A szelektív epitympanalis dysventilatiós szindróma a cholesteatomaképződés egy új elmélete, melynek leírását a néhány éve megjelent új, minimálinvazív műtéti technika, az endoszkópos fülsebészet (transcanal endoscopic ear surgery) tette lehetővé. Az endoszkópos fülsebészeti részletes anatómiai ismeretek biztosítottak lehetőséget az elméletet magyarázó új patofiziológiai tényező beazonosítására, mely a középfül ventilatiós útjainak elzáródása. Jól működő fülkürt mellett ezen ventilatiós útvonalak elzáródásának következtében kialakulhat a szelektív epitympanalis dysventilatiós szindróma. Célunk volt felhívni a gyakorló fülsebészek figyelmét a kórképre, mivel a megfelelő diagnózis felállítását követően endoszkópos technikával visszafordítható vagy akár megelőzhető a cholesteatomaképződés, a következményes életminőség-rosszabbodást okozó fülváladékozás és vezetéses halláscsökkenés. A szindrómát saját készítésű anatómiai grafikai ábráink által tesszük még inkább átláthatóvá, érthetővé. A jelen közleményben a szerzők a nemzetközi irodalom áttekintése mellett saját esetbemutatásukon keresztül ismertetik a szelektív epitympanalis dysventilatiós szindróma tüneteit, felismerésének fontosságát és endoszkópos fülsebészeti megoldását. Purpose Single-sided deaf patients following cochlear implantation often compare the sound quality of their implanted ear with normal hearing. The interaural differences can result in dissatisfaction with speech comprehension and reduced time of usage of the speech processor; hence, prolonging auditory adaptation time. The proposed calibration method presented in this study demonstrates how the frequency distribution of the cochlear implant can be set to adequately approximate the pitch perception of the contralateral normal hearing ear towards improving speech intelligibility in a noisy environment. Methods In 12 postlingual single-sided deaf patients, subjective interaural pitch-matching was carried out to determine new central frequencies for the reallocation of the frequency bands of their speech processor (CP910, CP950 or CP1000, Cochlear, Australia). The patients were asked to compare the pitch of the tones presented to their normal hearing ear to the pitch of individual channels of their cochlear implant (CI522 or CI622, Cochlear, Australia). A third-degree polynomial curve was fit to the acquired matching frequencies to create the new frequency allocation table. Audiological measurements (free-field aided thresholds, speech reception thresholds, and monosyllabic word recognition score) in noise, together with a Speech, Spatial and Qualities of Hearing Scale (SSQ12) questionnaire (short version of the original SSQ) results were evaluated prior to the pitch-matching procedure, and again, 2 weeks later. Results The free-field aided thresholds of the patients showed no greater shift than ± 5 dB following the procedure; however, their monosyllabic word recognition score in noise improved significantly (mean − 9.58%, SD 4.98%, matched pairs t test comparison: p 0.001). The results of the SSQ12 questionnaire also showed significant improvement in speech intelligibility, sound localization, and sound quality (mean 0.96 points, SD 0.45 points, matched pairs t test comparison: p 0.001). Conclusions Matching the pitch perception of the implanted cochlea with the sensation of the normal hearing contralateral ear, resulted in significant changes in the quality of hearing in patients with single-sided deafness. It is plausible the procedure can usher positive results in bimodal patients or following sequential bilateral cochlear implantation.

Mastoid obliteration technique combines the advantages of canal wall-up (CWU) and canal wall-down (CWD) approaches in the surgery of chronic suppurative otitis media with cholesteatoma. We aim to demonstrate our experience with mastoid obliteration technique using bone dust and BonAlive® (S53P4) bioactive glass granule in a comparative prospective clinical study.Between 1st of March 2012 and 31st of November 2021, mastoid obliteration surgery was performed in 14 patients using bone dust (7 cases) and BonAlive® granule (7 cases). Prior to these interventions, the patients had undergone more than three ear surgeries (CWU and CWD) generally in both groups. Changes of complaints, audiological results, and changes in quality of life were analysed in both groups, postoperatively.Having performed the mastoid obliteration technique, cochlear damage did not occur in either patient group. Long-term ear discharge and vertigo were occasionally observed after performing obliteration with bone dust. However, these complaints disappeared after a while. Complications were not reported in the case of obliteration with BonAlive®. Outstanding improvement was experienced in both groups.In our practice, mastoid obliteration surgery, using either bone dust or BonAlive® granule, has proved to be a safe and effective method in the management of chronic suppurative otitis media with cholesteatoma, resulting in continuous putrid ear discharge after CWU or CWD tympanoplasty. Obliteration with BonAlive® granule provides several advantages for patients, such as antibacterial effect, osteoconductive effect ensuring frame for bone growth and osteoproductive effect stimulating the ossification. Orv Hetil. 2022; 163(21): 838-845.Bevezetés és célkitűzés: Az otitis media suppurativa chronica cholesteatomatosa sebészetében a mastoidobliterációval végzett megoldás egyesíti a nyitott és a zárt technikával végzett műtétek előnyeit. Összehasonlító prospektív klinikai tanulmányunkban a csontporral és a BonAlive® (S53P4) bioaktív üveggranulátummal végzett mastoidobliterációs műtéteinkkel szerzett tapasztalatainkat összegeztük. Beteganyag és módszerek: 2012. március 1. és 2021. november 31. között 14 felnőtt betegünknél végeztünk mastoidobliterációs műtétet csontporral (7 beteg), illetve BonAlive® granulátummal (7 beteg). A mastoidobliterációt megelőzően a betegek mindkét csoportban átlagosan több mint 3 fülműtéten (nyitott vagy zárt technikájú tympanoplastica) estek át. A műtéteket követően mindkét csoportban elemeztük a panaszok változását, az audiológiai eredmények alakulását és a betegek életminőségében bekövetkezett változásokat. Eredmények: A mastoidobliterációs műtéti megoldás egyik betegcsoportban sem okozott cochlearis károsodást. A csontporral történt obliterációt követően gyakrabban tapasztaltunk elhúzódó fülváladékozást és esetenként szédülést, mely panaszok idővel megszűntek. A BonAlive® granulátummal obliterált esetekben komplikációt nem észleltünk. Mindkét betegcsoportban igen jelentős életminőség-javulást figyeltünk meg. Következtetés: A mastoidobliterációs műtétek – akár csontporral, akár BonAlive® granulátummal végezve – gyakorlatunkban biztonságos és hatásos megoldásnak bizonyultak korábban otitis media suppurativa chronica cholesteatomatosa miatt akár nyitott, akár zárt technikával végzett, állandó bűzös fülváladékozással járó esetekben. A BonAlive® granulátummal végzett obliterációk számos előnnyel járnak a betegek számára: az üveggranulátum antibakteriális hatású, oszteokonduktív hatású (keretet biztosít a csont növekedéséhez), és oszteoproduktív hatást is kivált (stimulálja a csontosodást). Purpose Endoscopic type I tympanoplasty was originally introduced in the 1990s and the extensive spread of this practice can be easily observed. The conventional technique performed involves the repair of a tympanic membrane perforation and is defined as microscopic type I tympanoplasty. The aim of this study is the comparison of quality-of-life (QoL) outcomes with endoscopic to that with microscopic type I tympanoplasty. Methods All patients, or in the case of children with the aid of a parent, were asked to complete a novel QoL questionnaire drafted by our study group. The analysis was performed with descriptive statistics—mean, SD and relative frequency—and with a mixed model (generalized least squares fit). A two-sided p value of Results A total of 83 patients completed the questionnaire, 38 in the endoscopic group and 45 in the microscopic group. Every question represented a different. A statistically significant result was found in favor of the endoscopic approach regarding average hospitalization rate (p = 0.003) and cosmetic outcomes (p = 0.015). No statistically significant difference was otherwise observed between the groups. Conclusions Based on our prospective cohort study, the QoL outcomes of endoscopic type I tympanoplasty in terms of postoperative pain, headache, nausea, vomiting, dizziness, taste disorder and hearing were comparable to the microscopic type I tympanoplasty. In regard to cosmetics, an increase in desirable results was achieved in the endoscopic group, particularly the average hospitalization rate proved to be statistically significantly lower than in the microscopic group.

