Abstract: A search for mixing between active neutrinos and light sterile neutrinos has been performed by looking for muon neutrino disappearance in two detectors at baselines of 1.04 km and 735 km, using a combined MINOS and MINOS+ exposure of $16.36\times10^{20}$ protons-on-target. A simultaneous fit to the charged-current muon neutrino and neutral-current neutrino energy spectra in the two detectors yields no evidence for sterile neutrino mixing using a 3+1 model. The most stringent limit to date is set on the mixing parameter $\sin^2\theta_{24}$ for most values of the sterile neutrino mass-splitting $\Delta m^2_{41} > 10^{-4}$ eV$^2$., Comment: 7 pages, 4 figures, additional analysis details and a data release in the ancillary materials

Abstract: We report new constraints on the size of large extra dimensions from data collected by the MINOS experiment between 2005 and 2012. Our analysis employs a model in which sterile neutrinos arise as Kaluza-Klein states in large extra dimensions and thus modify the neutrino oscillation probabilities due to mixing between active and sterile neutrino states. Using Fermilab's NuMI beam exposure of $10.56 \times 10^{20}$ protons-on-target, we combine muon neutrino charged current and neutral current data sets from the Near and Far Detectors and observe no evidence for deviations from standard three-flavor neutrino oscillations. The ratios of reconstructed energy spectra in the two detectors constrain the size of large extra dimensions to be smaller than $0.45\,\mu\text{m}$ at 90% C.L. in the limit of a vanishing lightest active neutrino mass. Abstract: Forward single $\pi^0$ production by coherent neutral-current interactions, $\nu \mathcal{A} \to \nu \mathcal{A} \pi^0$, is investigated using a 2.8$\times 10^{20}$ protons-on-target exposure of the MINOS Near Detector. For single-shower topologies, the event distribution in production angle exhibits a clear excess above the estimated background at very forward angles for visible energy in the range~1-8 GeV. Cross sections are obtained for the detector medium comprised of 80% iron and 20% carbon nuclei with $\langle \mathcal{A} \rangle = 48$, the highest-$\langle \mathcal{A} \rangle$ target used to date in the study of this coherent reaction. The total cross section for coherent neutral-current single-$\pi^0$ production initiated by the $\nu_\mu$ flux of the NuMI low-energy beam with mean (mode) $E_{\nu}$ of 4.9 GeV (3.0 GeV), is $77.6\pm5.0\,(\text{stat}) ^{+15.0}_{-16.8}\,(\text{syst})\times10^{-40}\,\text{cm}^2~\text{per nucleus}$. Abstract: We report results of a search for oscillations involving a light sterile neutrino over distances of 1.04 and $735\,\mathrm{km}$ in a $\nu_{\mu}$-dominated beam with a peak energy of $3\,\mathrm{GeV}$. The data, from an exposure of $10.56\times 10^{20}\,\textrm{protons on target}$, are analyzed using a phenomenological model with one sterile neutrino. We constrain the mixing parameters $\theta_{24}$ and $\Delta m^{2}_{41}$ and set limits on parameters of the four-dimensional Pontecorvo-Maki-Nakagawa-Sakata matrix, $|U_{\mu 4}|^{2}$ and $|U_{\tau 4}|^{2}$, under the assumption that mixing between $\nu_{e}$ and $\nu_{s}$ is negligible ($|U_{e4}|^{2}=0$). No evidence for $\nu_{\mu} \to \nu_{s}$ transitions is found and we set a world-leading limit on $\theta_{24}$ for values of $\Delta m^{2}_{41} \lesssim 1\,\mathrm{eV}^{2}$., Comment: 7 pages, 4 figures

