Abstract

We describe a giant gastric phytobezoar in a child with repaired congenital esophageal atresia. At age two, a gastric interposition (pull-up) procedure was performed for severe and recurrent esophageal strictures. For 12 months post-gastric interposition, he experienced frequent respiratory illnesses requiring hospital admissions but it was not initially appreciated that these episodes were likely secondary to recurrent aspiration from a gastric bezoar with “spill-over” aspiration.

Abstract

Swimming skills, escape from water entrapment and the history of water safety are themes which enjoin all who work, volunteer and serve in the domains of the aquatic professions. The imperative of being able to swim and the importance of survival skills are curiously illustrated by two examples from the history of medicine. One relates to the Code of Hammurabi (1750 BC); the other relates to the punishment by drowning, inflicted by King John of Bohemia (1296-1346 AD). In these two examples, escape from water entrapment and the ability to swim were more important skills than the possession of medical interventions with curative outcomes, or even the presumption of innocence following criminal arrest and subsequent judicial trial. 15. Cough and exhaled nitric oxide levels: what happens with exercise?

Abstract

Cough associated with exertion is often used as a surrogate marker of asthma. However, to date there are no studies that have objectively measured cough in association with exercise in children. Our primary aim was to examine whether children with a pre-existing cough have an increase in cough frequency during and post-exercise. We hypothesized that children with any coughing illness will have an increase in cough frequency post-exercise regardless of the presence of exercise-induced broncho-constriction (EIB) or atopy. In addition, we hypothesized that Fractional exhaled nitric oxide (FeNO) levels decreases post-exercise regardless of the presence of EIB or atopy. Children with chronic cough and a control group without cough undertook an exercise challenge, FeNO measurements and a skin prick test, and wore a 24-h voice recorder to objectively measure cough frequency. The association between recorded cough frequency, exercise, atopy, and presence of EIB was tested. We also determined if the change in FeNO post exercise related to atopy or EIB. Of the 50 children recruited (35 with cough, 15 control), 7 had EIB. Children with cough had a significant increase in cough counts (median 7.0, inter-quartile ranges, 0.5, 24.5) compared to controls (2.0, IQR 0, 5.0, p = 0.028) post-exercise. Presence of atopy or EIB did not influence cough frequency. FeNO level was significantly lower post-exercise in both groups but the change was not influenced by atopy or EIB. Cough post-exertion is likely a generic response in children with a current cough. FeNO level decreases post-exercise irrespective of the presence of atopy or EIB. A larger study is necessary confirm or refute our findings.

33. Ceftazidime alone and in combination in patients with cystic fibrosis: lack of efficacy in treatment of severe respiratory infections caused by Pseudomonas cepacia.

Abstract

Fourteen patients with cystic fibrosis received 18 treatment courses with ceftazidime for acute respiratory illnesses associated with Pseudomonas cepacia. All patients had severe chronic lung disease. Clinical improvement occurred in only six treatment courses; eight treatment courses resulted in failure and four patients died. Severe illness was characterized by high fever, marked elevation of WBC and ESR. Treatment had no effect on sputum colony counts of Ps. cepacia in 17 of 18 courses, but significantly reduced counts of Ps. aeruginosa in 90% of those patients infected with both bacteria. 37. BREAST CANCER IN QUEENSLAND FIFTEEN YEARS AFTER TREATMENT

Abstract

The incidence of carcinoma of the breast in this Australian series is as high as in most countries of the world, and remains high with increasing age. The mortality rate from cancer is highest in the first three years after treatment of the primary disease, but, even after 10 years, deaths from cancer are nearly as frequent as those from intercurrent disease. At least 25% of patients had advanced disease at initial presentation. A 25% 15‐year survival rate is reported in a series of mixed stage at the time of initial treatment, which was by simple mastectomy and radiotherapy in 75% of cases. Large tumours without evidence of axillary metastasis T3N0M0) often had a surprisingly good prognosis. The site of first reappearance of the tumour was local in 30 6% of cases, systemic in 38.4% of cases, and in the opposite breast in 7 6% of cases. It was not possible to classify the remaining 23 4% either because of incomplete information or, more usually, because of synchronous development of local and systemic recurrence. Of patients who developed local recurrence in this group, 83% showed generalized metastatic disease within 10 years of the appearance of the recurrent disease.