We argue that two seemingly different phenomena, namely the well-known saturation of the Hagedorn exponential distribution and the less familiar saturation of Regge trajectories at resonance masses $m\approx$ 2-2.5 GeV are related and have the same origin: quark deconfinement. We show that the slope of the real part of non-linear Regge trajectories determines the prefactor $f(m)$ in Hagedorn's resonance mass density distribution $\rho(m)$. Összefoglaló. A hirtelen halláscsökkenés patofiziológiája még nagyrészt tisztázatlan, így oki terápia nem lehetséges. Az elsődleges kezelést a helyileg vagy szisztémásan adott kortikoszteroid jelenti, egységes protokoll azonban nem áll rendelkezésre. Nagy vagy súlyos fokú hirtelen halláscsökkenés esetén kóroki tényezőként felmerül a perilymphafistula lehetősége még azoknál a betegeknél is, akiknél nem szerepel trauma az anamnézisben. A kórkép műtéti kezelése a dobüreg feltárását követően a belső fül ablakainak obliterálása. Amennyiben ez a megoldás nem eredményez megfelelő hallásjavulást, hagyományos vagy implantálható hallókészülékek alkalmazása javasolt. A közleményben részletezett esetünkben teljes siketséggel járó, jobb oldali hirtelen halláscsökkenés alakult ki, melynek hátterében egyértelmű okot azonosítani nem sikerült. Az eredménytelen kombinált, intratympanalis és szisztémás szteroidkezelést követően exploratív tympanotomiát végeztünk, melynek során a belső fül ablakait obliteráltuk. Hallásjavulást ezt követően sem sikerült kimutatni, így cochlearis implantáció elvégzése mellett döntöttünk. Az implantációt a kerek ablakon keresztül végeztük, mely alapján kijelenthetjük, hogy az előzetes kerekablak-obliteráció nem zárja ki a későbbi cochlearis implantációt. Orv Hetil. 2021; 162(51): 2055–2060. Summary. The pathophysiology of sudden sensorineural hearing loss is mainly unknown, therefore no causative treatment exists. Systemic and local administration of corticosteroids serves as first line therapy although protocols vary. In cases of severe or profound hearing loss with no improvement for medical therapy, perilymphatic fistulae can be assumed even without any history of trauma. Therefore, inner ear window obliteration as a primary surgical option in the early stage can be considered. For patients without complete recovery, conventional hearing aids or implantable hearing devices can be offered. In our case report, we present a patient with right sided idiopathic sudden deafness. After failure of conservative combined intratympanic and systemic steroid therapy, explorative tympanotomy and obliteration of the inner ear windows were performed. As no hearing improvement was witnessed, successful cochlear implantation via round window insertion was performed. Our case justifies that obliterating the round window membrane does not rule out further successful cochlear implantation. Orv Hetil. 2021; 162(51): 2055–2060.

