues were identified. Mean Visual Analog Scale (VAS) values after 12 weeks and 1 year after the first dose were significantly lower than the baseline (p < 0.0001 for both treatment durations). Mean severity scores (Kawashima’s classification scheme) were significantly lower than the pretreatment score at 12 weeks and 1 year after the first dose (both p < 0.0001). No concerns were identified in the efficacy and safety of nalfurafine in patients with specific background, ie, the elderly (aged ≥ 65 years), those with renal impairment, and those on long-term treatment (≥ 365 days) compared with patients without corresponding background.Conclusion: No new safety issues of concern or cases of insufficient efficacy were identified in this Specific Drug Use Survey of the safety and efficacy of nalfurafine for the improvement of pruritus in patients with chronic liver diseases.Keywords: post-marketing surveillance, safety, efficacy, nalfurafine, pruritus, chronic liver disease

13. Self-organized hierarchical structure in a plastic network of chaotic units

Abstract

Formation of a layered structure is studied in a globally coupled map of chaotic units with a plastic coupling strength that changes depending on the states of units globally and an external input. In the parameter region characterized by weakly chaotic and desynchronized dynamics, units spontaneously form a hierarchical structure due to the influence of the input, with regards to the coupling strength. This hierarchical structure is not fixed in time, and is successively reorganized. It is found that the distribution of lifetimes of the structure obeys a power law. Abstract

The SARS-CoV-2 Omicron BA.1 variant emerged in 20211 and has multiple mutations in its spike protein2. Here we show that the spike protein of Omicron has a higher affinity for ACE2 compared with Delta, and a marked change in its antigenicity increases Omicron’s evasion of therapeutic monoclonal and vaccine-elicited polyclonal neutralizing antibodies after two doses. mRNA vaccination as a third vaccine dose rescues and broadens neutralization. Importantly, the antiviral drugs remdesivir and molnupiravir retain efficacy against Omicron BA.1. Replication was similar for Omicron and Delta virus isolates in human nasal epithelial cultures. However, in lung cells and gut cells, Omicron demonstrated lower replication. Omicron spike protein was less efficiently cleaved compared with Delta. The differences in replication were mapped to the entry efficiency of the virus on the basis of spike-pseudotyped virus assays. The defect in entry of Omicron pseudotyped virus to specific cell types effectively correlated with higher cellular RNA expression of TMPRSS2, and deletion of TMPRSS2 affected Delta entry to a greater extent than Omicron. Furthermore, drug inhibitors targeting specific entry pathways3 demonstrated that the Omicron spike inefficiently uses the cellular protease TMPRSS2, which promotes cell entry through plasma membrane fusion, with greater dependency on cell entry through the endocytic pathway. Consistent with suboptimal S1/S2 cleavage and inability to use TMPRSS2, syncytium formation by the Omicron spike was substantially impaired compared with the Delta spike. The less efficient spike cleavage of Omicron at S1/S2 is associated with a shift in cellular tropism away from TMPRSS2-expressing cells, with implications for altered pathogenesis.

34. The effects of the online gatekeeping using search-based advertising on users’ suicidal ideation

Abstract

Background: Recently, the practice of “online gatekeeping” related to suicide prevention—placing online advertisements directed at users who search the web for suicide-related terms and leading them to email-based consultations—has been implemented. However, the effectiveness of this practice has not been examined. Aim: To quantitatively examine the effectiveness of suicide-related online gatekeeping. Methods: This was a non-randomized, single-arm study of online gatekeeping among Japanese Internet users. Outcomes were measured at the beginning of the use of the consultation service (T1) and approximately four weeks later (T2). The primary outcome measured was suicidal ideation. Results: In total, 167 participants completed two assessment surveys; approximately 80% were women, and 59.3% were young people (in their twenties or younger). Data analysis of 167 online gatekeeping service users showed that the mean suicidal ideation at T2 was statistically significantly lower than that at T1, and its effect size was small to medium (d = 0.38). Limitations: This study used a single-arm design with no control group. Additionally, the study did not include a second follow-up period. Conclusions: Online gatekeeping is a promising suicide-prevention tool, although further research is warranted to examine its effectiveness using a randomized controlled trial.