Abstract: Searches for a light sterile neutrino have been performed independently by the MINOS and the Daya Bay experiments using the muon (anti)neutrino and electron antineutrino disappearance channels, respectively. In this Letter, results from both experiments are combined with those from the Bugey-3 reactor neutrino experiment to constrain oscillations into light sterile neutrinos. The three experiments are sensitive to complementary regions of parameter space, enabling the combined analysis to probe regions allowed by the LSND and MiniBooNE experiments in a minimally extended four-neutrino flavor framework. Stringent limits on $\sin^2 2\theta_{\mu e}$ are set over 6 orders of magnitude in the sterile mass-squared splitting $\Delta m^2_{41}$. The sterile-neutrino mixing phase space allowed by the LSND and MiniBooNE experiments is excluded for $\Delta m^2_{41} < 0.8$ eV$^2$ at 95% CL$_s$., Comment: 8 pages, 4 figures, published in Physical Review Letters. Abstract: We report new constraints on flavor-changing non-standard neutrino interactions from the MINOS long-baseline experiment using $\nu_{e}$ and $\bar{\nu}_{e}$ appearance candidate events from predominantly $\nu_{\mu}$ and $\bar{\nu}_{\mu}$ beams. We used a statistical selection algorithm to separate $\nu_{e}$ candidates from background events, enabling an analysis of the combined MINOS neutrino and antineutrino data. Abstract: Data from the MINOS experiment has been used to search for mixing between muon neutrinos and muon antineutrinos using a time-independent Lorentz-violating formalism derived from the Standard-Model Extension (SME). MINOS is uniquely capable of searching for muon neutrino-antineutrino mixing given its long baseline and ability to distinguish between neutrinos and antineutrinos on an event-by-event basis. Neutrino and antineutrino interactions were observed in the MINOS Near and Far Detectors from an exposure of 10.56$\times10^{20}$ protons-on-target from the NuMI neutrino-optimized beam. No evidence was found for such transitions and new, highly stringent limits were placed on the SME coefficients governing them. We place the first limits on the SME parameters $(c_{L})^{TT}_{\mu\mu} $ and $(c_{L})^{TT}_{\tau\tau}$ at $-8.4\times10^{-23} < (c_{L})^{TT}_{\mu\mu} < 8.0\times10^{-23}$ and $-8.0\times10^{-23} < (c_{L})^{TT}_{\tau\tau} < 8.4\times10^{-23}$, and the world's best limits on the $\tilde{g}^{ZT}_{\mu\overline{\mu}}$ and $\tilde{g}^{ZT}_{\tau\overline{\tau}}$ parameters at $|\tilde{g}^{ZT}_{\mu\overline{\mu}}| < 3.3\times 10^{-23}$ and $|\tilde{g}^{ZT}_{\tau\overline{\tau}}| < 3.3\times 10^{-23}$, all limits quoted at $3\sigma$., Comment: This paper has been withdrawn by the author due to a mistake in the calculations

Abstract: The charge ratio, $R_\mu = N_{\mu^+}/N_{\mu^-}$, for cosmogenic multiple-muon events observed at an under- ground depth of 2070 mwe has been measured using the magnetized MINOS Far Detector. The multiple-muon events, recorded nearly continuously from August 2003 until April 2012, comprise two independent data sets imaged with opposite magnetic field polarities, the comparison of which allows the systematic uncertainties of the measurement to be minimized. The multiple-muon charge ratio is determined to be $R_\mu = 1.104 \pm 0.006 {\rm \,(stat.)} ^{+0.009}_{-0.010} {\rm \,(syst.)} $. Abstract: We report the first observation of seasonal modulations in the rates of cosmic ray multiple-muon events at two underground sites, the MINOS Near Detector with an overburden of 225 mwe, and the MINOS Far Detector site at 2100 mwe. At the deeper site, multiple-muon events with muons separated by more than 8 m exhibit a seasonal rate that peaks during the summer, similar to that of single-muon events. In contrast and unexpectedly, the rate of multiple-muon events with muons separated by less than 5-8 m, and the rate of multiple-muon events in the smaller, shallower Near Detector, exhibit a seasonal rate modulation that peaks in the winter.

Abstract: Kinematic distributions from an inclusive sample of 1.41 x 10^6 charged-current nu_mu interactions on iron, obtained using the MINOS Near Detector exposed to a wide-band beam with peak flux at 3 GeV, are compared to a conventional treatment of neutrino scattering within a Fermi gas nucleus. Results are used to guide the selection of a subsample enriched in quasielastic nu_mu Fe interactions, containing an estimated 123,000 quasielastic events of incident energies 1 < E_nu < 8 GeV, with = 2.79 GeV. Four additional subsamples representing topological and kinematic sideband regions to quasielastic scattering are also selected for the purpose of evaluating backgrounds. Comparisons using subsample distributions in four-momentum transfer Q^2 show the Monte Carlo model to be inadequate at low Q^2. Its shortcomings are remedied via inclusion of a Q^2-dependent suppression function for baryon resonance production, developed from the data. A chi-square fit of the resulting Monte Carlo simulation to the shape of the Q^2 distribution for the quasielastic-enriched sample is carried out with the axial-vector mass M_A of the dipole axial-vector form factor of the neutron as a free parameter. The effective M_A which best describes the data is 1.23 +0.13/-0.09 (fit) +0.12/-0.15 (syst.) GeV., Comment: 20 pages, 13 figures, 3 Tables Accepted for publication in PRD