Összefoglaló. A Vibrant Soundbridge aktív középfül-implantátum vezetéses, sensorineuralis és kevert típusú halláscsökkenés esetén is megoldást jelenthet a beteg hallásrehabilitációja során. Sensorineuralis halláscsökkenés esetén akkor indikálható, ha a légvezetéses hallásküszöb nem haladja meg a 80–85 dBHL-t a 3–4 kHz frekvenciákon, vezetéses, illetve kevert típusú halláscsökkenés esetén pedig akkor, ha a csontvezetéses hallásküszöb nem haladja meg a 45–65 dBHL-t a 0,5–4 kHz közti frekvenciatartományban. Az implantátum beültetését eleinte tisztán sensorineuralis halláscsökkenés esetén végezték, és csupán egyféleképpen történhetett: a rezgéskeltő rendszer oldalán lévő rögzítőcsipeszt rá kellett applikálni az incus hosszú nyújtványára. Azokra az esetekre, amikor a rezgéskeltő rögzítése nem kivitelezhető, különböző rögzítőelemeket (coupler) fejlesztettek ki. Az incusra való rögzítés hosszúnyújtvány-couplerrel, illetve rövidnyújtvány-couplerrel lehetséges. Vezetéses és kevert típusú halláscsökkenés esetén a kerek ablak membránjához is illeszthető a rendszer, előrehaladott otosclerosis esetén pedig a Soundbridge-implantáció stapedotomiával kiegészített változata („power stapes”) hozhat kielégítő halláseredményt. Ezek a technikák meglehetősen megnövelték a sebész szabadságát, így széles körben alkalmazott, megbízható megoldássá váltak. A Pécsi Tudományegyetem Fül-Orr-Gégészeti és Fej-Nyaksebészeti Klinikáján az elmúlt évtizedben számos incusvibroplastica történt, de kerekablak-vibroplasticára is több alkalommal sor került. A jelen összefoglaló tanulmányban a Vibrant Soundbridge implantátum technikai fejlődésének történeti áttekintésén felül az alkalmazható műtéti megoldásokat mutatjuk be. The Vibrant Soundbridge active middle ear implant can provide a reliable solution for hearing rehabilitation of patients with conductive, sensorineural or mixed hearing loss. For sensorineural hearing loss, the air conduction threshold of the patient should not be more increased than 80–85 dBHL at the range of 3–4 kHz, and for conductive and mixed hearing loss, the bone conduction threshold should not be more increased than 45–65 dBHL between 0.5 and 4 kHz. The standard surgical procedure was originally designed for purely sensorineural hearing loss, and the fixation clip of the vibrating transducer needed to be crimped onto the long process of the incus. In many cases, it is impossible to crimp the vibrator onto the incus. In order to solve such circumstances, fixation clips (couplers) have been developed. There are two options to crimp the device on the incus: applying a long process coupler or a short process coupler. For conductive or mixed hearing loss, a round window soft coupler has been introduced. In advanced otosclerotic cases, a special combined technique of Soundbridge implantation with simultaneous stapedotomy can result in sufficient hearing rehabilitation. These techniques significantly broadened the scale of possibilities for the implantation, therefore, it became a widely utilized, reliable procedure. At the Department of Otorhino-laryngology, Clinical Center, University of Pécs, in addition to a noteworthy amount of incus vibroplasty, several cases of round window vibroplasty have also been performed. ObjectiveTo statistically analyse the hearing thresholds of two cohorts undergoing stapedotomy for otosclerosis with two different prostheses.MethodA retrospective study was conducted comparing NiTiBOND (n = 53) and Nitinol (n = 38) prostheses.ResultsAverage follow-up duration was 4.1 years for NiTiBOND and 4.4 years for Nitinol prostheses. The post-operative air–bone gap was 10 dB or less, indicating clinical success. The p-values for differences between (1) pre- and post-operative values in the NiTiBOND group, (2) pre- and post-operative values in the Nitinol group, (3) pre-operative values and (4) post-operative values in the two groups were: air–bone gap – p < 0.001, p < 0.001, p = 0.631 and p = 0.647; four-frequency bone conduction threshold – p = 0.076, p = 0.129, p < 0.001 and p = 0.005; four-frequency air conduction threshold – p < 0.001, p < 0.001, p = 0.043 and p = 0.041; three-frequency (1, 2 and 4 kHz) bone conduction threshold pre-operatively – p = 0.639, p = 0.495, p = 0.001 and p = 0.01; and air conduction threshold at 4 kHz: – p < 0.001, p < 0.001, p = 0.03 and p = 0.058.ConclusionPost-operative audiological outcomes for NiTiBOND and Nitinol were comparable.

