Abstract

AimsPulmonary enteric adenocarcinoma (PEAC) is a rare variant of pulmonary adenocarcinoma. Due to its rarity, few pathological and molecular studies have been performed on PEAC. We herein conducted clinicopathological, immunohistochemical and molecular analyses of PEAC with a focus on its differentiation from invasive mucinous adenocarcinoma (IMA).MethodsWe examined the clinicopathological features of 16 cases of PEAC and performed a genetic analysis using next-generation sequencing (NGS). The results obtained were compared with those for IMA.ResultsThe average age of patients with PEAC (seven men and nine women) was 72.9 years. A comparison of clinical data on PEAC and IMA revealed no significant differences in age, sex or smoking history. Fifteen PEAC cases had dirty necrosis. Immunohistochemically, the positive rates for each antibody in PEAC were as follows: CK7, 88% (14/16); CK20, 81% (13/16); CDX2, 88% (14/16); p53, 69% (11/16); MUC1, 100% (16/16); MUC2, 19% (3/16); MUC5AC, 69% (11/16); MUC6, 19% (3/16). The positive rates for these antibodies in IMA were 100%, 87%, 0%, 7%, 93%, 0%, 100% and 80%, respectively. EGFRmutations, the METexon 14 skipping mutation, BRAFmutations, the ALKfusion gene and ROS-1fusion gene were not detected in any cases of PEAC or IMA. Among PEAC cases, NGS identified KRASmutations in seven (44%, 7/16) and TP53mutations in nine (56%, 9/16). Among IMA cases, the most commonly mutated gene was KRAS(90%).ConclusionsThe rates of dirty necrosis, immunopositivity for CDX2 and TP53mutations were significantly higher, while that of KRASmutations was significantly lower in PEAC cases than in IMA cases.

Abstract

We present a case of therapy-related myelodyspastic syndrome in which the t(3;8)(q26;q24) translocation appeared, even though no chromosomal abnormalities were found at the initial diagnosis of acute myeloid leukemia. To the best of our knowledge, there have only been around 20 reported cases of myeloid malignancies involving t(3;8)(q26;q24). We discuss the characteristics of t(3;8)(q26;q24) along with a review of literature.

Abstract

Clinical data or investigation of large cell neuroendocrine carcinoma (LCNEC) have accumulated, some of which however, can be flawed by erroneous cytological diagnosis. In this report, we present a case of LCNEC suspected of originating in the lung, and discuss cytological features Qr differential diagnosis. Aspiration cytology showed clusters of tumor cells with large and polymorphic nuclei and distinct nucleoli. The cytoplasm was positive for some markers for neuroendocrines, is similarly as demonstrated in histological findings of metastasis in the cervical lymph node. LCNEC rapidly develops and often reaches an advanced stage, and we have sometirnes found that fine needle biopsy or cytology is the only tool for pathological suggestion or diagnosis. Therefore, it is necessary to accumulate case reports as presented here and confirm cytological features, which might help us to make an accurate diagnosis of LCNEC and to distinguish it from other neuroendocrine neoplasms including small cell carcinoma or poorly differentiated tumors.

Abstract

The proliferating trichilemmal cyst (PTC)is a tumor that originates from hair follicles. It is almost always benign, but rare lesions have been reported with malignant potential. In the present case, the patient had a mass on the right side of her neck for a few years, which suddenly increased in size. Cytologic examination revealed many atypical squamous cells staining well with orange G and light green, suggestive of a squamous cell carcinoma. Total extirpation was performed under general anesthesia, gross pathology revealing a focally cystic tumor approximately 5 cm in diameter. The final diagnosis was malignant PTC from the characteristic features. We here report cytologic findings for this case with regional lymph node metastasis.
