However, the association between IVF/ICSI and the bleeding risk particularly in cesarean delivery has not been systematically assessed. The aim of this study was to evaluate bleeding risk during and after cesarean delivery in parturients who conceived using IVF or ICSI. Methods This is a retrospective observational study analyzing data from 310 parturients who underwent cesarean delivery: 155 who had conceived using IVF or ICSI (IVF/ICSI group) and 155 age and year of delivery matched controls who had conceived spontaneously (control group). The primary outcome measure was the amount of blood lost during and within 24 h after cesarean delivery. Secondary outcome measure was the incidence of severe PPH. With 132 parturients in each group, we had 90% power to detect a 200 mL difference in the bleeding amount, at a 0.05 two-sided significance level. Results The amount of bleeding in the IVF/ICSI group was 1234 ± 669 mL, which was 124 mL (11.2%) greater than that in the control group (95% CI - 34 to 282; p = 0.12). The incidence of severe PPH in the IVF/ICSI group and in the control group was 23.9% and 16.8%, respectively (p = 0.16), and the unadjusted odds ratio was 1.6 (95% CI, 0.9-2.7; p = 0.12). No significant independent effect of IVF/ICSI on the bleeding amount and the incidence of severe PPH was observed in multivariable regression analyses (p = 0.22, p = 0.16). Conclusion In this study, IVF and ICSI were not associated with increasing risk of bleeding in cesarean delivery.

Abstract

Corticosteroids are most commonly used to treat HTLV-1-associated myelopathy (HAM); however, their clinical efficacy has not been tested in randomized clinical trials. This randomized controlled trial included 8 and 30 HAM patients with rapidly and slowly progressing walking disabilities, respectively. Rapid progressors were assigned (1:1) to receive or not receive a 3-day course of intravenous methylprednisolone in addition to oral prednisolone therapy. Meanwhile, slow progressors were assigned (1:1) to receive oral prednisolone or placebo. The primary outcomes were a composite of ≥1-grade improvement in the Osame Motor Disability Score or ≥30% improvement in the 10 m walking time (10 mWT) at week 2 for rapid progressors and changes from baseline in 10 mWT at week 24 for slow progressors. In the rapid progressor trial, all four patients with but only one of four without intravenous methylprednisolone achieved the primary outcome (p = 0.14). In the slow progressor trial, the median changes in 10 mWT were -13.8% (95% CI: -20.1--7.1; p < 0.001) and -6.0% (95% CI: -12.8-1.3; p = 0.10) with prednisolone and placebo, respectively (p for between-group difference = 0.12). Whereas statistical significance was not reached for the primary endpoints, the overall data indicated the benefit of corticosteroid therapy. (Registration number: UMIN000023798, UMIN000024085).

Abstract

A few years ago, we found the supersymmetric(SUSY) counterpart of the spectral triple which specified noncommutative geometry(NCG). Based on "the triple", we considered the SUSY version of the spectral action principle and had derived the action of super Yang--Mills theory, minimal supersymmetric standard model, and supergravity. In these theories, we used vector notation in order to express a chiral or an anti-chiral matter superfield. We also represented the NCG algebra and the Dirac operator by matrices which operated on the space of matter field. In this paper, we represent the triple in the superspace coordinate system $(x^\mu,\theta,\bar{\theta})$. We also introduce "extracting operators" and the new definition of the supertrace so that we can also investigate the square of the Dirac operator on the Minkowskian manifold in the superspace. Abstract

Two years ago, we found the supersymmetric counterpart of the spectral triple which specified noncommutative geometry. Based on the triple, we derived gauge vector supermultiplets, Higgs supermultiplets of the minimum supersymmetric standard model and its action. However, unlike the famous theories of Connes and his co-workers, the action does not couple to gravity. In this paper, we obtain the supersymmetric Dirac operator $\mathcal{D}_M^{(SG)}$ on the Riemann-Cartan curved space replacing derivatives which appear in that of the triple with the covariant derivatives of general coordinate transformation. We apply the supersymmetric version of the spectral action principle and investigate the heat kernel expansion on the square of the Dirac operator. As a result, we obtain a new supergravity action which does not include the Ricci curvature tensor., Comment: to be published in "Progress of Theoretical and Experimental Physics"

Abstract

We have obtained the supersymmetric extension of spectral triple which specify a noncommutative geometry(NCG). We assume that the functional space H constitutes of wave functions of matter fields and their superpartners included in the minimum supersymmetric standard model(MSSM). We introduce the internal fluctuations to the Dirac operator on the manifold as well as on the finite space by elements of the algebra A in the triple. So, we obtain not only the vector supermultiplets which meditate SU(3)xSU(2)xU(1)_Y gauge degrees of freedom but also Higgs supermultiplets which appear in MSSM on the same standpoint. Accoding to the supersymmetric version of the spectral action principle, we calculate the square of the fluctuated total Dirac operator and verify that the Seeley-DeWitt coeffients give the correct action of MSSM. We also verify that the relation between coupling constants of $SU(3)$,$SU(2)$ and $U(1)_Y$ is same as that of SU(5) unification theory.

Abstract

When we restrict the representation space to the fermionic functions of matter fields, the counterpart which we name "the triple" reduces to the original spectral triple which defines noncommutative geometry. We see that the fluctuation to the supersymmetric Dirac operator induced by algebra in the triple generates vector supermultiplet which mediates gauge interaction. Abstract

We extend naturally the spectral triple which define noncommutative geometry (NCG) in order to incorporate supersymmetry and obtain supersymmetric Dirac operator D_M which acts on Minkowskian manifold. Inversely, we can consider the projection which reducts D_M to $\not{D}$, the Dirac operator of the original spectral triple. We investigate properties of the Dirac operator, some of which are inherited from the original Dirac operator. Z/2 grading and real structure are also supersymmetrically extended. Using supersymmetric invariant product, the kinematic terms of chiral and antichiral supermultiplets which represent the wave functions of matter particles and their superpartners are provided by D_M. Considering the fluctuation given by elements of the algebra to the extended Dirac operator, we can expect to obtain vector supermultiplet which includes gauge field and to obtain super Yang-Mills theory according to the supersymmetric version of spectral action principle., Comment: arXiv admin note: text overlap with arXiv:1201.3448

Abstract

Introduction of supersymmetry into the noncommutative geometry is investigated. We propose a new Dirac operator which plays the role of the metric over the extended algebra of chiral and antichiral supermultiplets and is invariant under the supersymmetry transformations. Inner automorphisms for the algebra generate vector supermultiplets as an internal fluctuation of the metric. Intraprocedural sedation management is important for successful performance of BT. However, the results of general anesthesia in patients undergoing BT have not been well described. The aim of this study was to evaluate the feasibility and safety of general anesthesia in patients undergoing BT. We retrospectively reviewed the records of 10 consecutive BT treatments performed under general anesthesia in 4 patients. The feasibility outcomes were coughing and body movement during the procedure, procedure abandonment, and the relative frequency of thermal activation failure. The safety outcomes were bronchospasm and hypoxemia during the procedure, respiratory symptoms, and the need for oxygen after the procedure. Coughing occurred in two treatments. Neither body movement nor procedure abandonment occurred in any treatments. Neither intraprocedural bronchospasm nor hypoxemia occurred in any treatments. Respiratory symptoms occurred in 7 of 10 treatments within 1 day after the procedure and resolved within 4 days, which is comparable with a previous report. These results indicate that general anesthesia is feasible and safe for patients undergoing BT.