The selective epitympanic dysventilation syndrome is a new theory of the cholesteatoma formation, the description of which was allowed by a minimally invasive surgical technique – the transcanal endoscopic ear surgery – some years ago. The transcanal endoscopic ear surgery provided the possibility of a certain amount of detailed anatomical knowledge to identify the new factor behind this process, namely obstruction of the ventilation pathways of the middle ear. Our goal was to draw the ear surgeons’ attention to this clinical picture as cholesteatoma development and subsequent worsening of the quality of life caused by ear discharge and conductive hearing loss may be reversible or preventable. We make the syndrome more understandable by using our own anatomical drawings. Absztrakt: A pars petrosa cholesteatoma egy lassan növekvő, a temporalis csontot roncsoló, elhúzódóan tünetmentes kórkép. A diagnózis felállítása gyakran késlekedik, hiszen ritka volta miatt még a tapasztalt klinikusok sem gondolnak rá időben. A végső diagnózist jellemzően számos eredménytelen középfülsebészeti beavatkozás előzi meg. A modern radiológiai módszerek nemcsak a diagnózist, hanem a betegek hosszú távú követését is lehetővé teszik. Retrospektív klinikai tanulmányunkban a pars petrosa cholesteatomák sikeres műtéti kezelésére kívánjuk felhívni a figyelmet, hazai beteganyagon első alkalommal elemezve a műtéti megoldások eredményességét. Kezdeti tapasztalatainkat a nemzetközi irodalomból nyerhető adatokkal hasonlítottuk össze. 2015 januárja és 2020 januárja között 6 beteget (4 férfi, 2 nő) kezeltünk különböző lokalizációjú pars petrosa cholesteatomával. Valamennyi beteg a cholesteatoma lokalizációjának megfelelő agyalapi sebészeti beavatkozáson esett át. A betegek átlagos követési ideje 2,7 év volt. A betegek átlagéletkora 47 év volt. A leggyakoribb panaszok a halláscsökkenés, gennyes fülfolyás, szédülés, fülzúgás voltak. Egy beteg már a felvételekor a komplett facialis paralysis tüneteit mutatta. A Sanna-féle osztályozás szerint 2-2 betegünk szenvedett supralabyrinthaer, illetve apicalis, 1-1 beteg pedig masszív, illetve infralabyrinthaer pars petrosa cholesteatomában. Komplikáció sem a peri-, sem a posztoperatív időszakban nem fordult elő. Egy betegnél észleltük a residualis betegség radiológiai jeleit. A pars petrosa cholesteatoma diagnózisa és terápiája komoly nehézséget jelent még a tapasztalt klinikusok számára is. A korai diagnózist a nem jellegzetes tünettan ismerete és a modern képalkotó modalitások teszik lehetővé. A sziklacsontot roncsoló patológiás folyamat eltávolítására korszerű agyalapi sebészeti módszerek állnak rendelkezésre. Absztrakt: Bevezetés: A subtotalis petrosectomia (STP) évtizedek óta ismert, de túlzott radikalitása és a hallásra gyakorolt kedvezőtlen hatása miatt a közelmúltig szinte feledésbe merült műtét a fülsebészet és az agyalapi sebészet határán. Az elmúlt évtizedben ez a beavatkozás számos előnye, valamint az új hallásrehabilitációs módszerekkel való kombinálhatósága miatt újra előtérbe került, hiszen számos problémás, a középfület érintő megbetegedésre nyújt végleges megoldást. Célkitűzés: Retrospektív klinikai tanulmányunkban a STP eredményességére kívántuk felhívni a figyelmet, hazai beteganyagon első alkalommal elemezve a műtét eredményességét, közreadva eddigi tapasztalainkat. Módszer: 44 betegünkön elvégzett 45 műtét adatait dolgoztuk fel. A sokrétű műtéti indikáció bemutatása mellett áttekintjük a műtét egyes lépéseit, elemezzük az audiológiai eredményeket – kitérve a különféle hallásrehabilitációs módszerekre is –, s elemezzük az előfordult komplikációkat is. Eredményeinket a nemzetközi szakirodalom tükrében is górcső alá vesszük. Eredmények: 44 betegünk közül 23 volt nő, 21 férfi. A betegek átlagéletkora 44,6 ± 20,5 év volt, átlagos követési idejük pedig 23 ± 16 hónap. Betegeink közül 25 (57%) korábban legalább két sikertelen fülműtéten esett át, 6 beteg (14%) a műtétet megelőzően már siket volt. A leggyakoribb indikáció a krónikus cholesteatomás otitis media volt. 13 betegen 14 cochlearis implantációt (23%) végeztünk, ezenfelül 6 alkalommal csontvezetéses implantátumot (BAHA) (14%) helyeztünk be; 2 betegnél (5%) kerekablak-VSB (Vibrant Soundbridge)-implantációra került sor. Komplikáció 11 betegünknél jelentkezett, ezek közül a leggyakrabban a liquorfistula (5 eset, 1%) és a sebszétválás (3 eset, 7%) fordult elő. Az audiológiai eredmények feldolgozására 3 betegünk adatai álltak rendelkezésre. Következtetés: A STP rendkívül hasznos műtéti megoldás számos, korábban jelentős problémát okozó fülészeti kórképben. Egyre növekvő népszerűségének hátterében az áll, hogy kombinálható számos modern, új hallásrehabilitációs módszerrel. Orv Hetil. 2020; 161(14): 544–553.

The aim of this study was to visualize the tumor propagation and surrounding mucosal field in radiography-based 3D model for advanced stage HNSCC and combine it with HPV genotyping and miRNA expression characterization of the visualized area. 25 patients with T1-3 clinical stage HNSCC were enrolled in mapping biopsy sampling. Biopsy samples were evaluated for HPV positivity and miR-21-5p, miR-143, miR-155, miR-221-5p expression in Digital Droplet PCR system. Significant miRNA expression differences of HPV positive tumor tissue biopsies were found for miR-21-5p, miR-143 and miR-221-5p compared to the HPV negative tumor biopsy series. Peritumoral mucosa showed patchy pattern alterations of miR-21-5p and miR-155 in HPV positive cases, while gradual change of miR-21-5p and miR-221-5p was seen in HPV negative tumors. In our study we found differences of the miRNA expression patterns among the HPV positive and negative tumorous tissues as well as the surrounding mucosal fields. The CT based 3D models of the cancer field and surrounding mucosal surface can be utilized to improve proper preoperative planning. Complex evaluation of HNSCC tissue organization field can elucidate the clinical and molecular differentiation of HPV positive and negative cases, and enhance effective organ saving therapeutic strategies.