Abstract: A sample of 1.53$\times$10$^{9}$ cosmic-ray-induced single muon events has been recorded at 225 meters-water-equivalent using the MINOS Near Detector. The underground muon rate is observed to be highly correlated with the effective atmospheric temperature. The coefficient $\alpha_{T}$, relating the change in the muon rate to the change in the vertical effective temperature, is determined to be 0.428$\pm$0.003(stat.)$\pm$0.059(syst.). An alternative description is provided by the weighted effective temperature, introduced to account for the differences in the temperature profile and muon flux as a function of zenith angle. Using the latter estimation of temperature, the coefficient is determined to be 0.352$\pm$0.003(stat.)$\pm$0.046(syst.)., Comment: 9 pages 10 figures

Abstract: We report on a new analysis of neutrino oscillations in MINOS using the complete set of accelerator and atmospheric data. The analysis combines the $\nu_{\mu}$ disappearance and $\nu_{e}$ appearance data using the three-flavor formalism. We measure $|\Delta m^{2}_{32}|=[2.28-2.46]\times10^{-3}\mbox{\,eV}^{2}$ (68% C.L.) and $\sin^{2}\theta_{23}=0.35-0.65$ (90% C.L.) in the normal hierarchy, and $|\Delta m^{2}_{32}|=[2.32-2.53]\times10^{-3}\mbox{\,eV}^{2}$ (68% C.L.) and $\sin^{2}\theta_{23}=0.34-0.67$ (90% C.L.) in the inverted hierarchy. The data also constrain $\delta_{CP}$, the $\theta_{23}$ octant degeneracy and the mass hierarchy; we disfavor 36% (11%) of this three-parameter space at 68% (90%) C.L., Comment: 6 pages, 2 figures

Abstract: We report measurements of oscillation parameters from $\nu_{mu}$ and $\bar{\nu}_{\mu}$ disappearance using beam and atmospheric data from MINOS. The data comprise exposures of \unit[$10.71 \times 10^{20}$]{protons on target (POT)} in the $\nu_{\mu}$-dominated beam, $\unit[3.36\times10^{20}]{POT}}$ in the $\bar{\nu}_{\mu}$-enhanced beam, and 37.88 kton-years of atmospheric neutrinos. Assuming identical $\nu$ and $\bar{\nu}$ oscillation parameters, we measure \mbox{$|\Delta m^2}| = \unit[2.41^{+0.09}_{-0.10}) \times 10^{-3}]{eV^{2}}$} and $\sin^{2}/!/left(2\theta \right) = 0.950^{+0.035}_{-0.036}$. Allowing independent $\nu$ and $\bar{\nu}$ oscillations, we measure antineutrino parameters of $|\Delta \bar{m}^2| = \unit[(2.50 ^{+0.23}_{-0.25}) \times 10^{-3}]{eV^{2}}$ and $\sin^{2}/!/left(2\bar{\theta} \right) = 0.97^{+0.03}_{-0.08}$, with minimal change to the neutrino parameters., Comment: 5 pages, 3 figures

Abstract: We report new constraints on flavor-changing non-standard neutrino interactions (NSI) using data from the MINOS experiment. We analyzed a combined set of beam neutrino and antineutrino data, and found no evidence for deviations from standard neutrino mixing. The observed energy spectra constrain the NSI parameter to the range $-0.20 < \varepsilon_{\mu\tau} < 0.07\;\text{(90% C.L.)}$

Abstract: We report on $\nu_e$ and $\bar{\nu}_e$ appearance in $\nu_\mu$ and $\bar{\nu}_\mu$ beams using the full MINOS data sample. The comparison of these $\nu_e$ and $\bar{\nu}_e$ appearance data at a 735 km baseline with $\theta_{13}$ measurements by reactor experiments probes $\delta$, the $\theta_{23}$ octant degeneracy, and the mass hierarchy. This analysis is the first use of this technique and includes the first accelerator long-baseline search for $\bar{\nu}_\mu\rightarrow\bar{\nu}_e$. Our data disfavor 31% (5%) of the three-parameter space defined by $\delta$, the octant of the $\theta_{23}$, and the mass hierarchy at the 68% (90%) C.L. We measure a value of 2sin$^2(2\theta_{13})$sin$^2(\theta_{23})$ that is consistent with reactor experiments.

