Abstract

Coastal flood damage is primarily the result of extreme sea levels. Climate change is expected to drive an increase in these extremes. While proper estimation of changes in storm surges is essential to estimate changes in extreme sea levels, there remains low confidence in future trends of surge contribution to extreme sea levels. Alerting local populations of imminent extreme sea levels is also critical to protecting coastal populations. Both predicting and projecting extreme sea levels require reliable numerical prediction systems. The SurgeMIP (surge model intercomparison) community has been established to tackle such challenges. Efforts to intercompare storm surge prediction systems and coordinate the community's prediction and projection efforts are introduced. An overview of past and recent advances in storm surge science such as physical processes to consider and the recent development of global forecasting systems are briefly introduced. Selected historical events and drivers behind fast increasing service and knowledge requirements for emergency response to adaptation considerations are also discussed. The community's initial plans and recent progress are introduced. These include the establishment of an intercomparison project, the identification of research and development gaps, and the introduction of efforts to coordinate projections that span multiple climate scenarios.

Abstract

There are longstanding questions about the origins and ancestry of the Picts of early medieval Scotland (ca. 300-900 CE), prompted in part by exotic medieval origin myths, their enigmatic symbols and inscriptions, and the meagre textual evidence. The Picts, first mentioned in the late 3(rd) century CE resisted the Romans and went on to form a powerful kingdom that ruled over a large territory in northern Britain. In the 9(th) and 10(th) centuries Gaelic language, culture and identity became dominant, transforming the Pictish realm into Alba, the precursor to the medieval kingdom of Scotland. To date, no comprehensive analysis of Pictish genomes has been published, and questions about their biological relationships to other cultural groups living in Britain remain unanswered. Here we present two high-quality Pictish genomes (2.4 and 16.5X coverage) from central and northern Scotland dated from the 5(th)-7(th) century which we impute and co-analyse with >8,300 previously published ancient and modern genomes. Using allele frequency and haplotype-based approaches, we can firmly place the genomes within the Iron Age gene pool in Britain and demonstrate regional biological affinity. We also demonstrate the presence of population structure within Pictish groups, with Orcadian Picts being genetically distinct from their mainland contemporaries. When investigating Identity-By-Descent (IBD) with present-day genomes, we observe broad affinities between the mainland Pictish genomes and the present-day people living in western Scotland, Wales, Northern Ireland and Northumbria, but less with the rest of England, the Orkney islands and eastern Scotland-where the political centres of Pictland were located. The pre-Viking Age Orcadian Picts evidence a high degree of IBD sharing across modern Scotland, Wales, Northern Ireland, and the Orkney islands, demonstrating substantial genetic continuity in Orkney for the last similar to 2,000 years. Analysis of mitochondrial DNA diversity at the Pictish cemetery of Lundin Links (n = 7) reveals absence of direct common female ancestors, with implications for broader social organisation. Overall, our study provides novel insights into the genetic affinities and population structure of the Picts and direct relationships between ancient and present-day groups of the UK. Author summary: We report two high-quality autosomal and eight mitochondrial genomes sequenced from individuals associated with the Pictish period of early medieval Scotland (ca. 300-900 CE). We demonstrate genetic affinities between the Pictish genomes and Iron Age people who lived in Britain, which supports current archaeological theories of a local origin. The autosomal genomes also allowed us to detect haplotype sharing between the Pictish genomes and present-day Europeans. Our results demonstrate a proportionally higher degree of haplotype sharing, and thus genetic affinity, between the Pictish genomes and individuals from western Scotland, Wales, Northern Ireland and Northumbria. We also detected genetic structure in Scotland during the Iron Age, likely driven by the combination of genetic drift and small population size, which we also detect in present-day Orcadians. Lastly, the seven mitochondrial DNA from the Lundin Links cemetery showed that these individuals had no direct maternal ancestors which could suggest exchanges of people, or at least females, between groups during the Pictish period, challenging older ideas that the Picts were a matrilineal society. Overall, our results show that high-quality ancient genomes combined with haplotype imputation are highly informative for obtaining novel insights to population structure and migration over the past 2,000 years.