OBJECTIVE Compare intraoperative and postoperative outcomes of endoscopic and microscopic stapes surgery to provide objective evidence on whether the former is a better alternative than the latter. METHODS We performed a systematic review and meta-analysis for studies that compared endoscopic stapes surgery with microscopic stapes surgery. Only studies that met predetermined criteria were selected and assessed for bias and quality. Primary outcomes were postoperative air-bone gap (ABG) and chorda tympani nerve injury. Secondary outcomes were average operating time, tympanic membrane (TM) perforation, and postoperative taste disturbance, pain, and dizziness. We calculated pooled odds ratios (ORs) with 95% confidence intervals (CIs) for dichotomous outcomes and weighted mean difference (WMD) with 95% CI for continuous outcomes. A confidence interval starting above 1.0 was considered as statistically significant. I2 and χ2 tests were used to quantify statistical heterogeneity. We used funnel plots to look for publication bias and performed a sensitivity analysis. RESULTS Six nonrandomized cohort studies were eligible. The primary outcomes were ABG 20 dB: OR = 2.51 (95% CI: 0.77 to 8.22), and chorda tympani injury: OR = 3.51 (95% CI: 1.55 to 7.93). Secondary outcomes were taste: OR = 2.36 (95% CI: 1.01 to 5.51), average operation time: WMD = 0.14 (95% CI: -11.69 to 11.98), TM perforation: OR = 1.70 (95% CI: 0.44 to 6.58); pain: OR = 0.84 (95% CI: 0.36 to 1.96), and dizziness: OR = 2.15 (95% CI: 0.94 to 4.89). Összefoglaló. A hirtelen halláscsökkenés patofiziológiája még nagyrészt tisztázatlan, így oki terápia nem lehetséges. Az elsődleges kezelést a helyileg vagy szisztémásan adott kortikoszteroid jelenti, egységes protokoll azonban nem áll rendelkezésre. Nagy vagy súlyos fokú hirtelen halláscsökkenés esetén kóroki tényezőként felmerül a perilymphafistula lehetősége még azoknál a betegeknél is, akiknél nem szerepel trauma az anamnézisben. A kórkép műtéti kezelése a dobüreg feltárását követően a belső fül ablakainak obliterálása. Amennyiben ez a megoldás nem eredményez megfelelő hallásjavulást, hagyományos vagy implantálható hallókészülékek alkalmazása javasolt. A közleményben részletezett esetünkben teljes siketséggel járó, jobb oldali hirtelen halláscsökkenés alakult ki, melynek hátterében egyértelmű okot azonosítani nem sikerült. Az eredménytelen kombinált, intratympanalis és szisztémás szteroidkezelést követően exploratív tympanotomiát végeztünk, melynek során a belső fül ablakait obliteráltuk. Hallásjavulást ezt követően sem sikerült kimutatni, így cochlearis implantáció elvégzése mellett döntöttünk. Az implantációt a kerek ablakon keresztül végeztük, mely alapján kijelenthetjük, hogy az előzetes kerekablak-obliteráció nem zárja ki a későbbi cochlearis implantációt. Orv Hetil. 2021; 162(51): 2055-2060. Summary. The pathophysiology of sudden sensorineural hearing loss is mainly unknown, therefore no causative treatment exists. Systemic and local administration of corticosteroids serves as first line therapy although protocols vary. In cases of severe or profound hearing loss with no improvement for medical therapy, perilymphatic fistulae can be assumed even without any history of trauma. Therefore, inner ear window obliteration as a primary surgical option in the early stage can be considered. For patients without complete recovery, conventional hearing aids or implantable hearing devices can be offered. In our case report, we present a patient with right sided idiopathic sudden deafness. After failure of conservative combined intratympanic and systemic steroid therapy, explorative tympanotomy and obliteration of the inner ear windows were performed. As no hearing improvement was witnessed, successful cochlear implantation via round window insertion was performed. Background Endoscopic type I tympanoplasty was originally introduced in the 1990s, and the extensive spread of this practice can be easily observed. The conventional technique performed involves the repair of a tympanic membrane perforation, and is defined as microscopic type I tympanoplasty. Objective of review The aim of this study was the comparison of postoperative outcomes of both the endoscopic and the microscopic type I tympanoplasty. Type of review We conducted a meta-analysis in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-analysis (PRISMA) guidelines. Search strategy A systematic literature search was performed in the databases of PubMed, Embase, Cochrane Library, Clarivate Analytics-Web of Science, ClinicalTrials.gov, World Health Organization Library, and Scopus by inserting, 'myringoplasty OR (tympanoplasty AND perforation)' into the search query. We applied only a 'human' filter. We excluded non-English studies. Additional records were identified by checking the references of relevant studies. Evaluation method Comparative studies were included in our analysis. We calculated the pooled odds ratio (OR) with 95% confidence interval (CI) for dichotomous outcomes and weighted mean difference (WMD) with a 95% CI for continuous outcomes. Additionally, we assessed the risk of bias and estimated the quality of evidence for each outcome. Results Our systematic search yielded 16 studies (involving 1179 interventions), eligible for analysis. The pooled graft uptake rate (OR: 1.21, CI: 0.82-1.77; I2 = 0.0%), the postoperative hearing results (WMD = -1.13; 95% CI: -2.72-0.45; I2 = 78.1%) and the operation time (WMD = -21.11; 95% CI: -42.60-0.38; I2 = 99.3%), were all comparable amongst the two techniques. In contrast, the endoscopic type I tympanoplasty outperforms when regarding the pooled canaloplasty rate (OR = 7.96; 95% CI: 4.30-14.76; I2 = 0.0%, P = 1.000) and features an increase in desirable cosmetic results (OR = 19.29; 95% CI: 11.37-32.73; I2 = 0.0%, P = 0.839), when compared with the microscopic approach. Conclusions Based on our meta-analysis, the surgical outcomes of endoscopic type I tympanoplasty in terms of graft uptake rate, postoperative hearing results and operation time were comparable to the microscopic type I tympanoplasty. In regards to cosmetics, an increase in desirable results was achieved in the endoscopic group, particularly the incidence of canaloplasty which proved to be significantly lower.

In the framework of a Regge-pole model, we discussthe forward phase of the proton-proton elastic scattering amplitude (ρ(13 TeV) = 0.1 ± 0.01), recently measured at the LHC. Our model includesa dipole pomeron, odderon and secondary reggeon exchanges. It accommodetes forthe low- |t| «break» (departure from the exponential behavior of the diffraction cone), the accelerating rise with energy of the forward slope B(s, t = 0) and absence of secondary dips and bumps. Contrary to the majority of Regge-pole models, our approach is based on he use of a dipole (double pole) pomeron (DP). It has a number of advantages over the conventional simple pole pomeron: DP generates rising cross sections even at unit pomeron intercept and, what is even more important, it produces a single diffraction minimum and maximum in the differential cross section, in accord with the experimental data. Relative contributions from different components to the scattering amplitude are evaluated from the fitted model.