Abstract: This paper reports measurements of atmospheric neutrino and antineutrino interactions in the MINOS Far Detector, based on 2553 live-days (37.9 kton-years) of data. A total of 2072 candidate events are observed. These are separated into 905 contained-vertex muons and 466 neutrino-induced rock-muons, both produced by charged-current $\nu_{\mu}$ and $\bar{\nu}_{\mu}$ interactions, and 701 contained-vertex showers, composed mainly of charged-current $\nu_{e}$ and $\bar{\nu}_{e}$ interactions and neutral-current interactions. The curvature of muon tracks in the magnetic field of the MINOS Far Detector is used to select separate samples of $\nu_{\mu}$ and $\bar{\nu}_{\mu}$ events. The observed ratio of $\bar{\nu}_{\mu}$ to $\nu_{\mu}$ events is compared with the Monte Carlo simulation, giving a double ratio of $R^{data}_{\bar{\nu}/\nu}/R^{MC}_{\bar{\nu}/\nu} = 1.03 \pm 0.08 (stat.) \pm 0.08 (syst.)$. The $\nu_{\mu}$ and $\bar{\nu}_{\mu}$ data are separated into bins of $L/E$ resolution, based on the reconstructed energy and direction of each event, and a maximum likelihood fit to the observed $L/E$ distributions is used to determine the atmospheric neutrino oscillation parameters. This fit returns 90% confidence limits of $|\Delta m^{2}| = (1.9 \pm 0.4) \times 10^{-3} eV^{2}$ and $sin^{2} 2\theta > 0.86$. The fit is extended to incorporate separate $\nu_{\mu}$ and $\bar{\nu}_{\mu}$ oscillation parameters, returning 90% confidence limits of $|\Delta m^{2}|-|\Delta \bar{m}^{2}| = 0.6^{+2.4}_{-0.8} \times 10^{-3} eV^{2}$ on the difference between the squared-mass splittings for neutrinos and antineutrinos., Comment: 22 pages, 19 figures, submitted to Phys. Abstract: We report an improved measurement of muon anti-neutrino disappearance over a distance of 735km using the MINOS detectors and the Fermilab Main Injector neutrino beam in a muon anti-neutrino enhanced configuration. From a total exposure of 2.95e20 protons on target, of which 42% have not been previously analyzed, we make the most precise measurement of the anti-neutrino "atmospheric" delta-m squared = 2.62 +0.31/-0.28 (stat.) +/- 0.09 (syst.) and constrain the anti-neutrino atmospheric mixing angle >0.75 (90%CL). Abstract: We have searched for sidereal variations in the rate of antineutrino interactions in the MINOS Near Detector. Using antineutrinos produced by the NuMI beam, we find no statistically significant sidereal modulation in the rate. When this result is placed in the context of the Standard Model Extension theory we are able to place upper limits on the coefficients defining the theory. Abstract: We report constraints on antineutrino oscillation parameters that were obtained by using the two MINOS detectors to measure the 7% muon antineutrino component of the NuMI neutrino beam. In the Far Detector, we select 130 events in the charged-current muon antineutrino sample, compared to a prediction of 136.4 +/- 11.7(stat) ^{+10.2}_{-8.9}(syst) events under the assumption |dm2bar|=2.32x10^-3 eV^2, snthetabar=1.0. Assuming no oscillations occur at the Near Detector baseline, a fit to the two-flavor oscillation approximation constrains |dm2bar|<3.37x10^-3 eV^2 at the 90% confidence level with snthetabar=1.0., Comment: Accepted for publication by Phys. Abstract: We report the results of a search for $\nu_{e}$ appearance in a $\nu_{\mu}$ beam in the MINOS long-baseline neutrino experiment. With an improved analysis and an increased exposure of $8.2\times10^{20}$ protons on the NuMI target at Fermilab, we find that $2\sin^2(\theta_{23})\sin^2(2\theta_{13})<0.12\ (0.20)$ at 90% confidence level for $\delta\mathord{=}0$ and the normal (inverted) neutrino mass hierarchy, with a best fit of $2\sin^2(\theta_{23})\sin^2(2\theta_{13})\,\mathord{=}\,0.041^{+0.047}_{-0.031}\ (0.079^{+0.071}_{-0.053})$. Abstract: Results are reported from a search for active to sterile neutrino oscillations in the MINOS long-baseline experiment, based on the observation of neutral-current neutrino interactions, from an exposure to the NuMI neutrino beam of $7.07\times10^{20}$ protons on target. A total of 802 neutral-current event candidates is observed in the Far Detector, compared to an expected number of $754\pm28\rm{(stat.)}\pm{37}\rm{(syst.)}$ for oscillations among three active flavors. Abstract: This letter reports the first direct observation of muon antineutrino disappearance. The MINOS experiment has taken data with an accelerator beam optimized for muon antineutrino production, accumulating an exposure of $1.71\times 10^{20}$ protons on target. In the Far Detector, 97 charged current muon antineutrino events are observed. The no-oscillation hypothesis predicts 156 events and is excluded at $6.3\sigma$. The best fit to oscillation yields $\Delta \bar{m}^{2}=(3.36^{+0.46}_{-0.40}\textrm{(stat.)}\pm0.06\textrm{(syst.)})\times 10^{-3}\,\eV^{2}$, $\sin^{2}(2\bar{\theta})=0.86^{+0.11}_{-0.12}\textrm{(stat.)}\pm0.01\textrm{(syst.)}$. Abstract: Measurements of neutrino oscillations using the disappearance of muon neutrinos from the Fermilab NuMI neutrino beam as observed by the two MINOS detectors are reported. New analysis methods have been applied to an enlarged data sample from an exposure of $7.25 \times 10^{20}$ protons on target. A fit to neutrino oscillations yields values of $|\Delta m^2| = (2.32^{+0.12}_{-0.08})\times10^{-3}$\,eV$^2$ for the atmospheric mass splitting and $\rm \sin^2\!(2\theta) > 0.90$ (90%\,C.L.) for the mixing angle. Abstract: We searched for a sidereal modulation in the MINOS far detector neutrino rate. Such a signal would be a consequence of Lorentz and CPT violation as described by the Standard-Model Extension framework. It also would be the first detection of a perturbative effect to conventional neutrino mass oscillations. We found no evidence for this sidereal signature and the upper limits placed on the magnitudes of the Lorentz and CPT violating coefficients describing the theory are an improvement by factors of $20-510$ over the current best limits found using the MINOS near detector., Comment: 5 pages, 2 figures