Abstract

Licensure examinations serve as a critical gateway to professional practice, often creating significant stress and challenging experiences for those who fail and must retake them. This study explored how the experience of failing and retaking the psychometrician licensure examination impacted the self-concept of seven examinees from Region 1, Philippines. Using interpretative phenomenological analysis of online interviews, the research revealed that retaking the exam had profound and varied effects on their self-perception, ranging from restoring confidence to completely transforming their sense of self. This research highlights the often-overlooked experiences of licensure exam retakers and contributes to understanding the psychological impact of setbacks and perseverance on the path to success. By focusing on the lived experiences of these individuals, the study offers valuable insights into the challenges and triumphs faced by those striving to achieve professional licensure.

Abstract

Background: Current sentinel lymph node biopsy (SLNB) techniques, including use of radioisotopes, have disadvantages including the use of a radioactive tracer. Indocyanine green (ICG) based near-infrared (NIR) fluorescence imaging and cone beam CT (CBCT) have advantages for intraoperative use. However, limited literature exists regarding their use in head and neck cancer SLNB. Methods: This was a prospective, non-randomized study using a rabbit oral cavity VX2 squamous cell carcinoma model (n = 10) which develops lymph node metastasis. Pre-operatively, images were acquired by MicroCT. During surgery, CBCT and NIR fluorescence imaging of ICG was used to map and guide the SLNB resection. Results: Intraoperative use of ICG to guide fluorescence resection resulted in identification of all lymph nodes identified by pre-operative CT. CBCT was useful for near real time intraoperative imaging and 3D reconstruction. Conclusions: This pre-clinical study further demonstrates the technical feasibility, limitations and advantages of intraoperative NIR-guided ICG imaging for SLN identification as a complementary method during head and neck surgery.

Abstract

In oral cavity (OC) squamous cell cancer, the incidence of occult nodal metastases varies from 20% to 50% depending and tumor size and thickness. Besides clinical and histopathological factors, image-derived biomarkers may help estimate the probability of LN (lymph nodes) metastasis using a non-invasive approach to further stratify patients' need for neck dissection. We investigated the role of MR-based radiomics in predicting positive lymph nodes in OC patients, prior to surgery. We also investigated different supervised and unsupervised dimensionality reduction techniques, as well as different classifiers. Results showed that the combination of radiomics+clinical factors outperform radiomics and clinical predictors alone. Overall, a combination of supervised and supervised machine learning algorithms seems more suitable for better performances in radiomic studies.

Abstract

Objectives/Hypothesis The endoscopic endonasal approach (EEA) for nasopharyngectomy is an alternative to the maxillary swing approach (MSA) for selected recurrent nasopharyngeal carcinomas (NPC). We compare the access between these approaches. Methods Three cadaver specimens were used to compare access volumes of the EEA and MSA. Exposure volumes were calculated using image guidance registration to cone beam computed tomography and tracking of accessible tissue with volumetric quantification. The area of exposure to the carotid artery was measured. Results The MSA provided higher volumes for access volume compared with the EEA (66.6 vs 39.1 cm 3, p = 0.009). The working area was larger in the MSA (80.2 vs 56.9 cm 2, p = 0.06). The exposure to the carotid artery was higher in the MSA (1.88 vs 1.62 cm 2, p = 0.04). The MSA provided larger volume of exposure for tumors of the parapharyngeal space with exposure below the palate. Conclusions This study suggests that the MSA for nasopharyngectomy provides a larger volume of exposure. However, much of the increased exposure relates to exposure of the parapharyngeal space below the palate. The EEA provides adequate access to superior anatomical structures.

Abstract

Background At the request of the Head and Neck Cancers Advisory Committee of Ontario Health (Cancer Care Ontario), a working group and expert panel of clinicians with expertise in the management of head-and-neck cancer developed the present guideline. The purpose of the guideline is to provide advice about the organization and delivery of health care services for adult patients with head-and-neck cancer. Methods This document updates the recommendations published in the Ontario Health (Cancer Care Ontario) 2009 organizational guideline The Management of Head and Neck Cancer in Ontario. The guideline development methods included an updated literature search, internal review by content and methodology experts, and external review by relevant health care providers and potential users. Results To ensure that all patients have access to the highest standard of care available in Ontario, the guideline establishes the minimum requirements to maintain a head-and-neck disease site program. Recommendations are made about the membership of core and extended provider teams, minimum skill sets and experience of practitioners, cancer centre-specific and practitioner-specific volumes, multidisciplinary care requirements, and unique infrastructure demands. Conclusions The recommendations contained in this document offer guidance for clinicians and institutions providing care for patients with head-and-neck cancer in Ontario, and for policymakers and other stakeholders involved in the delivery of health care services for head-and-neck cancer.