Összefoglaló. A Vibrant Soundbridge aktív középfül-implantátum vezetéses, sensorineuralis és kevert típusú halláscsökkenés esetén is megoldást jelenthet a beteg hallásrehabilitációja során. Sensorineuralis halláscsökkenés esetén akkor indikálható, ha a légvezetéses hallásküszöb nem haladja meg a 80-85 dBHL-t a 3-4 kHz frekvenciákon, vezetéses, illetve kevert típusú halláscsökkenés esetén pedig akkor, ha a csontvezetéses hallásküszöb nem haladja meg a 45-65 dBHL-t a 0,5-4 kHz közti frekvenciatartományban. Az implantátum beültetését eleinte tisztán sensorineuralis halláscsökkenés esetén végezték, és csupán egyféleképpen történhetett: a rezgéskeltő rendszer oldalán lévő rögzítőcsipeszt rá kellett applikálni az incus hosszú nyújtványára. Azokra az esetekre, amikor a rezgéskeltő rögzítése nem kivitelezhető, különböző rögzítőelemeket (coupler) fejlesztettek ki. Az incusra való rögzítés hosszúnyújtvány-couplerrel, illetve rövidnyújtvány-couplerrel lehetséges. Vezetéses és kevert típusú halláscsökkenés esetén a kerek ablak membránjához is illeszthető a rendszer, előrehaladott otosclerosis esetén pedig a Soundbridge-implantáció stapedotomiával kiegészített változata ("power stapes") hozhat kielégítő halláseredményt. Ezek a technikák meglehetősen megnövelték a sebész szabadságát, így széles körben alkalmazott, megbízható megoldássá váltak. A Pécsi Tudományegyetem Fül-Orr-Gégészeti és Fej-Nyaksebészeti Klinikáján az elmúlt évtizedben számos incusvibroplastica történt, de kerekablak-vibroplasticára is több alkalommal sor került. A jelen összefoglaló tanulmányban a Vibrant Soundbridge implantátum technikai fejlődésének történeti áttekintésén felül az alkalmazható műtéti megoldásokat mutatjuk be. The Vibrant Soundbridge active middle ear implant can provide a reliable solution for hearing rehabilitation of patients with conductive, sensorineural or mixed hearing loss. For sensorineural hearing loss, the air conduction threshold of the patient should not be more increased than 80-85 dBHL at the range of 3-4 kHz, and for conductive and mixed hearing loss, the bone conduction threshold should not be more increased than 45-65 dBHL between 0.5 and 4 kHz. The standard surgical procedure was originally designed for purely sensorineural hearing loss, and the fixation clip of the vibrating transducer needed to be crimped onto the long process of the incus. In many cases, it is impossible to crimp the vibrator onto the incus. In order to solve such circumstances, fixation clips (couplers) have been developed. There are two options to crimp the device on the incus: applying a long process coupler or a short process coupler. For conductive or mixed hearing loss, a round window soft coupler has been introduced. In advanced otosclerotic cases, a special combined technique of Soundbridge implantation with simultaneous stapedotomy can result in sufficient hearing rehabilitation. These techniques significantly broadened the scale of possibilities for the implantation, therefore, it became a widely utilized, reliable procedure. At the Department of Otorhino-laryngology, Clinical Center, University of Pécs, in addition to a noteworthy amount of incus vibroplasty, several cases of round window vibroplasty have also been performed. The aim of the present study is to summarize the history of development of the Vibrant Soundbridge and to present an overview of the applicable surgical techniques. Herpes zoster (HZ) causes considerable pain and distress, and γ-Aminobutyric acid (GABA) and its derivatives are assumed to control this, but the available data are inconsistent. This meta-analysis and systematic review aimed to assess the effectiveness of GABA derivatives in the prevention of acute herpetic pain. The metaanalysis was conducted following the PRISMA guidelines using PICO format, registered in PROSPERO number CRD42018095758. PubMed, Web of Science, Ovid, Scopus, and EMBASE databases were searched. Records were included if they were randomized controlled trials of patients undergoing HZ infection, investigating the effect of GABA derivatives versus placebo in the treatment of HZ pain. Eligible trials were evaluated for the risk of bias. Then data were extracted and analysed. The number of patients with observed presence of pain after treatment was used to calculate odds ratio in a random effect model with the DerSimonian-Laird estimator. The I2 statistic was analysed for heterogeneity. The potential risk of bias was measured using Egger’s regression test. The meta-analysis included three randomized controlled trials with a total of 297 patients. The incidence of acute HZ pain events for GABA group was significantly lower compared to placebo group,18/148 vs 44/149, respectively (OR = 0.36; 95% CI = 0.14 to 0.93; Z = 2.11; P = 0.035), Egger’s test yielded P = 0.308. In conclusion, the present meta-analysis demonstrates that GABA derivatives reduce the incidence of acute herpetic pain. However, additional, well-designed randomized clinical trials are needed to determine their dose- and time-dependency regarding this symptom.

Background/Aim: The role of microRNAs (miRNA) in carcinogenesis is related to their genome-regulatory function. The aim of the present study was to identify and compare miRNA expression signatures of meso- and hypopharynx squamous cell cancers in consideration of the cancer field hypothesis. Patients and Methods: Thirteen snap-frozen biopsy series of tumors and peritumoral tissues from the meso- and hypopharynx were analyzed regarding their miRNA expression with quantitative real-time polymerase chain reaction (qRT-PCR). Results: Significant miRNA expression changes of miR-21, -27a, -34a, -143 and -146a were found in peritumoral tissues that were dependent from the tumor location and the distance from the primary tumor site. Conclusion: miRNA expression analysis was found to be appropriate for molecular segregation of tumor location and peritumoral tissue segments, and appears to be a promising marker for cancer field characterization.