Abstract: In an experiment designed to search for and study very rare two-body decay modes of the K-long, we have observed four examples of the decay K-long -> e+ e-, where the expected background is 0.17+-0.10 events. This observation translates into a branching fraction of 8.7^{+5.7}_{-4.1} X 10^{-12}, consistent with recent theoretical predictions. Abstract: We describe the development and testing of a novel beam stop for use in a rare kaon decay experiment at the Brookhaven AGS. The beam stop is located inside a dipole spectrometer magnet in close proximity to straw drift chambers and intercepts a high-intensity neutral hadron beam. The design process, involving both Monte Carlo simulations and beam tests of alternative beam-stop shielding arrangements, had the goal of minimizing the leakage of particles from the beam stop and the resulting hit rates in detectors, while preserving maximum acceptance for events of interest. The beam tests consisted of measurements of rates in drift chambers, scintilation counter hodoscopes, a gas threshold Cherenkov counter, and a lead glass array. Measurements were also made with a set of specialized detectors which were sensitive to low-energy neutrons, photons, and charged particles. Abstract: Background: FDA had been criticized for its slow review of new drugs. Critics complained of a “drug lag” from which US patients suffered when compared to Europeans. Since the advent of PDUFA, however, the FDA has demonstrated a possible slight advantage in review time when compared to the EMA. Methods: Submission and approval dates for monoclonal antibodies were collected from the FDA and EMA websites. Results: When using monoclonal antibodies as examples of complex, yet important new therapeutic agents, it was determined that the FDA reviews these agents on average 5 months faster than the EMA. Conclusion: The review processes within each agency may have reached their highest efficiencies without making further changes in a review system.