The aim of this study was to evaluate the short and medium-term effects of radiofrequency (RF) and potassium titanyl phosphate (KTP) and neodymium-yttrium-aluminum garnet (Nd:YAG) laser treatment on the inferior turbinate mucosa in a porcine model. Following randomization, the inferior turbinates were treated either with RF submucosally or with the KTP or the Nd:YAG laser on the surface under videoendoscopic control. Tissue samples were taken at the end of postoperative weeks 1 and 6, and were evaluated macroscopically and histopathologically. Scanning electron microscopy was implemented to demonstrate the morphological changes in the respiratory epithelium. Six weeks following the RF procedure, the mucosa was intact in all cases, and the volume of the inferior turbinates was reduced in the majority of the cases. Although a volume reduction occurred in both laser groups, more complications associated with the healing procedure were noted. With hematoxylin and eosin and periodic acid–Schiff staining, intact epithelium, and submucosal glands remained after the RF procedures at the end of postoperative week 6. Following the KTP-laser intervention, necrotizing sialometaplasia and cartilage destruction occurred, and squamous metaplasia was also apparent in the Nd:YAG group. In both laser groups, dilated glands with excess mucus were seen. The scanning electron microscopic findings demonstrated that cilia were present in all cases. In conclusion, the medium-term macroscopic results were similar in all 3 groups, but the postoperative complications were less following the RF procedure. RF procedure is minimally invasive due to the submucosal intervention that leads to a painless, function preserving recovery.

We predict glueball/oddball resonances lying on the pomeron/odderon trajectories. A simple new form of the trajectories, with threshold and asymptotic behaviour required by analyticity and unitarity, is proposed. The parameters of these (pomeron and odderon) trajectories are fitted to the data on high-energy elastic proton-proton scattering. The fitted trajectories are extrapolated to the resonance region to predict masses and widths of glueballs and oddballs. The (pomeron and odderon) trajectories may be used to calculate processes of central exclusive diffraction (CED)., Comment: 37 pages, 15 figures, 5 tables

The proton-proton and proton-antiproton inelasticity profiles in the impact parameter display very interesting and sensitive features which cannot be deduced solely from the current large body of high-energy scattering data. In particular, phenomenological studies exhibit a link between the ratio of the real to imaginary parts of the elastic scattering amplitude at a finite momentum transfer, and the corresponding change of character of the inelastic processes from central to peripheral collisions. We describe how a theoretical model, accommodating the existing data, based on the Regge hypothesis including both the Pomeron and odderon as double poles, and $\ensuremath{\omega}$ and $f$ mesons as single poles in the complex-$J$ plane, generates a hollow in the inelasticity at low impact parameters. The hollowness effect, which generally may be sensitive to model details, does unequivocally take place both for $pp$ and $p\overline{p}$ collisions within the applied Regge framework, indicating inapplicability of inelasticity-folding geometric approaches.

Proton-proton differential and total cross sections provide information on the energy dependence of proton shape and size. We show that the deviation from exponential behavior of the diffraction cone observed near $t=-0.1$ GeV$^2$, (so-called break), both at the ISR and the LHC follows from the $t$-channel two-pion loop contributions, imposed by unitarity. By using a simple Regge-pole model, we extrapolate the "break" from the ISR energy region to that of the LHC. This allows us to answer two important questions: 1) To what extent is the "break" observed recently at the LHC a "recurrence" of that seen at the ISR (universality)? 2) What is the relative weight of two-pion effect to the vertex coupling (Regge residue) compared to expanding size (pomeron propagator) in producing the "break"? We find that the effect comes both from the Regge residue (proton-pomeron coupling) and from the Regge propagator. The dip at 13 TeV seems to become more shallow with respect to lower energies, and we attribute this important new phenomena to the odderon contribution. These findings call for a revision of the standeard Regge-pole theory.

The aim of this retrospective study was to compare the 3-month postoperative hearing results following laser stapedotomy with the use of NiTiBOND versus Nitinol prostheses (31 and 39 patients, respectively). The operations were performed between September 2012 and September 2014, and between March 2006 and December 2012 regarding NiTiBOND and Nitinol, respectively. Twenty of the consecutive 31 patients were female and 11 were male for NiTiBOND, while 11 were male and 28 were female for Nitinol. The mean age was 43.8 years (range 22–61) and 46.9 years (range 28–83) for NiTiBOND and Nitinol, respectively. No significant cochlear trauma was documented postoperatively. The mean air–bone gap (ABG) for the frequencies 0.5, 1, 2 and 3 kHz at the 3-month postoperative follow-up was 7.6 dB (SD 4.7), and 9.3 dB (SD 4.1) for NiTiBOND and Nitinol, respectively. The differences between the mean pre- (p = 0.179), and postoperative (p = 0.059) ABG of the two groups were not significant. ABG closure within 10 dB was achieved in 77.4 and 59 % for NiTiBOND and Nitinol, respectively, the difference was not significant (p = 0.10). Two cases of delayed facial paralysis occurred, 1 with Nitinol and 1 with the NiTiBOND. All patients attained an ABG

Anticipating forthcoming publication by the TOTEM collaboration of low-t elastic scattering data at \sqrt{s}=13 and 2.76 TeV, hereby we emphasize the correlation between two prominent structures seen upon the otherwise exponential diffraction cone, namely a "break" staying fixed around t=-0.1 GeV^2 and a dip moving with energy logarithmically inwards; while at the ISR the two structures are separated by a distance of about 1~GeV^2, at the LHC the dip comes close to the periphery of the "break", thus affecting its parametrization. Despite modern diagnostic procedures and up-to-date therapy, the survival of head and neck tumour patients is unfavourable. This can be explained by several factors, one of which is the late recognition of the tumour. This study related to the changes in expression of the c-myc and Ha-ras oncogenes and the p53 tumour suppressor gene as biomarkers in head and neck cancer cases. The gene expressions were investigated on RNA gained from peripheral white blood cells of head and neck cancers patients before and after definitive treatment. The results were compared with those on a control group of patients with non-tumorous diseases. The gene expressions were significantly higher in the cancer group than that in the control group (volunteer medical staff and medical students). After definitive treatment, the expressions of all these genes were decreased in patients in whom there was no recurrence of the tumour, but enhanced in the event of recurrence. Such measurement may serve as reliable biomarkers to monitor tumour development and the efficiency of therapy. The method may also be useful for the early identification of populations exposed to noxe, which may lead to the development of head and neck cancers.

We review elastic and diffractive scattering of protons (called also "forward physics") with emphasis on the LHC data, especially those deviating from the expectations based on extrapolations from earlier measurements at the ISR, Fermilab and thus triggering searches for new ideas, models and theories. We list these new data and provide a brief introduction of available theoretical approaches, mainly those based on analyticity, crossing symmetry and unitarity, particularly the Regge-pole model realizing these concepts. Fits to the data are presented and tensions between theoretical predictions and the data that may indicate the way to further progress are in the focus of our paper., 45 pages, 38 figures, 2 tables

New phenomena discovered at the LHC in nearly forward scattering are discussed. The properties of elastic and inelastic diffraction are related due to Regge factorisation and pomeron dominance.

Conventional septoplasty cannot be the answer to all types of septal deviation. Indications of extracorporal septal reconstruction with polydioxanone (PDS) foil: (i) selected cases of very high septal deviations, (ii) post-traumatic and (iii) extremely pronounced congenital septal deviations and/or aesthetic deformities. Polydioxanone foil facilitates the incorporation of a newly implanted septal graft without significant postoperative complications and is absorbed within 25 weeks. The surgical technique is described in detail, and the early postoperative functional and aesthetic results (mean follow-up: 11 months) on 16 patients (mean age: 42 years) are reported. Overall, 88% of the patients responded positively to the question of whether they would choose to undergo the same procedure again, knowing the postoperative result. The procedure is easy to learn and has already proved to be an excellent combination of modern functional and aesthetic nasal surgery.

A vezetéses halláscsökkenést okozó otoscleroticus stapesfixatio műtéti megoldásának folyamatos fejlődése az elmúlt fél évszázad egyik fülsebészeti sikertörténete. A stapedectomia, illetve parciális stapedectomia mellett manapság a stapedotomia is elterjedt műtéti megoldásnak tekinthető. Cél: A minimálisan invazív lézerstapedotomia hazai bevezetése, tapasztalatok szerzése, valamint a műtéti halláseredmények és a posztoperatív komplikációk elemzése. Módszer: A szerzők közleményükben KTP-lézerrel asszisztált, Nitinol-pisztonnal végzett 14 esetük kapcsán ismertetik lézerstapedotomiákkal szerzett első tapasztalataikat. 2006 márciusa és 2007 áprilisa között 11 nőbeteget és 3 férfi beteget operáltak. A betegek átlagéletkora 42,2 év volt, 6 esetben bal, 8 esetben jobb fülön történt a műtét. A betegek átlagos követési ideje 9 hónap volt. Eredmények: A posztoperatív audiogramok értékelésekor az 500–1000–2000–3000 Hz frekvenciákat figyelembe véve a légvezetés átlagos javulása 21,5 dB volt. Ugyanezen frekvenciákon a csontlégközértékek átlagos javulása 18,7 dB volt. Kilenc hónap átlagos követési idő mellett az eddigi esetekben a műtéteket követően a betegek 85%-ában az átlagos csontlégköz < 10 dB, de valamennyi beteg esetében < 20 dB. Az átlagos légvezetés értéke egy beteg kivételével < 30 dB. Belsőfül-károsodásra utaló, a magashang-frekvenciákat érintő csontvezetéses romlást, facialis paresist nem észleltek. Következtetések: A szerzők tapasztalatai és az irodalmi adatok alapján a Nitinol-pisztonnal végzett lézerstapedotomiáknak számos előnyük van: 1) a piszton hurkának hőre aktiválódó memóriaeffektusa kiküszöböli az incus hosszú szárára két ponton történő rászorítás hátrányait; 2) a posztoperatív szédülés mértéke enyhébb, időtartama rövidebb, ezért az ápolási idő csökken; 3) a műtét költséghatékony, és minimálisan invazív; 4) a lézer alkalmazása minimális cochlearis traumát, vérmentes környezetet biztosít; 5) a protézis migrációja kiküszöbölhető, körülötte a granuláció mérsékeltebb; 6) magas frekvenciákon tartósan jó halláseredmény érhető el; 7) a komplikációk súlyossága, gyakorisága csökken, ezért a technika jó megoldás kevésbé gyakorlott operatőrök számára is; 8) problémás esetekben (arteria stapedialis jelenléte, obliterativ otosclerosis, úszó stapestalp, rendellenesen futó n. facialis a dobüregben, revíziós műtétek) is alkalmazható; 9) a halláseredmények interindividuális különbsége kisebb. – Eddigi kedvező tapasztalataik alapján a szerzők lézerstapedotomián átesettek hosszú távú követését tervezik nagyobb beteganyagon.

Paranasal sinus cancer is rather rare, with an incidence of less than 1 per 100,000 per year, the frontal sinus being the primary site in only 0.3%. Following clinical and radiological examinations, the diagnosis often remains elusive, the condition mistakenly being considered to involve mucocele, pyocele or osteomyelitis. This article reports on a 60-year-old pigeon breeder with squamous cell carcinoma originating in the frontal sinus. The presenting symptoms were a gradually worsening sharp headache, double vision and an oedematous left eyelid. CT and magnetic resonance imaging demonstrated erosion of the orbital roof around an area 10 mm in diameter, without direct infiltration of the bulbus. Twelve months following a frontal craniotomy, tumour excision and postoperative radiotherapy, the patient is symptom-free. A detailed description of this patient and a review of the published work are presented.