Abstract: Context Vitamin D regulates glucose homeostasis pathways, but effects of vitamin D supplementation on β-cell function remain unclear. Objective To investigate the effects of vitamin D3 supplementation on insulin sensitivity and β-cell function. Methods This is a prespecified secondary analysis of the Vitamin D and Type 2 Diabetes study. Overweight/obese adults at high risk for type 2 diabetes (prediabetes) were randomly treated with vitamin D3 4000 IU or matching placebo daily for 24 months. Main Outcome Disposition index (DI), as an estimate of β-cell function, was calculated as the product of Homeostasis Model Assessment 2 indices derived from C-peptide values (HOMA2%Scpep) and C-peptide response during the first 30 minutes of a 75-g oral glucose tolerance test (OGTT). Results Mean age was 60.5 ± 9.8 years and body mass index was 31.9 ± 4.4 kg/m2. Mean serum 25(OH)D level increased from 27.9 ± 10.3 ng/mL at baseline to 54.9 ng/mL at 2 years in the vitamin D group and was unchanged (28.5 ± 10.0 ng/mL) in the placebo group. The baseline DI predicted incident diabetes independent of the intervention. In the entire cohort, there were no significant differences in changes in DI, HOMA2%Scpep, or C-peptide response between the 2 groups. Among participants with baseline 25(OH)D level Conclusions Supplementation with vitamin D3 for 24 months did not improve an OGTT-derived index of β-cell function in people with prediabetes not selected based on baseline vitamin D status; however, there was benefit among those with very low baseline vitamin D status.

Abstract: BACKGROUND AND OBJECTIVES: Low serum 25-hydroxyvitamin D (25[OH]D) concentration has been associated with higher levels of proteinuria and lower levels of eGFR in observational studies. In the Vitamin D and Type 2 Diabetes (D2d) study, we investigated the effect of vitamin D supplementation on kidney outcomes in a population with prediabetes. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: Overweight/obese adults with high risk for type 2 diabetes (defined by meeting two of three glycemic criteria for prediabetes) were randomized to vitamin D(3) 4000 IU per day versus placebo. Median duration of treatment was 2.9 years (interquartile range 2.0–3.5 years). Kidney outcomes included (1) worsening in Kidney Disease: Improving Global Outcomes (KDIGO ) risk score (low, moderate, high, very high) on two consecutive follow-up visits after the baseline visit and (2) mean changes in eGFR and urine albumin-to-creatinine ratio (UACR). RESULTS: Among 2166 participants (mean age 60 years, body mass index 32 kg/m(2), serum 25(OH)D 28 ng/ml, eGFR 87 ml/min per 1.73 m(2), UACR 11 mg/g, 79% with hypertension), 10% had moderate, high, or very high KDIGO risk score. Over a median follow-up of 2.9 years, there were 28 cases of KDIGO worsening in the vitamin D group and 30 in the placebo group (hazard ratio, 0.89; 95% confidence interval [95% CI], 0.52 to 1.52]). Mean difference in eGFR from baseline was −1.0 ml/min per 1.73 m(2) (95% CI, −1.3 to −0.7) in the vitamin D group and −0.1 ml/min per 1.73 m(2) (95% CI, −0.4 to 0.2) in the placebo group; between-group difference was −1.0 ml/min per 1.73 m(2) (95% CI, −1.4 to −0.6). Mean difference in UACR was 2.7 mg/g (95% CI, 1.2 to 4.3) in the vitamin D group and 2.0 (95% CI, 0.5 to 3.6) in the placebo group; between-group difference was 0.7 mg/g (95% CI, −1.5 to 2.9). CONCLUSIONS: Among persons with prediabetes, who were not preselected on the basis of serum 25(OH)D concentration, vitamin D supplementation did not affect progression of KDIGO risk scores and did not have a meaningful effect on change in UACR or eGFR.

Abstract: Context Observational studies suggest that low vitamin D status may be a risk factor for cancer. Objective In a population with prediabetes and overweight/obesity that is at higher risk of cancer than the general population, we sought to determine if vitamin D supplementation lowers the risk of cancer and precancers. Methods The Vitamin D and type 2 diabetes (D2d) cancer outcomes study (D2dCA) is an ancillary study to the D2d study, which was conducted at 22 academic medical centers in the United States. Participants had prediabetes and overweight/obesity and were free of cancer for the previous 5 years. Participants were randomized to receive vitamin D3 4000 IU daily or placebo. At scheduled study visits (4 times/year), cancer and precancer events were identified by questionnaires. Clinical data were collected and adjudicated for all reported events. Cox proportional hazard models compared the hazard ratio (HR) of incident cancers and precancers between groups. Results Over a median follow-up period of 2.9 years, among 2385 participants (mean age 60 years and 25-hydroxyvitamin D 28 ng/mL), there were 89 cases of cancer. The HR of incident cancer for vitamin D vs placebo was 1.07 (95% CI 0.70, 1.62). Of 241 participants with incident precancers, 239 had colorectal adenomatous polyps. The HR for colorectal polyps for vitamin D vs placebo was 0.83 (95% CI 0.64, 1.07). Conclusion In the D2d population of participants with prediabetes and overweight/obesity, not selected for vitamin D insufficiency, vitamin D supplementation did not have a significant effect on risk of incident cancer or colorectal polyps.

Abstract: Objectives: High rates of recidivism are reported after paediatric cholesteatoma surgery. Our practice has adapted to include non-echoplanar diffusion-weighted magnetic resonance imaging for the diagnosis of residual or recurrent cholesteatoma. This audit aimed to evaluate the performance of non-echoplanar diffusion-weighted magnetic resonance imaging in our paediatric population. Methods: A retrospective review was conducted of non-echoplanar diffusion-weighted magnetic resonance imaging scans performed to detect residual disease or recurrence after surgery for cholesteatoma in children from 1 January 2012 to 30 November 2017 in our centre. Follow-up diffusion-weighted magnetic resonance imaging scans were reviewed to 16 August 2019. Results: Thirty-four diffusion-weighted magnetic resonance imaging scans were included. The sensitivity and specificity values of diffusion-weighted magnetic resonance imaging for detecting post-operative cholesteatoma were 81 per cent and 72 per cent, respectively. Positive predictive and negative predictive values were 72 per cent and 81 per cent, respectively. Conclusion: Use of diffusion-weighted magnetic resonance imaging is recommended as a replacement for routine second-look surgical procedures in the paediatric population. However, we would caution that patients require close follow up after negative diffusion-weighted magnetic resonance imaging findings. [ABSTRACT FROM AUTHOR]

Abstract: Objective Fasting plasma glucose (FPG), 2-hour plasma glucose (2hPG) from a 75-g oral glucose tolerance test (OGTT) and glycated hemoglobin (HbA1c) can lead to different results when diagnosing prediabetes and diabetes. The Hemoglobin Glycation Index (HGI) quantifies the interindividual variation in glycation resulting in discrepancies between FPG and HbA1c. We used data from the Vitamin D and Type 2 Diabetes (D2d) study to calculate HGI, to identify HGI-associated variables, and to determine how HGI affects prediabetes and diabetes diagnosis. Measurements A linear regression equation [HbA1c (%) = 0.0164 × FPG (mg/dL) + 4.2] was derived using the screening cohort (n = 6829) and applied to calculate predicted HbA1c. This was subtracted from the observed HbA1c to determine HGI in the baseline cohort with 2hPG data (n = 3945). Baseline variables plus prediabetes and diabetes diagnosis by FPG, HbA1c, and 2hPG were compared among low, moderate, and high HGI subgroups. Results The proportion of women and Black/African American individuals increased from low to high HGI subgroups. Mean FPG decreased and mean HbA1c increased from low to high HGI subgroups, consistent with the HGI calculation; however, mean 2hPG was not significantly different among HGI subgroups. Conclusions High HGI was associated with Black race and female sex as reported previously. The observation that 2hPG was not different across HGI subgroups suggests that variation in postprandial glucose is not a significant source of population variation in HGI. Exclusive use of HbA1c for diagnosis will classify more Black individuals and women as having prediabetes compared with using FPG or 2hPG.
